Abstract:Objective The purpose of this study was to retrospectively analyze and evaluate the clinical effects of surgical treatment for the accessory navicular syndrome with modified excision.Methods From January 2006 to November 2010,46 patients(55 feet) with accessory navicular syndrome received surgical treatment in our institute.There were 11 males and 35 females,including 21 athletes and 25 non-athletes.Their mean age was 19.8±8.6 years,ranging from 9 to 49 years.The present history ranged from 14 days to 10 years.Excision of the accessory navicular bone and reinforcement of posterior tibial tendon were preformed for each case.The foot was immobilized with cast in inversion position and no weight bearing for 6 weeks.The rational symptom assessment and the AOFAS scores were used to evaluate the clinical effects before and after surgery.Results There were 3 cases of type I lesion,47 cases of type II and 5 cases of type III.17 of athletes(70.8%) with sprain history on feet were identified,which were significantly higher than the non-athlete group.43 patients(52 feet) were followed-up for an average of 31.5 months,ranging from 16 months to 63 months.93% patients revealed good and excellent recovery of foot function.The AOFAS scores in both groups were significantly improved after surgical treatment.Conclusion Type II is the most common one in painful accessory navicular syndrome,and sprain the main factor in athletic patients.Excision of accessory navicular bone and reinforcement of posterior tibial tendon is an effective method for treating painful accessory navicular syndrome.

Abstract:Objective To evaluate the effectiveness and safety of fusion of the accessory and primary navicular bones for painful type II accessory navicular bone.Methods From February 2009 to December 2012, 16 patients ( 17 feet ) whose average age was 26.9 years old ( range: 17-35 years ) underwent fusion of the accessory and primary navicular bones. The patients complained of local and chronic pain and swelling. The physical examination results showed tenderness in all the patients. The conservative treatment was ineffective.Results The patients were followed up for a mean period of 22 months ( range: 14-60 months ). According to the patients’ satisfaction rating system, the evaluation results of 12 feet were excellent, 4 feet good and 1 foot fair. The postoperative X-rays showed bone consolidation was conifrmed in 13 feet ( 76% ) within 3 months and in 16 feet ( 94% ) within 1 year. One foot of non-union was found in the latest follow-up of half a year after the operation. The preoperative and postoperative talonavicular coverage angles, lateral talo-ifrst metatarsal shaft angles and calcaneal pitch angles were compared based on the X-rays, and the differences were not statistically signiifcant (P>0.05 ).Conclusions Fusion of the accessory and primary navicular bones is a safe and effective method in the treatment of painful Type II accessory navicular bone. Its effects on the function of the tibialis posterior and the progression of lfat feet are still ambiguous, so a long-term follow-up study is needed.
[Surgical treatment of the accessory navicular syndrome with simple excision]

Abstract:Objective: To observe and evaluate the clinical effects of surgical treatment for the accessory navicular syndrome with simple excision. Methods: From November 2006 to December 2008, 23 patients (twenty-five feet) with accessory navicular syndrome received simple excision of the accessory navicular bone. The chief complains were intermittent pain of feet after running or walking. Physical examination showed local tenderness on palpation in the region of the navicular bone. X-ray or CT showed there was an accessory navicular bone. The present history ranged from 6 months to 12 years. There were 14 males and 9 females. The mean age was 14.6 years, ranging from 8 to 35 years. About 2 cm long incision was made at the tip of the medial prominence of the navicular bone. After partial dissection of the posterior tibial tendon, the accesssory navicular bone was exposed and excisied. The prominence of the tuberosity of the navicular bone was cut and shaved. The posterior tibial tenden was repaired before closing the wound. The foot was immobilized with cast or brace in inversion position and no weight-bearing for 2 weeks. Strenuous jumping or dancing must be avoided in 3 months after surgery. The patients with residual symptoms and signs received physical therapy and an arch support for shoes without flatfoot deformity. Results: The average clinical follow-up during was 12 month (ranged, 3 to 18 months). The excellent results in 21 feet and good in 4 feet (3 feet with mild flat deformity and 1 foot with old sprain injury). The average hospital stay was 5 days and no wound infection occurred. All patients resumed the normal life and study after operation. Conclusion: Surgical treatment of the accessory navicular syndrome with simple excision has the advantages of less invasive to the posterior tibial tenden and the medial longitudinal arch of the foot, shorter time of immobilization of the foot and stay in hospital, small incision and good clinical results. This procedure is one of the best selective treatments for the accessory navicular syndrome, especially for the patients without flatfoot deformity and old sprain injury.
A Retrospective Study of Posterior Malleolus Fractures

Abstract:PURPOSE:In this retrospective study, we evaluated the treatment effect of ankle joint fracture surgery involving the posterior malleolus, and discuss relevant factors influencing the occurrence of traumatic arthritis of the ankle joint.METHODS:A total of 102 cases of ankle joint fractures involving the posterior malleolus in five large-scale skeletal trauma centres in China, from January 2000 to July 2009, were retrospectively analysed in terms of surgical treatment and complete follow-up. Ankle joint mobility, posterior malleolus fragment size, articular surface evenness, Ankle-Hindfoot Scale of the American Orthopedic Foot and Ankle Society (AOFAS) score, and imaging scale score for arthritis were recorded. The degree of fracture pain during rest, active movement, and weight-bearing walking, and satisfaction with treatment were evaluated using a visual analogue scale (VAS).RESULTS:The average AOFAS score was 95.9, excellence rate was 92.2 %, and average VAS scores for degree of fracture pain during rest, active movement, and weight-bearing walking were 0.15, 0.31, and 0.68, respectively. Thirty-six cases showed arthritic manifestations. Ankle joint mobility along all directions on the injured side was lower than that on the unaffected side. There was no obvious difference in treatment effect between the fixed and unfixed posterior malleolus fragment groups for all and for fragment size of < 25 %; between fixing the posterior malleolus fragment from front to back or from back to front; or between elderly patients (≥ 60 years old) and young patients (< 60 years old). There was a distinct difference in the treatment effect between articular surface evenness and unevenness for all and for fragment size of ≥ 25 %.CONCLUSIONS:For all 102 cases of ankle joint fracture involving the posterior malleolus, the treatment effect was satisfactory. Restoration of an even articular surface, especially when fragment size ≥ 25 %, should be attempted during treatment.
Clinical observation of internal fixation with twin fix anchor for adult typeⅡaccessory navicular bones

Abstract:Background: Surgical treatment with twin fix anchor can significantly decrease the pain and improve function for adult typeⅡaccessory navicular bones. Objective: To evaluate the clinical efficacy of surgical treatment with twin fix an-chor on adult typeⅡaccessory navicular bones. Methods: We retrospectively analyzed 15 cases (18 feet) of adult typeⅡac-cessory navicular bones treated by twin fix anchor from May 2011 to October 2015. AOFAS and VAS scoring system were used to assess clinical outcomes. Results: All the 15 patients were followed up for (11.6±4.2) months on average (range, 6-20 months). No infection, delayed union, fixation failure, or deep venous thrombosis occurred. Bony union were achieved in all patients. The pain disappeared in 15 feet and was obviously alleviated in 3 feet. The range of motion of the 18 feet were all normal. Conclusions: Surgical procedure with twin fix anchor can achieve satisfactory outcomes and significantly de-crease the pain in patients with adult typeⅡaccessory navicular bones.
Hazard of Failed Nonoperative Management for Symptomatic Accessory Navicular in Children and Adolescents: A Population-Based Case-Cohort Study

Abstract:Background: The accessory navicular (AN) is an idiopathic condition of the foot present in 4% to 21% of the population. Most ANs remain asymptomatic, but children and adolescents who develop symptoms can have remarkably reduced quality of life. Although many respond to conservative measures, surgery is occasionally needed. Our purpose was to determine factors associated with the failure of nonoperative management. Methods: This single-institution retrospective case-cohort study included patients up to age 19 years presenting between 2000 and 2021 with symptomatic AN and treated with standard-of-care. All 298 surgical cases, indicating failed nonoperative treatment, were included. For the subcohort, 299 patients were randomly sampled from all eligible patients, regardless of treatment. Baseline characteristics were summarized for the surgical cases and subcohort. Proportional hazards assumptions were checked and stratification implemented when necessary. Marginal structural proportional hazard modeling was used to estimate hazard ratios with 95% confidence intervals via inverse probability and LinYing weighting methods. Results: The 298 surgical cases failed nonoperative management at a median of 5.2 months (IQR, 2.0–11.6 mo). In the subcohort, 86 failures of nonoperative management and 213 nonfailures constituted a 28.8% surgery rate. In both cohorts, nearly all patients played sports. Univariate proportional hazard modeling found older age ( P =0.02) and activity limitation ( P <0.001) at presentation, female sex ( P =0.002), higher BMI ( P =0.01), AN on the right ( P <0.001), and bone marrow edema of the AN ( P <0.001) and navicular body ( P <0.001) on MRI were associated with increased hazard of nonoperative failure. Nearly all of the surgical cohort reported improvement in pain (278/296, 94%) and returned to their primary sport (236/253, 93%) after surgery. Most also experienced full resolution of symptoms (187/281, 67%). Conclusions: Symptomatic AN predominantly affects female athletes, leading to surgery in 28.8% of our subcohort. Conservative treatment may be less successful—and therefore surgery could be more strongly considered—in older age, activity limitation at presentation, female sex, higher BMI, right-sided AN, and bone marrow edema on MRI. Surgery is effective for symptomatic and functional improvement. Level of Evidence: Case-cohort—Level III.

Abstract:Due to the complex anatomical structure of ankle and the various mechanisms of ankle injury, manyeponyms and systematic classifications of ankle fracture have been developed in the past centuries. Before the emergence and constant im?provement of classifications of ankle fracture, ankle fractures were most commonly named afterthe physicians who first described them. Now,these ankle fracture eponyms are still found in medical literature, textbooks and even mass media. Many special named ankle fractures can be explained by the Lauge?Hansen classification now, and they may have little practical use. However, it is still necessary to summarize and review these ankle fracture eponyms, in order to correct the wrong usage of these and to remem?ber the sages in this field.We reviewed both common and less frequently used ankle fracture eponyms, focusing on imaging fea?tures to identify and differentiate these injuries. We also briefly reviewed the mechanism of each injury, associated complications, its diagnosis and treatment.

Abstract:Background The Trigen short reconstruction trochanteric antegrade nail (TAN) is considered most suitable for the small skeletons of Asian patients. We have evaluated the results of TAN for intertrochanteric fractures in Asian patients.Methods Between January 2006 and March 2009, 174 fractures in 164 patients were treated by TAN in our department. Epidemiological data, radiological changes, fracture types, cortical bone indices, clinical outcomes, and complications over a mean of 26 months were analyzed retrospectively.Results We observed a 17.0% fracture rate at the tip of the nail, a poor reduction rate of 15.9%, and a 41.4% cracking rate of the lateral trochanter for type 31-A3 fracture. Review of these cracking cases showed that they were all type 31-A3 unstable fractures combined with comminuted lateral trochanter fractures. Two shaft fractures required revision, with other fractures showing no delayed union, nonunion or infection. No screw breakage but one cut-out occurred, which received total hip arthroplasty revision. All other fractures healed at last. Risk of distal shaft fractures was not associated with patient age, gender, fracture type or cortical bone index.Conclusion Despite the theoretically better design of TAN and the healing of most fractures and good activity at last follow-up, poor reduction and coxa vara caused by insertion of the nail could not be completely avoided, and the design of the distal part of the nail had the potential to cause femoral shaft fracture. Future improvements are necessary to enable the use of this system in Asians.

Abstract:Background Few data on ankle fractures in China from large multicenter epidemiological and clinical studies are available. The aim of this research was to evaluate the epidemiological features and surgical outcomes of ankle fractures by reviewing 235 patients who underwent ankle fracture surgery at five hospitals in China.Methods This study included patients who underwent ankle fracture surgery at five Chinese hospitals from January 2000 to July 2009. Age, gender, mechanism of injury, Arbeitsgemeinschaft fur Osteosynthesefragen (AO) fracture type, fracture pattern, length of hospital stay and treatment outcome were recorded. Statistical analyses were conducted using SPSS software. The American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot scale, visual analogue scale (VAS), and arthritis scale were used to evaluate outcome.Results Of 235 patients with ankle fractures, 105 were male with an average age of 37.8 years and 130 were female with an average age of 47.3 years. The average follow-up period was 55.7 months. There were significant differences in the ratios of patients in different age groups between males and females, and in mechanisms of injury among different age groups. There were also significant differences in the length of hospital stay among different fracture types and mechanisms of injury. In healed fractures, the average AOFAS ankle-hindfoot score was 95.5, with an excellence rate of 99.6%, the average VAS score was 0.17, and the average arthritis score was 0.18. Movement of the injured ankle was significantly different to that of the uninjured ankle. There were no significant differences between AO fracture types, fracture patterns or follow-up periods and AOFAS score, but there were some significant differences between these parameters and ankle joint movements, pain VAS score and arthritis score.Conclusions Ankle fractures occur most commonly in middle-aged and young males aged 20-39 years and in elderly females aged 50-69 years. The most common mechanisms of injury are twisting injuries and falls from a standing height or less. The results of surgical treatment are satisfactory. Chin Med J 2012;125(4):574-578

Abstract:Purpose: Congenital accessory navicular bone (ANB) is a common variant in the foot and is prone to cause several clinical symptoms. Wearing custom-made foot orthosis is considered a desirable option; however, there is limited evidence of its effectiveness. This study aims to report the mid-term effect of foot orthosis for symptomatic pediatric ANBs. Methods: School-age children with symptomatic ANBs combined with flexible flatfoot in the authors' institute were recruited and got custom-made foot orthosis treatment. They were followed up over 4 years. The general characteristics of these children were collected before treatment, including age, gender, and body mass index (BMI). The indicators of foot symptoms, including frequency and location of pain, visual analogue scale (VAS), arch index (AI), and hind foot valgus angle (HVA), were measured during pretreatment and at the last follow-up. Results: Twenty-seven children were recruited for this study. After 4 years of custom-made foot orthosis treatment, significant improvements showed in pain frequency, VAS, AI, and HVA (P < 0.001). Type II ANBs showed a higher pain index pretreatment (P < 0.001) and reduced after treatment (P < 0.001). Conclusion: Mid-term effect of custom-made foot orthosis is inspiring in clinical symptoms of pediatric congenital ANBs combined with flexible flatfoot and may be an optional nonoperative treatment. Accessory Anterolateral Talar Facet Syndrome After Treatment of Neglected Fracture of the Lateral Process of the Talus: A Case Report

Abstract:Fracture of the lateral process of the talus has often been missed or confused with lateral ankle sprain, and this can lead to long-term untreated cases of nonunion, malunion, and subtalar joint osteoarthritis. In a review of the published data, accessory anterolateral talar facet syndrome was not found after treatment of nonunion of the lateral process of the talus fracture. This report presents the case of a 40-year-old male snowboard instructor who suffered from a neglected fracture of the lateral process of the talus and was treated with open reduction using an iliac autogenous bone graft. Subsequently, partial resection of the anterior aspect of the process was required to treat the impingement syndrome that developed secondary to the united but enlarged lateral process. The patient resumed his activities a year after the second operation. We describe the consequences of misdiagnosis of the lateral process of the talus and its management.

Abstract:Fractures of the anterior process of the calcaneus (APC) are easily overlooked in clinical practice. Most patients have good to excellent clinical outcome after conservative treatment, while some patients may have persisting symptoms and unfavorable functional outcomes. The aim of this study was to identify the risk factors associated with unfavorable functional outcome after conservative treatment in APC fractures. All patients presenting with APC fractures and receiving conservative treatment from April 2019 to April 2020 were retrospectively assessed. The primary outcome measurement was the ankle joint function assessed using Karlsson Scoring System at 2 years post-injury. The risk factors associated with unfavorable functional outcomes (Karlsson score≤80) were evaluated by logistic regression analysis. Eighty-four patients were included with a mean age of 40 years. Twenty-six (31 %) patients presented with unfavorable functional outcome at 2 years post-injury. In multivariate logistic regression, concomitant fractures of talonavicular (TN) joints and older age were significantly associated with unfavorable functional outcome (p < 0.05). Patients with concomitant fractures of TN joints had an odds ratio of 3.623 for unfavorable functional outcome. The optimal cutoff age for an unfavorable outcome was ≥ 47.5 years, with an odds ratio of 5.010. Most patients with APC fractures achieved good to excellent results when treated conservatively. Attention should be paid to those with concomitant fractures of TN joints and with age ≥ 47.5 years, which might lead to unfavorable functional recovery. IV; Case series.
Treatment Of The Intra-Articular Calcaneal Fractures: An Efficacy Comparison Among Closed Percutaneous Reduction Surgery, Open Reduction Via Extended L-Shaped Lateral Approach And Small Sinus Tarsi Approach

Abstract:The goal of this study was to retrospectively compare the complications and outcomes of three operations to determine a better approach with less complications and satisfied outcomes. Between 1990 and 2010, 175 patients with intra-articular calcaneal fractures underwent the closed percutaneous reduction (50 feet, PR group), open reduction via an extended L-shaped lateral approach (70 feet, EL group) or minimally invasive small sinus tarsi approach (75 feet, MI group) in our hospital. Outcome parameters included complications, radiographic analysis, the American Orthopaedic Foot and Ankle Society (AOFAS) ankle score and the Short Form 36 (SF-36) score. Although the calcaneal height, length, width, Bohler angle, and Gissane angle were significantly improved in the three groups postoperatively, a more pronounced improvement was observed in the EL and MI groups. At the final follow-up, the foot function seemed to be better recovered by the EL and MI treatments with higher AOFAS and SF-36 scores. The complication incidence was significantly lower in the PR (0%) and MI (5.33%) groups than that in the EL group (22.86%). Furthermore, reoperation occurred in 6 feet (12.00%) in the PR group, 11 feet (15.71%) in the EL group and 2 feet (2.67%) in the MI group. The sinus tarsi approach may be the most effective and safest for the intra-articular calcaneal fractures, with an excellent reduction, but lower complication and reoperation rates.

Abstract:Objective: To evaluate the efficacy of titanium locking plate combined with suture anchor (SA) repair in the treatment of ankle fractures (AF). Methods: Retrospective analysis of clinical data of 170 patients with AF who underwent surgical treatment at the Department of Traumatology and Orthopedics of Wuhan Fourth Hospital, China from May 2020 to June 2023 was conducted. A total of 83 patients underwent conventional open reduction and internal fixation (control group); 87 patients received titanium locking plates combined with SA repair (observation group). Ankle joint functional recovery (measured using the Mazur Ankle Grading Scale), quality of life (measured using the Generic Quality of Life Inventory-74), and incidence of complications were compared between two groups. Active range of motion and muscle strength of the ankle joint in plantar flexion and dorsiflexion states, and medial clear space and talar tilt angle were also analyzed. Results: The rate of excellent recovery of ankle joint function in the observation group was significantly higher than that in the control group (P<0.05). Six months after surgery, active range of motion and muscle strength of the ankle joints in the plantar flexion and dorsiflexion states in the observation group was significantly higher than that in the control group (P<0.05). The medial clear space and talar tilt angle of the observation group was significantly smaller compared to the control group (P<0.05). The quality of life in the observation group was significantly higher than that in the control group, and the incidence of complications in the observation group was significantly lower than that in the control group (P<0.05). Conclusions: Titanium locking plates combined with SA repair for the treatment of AF can significantly improve ankle joint functional recovery, improve the quality of life of patients, and is associated with a lower incidence of complications. doi: https://doi.org/10.12669/pjms.40.10.10308 How to cite this: Huang Z, Chen M, Ye Z. Abstract:Objective To study the incidence and radiologic findings of calcaneonavicular coalition.Methods CR films of foot andankle in 1361 cases were presented,which were evaluated for acute trauma or chronic pain.There were 588 cases of foot CR and 773 cases of ankle CR,age ranged from 10 years to 91 years(984 cases of 20~40 years).The prevalence of calcaneonavicular coalition was determined and the different significance of both male and female,acute trauma and chronic pain group were analysed.Results In 1361 cases,72 cases of calcaneonavicular coalition(5.3%) were demonstrated,8.7%(47/588)on foot CR films and 3.2%(25/773) on ankle CR films.Calcaneonavicular coalition was more dipicted on foot CR films than on ankle CR films(P0.005).There was no obvious different significance of observing calcaneonavicular coalition in both male and female,or acute trauma and chronic pain groups(P0.05).Conclusion The foot CR films is more superexcellent than the ankle CR films on demonstrating calcaneonavicular coalition.
Radiological Characteristics and Anatomical Risk Factors in the Evaluation of Hallux Valgus in Chinese Adults

Abstract:BACKGROUND:There are no unified theories as to the anatomical changes that occur with hallux valgus, we investigated the radiological characteristics and anatomical risk factors for hallux valgus deformity in Chinese adults. METHODS:We reviewed 141 patients with hallux valgus (206 feet; 15 males, 126 females; mean age, 58.5 years). These patients attended Peking University People's Hospital from April 2008 to March 2014. All feet had intact radiological data, obtained using the Centricity RIS/PACS system. We measured hallux valgus angle (HVA), 1-2 intermetatarsal angle (IMA), proximal articular set angle (PASA), distal articular set angle, hallux interphalangeal angle, metatarsocuneiform angle, size of the medial eminence of the distal first metatarsal, tibial sesamoid position, and joint congruity of the first metatarsophalangeal joint (MTPJ). RESULTS:We found positive correlations between the HVA and IMA (r = 0.279, P < 0.01) and HVA and PASA (r = 0.358, P < 0.01), but not for IMA and PASA (P > 0.05). Feet were divided into three groups based on HVA severity. IMA (P < 0.05) and PASA (P < 0.05) in the mild group were significantly lower than that in the moderate and severe groups, with no significant difference determined for IMA or PASA between the moderate and severe groups (P > 0.05). Feet were then grouped based on the shape of the first metatarsal head. Using this grouping, HVA was significant higher in the rounded shape (19.92°) than in a flat shape (17.66°). The size of the medial eminence of the distal first metatarsal was positively correlated with HVA (r = 0.185, P < 0.01). The medial eminence in the moderate and severe groups was significantly larger than that in the mild group; moderate and severe groups were not significantly different. CONCLUSIONS:PASA enlargement is an adaptive change during early hallux valgus formation, and decompensation leads to subdislocation in the first MTPJ. A rounded first metatarsal head would thus predispose a foot to hallux valgus. Furthermore, bone proliferation at the medial eminence may also lead to early hallux valgus development.
Post-operative radiological predictors of satisfying outcomes occurring after intra-articular calcaneal fractures: a three dimensional CT quantitative evaluation

Abstract:PurposeNo functional outcome related to 3-D CT evaluations for calcaneal fractures has been presented. The aims of this study are to analyze the post-operative morphological parameters of calcaneal fractures in 3-D space and to correlate the 3-D morphological parameters with functional outcomes.MethodsBetween 2009 and 2015, 156 patients operatively treated for displaced calcaneal fractures were retrospectively reviewed with an average follow-up time of 32.4 months. Böhler’s angle, Gissane’s angle, the length of calcaneal axis, the height of posterior facet, the length of posterior facet, and the subtalar joint congruity were evaluated post-operatively in accordance with 3-D CT. Each parameter was quantified, and then its association with the clinical outcomes assessed by the AOFAS score and Short Form-36 questionnaire was evaluated.ResultsThe restoration of the length of calcaneal axis, the height of posterior facet, and the length of posterior facet had no significant correlation with the clinical outcomes (P > 0.05). The restoration of Böhler’s angle and Gissane’s angle had significant correlation with the better AOFAS score (P < 0.001), while no correlation was found with the SF-36 physical component summary score (P > 0.05). No significant association emerged between the clinical outcomes and reconstruction of the posterior facet congruity (P > 0.05).ConclusionThe predictable functional outcome is related to the reconstruction of post-operative morphological characteristics of the calcaneus as evaluated on 3-D CT, especially the Böhler’s angle and Gissane’s angle. Despite post-operative step-off of the posterior facet exiting, the clinical outcomes appear to be not related to the posterior facet congruity.

Treatment of Symptomatic Os Supranaviculare in Athletes

Abstract:Foot &Ankle International, Ahead of Print. Background:Although the incidence of os supranaviculare (OSSN) is generally low, symptomatic OSSN affects athletes. The aim of this study was to assess the variations of OSSN and the results of osteosynthesis between the OSSN and the navicular bone.Methods:Eleven feet of 10 elite athletes with symptomatic OSSN were treated. There were 3 male and 7 female patients with an average age of 19 years. Eight feet exhibited navicular stress fracture (NSF). Operative treatment was performed in 9 feet of 8 patients and 2 conservatively. Seven OSSNs were fixed with 1 or 2 screw(s) according to their size, using an autologous bone graft. The accompanying NSF was also treated surgically in 4 feet. Foot condition was evaluated using the Japanese Society for Surgery of the Foot (JSSF) midfoot rating scale and sports activity score of the Self-Administered Foot Evaluation Questionnaire (SAFE-Q).Results:The median OSSN was 12.7 mm in width, 5.6 mm in length, and 6.6 mm in height. The dorsal surface of the OSSN required at least 70 mm2 to be fixed using 2 screws. Seven OSSNs of 6 patients treated surgically successfully fused with the navicular. Two small OSSNs that were not stabilized with screws also fused after surgical treatment for NSF. However, one of the 2 OSSNs with NSF treated nonoperatively did not achieve fusion. The patients were followed up for 24-161 months. The median JSSF score improved from 87 to 97.7 postoperatively (P = .00312). The median postoperative SAFE-Q sports score was 84.8. All patients returned to their original activities.Conclusion:Our results suggest that osteosynthesis with autologous bone graft was effective for symptomatic OSSNs. Even when the OSSN was small and not suitable for internal fixation, treatment of NSF was effective for union of OSSNs. The OSSN possibly belongs to a part or subtype of NSF.Level of Evidence:Level IV, retrospective case series.

Abstract:Objective To find out the correlation of the ankle-hindfoot function to the three-dimensional morphological characteristic of calcaneal fractures after surgical treatments based on the post-processing technique of CT images.Methods From December 2009 to January 2012,the original data of clinical information and CT scan of 146 patients with calcaneal fractures was collected in five hospitals according to the inclusion criteria,the exclusion criteria and the exit criteria.The original data was scanned by 16-detector row CT scanner and the clinical information was collected from five hospitals.The restoration of the subtalar facet was observed with the volume renderings.The surface shaded display (SSD) reconstruction combining three-dimensional measurement was utilized to measure the calcaneus related morphological parameters.The correlation study was carried out between the American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and the outcome after observation and measurement was analyzed.Results No significant correlation was found between the AOFAS score and the length of the calcaneus,the height of the posterior facet and the length of the posterior facet in male and female group.A significant inverse correlation was found between the Gissane angle and the AOFAS score in male group as well as in female group.A significant positive correlation was found between the B(o)hler angle and the AOFAS score in male group as well as in female group.The congruity of the subtalar joint was observed in 38 cases,and the excellent and good rate of the AOFAS score was 89.5％.Unflat articular surface was found in 108 cases,and the excellent and good rate of the AOFAS score was 90.7％.Conclusion The restoration of ankle-hindfoot function was affected largely by part of angular parameters after calcaneal fractures,such as the Gissane angle and the Gissane angle; nevertheless part of morphological parameters and the congruity of the subtalar facet had no correlation to the restoration of ankle-hindfoot functions.
