Abstract:BACKGROUND:The induced expression of heme oxygenase-1 (HO-1) in donor islets improves allograft survival. Cobalt protoporphyrin (CoPP) could significantly enhance the expression of HO-1 mRNA and protein in rat islet safely. Our work was to study how to protect pancreatic islet xenograft by CoPP-induction.METHODS:Islet xenografts treated with CoPP-induction and CoPP + Zinc protoporphyrin (ZnPP) in vitro and in vivo were randomly transplanted into murine subrenal capsule; then the graft survival time was compared by blood glucose level and pathological examination and meanwhile the interferon γ (IFN-γ), tumor necrosis factor α (TNF-α), interleukin 10 (IL-10) and IL-1β level in serum and their mRNA and HO-1 mRNA and protein expression were examined.RESULTS:Islets with CoPP-induction under low- and high-glucose stimulation exhibited much higher insulin secretion compared with other three groups. CoPP-induction could increase higher expression of HO-1 (mRNA: 3.33- and 76.09-fold in vitro and in vivo; protein: 2.85- and 58.72-fold). The normoglycemia time in induction groups ((14.63 ± 1.19) and (16.88 ± 1.64) days) was significantly longer. The pathological examination showed less lymphocyte infiltration in induction groups. The IL-10 level and its mRNA in induction groups were significantly higher.CONCLUSIONS:The HO-1 induced by CoPP would significantly improve function, prolong normoglycemia time and reduce lymphocyte infiltration. Meanwhile CoPP-induction in vivo had more beneficial effects than in vitro. Its mechanism could be related to immune-modulation of IL-10.

Abstract:Objectlve To explore the mechanism of the protection in high expression of HO-1 induced by CoPP on murine islet xenografts. Method An islet transplantation of a SD rat-to-C57 BL/6 mouse model was established.Mice were randomized into five groups i.e.control,CoPP-induction in vivo,CoPP+ZnPP in vivo.CoPP-induction in vitro and CoPP+ZnPP in vitro and the islet xenografts were transplanted into the subrenal capsule.Normoglycemia time was recorded and insulin-releasing test was performed.IL-10、TNF-α、IL-1β and INF-γ in serum and their cytokine mRNA and HO-1 in xenografts were measured by RT-PCR and Western-blotting.The pathological examination was done to observe the lymphocyte infiltration. Results There Was significant difference in the normoglycemia time between CoPP-induction in vivo and in vitro and other three groups.The results of insulin-releasing stimulated by low level glucose were identical among groups,but that of insulin-releasing stimulated by high-glucose in in vivo group were the hiishest as in CoPP-induction in vivo and in vitro and control group were 187.68 ±19.93、137.22±11.73,91.25±12.64 μIU·ml~(-1)·10islets~(-1)·45 min~(-1),(P<0.05).The IL-10 in serum in CoPP-induction in vivo and in vitro(in vivo:72.97±9.74 pg/ml;in vitro:70.84±3.56 pg/ml)was significantly hisher than other three groups(control:30.57±3.93 pg/ml;CoPP+ZnPP in vivo:39.78±3.00 pg/ml;CoPP+ZnPP in vitro:35.42±4.30 pg/ml).The expression tendency of IL-10 mRNA was similar to that of insulin secretion.There was no significant difieFence in TNF-α、IL-1β and INF-γ.The expression of HO-1 by PCR and Western-blot analysis in CoPP-induction in vivo and vitro was higher than other three groups.The pathological examination showed that fewer lymphocytes infiltrated into the islet xenografts from CoPP-treated in comparison with xenografts from other three groups. Conclusion HO-1 could improve the survival of islet xenograft:the induetion of HO-1 expression in vivo was much mole efficient than in vitro.The CoPP-induction could be related to immune modulation of IL-10.
The Protective Effect of Heme-Oxygenase-1 in Isolated Islets Xenotransplantation

Abstract:Objective To investigate the protective effect of heme-oxygenase-1(HO-1) in islets xenotransplantation and its probable mechanism. Methods Male SD rats were used as donors and male C57BL/6 mouse as recipients. Thirty-three recipients were randomly divided into 3 groups according to different mode of pretreatment for donor islets. Group A: islets were cultured for 36 h; Group B: islets were cocultured with 50 mmol/L of cobalt-protoporphyrin (CoPP), a HO-1 inducer, for 36 h; Group C: islets were cocultured with 50 mmol/L of CoPP and 50 mmol/L of zinc protoporphyrin (ZnPP), a HO-1 inhibitor, for 36 h. The expression of HO-1 mRNA and protein were detected by RT-PCR and Western-Blot respectively. Time of normoglycemia was observed after transplantation. The serum concentration of IFN-γ, TNF-α, IL-10 and IL-1β were detected by ELISA. Results The expression of HO-1 mRNA and protein of Group B were significantly higher than those of other groups (P0.01). The time for maintenance of normoglycemia of Group B was (14.63±1.19) d,significantly longer than that of Group A(9.88±2.17) d and Group C (9.38±1.60 d) (both P0. 01), while there was no significance between groups A and C(P0.05). The serum concentration of IL-10 in group B was (72.97±9.74) pg/mL, significantly higher than those of Group A (30.57±3.94) pg/mL and Group C (45.55±8.26) pg/mL(both P0.05), while it had the same level in groups A and C(P0.05). No significant difference in IFN-γ, TNF-α and IL-1β concentration was noted among the 3 groups(P0.05). Conclusions HO-1 induced by CoPP in vitro enhances xenotransplantation of islets. IL-10 might play a role in the process.
Ginsenoside Rg1 Improves Inflammation and Autophagy of the Pancreas and Spleen in Streptozotocin-Induced Type 1 Diabetic Mice.

Abstract:Background. Ginsenoside Rg1 (Rg1) is one of the key bioactive components of the precious Traditional Chinese Medicine that has been used to treat diabetes in China. Ginsenosides have been reported to protect diabetics from tissue damage, inflammation, and insulin resistance. Type 1 diabetes (T1D) is an organ-specific autoimmune disease that occurred frequently among adolescents over the world, its development was related to inflammation and β-cells immunodeficiency. The aim of this study is to explore the biological mechanism of Rg1 on inflammation and autophagy of β-cells in T1D and its therapeutic potential. Methods. The model of T1D mice was established by injecting Streptozotocin (STZ) (55 mg/kg) or citric acids once a day for 5 days and from the fourth day of injection, mice were administered with Rg1 (20 mg/kg) or saline by gavage once a day for 12 days. Hematoxylin-eosin staining, immunofluorescence, ELISA, quantitative real-time PCR, and Western blot were used to observe the histopathological changes, inflammatory factor levels, and autophagy markers after administration of ginsenoside Rg1. Results. Compared to the T1D mice, Rg1 improved the weight (p<0.05) and blood glucose (p<0.01) of mice, advanced the injury and apoptosis of β-cells in islets (p<0.01), and markedly inhibited the protein expression degrees of CD45, CXCL16, ox-LDL, and TF in the pancreas and spleens (p<0.01), also activated the degrees of insulin in serum (p<0.01). Besides, in T1D mice’ pancreas and spleen, Rg1 markedly repressed the IL-1β, TNF-α, and NOS2 mRNA levels (p<0.05 or p<0.01), inhibited the CXCL16, NF-κB, and TF proteins (p<0.05 or p<0.01), while elevating the ratio of LC3 II/I (p<0.01) and P62 (p<0.05) protein level. Conclusions. This study proved that Rg1 protected mice against T1D possibly by improving islet injury and tissue inflammation, raising serum insulin, and tissue autophagy marker.

Abstract:Objective To analyze the dose-effect relationship between cobalt protoporphyrin (CoPP) and heme oxygenase-1 (HO-1) expression in islets and to investigate the protective effect of strongly expression of HO-1 in islet xenotransplantation. Methods Donor islets isolated and purified from SD rats were randomly divided into 5 groups and incubated with different doses of CoPP for 24 h.Group A: 0 mmol/L; Group B: 5 mmol/L; Group C: 25 mmol/L; Group D: 50 mmol/L; Group E:75 mmol/L. The expression of HO-1 mRNA and protein in islets was detected by RT-PCR and Western blot respectively. Glucose of low and high concentrations was added to islets in vitro to test insulin-releasing function. The optimal dose of CoPP which could induce the strongest HO-1 expression was chosen according to the results. Recipients were randomly divided into 2 groups. Control group received untreated xeno-islets, and the experimental group received islets incubated with optimal CoPP close in vitro. Glycemia and rejection were observed after transplantation daily. Results The expression of HO-1 mRNA and protein in xeno-islets of group D was significantly higher than in other groups (P＜0.05). After stimulation of glucose, the insulin concentration in group D was significantly higher than in other groups (P＜0.05). The optimal dose of CoPP which could induce the strongest HO-1 expression was 50 mmol/L. The time for normoglyeemia in experimental group was (14.63±1.19) days, significantly longer than that in control group (9.88±2.17)days (P＜0.01). Conclusion The strongest expression of HO-1 induced by CoPP in vitro promotes the glucose-stimulated insulin secretion of islets and prolonged the survival of xeno-islet grafts by protecting them from rejection.
Protective Effects of Heme Oxygenase-1 on Severe Acute Pancreatitis Model: in Vivo Study

Abstract:Objective Objective:To investigate the protective effects of hemo oxygenase-1 (HO-1) on the pancrease and liver in severe acute pancreatitis (SAP) rat model, and explore its possible mechanism. Methods A total of 70 male SD rats were randomly divided into 4 groups: (1) control group (n=10), (2) SAP group (n=20), 3% Sodium Cholate was retrogradedly injected into the pancreatic duct to produce the SAP model, (3) HO-1 stimulation group (n=20), 75 μg/kg hemin was injected intraperitoneally at 30 minutes after model establishment, and (4) HO-1 inhibition group (n=20), 20 μmol/kg ZnPP was injected intraperitoneally at 30 minutes after model establishment. Ten rats in each group were sacrificed 24 hours after the SAP model establishment, and the other 10 were remained for survival observation. Serum concentration of IL-10, HO-1 and TNF-α were detected by ELISA assay, serum ALT, AST, TBIL,BUN, and Creatinine were determined via biochemistry analysis. The NF-κB p65 protein expression in pancrease and liver tissues were detected by immunohistochemistry staining. Meanwhile, HE staining was performed to observe the histopathological changes in different groups. Results Compared with those in SAP model group, the serum HO-1 and IL-10 concentrations were significantly elevated in HO-1 stimulation group (P0.05), while the serum TNF-α was significantly decreased (P0.05). The hepato-renal parameters were ameliorated (P0.05) in HO-1 stimulation group. The severity of pathological changes of the pancrease and liver in HO-1 stimulation group were also palliated. Conclusions The results of this in vivo study demonstrated that HO-1 over-expression might compensate the damages of SAP on the pancrease and liver, as well as the hepato-renal function. Up-regulated IL-10 expression, down-regulated NF-κB and TNF-αmight be served as its potential mechanism.

Abstract:To maintain islets survival and function is critical in successful pancreatic transplantation. Pancreatic progenitors cells (PPCs) with lineage potentials, giving rise to exocrine, endocrine, and duct cells, reside in developing and adult pancreas. As tissue-specific stem cells, they can produce pancreatic tissue-specific matrix factors to promote islets survival and function. The aim of our research was to investigate the protective effect of rat pancreatic–duodenal homeobox 1 (Pdx1) + /nestin + PPCs on islets. In vitro, co-culturing islets with Pdx1 + /nestin + PPCs prolonged the former survival from 7 to 14 days. Furthermore, with high glucose (300.8 mg/dl) stimuli, the yield of insulin in co-cultures was significantly higher than that in control group (single islets group). In vivo, co-transplanting islets and Pdx1 + /nestin + PPCs for 3 days, the blood glucose of diabetic rat was significantly decreased to normal level and sustained for 2 weeks. Without Pdx1 + /nestin + PPCs in islets transplantation, hyperglycemia was reversed at day 7 and recovered at day 15. Pathology analysis showed that islets had remnants in co-transplantation at day 21, as complete graft rejection in alone islets transplantation. Our study showed that Pdx1 + /nestin + PPCs displayed the ability of preserving islets viability and function in vitro and prolonging their survival in vivo.

Blockade of the OX40/OX40L Pathway and Induction of PD-L1 Synergistically Protects Mouse Islet Allografts from Rejection.

Abstract:Background OX40/OX40 ligand (OX40/OX40L) and programmed death-1/programmed death ligand-1 (PD-1/PD-L1) costimulatory signals play important roles in T cell-induced immune responses. The aim of this study was to investigate the roles of OX40/OX40L and PD-1/PD-L1 costimulatory pathways in mouse islet allograft rejection.Methods Lentiviral vectors containing OX40L siRNA sequences and an adenovirus vector containing the PD-L1 gene were constructed. The streptozotocin-induced model of diabetes was established in C57BL/6 (H-2(b)) mice. Diabetic C57BL/6 mice were randomly allocated into five groups: group 1, untreated control; group 2, Ad-EGFP treatment; group 3, Ad-PD-L1 treatment; group 4, OX40L-RNAi-LV treatment; group 5, OX40L-RNAi-LV combined with Ad-PD-L1 treatment. Lentiviral vector and the adenovirus vector were injected, singly or combined, into the caudal vein one day before islet transplantation. The islets of DBA/2 (H-2(d)) mice were transplanted into the renal subcapsular space of the diabetic recipients. Recipient blood glucose and the survival time of the allografts were monitored. Antigen-specific mixed lymphocyte reaction was also evaluated.Results The recombinant lentiviral RNA interference vector OX40L-RNAi-LV reduced OX40L protein expression by 70%. The recombinant adenovirus vector Ad-PD-L1 increased PD-L1 protein expression in vivo in C57BL/6 recipient mice. Combined OX40L-RNAi-LV/Ad-PD-L1 treatment induced a synergistic protective effect in pancreatic islet allografts. Allograft survival time in the combined treatment group was (92.27+/-9.65) days, not only longer than that of the control ((6.51+/-0.27) days) and Ad-EGFP groups ((7.09+/-0.13) days) (P<0.01), but also significantly longer than that of Ad-PD-L1 and OX40L-RNAi-LV single treatment groups ((40.64+/-3.95) days and (55.14+/-5.48) days respectively, P<0.01). The blood glucose concentration of recipient mice in the combined treatment group was also stable and kept within the normal range. Flow cytometry analysis showed that combined OX40L-RNAi-LV/Ad-PD-L1 treatment significantly decreased proliferation in an antigen-specific mixed lymphocyte reaction. After donor DBA/2 lymphocyte stimulation, 89.71% of lymphocytes from recipient combination treatment C57BL/6 mice were not split and proliferated. In contrast, after stimulation with third party Lewis rat lymphocytes, only 45.84% lymphocytes of C57BL/6 mice were not split and proliferated.Conclusions This study demonstrates the successful construction of the recombinant lentivirus vector OX40L-RNAi-LV and adenovirus vector Ad-PD-L1 for the blockade of OX40/OX40L and activation of PD-1/PD-L1 costimulatory pathways simultaneously in pancreatic islet allografts in diabetic mice. Combination therapy with these two vectors resulted in inhibition of T cell activation, synergistically prolonging the survival time of pancreatic islet allografts.

Abstract:Objective To investigate the effect of Heine oxygenase-1(HO-1)gene transfer on the function of cultured rat islets.Methods Adenovirns vector containing human HO-1 gene(Ad-HO-1) or enhanced green fluorescent protein gene(Ad-EGFP)was employed to transfect rat islets.Transfection was confirmed by the expression of EGFP and human HO-1 protein at 48 h post-transfection.Different groups of islets were stimulated by low glucose(2.8 mmol/L),and then high glucose(16.7 mmol/L). The insulin release upon glucose stimulation was detected using insulin radioimmunoassay kit,and stimula- tion index(SI)was calculated.Results After culture for 7 days,the insulin release upon either low or high glucose stimulation was significantly lower than that of fresh islets[(4.57±0.40)vs(6.47±0.55)mIU/L/30 IEQ,(9.63±0.71)vs(14.93±1.17)mIU/L/30IEQ,P<0.05].There was no sig- nificant difference in the insulin release upon low glucose stimulation among different groups of islets after culture for 7 days(P>0.05),but the insulin release upon high glucose stimulation of HO-1 group was significantly higher than that of EGFP group and control group[(12.50±2.17),(8.87±0.65)and (9.63±0.71)mIU/L/30 IEQ respectively,P<0.05].The SI in HO-1 group was also significantly high- er than in EGFP group and control group[(2.21±0.02),(2.08±0.05)and(2.11±0.03)respec- tively,P<0.05].Conclusion HO-1 gene transfer can protect the function of cultured rat islets.

Abstract:Objective: To investigate whether human heme oxygenase-1 (HO-1) gene has protective action on islets cultured in vitro, and to explore whether transduction of HO-1 gene to donor islets could enhance engrafted islets survival and suppress local lymphocytic infiltration in islet grafts. Methods: Newly isolated rat islets were isolated from the Sprague-Dawley rats and were divided into 3 groups in vitro study as follows: enhanced green fluorescent protein (EGFP) group, islets transduced with adenovirus vectors containing EGFP gene using multiplicities of infection (MOI) = 2, 5, 10, and 20 to determine the transduction efficacy; HO-1 group, islets transduced with adenovirus vectors containing human HO-1 gene using MOI = 20; and control group, mock transduced islets. Flow cytometry was used to detect apoptotic cells after induction by recombinant human tumor necrosis factor-alpha (rTNF-alpha) and cycloheximide (CHX) for 48 hours. Diabetic recipients were randomly divided into the following groups: HO-1 group (n = 9), receiving islets transduced with recombinant adenovirus-HO-1; control group (n = 9), receiving mock transduced islets; and phosphate buffer solution (PBS) group (n = 6), receiving only 0.8 mL PBS. About 1200 rat islet equivalents were transplanted into each recipient rendered by streptozotocin using the portal vein as transplant site. Allograft survival, apoptosis, and the state of lymphocytic infiltration were analyzed. Results: After treatment with rTNF-alpha and CHX, the apoptotic ratio of islet cells was 4.22% +/- 2.38% in the HO-1 group (MOI = 20), significantly lower than 23.81% +/- 8.51% in the control group (P < 0.05), and 28.76% +/- 14.76% in the EGFP group (MOI = 20; P < 0.05). Maintenance of normoglycemia was prolonged in the HO-I group, indicated by results that islet survival time was 10.56 +/- 4.33 days significantly longer than that of untreated islets which was 5.33 +/- 4.18 days (P < 0.05). The lymphocytic infiltration degree in engrafted islets treated with HO-1 gene was lower than that in the control group. Conclusions: HO-1 gene overexpression in rat islets by adenovirus transduction can protect cultured islets against rTNF-alpha and CHX-mediated cytotoxicity. HO-1 gene has cytoprotective effects on engrafted islets, which could prolong engrafted islets survival in allogenic transplantation model, and diminish the degree of lymphocytic infiltration in islet grafts. These findings suggest a potential therapeutic application for HO-1 gene in improving islet survival/function in human islet transplantation.
Effect of Heme Oxugenase-1 on Delayed Xenograft Rejection: Experiment of Guinea Pig-to-rat Liver Xenotransplantation

Abstract:OBJECTIVE:To investigate the effects and mechanism of heme oxygenase-1 (HO-1) in liver xenotransplantation and mechanism thereof. METHODS:Thirty male guinea-pigs used as donors were injected intravenously with cobra venom factor (CVF) and then randomly divided into 3 groups 24 hours after: Group A injected intraperineally with NaCl, Group B injected intraperineally with cobalt-protoporphyrin (CoPP), heme oxygenase-1 inducer, and Group C injected intraperineally with CoPP and zinc protoporphyrin (ZnPP), HO-1 inhibitor zinc before their livers were harvested. Thirty male SD rats used as recipients underwent the above-mentioned treatment 24 hours before receiving the xenografts. Five pairs of guinea pigs and rats in each group underwent collection of blood and liver tissues 3 hours after the recovery of blood perfusion in the transplanted livers for detection of serum enzymes by biochemical methods and expression of HO-1 mRNA and protein in the transplanted livers by RT-PCR and Western blotting respectively. The other 5 pairs in each group were used to observe the survival time. RESULTS:The survival time of Group B was 15.5 h +/- 3.8 h, significantly longer than those of Group A (7.3 h +/- 2.1 h) and Group C (6.7 h +/- 2.9 h, both P < 0.01). The values of ALT and LDH of Group B were significantly lower than those of Group A and C (all P < 0.05). HOI-1 mRNA expression was not detected or only expressed in trace amount in the livers of normal guinea pigs, expressed in a small amount in the transplanted livers of Group A. The expression of HO-1 mRNA and that of HO-1 protein in the transplanted livers of Group B were significantly higher than those of Group A (both P < 0.01), and the expression of HO-1 mRNA and that of HO-1 protein in the transplanted livers of Group C were not significantly different from those of Group A (both P > 0.05). Remarkable NF-kB band was detected in Groups A and C, and only weak NF-kB band was seen in Group B. The E-selectin expression was significantly lower in the transplanted livers of Group B than in those of Group A and C (both P < 0.05). CONCLUSION:HO-1 delays the occurrence of delayed xenograft rejection in liver xenotransplantation. This effect depends, at least in part, on HO-1-mediated inhibition of endothelium activation in xenografts.
Effects of pre-vascularization of tissue-engineered constructs on the survival of transplanted pancreaticislets

Abstract:BACKGROUND:Pancreatic islet transplantationviaportal vein system leads to the apoptsis of a number of islet cels due to local hypoxia,therebyaffecting transplant outcomes. OBJECTIVE:To explore theeffect ofpre-micrvascularization network of tissue-engineered constructs on the survival of transplanted islets and the feasibility of xenogenic islet transplantation. METHODS:A 5-mm-long cylindrical silicone tube filed with Matrigel TM matrix surrounding the superficial epigastric vessel was placed in the groin ofdiabetic mice. After the syngeneic islets with 300 islet equivalents (IEQ) were transplanted into the silicone chamber on days 0, 14 and 28 post-chamber implantation, respectively, the recovery time of blood glucose was observed. The islets with the quantity of 100 IEQ, 200 IEQ and 300 IEQ, respectively, were transplanted on day 28 post-implantation and then the blood glucoselevelwas determined. Moreover, the survival of human pancreatic islets with 1 000 IEQ transplanted into the pre-vascularizated chamber or under the renal capsule of diabetic mice, folowed by the treatment of anti-CD45RB and/or anti-CD40L (MR-1) was analyzed. RESULTS AND CONCLUSION:An abundant micro-vascularized network was established in the silicone chamber on day 28 post-implantation. The time of the blood glucose returningto normal level in diabetic mice was negatively correlated with the time required for pre-vascularization and the number of implanted islets. No islet grafts implanted in the silicone chamber and treated by anti-CD45RB survived for long term. However, one of seven (14.3%) grafts survived for long term, which was not significantly different from the transplantation under the renal capsule group (n=8, MST > 71 days,P> 0.05). The tissue-engineered pre-vascularization network markedly extends the survival time of the islet grafts before transplantation. The transplantation of the xenogenic pancreatic islets into the vascularized silicone chamber might be a promising method in the future clinical application.

Abstract:BACKGROUND:Islet transplantation represents an ideal therapeutic approach for treatment of type 1 diabetes but islet function and regeneration may be influenced by necrosis or apoptosis induced by oxidative stress and other insults. Heme oxygenase-1 (HO-1) is the rate-limiting enzyme in the catabolism of heme into biliverdin, releasing free iron and carbon monoxide. It has also been reported to be an antioxidant enzyme which can improve the function of grafted islets by cytoprotection via free radical scavenging and apoptosis prevention. In the present study, we investigated whether transduction of HO-1 genes into human islets with an adenovirus vector has cytoprotective action on islets cultured in vitro and discuss this method of gene therapy for clinical islet transplantation.METHODS:Cadaveric pancreatic islets were isolated and purified in vitro. Transduction efficiency of islets was determined by infecting islets with adenovirus vector containing the enhanced green fluorescent protein gene (Ad-EGFP) at multiplicities of infection (MOI) of 2, 5, 10, or 20. Newly isolated islets were divided into three groups: EGFP group, islets transduced with Ad-EGFP using MOI = 20; HO-1 group, transduced with adenovirus vectors containing the human HO-1 gene using MOI = 20; and control group, mock transduced islets. Insulin release after glucose stimulation of the cell lines was determined by a radioimmunoassay kit and the stimulation index was calculated. Flow cytometry was used to detect apoptotic cells in the HO-1 group and in the control group after induction by recombinant human tumor necrosis factor-alpha (rTNFalpha) and cycloheximide (CHX) for 48 hours.RESULTS:Adenovirus vectors have a high efficiency of gene transduction into adult islet cells. Transduction of islets with the Ad-EGFP was most successful at MOI 20, at which MOI fluorescence was very intense on day 7 after transduction and EGFP was expressed in cultured islet cells for more than four weeks in vitro. The insulin release in the control group was (182.36 +/- 58.96) mIU/L after stimulation by high glucose media (16.7 mmol/L), while insulin release from the HO-1 group and the EGFP group were (270.09 +/- 89.37) mIU/L and (175.95 +/- 75.05) mIU/L respectively. Compared to the control group and the EGFP group, insulin release in the HO-1 group increased significantly (P < 0.05). After treatment with rTNFalpha and CHX the apoptotic ratio of islet cells was (63.09 +/- 10.86)% in the HO-1 group, significantly lower than (90.86 +/- 11.25)% in the control group (P < 0.05).CONCLUSIONS:Transduction of human islets with Ad-HO-1 can protect against TNF-alpha and CHX mediated cytotoxicity. The HO-1 gene also appears to facilitate insulin release from human islets. Transduction of donor islets with the adenovirus vector containing an HO-1 gene might have potential value in clinical islet transplantation.
Inhibition of Heparanase Protects Against Pancreatic Β-Cell Death in Streptozotocin-Induced Diabetic Mice Via Reducing Inflammatory Cell Infiltration

Abstract:Background and Purpose Intra-islet heparan sulfate (HS) plays an important role in the maintenance of the pancreatic islet function. The aim of this study was to investigate the effect mechanism of HS loss on the functioning of islets in diabetic mice. Experimental Approach The hypoglycemic effect of a heparanase inhibitor, OGT2115, was tested in streptozotocin-induced diabetic mice. The islets of pancreas sections were also stained to reveal their morphology. An insulinoma MIN6 cell line and primary isolated murine islets were used to investigate the effect of OGT2115 in vitro. Key Results Intra-islet HS was clearly lost in streptozotocin-induced diabetic mice due to the increased heparanase expression in damaged islets. OGT2115 prevented intra-islet HS loss to improve the glucose profile and insulin secretion in streptozotocin-treated mice. The apoptosis of pancreatic beta cells, the infiltration of mononuclear macrophages, CD4 and CD8 positive T-cells in islets was reduced by OGT2115 in streptozotocin-treated mice, but OGT2115 did not alter the direct streptozotocin-induced damage in vitro. The expression of heparanase was increased in high glucose-treated isolated islets but not in response to direct streptozotocin stimulation. Further experiments showed that high glucose stimuli could decrease the expression of peroxisome proliferator-activated receptor gamma (PPARγ) in cultured islets, thereby relieving the PPARγ-induced inhibition of heparanase gene expression. Conclusion and Implications Hyperglycemia could cause intra-islet HS loss by elevating the expression of heparanase, thereby aggravating inflammatory cell infiltration and islet damage. Inhibition of heparanase might provide benefit for pancreatic beta cell protection in type 1 diabetes.

Abstract:Activation of NF-kappa B pathway and co-stimulatory system CD40/CD40L promotes the inflammation, which plays a key role in the failure of islet graft. Therefore, the purpose of this study was to determine if simultaneous blockade of CD40/CD40L and I kappa B/NF-kappa B pathways could protect islet graft. Streptozocin-induced diabetic Wistar rats were transplanted intraportally with 2000 IEQ islets isolated from Sprague-Dawley rats. The rats were divided into five groups: nontreatment group, AdGFP-treated group, Ad-I kappa B alpha-treated group, Ad-sCD40LIg-treated group, and Ad-I kappa B alpha-IRES2-sCD40L-treated group. The islet graft mean survival time (MST), insulin expression of islet grafts, and the levels of cytokines in peripheral blood, were measured for the animals in each group. Our study confirmed that islet cells transfected with low doses of adenovirus could achieve high transfection efficiency, and would not affect the function of islet cells (P > 0.05). Splenocytes cultured with Ad-I kappa B alpha-IRES2-CD40L-transfected islets resulted in homospecific hyporesponsiveness. The islet graft MST (>100 d) in the Ad-I kappa B alpha-IRES2-sCD40L-treated group was dramatically prolonged compared with that in the nontreatment group (7.1 +/- 1.16 d). In addition, TNF-alpha, IL-1 beta, and IFN-gamma were diminished in the Ad-I kappa B alpha-IRES2-sCD40L-treated group, which was commensurate with the reduced cellular infiltration (P < 0.01). Simultaneous blockade of the CD40/CD40L and I kappa B/NF-kappa B pathways could effectively extend the survival of islet grafts.

Abstract:Apoptosis during engraftment and inflammation induce poor islet xenograft survival. We aimed to determine whether overexpression of human heme oxygenase-1 (HO-1) or soluble tumor necrosis factor-α receptor type I with human IgG1 Fc (sTNF-αR-Fc) in porcine islets could improve islet xenograft survival. Adult porcine islets were transduced with adenovirus containing human HO-1, sTNF-αR-Fc, sTNF-αR-Fc/HO-1 or green fluorescent protein (control). Humanized mice were generated by injecting human cord blood-derived CD34(+) stem cells into NOD-scid-IL-2Rγ(null) mice. Both HO-1 and sTNF-αR-Fc reduced islet apoptosis under in vitro hypoxia or cytokine stimuli and suppressed RANTES induction without compromising insulin secretion. Introduction of either gene into islets prolonged islet xenograft survival in pig-to-humanized mice transplantation. The sTNF-αR-Fc/HO-1 group showed the best glucose tolerance. Target genes were successfully expressed in islet xenografts. Perigraft infiltration of macrophages and T cells was suppressed with decreased expression of RANTES, tumor necrosis factor-α and IL-6 in treatment groups; however, frequency of pig-specific interferon-γ-producing T cells was not decreased, and humoral response was not significant in any group. Early apoptosis of islet cells was suppressed in the treatment groups. In conclusion, overexpression of HO-1 or sTNF-αR-Fc in porcine islets improved islet xenograft survival by suppressing both apoptosis and inflammation. HO-1 or sTNF-αR-Fc transgenic pigs have potential for islet xenotransplantation.
Protective Mechanism of Ischemic Preconditioning on Ischemic/reperfusion Injury after Pancreas Transplantation in Rats

Abstract:Objective To investigate the protective mechanism of ischemic preconditioning on ischemic/reperfusion injury after pancreas transplantation in rats.Methods The model of diabetic SD rat was established. Twenty-four diabetic SD rats were randomly assigned to ischemic/reperfusion group (I/R group, n=6) and ischemic preconditioning group (IPC group, n=18). The rats in group IPC were averagely assigned to 3 sub-groups: group IPC_ 1 (5 min ischemic and 5 min reperfusion), IPC_ 2 (5 min ischemic and 5 min reperfusion twice) and IPC_ 3 (5 min ischemic and 5 min reperfusion thrice). Six normal SD rats whose abdomen was opened only served as control group, and they did not receive pancreas transplantation. I/R group and IPC group received pancreas transplantation. The superoxide dismutase (SOD) and myeloperoxidase (MPO) activity of grafts were monitored 2 h after reperfusion, the apoptotic cells in grafts were observed by TUNEL method, and the expression of Bcl-2 and Bax gene of the grafts was detected by Western blot.Results As compared with I/R group, the SIOD activity and the expression of Bcl-2 gene of grafts were significantly increased, while MPO activity, apoptotic index and the expression of the Bax gene in the grafts were markedly reduced in IPC group (P 0.05 ). Among the IPC groups, group IPC_ 2 showed the most significant difference (P 0.05 ).Conclusions Ischemic preconditioning can reduce apoptosis of the grafts after pancreas transplantation in rats, especially IPC_ 2 protocol. The possible mechanism might be as follows: relieving conglutination and aggregation of neutrophils, decreasing oxygen radical, up-regulating the expression of Bcl-2 gene and down-regulating the expression of Bax gene.
Heme Oxygenase 1 Prolongs the Survival of Acute Cardiac Xenograft Rejection

Abstract:In order to investigate the effect of heme oxygenase 1(HO-1) to prolong the survival of acute cardiac xenograft rejection,the guinea pig to rat cardiac allograft model was established,and the model animals were pre-treated with hemin before transplantation to up-regulate the expression of the HO-1 gene,with the concomitant use of cobra venom factor(CVF) and cyclosporin A(CsA) to prevent the occurrence of the hyperacute rejection.The expression of HO-1 in cardiac tissues and the levels of the serum xeno-reactive antibody IgM and the deposits of the xeno-reactive antibody IgM and C3 in vascular tunica intima in the experimental animals were detected respectively.In addition,apoptosis of the cardiac tissue cells and the level of CD40L mRNA were determined with TUNEL and RT-PCR assay respectively. The experimental results showed that the over-expression of HO-1 could prolong the survival of the acute cardiac allograft rejection and reduce the occurrence of serum xeno-reactive antibody IgM as well as the xeno-reactive antibody IgM and C3 deposits in vascular tunica intima.Apoptosis of myocardial cells could bereduced and the expression of CD40L mRNA could be suppressed with the expression of HO-1 in cardiac cells in the period of acute vascular rejction.It is evident the expression of heme oxygenase 1 suppresses the acute vascular rejection and prolongs the survival of the acute xenograft rejection in guinea pig to rat cardiac xenograft model through blockage of the CD40-CD40L costimulatory pathway.
