Abstract:PURPOSE:This study aimed to analyze the clinicopathological features, treatment, and feto-maternal outcomes of pregnancy complicated by malignant ovarian germ cell tumors (MOGCTs), to increase the awareness on this condition.PATIENTS AND METHODS:We retrospectively reviewed the medical records of patients diagnosed with MOGCTs during pregnancy, who were treated and followed-up at Peking Union Medical College Hospital from January 2000 to December 2017. The demographic characteristics, pathological features, treatment and prognosis were analyzed.RESULTS:The histological subtypes varied in 14 patients (dysgerminoma, n=1; immature teratoma, n=4; yolk sac tumor, n=6; and mixed germ cell tumors, n=3). Ten (71.4%) patients, including three who opted for conservative therapy until childbirth, one who only received salvage chemotherapy during pregnancy, and six who underwent cystectomy or unilateral salpingo-oophorectomy during pregnancy, desired fetal preservation. After undergoing surgery, four patients chose surveillance instead of timely adjuvant chemotherapy. Eight patients delivered their babies, and the preterm delivery rate was 50.0%. One newborn died of premature birth. The median follow-up period was 44 (range: 13 to 86) months. During the current study period, 12 patients had survived and did not report any diseases. However, two died due to disease progression.CONCLUSION:Pregnant women with MOGCTs had favorable outcomes. However, when a malignant tumor is suspected, surgery cannot be avoided. Thus, instead of timely postoperative adjuvant chemotherapy, close surveillance may be an acceptable alternative for pregnant women with low-risk MOGCTs.

Abstract:Purpose: The aim of this study was to investigate the clinical characteristics and management of malignant ovarian tumors during pregnancy, as well as the feto-maternal outcomes and analyze the influential factors on the pregnancy outcomes. Patients and Methods: Eighty-five patients with ovarian malignancies during pregnancy treated at 12 tertiary hospitals between 2009 and 2019 were analyzed in this study. The clinical features, histopathological characteristics, clinical management, and maternal and perinatal outcomes were retrospectively analyzed. The clinical features and managements were compared between abortion group and live birth group. Results: The following diagnoses were made: 41 (48.24%) patients with borderline ovarian tumors, 18 (21.18%) patients with epithelial ovarian cancers, 17 (20.00%) patients with non-epithelial ovarian malignancies and 9 (10.59%) patients with metastatic ovarian tumors. Thirty-si x (42.45%) patients underwent conservative surgical treatment. Thirty-four (40.00%) patients opted for fertility-sparing surgery, and fifteen (17.56%) patients received radical surgery. Chemotherapy was administered to 32.94% of the patients. The proportion of ovarian malignancies diagnosed in the first trimester in the abortion group was higher than that in the live birth group (P<0.05). However, tumor diameter, reproductive history, stage and surgical indications showed no significant differences between groups. A total of 67 live babies were recorded in this study, including 19 premature babies and 1 full-term newborn who died of respiratory distress. All of the BOTs were diagnosed with stage I, among whom 38 (92.68%) patients exhibited disease-free survival. Twenty-eight ovarian cancers were in stage I-II and 26 of them had disease-free survival with the longest follow-up time of 10 years. Five of the sixteen patients in advanced stage (stage III-IV) died, four of whom had metastatic tumors. Conclusion: Pregnant women with early-stage malignant ovarian tumors appear to have favorable outcomes. Conservative surgery is acceptable for early-stage borderline ovarian tumors during pregnancy The gestational age of ovarian malignancy detection is key for pregnancy outcomes.
Taxanes in Combination with Platinum Derivatives for the Treatment of Ovarian Cancer During Pregnancy: A Literature Review

Abstract:Ovarian cancer is one of the most common types of solid carcinoma diagnosed during pregnancy. Taxane plus a platinum derivative is a combination therapy that is predominantly used in the treatment of ovarian cancer in non-pregnant women. Pregnancy adds various complexities to a course of treatment. In pregnant patients diagnosed with cancer during the first trimester, the risks of fetal malformations and fetal loss increase following the administration of cytotoxic drugs, and this is higher with multi-agent vs. single-agent chemotherapy (similar to 25 vs. 10%). Exposure during the second and third trimester has little influence on teratogenic effects but increases the risk of intrauterine growth retardation, prematurity, low birth weight, and bone marrow toxicity. The present study aimed to review the maternal and fetal safety of treatment with taxane plus platinum derivatives for ovarian cancer during pregnancy. Relevant literature was retrieved from the Embase and PubMed databases using the search terms "ovarian cancer", "pregnancy", "taxane", "paclitaxel", "docetaxel", "platinum", "cisplatin", and "carboplatin". All available data up until September 2016 was synthesized, with no language restrictions. A total of 11 articles (including 13 pregnancies and 14 newborns) were retrieved that reported on the use of standard-dose taxane and platinum chemotherapy, including 9 cases treated with paclitaxel and carboplatin, 3 cases treated with paclitaxel and cisplatin, and 1 case treated with docetaxel and cisplatin. In 13 of the 14 (92.9%) births included, a healthy neonate was born, with follow-up ranging from 2 to 160 months. The average weight of the neonates at the time of delivery was 2,442.1 g. In 7 of 9 the case reports that provided survival data, the mother was alive and disease-free at the end of follow-up (ranging from 2 to 40 months). In conclusion, combination therapy with taxanes and a platinum derivative may play a significant role in the management of pregnant patients with ovarian cancer during the second and third trimester. Exposure to this combination of agents during the second and third trimester does not appear to have a significant bearing on fetal mortality and abortion.

Abstract:OBJECTIVE:To determine the clinical characteristics and management of ovarian tumor in infertility women.METHODS:During the period from Jan. 1 1999 to Dec. 31 2004, 110 infertility women with ovarian tumor were analysed retrospectively, in terms of clinical and pathological features. According to the histological findings, the tumors were divided into three groups: epithelial tumor, teratoma and others.RESULTS:Among 110 cases, 49 cases were epithelial tumor, 42 cases were teratoma, and 19 cases were others. A total of 97 cases were small ovarian tumor, and the maximal diameter was less than 5 cm. In epithelial tumor group only 29% were diagnosed before operation, significantly lower than the other two groups (P < 0.01). In epithelial tumor group 61% were complicated with pelvic inflammatory disease (PID), significantly higher than teratoma group (26%, P < 0.01). A total of 102 cases (91%) were followed-up for 1.5 to 5 years. A total of 45 patients (44%) had become pregnant. Among them 18 conceived spontaneously, 21 underwent in vitro fertilization-embryo transfer, and 6 received artificial insemination.CONCLUSIONS:Most ovarian tumors found in infertility women are small cysts and only a few epithelial tumors are diagnosed before the operation. Physicians should be alert of infertility women with ovarian mass and laparoscopy should be performed as early as possible. Our data show the incidence of PID in epithelial tumor group is higher than others. After treatment by surgery and assisted reproductive technology, those infertility women with ovarian tumor may achieve fair pregnancy outcomes.

Abstract:Objective To study the association of management of ovarian tumors during pregnancy and its obstetrical outcome.Methods One hundred and seventeen pregnant women with ovarian tumors treated at Beijing University Third Hospital between January 1992 and December 2003 were reviewed.Different treatment effected on pregnant outcomes was analyzed.Results Torsion and rupture of ovarian cyst as a complication of acute abdomen was higher in patients with ovarian tumors.Those pregnant women treated conservatively in second trimester had higher incidence of acute abdomen complications than those patients who underwent elective celiotomy or emergency laparotomy for removal of the masses in second trimester.Premature delivery was more frequently on patients who underwent surgery treatment during third trimester.Conclusions Our findings suggest that the best time for surgery treatment of ovarian tumors in pregnancy is during second trimester.Surgery in third trimester may increase the risk of premature delivery.
Large Malignant Ovarian Tumors During Pregnancy: Two Cases

Abstract:The present study reports two cases of large ovarian malignancy during pregnancy, which is very rare. The two patients were received between Nov 2012 and Feb 2013 at Gynecological and Obstetrical Department, Women's Hospital, School of Medicine, Zhejiang University (People's Republic of China). Both cases present tumor sized more than 20 cm, with one case of 40 cm. Both patients underwent timely cesarean section, with survival of the Child, and successful removal of the tumor. All patients showed good outcome in the follow-up period. Therefore the large ovarian malignancy during pregnancy could be well treated after careful clinical evaluation.
Ovarian tumor in pregnancy: A review of 60 cases

Abstract:Objective To study the clinical characters, diagnosis, and treatment of ovarian tumor in pregnancy. Methods 60 cases with ovarian tumor in pregnancy were reviewed. Results The incidence of pregnancy with Ovarian tumor was 5. 30 (60/11 396). Tumor-like lesions were the most common cases (31/60, 51.7%), followed by benign tumors (27/60, 45.0%)and malignant tumors (2/60, 3.3%). Mature cystic teratoma accounted for 30.0%, endometrioma cyst 20. 0%, adnexal torsion 11. 7%, the rupture of the tumor 3. 3%, and the arrest of descent of the presentation 3. 3%, Of the 60 cases, 53 cases were presented as term delivery, 4 premature birth, 2 spontaneous abortion, and 1 induced abortion. No spontaneous abortion occurred in the 5 patients who underwent laparotomy during 16 to 22 weeks of gestation. Conclusion To avoid adverse pregnancy outcome, excision of ovarian tumor during 16 to 22 weeks of gestation is recommended.

Abstract:Objective To analysis the diagnosis,treatment and outcome of malignant tumor in pregnancy,in order to improve the outcome of mother and fetal.Methods Data of 9 pregnant patients with malignant tumor who were treated in Shanghai Jiaotong University No.6 People's Hospital from 2007 to 2010 were reviewed.The patients include 2 lung cancer,2 gastric carcinoma,1 hepatocellular carcinoma,1 Nasopharyngeal carcinoma,1 brain neurogliocytoma,1 breast adenocarcinoma and 1 ovarian cancer.Results Of the 9 patients,only 4 had regular prenatal examination;2 patients were diagnosed before pregnancy,the other 7 were diagnosed during pregnancy;7 patients were diagnosed by pathology and 2 patients were diagnosed by clinical imageology.7 patients were in advanced stage,the other 2 were in early stage.Pregnant outcomes included 1 early induced abortion,2 middle induced abortion,2 vaginal delievery and 4 cesarean section delievery.As for the fetus outcome,there were 5 preterm infant and 2 term infant;6 newborn infants had good outcome,1 preterm fetal dead in uterus;5 patients received treatment after delivery,other 4 didn't received any treatment after delivery;6 patients died during the first year's follow-up.Conclusions The deficient of the recognition of malignant tumor,lack of full-scale perinatal monitoring,short of perfect analyses of history,symptoms and sign of malignant disease and the delayed treatment for malignant tumor being considering the adverse effect on fetal were the main causes of the delay diagnosis and treatment of malignant disease in pregnancy.The pregnancy with malignant tumor was associated with poor maternal outcome but no unnormal fetal development was found.
Multicenter randomized controlled clinical study for the operative treatment of malignant ovarian germ cell tumors

Abstract:Objective To investigate the operative treatment for first-treated patients with malignant ovarian germ cell tumors who need preservation of fertility.Methods The clinical data of 105 patients who were treated with fertility-sparing surgery in 11 hospitals from 1992 to 2010 were collected to evaluate the outcomes of different primary surgical operative procedures.All 105 cases were performed the surgeries that preserved fertility and divided into three groups according to the surgical approaches,comprehensive staging surgery group:47 cases (44.8％) received comprehensive staging surgeries that including the ipsilateral oophorectomy + omentectomy + retropertoneal lymph node dissection ± appendectomy + multiple biopsies;oophorcctomy group:45 cases (42.9％) received ipsilateral oophorectomy ± biopsy of contralateral ovary ±omentectomy;tumor resection group:13 cases(12.4％)received enucleation of the mass with preservation of the ovary.Differences were compared among the three groups of patients in the surgery-related indicators,complications,fertility and prognosis.Results (1) Surgery-related indicators:the average blood loss of the comprehensive staging surgery group,the oophorectomy group and the tumor resection group were 496,104 and 253 ml,the mean operation time were 176,114 and 122 minutes,respectively,and there were significant differences among three groups (P =0.011,P =0.000).(2) Complication:the surgical complication rates of the three groups were 17％ (8/47),0 and 1/13,with significant differences (P =0.015).(3) Reproductive function status:the pregnancy rate and birth rate of the three groups were no significant differences(9/19 vs.7/19 vs.2/3,P =0.515 ;8/19 vs.5/19 vs.2/3,P =0.636).(4) Prognosis:the recurrence rate of the three groups were significant differences[13％ (6/47) vs.0 vs.2/13,P =0.013],but the death rate with no significant differences[6％ (3/47) vs.0 vs.0,P =0.129] ;The five-year survival rate of three different groups were 89％,100％ and 100％ (P ＞ 0.05),while disease free survival rate were 85％,100％ and 83％ (P ＜ 0.05),respectively.Conclusions Compared with comprehensive staging surgery,oophorectomy group have higher surgical security and satisfactory prognosis,considerable pregnancy rates and birth rate.The tumor resection security may be reliable,but the prognosis is poor.
Clinical analysis of 236 pregnant patients with ovarian tumor

Abstract:Objective The aim of this study was to describe the general characteristics of the ovarian tumor in pregnant women at Obstetrics department of Peking University Shenzhen Hospital. Methods A retrospective analysis focusing on clinical characters, diagnosis & treatment and pregnancy outcomes was performed among patients treated at Obstetrics department of Peking University Shenzhen Hospital between 2012-2017. Results A total of 236 cases of ovarian tumors were confirmed by surgery and pathology within 6 years, of which ovarian benign tumors accounted for 52. 54％ (123/236), followed by tumor-like lesions (42. 80％, 101/236) and borderline and malignant tumors (4. 66％, 11/236). The most common pathological types were mature cystic teratomas, accounting for 24. 15％ (57/236), followed by ovarian corpus luteum cysts and ovarian endometriotic cysts, which were 14. 41％ (34/236). Ovarian tumor pedicle torsion was the most common complication, accounting for 5. 51％ (13/236), of which 4 were mature cystic teratomas, 1. 27％ (3/236) of ovarian tumor ruptures, and 1 case of ovarian purulent inflammation with abscess formation, accounting for 0. 42％ (1/236), 2 cases of hospitalization due to compression symptoms in secondary or the third trimester pregnancy, accounting for 0. 85％ (2/236), which were both borderline ovarian and malignant tumors. Of the 236 cases, 203 cases were presented as term delivery, 30 premature births, and 2 induced abortions. Of nineteen patients who underwent laparotomy for lower abdominal pain or abdominal distension, 8 patients underwent cesarean section for termination of pregnancy. Six of them had premature delivery and there was no neonatal death. Ten patients underwent adnexectomy or cystectomy and continued pregnancy, and all of them were with a full-term delivery and a maternal-fetal prognosis. Conclusions Ovarian tumors of pregnant women are mainly benign. Ovarian malignant tumors are difficult to find during pregnancy and should be excluded before pregnancy.
Ovarian Cancer Immature Teratoma Type in Pregnancy: Management and Feto-Maternal Outcomes

Abstract:Background: Immature teratoma is malignant ovarian germ cell tumours (MOGCTs). The case in pregnancy is very rare which less than 1% of all ovarian teratoma cases. The aim is to reach optimal and comprehensive management for immature ovarian teratoma in pregnancy to gain the healthiest maternal and fetal outcomes. Case presentation: Thirty-one years old female G2P1A0, 8 weeks 1-day pregnancy, with left ovarian solid tumour 15 x 15 x 15 cm in size. At gestational age (GA) of 19 weeks 5 days, the size of the tumour was increasing rapidly to 30 x 30 x 30 cm. Alfa-fetoprotein raised to 699.9 IU/mL and LDH 579 U/L. The patient had gone primary conservative left oophorectomy, omentectomy, and ascites fluid cytology with histopathological conclusion grade II immature teratoma of left ovary containing the immature neuroepithelial and fat component: magnetic resonance imaging (MRI) at 25 weeks 3 days GA, no spreading. Amniocentesis performed at 27 weeks 2 days GA, the fetus had normal 46 chromosomes and sex XX without major structural abnormality. The patient had BEP chemotherapy start at 27 weeks 2 days GA. Patient in labour at 40 weeks 2 days GA. The female baby had spontaneous delivery with 2700 grams in body weight without congenital abnormality. Complete surgical staging performed at 58th days postpartum and histopathological result there was no malignant cell anymore, but post-chemotherapy ovarian atrophy feature had found on the contralateral ovary. The patient showed psychosocial problem including post-chemotherapy depression and premature ovarian failure (POF). Immunohistochemistry (IHC) ER and PR of teratoma tissue showed immature component had ER (-) and PR (+). Follow up of the baby was in good condition. Conclusion: BEP chemotherapy become regimen choice for this case with fetal outcomes was good, but there was a POF sign on the mother. Survival of patient on this case is 62%, free recurrence survival post-BEP 84% and progressivity post complete surgical staging 8% without delay the chemotherapy.
The pregnancy related problems after fertility-sparing therapy in non-benign ovarian tumor patients

Abstract:Objective To investigate the factors related to the ability of pregnancy, maternal and fetal outcomes,and problems in prenatal care after fertility-sparing therapy in non-benign ovarian tumor patients. Methods Patients, who were treated with fertility-sparing therapy for borderline or malignant ovarian tumors in People's Hospital, Peking University between January 2000 and April 2010, not elder than 40-year-old, and nulliparous, were followed up. Only the patients who succeeded in following-up, wished to be pregnant, and had no contraception for more than 1 year were divided into pregnant and non-pregnant group for the statistical study. Results 57 patients meet to the criteria. Among them, 42 patients were succeeded in contact, and 23 patients were studied finally. 12 patients were pregnant, and 11 patients were non-pregnant. The median time from the last surgery or chemotherapy to pregnancy was 10 months (range 3～60 months). The non-pregnant group had significant higher proportion in patients who were sterile before, or treated with staging surgery, or with more than stage I tumors than the pregnant group. The ratio of patients who were treated with oophorectomy in the non-pregnant group were higher than that of the pregnant group.The differences were statistically significant (P<0.05). The common maternal and fetal complications were premature rupture of membrane, abortion, premature delivery and hyperbilirubinemia. 3 relapses in pregnant group were reported, of which, 2 happened during their late pregnancy. Conclusions The medical history of infertility, the type of conservative surgery, staging surgery or not, and stage may influence the pregnant ability of patients with borderline or malignant ovarian neoplasms after conservative treatment. Once pregnant, the prenatal care should be strengthened, in particular, the recurrence of tumors.
Early Stage Primary Ovarian Mucinous Carcinoma: Outcome-based Clinicopathological Study in Comparison with Serous Carcinoma.

Abstract:OBJECTIVE:To compare clinicopathological characteristics and survival rates between patients with primary ovarian mucinous carcinoma and those with primary ovarian serous carcinoma.METHODS:This retrospective study reviewed archival tumour specimens, originally diagnosed as primary ovarian mucinous carcinoma, using refined histological criteria. All patients were contacted to establish survival status. Clinicopathological characteristics and patient survival data were compared with a group of control patients with primary ovarian serous carcinoma.RESULTS:Of the 33 patients originally diagnosed with primary ovarian mucinous carcinoma, this diagnosis was only confirmed in 18. Primary ovarian mucinous carcinoma was more commonly associated with early International Federation of Gynecology and Obstetrics tumour stages and low-grade histology than primary ovarian serous carcinoma. Patients with primary ovarian mucinous carcinoma had a significantly higher overall 5-year survival rate than those with primary ovarian serous carcinoma (12/12 [100%] versus 14/24 [58%]). Kaplan-Meier survival plots demonstrated that patients with primary ovarian mucinous carcinoma had a survival advantage over patients with primary ovarian serous carcinoma.CONCLUSIONS:Primary ovarian mucinous carcinomas are frequently low-grade, stage I tumours and have an excellent prognosis.
Clinical analysis on 112 cases of ovarian tumors during pregnancy

Abstract:To explore the clinical characteristics,diagnosis,treatment,prognosis of ovarian tumor in patients during pregnancy.A retrospective analysis of 112 cases of ovarian tumor during pregnancy was carried out,and the patients were treated,operated and pathologically examined.About 96.43% of the 112 cases were diagnosed as benign tumors and tumour-like lesions.Their diameters of ovarian tumor were more than 6 cm among 9 emergent operations,13 cases who had chosen surgical operation during 16 ～ 20 weeks of pregnancy for experienced pregnancy process safely.Ultrasound characteristics were of principal importance in the diagnosis and detection of ovarian tumor during pregnancy.Patients with ovarian tumors greater than 6 cm had frequent complications.Suitable surgery did not influence the outcome of the pregnancy.
The significance of comprehensive staging surgery in malignant ovarian germ cell tumors.

Abstract:The diagnosis of malignancy, particularly brain tumors, in pregnancy is uncommon but poses a complex dilemma for the management of both the patient and her fetus, as the interplay of disease with the physiological state of pregnancy affects both outcomes. The routine evaluations (symptomatology, imaging, and hormonal assessments) and treatments (surgery, radiation therapy, and chemotherapy) that are commonplace in brain tumor management may need to be omitted or modified keeping in mind the risk to offspring. Multidisciplinary care and extensive prenatal and perinatal counseling and monitoring are essential. In this review, we discuss the available data addressing these issues and factors which may affect considerations of therapeutic abortions, changes in surgical or medical practices, and outcomes thereof.

Abstract:Purpose To analyze the clinical characteristics and prognosis of pregnancies that are complicated by cervical cancer and to identify appropriate therapeutic protocols. Patients and Methods Clinical data were acquired from 14 subjects attending hospitals in Beijing and Shanghai, China, between January 2015 and December 2019. Then, we analyzed clinical characteristics, diagnosis, treatments, and prognosis. Results Of the 14 cases, there were two cases of abortion, two cases of chemotherapy during the second trimester, two cases of cold knife conization, and eight cases of expectant management and extended postpartum treatment. The gestational age ranged from 28 to 40 weeks. All subjects and infants survived except for the fetuses that underwent abortions. Conclusion Early diagnosis of cervical therapy, and the provision of multivariate therapy, can prolong gestational age and improve the survival rate of both pregnant women and infants.
Feto-maternal outcome of pregnancy complicated by vulvar cancer: a systematic review of literature

Abstract:Vulvar cancer is an extremely rare complication during pregnancy, and its effect on pregnancy and survival is not well understood. A systematic literature review was conducted in order to examine the fetal and maternal outcomes and optimal management of pregnancy complicated by vulvar cancer. PubMed/MEDLINE were used to identify case reports with searching keywords "pregnancy" and "vulvar cancer" between January 1955 and February 2014 that identified 36 cases for analysis. Mean age was 30.7. The most common presenting symptom and gestational age were vulvar mass/swelling (75.0%) and the second trimester of pregnancy (54.8%), respectively. Vulvar biopsy at the time of initial presentation to care during pregnancy was performed in only 46.7% of cases. Among delayed cases for biopsy, mean duration of delay was 12.8 weeks and the majority had a delay for more than 8 weeks (62.5%). The majority of vulvar cancer was squamous histology (47.2%) and stage I disease (60.0%). Vulvectomy and inguinal-femoral lymphadenectomy were performed in 97.1% and 63.9%, respectively. Abdominal delivery was recorded in 46.2% of cases. Live birth and full term delivery rates were 96.3% and 74.0%, respectively. For survival analysis, delay in diagnosis and advanced stage disease were commonly associated with decreased disease-free survival (5-year rate, delay in diagnosis >8 versus ≤8 weeks, 0% versus 69.1%, hazard ratio (HR) 7.86, 95% confidence interval (CI) 2.03-30.6, p=0.001; and stage III-IV versus stage I-II, 0% versus 59.8%, HR 3.35, 95% CI 1.16-9.68, p=0.011) and overall survival (5-year rate, delay in diagnosis >8 versus ≤8 weeks, 0% versus 67.1%, hazard ratio (HR) 14.8, 95% CI 1.77-124, p=0.001; and stage III-IV versus stage I-II, 0% versus 86.4%, HR 8.22, 95% CI 2.06-33.2, p<0.001). In conclusion, while the majority of cases resulted in good pregnancy outcomes, diagnosis of vulvar cancer during pregnancy is frequently delayed. Since delayed diagnosis is a significant prognosticator of decreased survival outcomes, early recognition is integral in the management of pregnancy complicated by vulvar cancer.
Diagnosis, detection and treatment of ovarian tumor during pregnancy

Abstract:Objective To explore ovarian tumor's characteristics,diagnosis,monitoring,treatment as well as its prognosis during pregnancy.Methods A retrospective analysis of 82 cases of ovarian tumor during pregnancy,which were treated,operated and pathologically conducted in Shanghai First Maternity and Infant Health Hospital from January,2003 to December,2005,was carried out.Results About 97.56% of the 80 cases were diagnosed as benign tumors,majority of which were demoid cysts.For the other 2 cases,one was clear cell tumor and the other was borderline mucinous cystadenoma.Fourteen cases were diagnosed by ultrasound examination during the first trimester(48.28%).Among 8 urgent operations,5 of them were caused by torsion,three by rupture.Serum samples for CA125 estimation were obtained from 11 patients prior to their operations.As a result,7 cases were elevated,one of which was borderline mucinous cystadenoma.As high as 95.12% of the cases reached to term.Conclusion Ultrasound characteristics are of principal importance in the diagnosis and detection of ovarian tumor during pregnancy.Suitable surgery intervention does not influence the outcome of the pregnancy.The ability of CA125 levels value in distinguishing benign from malignant disease remains uncertain and needs to be further studied.
Oncological and Reproductive Outcomes in Patients With Advanced-Stage Ovarian Immature Teratoma: Experience From a Tertiary Center

Abstract:Objective To evaluate the oncological and reproductive outcomes in patients with advanced-stage ovarian immature teratoma (IMT). Methods We retrospectively reviewed the medical records of patients with advanced-stage IMT who were treated with surgery between January 1985 and December 2020. Fertility-sparing surgery (FSS) was defined as preservation of the uterus and at least one adnexa. Oncological outcomes were compared between patients who underwent FSS and radical surgery. Patients who underwent FSS were also contacted to gather information about their menstrual history and reproductive outcomes. Results Forty-six patients fulfilled the inclusion criteria, of whom 38 underwent FSS and eight were treated with radical surgery. Fifteen patients suffered recurrence after a median follow-up time of 74.2 months (range: 4.1–434.1 months). The 5-year disease-free survival (DFS) and overall survival (OS) rates were 69.1% and 89.9%, respectively. Multivariate analysis identified suboptimal cytoreductive surgery as the only independent risk factor for recurrence. There was no significant difference in DFS or OS between patients with different surgical procedures. Ten of the 15 relapsed patients had optimal salvage surgery and all remained alive with no evidence disease. Among the 32 patients who underwent FSS, 29 resumed menstruation after surgery, and five of seven patients who designed pregnancy achieved a total of five successful pregnancies. Conclusions Ovarian IMT has a favorable prognosis, even when diagnosed at an advanced stage. FSS is feasible in patients with advanced-stage IMT who wish to preserve their fertility. Patients may benefit from optimal cytoreductive surgery during initial and salvage surgery.
