The contribution of germline genetic variants to ALL susceptibility varies substantially by tumor subtype [eg, ARID5B polymorphism rs10821936 has a particularly strong effect on disease risk for hyperdiploid ALL]. This is also true for PIP4K2A variants. Following our genome-wide association study, two recent reports independently confirmed the association of the BMI1–PIP4K2A locus with ALL susceptibility and went further to show that rs7088318 was preferentially related to hyperdiploid ALL (1,2). Examining our original data, we also observed similar enrichment of the PIP4K2A risk allele in this subtype (P = .01; n = 972). It is unclear which ALL subtypes were included by Lopez et al. (or the proportion of pediatric cases), and one can speculate that the lack of association at rs7088318 in their cohort was due to undersampling of hyperdiploid ALL. Second, as Lopez et al. already pointed out, the A allele frequency at rs7088318 was substantially higher in their control subjects compared with that in our study and other non-ALL populations of European descent (eg, HapMap CEU). This directly influenced their association signal at this locus and also raises questions regarding the genetic ancestry of populations in Spanish Basque Country. Because rs7088318 risk allele frequency in their ALL subjects was comparable with that in our original cohort, population stratification between their ALL case patients and control subjects is possible. Further analysis of their data by testing the association with genetic ancestry as covariables would be informative. Future follow-up is warranted to comprehensively canvas the variants at this locus and identify causal variant(s) and, importantly, to describe the biological pathways linking BMI1–PIP4K2A to leukemogenesis.
