Abstract:0bjective To make genetic and prenatal diagnosis of a female with Haemophilia A.Methotis The FⅧ:C.BT and VWF were detected to make phenotypic diagnosis.LD-PCR was adopted for screening the intron 22 inversion and PCR was adopted for the screening the intron 1 inversion.The coding and boundary sequences of FⅧgene were analyzed by PCR and DNA equencing.Eight combined polymorphie markers(Amelo,F8IVS13,CA22,DXS15,DXS9901,G6PD,DXS1073 and DXS1108)were applied for linkage analysis of the family by multiplex fluorescent PCR.The polymorphism of DXS52 (ST14)was analyzed by PCR and electrophoresis. Assessment of X inactivation was performed using an Hpa II-polymerase chain eaction (PCR)assay for the X-inked human androgen receptor gene(HUMARA). Results The female HA patient showed severe FW deficiency(FⅧ:C 2.1%)and other phenotypie tests were normal.Her family members showed normal in all tests.The female proposita was found to be a carrier of FW gene intron 22 inversion.But her family members as well as her etus showed negative results.Except this inversion,no other mutation Wag found then.The female inherited two X chromosomes from both her parents' and her fetus inherited the maternally derived X chromosome from the female proposita according to the linkage analysis.Furthermore.X-inactivation paRern of the female was unbalanced and her aternally derived X chromosome Wag inaetived mostly while the majority of her paternal derived one kept active.Conclusions The severe haemophilia A in the proposita resulted from the de novo Ⅷ intron 22 inversion which most probably arose in the paternal germ line.Associated with a skewing pattern of inactivation of the maternally derived X chromosome.Her etus is normal female.
A female haemophilia caused by FⅧ gene intron 1 inversion and non-random inactivation of X-chromosome

Abstract:Objective To perform gene analysis in a female hemophilia A patient and to explore its molecular pathogenesis.Methods APPT,PT,FⅧ:C,FⅨ:C and vWF were tested for establishing phenotypic diagnosis.LD-PCR and sequence specific PCR were adopted for detecting FⅧgene intron 22 and intron 1 inversion.All the extrons of FⅧgene and its flank sequences were sequenced.Hereditary linkage analysis was performed on the basis of 8 polymorphic locus in and out of FⅧgene.Agar electrophoresis was used to assess PCR products and to analyze the polymorphism of DXS 52(ST14)locus.Multiplex fluorescent PCR was used to test the polymorphism of 7 STR locus(F8Civs 13,CA22, DXS15,DXS9901,G6PD,DXSI073 anti DXS1108).Methylation sensitive HpaⅡendonuclease was used to slice the genome DNA.Fluorescent PCR was used to amplify the CAG repeated sequence of human androgen receptor gene (HUMARA)in genome DNA before and after HpaⅡendonuclease slicing.The amplified product was assessed by fluorescent scanning.Non-randnm inactivation of X chromosome was determined by change of peak value of PCR produet before and after endonuclease slicing.Results This female hemophilia patient had prolongation of APTT(128.5 see)and marked deerease of FⅧ:C(<1%).Other 2 patients of this family also had marked deerease of FⅧ:C.Inversion of intron 22 of FⅧgene was negative in all members of this family.This female hemophilia patient had inversion of intron 1 ofⅧgene indicating thai she was a positive carrier;2 male patients of this family also had inversion of intrnn 1.No other mutation of FⅧgene was found in this female hemophilia patient.Hereditary linkage analysis showed that the chromosome canying pathogenic gene was derived from her mother.Non-random inactivation of X-chromosome occurred in this female hemophilia patient;the X-chromosomes derived from her father was mostly inactivated,while the X-chromosomes from her mother was mostly conserving their activity.Conclusions The severe haemophilia A in this female patient results from the FWm gene intron 1 inversion which was inherited from her maternal germ line,associated with a non-random inactivation of her paternally derived X chromosome.
A Pericentric Inversion of Chromosome X Disrupting F8 and Resulting in Haemophilia A

Abstract:Aims The frequency of X chromosome pericentric inversion is much less than that of autosome chromosome. We hereby characterise a pericentric inversion of X chromosome associated with severe factor VIII (FVIII) deficiency in a sporadic haemophilia A (HA) pedigree.Methods PCR primer walking and genome walking strategies were adopted to identify the exact breakpoints of the inversion. Copy number variations (CNVs) of the F8 and the whole chromosomes were detected by AccuCopy and Affymetrix CytoScan High Definition (HD) assays, respectively. A karyotype analysis was performed by cytogenetic G banding technique.Results We identified a previously undescribed type of pericentric inversion of the X chromosome [inv(X) (p11.21q28)] in the proband with FVIII: C < 1%. One breakpoint was located in the intron 7 of the F8, which disrupted the transcription of the F8, and the other located in the upstream of the PFKFB1 of the X chromosome. The inversion segment was approximately 64.4% of the total chromosomal length. The karyotype analysis of the X chromosome confirmed the pericentric inversion of the X chromosome in the proband and his mother. A haplotype analysis traced the inversion to his maternal grandfather, who was not a somatic mosaic of the inversion. This finding indicated that the causative mutation may originate from his germ cells or a rare possibility of germ-cell mosaicism.Conclusions The characterisation of pericentric inversion involving F8 extended the molecular mechanisms causing HA. The pericentric inversion rearrangement involves F8 by non-homologous end joining is responsible for pathogensis of severe HA.
Skewed X-Chromosome Inactivation and Parental Gonadal Mosaicism Are Implicated in X-Linked Recessive Female Hemophilia Patients

Abstract:Background: Hemophilia A (HA) and B (HB) are X-linked recessive disorders that mainly affect males born from a mother carrier. Females are rarely affected but a number of mechanisms have been suggested in symptomatic females, such as skewed X-chromosome inactivation (XCI), chromosomal rearrangements, and hermaphrodites. Different methodologies are required to elucidate the underlying causes of such diseases in female patients. Methods: Three families with female hemophilia patients, including two HA and one HB, were enrolled for genetic analyses. Cytogenetics, molecular examinations on F8 and F9 genes, XCI assay, and linkage analysis were performed. Results: All three female patients are demonstrated to be heterozygous for an F8, or F9 mutation: one patient is inherited from her unaffected mother and the other two are sporadic cases. All three patients exhibit skewed XCI. The inherited patient is found to be unmethylated in the maternal X chromosome, which increases the potential for the expression of the mutant allele. The two sporadic cases are hypomethylated or unmethylated in the paternal X chromosome, suggesting that paternal gonadal mosaicism may exist in these families. Conclusions: In addition to screening for coagulation function, different genetic analyses are mandatory to explore the nature of mechanisms responsible for the X-linked recessive disorders in female patients as shown in this study. Our results confirm that skewed XCI is responsible for hemophilia in heterozygous female patients. Likewise, our results implicate that parental gonadal mosaicism, followed by skewed XCI, contributes to hemophilia in "sporadic" female patients.

Abstract:INTRODUCTION:At present, many methods are available for the genetic diagnosis of haemophilia, including indirect linkage analysis, direct sequencing. However, these methods are time-consuming, labourious, and limited in their application. Therefore, the development of new, more effective techniques is necessary.AIM:To detect the F8 and F9 gene mutations in patients with haemophilia and their female relatives in 29 haemophilia A (HA) and 11 haemophilia B (HB) families.METHODS:FVIII:C and FIX:C were analyzed using one-stage method, and factor VIII and factor IX inhibitors were tested using the Bethesda method. Intron 22 and one inversions were identified using long-distance polymerase chain reaction (PCR) and standard PCR. Non-inversion mutations of the F8 and F9 gene were identified by targeted high-throughput sequencing. All mutations were verified by Sanger sequencing.RESULTS:Intron 22 inversion was detected in eight HA families and intron one inversion was detected in one HA family. Apart from the inversion mutations, 20 mutations were identified in HA families, including 17 previously reported and three novel mutations: c.5724G>A (p.Trp1908*), c.6116-1_6120delGAGTGTinsTCC (p.Lys2039Ilefs*13), and c.5220-2A>C. We found a complex rearrangement in HA: intron one inversion concomitant with exon one deletion. In HB, eight recurrent mutations were detected, including six missense mutations and two nonsense mutations.CONCLUSION:Targeted high-throughput sequencing is an effective technique to detect the F8 and F9 gene mutations, especially for the discovery of novel mutations. The method is highly accurate, time-saving and shows great advantage in uncovering large deletion mutations and also in distinguishing the wild-type genotype and heterozygous large deletions.
Molecular pathogenesis of two novel splice site mutations of F8 in hemophilia A

Abstract:Objective: To study the molecular pathogenesis of two novel splice site mutations of F8 in hemophilia A and to provide the evidence for genetic counseling. Methods: Coagulation tests were performed to establish the diagnosis of hemophilia A. A serial of genetic tests were performed to analyze F8, including directly sequencing the PCR amplification products of 26 exons and flanking regions of F8, detection of copy number variations(CNVs) and detection of intron 1 and 22 inversion for defining the pathogenetic mutation. Nested PCR was applied to detect F8 ectopic transcripts in leukocyte to analyze the molecular pathogenesis of the splice site mutations. Multiplex fluorescent PCR was used to detect six STR locus(F8 Up226, F8 Up146, F8 Int13, F8 Int25, F8 Down48 and DXS1073) for gene linkage analysis. Mosaic analysis was performed with peripheral blood cells, oral mucosal cells and hair follicle cells by SNa Pshot SNP technique to analyze the origin of mutation. Results: Levels of FⅧ:C were 0.9% and 5.1% in probands 1 and 2, respectively. Proband 1 presented with a splice site mutation in intron 9(c.1444-2 dup A); m RNA analysis showed a smaller band, resulting from exons10 and 11 skipping. Proband 2 had a T >G substitution in intron 18(c.5999-29 T >G); m RNA analysis showed a smaller band and a weak normal size band. The smaller band presented exon 19 skipping. In pedigree 1, the mutation was inheri-ted from proband’s grandfather who was with normal phenotype and genotype. Mosaic analysis showed no mosaic existing in his peripheral blood cells, oral mucosal cells and hair follicle cells. Conclusions: Mutations of c.1444-2 dup A and c.5999-29 T>G are first reported,which cause severe and mild hemophilia A, respectively. c.1444-2 dup A in pedigree 1 is a de novo mutation which might arise from sperm of proband’s grandfather at meiosis.

Abstract:Objectives: Hemophilia A (HA, OMIM: 306700) is an X-linked recessive bleeding disorder, caused by defects of the F8 gene which encodes the coagulation factor VIII (FVIII). F8 intron 22 and intron 1 inversion (Inv22 and Inv1) account for ∼45% and 1-5% of severe HA cases, respectively. We herein described an aberrant Inv1 with concomitant large duplication and deletion in a Chinese severe HA patient. Methods: Long distance PCR and multiplex PCR were used to detect Inv22 and Inv1. Multiplex ligation-dependent probe amplification (MLPA) was applied to examine exonic duplication and deletion of the F8 gene. Coverage analysis of read depth data from whole-genome sequencing (WGS) was used to analyze the intronic duplication and deletion of the F8 gene. Results: We have identified an aberrant F8 Inv1 in a 1-year-old Chinese severe HA patient showing inversed int1h-1 and normal int1h-2. Coverage analysis of WGS data further illustrated the aberrant Inv1 with concomitant a duplication of 117 kb and a deletion of 1.8 kb. Conclusion: In conclusion, we reported an aberrant Inv1 with concomitant large duplication and deletion in a severe Chinese HA patient. Moreover, WGS provides rapid genetic diagnosis of hereditary disorders with point mutations, deletions, insertions and CNVs.
A Novel Deletion Mutation of the F8 Gene for Hemophilia A

Abstract:Background: Hemophilia A (HA) is an X-linked recessive blood coagulation disorder caused by a variety of abnormalities in F8 gene, resulting in the absence of impaired molecule production of factor VIII (FVIII) in the plasma. The genetic testing of the F8 gene encoding FVIII is used for confirmation of HA diagnosis, which significantly reduced serious complications of this disease and, ultimately, increased life expectancy. Methods: Sanger sequencing was performed in F8 gene exons of the suspected patients with blood coagulation-related indicators. Results: A novel F8 indel variant c.6343delC, p.Leu2115SerfsTer28 in exon 22 of the F8 gene was identified in the suspected families. The infant with this novel variant appeared the symptom of minor bleeding and oral cavity bleeding, and decreased activity of FVIII, which is consistent with that of F8 deleterious variants. The 3'D protein structural analysis of the novel variant shows a change in FVIII protein stability, which may be responsible for the pathogenesis of HA. Conclusions: A novel deleterious variant was identified in our case, which expands the F8 variants spectrum. Our result is helpful for HA diagnosis and benefits carrier detection and prenatal diagnosis. Our study also reveals that mutation screening of the F8 gene should be necessary for HA suspected patients.

Molecular Diagnosis of Hemophilia A in Chinese Patients by an Analysis of Inversions in the Factor VIII Gene.

Abstract:Hemophilia A is an X-linked bleeding disorder caused by deleterious mutations in the factor VIII gene. An inversion caused by introchromosomal homologous recombination between the A gene located in intron 22 of the factor VIII gene and one of the two telomeric A genes has been recently described as the common cause of about 50% of cases of severe hemophilia A. The rearrangement can be readily detected by a Southern blotting procedure. We report use of this procedure to detect rearrangements in 106 unrelated Chinese hemophilia A cases. In 49.3% of the patients with severe disease an inversion was found, but no inversion was detected in any of the patients with moderate or mild disease. The majority of inversions (91.4%) involved the most distal A gene; in a minority (8.6%) the more proximal A gene was involved. These results indicate that intron 22 inversion is the most important molecular defect causing Chinese hemophilia A and that analysis for intron 22 inversion may be the first-line test in the molecular diagnosis of severe hemophilia A.

Abstract:A 5‐year‐old female diagnosed with severe hemophilia B began experiencing frequent muscular and joint bleeds at 19 months old. Molecular studies, including Sanger sequencing, Giemsa banding, human androgen receptor (HUMARA) assay, array‐based comparative genomic hybridization (aCGH), whole‐exome sequencing (WES), and multiplex ligation‐dependent probe amplification (MLPA), revealed a heterozygous factor IX (F9) intron 3 substitution (c.277+1G>T) inherited from her mother and a de novo heterozygous 441 kb deletion in the Xq28 region, which flanked intron 22 homologous regions 1 (int22h1) and 2 (int22h2). This rare genetic profile explains her severe phenotype and guides hereditary consultation for family planning.

Abstract:: Hemophilia A is a bleeding disorder caused by coagulation factor VIII protein deficiency or dysfunction, which is classified into severe, moderate, and mild according to factor clotting activity. An overwhelming majority of missense and nonsense mutations occur in exons of F8 gene, whereas mutations in introns can also be pathogenic. This study aimed to investigate the effect of an intronic mutation, c.6430-3C>G (IVS22-3C>G), on pre-mRNA splicing of the F8 gene. We applied DNA and cDNA sequencing in a Chinese boy with hemophilia A to search if any pathogenic mutation in the F8 gene. Functional analysis was performed to investigate the effect of an intronic mutation at the transcriptional level. Human Splicing Finder and PyMol were also used to predict its effect. We found the mutation c.6430-3C>G (IVS22-3C>G) in the F8 gene in the affected boy, with his mother being a carrier. cDNA from the mother and pSPL3 splicing assay showed that the mutation IVS22-3C>G results in a two-nucleotide AG inclusion at the 3' end of intron 22 and leads to a truncated coagulation factor VIII protein, with partial loss of the C1 domain and complete loss of the C2 domain. The in-silico tool predicted that the mutation induces altered pre-mRNA splicing by using a cryptic acceptor site in intron 22. The IVS22-3C>G mutation was confirmed to affect pre-mRNA splicing and produce a truncated protein, which reduces the stability of binding between the F8 protein and von Willebrand factor carrier protein due to the loss of an interaction domain.
Genetic Analysis and Reproductive Interventions for Two Rare Families Affected by Severe Haemophilia A

Abstract:Background Haemophilia A (HA) is a rare bleeding disorder caused by variants in F8. Although traditional mutational analyses have identified numerous pathogenic variants, the aetiology of HA in certain patients remains unclear. Furthermore, female patients with severe HA are rare. Aim To investigate the molecular defects underlying severe HA in two patients and provide personalised reproductive interventions for their families. Methods Two patients diagnosed with severe HA without other clinical phenotypes were enrolled in the study. A combination of whole‐exome sequencing, real‐time quantitative polymerase chain reaction and long‐read sequencing (LR‐sequencing) was performed to reveal the molecular defects of them, followed by the application of different reproductive intervention strategies. Results Proband 1, a 29‐year‐old man with FVIII activity of 0.8%, did not exhibit common F8 variants, including Inv1 or Inv22, in the coding region. However, he carried a rare maternal novel inversion on ChrX:154148973_154170321, spanning approximately 21.345 Kbp, with breakpoints in introns 13 and 14 of F8. Finally, the couple of Proband 1 opted for assisted reproductive technology using preimplantation genetic testing and successfully conceived. Proband 2, a 20‐year‐old female with severe HA and FVIII activity of 0.6%, carried inv22 of F8. Further investigation combining whole exome sequencing (WES) and pedigree analysis revealed that she carried a maternal cross‐deletion encompassing exons 1–22 of F8, FUNDC2, BRCC3 and CLIC2, along with a de novo missense variant c.5852T>C (p.Leu1951Ser) on her paternal X‐chromosome. Chromosome X‐inactivation (XCI) analysis demonstrated a highly skewed inactivation of the maternal X chromosome, with a ratio of 98:2. Subsequently, prenatal diagnosis confirmed that the third child in this family did not carry any of the F8 variants present in Proband 2. Conclusion Our findings provide novel insights into the genetic aetiology of HA and emphasise the importance of a definitive diagnosis in guiding genetic counselling and personalised reproductive interventions for affected individuals and their families.

[Intron 1 and 22 Inversions in Factor VIII Gene in Patients with Haemophilia A].

Abstract:OBJECTIVE:To analyze intron 1 and 22 inversions in factor VIII (FVIII) gene in hemophilia A (HA) patients and and their families and to investigate the correlation between intron inversion and FVIII antibody.METHODS:All patients were detected FVIII: C and FVIII antibody. In addition, 81 unrelated HA patients were directly detected by multiplex PCR and long-distance PCR for intron 1 and 22 inversions in FVIII gene. Pedigree investigation for some patients were conducted.RESULTS:In 81 unrelated HA patients, 3 severe cases were found intron 1 inversion which accounted for 4.6% of total 65 severe cases. Of the 3 cases, one was FVIII antibody positive. Two female family members of a intron 1 inversion patient were identified as one carrier and one non-carrier. Twenty five of 65 (38.5%) severe cases were found intron 22 inversion. Of the 25 cases 1 was FVIII antibody positive. Nine female members in 5 HA families which had patients with intron 22 inversion were identified as 7 carries and 2 non-carriers.CONCLUSION:Besides intron 22 inversion, intron 1 inversion was another important molecular defect in resulting in severe HA. Intron inversion analysis can also be used for deviation rectification of experiment grouping in HA patients. Intron 1 and 22 inversions may be one of the higher risk factors for resulting in FVIII antibodies.
Gene diagnosis of co-existed hemophilia A and B in a pedigree

Abstract:Objective To make gene diagnosis of a pedigree with co-existence of hemophilia A and B and to detect the carrier state of female members of this pedigree. Methods The APPT, PT, TT, Fg,F Ⅷ:C, FⅨ:C and vWF were tested to make phenotypic diagnosis. LD-PCR was used for detecting F 8 gene intron 22 inversion, two sets sequence specific PCR was used for detecting F 8 gene intron 1 inversion. Nucleic acid direct sequencing was used for detecting F 9 gene exons and their flank sequences. Pedigree hereditary linkage analysis was performed by analyzing the six F 9 gene related microsatellite loci (DXS8094, DXS1211, DXS1192, DXS102, DXS1227 and DXS8013). Results Family studies showed that proband 1 was a severe HA caused by F 8 gene intron 22 inversion. Proband 2 was a hemophilia B caused by F 9 gene mutation nt32772A→T (p.Ser365Cys, GI:22385320), the mutated gene come from his mother and originated form his maternal grandfather’s X chromosome. His maternal grandfather was a chimera of mutated somatocyte and gonadocyte; his aunt was a normal female. Conclusions The gene diagnosis of the two probands of the pedigree agreed with their phenotypic diagnosis. Single base pair extension method can be used for the detection of mutation chimera rate, and can provide reliable information for analyzing the origin of mutation in sporadic hemophilia pedigree.

Abstract:Haemophilia A (HA) is a common X-linked recessive bleeding disorder and almost one half of patients with severe HA are caused by intron 22 inversion (Inv22) in the F8. Inv22 is considered to be almost exclusively of meiotic origin in germ cells during spermatogenesis and only one mosaic Inv22 female carrier with the mutation possibly occurring during mitosis of the embryo has been reported so far. Previously we have identified a novel complex recombination mediated by int22h copies in a sporadic severe HA pedigree and herein we have localised the sequences flanking the breakpoint region using genome walking technique, AccuCopy technique, gene chip and real-time PCR. The disease causing genetic variant registered an 18.1 kb deletion including part of int22h-1 through the intron 23 of F8 and a 113.3 kb duplication of part of int22h-2 through the intron 1 of TMLHE inserted in the religated region of the F8. Two intrinsically linked mechanisms of recombination-dependent DNA replication: microhomology-mediated break-induced replication (MMBIR) followed by break-induced replication (BIR) might be responsible for the incident of the complex recombination during early embryogenesis of the proband's mother.

Abstract:Objective To analyze intron 1 and 22 inversions in factor Ⅷ(FⅧ)gene in hemophilia A(HA)patients and and their famihes and to investigate the correlation between intron inversion and FⅧ antibedy.Methods AIl patients were detected FⅧ:C and FⅧ antibody.In addition,81 unrelated HA pafients were directly detected by multiplex PCR and long.distance PCR for intron 1 and 22 inversions in FⅧ gene.Pedigree investingation for gome patients were conducted.Results In 81 unrelated HA patients.3 severe cases were found intron 1 inversion Whieh accounted for 4.6% of total 65 severe cases.Of the 3 cases,one was FⅧ antibody positive.Two female family membets of a intron 1 inversion patient were identified as one carrier and one non-carrier.Twenty five of 65(38.5%)severe cases were found intron 22 inversion.of the 25 cases 1 was FⅧantibody positive.Nine female members in 5 HA families which had patients with intron 22 inversion were identified as 7 carries and 2 non-carriers.Conclusion Besides intron 22 inversion,intron 1 inversion was another important molecular defect in resulting in severe HA.Intron invemion analysis can also be used for deviation rectifieation of experiment grouping in HA patients.Intron 1 and 22 inversions may be one of the higher risk factors for resulting in FⅧ antibodies.

Abstract:Hemophilia A (HA) in females is rare. Female HA cases are often misdiagnosed as acquired HA (AHA) or as von Willebrand disease type 2N (vWD-2N). Here, we report the case of a 37-year-old female HA patient with a moderate factor VIII (FVIII) deficiency. The patient had no personal or family history of bleeding disorders, but presented with heavy uterine bleeding following surgery to remove an intrauterine device. IgG inhibitory antibodies against FVIII were undetected. A compound heterozygote mutation of the FVIII gene (F8) was found in this patient. The p.Val502Asp mutation, which has been reported previously, affects A2 domain function. A novel missense point mutation, p.Met1093Ile, was identified in the B domain. The compound heterozygote mutations in F8, p.Val502Asp and p.Met1093Ile, caused HA in this female patient, with a moderate phenotype.

Abstract:Hemophilia A is an x-linked recessive inherited bleeding disorder. So far, more than 1,885 disease-causing mutations of factor VIII gene have been identified. Clinic confers a great challenge for the molecular diagnosis. We aim to make a better strategy for the molecular diagnosis in Hemophilia A. First, factor VIII intron 22 inversion and intron 1 inversion mutations were detected using Inversion-PCR and double-tube multiple PCRs. And then, non-inversion mutations were analyzed by denaturing high performance liquid chromatography and/or direct sequencing. Novel mutations were further analyzed the conservation and 3D structures by a B domain deleted crystallographic model and bioinformatics. Finally, we can indirectly confirm the diagnosis by linkage analysis for the patients with the confusing diagnosis by the techniques mentioned above. Eleven patients with the factor VIII Inv 22 were found, and the remaining 16 patients were found with 11 different mutations, of which 3 was novel mutations affecting A1, B domains and splicing site. Moreover, the prenatal diagnosis was performed on 14 fetuses. Ten fetuses were successfully confirmed to be normal, 1 fetus to be a heterozygote with factor VIII c.3275–3276 ins A and 3 fetuses to be hemizygotes with factor VIII Inv 22 mutation.

Abstract:Hemophilia, an X-linked recessive disorder, is characterized by spontaneous or trauma-induced prolonged bleeding. It is classified as hemophilia A when caused by variants in the F8 gene, and hemophilia B when caused by F9 variants. Few studies have described hemophilia variants in the Chinese population. This study aimed to investigate the clinical and genetic profiles of 193 hemophilia patients from southern China. Utilizing Sanger sequencing, multiplex ligation-dependent probe amplification, gap detection, long-range PCR, and multiplex PCR, we identified both F8 and F9 gene variants. Pregnant women with a history of hemophilia A offspring underwent amniocentesis or villus sampling for the variant detection. Variants in F8 and F9 were pinpointed in 183 patients, with 26 being novel discoveries. Notably, genetic testing was absent in the initial evaluation of 133 out of 161 patients, leading to a protracted average definitive diagnosis timeline of 2 years. Remarkably, two hemophilia A cases with anticipated severe phenotypes due to protein-truncating variants presented with only moderate or mild clinical manifestations. Among the 40 fetuses tested, 34 were males, with 17 exhibiting hemizygous variants in the F8 gene. Our results contribute to the broader understanding of F8 and F9 variant spectrum and highlight the underuse of genetic analyses in southern China.

Abstract:Intron 1 inversion (Inv1) is a recurrent causative mutation of haemophilia A (HA) and is responsible for 1-5% of severe HA. Inv1 occurs as a result of intra-chromosomal homologous recombination between int1 h-1 within intron 1 and int1 h-2 located in approximately 125 kb telomeric to the F8 gene. In this report, we presented a previously undescribed aberrant type of Inv1 with complex genomic rearrangement in a pedigree with severe HA. The breakpoints of the rearrangement were identified by the genome walking technique; copy number variations (CNVs) of the F8 gene and X chromosome were detected by Ac-cuCopy technique, Affymetrix CytoScan HD CNV assay and quantitative PCR (qPCR); the F8 transcripts related to the aberrant Inv1 were analysed by reverse transcription PCR (RT-PCR). We have characterised the exact breakpoints of the complex rearrangement, and determined the location and size of the insertion and deletion. The rearrangements can be summarised as an aberrant pattern of Inv1 with a deletion of 2.56 kb and a duplication of 227.3 kb inserted in the rejoining junction within the F8 gene. Our results suggested that this complex genomic rearrangement was generated by two distinct repair mechanisms of fork stalling and template switching/microhomologymediated break-induced replication (FoSTeS/MMBIR) and nonallelic homologous recombination (NAHR).
