Abstract:Objective To study a quick and effective assessing method used in the emergency trauma patients in order to prevent delaying treatment for patients from overlook or misdiagnosis during triage. Method From 1 January 2006 to 31 September 2006, there were 4023 patients evaluated with trauma assessment method, which was composed of CRAMS scale and trauma assessment program. Results Of 4023 patients, there were 2430 cases with CRAMS below 1( < 7) ,of whom 1979 cases assessed with trauma assessment program were found to be injuried severely to threaten their lives. And more noticeably,of 1593 patients with CRAMS ≥7 there were 46 patients found to have life-threatening injuries assessed with trauma assessment method. Conclutions CRAMS score can be used for triage, and trauma assessment program can be used to evaluate the injuries anatomically. Trauma assessment program and CRAMS employed conjointly can increase the sensitivity and specificity for the stratification of the severity in the injuried patients in order to get a successful triage and lo take advantage of the resources of e-mergency care appropriately.

Abstract:Emergency care for mass casualty incidents is a sophisticated multi-participant process. To manage this process effectively, many information systems have been proposed. However, their performance in improving the efficiency and accuracy of patient triage is not satisfactory. This paper is concerned with the development of a mobile-based system for supporting emergency triage in the emergency care process for mass casualty incidents. This system collects the patient’s emergency data throughout the whole emergency care process through a mobile application and data transfer mechanism. Using a Cox proportional hazard model, the system has the capacity to present the survival curve to the triage officer, helping him/her to make triage and transportation decisions. This system offers an alternative injury assessment tool based on the vital signs data of the injury patient. With the help of this system, the triage officer can more directly and comprehensively learn about each patient’s situation and deterioration without additional operations at the incident site.
Reliability and Validity of the Four-Level Chinese Emergency Triage Scale in Mainland China: A Multicenter Assessment

Abstract:Background: An effective emergency triage system is lacking in mainland China. The Chinese Emergency Triage Scale (CETS) was developed based on vital complaints and vital parameters, according to Chinese data. Objective: The aim of this study was to assess the reliability and validity of the CETS by emergency department (ED) nurses in eight EDs in mainland China. Design: A cross-sectional multi-center study was conducted. Settings: Eight EDs in the eastern, western, northern, and central areas of mainland China. Participants: A total of 51 ED nurses and 8000 active ED patients participated in the study between May and September 2018 in eight EDs. Methods: Standardized triage scenarios and active patients were assigned to nurses who used the CETS for evaluative purposes. Accuracy was defined by concordance with the key for the scenarios and was calculated as percentages. Inter-rater reliability was measured by weighted kappa to compare the triage nurse's rating with the research nurse's acuity level for each patient. The triage time and clinical outcome of 1000 active patients per site also were collected. The criterion-related validity of the CETS was evaluated; criteria included ED mortality, number of patients discharged, and admission to ICU or general ward. The relationship between the CETS acuity levels triaged by the research nurse and four possible outcomes was assessed with a cross-classification table, using a chi-square test. The ability of the CETS to predict ED mortality was assessed via the receiver-operating characteristic (ROC) area under the curve (AUC). Results: Mean pooled accuracy from all sites was 89.4% (95% CI = 86.9%-91.8%), and the proportion of over-triage slightly exceeded under-triage (6.5% vs. 4.1%). There was no difference in accuracy between sites or according to nurse experience. Inter-rater reliability values for triage nurses and research nurses were 0.96 (95% CI = 0.95-0.97), based on the quadratic weight kappa. Trauma cases were assigned to triage with greater accuracy than were nontrauma cases, both for the scenarios and for active patients. The average triage time of ED nurses in all sites was 151.5 +/- 26.3 s, using the computer-based triage instrument. The chi-square test showed that there was a significant difference in triage time between the CETS levels (p < 0.001). The AUC was 0.968 (95% CI = 0.958-0.979). Conclusions: The CETS is a reliable system for ED triage and can promote rapid and effective triage in mainland China. (C) 2019 The Author(s). Published by Elsevier Ltd. Highlight the Importance of Psychological Service in Disaster Emergency Management

Abstract:Disasters are traumatic events that are always incongruent with our existing cognitive structures. Immediate negative psychological impact and traumatic related psychiatric disorders are popular among survivals involving the disasters. Super-half of survivals involving the "5" 12" Wenchuan earthquake experienced the symptoms of anxiety or increased arousal. 12.59% of survivals were met with the ASD diagnosis. 21.29% of adults and 10.21% of adolescents were met with the PTSD diagnosis 5 months after the major earthquake. Immediately negative psychological impact and high prevalence of traumatic related psychiatric disorders among survivals were confirmed.More effort should be paid on the disaster related patients in emergency department with psychiatric issues. Also, the long-term psychological service is necessary for Sichuan Pest-Disaster Reconstruction Projects.
[A Survey of Mental Health among Transferred Injured Survivors in Wenchuan Earthquake in Sichuan].

Abstract:OBJECTIVE:To assess the depression and anxious status among transferred injured survivors in Wenchuan earthquake in Sichuan province.METHODS:A total of 43 transferred injured survivors were investigated by questionnaire exploring their trauma symptoms and mental health status.RESULTS:High rates of trauma symptoms were remarkably observed in these survivors. Of all the respondents, 60% had some emotional symptoms and sleeping difficulties. About one third of respondents experienced recurrent and intrusive distressing recollection of event, 16 (37.21%) experienced nightmare, 15 (34.88%) had flashback and 7 (16.28%) of them tried to avoid relative stress.CONCLUSION:Many mental symptoms were observed in transferred injured survivors. The two major factors of mental stress were emotional symptoms and re-experience of the disaster.
The establishment and practice of two-level triage system in outpatient visit management

Abstract:Objective: To explore the applicability and feasibility of two-level triage system in our tertiary hospital outpatient visit management. Methods: The authors established two-level triage system based on the JCI outpatient safety management goal and homogeneity service, they defined the job description of clinic nurses, established outpatient safety management system, improved clinic environment, using outpatient triage software to support the two-level triage management. Results: The average waiting time decreased from 56.33 min to 40.95 min (P<0.001), the overall satisfaction for outpatient increased from 91.38% to 94.63% (P<0.001) after three-year practice. No one suffered sudden accident death in clinic. The rate of one clinic for one patient was 96.8%. Conclusion: The two-level triage system under JCI standard was safe and efifcient for improving clinic service and outpatient satisfaction. It is an important means to increase outpatient service quality.

Abstract:OBJECTIVE:To explore the mental health status of residents scattered living in Anxian after Wenchuan earthquake so as to provide scientific basis for further mental health intervention.METHODS:A face to face interview was conducted among the scattered residents with designed questionnaire, which had three parts of the physical and emotional reaction, the relax methods and the social care and supports expected. Two-stage probability proportional to size (PPS) sample method was performed to sample 2184 from 0.44 million scattered residents in Anxian. On the basis of statistical description, mental health of different characteristics groups was compared.RESULTS:Three main symptoms of posttraumatic stress disorders in 2184 residents (11.23+/-3.44) were higher than the 103 fire victims in Hunan in 2003 (10.06+/-3.26), three factor scores of SCL-90 (5.76+/-1.74) were higher than normal in 1998 repair mode (n=23 891) (4.72+/-1.44), and the statistical difference was observed (t=10.77, P<0.05; t=706.04, P<0.05). Comparing the mental health of different groups, some significant differences were found by age, gender and education background.CONCLUSION:The earthquake disaster brought prevalent physical and emotional reaction. Elderly people, female, junior students need mental intervention immediately. Therefore, strengthen the mental education and assistance (especially in high risk groups) would be of more significance.

Abstract:Objective To provide scientific basis for developing mental interventions for the wounded in major disasters by assessing the mental health status and the mental intervention effect among the Wenchuan earthquake survivors transferred to Chongqing.Methods A total of 389 transferred wounded survivors were examined by using questionnaires including general information questionnaire,Psychological Health Self-rating Questionnaire (PHSQ),Symptom Checklist 90 (SCL-90),Perceived Social Support Scale (PSSS) and Coping Styles Questionnaire (CSQ).According to PHSQ,the wounded were divided into control group and stress group.Then,the stress group was given mental intervention and taken the second mental evaluation one month later.Results Acute stress disorder (ASD) rate (30.08％) was remarkably observed in these transferred wounded,who were subsequently administered a set of mental intervention.The wounded whose PHSQ mark ≥8 were defined as the stress group (Group A) and the others as the control group ( Group B).Compared with Group B,Group A had higher scores in depression ( P ＜ 0.01 ),anxiety ( P ＜ 0.01 ) and consternation ( P ＜ 0.01 ),and lower scores in PSSS (P ＜0.01 ).Group A inclined to negative coping styles mainly including shrink back (P ＜ 0.01 ) and fantasy (P ＜0.01 ),while Group B inclined to coping styles mainly including rationalization and help.The PSSS score in Group A was significant lower than that in Group B (P ＜ 0.01 ).The scores in PHSQ,SCL-90 (anxiety and consternation),CSQ and PSSS were significantly improved after the mental intervention in Group A.Conclusions The incidence of ASD is high in the transferred wounded survivors and is mainly related with mental stress,such as emotional symptoms,negative coping styles and low perception of social support.Early mental interventions will help improve the psychological status of the transferred wounded.

Abstract:Objective To detect the reliability and validity of the serf-reporting questionnaire (SRQ-20),and to make a rapid assessment on mental health among victims in sudden crisis.Methods The SRQ-20 which is on appendix of the Ministry of Health adopted as "emergency psychological crisis intervention guiding principles" was administrated among 1222 victims in Wenchuan earthquake.The reliability of the questionnaire was tested by split-half coefficient and Crenbach's α coefficient; the validity was tested by means of principal component factor analysis.Results The self-reporting questionnaire and split-half coefficient was 0.748 ,each item of Cronbach's α in the reliability coefficient was between in 0.778 to 0.789 ,overall Cronbach's α in the reliability coefficient was 0.792.There were five main components in principal component factor analysis: lacking of power,somatization/biological symptoms,depression thought, emotions and awareness.Conclusion The self-reporting questionnaire (SRQ-20) should have good reliability,and validity,and China's psychological crisis intervention in processing assessment of the mental health should be a fast,accurate and effective screening tooL

Abstract:We established the rapid response system for non-hospitalized patients from 2012 in order to improve the effectiveness of emergent critical care for non-hospitalized patients when emergency happened.From January 2013 to December 2016,there were 122 cases with RRS activation for non-hospitalized patients.The time to arrive was 3.16±0.41 min,and 107 cases(86.89％)were sent to the emergency department.Fifteen patients(14.02％)were classified as level 1,26(24.03％)as level Ⅱ,48(16.82％)as level Ⅲ,and 18(16.82％) as level ⅣV,and 83％ were critical patients.Rapid response system is important to cope with emergency in non-hospitalized patients.

Abstract:Objective: To explore the psychological health situation and related factors of basic medical personnel in 6 severe disaster-stricken areas at 14 months after Wenchun Earthquake.Methods: A two-step evaluation was undertaken in the enrolled basic medical personnel.Firstly,all subjects were screened with the Self-Report Questionnaire(SRQ20) and 7-symptom Screening Scale for Post Traumatic Stress Disorder(PTSD7).Secondly,the subjects with SRQ20 scores ≥10 or PTSD7 scores ≥4 were assessed with the Mini-International Neuropsychiatric Interview(MINI) for making diagnoses of depression,suicide and PTSD.Results: Totally 307 subjects finished the evaluation.Eighty-one people(26.4%) had scores of SRQ20≥10 or PTSD7≥4.Seven people(2.3%) met the diagnosis of current depression,7 people(2.3%) met the diagnosis of PTSD,and 3 people met both.Ten people(3.3%) had suicide attempt.In those with SRQ20 scores ≥10 or PTSD7 scores ≥4,there were no gender and age differences in rates of current depression,PTSD and suicide attempt.The rates of diagnosed PTSD were different in different locations with the highest rate of 40% in Beichuan.Logistic regression analysis showed that frequent residence change was a risk factor of mental distress(OR=1.24).Conclusion: The basic medical personnel in 6 severe disaster-stricken areas still show obvious mental distress at 14 months after the earthquake,some even have suicide attempt and symptoms of depression and PTSD.Frequent residence change might accelerate the occurrence of the mental distress and aggravate the symptoms.
Application and effects of team-triage system in the rescue of mass blast casualties

Abstract:Objective To evaluate the effects of team-triage system in the rescue of mass casualties in grade-Ⅲ hospitals.Methods A triage team was set up based on the mode of team-triage system.Totally 124 blast injured patients were triaged and rescued according to the simple triage and rapid treatment procedure.Results Of the 124 wounded patients,35 patients were triaged to red area,30 patients to yellow area,and 59 patients to green area.The triage qualification rate was 100%.Thirty-four patients suffered from severe multiple injuries or combined injuries.One hundred and eighteen patients received debridement operation and ten patients received damage control operation.One patient died and the rescue success rate was 97.1%.Conclusions The mode of team-triage system can ensure the rapid and orderly triage and rescue procedure,raise the efficiency of triage management and increase patient satisfaction in the rescue of group wounded patients.
Injury triage and process during transportation of the wounded by Beijing rescue team following China Wenchuan earthquake

Abstract:Objective To discuss the injury triage and process during large scale transportation of the wounded during earthquake by understanding injury severity and detecting vital signs of the woun-ded needing long distance transportation in Wenchuan earthquake. Methods Second medical rescue team of Beijing Health Administration finished transportation of 197 patients including 106 males and 91 females at age range of 6-81 years (average 37.5 years). During whole transportation process, we carried out 2 examinations, 4 checkouts and 1 scoring twice per patient and strengthened close collaboration a-mong different sub-transportation teams. Results Of 197 patients, 194 were smoothly transported ex-cept for three who returned to original hospital due to unstable situation on the way of transportation. Of successfully transported patients, 13 stated complaints including pain of affected extremity in seven pa-tients with fractures, chest distress and short breath in two with crush syndrome, aggravated chest pain in two with crush injury of the chest as well as dizziness and sicchasia in two during the course of transporta-tion. In the meantime, monitoring of vital signs and symptomatic treatment were done in all 13 patients who stated complaints. Conclusions Sufficient preparation, strict medical examination, sound medi-cal collaboration, understanding of injury condition, necessary monitoring and treatment, active communi-cation with patients and detailed relay are preconditions to guarantee successful transportation during large scale transportation of the wounded caused by earthquake.

Abstract:[Abstract ] Objective To combine the modified early warning score (MEWS)and precheck standard in chest specialist,and to construct the revised MEWS system for the triage of the chest specialist emergency, and also discuss the application effect.Methods By convenience sampling,8994 cases of emergency patients from September to December 2015 were selected as control group,9138 cases of emergency patients from January to March2016 were selected as observation group.Control group patients was applied with proce-dure,while the observation group was applied with revised MEWS system.The triage time,accuracy and ef-fectiveness in the identification of high risk chest pain patients,emergency treatment rate,doctors,nurses and patients’satisfaction rate were compared.Results The triage time,accuracy and effectiveness in the i-dentification of high risk chest pain patients,emergency treatment rate,patients’satisfaction rate in obser-vation group were better than that of control group (all P<0.05).Conclusions Revised MEWS system helpal to quickly and accurately triage,and effectively identify high-risk patients with chest pain,raise the chest specialist emergency triage work operation and accurary,and for emergency patients to received the standardized,scientific,appropriate,proper and timely treatment in the short time period.

Abstract:Objective To explore the application effect of triage-registration information systems in emergency patients. Methods A total of 18 964 emergency patients from 1 December 2016 to 31 December 2016 in Zhongshan Hospital of Fudan University Emergency Department were selected as control group and 18798 emergency patients as experimental group from March 1, 2017 to March 31, 2017. In the control group, the patients were used traditional mode of triage-registration and registration office respectively line. In the experimental group, the patients were used triage-registration information systems in line once. The emergency waiting time, patient satisfaction and medical staff satisfaction were compared between the two groups before and after the application of triage-registration information systems. Results The waiting time in the experimental group was significantly shorter than that in the control group, median(P25, P75):11(6-28)min vs. 10(4, 27)min, The difference between both groups was statistically significant(Z=-14.5433, P=0.000). The patient satisfaction in the experimental group were better than those in the control group, (2.89 ± 0.41 vs. 2.98 ± 0.13), and with statistical difference (Z=-10.317, P＜0.05). And the acceptance of the medical staff in the experimental group were better than those in the control group, (2.90±0.35 VS 2.97±0.16), and with statistical difference (Z=-5.527, P<0.05). Conclusions The development and application of the triage- registration information system can effectively shorten the waiting time of patients, improve the experience of emergency patients, improve the satisfaction of medical staff, and realize the optimization of the process of seeking medical treatment.
Needs assessment for the psychosocial support at the severely-affected counties after Wenchuan earthquake

Abstract:Objective:To understand the psychosocial support needs by the survivals at the severely-affected counties after Wenchuan earthquake,and to provide valuable information for the mental health and psychosocial support services.Methods:Nine months after the earthquake,a field survey was conducted to 138 survivals in Beichuan and Anxian by using of self-formulated questionnaire and in-depth interview.Results:(1)74.6% of the survivals were willing to receive psychological support,51.4% had heard about ' psychology','psychological assistance' or 'psychological intervention'.Merely 34.1% had received psychological support or assistance.(2)Survivals who had ever received psychological assistance were more likely to think it helped them much(23.4% vs.8.4%,P=0.002).There was a low correlation(r=-0.28,P=0.001)between the number of times survivals received psychological assistance and survivals' impression upon psychosocial support.(3)27.7% of the survivals thought that communication was a good way to do help,and psychological supporting(23.4%)and understanding of their needs and concerned issues(17.0%)were also useful.(4)Survivals agreed that housing problems,losing families,difficulties with economic resources and problem of employment served as major issues that might affect their psychological status since the earthquake happened.Conclusion:Better survivals' impression on psychosocial support service is associated with more times of having received assistance.A large part of psychosocial support needs by the victims are not satisfactorily met.It is needed to improve the service capacity of basic level health workers and increase the accessibility of service.It is implicated that a variety of psychosocial supports from health sector and non-health sectors should work together and promote each other,and the services should be adapted according to the different features and needs of survivals.

In this project we aimed to identify public health issues arising from the physical and mental health symptoms suffered by the soldier volunteers deployed in an emergency relief task during the Wenchuan earthquake. Methods: Health problems identified in other emergency volunteer populations guided the development of a questionnaire. A cohort of 1,187 soldier volunteers completed the questionnaire, which measured physical and mental health symptoms during their rescue mission. The results were compared with a population norm of soldiers, although baseline data of the respondents were unavailable. Results: Half the respondents reported suffering from skin and mucous membrane problems, followed by respiratory symptoms (38%), digestive (29%) and nervous (22%) symptoms. Despite a low response rate (53%) to the mental health component, nearly half (49%) of those who did respond reported mental health problems. The incidence of the above symptoms were significantly higher than the general soldier population. Conclusions: Health complaints were common in the soldiers, who had not received any formal training in rescue operations. Implications: Non-professional rescue workers who are not appropriately prepared for the role may suffer more than their professional counterparts. Attention needs to be paid to the health and safety of non-professional rescue workers, which has been ignored in most disaster management plans. These findings can be used to enhance the understanding of emergency response programs within and outside China, where this particular disaster occurred.

Abstract:Objective:To explore the mental distress and the related factors of people who suffered 5.12 Earthquake in Sichuan at 9 months after the earthquake.And try to find the methods to quickly detect the individuals with depression,posttraumatic stress disorder(PTSD)and suicide risk.Methods:A total of 321 subjects in Yongxing temporal board houses of Mianyang and in the rural areas of Xiushui were enrolled and assessed by a three-step evaluation.First,all subjects were screened with the Self-Report questionnaire(SRQ20)and 7-items Screening Scale for PTSD(PTSD7).Second,the subjects with SRQ20 score ≥10 or PTSD7 score ≥4 were assessed with the Self-rating Depression Scale(SDS),Self-rating Anxiety Scale(SAS)and the suicide module of Mini-International Neuropsychiatric Interview(MINI).And the subjects with PTSD7 score ≥4 were also assessed the MINI PTSD module.Finally,the subjects with SDS score ≥50 were evaluated using MINI depression and PTSD modules.Results:(1)Totally 315 subjects finished the evaluations,in which 53.0% had scores of SRQ20≥10 or PTSD7≥4,37.1% had scores of SDS≥50,and 18.4% had a suicide risk.25.7% and 27.3% of them met the diagnoses of MINI current depression and PTSD respectively,and 18.1% were diagnosed as both PTSD and depression.(2)The PTSD7 score and MINI PTSD rate(3.6±1.8,33.0%)of the subjects in Yongxing board houses were higher than those in Xiushui(2.9±1.7,16.0%),Ps<0.001.(3)Logistic regression analysis showed that the physical injury was a risk factor for current depression(OR=2.579)and PTSD(OR=1.917)and frequent residence change after the earthquake was a risk factor for PTSD(OR=1.189).Conclusions:(1)People in the earthquake-hit area still have obvious depression,PTSD and suicide risk at 9 months after the earthquake.The physical injury happened during the earthquake has a long-term influence for people to get depression and PTSD,and frequent residence change after the earthquake maybe promote PTSD episode and more serious.(2)The combination of SRQ20,PTSD7 and MINI should be used to assess mental distress quickly after disasters.

Abstract:Background: This study examined the trajectories of posttraumatic stress disorder (PTSD) among adolescents in the area worst-hit by the Wenchuan earthquake, and assessed the role of trauma exposure in differentiating distinct PTSD trajectories. Methods: In total, 391 adolescents were selected from the Wenchuan and Maoxian counties in Sichuan province (the area worst-hit by the earthquake). Participants completed a trauma exposure questionnaire at 1 year, and the Child PTSD Symptom Scale at 1 year, 1.5 years, 2 years, and 2.5 years after the earthquake. Results: Three latent PTSD trajectories were found in adolescents: moderate-stable (81.6%), decreasing (8.7%), and increasing trajectories (9.7%). T1 trauma exposure was more likely to be related to the decreasing trajectory group. Limitations: All variables were measured by self-report scales. There was high non-random dropout. Conclusion: The course of PTSD was heterogeneous among adolescents following the same natural disaster.
