Abstract:Kimura disease is a chronic inflammatory disease, which frequently affects middle-aged Asian men, although children are seldom affected by it. Therefore, the characteristics of the Kimura disease of childhood have not been well illustrated. In this report, we have described a case of Kimura disease and have summarized 29 childhood cases reported in Chinese people from 1988 to 2009 by using 3 Chinese journal search engines. Most of these cases were from provinces near the eastern and southern coast of China. Boys were predominantly affected. Head and neck are the most frequently involved sites of subcutaneous masses (24 of the 29, 82.8%). On laboratory investigations, peripheral blood eosinophilia (25 of 26, 96.2%) and elevated immunoglobulin E level (13 of 13, 100%) were common. Kimura disease was associated with high incidence of nephritic syndrome (8 of the 29, 27.6%) and eczema rash (13 of 29, 44.8%). Surgery excision, steroids, and chemotherapy were the main therapy methods in this cohort. For patients with follow-up data, 12 experienced recurrence, with a recurrence rate of 60.0%. Pediatricians need to be aware of this disease when dealing with patients with lymphadenopathy. A standard and effective treatment protocol would improve the outcome of the Kimura disease.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Objective: We analyzed the clinical characteristics and treatment results in surgical patients with a final diagnosis of Kimura disease. Patients and Methods: A total of 8 patients with a final diagnosis of Kimura disease of the head and neck region after surgery from January 2007 to December 2022 were enrolled. Results: The most common symptom was a mass in the head and neck region (n = 7), followed by incidental detection (n = 1). Multiple head and neck lesions were observed except for 1 patient with Kimura disease in the buccal space. Kimura disease could not be diagnosed in 5 patients who underwent preoperative fine-needle aspiration cytology. All patients underwent surgical resection. No major complications after surgery occurred. Six patients received steroids after surgery, with 1 patient undergoing radiotherapy as a side effect of steroid treatment. During the follow-up period, 5 patients relapsed. Of the parameters assessed, only longer symptom duration was statistically significant with relapse of Kimura disease. Conclusion: Clinicians should suspect Kimura disease based on asymptomatic masses, peripheral blood eosinophilia, and imaging examinations and make a definitive diagnosis via surgery.

Objective: This study aimed to provide a comprehensive analysis of the clinical and radiological features of KD in a retrospective case series, to assess treatment outcomes, and to discuss the implications for diagnosis and management. Methods: We retrospectively analyzed four histologically confirmed cases of KD admitted to Zhejiang Provincial People's Hospital from January 2018 to October 2023. Clinical and radiological data were retrospectively analyzed, and imaging findings were analyzed by two neuroradiologists to determine lesion characteristics and contrast enhancement patterns. Results: Our findings showed that the patients were predominantly male, with a mean age of 43 years and an age range of 13– 71 years. All patients presented with painless subcutaneous masses and three of them had peripheral blood eosinophilia and elevated serum IgE levels. Radiographically, the lesions were predominantly ill-defined with heterogeneous enhancement, accompanied by subcutaneous fat atrophy. Complete surgical excision and oral corticosteroids were effective treatments, and no recurrence was noted during follow-up. Conclusion: KD should be considered in the differential diagnosis of painless subcutaneous masses in the head and neck region, especially in the presence of eosinophilia and elevated IgE levels. Our findings contribute to the understanding of KD's clinical and radiological spectrum and highlight the need for long-term follow-up due to the risk of recurrence. It is characterized by angiolymphoid proliferation accompanied by elevated peripheral eosinophilia and increased serum immunoglobulin E (IgE) levels. 1 Kimura et al provided a systematic characterization of the disease as "unusual granulations associated with hyperplastic alterations in lymphoid tissue" in 1948 led to the current name, Kimura disease. 2 KD is a rare disease, with less than 300 cases reported worldwide according to histopathological reports. 3 The disease typically affects young Asian males aged 20 to 40 years. It is most common in China and Japan but rarely seen in European and American countries. 4–6 The male-to-female ratio is generally 4:1, and the incidence in children is relatively lower. 7,8 KD is prevalent in the head and neck region, particularly in the posterior auricular and parotid regions, and its incidence is as high as 73.9%. 9 The predominant clinical feature of KD is a painless subcutaneous mass with or without regional lymphadenopathy. A Rare Case of Kimura's Disease with Superior Vena Cava Syndrome and Biatrial Masses

Abstract:Abstract Abstract 5068 A 33-year-old man of Asian descent was admitted to our hospital 3 years ago with a main complaint of palpitations for 7 months. There was no fever, no cough, no gain or loss of weight, no night sweats, no medical history of allergic diseases or contact with parasites. Physical examination showed marked edema in the neck and upper chest, and the hepatojugular reflux was positive. There was no change in skin appearance or hepatosplenomegaly. A soft rubbery, non-tender lymph node (1×2cm) was palpated in the left axillary area. The laboratory data showed mild eosinophilia (leukocyte count 6000 /ul with eosinophils 582/ul). The serum IgE concentration was increased (1,250 U/ml). Serological tests for HBsAg, anti-HCV, anti-HIV, and antibodies for distoma pulmonala, distoma japonicum, schistosome, bladder worm, sparganum, trichina cystica and filarial were negative, however toxoplasma gondii antibody IgG was positive, while the IgM was negative. Antinuclear antibodies were all negative. Echocardiogram revealed a mass (7.1×7.0×5.5cm) displacing the base of the heart and involving both atria. CT showed enlarged biaxillary lymph nodes and a large mediastinal mass extending into both atria and obstructing the superior vena cava with right ventricular hypertrophy (Fig.1a, b). Positron emission tomography (18F-FDG-PET) scan indicated negative fluorodeoxyglucose (FDG) uptake in the lymph nodes, spleen and liver. Biopsy of an enlarged right axillary lymph node was performed and the pathology revealed a normal preserved nodal architecture with proliferation of lymphoid follicles infiltrated by abundant eosinophils partially forming eosinophilic microabscesses. CD20 staining was positive. The bone marrow examination indicated normal cellularity except a little increase of eosinophils at 7.5%. Biopsy of the cardiac mass was not performed due to technical challenges related to its anatomical location. Based on the clinical and laboratory findings, a diagnosis of Kimura disease was established. Treatment was started with a low dose of oral prednisone 20–30mg/d. After treatment, the enlarged lymph nodes diminished gradually. However, 4 months later, echocardiogram showed the mediastinal mass remained, with two new masses (left 5.4×3.2cm, right 4.3×2.6cm) attached to the atrial septum respectively (Fig2). Prednisone was gradually increased to 50mg/d. 3 months later, the patient was admitted to hospital with chest pain and shortness of breath. Thoracic CT showed severe obstruction of the right middle lobe bronchus by the enlarged mediastinal mass. A bone marrow smear and flow cytometric analysis did not show any clonal hematopoietic abnormality. The echocardiogram remained unchanged. Prednisone (40mg/d) was re-administered but the patient's situation was getting worse quickly. He developed dyspnea and hypoxemia, apparent SVCS, edema of the inferior extremities and recurrent arrhythmia. CHOP chemotherapy was initiated (CTX 1.0 dl, VDS 4mg dl, EPI-ADM 60mg dl, DXM 10mg dl-2) but he died due to cardiac and respiratory failure. Kimura's Disease is a distinct clinicopathological entity accompanied by peripheral blood eosinophilia and elevated serum IgE level. Though rare, KD with cardiovascular involvement has been reported in several cases. This kind of involvement is also named as Eosinophilic Myocarditis (EM), occurs in up to 60% of patients diagnosed with hypereosinophilic syndrome (HES). However, this patient only has partial response of prednisone, and relapsed after 9 months therapy with prednisone 20–50 mg/d. Although chemotherapy was added, it was too late to control the progression of disease. This instructive case demonstrates that although Kimura's disease is a benign process, infiltration of eosinophils in multiple organs may result in critical illness and can be fatal. Early intervention and frequent close followup appears essential for controlling disease in KD patients with cardiac or other critical complications. The Clinical Characters and MRI Findings of Kimura Disease of Head and Neck

Abstract:Objective To investigate the clinical characters and MRI findings of Kimura disease and improve the understanding of this disease.Methods 11 cases of Kimura disease in the head and neck were verified with histopatholgy.Pre-and post-contrast MR images were preformed preoperatively in all patients then the clinical data and MRI findings were reviewed retrospectively.Results The lesions typically involve the subcutaneous soft tissues of the head and neck area in the young adult men.Typical Kimura disease featured subcutaneous lesions,continuously infiltrated the important glands such as salivary glands.Compared with the adjacent muscle,the MR signal intensity of all lesions was iso-to slightly hyperintense on T1-weighted images and hyperintense on T2-weighted images.Most of the lesions demonstrated marked enhancement on postcontrast MR images.MR images also showed tubular signal-intensity voids and strand-like hypointensities.The skin near the lesions was usually involved,as thickening of the epidermis and atrophy of the subcutaneous fat.Associated intraparotid lymphadenopathies,or reactive cervical lymphadenopathies were demonstrated in all patients.Conclusion There are some MRI characteristic features of Kimura disease.The accuracy rate can be increased when combined with the laboratory examination and clinical findings before operation.

All imaging data, including the location, quantity, size, uniformity, boundary, and enhanced appearance of the lesion were evaluated by two independent radiologists. Results: Of the five patients, four were asymptomatic, and one experienced localized skin itching. Four cases involved subcutaneous nodules in the upper arm, while one was in the inguinal region. The main manifestations were single (three cases) or multiple (two cases) subcutaneous nodules/masses, with three patients accompanied by local lymph node enlargement. Four patients exhibited elevated eosinophil counts in their peripheral blood. Four patients had lesions with vascular flow voids; in three of these, the lesions also showed prominent enhancement. Notably, the lesion in a 5-year-old did not show vascular flow voids but displayed significant enhancement. Additionally, two patients showed edema around the lesions. Conclusion: The presence of solitary or multiple subcutaneous nodules/masses in the upper arm or inguinal area, accompanied by lymph node enlargement, elevated eosinophils in the peripheral blood, and the observation of internal vascular within the lesion, can aid in the diagnosis of KD occurring in uncommon anatomical locations. Other rare areas include the armpit, groin, trunk, abdomen, and subcutaneous areas of the limbs. 2–6 The main clinical features include painless subcutaneous soft tissue masses, local lymph node enlargement, increased peripheral blood immunoglobulin E, and eosinophilia. 4,7 Given that KD is clinically similar to tumors, 8 especially in rare areas, differential diagnosis can be challenging. This study collected clinical data and imaging examination information of five rare sites KD patients admitted and confirmed in our hospital from 2015 to 2023, aiming to summarize the clinical and imaging features of rare site KD and achieve early and accurate diagnosis. Five KD patients with lesions occurring in rare areas (outside the head and neck) between 2015 and 2023 were included, and all lesions were confirmed by pathology after puncture biopsy or surgical resection. All five patients were male, aged 5–48 years, with a median age of 34. The duration of their disease course varied from 0.5 to 240 months. Retrospective analysis of clinical manifestations, laboratory examinations, pathological and imaging characteristics, and comprehensive evaluation were conducted for the included patients. The Sun Yat-sen University's Fifth Affiliated Hospital Medical Ethics Committee approved the study. Being retrospective, the study involved no alterations to treatment protocols and maintained patient confidentiality by anonymizing information during clinical and imaging analyses. Abstract:Introduction Kimura disease is a rare inflammatory condition predominantly seen in Asian males. Cases are characterized by painless subcutaneous nodules typically located in the head and neck, peripheral eosinophilia, and elevated IgE levels. We report a case of Kimura disease successfully treated with surgical excision and dupilumab. Case Description A 32-year-old Filipino woman presented for peripheral eosinophilia and a pruritic mass near the right epitrochlear region. Her history was notable for minimal change disease and a pruritic, papular rash to the bilateral extremities, consistent with eczema and prurigo nodularis on biopsy. She otherwise denied any fevers, chills, diaphoresis, or weight loss. Laboratory results showed an elevated absolute eosinophil count of 4284 cells/μL and elevated IgE of 8730 kU/L. Imaging revealed right axillary and left inguinal lymphadenopathy. Fine needle aspiration of the mass revealed a benign lymph node with increased eosinophils. Given the above findings paired with her Asian ethnicity, Kimura disease was suspected. Infectious and malignant causes were ruled out. Surgical excision showed follicular hyperplasia with prominent infiltrates of eosinophils to both lymph node and dermal tissue, confirming Kimura disease. Dupilumab was started, resolving her pruritus and eosinophilia. Discussion Kimura disease often affects the head and neck but can also involve the epitrochlear regions as well. Peripheral hypereosinophilia is common, and nephrotic syndrome can be present in over 50% of patients. Treatment typically involves surgical excision with additional therapies as needed. Dupilumab was used in this case due to concurrent prurigo nodularis. Prognosis is typically excellent, though recurrence is common.

Clinical and Pathological Study of Kimura's Disease with Renal Involvement.

Abstract:OBJECTIVE:Our objective was to retrospectively analyze the clinical and histological features of 8 Chinese patients with Kimura's disease and renal involvement.METHODS:Eight male patients with Kimura's disease admitted to Jinling Hospital from 1998 to 2006 were included. The diagnoses were confirmed by lymphoid biopsy. Peripheral blood eosinophil, CD4+ and CD8+ T lymphoid cell count and total serum IgE level were examined. Renal biopsy, IL-4 expression cells and IgE-positive cell counts in renal tissue were performed. Follow-up data were recorded in detail.RESULTS:Six patients were observed with the onset symptoms of subcutaneous mass or enlarged lymph nodes, and the other 2 with edema. Constitutional symptoms included bronchial asthma, enteritis, eczema, neuritis and nephrotic syndrome. Renal biopsy revealed mesangial proliferation with or without IgA deposition under immunofluorescence (n=6) and membranous nephropathy (n=2). Tubulointerstitial infiltration of eosinophils was found in 6 patients. IL-4-positive cells in renal interstitium were detected in 5 cases. IgE-positive cells were negative in all cases. Fusion of the epithelial foot process was observed under electron microscopy in patients with mesangial proliferation. All patients were sensitive to treatment with systemic prednisone, but 4 patients had a renal or extrarenal relapse after the dose tapered. Three patients who presented with solitary masses and underwent lymphoidectomy or mass excision were free from renal and extrarenal relapse. All patients had normal renal function at last follow-up.CONCLUSIONS:Mesangial proliferation, eosinophilic infiltration and podocyte confusion are prominent histological features of this cohort of patients. Patients are sensitive to prednisone therapy but apt to relapse. Lymphoidectomy can be helpful to prevent relapse.
Eosinophilic Peritonitis and Nephrotic Syndrome in Kimura’s Disease: a Case Report and Literature Review

Abstract:Background Eosinophilic peritonitis is a relatively rare entity. Kimura's disease is a rare chronic inflammatory disorder of unknown etiology, characterized by subcutaneous nodules mainly in the head and neck region, regional lymphadenopathy and occasional involvement of kidney. There is currently no report of eosinophilic peritonitis in Kimura's disease. Case presentation A 44-year-old Chinese man presented with abdominal distention, nausea, vomiting and edema in lower limbs for 1 month. Laboratory data showed elevated eosinophils in peripheral blood and ascites, nephrotic syndrome with progressively renal dysfunction, and elevated IgE. Ultrasonography of lymph nodes showed multiple lymphadenopathy in bilateral inguinal regions. Surgical excision was performed for one of the enlarged lymph nodes and histopathology revealed diagnosis of Kimura's disease. Renal biopsy indicated focal segmental glomerulosclerosis (FSGS) and acute tubulointerstitial nephritis with infiltration of eosinophils in renal interstitium. The patient was prescribed with oral prednisolone therapy (30 mg/day), and underwent continuous ambulatory peritoneal dialysis (CAPD). The peripheral and peritoneal eosinophil count decreased rapidly and normalized within 2 days. Forty-five days after prednisolone therapy, partial remission of nephrotic syndrome and decrease of serum creatinine were achieved while peritoneal dialysis dosage had decreased. Inguinal lymph nodes gradually shrunk in size. The overall conditions remain stable afterwards. Conclusions This rare case highlighted the clinical conundrum of a patient presenting with eosinophilic peritonitis, lymphadenopathy, nephrotic syndrome and renal failure associated with Kimura's disease. The remarkable eosinophilia, pathology of lymph node and kidney, as well as significant response to steroids should guide towards the diagnosis.

Methods: We report a case of Kimura disease that was diagnosed recently in our ward, with steroid-responsive but recurrent minimal-change nephrotic syndrome. Meanwhile, we also used three powerful Chinese journal search engines (Cqvip.com, Wanfang.data and ScienceChina) to search the cases reported in Chinese from 1984 to 2007. Results: The nephrotic syndrome of our patient occurred 20 months after the onset of Kimura disease. Renal biopsy revealed minimal-change lesions. The patient was responsive to the steroid, but proteinuria recurred. In most of the 19 other cases, the onset of nephrotic syndrome occurred after subcutaneous masses. Renal biopsy in 13 cases showed mesangial proliferative glomerulonephritis in 9, minimal change disease in 2 and membrane nephropathy in 2 cases. Serum creatinine levels were elevated in 5 patients. Conclusion: Normally, Kimura disease-associated nephrotic syndrome patients are sensitive to prednisone therapy but are likely to relapse. In patients with recurrent nephrotic syndrome, renal insufficiency is not uncommon. Copyright (C) 2008 S. Karger AG, Basel

Abstract:Background Kimura disease is characterized by inflammation, with its underlying causes remaining uncertain. There is a lack of comprehensive and systematic research on the pathology of this condition in pediatric patients. Our objective is to study the clinical and pathological attributes of Kimura disease in pediatric patients and investigate the potential diagnostic significance of immunoglobulin E (IgE) in this context. Methods Clinical and laboratory information, pathological characteristics, and follow-up data were correlated to examine the distinctive features. Immunohistochemistry, acid-fast staining, and molecular assay were used to identify the presence of IgE and pathogens. Results We conducted an analysis of five cases of Kimura disease in pediatric patients at our hospital. The patients’ ages ranged from 5 years and 7 months to 14 years and 2 months, with 4 (80%) being male. The most common site was the head and neck region, particularly the postauricular subcutaneous area. Eosinophilia was observed in four patients (80%), and two patients (40%) had elevated serum immunoglobulin E (IgE) levels. Histopathological changes included eosinophilic infiltrates, follicular hyperplasia, and the proliferation of postcapillary venules. Immunohistochemical results supported the reactive nature of the lymphoid process and IgE deposition in the follicle, while no specific pathogens were discovered by special staining. All patients underwent surgical excision, and none experienced recurrence in their original location. Conclusion Children with Kimura disease show distinct eosinophilic and IgE alterations in both laboratory findings and pathological features. The application of immunohistochemical staining of IgE could serve as a promising marker for diagnosing Kimura disease.

Abstract:Objective To explore the clinical characteristics, diagnosis and the therapeutic effect of Kimura's disease (KU). Methods Clinical data of 20 patients with pathologically confirmed KD admitted to Peking University People's Hospital from June 2000 to June 2019 were analysed. A total of 20 confirmed KD patients were enrolled in the study, 18 male and 2 female, with age-onset ranging from 2 to 58 years. Results The masses appear as focal, painless, and immovable with an unclear boundary. The most common predilection is head-neck region (n=15, 75%). 15 patients showed peripheral blood eosinophilia. 14 of 14 patients presented with increased serum IgE level. The prominent pathological characteristic is marked lymphoid hyperplasia accompanied by various degrees of vascular hyperplasia and eosinophil infiltration. Among the 20 patients, 12 experienced recurrence of disease after treatment (surgical resection alone: 9/9; oral corticosteroids combined with immunosuppressants: 1/3; surgical resection followed by oral corticosteroids combined with immunosuppressants: 2/6). Conclusion KD should be considered when the patient presents with head-neck swellings and lymphadenopathy, accompanied by an increase of IgE and eosinophil. Compared with surgery alone, combined therapy seems to be a promising treatment option to reduce the recurrence rate.
Kimura's disease or IgG4-related disease? A case-based review

Abstract:Kimura's disease usually presents with benign painless soft tissue swelling around the head and neck areas with eosinophilia and high immunoglobulin E (IgE) levels. IgG4-related disease is characterized by fibrosis and sclerosis of the involved organs, with infiltration of IgG4-positive plasma cells. There are some similar clinical manifestations of these two diseases, with eosinophilia or high IgE levels often found in patients with IgG4-related disease. However, the relation between these two diseases is not well known. We report a 23-year-old man who had multiple neck lumps for longer than 1 year. He had remarkable eosinophilia and elevated serum IgE levels. Biopsy of his lymph nodes suggested the diagnosis of both Kimura's disease and IgG4-related disease. In this article, we discuss the relationship between Kimura's disease and IgG4-related disease. We also suggest a possible model ("allergen-specific immunotherapy") to illustrate our view that an increase in IgG4-positive plasma cells is an epiphenomenon of Kimura's disease.

Here, we perform bulk-RNA sequencing and reveal a higher expression of transmembrane 176A (TMEM176A) with over-activated extracellular-signal-regulate kinase/mitogen-activated protein kinases (Erk/MAPK) signaling pathway in eosinophils of Kimura's disease compared with healthy controls. Flow cytometry analysis shows that the composition of lymphocytes, monocytes, and dendritic cell subsets are similar between Kimura's disease and healthy controls, which is further validated by scRNA-seq. Loss of S100 calcium binding protein P (S100P) is found in the CD24+ myeloid subset of Kimura's disease. In vitro functional assays show that S100P may participate in promoting reactive oxygen species (ROS) production in myeloid cells. Taken together, we are the first to study the immune pathogenesis systematically and demonstrate that Erk/MAPK signaling pathway might be a potential therapeutic target for Kimura's disease.

Abstract:Kimura’s disease (KD) is a rare chronic inflammatory disorder that commonly occurs in Asian males. It mainly presents as painless subcutaneous masses or lymphadenopathy in the head and neck region. The incidence of KD in the oral cavity is quite rare. We reported a rare case of a 53-year-old male who had KD in his soft palate, hard palate and bilateral tonsils associated with severe sleep apnea. This patient underwent radiotherapy and exhibited a good response to the treatment. Throughout the 12-month follow-up period, the patient’s condition remained satisfactory. Of the other 14 reviewed cases of KD in the oral cavity, the lesions can occur in the buccal mucosa, hard and soft palate, and mouth floor with specific clinical features. We further summarized their manifestations and treatments in order to guide the future identification and management of KD with lesions in the oral cavity.

Abstract:Background Kimura disease (KD) is a rare, chronic, inflammatory disorder characterized by painless subcutaneous masses, lymphadenopathy, elevated eosinophil counts and increased serum immunoglobulin E (IgE) levels. Due to the heterogeneity and rarity, current diagnostic and treatment approaches for KD are often delayed and have limited efficacy. Methods The clinicopathological data of 51 KD patients between January 2014 and December 2023 were retrieved and stratified based on age and mass size, respectively. A case ofrecurrent KD who responded well to a low dose of mepolizumab was described. Results We found that pruritus occurred in 45.8% of patients, with higher rates in older patients (P=0.033). Edema was observed in 29.2%, with higher prevalence in younger patients (P=0.010). The mean maximum mass size was 3.1cm, varying by age group (P=0.005). The mean time to diagnosis was 36 months, increasing with mass size (P=0.024). Meanwhile, we reported that one of these 51 patients was successfully treated with mepolizumab at a dose of 100mg every 4 weeks. Conclusion We conclude that pruritus and edema are the two main clinical manifestation of KD, and the size of mass is related to the time of diagnosis. Biologic agents targeting the Th2 pathway show promise for KD treatment.

Abstract:CASE PRESENTATION: A 6-year-old boy was referred to our hospital with an anterior mediastinal mass. This was discovered by chest radiography performed when the boy was examined after being caught by an elevator door about 2 weeks earlier. The patient had been born full term without any complications during pregnancy or delivery. No clinical symptoms were observed during this presentation, and he had no history of previous infections.
