Abstract:Objective: To develop and prospectively evaluate a web-based tool that forecasts the daily bed need for admissions from the cardiac catheterization laboratory using routinely available clinical data within electronic medical records (EMRs). Methods: The forecast model was derived using a 13-month retrospective cohort of 6384 catheterization patients. Predictor variables such as demographics, scheduled procedures, and clinical indicators mined from free-text notes were input to a multivariable logistic regression model that predicted the probability of inpatient admission. The model was embedded into a web-based application connected to the local EMR system and used to support bed management decisions. After implementation, the tool was prospectively evaluated for accuracy on a 13-month test cohort of 7029 catheterization patients. Results: The forecast model predicted admission with an area under the receiver operating characteristic curve of 0.722. Daily aggregate forecasts were accurate to within one bed for 70.3% of days and within three beds for 97.5% of days during the prospective evaluation period. The web-based application housing the forecast model was used by cardiology providers in practice to estimate daily admissions from the catheterization laboratory. Discussion: The forecast model identified older age, male gender, invasive procedures, coronary artery bypass grafts, and a history of congestive heart failure as qualities indicating a patient was at increased risk for admission. Diagnostic procedures and less acute clinical indicators decreased patients' risk of admission. Despite the site-specific limitations of the model, these findings were supported by the literature. Conclusion: Data-driven predictive analytics may be used to accurately forecast daily demand for inpatient beds for cardiac catheterization patients. Connecting these analytics to EMR data sources has the potential to provide advanced operational decision support.

Abstract:Emergent and unscheduled cardiology admissions from cardiac catheterization laboratory add complexity to the management of Cardiology and in-patient department. In this article, we sought to study the behavior of cardiology admissions from Catheterization laboratory using time series models. Our research involves retrospective cardiology admission data from March 1, 2012, to November 3, 2016, retrieved from a hospital in Iowa. Autoregressive integrated moving average (ARIMA), Holts method, mean method, naïve method, seasonal naïve, exponential smoothing, and drift method were implemented to forecast weekly cardiology admissions from Catheterization laboratory. ARIMA (2,0,2) (1,1,1) was selected as the best fit model with the minimum sum of error, Akaike information criterion and Schwartz Bayesian criterion. The model failed to reject the null hypothesis of stationarity, it lacked the evidence of independence, and rejected the null hypothesis of normality. The implication of this study will not only improve catheterization laboratory staff schedule, advocate efficient use of imaging equipment and inpatient telemetry beds but also equip management to proactively tackle inpatient overcrowding, plan for physical capacity expansion and so forth.

Abstract:Over the past several years, across the globe, there has been an increase in people seeking care in emergency departments (EDs). ED resources, including nurse staffing, are strained by such increases in patient volume. Accurate forecasting of incoming patient volume in emergency departments (ED) is crucial for efficient utilization and allocation of ED resources. Working with a suburban ED in the Pacific Northwest, we developed a tool powered by machine learning models, to forecast ED arrivals and ED patient volume to assist end-users, such as ED nurses, in resource allocation. In this paper, we discuss the results from our predictive models, the challenges, and the learnings from users' experiences with the tool in active clinical deployment in a real world setting.

Abstract:BACKGROUND AND OBJECTIVES:Excess fluid balance in acute kidney injury (AKI) may be harmful, and conversely, some patients may respond to fluid challenges. This study aimed to develop a prediction model that can be used to differentiate between volume-responsive (VR) and volume-unresponsive (VU) AKI. METHODS:AKI patients with urine output < 0.5 ml/kg/h for the first 6 h after ICU admission and fluid intake > 5 l in the following 6 h in the US-based critical care database (Medical Information Mart for Intensive Care (MIMIC-III)) were considered. Patients who received diuretics and renal replacement on day 1 were excluded. Two predictive models, using either machine learning extreme gradient boosting (XGBoost) or logistic regression, were developed to predict urine output > 0.65 ml/kg/h during 18 h succeeding the initial 6 h for assessing oliguria. Established models were assessed by using out-of-sample validation. The whole sample was split into training and testing samples by the ratio of 3:1. MAIN RESULTS:Of the 6682 patients included in the analysis, 2456 (36.8%) patients were volume responsive with an increase in urine output after receiving > 5 l fluid. Urinary creatinine, blood urea nitrogen (BUN), age, and albumin were the important predictors of VR. The machine learning XGBoost model outperformed the traditional logistic regression model in differentiating between the VR and VU groups (AU-ROC, 0.860; 95% CI, 0.842 to 0.878 vs. 0.728; 95% CI 0.703 to 0.753, respectively). CONCLUSIONS:The XGBoost model was able to differentiate between patients who would and would not respond to fluid intake in urine output better than a traditional logistic regression model. This result suggests that machine learning techniques have the potential to improve the development and validation of predictive modeling in critical care research.

Abstract:Objectives: This study sought to report outcomes from an efficiency improvement project in a large cardiac cath lab. Background: Operational inefficiencies are common in the cath lab, yet solutions are challenging. A detailed report describing and providing solutions for these inefficiencies may be valuable in guiding improvements in productivity. Methods: In this observational study, the authors report metrics of efficiency before and after a cath lab quality improvement program in June 2014. Main outcomes included lab room start times, room turnaround times, laboratory use, and employee satisfaction. Time series analysis was used to assess trend over time. Chi-square testing and analysis of variance were used to assess change before and after the initiative. Results: The principal changes included implementation of a pyramidal nursing schedule, increased use of an electronic scheduling system, and increased utilization of a preparation and recovery area. Comparing before with after the program, start times improved an average of 17 min, and on-time starts improved from 61.8% to 81.7% (p = 0.0024). Turnaround times improved from 20.5 min to 16.4 min (trend p < 0.0001), and the proportion of days at full lab utilization improved from 7.7% to 77.3% (p < 0.00001). There were no increases in overtime, night, or weekend cases. There was a reduction in full time employees from 36.1 in 2013 to 29.6 in 2016, with an improvement in employee satisfaction. Conclusions: A systematic approach to reducing inefficiencies can improve cath lab start times, turnaround times, and overall productivity. This knowledge may be helpful in assisting other cath labs in similar efficiency improvement initiatives.

Methods: Based on the Hadoop parallel computing platform, construct the data warehouse of the patients, and use big data mining analysis technology to analyze the behavior of the patients and the direction of the flow. Results: Combined with hospital emergency information construction, the introduction of Hospital Internet applications, emergency flow analysis and prediction model can provide the emergency population to provide advice, and provide data for emergency management. Development and validation of a machine learning framework for improved resource allocation in the emergency department

Abstract:Objective: The Emergency Severity Index (ESI) is the most commonly used system in over 70% of all U.S. emergency departments (ED) that uses predicted resource utilization as a means to triage [1], Mistriage, which includes both undertriage and overtriage has been a persistent issue, affecting 32.2% of total ED visits [2]. Our goal is to develop a machine learning framework that predicts patients' resource needs, thereby improving resource allocation during triage. Methods: This retrospective study analyzed ED visits from the Medical Information Mart for Intensive Care IV, dividing the data into training (80%) and testing (20%) cohorts. We utilized data available during triage, including patient vital signs, age, gender, mode of arrival, medication history, and chief complaint. Azure AutoML was used to create different machine learning models trained to predict the 144 target columns including laboratory panels and imaging modalities as well as medications required during patients' ED visits. The 144 models' performance was evaluated using the area under the receiver operating characteristic curve (AUROC), F1 score, accuracy, precision and recall. Results: A total of 391,472 ED visits were analyzed. 144 Voting ensemble models were created for each target. All frameworks achieved on average an AUC score of 0.82 and accuracy of 0.76. We gathered the feature importance for each target and observed that 'chief complaint', among others, had a high aggregate feature importance across different targets. Conclusion: This study shows the high accuracy in predicting resource needs for patients in the ED using a machine learning model. This can greatly improve patient flow and resource allocation in already resource limited emergency departments.

Abstract:Background The healthcare industry increasingly values high‐quality and personalized care. Patients with heart failure (HF) receiving home health care (HHC) often experience hospitalizations due to worsening symptoms and comorbidities. Therefore, close symptom monitoring and timely intervention based on risk prediction could help HHC clinicians prevent emergency department (ED) visits and hospitalizations. This study aims to (1) describe important variables associated with a higher risk of ED visits and hospitalizations in HF patients receiving HHC; (2) map data requirements of a clinical decision support (CDS) tool to the exchangeable data standard for integrating a CDS tool into the care of patients with HF; (3) outline a pipeline for developing a real‐time artificial intelligence (AI)‐based CDS tool. Methods We used patient data from a large HHC organization in the Northeastern US to determine the factors that can predict ED visits and hospitalizations among patients with HF in HHC (9362 patients in 12,223 care episodes). We examined vital signs, HHC visit details (e.g., the purpose of the visit), and clinical note–derived variables. The study identified critical factors that can predict ED visits and hospitalizations and used these findings to suggest a practical CDS tool for nurses. The tool's proposed design includes a system that can analyze data quickly to offer timely advice to healthcare clinicians. Results Our research showed that the length of time since a patient was admitted to HHC and how recently they have shown symptoms of HF were significant factors predicting an adverse event. Additionally, we found this information from the last few HHC visits before the occurrence of an ED visit or hospitalization were particularly important in the prediction. One hundred percent of clinical demographic profiles from the Outcome and Assessment Information Set variables were mapped to the exchangeable data standard, while natural language processing–driven variables couldn't be mapped due to their nature, as they are generated from unstructured data. The suggested CDS tool alerts nurses about newly emerging or rising risks, helping them make informed decisions. Conclusions This study discusses the creation of a time‐series risk prediction model and its potential CDS applications within HHC, aiming to enhance patient outcomes, streamline resource utilization, and improve the quality of care for individuals with HF. Clinical Relevance This study provides a detailed plan for a CDS tool that uses the latest AI technology designed to aid nurses in their day‐to‐day HHC service. Our proposed CDS tool includes an alert system that serves as a guard rail to prevent ED visits and hospitalizations. This tool can potentially improve how nurses make decisions and improve patient outcomes by providing early warnings about ED visits and hospitalizations.

Enhancing Uncertain Demand Prediction in Hospitals Using Simple and Advanced Machine Learning

Abstract:Early and timely prediction of patient care demand not only affects effective resource allocation but also influences clinical decision-making as well as patient experience. Accurately predicting patient care demand, however, is a ubiquitous challenge for hospitals across the world due, in part, to the demand's time-varying temporal variability, and, in part, to the difficulty in modelling trends in advance. To address this issue, here, we develop two methods, a relatively simple time-vary linear model, and a more advanced neural network model. The former forecasts patient arrivals hourly over a week based on factors such as day of the week and previous 7-day arrival patterns. The latter leverages a long short-term memory (LSTM) model, capturing non-linear relationships between past data and a three-day forecasting window. We evaluate the predictive capabilities of the two proposed approaches compared to two naïve approaches - a reduced-rank vector autoregressive (VAR) model and the TBATS model. Using patient care demand data from Rambam Medical Center in Israel, our results show that both proposed models effectively capture hourly variations of patient demand. Additionally, the linear model is more explainable thanks to its simple architecture, whereas, by accurately modelling weekly seasonal trends, the LSTM model delivers lower prediction errors. Taken together, our explorations suggest the utility of machine learning in predicting time-varying patient care demand; additionally, it is possible to predict patient care demand with good accuracy (around 4 patients) three days or a week in advance using machine learning.

Since the procedure of cardiac catheterization is very complex, the reporting system for cardiac catheterization, as we knew, has not yet well developed in most of the hospitals until now. Our approach is building a system fit into physician workflow and integration the PACS and HIS. The experimental results show that we can reduce operating costs and increase healthcare efficiency by using this system. The implementation of the system paves the way toward a comprehensive EPR of healthcare industry.

Abstract:BACKGROUND: Hospitals are complex adaptive systems within which multiple components such as patients, practitioners, facilities, and technology interact. A careful approach to optimization of this complex system is needed because any change can result in unexpected deleterious effects. One such approach is discrete event simulation, in which what-if scenarios allow researchers to predict the impact of a proposed change on the system. However, studies illustrating the application of simulation in optimization of general internal medicine (GIM) team inpatient operations are lacking.METHODS: Administrative data about admissions and discharges, data from a time-motion study, and expert opinion on workflow were used to construct the simulation model. Then, the impact of four changes: aligning medical teams with nursing units, adding a hospitalist team, adding a nursing unit, and adding both a nursing unit and hospitalist team with higher admission volume were modeled on key hospital operational metrics.RESULTS: Aligning medical teams with nursing units improved team metrics for aligned teams but shifted patients to unaligned teams. Adding a hospitalist team had little benefit, but adding a nursing unit improved system metrics. Both adding a hospitalist team and a nursing unit would be required to maintain operational metrics with increased patient volume.CONCLUSION: Using simulation modeling, we provided data on the implications of four possible strategic changes on GIM inpatient units, providers, and patient throughput. Such analyses may be a worthwhile investment to study strategic decisions and make better choices with fewer unintended consequences.

Abstract:Problem definition: Access to accurate predictions of patients' outcomes can enhance medical staff's decision-making, which ultimately benefits all stakeholders in the hospitals. A large hospital network in the US has been collaborating with academics and consultants to predict short-term and long-term outcomes for all inpatients across their seven hospitals. Methodology/results: We develop machine learning models that predict the probabilities of next 24-hr/48-hr discharge and intensive care unit transfers, end-of-stay mortality and discharge dispositions. All models achieve high out-of-sample AUC (75.7%-92.5%) and are well calibrated. In addition, combining 48-hr discharge predictions with doctors' predictions simultaneously enables more patient discharges (10%-28.7%) and fewer 7-day/30-day readmissions ($p$-value $<0.001$). We implement an automated pipeline that extracts data and updates predictions every morning, as well as user-friendly software and a color-coded alert system to communicate these patient-level predictions (alongside explanations) to clinical teams. Managerial implications: Since we have been gradually deploying the tool, and training medical staff, over 200 doctors, nurses, and case managers across seven hospitals use it in their daily patient review process. We observe a significant reduction in the average length of stay (0.67 days per patient) following its adoption and anticipate substantial financial benefits (between \$55 and \$72 million annually) for the healthcare system.
Prediction of Obstetric Patient Flow and Horizontal Allocation of Medical Resources Based on Time Series Analysis

Abstract:Objective: Given the ever-changing flow of obstetric patients in the hospital, how the government and hospital management plan and allocate medical resources has become an important problem that needs to be urgently solved. In this study a prediction method for calculating the monthly and daily flow of patients based on time series is proposed to provide decision support for government and hospital management. Methods: The historical patient flow data from the Department of Obstetrics and Gynecology of the First Hospital of Jilin University, China, from January 1, 2018, to February 29, 2020, were used as the training set. Seven models such as XGBoost, SVM, RF, and NNAR were used to predict the daily patient flow in the next 14 days. The HoltWinters model is then used to predict the monthly flow of patients over the next year. Results: The results of this analysis and prediction model showed that the obstetric inpatient flow was not a purely random process, and that patient flow was not only accompanied by the random patient flow but also showed a trend change and seasonal change rule. ACF,PACF,Ljung_box, and residual histogram were then used to verify the accuracy of the prediction model, and the results show that the Holtwiners model was optimal. R2, MAPE, and other indicators were used to measure the accuracy of the 14 day prediction model, and the results showed that HoltWinters and STL prediction models achieved high accuracy. Conclusion: In this paper, the time series model was used to analyze the trend and seasonal changes of obstetric patient flow and predict the patient flow in the next 14 days and 12 months. On this basis, combined with the trend and seasonal changes of obstetric patient flow, a more reasonable and fair horizontal allocation scheme of medical resources is proposed, combined with the prediction of patient flow.

Prediction of In-Hospital Cardiac Arrest in the Intensive Care Unit: Machine Learning-Based Multimodal Approach

Abstract:Background: Early identification of impending in-hospital cardiac arrest (IHCA) improves clinical outcomes but remains elusive for practicing clinicians. Objective: We aimed to develop a multimodal machine learning algorithm based on ensemble techniques to predict the occurrence of IHCA. Methods: Our model was developed by the Multiparameter Intelligent Monitoring of Intensive Care (MIMIC)-IV database and validated in the Electronic Intensive Care Unit Collaborative Research Database (eICU-CRD). Baseline features consisting of patient demographics, presenting illness, and comorbidities were collected to train a random forest model. Next, vital signs were extracted to train a long short-term memory model. A support vector machine algorithm then stacked the results to form the final prediction model. Results: Of 23,909 patients in the MIMIC-IV database and 10,049 patients in the eICU-CRD database, 452 and 85 patients, respectively, had IHCA. At 13 hours in advance of an IHCA event, our algorithm had already demonstrated an area under the receiver operating characteristic curve of 0.85 (95% CI 0.815-0.885) in the MIMIC-IV database. External validation with the eICU-CRD and National Taiwan University Hospital databases also presented satisfactory results, showing area under the receiver operating characteristic curve values of 0.81 (95% CI 0.763-0.851) and 0.945 (95% CI 0.934-0.956), respectively. Conclusions: Using only vital signs and information available in the electronic medical record, our model demonstrates it is possible to detect a trajectory of clinical deterioration up to 13 hours in advance. This predictive tool, which has undergone external validation, could forewarn and help clinicians identify patients in need of assessment to improve their overall prognosis.
Predicting Abnormalities in Laboratory Values of Patients in the Intensive Care Unit Using Different Deep Learning Models: Comparative Study

Abstract:Background: In recent years, the volume of medical knowledge and health data has increased rapidly. For example, the increased availability of electronic health records (EHRs) provides accurate, up-to-date, and complete information about patients at the point of care and enables medical staff to have quick access to patient records for more coordinated and efficient care. With this increase in knowledge, the complexity of accurate, evidence-based medicine tends to grow all the time. Health care workers must deal with an increasing amount of data and documentation. Meanwhile, relevant patient data are frequently overshadowed by a layer of less relevant data, causing medical staff to often miss important values or abnormal trends and their importance to the progression of the patient's case. Objective: The goal of this work is to analyze the current laboratory results for patients in the intensive care unit (ICU) and classify which of these lab values could be abnormal the next time the test is done. Detecting near-future abnormalities can be useful to support clinicians in their decision-making process in the ICU by drawing their attention to the important values and focus on future lab testing, saving them both time and money. Additionally, it will give doctors more time to spend with patients, rather than skimming through a long list of lab values. Methods: We used Structured Query Language to extract 25 lab values for mechanically ventilated patients in the ICU from the MIMIC-III and eICU data sets. Additionally, we applied time-windowed sampling and holding, and a support vector machine to fill in the missing values in the sparse time series, as well as the Tukey range to detect and delete anomalies. Then, we used the data to train 4 deep learning models for time series classification, as well as a gradient boosting-based algorithm and compared their performance on both data sets. Results: The models tested in this work (deep neural networks and gradient boosting), combined with the preprocessing pipeline, achieved an accuracy of at least 80% on the multilabel classification task. Moreover, the model based on the multiple convolutional neural network outperformed the other algorithms on both data sets, with the accuracy exceeding 89%. Conclusions: In this work, we show that using machine learning and deep neural networks to predict near-future abnormalities in lab values can achieve satisfactory results. Our system was trained, validated, and tested on 2 well-known data sets to ensure that our system bridged the reality gap as much as possible. Finally, the model can be used in combination with our preprocessing pipeline on real-life EHRs to improve patients' diagnosis and treatment.

Abstract:Chest computed tomography is one of the most common diagnostic tests, with 15 million scans performed annually in the United States. Coronary calcium can be visualized on these scans, but other measures of cardiac risk such as atrial and ventricular volumes have classically required administration of contrast. Here we show that a fully automated pipeline, incorporating two artificial intelligence models, automatically quantifies coronary calcium, left atrial volume, left ventricular mass, and other cardiac chamber volumes in 29,687 patients from three cohorts. The model processes chamber volumes and coronary artery calcium with an end-to-end time of ~18 s, while failing to segment only 0.1% of cases. Coronary calcium, left atrial volume, and left ventricular mass index are independently associated with all-cause and cardiovascular mortality and significantly improve risk classification compared to identification of abnormalities by a radiologist. This automated approach can be integrated into clinical workflows to improve identification of abnormalities and risk stratification, allowing physicians to improve clinical decision-making.

Abstract:Effective management of operating room resources relies on accurate predictions of surgical case durations. This prediction problem is known to be particularly difficult in pediatric hospitals due to the extreme variation in pediatric patient populations. We propose a novel metric for measuring accuracy of predictions which captures key issues relevant to hospital operations. With this metric in mind we propose several tree-based prediction models. Some are automated (they do not require input from surgeons) while others are semi-automated (they do require input from surgeons). We see that many of our automated methods generally outperform currently used algorithms and even achieve the same performance as surgeons. Our semi-automated methods can outperform surgeons by a significant margin. We gain insights into the predictive value of different features and suggest avenues of future work.

Abstract:Periodic Automatic Replenishment (PAR) is an inventory management policy that assists the healthcare sector in keeping the right amount of stock on hand to avoid excess stock and the potential for products to expire. Traditionally, hospitals rely on the experience and firsthand knowledge of stock management technicians to keep their store supplies with enough equipment. However, manual management based on “gut feeling“ and/or nursing feedback may lead to missing products or incorrect stock orders. Extracting accurate data is often too complex or time-consuming, resulting in a lack of critical reporting data to manage product inventories across the organization properly. However, adopting forecasting techniques and incorporating them into traditional PAR management policies can provide efficient solutions for controlling hospital warehouse inventory at the lowest cost. We have proposed a deep learning-based framework to monitor inventories to reduce costs and variation, create efficiencies, and improve the quality of patient care in hospitals. Furthermore, the proposed forecasting framework's performance is assessed using a real-world scenario within a hospital. The findings indicate that the system is capable of accurately tracking the usage of medical equipment, which results in a significant reduction in the unavailability of assets.

Abstract:Objectives: This project aims to determine the feasibility of predicting future critical care bed availability using data-driven computational forecast modelling and routinely collected hospital bed management data. Methods: In this proof-of-concept, single-centre data informatics feasibility study, regression-based and classification data science techniques were applied retrospectively to prospectively collect routine hospital-wide bed management data to forecast critical care bed capacity. The availability of at least one critical care bed was forecasted using a forecast horizon of 1, 7 and 14 days in advance. Results: We demonstrated for the first time the feasibility of forecasting critical care bed capacity without requiring detailed patient-level data using only routinely collected hospital bed management data and interpretable models. Predictive performance for bed availability 1 day in the future was better than 14 days (mean absolute error 1.33 vs 1.61 and area under the curve 0.78 vs 0.73, respectively). By analysing feature importance, we demonstrated that the models relied mainly on critical care and temporal data rather than data from other wards in the hospital. Discussion: Our data-driven forecasting tool only required hospital bed management data to forecast critical care bed availability. This novel approach means no patient-sensitive data are required in the modelling and warrants further work to refine this approach in future bed availability forecast in other hospital wards. Conclusions: Data-driven critical care bed availability prediction was possible. Further investigations into its utility in multicentre critical care settings or in other clinical settings are warranted.
