Abstract:Objective To investigate the expression of Cyclooxygenase-2 (COX-2) in uterine adenomyosis and its clinical significance. Methods The expression of COX-2 was examined by immunohistochemistry SP method, in ectopic endometrium and eutopic endometrium of patients with uterine adenomyosis, the results were compared with normal endometrium. Results COX-2 expression in ectopic or eutopic endometrium with adenomyosis was significantly higher than that in normal endometrium. (P< 0.05) . No statistically significant difference was found between ectopic endometrium and eutopic endometrium of adenomyosis(P>0.05). And there were no significant differences in COX-2 expression between proliferative phase and secretive phase of endometrium. Conclusion High expression of COX-2 may be involved in the pathogenesis and development of uterine adenomyosis, which may provide potent target for treatment of adenomyosis.

Abstract:Mifepristone (RU486), a synthetic steroid compound used as an abortifacient drug, has received considerable attention to its anticancer activity recently. To explore the possibility of using mifepristone as a cancer metastasis chemopreventive, we performed a systems pharmacology analysis of mifepristone-related molecules in the present study. Data were collected by using Natural Language Processing (NLP) and 513 mifepristone-related genes were dug out and classified functionally using a gene ontology (GO) hierarchy, followed by KEGG pathway enrichment analysis. Potential signal pathways and targets involved in cancer were obtained by integrative network analysis. Total thirty-three proteins were involved in focal adhesion-the key signaling pathway associated with cancer metastasis. Molecular and cellular assays further demonstrated that mifepristone had the ability to prevent breast cancer cells from migration and interfere with their adhesion to endothelial cells. Moreover, mifepristone inhibited the expression of focal adhesion kinase (FAK), paxillin and the formation of FAK/Src/Paxillin complex, which are correlated with cell adhesion and migration. This study set a good example to identify chemotherapeutic potential seamlessly from systems pharmacology to cellular pharmacology and the revealed hub genes may be the promising targets for cancer metastasis chemoprevention.
Investigation of the 12-Month Efficacy and Safety of Low-Dose Mifepristone in the Treatment of Painful Adenomyosis

Abstract:Objective::To study the 12-month effects and possible mechanisms of low-dose mifepristone in the treatment of adenomyosis.Methods::Patients included in this retrospective study had painful adenomyosis and previously received 5 mg mifepristone daily (group A, n = 45) or 5 mg mifepristone daily with a poor-effect levonorgestrel-releasing intrauterine device (group B, n = 13) for 12 months. Uterine size, serum CA125 levels, estradiol levels, Visual Analogue Scale (VAS) score, endometrial thickness, and hemoglobin levels were compared before and after treatment and investigated again at 3 to 6 months after drug withdrawal. Another 8 patients with adenomyosis (group C, n = 8) who underwent surgery for severe dysmenorrhea during the same period were only used as a control group for immunohistochemical research. Endometrial biopsy results and expression of nerve growth factor (NGF), cyclooxygenase-2 (COX-2), and nuclear-associated antigen Ki-67 (Ki-67) in endometrial tissues and adenomyotic lesions were also analyzed. Results::The VAS scores in both experimental groups at all time points during treatment and follow-up were significantly lower ( P <0.001) than those before treatment. The uterine size was significantly reduced, and endometrial thickness was distinctly thicker after 12 months of treatment than that before treatment in group A receiving 5 mg/d mifepristone. The immunohistochemical expression of NGF and COX-2 decreased in both eutopic and ectopic endometrium after treatment, whereas that of Ki-67 slightly increased in eutopic endometrium after treatment and rapidly recovered to the baseline value after stopping mifepristone. There were no signs of hyperplasia, atypical hyperplasia, or malignancy in the endometrial biopsies. Conclusions::The results suggested that a daily dose of 5 mg mifepristone for 12 months down-regulated the expression of NGF and COX-2 and was effective in treating painful adenomyosis with few side effects.

Abstract:Mifepristone, a progesterone receptor antagonist, was initially used to terminate early pregnancy. As scientific research advanced, it emerged to be effective in the treatment of various tumors and tumor-like conditions such as endometriosis. Despite the therapeutic potential of mifepristone, its therapeutic effect is still far from ideal because the drug is difficult to dissolve and to accumulate in the target tissue sites. To address this issue, mifepristone-loaded nanostructured lipid carriers (Mif-NLC) were prepared by a simple solvent diffusion method and their anti-endometriosis performance and mechanisms were initially investigated. By optimizing the preparation protocol, we obtained uniform and spheroidal Mif-NLC with an average particle size of 280 nm. The encapsulation rate and drug loading capacity were 64.67
Expression of Estrogen Receptor, Progesterone Receptor and Leukemia Inhibitory Factor on Endometrium During Different Ovarian Stimulation Protocols in Mice

Abstract:OBJECTIVE:To evaluate the influence of superovulation by GnRHa protocol and pregnant mare's serum gonadotropin (PMSG) alone on the expression of estrogen receptor (ER), progesterone receptor (PR) and leukemia inhibitory factor (LIF) mRNA on endometrium.METHODS:Forty-five female ICR mice were randomly allocated into 3 groups:(1) GnRHa+PMSG group: alarelin was give first for desensitizing the pituitary, then superovulation with PMSG; (2) PMSG group: mice were injected with PMSG only; (3) Natural cycle group: mice were given with same volume of saline. Endometrium samples were taken at 48 hours after given hCG or ovulation (control group). ER and PR in glandular cells were detected with SP immunohistochemistry semiquantitatively. Expression of LIF mRNA on endometrium was detected with reverse transcriptase-polymerase chain reaction (RT-PCR) analysis.RESULT:The positive rate(%) and expression intense (AU) of ER and PR on glandular epithelium cells were significantly lower in GnRHa+PMSG group and PMSG group than those in natural cycle group (all P <0.01). The expression of LIF mRNA was significantly lower in GnRHa+PMSG group and PMSG group than that in natural cycle group (all P <0.01); but the expressions of ER, PR and LIF in GnRHa+PMSG group were higher than those in PMSG group.CONCLUSION:The protocol with GnRHa down regulates the expressions of ER, PR and the LIF mRNA on the mice of secretive phase endometrium, suggesting it may have an adverse effect on the endometrial receptivity in mice, but it may still be better than PMSG alone.

Abstract:Objective To explore the effects of mifepristone on the expression of tumor necrosis factor alpha(TNF-α) in endometriosis rat models.Methods Endometriosis model rats were randomly divided into control group and experimental group.Experimental rats were given 3 weeks of oral administration of mifepristone.Expression of TNF-α in ectopic uterine endometrial tissue and endometrium in both groups were determined by immunohistochemical method,and TNF-α mRNA in both groups were measured by Real-time PCR.Results Expression of TNF-α in ectopic endometrial tissue was significant lower than that in the control group(P<0.05).Conclusion The efficiency of mifepristone on endometriosis may be related with the decreasing expression of TNF-α.

Abstract:OBJECTIVE:To explore the effects of Yiweining Recipe (YWNR), a compound Chinese herbal medicine, on expressions of metalloproteinase-2 (MMP-2) and cyclooxygenase-2 (COX-2) mRNAs in rats with endometriosis (EM).METHODS:Operational self-transplantation was applied in establishing the rat models. Detection of MMP-2 and COX-2 mRNAs was conducted with hybridization in situ.RESULTS:There were significant differences in the expressions of MMP-2 and COX-2 mRNAs between the untreated group and the high-dose YWNR-treated group. YWNR could reduce the expressions of MMP-2 and COX-2 mRNAs.CONCLUSION:YWNR can treat EM through reducing the positive expressions of MMP-2 and COX-2 mRNAs.
Effect of mifepristone on estrogen and progesterone receptors in human endometrial and endometriotic cells in vitro

Abstract:Objective: To investigate the expressions of estrogen (E) receptor and progesterone (P) receptor in human eutopic and ectopic endometrium and the effect of mifepristone (RU486) on them. Design: Prospective study. Setting: University hospital. Patient(s): Twenty-two patients with ovarian endometriosis and 13 patients with uterine leiomyoma were recruited. Intervention(s): Samples of ovarian endometrioma cyst tissue and endometrium were obtained from the 22 patients. A sample of endometrium was obtained from the 13 patients. Main outcome measure(s): Expressions of E and P receptors were determined using immunocytochemical method. Result(s): P receptor expression in endometrial epithelial cells with endometriosis was significantly higher than that without endometriosis in the early secretory phase. Estrogen receptor and epithelial P receptor expressions in endometriotic cells were significantly lower than those of endometrial cells during the proliferative phase, similar with the latter during the early secretory phase and significantly higher during the late secretory phase. RU486 down-regulated the expressions of E and P receptors in both the eutopic and the ectopic endometrial cells, and in some cases this down-regulating effect was more apparent when the concentration of RU486 was higher. Conclusion(s): Different steroid receptor expressions indicate different hormonal regulation between endometriotic and endometrial cells. The down-regulating effect on E and P receptors may be one of the therapeutic mechanisms of RU486 on endometriosis.
Probe into the Effect of Mifepristone on Angiogenesis of Human Chorionic Villi and Decidual Cells

Abstract:Objective:To probe into the effect of mifepristone on angiogenesis of human chorionic villi and decidual cells.Methods:Thirty-five specimens were obtained from pregnant women with amenorrhea of 5~7 weeks duration.12 women were treated with mifepristone and 10 cases were given mifepristone combined with misoprostol,while women administered with artificial abortion were as the control group.The levels of Vascular endothelial growth factor(VEGF) and microvascular density(MVD) of the specimens were assessed by immunohistochemistry for VEGF and CD34 antigen,and the effect of mifepristone on VEGF and MVD was observed.Results:The levels of VEGF in human chorionic villi and decidual cells administered with mifepristone(0.83±0.58scores and 0.83±0.39scores,respectively) or mifepristone combined misoprostol(0.80±0.63scores and 0.70±0.67scores,respectively) were significantly decreased(P<0.001 and P<0.05,respectively) comparing with the control group(1.61±0.65scores and 1.61±0.77scores,respectively).The levels of MVD in human chorionic villi and decidual cells administered with mifepristone(5.25±1.95scores and 18.58±4.83scores,respectively) or mifepristone combined misoprostol(4.30±1.06scores and 17.80±4.29scores,respectively) were also significantly decreased(P<0.001 and P<0.01,respectively) comparing with the control group(10.70±2.24scores and 28.08±8.81scores,respectively).However,no significant differences were showed between mifepristone and mifepristone combined misoprostol groups(P>0.05).Conclusions:Mifepristone can inhibit angiogenesis in human chorionic villi and decidual cells,which may be the mechanism of termination of early pregnancy by mifepristone.

Abstract:ObjectiveTo investigate the effects of the antiprogestin mifepristone on expression of endothelial (eNOS) nitric oxide synthase in human endometrium during the implantation phase.DesignProspective clinical study.SettingTeaching university hospital.Patient(s)Endometrial samples were obtained from nine healthy women with proven fertility.Intervention(s)One control and one treatment cycle were performed. Treatment with 200 mg of mifepristone was administered on LH day 2. Endometrial biopsy specimens were obtained on LH day 6 to 8.Main outcome measure(s)Expression of eNOS was identified by using immunohistochemistry and Western blot assay, and messenger RNA levels were determined with semiquantitative reverse transcription polymerase chain reaction.Result(s)Endothelial nitric oxide synthase was detected in vascular endothelium and glandular epithelium in the endometrium. Treatment with mifepristone significantly decreased eNOS expression in the endometrial glandular epithelium but did not affect endothelial eNOS.Conclusion(s)Mifepristone has differential effects on eNOS expression in the epithelium and endothelium in the human endometrium at the time of implantation. The results support a role of epithelial eNOS in human endometrial receptivity.
Mifepristone modulates the function of TGF-β/smads signal transduction pathway in early pregnant chorionic villi

Abstract:Objective To investigate the effect of mifepristone on the function of transforming growth factor β signal transduction in chorionic villi and the molecular mechanism of mifepristone inducing abortion.Methods Early pregnant chorionic villi were obtained from the surgical aspiration group(n=13) and the mifepristone inducing abortion group(n=20).Expression of smad7 which was the targeting gene of the TGF-β/smads signal transduction pathway in chorionic villi was determined by immunohistochemical staining with specific antibody and RT-PCR.Mean optic density and density scale were anzlyzed.Results Mean optic density of Smad7 in chorionic villi in the mifepristone inducing abortion group(0.346?6±0.046?9) was statistically higher than that in the surgical aspiration group(0.233?7±0.065?7)(P<0.01).Also,density scale of smad7mRNA in chorionic villi in the mifepristone inducing abortion group(0.561?7±0.410?3) was statistically higher than that in the surgical aspiration group(0.222?2±0.244?1)(P<0.02). Conclusion Mifepristone can up-regulate the function of the TGF-β/smads signal transduction pathway and expression of targeted genes,such as plasminogen activator inhibitor 1,affect migration and evasion of tropholast cells,which may be one of the molecular mechanisms for termination of early pregnancy by this drug.

Abstract:Objective: To examine the effect of progesterone and mifepristone(RU486,which is a progesterone antagonist) on HOXA11 gene expression in human endometrial epithelial cells.Methods: Endometrial samples were obtained from six normal cycling women in the proliferative phase.Endometrial glandular epithelial cells were cultured and treated with progesterone or/and 17β-estradiol,or pre-treated with RU486 for two hours,then incubated with progesterone or/and 17β-estradiol for 4,6,8 days.Semi-quantitative RT-PCR was used to examine HOXA11 messenger RNA levels.Results: HOXA11 expression was increased in endometrial epithelial cells after treatment with progesterone from the 4th day,peaked at the 6th day,but was equivalent to that of the 4th day at the 8th day.With the pre-treatment of RU486,these changes in HOXA11 expression disappeared.Conclusion: HOXA11 gene expression in human endometrial epithelium is up-regulated by progesterone in vitro,and this effect is mediated by progesterone receptor.

Abstract:Insulin-like growth factor binding protein-1 (IGFBP-1) and prolactin are recognized as crucial signals for the initiation and maintainance of decidualization. The purpose of the study was to investigate the effect of mifepristone on the expression of IGFBP-1, prolactin and progesterone receptors (PR) during the implantation phase in human endometrium. Eight fertile women were studied during control and treatment cycles. Treatment with 200 mg of mifepristone was administered on day LH +2. Endometrial samples were collected on day LH +6 to +8. Expression of IGFBP-1, prolactin and PR was identified using immunohistochemistry, and mRNA levels were determined with RT-PCR. In control specimens, IGFBP-1 and prolactin were localized to the cytoplasm of the endometrial glandular and to a lesser extent in stromal cells. In the same samples, PR immunoreactivity was detected in the nucleus of the endometrial stromal cells, and was absent from the glandular cells. After mifepristone treatment, there was a significant increase in the immunostaining and mRNA expression for IGFBP-1 and PR. Prolactin expression increased only slightly after treatment. These results support the view that administration of mifepristone in the early luteal phase does not simply retard endometrial development. Our findings provide further insight into the regulation of IGFBP-1 and prolactin by PR in the human endometrium in vivo.

Abstract:Objective: To study the effect and mechanism of Yiweining on treatment of endometriosis. Methods: To set up endometriosis model by rat self-endometria transplantation, IL-2、IL-10、E_2、P of serum and VEGF of endometriosis tissue were detected. Results: The five parameters were depressed by Yiweining. There was significant difference of each target between Yiweining group and model group(P<0.05). There was no significant difference of each target between the high dose of Yiweining group and Danazol group (P>0.05). Conclusion: Yiweining can inhibit the growth of endometrial transplants, which is relates with the VEGF of dometriotic tissue and serum cytokines and hormone.
Proteomics reveals protein profile changes in cyclooxygenase-2 inhibitor-treated endometrial cancer cells

Abstract:Objective: To examine effects of an inhibitor of cyclooxygenase (COX)-2, NS-398, on the proliferation, apoptosis and invasion characteristics of endometrial cancer cell RL95-2. Methods: (1) Western blotting was carried out to determine COX-2 protein expression in RL95-2 cells and normal endometrium specimens. (2) The effect of NS-398 treatment on the cell proliferation, apoptosis, and invasion was assessed by methyl thiazolyl tetrazolium assay, flow cytometry, and matrigel invasion assay, respectively. (3) Finally, the proteomic analysis was used to find out proteins that are differentially expressed because of NS-398 treatment. Results: (1) COX-2 protein in RL95-2 cell line was significantly higher than that in normal endometrium. (2) NS-398 had significant growth inhibition effects on RL95-2 cells in a dose- and time-dependent manner. (3) NS-398 increased the proportion of cells in G1 and decreased the proportion of cells in the G2 phase in RL95-2 cells. (4) NS-398 could restrain endometrial cancer cells invasion. (5) The proteomic analysis revealed several proteins that are differentially expressed because of NS-398 treatment; the down-regulated proteins identified are hnRNP K, alpha enolase, Hsp70, tropomyosin, and protein disulfide isomerase, the up-regulated protein is phosphatidylethanolamine binding protein. Conclusions: The expression of COX-2 plays an important role in tumorigenesis of endometrial cancer. NS-398 can inhibit the ability of RL95-2 cell proliferation, viability, and invasion. In this study, the well-resolved reproducible 2-DE maps of NS-398 treated and control RL95-2 cells were established, and the significantly different expressed proteins are preliminary identified.

Abstract:OBJECTIVE:To study the effects of estrogen, progestogen and mifepristone on endometrial carcinoma cell lines in vitro.METHODS:The well-differentiated endometrial cancer cell line Ishikawa and moderate-differentiated endometrial cancer cell line Hec-1B were cultured in vitro. The cells were divided into four groups: control, estrogen, estrogen and progestogen, estrogen and progestogen and mifepristone, then we implanted cells in 96-well plates to search the response in distinct hormones.RESULTS:When stimulated by estrogen for 72 hours, the growth of Ishikawa cells was significantly higher than the control, Hec-1B cells only grew higher than the control, but it was no significance in statistics. Progestogen inhibited Ishikawa cells significantly after being stimulating for 72 hours, there was the same effect on Hec-1B cells after being stimulating for 96 hours. On the base of estrogen and progestogen, we added mifepristone, cells developed after 96 hours in Ishikawa cells and cells developed after 48 hours in Hec-1B cells.CONCLUSION:Estrogen can cause endometrial carcinoma cell growth; progestogen inhibits the hyperplasia induced by estrogen; mifepristone antagonizes the effect of progestogen on cell growth.
INHIBITORY EFFECTS OF MIFEPRISTONE ON THE GROWTH OF HUMAN GASTRIC CANCER CELL LINE MKN-45 IN VITRO AND IN VIVO

Abstract:OBJECTIVE:To explore the effects of mifepristone on the growth of human gastric cancer cell line MKN-45 and its possible mechanisms.METHODS:In situ hybridization was used to detect the expression of progesterone receptor (PR) mRNA in MKN-45 cells. Proliferation, cell cycle distribution, and the expression of Bcl-xL and vascular endothelial growth factor (VEGF) of MKN-45 cells incubated with various concentrations of mifepristone (1, 5, 10, and 20 micromol/L) were analyzed using MTT reduction assay, flow cytometry, reverse transcription-polymerase chain reaction (RT-PCR), and enzyme-linked immunoabsorbent assay (ELISA), respectively. After transplantation of MKN-45 cells underneath the skin of athymic mice, mifepristone was administrated with the dose of 50 mg/(kg x d) for 6 weeks to evaluate the tumor growth. Apoptosis and the expression of proliferating cell nuclear antigen (PCNA) in xenografted tumors were detected using transmission electron microscopy and immunohistochemical staining, respectively.RESULTS:PR mRNA was highly expressed in cultured MKN-45 cell. Mifepristone dose-dependently inhibited the proliferation of MKN-45 cells, and the inhibitory rate was dramatically increased from 7.21% to 47.23%. The inhibitory effect was accompanied by a dose-dependent increase in the percentage of cells in G0/G1 phase, and with a concurrent decrease in the proportion of S- and G2M-phase cells and the proliferative index from 57.65% to 24.54%. Meanwhile, mifepristone down-regulated the expression of Bcl-xL and VEGF in a dose-dependent manner. In vivo, mifepristone effectively inhibited the growth of xenografted tumors in nude mice (55.14% for inhibitory rate), induced apoptosis, and down-regulated PCNA expression in gastric cancer.CONCLUSION:Mifepristone exerts significant growth inhibitory effects on PR-positive human MKN-45 gastric cancer cells via multiple mechanisms, and may be a beneficial agent against the tumor.

Abstract:AIM To explore the effects of mifepristone, a progesterone receptor (PR) antagonist, on the proliferation of human gastric adenocarcinoma cell line SGC-7 901 in vitro and the possible mechanisms involved. METHODS In situ hybridization was used to detect the expression of PR mRNA in SGC-7901 cells. After treatment with various concentrations of mifepristone (2.5, 5, 10, 20 micromol/L) at various time intervals, the ultrastructural changes, cell proliferation, cell-cycle phase distribution, and the expression of caspase-3 and Bcl-XL were analyzed using transmission electron microscopy (TEM), tetrazolium blue(MTT) assay, 3H-TdR incorporation, flow cytometry, and reverse transcription-polymerase chain reaction (RT-PCR). RESULTS Mifepristone markedly induced apoptosis and inhibited cell proliferation of PR- positive SGC-7901 cells revealed by TEM, MTT assay and 3H-TdR incorporation, in a dose- and time-dependent manner. The inhibitory rate was increased from 8.98% to 51.29%. Flow cytometric analysis showed mifepristone dose-dependently decreased cells in S and G2/M phases, increased cells in G0/G1 phase, reduced the proliferative index from 57.75% to 22.83%. In addition, mifepristone up-regulated the expression of caspase-3, and down- regulated the Bcl-XL expression, dose-dependently. CONCLUSION Mifepristone effectively inhibited the proliferation of PR-positive human gastric adenocarcinoma cell line SGC-7901 in vitro through multiple mechanisms, and may be a beneficial agent against human adenocarcinoma.
