Contributions to Low Birth Weight by Different Components of Ambient Air Pollution During Gestation

Abstract:Introduction: While maternal exposure to ambient particulate air pollution with aerodynamic diameter under 2.5 μm (PM2.5) during gestation has been linked to low birth weight, a risk factor for hypoglycemia, respiratory disorders, and developmental delay, it is unclear which components of PM2.5 contribute most. We therefore explore the effect of sulfate, nitrate, organic carbon (OC), and elemental carbon (EC), which are PM2.5 components, on birth weight. Methods: For >500,000 live singleton births from 2001 to 2012 in Massachusetts, we link each birth to nitrate, sulfate, OC, EC exposure for the period spanning clinical gestational age. These component predictions come from a neural network model that uses monitoring, land use, emissions, chemical transport, and satellite data. We use linear mean and quantile regression to investigate the effect of each component on birth weight, adjusted for marital status, maternal education, government-supported prenatal care, maternal/paternal age and race, maternal cigarette smoking before/during pregnancy, parity, Kessner Index, risk factors for diseases, mode of delivery, infant sex, clinical gestational age, and PM2.5 mass. Results: In fully-adjusted models, decreases to mean birth weight was most strongly associated with EC, followed by nitrate, OC, and sulfate. Per µg/m^3 of EC, mean birth weight decreased 34.92 g (95% CI: 27.76 g, 42.07 g); per µg/m^3 of nitrate, mean birth weight decreased 6.75 g (95% CI: 3.12 g, 10.39 g). Quantile regression found that while the association between PM2.5 and birth weight was most statistically significant and negative at the lower end of the birth weight distribution, the association between EC or nitrate with decrease in birth weight was significant and equally-negative along most of the birth weight distribution. Conclusion: Certain particle air pollution components, such as EC and nitrate, showed stronger and more consistent associations with low birth weight than others.
