Abstract:Purpose To present the clinical manifestations and the outcomes of surgical treatments of 13 patients(11 of them were followed up) with discoid medial meniscal injury.Methods The epidemiology,clinical manifestations,operation methods,treatment outcome and radiographic characteristics of 13 patients with discoid medial meniscal injury was reviewed retrospectively.Results The incidence of discoid medial meniscal injury(including complete and incomplete types) accounted for 1.35‰ of the total meniscus injuries,and most of the patients were male.The average age of the patients with injury of the discoid medial meniscus was 28.8±14.0 years old.The majority of meniscal tears(longitudinal or horizontal tear) was caused by extreme internal rotation at knee flexion.The most common clinical symptoms and signs included pain,effusion,snap and clicking,and tenderness at the medial joint line.Total meniscectomy was performed in these patients.Tegner score and the Lysholm score at 2-year follow up were 7(4 to 9) and 94.8±2.4,respectively.However,long-term follow-up revealed degenerative changes in the medial compartment of all the involved knees.Conclusion The discoid medial meniscal injury occurs rarely.The clinical signs and symptoms of discoid medial menical injuries are similar to those of any other meniscal injury.Meniscectomy for injured discoid medial meniscus produces promising short-term results,and thereafter gradually causes secondary degeneration of cartilage in the medial compartment.

Abstract:The discoid meniscus is a common congenital meniscal malformation that is prevalent mainly in Asians and often occurs in the lateral discoid meniscus. Patients with asymptomatic discoid meniscus are usually treated by conservative methods such as observation and injury avoidance, while patients with symptoms and tears need to be treated surgically. Arthroscopic saucerization combined with partial meniscectomy and meniscus repair is the most common surgical approach., and early to mid-term reports are good. The prognostic factors are the patient's age at surgery、follow-up time and type of surgery. Some patients experience complications such as prolonged postoperative knee pain, early osteoarthritis, retears and Osteochondritis dissecans. The incidence of prolonged postoperative knee pain was higher and the incidence of Osteochondritis dissecans was the lowest. Retears of the lateral meniscus is the main reason for reoperation.
Clinical application of microendoscopic discectomy for the treatment of lumbar disc herniation

Abstract:Objective To analyse the clinical effects of microendoscopic discectomy(MED)with traditional discectomy by laminectomy fenestration,and explore the advantages of MED.Methods Retrospective analysis of the clinical data of 121 cases of lumbar disc herniation were made.The study group(52 cases)were treated with MED,the control group(69 cases)treated with traditional discectomy by laminectomy fenestration(open surgery).The incision length,hemorrhage in operation,operation time,ratio of improved raising degree by straight limb test,the time of out of bed,residence time after operation,recovery time,the rate of excellent and good outcomes according to Macnab scale after 3 months operation in study group were compared to the control group.The complications were recorded.Results The rate of excellent and good outcomes in study group treated with MED was 98.08% compared with 88.41% in the control group,and there was no statistical difference in two groups(χ 2=2.75,P>0.05).But there were statistical differences in incision length,hemorrhage in operation,ratio of improved raising degree by straight limb test,the time of out of bed,residence time after operation,and recovery time of the two groups(t=30.27,22.75,1.46,11.47,13.85,14.69,16.04,all P< 0.05)except in operation time(t=1.46,P>0.05).There was one case suffered more pain after MED,two cases occurred dura mater ruputure in control group.Conclusions To treat lumbar disc herniation,MED has the advantages of minimaltrauma to lumbar tissue and less recovery time compared with open surgery.
Clinical Outcome of Arthroscopic Meniscectomy Combined with Suture for Treatment of Unstable Discoid Lateral Meniscus

Abstract:Objective To prospectively analyze the arthroscopic meniscectomy and suture of the peripheral rim in treating patients with unstable discoid lateral meniscus and the short-term clinical outcome.Methods We conducted arthroscopic meniscectomy and suture of the peripheral rim in 49 patients(52 knees)suffered from unstable discoid lateral meniscus from Jun.2007 to Oct.2008 in the Department of Sports Medicine,Huashan Hospital.The patients were followed up for 14-30 months with an average of 20.8 months after operation by rechecking MRI to analyze the therapeutic effectiveness and the Lysholm score and HSS knee score(Hospital for Special Surgery knee score)were analyzed Results The postoperative Lysholm and HSS score improved from 43 ± 7.3 to 90 ± 5.3(P < 0.01)and 40±8.6 to 89±7.3(P < 0.01)respectively,excellent and good scoring was up to 86.5% and 87.9% respectively.The repeated MRI examination of 24 knees revealed that 21 knees healed and 3 knees healed partially.Conclusion Arthroscopic meniscectomy and suture of the peripheral rim in treating patients with unstable discoid lateral meniscus has the advantage of less trauma and satisfactory outcome.

Abstract:Discoid lateral meniscus is an intra-articular knee disorder that typically presents in children and adolescents. The natural history depends on the type of anomaly and the nature and presence of symptoms. Management of this disorder should be directed toward resolution of the symptoms while preserving meniscal tissue and function. Modern surgical techniques make suturing and preservation of meniscal tissue feasible. In the present article, the clinical manifestations, diagnostic criteria and practical management considerations are reviewed.

Abstract:Purpoes: To discuss the MRI manifestation and classification of discoid lateral meniscus and improve the accuracy of MRI diagnosis for discoid meniscus.Materials and Methods: 70 cases of discoid meniscus verified by arthroscopy were divided into impuberal group(n=30) and adult group (n =40).Following three quantitative parameters were measured.The meniscal width and tibial plateau width on the coronal slice showing the inttercondyloid spine( a),the number of the spine showing "butterfly" change when the layer thickness was 4mm(b) and the maximum width showing "butterfly" change on the sagittal slice(c).The discoid meniscus were classified as slab type (n=33),wedge type(n = 19),hypertrophied - horn type(n= 18) according to their shape.Results:69 cases had torn in different extent,a>or =20% (P<0.01) in all of cases.No significant difference in the incidence of each type existed between two groups(P>0.05).Significant difference in the incidence of each type existed in c(P<0.05).Conclusions: a >or = 20% is the most reliable criterion to diagnose discoid meniscus.Using b as the criterion of discoid meniscus is easy to occur missed diagnosis,c in the wedge type is bigger than the one in the hypertrophied type.Age can impact the classification of discoid meniscus.

Abstract:The discoid meniscus is a congenital morphological abnormality encountered far more commonly on the lateral than the medial side. The discoid lateral meniscus (DLM) is more prevalent in Asia with an incidence of 10%-13%, than in the Western world with an incidence of 3%-5%. DLM can be bilateral in more than 80% cases. Due to its abnormal shape and size, the discoid meniscus is prone to tearing and has an impact on gait mechanics. The discoid meniscus has deranged collagen arrangement and vascularity which can have implications for healing after a repair. Patients with a DLM may or may not be symptomatic with mechanical complaints of locking, clicking, snapping or pain. Symptoms often arise due to a tear in the body of the meniscus or a peripheral detachment. Asymptomatic patients usually do not require any treatment, while symptomatic patients who do not have locking are managed conservatively. When a peripheral detachment is present, it must be stabilised while preserving the meniscus rim to allow transmission of hoop stresses. Rehabilitation after surgery is highly individualised and return to sports is possible after more than 4 months in those undergoing a repair. The functional outcomes and onset of radiographic arthritis after saucerisation and repairing a discoid meniscus are better in the long term, compared with a subtotal meniscectomy. However, there is no compelling evidence currently favouring a repair as results deteriorate with increasing follow-up. Poor prognosis is reported in patients undergoing a total meniscectomy, a higher age at presentation and valgus malalignment.
Clinical efficacy and pain follow-up after arthroscopic discoid meniscoplasty of the knee: a retrospective study

Abstract:Background: To assess the viability and efficiency of performing arthroscopic meniscoplasty in treating discoid meniscus (DM) in the knee joint. Methods: A total of 29 patients diagnosed with symptomatic lateral DM between October 2014 and December 2019 were included in the study. Among them, 7 patients with intact DM underwent arthroscopic discoid meniscoplasty (group A), while 22 patients with torn DM received arthroscopic DM plasty along with repair and suturing (group B). Both Visual Analog Scale (VAS) score and Lysholm score assessments were conducted preoperatively and postoperatively. Result: The favorable and acceptable outcome rate was 85.71% in group A and 95.45% in group B (P > 0.05). The VAS scores post-operatively at each follow-up time point were consistently lower compared to pre-operative values, while the Lysholm knee function scores showed improvement. There were no significant differences in VAS score and Lysholm score between group A and group B at different stages (P > 0.05). Both surgical techniques (discoid meniscoplasty and discoid meniscoplasty combined with repair and suture) showed significant improvement in postoperative VAS score and Lysholm score, but there was no difference in outcomes between the two groups. Conclusion: Overall, the two surgical techniques studied in this study (discoid meniscoplasty and discoid meniscoplasty combined with repair and suture) produced similar results in terms of pain reduction and improved knee function.

Abstract:PurposeDiscoid lateral meniscus (DLM) is a common meniscal anatomic disorder and can cause irreversible knee consequences. The long-term clinical outcomes of DLM management remain unclear. Moreover, several potential factors would influence the long-term functional outcomes. The factors also remain unclear. This study aims to evaluate the long-term clinical outcomes of the arthroscopic management of the DLM and to identify the patients’ factors affecting the long-term results.MethodsMedical records were retrospectively examined for patients with symptomatic unilateral DLM who underwent arthroscopic procedures between January 2004 and August 2011. The characteristics of DLM, the preoperative and the long-term postoperative visual analog scale (VAS), the International Knee Documentation Committee (IKDC) score, Lysholm, and Yulish scores were evaluated. Data were collected, processed, and analyzed using SAS software version 9.2. Univariate and multivariate analyses were performed to identify the factors influencing the long-term outcomes of the DLM arthroscopy.ResultsA total of 128 patients were included, most of whom were females (68.7%). The median age of the included participants was 24 (16–31) years old. The median duration of symptoms was 23.3 (10, 31) months, and the median follow-up duration was 126.2 (113, 140) months. The functional scores significantly improved postoperatively compared to the preoperative scores; VAS (1.65 ± 1.17 vs. 6.08 ± 1.31), Lysholm (91.39 ± 5.05 vs. 77.51 ± 10.19), and IKDC (84.63 ± 7.69 vs. 67.89 ± 9.56), respectively; p<0.05. Multivariate analysis revealed that gender, the status of self-reported instability, preoperative VAS, Yulish MR cartilage grade, and Lysholm score had a significant correlation with the worsening of the final follow-up IKDC scores.ConclusionsArthroscopic procedure significantly improved the long-term joint function of the DLM patients, as evidenced by the sustained improvement of the VAS, Lysholm, and IKDC scores. These clinical outcomes were greatly influenced by gender, the status of self-reported instability, preoperative Yulish MR cartilage grade, VAS, and Lysholm score.

Research on Influencing Factors of Clinical Efficacy of Meniscus Resection Based on Logistic Regression Analysis

Abstract:In order to solve the factors affecting the clinical efficacy of meniscus resection, a method based on logistic regression analysis is proposed. From May 2019 to May 2020, 60 patients with discoid meniscus who underwent arthroscopic surgery in the Joint Department of the Second Hospital of a certain city were selected as the research objects; the surgical methods are divided into partial meniscus excision and plasty and total meniscus resection. The Lysholm function score was used to evaluate the clinical efficacy of arthroscopic surgery for discoid meniscus injuries before and 3, 6, and 12 months after surgery and postoperative application of Ikeuchi score and Tegner exercise ability score to assess age, gender, body mass index (BMI), duration of symptoms, and the influence of meniscus injury types and surgical methods on the efficacy of arthroscopic surgery for discoid meniscus injuries. Experimental results show that Ikeuchi's assessment of the excellent and good rate of arthroscopic knee joints was significantly higher than that of the control group, the incidence of postoperative pain was significantly lower than that of the control group, and the difference was statistically significant (P < 0.05). Postoperative pain and premature weight-bearing of discoid meniscus injury of knee joint, factors such as noncold compress after operation, articular cartilage damage, age, and time from onset to operation are closely related; the difference was statistically significant (P < 0.05). Postoperative evaluation according to Ikeuchi score: excellent in 38 cases, good in 14 cases, 8 cases were poor, and the excellent and good rate was 86.7%. The patient's age, type of meniscus tear, and duration of symptoms have a certain impact on the postoperative clinical efficacy of discoid meniscus injury; BMI and surgical methods have no significant impact. Logistic regression analysis results show that postoperative pain and premature weight-bearing of discoid meniscus injury of the knee joint, no cold compress after operation, accompanied by articular cartilage damage, age, and factors such as onset to operation time are closely related; the difference was statistically significant (P < 0.05). It proves that arthroscopic surgery for discoid meniscus injury has the advantages of less damage and faster recovery, it is the first choice for the treatment of discoid meniscus injury, and the postoperative effect is significant in young patients and those with short duration of symptoms; mixed tears have a greater impact on the postoperative recovery of patients with discoid meniscus injury; therefore, patients with discoid meniscus injury should undergo surgery as soon as possible and perform active rehabilitation exercises after the operation.

Introduction:Discoid lateral meniscus is a deformity of the meniscus characterized by hypertrophy of the central portion of the meniscus; the incidence rate ranges from 3% to 5% and reaches 15% in Asian ethnicities. Previous researchers have hypothesized that failure to internalize the central portion of the meniscus during embryonic and fetal development due to genetic mutations may be the cause of this disorder. In a number of studies, the detection and selection of appropriate measures have yielded positive results.Case:We reported a 33-year-old male patient complaining that the left leg feels like locking since 4 months ago, previously the patient complained of a left knee sounding "click" after falling while running fast. Patient was diagnosed with a lateral discoid meniscus of the left knee after undergoing a physical examination, an x-ray examination, and an MRI examination. For a torn lateral discoid meniscus, the patient underwent knee arthroscopy and saucerization.Discussion:Discoid meniscus is an anatomical variation of discus-related injuries. The diagnosis is based on the "bow-tie sign" and can be diagnosed by the MRI. The total arthroscopic approach is used to determine the normal position of the discoid, while the saucerization technique is used to maintain normal position. Studies have shown that the saucerization technique can improve the stability of the discoid, reduce neurovascular damage, and reduce the potential of the chondral structure.The combination of saucerization and arthroscopic techniques can help minimize the risk of discoid meniscus injuries.Conclusion:Cases of discoid meniscus are uncommon, and their etiology remains unknown. In asymptomatic cases, conservative is the treatment of choice, whereas in cases with symptoms, surgery can be performed. Saucerization with arthroscopy is currently the most commonly used procedure in its management.

Abstract:Discoid lateral meniscus (DLM) is the most common congenital variant of the lateral meniscus, which is prone to degeneration and lesions, and often leads to knee osteoarthritis. At present, there is no consensus on the clinical practice of DLM, and this expert consensus and practice guidelines on DLM was developed and approved by Chinese Society of Sports Medicine according to the Delphi method. Among 32 statements drafted, 14 statements were excluded for redundant information, and 18 statements achieved consensus. This expert consensus focused on the definition, epidemiology, etiology, classification, clinical manifestations, diagnosis, treatment, prognosis, and rehabilitation of DLM. Restoring the normal shape, retaining appropriate width and thickness, and ensuring the stability of the remnant meniscus is critical to sustaining the physiological function of the meniscus and preserving the knee. The partial meniscectomy with or without repair should be the first-line treatment when possible, given that the clinical and radiological long-term outcomes of total or subtotal meniscectomy are worse.
Multivariate ordered logistic regression analysis of the postoperative effect of symptomatic discoid lateral meniscus

Abstract:Abstract Introduction The postoperative effect of arthroscopy in the treatment of symptomatic discoid lateral meniscus (DLM) varies greatly among individuals. Therefore, this study aims to investigate the factors affecting the postoperative outcomes of symptomatic DLM. Materials and methods According to the inclusion and exclusion criteria, patients with symptomatic single-knee DLM who underwent arthroscopic surgery at our hospital from 9/2008 to 9/2015 were included. Retrospectively collected 16 factors probably affecting postoperative outcomes. The Ikeuchi grade system was used to evaluate the knee joint function. Univariate analysis was performed by Kruskal–Wallis rank-sum test or Mann–Whitney U test, and multivariate analysis by ordered logistic regression. P < 0.05 was considered statistically significant. Results A sum of 502 patients was included, including 353 females (70.3%) and 149 males (29.7%). Difference between preoperative and postoperative Ikeuchi grade was statistically significant ( P < 0.001). Female was bad to obtain a good Ikeuchi grade ( P = 0.009, OR 0.458). Outerbridge grade ( P = 0.018, OR 0.638) was negatively correlated with Ikeuchi grade. BMI ( P = 0.001, OR 0.875) and work intensity ( P = 0.020, OR 0.611) were inversely correlated with Ikeuchi grade. Age of onset ( P < 0.001, OR 0.956) and symptoms duration ( P < 0.001, OR 0.988) were negatively correlated with Ikeuchi grade. Besides, compared to total meniscectomy, meniscoplasty with a repair was an unfavourable factor for Ikeuchi grade ( P = 0.044, OR 0.245). Conclusions With the increase of BMI, work intensity, age of onset, duration of symptoms, and the severity of cartilage lesion, the postoperative results become worse. Moreover, female and meniscoplasty with repair are risk factors for the postoperative outcomes.

Bilateral discoid medial Meniscus: Two case reports

Abstract:Rationale: Discoid meniscus is an anatomical congenital anomaly more often found in the lateral meniscus. A discoid medial meniscus is a very rare anomaly, and it is difficult to diagnose a bilateral discoid medial meniscus because not all the discoid medial menisci are symptomatic. Herein, we report 2 cases of bilateral discoid medial meniscus. Patients concerns: Case1: a 23-year-old man presented with complaints of pain and occasional clicking of the right knee for 6 years, the left knee was asymptomatic. Case 2: a 51-year-old woman had a 1-month history of pain and a feeling of "giving away" in both knees. Diagnoses: Based on physical examination and magnetic resonance imaging (MRI) findings, they were diagnosed with bilateral discoid medial meniscal tears. Interventions: Case 1: partial meniscectomy and reshaping were performed for the torn discoid medial meniscus of the right knee only. Case 2: partial medial meniscectomy and reshaping were performed for the torn discoid medial meniscus on both knees. Outcomes: The postoperative pain and the recovery of joint function were observed. The average follow-up time of 2 patients was 1 year. The Lysholm score of 2 patients was improved from 55 to 95 and 40 to 90 respectively. The pain of knee joint was relieved and the symptoms of disharmony were obviously improved. Lessons: We recommend preservation of the discoid medial meniscus in asymptomatic patients. Meniscectomy and reshaping for injured discoid medial meniscus produces promising short-term results.
Prognostic Factors of Mid- to Long-term Clinical Outcomes after Arthroscopic Partial Meniscectomy for Medial Meniscal Tears

Abstract:Backgroud: Arthroscopic partial meniscectomy (APM) continues to be the popular treatment for meniscal tears, but recent randomized controlled trials have questioned its efficacy. To provide more evidence-based criteria for patient selection, we undertook this study to identify prognostic factors associated with clinical failure after APM for medial meniscus tears.Methods: Medical records of 160 patients followed up for at least 5 years after APM for medial meniscal tears were retrospectively reviewed. Demographic data (age, sex, and body mass index), radiographic variables (Kellgren-Lawrence [K-L] grade and hip-knee-ankle [HKA] angle), and clinical scores (International Knee Documentation Committee score, Tegner activity scale score, Lysholm score, and Knee injury and Osteoarthritis Outcome Score) were recorded. Clinical failure was defined as the need for an additional surgical procedure (arthroscopy, osteotomy, or arthroplasty) or the presence of intolerable pain. Survivorship analysis with clinical failure as an end point was performed using Kaplan-Meier survival curves. Factors related to clinical failure were analyzed using a Cox proportional hazard model. Cutoff values were determined using areas under receiver operating characteristic (ROC) curves. Radiographic progression of osteoarthritis was analyzed using the chi-square test, and serial changes of clinical scores were analyzed using a linear mixed model.Results: Clinical success rates were 95.7% at 5 years, 75.6% at 10 years, and 46.3% at 15 years. Age, HKA angle, and K-L grade (p = 0.01, p = 0.02, and p = 0.04, respectively) were found to be significant risk factors of clinical failure. Cutoff values at 10 years postoperatively as determined by ROC analysis were 50 years for age (sensitivity = 0.778, 1-specificity = 0.589), grade 2 for K-L grade (sensitivity = 0.778, 1-specificity = 0.109), and 5.5° for HKA angle (sensitivity = 0.667, 1-specificity = 0.258). In patients who had clinical success until 10 years after APM, radiological osteoarthritis progressed gradually. However, the clinical scores of patients who achieved clinical success did not decrease significantly over the 10-year follow-up.Conclusions: The poor prognostic factors found to be related to clinical failure after APM for a medial meniscal tear were patient age (≥ 50 years), preoperative K-L grade (≥ grade 2), and preoperative HKA angle (≥ varus 5.5°).

Abstract:Background: Surgical treatment options of discoid lateral meniscus in pediatric patients consist of saucerization with or without meniscal repair, meniscocapular stabilization, and, less often, subtotal meniscectomy. Purpose: To describe a large, prospectively collected multicenter cohort of discoid menisci undergoing surgical intervention, and further investigate corresponding treatment of discoid menisci. Study Design: Cohort study; Level of evidence, 3. Methods: A multicenter quality improvement registry (16 institutions, 26 surgeons), Sports Cohort Outcomes Registry, was queried. Patient characteristics, discoid type, presence and type of intrasubstance meniscal tear, peripheral rim instability, repair technique, and partial meniscectomy/debridement beyond saucerization were reviewed. Discoid meniscus characteristics were compared between age groups ( 14 years old), based on receiver operating characteristic curve, and discoid morphology (complete and incomplete). Results: In total, 274 patients were identified (mean age, 12.4 years; range, 3-18 years), of whom 55.6% had complete discoid. Meniscal repairs were performed in 55.1% of patients. Overall, 48.5% of patients had rim instability and 36.8% had >1 location of peripheral rim instability. Of the patients, 21.5% underwent meniscal debridement beyond saucerization, with 8.4% undergoing a subtotal meniscectomy. Patients <14 years of age were more likely to have a complete discoid meniscus ( P < .001), peripheral rim instability ( P = .005), and longitudinal tears ( P = .015) and require a meniscal repair ( P < .001). Patients ≥14 years of age were more likely to have a radial/oblique tear ( P = .015) and require additional debridement beyond the physiologic rim ( P = .003). Overall, 70% of patients 50% in this young age group required peripheral stabilization/repair. Conclusion: To preserve physiological “normal” meniscus, a repair may be indicated in >50% of patients <14 years of age but occurred in 14 years. Additional resection beyond the physiological rim may be needed in 15% of younger patients and 30% of those aged >14 years.

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Discoid lateral meniscus (DLM) is the most common congenital abnormality of the meniscus. Tears are common; treatment is frequently not definitive, often requiring reoperation.Purpose:To report the clinical manifestations, physical characteristics, operative treatments and findings, complications, and reoperations of DLM in pediatric patients from multiple centers across North America.Study Design:Case series; Level of evidence, 4.Methods:Consecutive patients who underwent treatment for symptomatic DLM at 9 institutions between 2000 and 2020 were included. Patient data, presenting symptoms and signs, surgical findings, treatments rendered, and postoperative complications, including reoperation rates, were collected. Means with ranges and counts with proportions are reported for continuous and categorical variables, respectively, and comparisons were made using either the chi-square or Fisher exact test.Results:In total, 784 patients (867 knees) were included with a mean age at diagnosis of 12 years (range, 1-22 years) and a mean follow-up of 22.6 months (range, 0-154 months). Common preoperative symptoms were locking (33%) and snapping (30%). At surgery, tears in the DLM were present in 647 knees (594 patients [76%]); 95 knees (11%) had multiple tears; and in 140 knees, tears extended into >1 zone. Tears, when present, were more common within the posterior horn (41%) or body (34%) than the anterior horn (25%). Peripheral rim instability was reported in 241 knees (28%). Significantly more knees had instability posteriorly (15%; P = .0004) and anteriorly (9%; P = .0013) than along the body (3%). Tear type was most commonly complex (38%) or horizontal (34%). A total of 358 knees in 333 patients with tears (42% of all patients) underwent repair (55% of knees with tears). A total of 175 complications were reported, occurring in 139 knees in 134 patients (17%); 116 of these knees with complications (83%) had a single complication, while 23 (17%) had >1. Of the 784 patients, 105 (13%) underwent reoperation, undergoing 135 additional procedures related to their DLM. Of those, 60 (44%) were repeat arthroscopy and meniscal trim; 40 (30%), arthroscopy and meniscal repair; and 17 (13%), an articular cartilage procedure.Conclusion:Locking and snapping were common presenting symptoms. Over three-quarters of patients had meniscal tears, which were most often complex and located posteriorly. Seventeen percent of patients experienced complications, and a sixth of patients with complications had >1. Reoperation was typically for persistent symptoms or meniscal retear.

Abstract:Introduction: Although ultrasound can reportedly diagnose discoid lateral meniscus (DLM) in children, its widespread application is challenging because the diagnostic criteria are based on qualitative descriptions of DLM morphology rather than quantitative parameters. Additionally, no studies have applied ultrasound in classifying DLM. Therefore, this study aims to establish the quantitative ultrasound parameters that reflect DLM morphology, evaluate the feasibility and validity of these parameters for identifying DLM and their classification, and develop the quantitative ultrasound diagnostic criteria for DLM and their classification in children. Methods and analysis: Patients will be recruited from the outpatient clinics of the orthopaedics department at West China Hospital, Sichuan University, the Fourth Medical Center of the Chinese People's Liberation Army General Hospital, Xinhua Hospital affiliated with Shanghai Jiao Tong University School of Medicine, Yibin Hospital affiliated with West China Hospital Sichuan University, Suining Central Hospital and the Third Hospital of Mianyang from August 2022 to July 2024. Eligible patients are those aged ≤14 years, with knee symptoms such as pain, locking and limited extension, and who planned to undergo arthroscopic surgery. Exclusion criteria are patients with contraindications to ultrasound examination, such as severe skin damage or fracture around the knee. The sample size is estimated to be 576 cases with a power of 0.9 for hypothesis testing, a two-sided α of 0.05, and an expected sensitivity and specificity of 95%. Three days before surgery, ultrasound will be used to observe the morphology of the lateral meniscus and measure its width, angle α formed by the chord of upper and lower arc-shaped articular surface at the free edge, and the movement distance of the peripheral rim. Participants will be categorised according to the arthroscopy results for the DLM and its classification. The diagnostic performance of each parameter will be assessed and compared in terms of the area under the curve, sensitivity, specificity, and positive and negative predictive values. Ethics and dissemination: This study was approved by the Ethics Committee of West China Hospital, Sichuan University (approval no. 2022-923), and this approval covers all study hospitals. Written informed consent is required from all participants before enrolment in the study. The study's findings will be disseminated through peer-reviewed publications and conference presentations. Trial registration number: Chinese Clinical Trial Registry (ChiCTR2200062000).

Abstract:At present, there few studies on the kinematic features of lateral discoid meniscus injury. In this study, a motion capture system was used to investigate the motion characteristics of knees with lateral discoid meniscus after injury, and the differences between the knees with lateral meniscus and intact knees were compared. Fourteen patients diagnosed with unilateral lateral discoid meniscus injury, fourteen patients diagnosed with unilateral lateral meniscus injury, and fourteen normal subjects with healthy knees were recruited and grouped. Through kinematic gait analysis, it was found that the subjects in the two groups with meniscus injuries exhibited significantly smaller ranges of rotation and translation than those with healthy knees on the sagittal, coronal, and horizontal planes, but not in proximal-distal translation. Maximum lateral tibial translation and maximum internal tibial rotation in the knees with lateral discoid meniscus injury were significantly decreased compared to those with lateral meniscus injury. The results show that the kinematic features of knees with lateral discoid meniscus injury are statistically different than those of healthy knees and knees with lateral meniscus injury. This study provides an important reference for the dynamic function of knees with lateral discoid meniscus injury.
