Abstract:Bismuth-based agents have been widely used in medicine, particularly as the active components for the treatment of Helicobacter pylori-associated gastrointestinal ailments. The selective toxicity of bismuth toward Helicobacter pylori stems from the intrinsic chemical and biological properties of bismuth. The molecular targets of bismuth agents and their antimicrobial mechanisms of action are reviewed. Beyond anti-Helicobacter pylori actions, novel bismuth-based agents with broad antimicrobial activities against both Gram-positive and Gram-negative bacterial pathogens are highlighted. Intriguingly, bismuth-based combination therapy could effectively treat infections caused by antibiotic resistant superbugs. This chapter strives to offer an account of state-of-the-art research progress on antibacterial mechanisms of action of bismuth and explore the frontiers of up-to-date potential medicinal applications of bismuth- based agents with reference to recent developments in the literature.

Abstract:Although it has been used for centuries, bismuth remains one of the least understood elements in the periodic table. Metallic bismuth and bismuth compounds have been widely used in the manufacture of alloys, pigments, cosmetics, and pharmaceuticals. As a "green" heavy metal, the substitution of lead with bismuth in some industries may partially resolve the environmental problems related to heavy metal pollution. In health care, as bismuth has low toxicity to humans, bismuth-based drugs such as colloidal bismuth subcitrate (CBS), ranitidine bismuth citrate (RBC), bismuth subsalicylate (BSS), bismuth iodoform and radioactive bismuth (212Bi/213Bi) complexes have been developed and used in clinics to treat various diseases. In most cases, bismuth therapies exhibit high therapeutic efficacies and little side effects; nevertheless, there are still reported cases of bismuth toxicity caused by bismuth over-dosage.

Abstract:In recent years, the chemical potential of bismuth and bismuth compounds has been actively exploited. Bismuth salts are known for their low toxicity, making them potential valuable reagents for large-scale synthesis, which becomes more obvious when dealing with products such as active pharmaceutical ingredients or synthetic intermediates. Conversely, bismuth compounds have been widely used in medicine. After extensive use in the treatments of syphilis and other bacterial infections before the advent of modern antibiotics, bismuth compounds remain important for the treatment of several gastrointestinal disorders and also exhibit antimicrobial properties and cytotoxic activity, among others. This review updates relevant advances in the past few years, concerning the application of bismuth reagents and catalysts in innovative synthetic processes for the preparation of compounds of medicinal interest, as well as the preparation, biological evaluation and potential medicinal uses of bismuth compounds.

Anti-Helicobacter Pylori Infection Treatment and Pulmonary Hypersensitivity: Case Series and Review of the Literature

Abstract:BACKGROUND:Helicobacter pylori (H. pylori) infection is currently widespread throughout the world. Bismuth-containing quadruple therapy is widely used, but it has rarely been associated with interstitial lung disease. CASE PRESENTATION:We described six cases with similar clinical symptoms and typical pulmonary interstitial imaging changes during anti-H. pylori therapy, usually on Days 7-12 following treatment. Anti-H. pylori infection treatment was discontinued when it was suspected to be the cause of the clinical symptoms, and all of the patients accepted observation therapy. All of them had a favorable outcome, the clinical symptoms returned to normal almost 1 week later, and the chest computed tomography (CT) scan images showed remarkable absorption 4 weeks later. CONCLUSIONS:Drug interactions could be the cause, and the most likely drug was furazolidone. All of the patients recovered quickly after drug discontinuation, and low-dose steroid may help shorten the recovery time.

Abstract:Recent reviews described the efficient syntheses of metallic bismuth nanoparticles. Nevertheless, few studies have been published on the medical applications of these nanoparticles compared to the number of studies on the well-documented clinical use of the bismuth(III) complex. An analysis of the literature revealed the significant potential of metallic bismuth nanoparticles in different theranostic applications. In the diagnostic field, preclinical proofs of concept have been demonstrated for X-ray, photoacoustic and fluorescence imaging. In the therapeutic field, several preclinical studies have shown the potential of bismuth nanoparticles as X-ray radiosensitizers for use in radiotherapy and as photothermal agents for applications in near infrared phototherapy. The properties of these metallic bismuth nanoparticles as bactericidal, fungicidal, antiparasitic and antibiofilm agents have also been studied. Although information concerning the toxic effects of these nanoparticles has been collected, these data are insufficient when considering the immediate clinical use of these new nanoparticles.

Abstract:Bismuth-based drugs have been used primarily to treat ulcers caused by Helicobacter pylori and other gastrointestinal ailments. Combined with antibiotics, these drugs also possess synergistic activity, making them ideal for multiple therapy regimens and overcoming bacterial resistance. Compounds based on bismuth have a low cost, are safe for human use, and some of them are also effective against tumoral cells, leishmaniasis, fungi, and viruses. However, these compounds have limited bioavailability in physiological environments. As a result, there is a growing interest in developing new bismuth compounds and approaches to overcome this challenge. Considering the beneficial properties of bismuth and the importance of discovering new drugs, this review focused on the last decade's updates involving bismuth compounds, especially those with potent activity and low toxicity, desirable characteristics for developing new drugs. In addition, bismuth-based compounds with dual activity were also highlighted, as well as their modes of action and structure–activity relationship, among other relevant discoveries. In this way, we hope this review provides a fertile ground for rationalizing new bismuth-based drugs.

In this paper, we summarized the commonly used construction methods for Bi-based nanomaterials, and focused on their application in computed tomography (CT), photoacoustic imaging (PA), radiotherapy (RT), photothermal therapy (PTT) and synergy progress in applied research. Abstract:Bismuth compounds are considered relatively non-toxic, with their low solubility in aqueous solutions (e.g., biological fluids) being the major contributing factor to this property. Bismuth derivatives are widely used for the treatment of peptic ulcers, functional dyspepsia, and chronic gastritis. Moreover, the properties of bismuth compounds have also been extensively explored in two main fields of action: antimicrobial and anticancer. Despite the clinical interest of bismuth-based drugs, several side effects have also been reported. In fact, excessive acute ingestion of bismuth, or abuse for an extended period of time, can lead to toxicity. However, evidence has demonstrated that the discontinuation of these compounds usually reverses their toxic effects. Notwithstanding, the continuously growing use of bismuth products suggests that it is indeed part of our environment and our daily lives, which urges a more in-depth review and investigation into its possible undesired activities. Therefore, this review aims to update the pharmaco-toxicological properties of bismuth compounds. A special focus will be given to in vitro, in vivo, and clinical studies exploring their toxicity.

Abstract:Historically, mercury and mercury-containing preparations have been widely used in traditional Chinese medicine and applied in many clinical practices mainly in the form of mercury sulfides. The clinical application, toxicity manifestations, and symptoms of these preparations largely depend on the route of administration and the dosage form. Commonly used mercury-containing medicinal materials and preparations in traditional Chinese medicine include Cinnabar, an excellent medicine for tranquilizing the nerves; Hongsheng Dan and Baijiang Dan, which have antibacterial, anti-inflammatory, promotion of tissue repair and regeneration and other pharmacological effects. Tibetan medicine commonly uses Zaotai and Qishiwei Zhenzhu pills, which have pharmacological effects such as sedation, anti-inflammatory, anti-convulsant, and improvement of cerebral apoplexy. Menggen Wusu Shibawei pills, commonly used in Mongolian traditional medicine, have the muscle growth and astringent effects. In India and Europe, mercury is often used for treating syphilis. This article summarizes the history, clinical application, pharmacology, toxicology, and pharmacokinetics of mercury and mercury-containing preparations in traditional medicines. In terms of clinical application, it provides suggestions for the rational use and safety of mercury-containing drugs in clinical practices and in public health issues. It will further provide a reference for formulation strategies related to mercury risk assessment and management.

Abstract:Bismuth sulfide is one of the important compounds of bismuth that has garnered much attention due to its interesting properties and numerous applications. The structural orientation of bismuth sulfide and the characterization techniques have been highlighted. An in depth discussion were made on the various methods of synthesizing bismuth sulfide including chemical, sol–gel, deposition, pyrolysis, mechanical milling, microwave, microemulsion, Bridgman and successive ionic layer and reaction methods. Its medical applications, use in storage and generation of hydrogen, production of energy saving devices, as sensor for gas and other biomolecules and as photocatalyst for the removal of pollutants from the environment were presented in details. Finally, the future perspective for studies involving bismuth sulfide were suggested.

Abstract:Cancer is still the leading disease threatening human life and health at present. With the development of nanotechnology, multifunctional nanomaterials integrated with different diagnostic and therapeutic functions have become the most active field in nano-research. Among them, Bi2S3-based nanomaterials have attracted great attention in the biomedical field due to their special photothermal effect and biocompatibility. In this review, we systematically summarize the synthetic strategies and their physical-chemical properties of Bi2S3-based nanomaterials. Then we outline the recent research progress in the diagnosis and treatment of cancer, which are highlighted and discussed in detail. In addition, the safety and biodistribution of Bi2S3-based nanomaterials are emphasized and discussed. It is believed that the emerging Bi2S3-based nanomaterials may realize the practical applications via unremitting efforts of scientific researchers in the near future.

Abstract:Interest in the synthesis of Bi(III) and Sb(III) dithiocarbamate complexes is on the rise, and this has been attributed to their wide structural diversity and their interesting application as biological agents and in solid state/materials chemistry. The readily available binding sites of the two sulphur atoms within the dithiocarbamate moiety in the complexes confers a wide variety of geometry and interactions that often leads to supramolecular assemblies. Although none of the bismuth or antimony metals are known to play any natural biological function, their dithiocarbamate complexes, however, have proven very useful as antibacterial, antileishmanial, anticancer, and antifungal agents. The dithiocarbamate ligands modulate the associated toxicity of the metals, especially antimony, since bismuth is known to be benign, allowing the metal ion to get to the targeted sites; hence, making it less available for side and other damaging reactions. This review presents a concise chemistry and some known biological potentials of their trivalent dithiocarbamate complexes.

Role of bismuth in improving Helicobacter pylori eradication with triple therapy

Abstract:In most regions of the world, antimicrobial resistance has increased to the point where empirical standard triple therapy for Helicobacter pylori eradication is no longer recommended. The treatment outcome in a population is calculated as the sum of the treatment success in the subpopulation with susceptible infections plus treatment success in the subpopulation with resistant infections. The addition of bismuth (ie, 14-day triple therapy plus bismuth) can improve cure rates despite a high prevalence of antimicrobial resistance. The major bismuth effect is to add an additional 30%-40% to the success with resistant infections. The overall result is therefore dependent on the prevalence of resistance and the treatment success in the subpopulation with resistant infections (eg, with proton-pump inhibitor-amoxicillin dual therapy). Here, we explore the contribution of each component and the mechanisms of how bismuth might enhance the effectiveness of triple therapy. We also discuss the limitations of this approach and provide suggestions how triple therapy plus bismuth might be further improved.

Abstract:Metallodrugs have been widely used as diagnostic and therapeutic agents. Understanding their mechanisms of action may lead to advances in rational drug design. However, to achieve this, diversified approaches are required because of the complexity of metal-biomolecule interactions. Bismuth drugs in combination with antibiotics as a quadruple therapy show excellent success rates in the eradication of Helicobacter pylori, even for antibiotic-resistant strains, and in fact, they have been used in the clinic for decades for the treatment of infection. Understanding the mechanism of action of bismuth drugs may extend their medicinal application beyond the treatment of H. pylori infection. This Account describes several general strategies for mechanistic studies of metallodrugs, including system pharmacology and metalloproteomics approaches. The application of these approaches is exemplified using bismuth drugs. Through a system pharmacology approach, we showed that glutathione- and multidrug-resistance-associated protein 1-mediated self-propelled disposal of bismuth in human cells might explain the selective toxicity of bismuth drugs to H. pylon but not the human host. The development of metalloproteomics has enabled extensive studies of the putative protein targets of metallodrugs with a dynamic range of affinity. Continuous-flow GE-ICP-MS allows simultaneous monitoring of metals and their associated proteins with relatively high affinity on a proteome-wide scale. The fluorescence approach relies on unique Mn+--NTA-based fluorescence probes and is particularly applicable for mining those proteins that bind to metals/metallodrugs weakly or transiently. Integration of these methods with quantitative proteomics makes it possible to maximum coverage of bismuth-associated proteins, and the sustained efficacy of bismuth drugs lies in their ability to disrupt multiple biological pathways through binding and functional perturbation of key enzymes. The knowledge acquired by mechanistic studies of bismuth drugs led to the discovery of UreG as a new target for the development of urease inhibitors. The ability of Bi(III) to inhibit metallo-beta-lactamase (MBL) activity through displacement of the Zn(II) cofactor renders bismuth drugs new potential as broad-spectrum inhibitors of MBLs. Therefore, bismuth drugs could be repurposed together with clinically used antibiotics as a cotherapy to cope with the current antimicrobial resistance crisis. We anticipate that the methodologies described in this Account are generally applicable for understanding the (patho)physiological roles of metals/metallodrugs. Our mechanism-guided discovery of new druggable targets as well as new medicinal applications of bismuth drugs will inspire researchers in relevant fields to engage in the rational design of drugs and reuse of existing drugs, eventually leading to the development of new effective therapeutics.
Safety and clinical use of microecological preparations

Abstract:Microecology is a branch of vital science for research in the relationship of microorganisms with the hosts. Recently, microecological preparations are emerging in large numbers with the advance of microecology. The microecological preparations can regulate microecosystem dysbiosis, maintain microecosystem balance, and confer a health benefit on the host. The microecological preparations can be classified into three types: probiotics (live microorganisms), prebiotics (nondigestible food ingredient), and synbiotics (a mixture of probiotics and prebiotics). The indications for microecological preparations are diarrhea (including diarrhea in children, antibiotic-associated diarrhea, and traveller's diarrhea), irritable bowel syndrome, inflamematory bowel disease, Helicobacter pylori infections, constipation, cirrhosis, allergy, and female genital urinary tract infections. The microecological preparations are relatively safe. However, the microecological preparations containing live microorganisms should be used in caution in patients with hypoimmunity or impaired intestinal mucosal barrier in order to avoid opportunistic infections occurring. General precautions for the use of microecological preparations are as follows: the tablet/capsule should be swallowed with warm water (at a temperature under 40℃); bismuth preparations, tannic acid, activated charcoal, tincture, and certain antibiotics in combination with microecological preparations comprising live microorganisms should be avoided; the microecological preparations should be preserved in light-protected containers and kept in cool place.

Abstract:Bacterial infections pose a seriously threat to the safety of the environment and human health. In particular, the emergence of drug-resistant pathogens as a result of antibiotic abuse and high trauma risk has rendered conventional therapeutic techniques insufficient for treating infections by these so-called "superbugs". Therefore, there is an urgent need to develop highly efficient and environmentally-friendly antimicrobial agents. Bismuth-based nanomaterials with unique structures and physicochemical characteristics have attracted considerable attention as promising antimicrobial candidates, with many demonstratingoutstanding antibacterial effects upon being triggered by broad-spectrum light. These nanomaterials have also exhibited satisfactory energy band gaps and electronic density distribution with improved photonic properties for extensive and comprehensive applications after being modified through various engineering methods. This review summarizes the latest research progress made on bismuth-based nanomaterials with different morphologies, structures and compositions as well as the different methods used for their synthesis to meet their rapidly increasing demand, especially for antibacterial applications. Moreover, the future prospects and challenges regarding the application of these nanomaterials are discussed. The aim of this review is to stimulate interest in the development and experimental transformation of novel bismuth-based nanomaterials to expand the arsenal of effective antimicrobials.
