Abstract:Liver was one of the most common distant metastatic sites in breast cancer. Patients with distant metastasis were identified as American Joint Committee on Cancer (AJCC) stage IV indicating poor prognosis. However, few studies have predicted the survival in females with T1-2N0-1 breast cancer who developed liver metastasis. This study aimed to explore the clinical features of these patients and establish a nomogram to predict their overall survival. 1923 patients were randomly divided into training (n = 1154) and validation (n = 769) cohorts. Univariate and multivariate analysis showed that age, marital status, race, estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor-2 (HER2), chemotherapy, surgery and bone metastasis, brain metastasis were considered the independent prognostic indicators. Clinical Characteristics and Survival Analysis of Breast Cancer Molecular Subtypes with Hepatic Metastases

Abstract:BACKGROUND: The liver is one of the most common metastatic sites of breast cancer, hepatic metastases developing in 6%-25% of patients with breast cancer and being associated with a poor prognosis. The aim of this study was to analyze the survival and clinical characteristics of patients with hepatic metastases from breast cancer of different molecular subtypes and to investigate the prognostic and predictive factors that effect clinical outcome.METHODS: We retrospectively studied the charts of 104 patients with breast cancer hepatic metastases diagnosed at Sun Yat-sen University Cancer Center from December 1990 to June 2009. Subtypes were defined as luminal A, luminal B, human epidermal growth factor receptor 2 (HER2) enriched, triple-negative (TN). Prognostic factor correlations with clinical features and treatment approaches were assessed at the diagnosis of hepatic metastases.RESULTS: The median survival time was 16.0 months, and the one-, two- three-, four-, five- year survival rates were 63.5%, 31.7%, 15.6%, 10.8%, and 5.4%, respectively. Median survival periods after hepatic metastases were 19.3 months (luminal A), 13.3 months (luminal B), 18.9 months (HER2-enriched), and 16.1 months (TN, P=0.11). In multivariate analysis, a 2 year-interval from initial diagnosis to hepatic metastasis, treatment with endocrine therapy, and surgery were independent prognostic factors. Endocrine therapy could improve the survival of luminal subtypes (P=0.004) and was a favorable prognostic factor (median survival 23.4 months vs. 13.8 months, respectively, P=0.011). Luminal A group of patients treated with endocrine therapy did significantly better than the Luminal A group of patients treated without endocrine therapy (median survival of 48.9 vs. 13.8 months, P=0.003).CONCLUSIONS: Breast cancer subtypes were not associated with survival after hepatic metastases. Endocrine therapy was a significantly favorable treatment for patients with luminal subtype.

An Analysis of Clinical Features and Survival for Brain Metastases in Different Subtypes of Breast Cancer

Abstract:Objective To analyze the clinical characteristics and prognosis of patients with breast cancer brain metastases in different subtypes.Methods Retrospective clinical and survival analyses were performed in 90 breast cancer patients with brain metastases.Patients were divided into estrogen receptor(ER) + and/or progesterone receptor(PR) +,human epidermal growth receptor-2(HER-2) +,and triple negative(ER-,PR-and HER-2-) subtypes.Results In the 90 patients,there were 51 patients(56.7%) with ER+ and/or PR+12 patients(13.3%) with HER-2+ and 27 patients(30%) with triple negative breast cancer.The median DFS(disease free survival) was 30.9,25.4 and 16.0 months(P=0.001)among ER+ and(or)PR+,HER-2+ and triple negative breast cancer.The median brain disease free survival was 36.0,38.0 and 22.0 months(P=0.008)among the above three types.The median OS(overall survival) was 52.0,25.5 and 36.6 months(P=0.075)among the three types.The median survival from brain metastasis was 11.0,5.5 and 8.0 months(P=0.829) among the three types.Conclusion The triple negative subtype was more prone to metastasize to brain with a shorter median disease free survival compared with other two subtypes.The median overall survival and survival from brain metastasis were similar among three subtypes.
Pattern of Distant Extrahepatic Metastases in Primary Liver Cancer: a SEER Based Study.

Abstract:Background and Aims: Primary liver cancer remains still the common cause of cancer-related deaths globally and the prognosis for patients with extrahepatic metastasis is poor. The aim of our study was to assess extrahepatic metastatic pattern of different histological subtypes and evaluate prognostic effects of extrahepatic metastasis in patients with advanced disease. Methods: Based on the Surveillance, Epidemiology and End Results (SEER) database, eligible patients diagnosed with primary liver cancer was identified between 2010 to 2012. We adopted Chi-square test to compared metastasis distribution among different histological types. We compared survival difference of patients with different extrahepatic metastasises by Kaplan-Meier analysis. Cox proportional hazard models were performed to identify other prognostic factors of overall survival. Results: We finally identified 8677 patients who were diagnosed with primary liver cancer from 2010 to 2012 and 1775 patients were in distant metastasis stages. Intrahepatic cholangiocarcinoma was more invasive and had a higher percentage of metastasis compared with hepatocellular carcinoma. Lung was the most common metastasis and brain was the least common site for both hepatocellular carcinoma and intrahepatic cholangiocarcinoma. Extrahepatic metastasis could consider as an independent prognostic factor for patients with liver cancer. Patients with brain metastasis had the worst prognosis, compared with other metastasis in overall survival (OS) and cancer-specific survival (CSS) analysis. Conclusions: Different histological subtypes of liver cancer had different metastasis patterns. There were profound differences in risk of mortality among distant extrahepatic metastatic sites. Results from our studies would provide some information for follow-up strategies and future studies.
Risk and prognostic factors of breast cancer with liver metastases

Abstract:Abstract Background Liver metastasis is a significant adverse predictor of overall survival (OS) among breast cancer patients. The purpose of this study was to determine the risk and prognostic factors of breast cancer with liver metastases (BCLM). Methods Data on 311,573 breast cancer patients from the Surveillance, Epidemiology, and End Results (SEER) database and 1728 BCLM patients from Fudan University Shanghai Cancer Center (FUSCC) were included. Logistic regression was used to identify risk factors for liver metastasis. Cox proportional hazards regression model was adopted to determine independent prognostic factors in BCLM patients. Results Young age, invasive ductal carcinoma, higher pathological grade, and subtype of triple-negative and human epidermal growth factor receptor 2 positive (HER2+) were risk factors for developing liver metastasis. The median OS after liver metastasis was 20.0 months in the SEER database and 27.3 months in the FUSCC dataset. Molecular subtypes also played a critical role in the survival of BCLM patients. We observed that hormone receptor-positive (HR+)/HER2+ patients had the longest median OS (38.0 for SEER vs. 34.0 months for FUSCC), whereas triple-negative breast cancer had the shortest OS (9.0 vs. 15.6 months) in both SEER and FUSCC. According to the results from the FUSCC, the subtype of HR+/HER2+ (hazard ratio (HR) = 2.62; 95% confidence interval (CI) = 1.88–3.66; P < 0.001) and HR−/HER2+ (HR = 3.43; 95% CI = 2.28–5.15; P < 0.001) were associated with a significantly increased death risk in comparison with HR+/HER2- patients if these patients did not receive HER2-targeted therapy. For those who underwent HER2-targeted therapy, however, HR+/HER2+ subtype reduced death risk compared with HR+/HER2- subtype (HR = 0.74; 95% CI = 0.58–0.95; P < 0.001). Conclusions Breast cancer patients at a high risk for developing liver metastasis deserve more attention during the follow-up. BCLM patients with HR+/HER2+ subtype displayed the longest median survival than HR+/HER2- and triple-negative patients due to the introduction of HER2-targeted therapy and therefore it should be recommended for HER2+ BCLM patients.

Stability and variability of molecular subtypes: comparative analysis of primary and metastatic triple-negative breast cancer

Abstract:Objective: Triple-negative breast cancer (TNBC) is a heterogeneous and aggressive cancer. Although our previous study classified primary TNBC into four subtypes, comprehensive longitudinal investigations are lacking. Methods: We assembled a large-scale, real-world cohort comprised of 880 TNBC patients [465 early-stage TNBC (eTNBC) and 415 metastatic TNBC (mTNBC) patients] who were treated at Fudan University Shanghai Cancer Center. The longitudinal dynamics of TNBC subtypes during disease progression were elucidated in this patient cohort. Comprehensive analysis was performed to compare primary and metastatic lesions within specific TNBC subtypes. Results: The recurrence and metastasis rates within 3 years after initial diagnosis in the eTNBC cohort were 10.1% (47/465). The median overall survival (OS) in the mTNBC cohort was 27.2 months [95% confidence interval (CI), 24.4-30.2 months], which indicated a poor prognosis. The prognostic significance of the original molecular subtypes in both eTNBC and mTNBC patients was confirmed. Consistent molecular subtypes were maintained in 77.5% of the patients throughout disease progression with the mesenchymal-like (MES) subtype demonstrating a tendency for subtype transition and brain metastasis. Additionally, a precision treatment strategy based on the metastatic MES subtype of target lesions resulted in improved progression-free survival in the FUTURE trial. Conclusions: Our longitudinal study comprehensively revealed the clinical characteristics and survival of patients with the original TNBC subtypes and validated the consistency of most molecular subtypes throughout disease progression. However, we emphasize the major importance of repeat pathologic confirmation of the MES subtype.
A Prognostic Model for Breast Cancer With Liver Metastasis

Abstract:Background: Breast cancer with liver metastasis consists of a group of heterogeneous diseases, and survival time may be significantly different, ranging from a few months to several years. The present study aimed to develop and externally validate a prognostic model for breast cancer with liver metastasis (BCLM).Methods: In total, 1022 eligible patients from January 2007 to December 2018 were selected from Fudan University Shanghai Cancer Center (FUSCC) and were temporally in the training (n = 715) and validation (n = 307) set. According to regression coefficients found in the multivariate Cox regression analysis, the final results were transformed into the prognostic scores. On the basis of these scores, patients were finally classified into three risk groups, including low-, intermediate-, and high-risk groups. Bootstrapping was used for internal validation. Then, time-dependent receiver operating characteristic (ROC) curves and calibration plots were used to assess discrimination and calibration of this prognostic model in the validation set.Results: Molecular subtypes, metastatic-free interval (MFI), extrahepatic metastasis, and liver function tests were identified as independent prognostic factors in the multivariate analysis. According to risk stratification, intermediate-risk (hazard ratio (HR) 2.12, 95% confidence interval (CI) 1.74–2.58, P < 0.001) and high-risk groups (HR 6.94, 95% CI 5.25–9.16, P < 0.001) had significantly worse prognoses in comparison with the low-risk group regarding overall survival (OS) from the time of metastasis. The median OS in these three groups were 39.97, 21.03, and 8.80 months, respectively. These results were confirmed in the internal and external validation cohorts.Conclusions: Based on molecular classification of tumors, routine laboratory tests, and other clinical information easily accessible in daily clinical practice, we developed a clinical tool for BCLM patients to predict their prognosis. Moreover, it may be useful for identifying the subgroup with unfavorable prognosis and individualization of treatment.

Management and prognosis of liver metastasis in breast cancer patients：a report of 163 cases

Abstract:Objective To assess the clinical characteristics , treatment and prognostic factors of liver metastasis in breast cancer patients. Methods One-hundred and sixty-three breast cancer patients with liver metastasis were included in this study from January 2006 to January 2016. The clinical, pathological and treatment information was collected , and the factors related to the survival were evaluated. All statistical analyses were performed using R software. Results The median overall survival of breast cancer with liver metastases was 29.5 (95%CI: 27.8-31.7) months, the median overall survival Group A (initial Stage Ⅳ patients with liver metastases) was 22.1 (95%CI: 17.8-25.1) months, while the median overall survival Group B (relapse with liver metastases) was 34.1 (95%CI: 30.6-37.6) months (P=1.83 ×10-10). If the patients had opportunity to perform local therapy, the prognosis became much better: the median overall survival extended from to 26.4 months to 33.0 months (P=9.95 ×10-3); 1 years, 2 years, 3 years survival rate in Group B are also significantly higher than Group A (Group A: 79.44%, 40.54%, 9.20%; Group B: 98.94%, 84.92%, 40.54%).In univariate analysis, the factors of hormone receptor status, Her-2 expression, Ki67 proliferation, molecular classification, whether multiple liver metastases were limited, and localtherapy opportunity were associated with the prognosis of breast cancer patients with liver metastasis . (P<0.05). In multivariate analysis, the factors of high Her-2 expression, multiple liver metastases were not limited, and no opportunity to do local therapy were independent predictors of poor survival after lung metastases (P<0.05). Conclusion The prognosis of breast cancer patients with liver metastasis is poor. We should pay more attention to the liver screening in order to early detection and early treatment. The preferred treatment is systemic comprehensive treatment. For patients with isolated single liver metastases and good response to systemic treatment , local therapy can obtain good effect.
De novo metastatic breast cancer: Subgroup analysis of molecular subtypes and prognosis

Abstract:The purpose of the present study was to analyze the clinical and pathological characteristics, treatment, and prognosis of de novo metastatic breast cancer (DnMBC). Information regarding 1,890 patients treated for advanced breast cancer at the Tianjin Medical University Cancer Hospital between January 2008 to December 2017 was collected. Clinicopathological characteristics, treatments and outcomes of these patients were compared using the chi-square test, log-rank test, and Cox regression analysis. A total of 171 patients were diagnosed with DnMBC. The median age at diagnosis was 53 years (range, 23-77). The percentage of T4 staging was higher (37.4%), 69.6% of patients were estrogen receptor (ER) positive, 59.1% were progesterone receptor positive, 29.8% had positive human epidermal growth factor receptor 2 (HER2) status, 68.4% had Ki-67 ≥20%, 55% had oligometastasis at the initial diagnosis, ~87.7% were treated with chemotherapy initially and 24% received palliative surgery for the primary tumor. After a median follow-up time of 26 months, the median progression-free survival (PFS) and overall survival (OS) among patients with DnMBC were 11 (8.7-13.3) months and 34 (27.9-40.1) months, respectively. In the multivariable model, ER status and sites of first metastasis (oligometastasis or polymetastasis) were identified to be independent predictors of PFS (P<0.05); ER status, primary tumor stage, and surgical treatment of primary tumors were identified to be independent predictors of OS (P<0.05). In conclusion, the clinicopathological characteristics of DnMBC are greater invasiveness and a higher risk of progression. Palliative surgical treatment may improve the prognosis of HR+/HER2-patients with oligometastasis. Therefore, individualized treatment as required is particularly important.
Unexpected Features of Breast Cancer Subtype

Abstract:BACKGROUND:Gene expression profiling of breast cancers identifies distinct molecular subtypes that affect prognosis. The aim of this study was to determine whether features of tumors especially the risks of lymph node (LN) metastases differ among molecular subtypes.METHODS:Subtypes were classified by immunohistochemical surrogates as luminal A, luminalHer2-, luminalHer2+, TNBC, and HER-2+. Data were obtained from an established, registered database of patients with invasive breast cancer treated at our hospital between July 2012 and October 2014. A total of 929 tumors were classifiable into molecular subtypes.RESULTS:The distribution of subtypes was luminal A (24.2%), luminalHer2- (27.8%), luminalHer2+ (9.1%), TNBC (21.3%), and HER-2+ (17.5%). Marked differences in age, tumor size, extent of lymph node involvement, and grade were observed among subtypes. On univariate analysis, the LN positivity varied across subtypes with 33.6% in luminal A, 40.3% in luminalHer2-, 37.3% in luminalHer2+, 37.6% in TNBC, and 47.4 % in HER-2+ (p=0.201). There was no significant difference in LN positivity among subtypes. On multivariable analysis, grade and tumor size were independent predictors of LN positivity.CONCLUSIONS:Predictors of LN metastases include higher grade and larger tumor size. Even though breast cancer subtype is not a statistically significant predictor of LN positivity, this information may still be useful in selecting the appropriate therapy in clinical practice.
Prognostic Value of Long Non-Coding RNAs in Triple Negative Breast Cancer

Abstract:Abstract Background Triple-negative breast cancer (TNBC) is the most aggressive and lethal subtype of breast cancer. Accumulating evidence showed long non-coding RNAs (lncRNAs) are abnormally expressed in TNBC and could be valuable prognostic tools for TNBC patients. This study aims to research the prognostic value of lncRNAs in TNBC, using the meta-analysis method. Methods We performed a detailed literature search on Pubmed, Scopus, and Web of Science for studies on the prognostic value of lncRNAs in TNBC. The meta-analysis method was used to determine the relationship between lncRNAs expression and survival of TNBC patients. Results A total of 2803 TNBC patients and 24 lncRNAs from 27 different articles were included in the present study. Subgroup analysis demonstrated that overexpression of lncRNAs in a group that is upregulated in TBNC showed a significant association with poor overall survival (HR = 1.86, 95%CI = 1.45–2.27, I2 = 41.9%) and disease-free survival (HR = 1.85, 95%CI = 1.37–2.33, I2 = 0%). Conversely, overexpression of lncRNAs in a downregulation group was markedly related to good overall survival (HR = 0.60, 95%CI = 0.43–0.77, I2 = 28.6%). Moreover, expression of lncRNA SNHG12, MALAT1, HOTAIR, HIF1A-AS2, HULC, LINC00096, ZEB2-AS1, LUCAT1, and LINC000173 showed a marked correlation with positive lymph node metastasis (LNM), while lncRNA MIR503HG, GAS5, TCONS_l2_00002973 showed the opposite effect. High expression level of MALAT1, HIF1A-AS2, HULC, LINC00096, ADPGK-AS1, ZEB2-AS1, LUCAT1 were positively correlated with distant metastasis (DM), while lncRNA MIR503HG showed the opposite effect. In addition, the mechanisms of lncRNAs in TNBC were summarized. Conclusions This meta-analysis demonstrated that abnormally expressed lncRNA were significantly associated with the survival of TNBC patients and may serve as biomarkers and therapeutic targets for TNBC prognosis.

Abstract:An imbalance of intracellular iron metabolism can lead to the occurrence of ferroptosis. Ferroptosis can be a factor in the remodeling of the immune microenvironment and can affect the efficacy of cancer immunotherapy. How to combine ferroptosis-promoting modalities with immunotherapy to suppress triple-negative breast cancer (TNBC) has become an issue of great interest in cancer therapy. However, potential biomarkers related to iron metabolism and immune regulation in TNBC remain poorly understand.

Prognostic Value of Tumor-Infiltrating Lymphocyte Subtypes in Residual Tumors of Patients with Triple-Negative Breast Cancer after Neoadjuvant Chemotherapy.

Abstract:Background::After neoadjuvant chemotherapy (NAC), non-pathological complete response of breast cancer patients can benefit from tailored adjuvant chemotherapy. However, it is difficult to select patients with poorer prognosis for additional adjuvant chemotherapy to maximize the benefits. Our study aimed to explore whether the subtypes of tumor-infiltrating lymphocytes (TILs) in residual tumors (RT) is related to the prognosis of triple-negative breast cancer (TNBC) after NAC.Methods::Data from patients with primary TNBC consecutively diagnosed at the Breast Disease Center of Peking University First Hospital from 2008 to 2014 were retrieved, and the cases with RT in the breast after NAC were enrolled. TILs subtypes in RT were observed by double-staining immunohistochemistry, and counted with the median TILs value per square millimeter as the cut-off to define high versus low TILs density in each subtype. The relationships between the TIL density of each subgroup and the clinicopathological characteristics of the RT after NAC patients were analyzed by Fisher exact test. Disease-free survival (DFS) and overall survival (OS) were analyzed by the Kaplan-Meier method and log-rank statistics.Results::A total of 37 eligible patients were included in this study, and the median follow-up period was 50 months (range 17–106 months). There was no significant correlation between the infiltrate density of CD4 +, CD8 +, CD20 +, and CD68 + lymphocytes and clinic-pathological characteristics. Significantly better prognosis was observed in patients with high CD4 +-TILs (DFS: P = 0.005, OS: P = 0.021) and high CD8 +-TILs (DFS: P = 0.018) and low CD20 +-TILs (OS: P = 0.042). Further analysis showed that patients with CD4 +/CD20 + ratio greater than 1 (DFS: P = 0.001, OS: P = 0.002) or CD8 +/CD20 + ratio greater than 1 (DFS: P = 0.009, OS: P = 0.022) had a better prognosis. Conclusions::Subtypes of TILs in RT is a potential predictive biomarker of survival in TNBC patients after NAC.

Abstract:We investigated whether molecular subtype can predict the response and prognosis in Chinese locally advanced breast cancer (LABC) patients treated with preoperative therapy. LABC patients treated with preoperative therapy in Cancer Hospital, Fudan University between August 2001 and May 2008 were retrospectively analyzed. Molecular subtypes were constructed from the immunohistochemical results of hormonal receptors (HR) and HER2 status, which were classified as luminal (HR+/HER2-), triple negative (HR-/HER2-) and HER2 positive subtypes. Preoperative tumor parameters, chemotherapy regimens and response as well as outcome were compared among these subtypes. A total of 225 cases were included into analysis. Univariate and multivariate analysis showed that the pathological complete remission (pCR) independent predictive factors were molecular subtype and preoperative regimens. Compared with luminal subtype, patients with HER2 positive or triple negative tumor had significantly higher pCR rate, with odds ratio 3.02 (95% CI=1.07-8.07; P=0.037) and 3.10 (95% CI=1.01-9.52; P=0.048), respectively. However, HER2 positive or triple negative breast cancer patients were also associated with increased recurrence (P=0.072) and death rates (P=0.019) compared with luminal subtype in the whole population, and was especially worse in patients with residual disease after preoperative therapy with decreased disease-free survival (P=0.022) and overall survival (P=0.007). Our results show that molecular subtype can predict the response and prognosis of Chinese LABC patients treated with preoperative therapy. Compared with luminal subtype, patients with HER2 positive or triple negative disease had increased pCR rates, but associated with significantly worse survival, especially in those with residual disease after preoperative therapy.

Abstract:Triple-negative breast cancer (TNBC) has high heterogeneity, poor prognosis, and limited treatment success. Recently, an immunohistochemistry-based surrogate classification for the "Fudan University Shanghai Cancer Center (FUSCC) subtyping" has been developed and is considered more suitable for clinical application. Seventy-one paraffin-embedded sections of surgically resected TNBC were classified into four molecular subtypes using the IHC-based surrogate classification. Genomic analysis was performed by targeted next-generation sequencing and the specificity of the subtypes was explored by bioinformatics, including survival analysis, multivariate Cox regression, pathway enrichment, Pyclone analysis, mutational signature analysis and PHIAL analysis. AKT1 and BRCA1 mutations were identified as independent prognostic factors in TNBC. TNBC molecular subtypes encompass distinct genomic landscapes that show specific heterogeneities. The luminal androgen receptor (LAR) subtype was associated with mutations in PIK3CA and PI3K pathways, which are potentially sensitive to PI3K pathway inhibitors. The basal-like immune-suppressed (BLIS) subtype was characterized by high genomic instability and the specific possession of signature 19 while patients in the immunomodulatory (IM) subtype belonged to the PD-L1 ≥ 1% subgroup with enrichment in Notch signaling, suggesting a possible benefit of immune checkpoint inhibitors and Notch inhibitors. Moreover, mesenchymal-like (MES) tumors displayed enrichment in the receptor tyrosine kinase (RTK)-RAS pathway and potential sensitivity to RTK pathway inhibitors. The findings suggest potential treatment targets and prognostic factors, indicating the possibility of TNBC stratified therapy in the future.

Abstract:Purpose To evaluate the efficiency of combined treatment of transcatheter arterial chemoembolization (TACE) and systemic chemotherapy (SC) for liver-only metastases from breast cancer after mastectomy. Methods We compared the outcomes of 44 patients who underwent combined treatment of TACE and systemic chemotherapy (TSC) with those of 43 patients who underwent systemic chemotherapy (SC). Results The median follow-up from the diagnosis of liver metastases was 29 months (range, 0-89 months). Response rates were 59.1% and 34.9% for TSC group and SC group ( P < 0.05), respectively. The 1-, 2- and 3-year survival rates for TSC group were 76.2, 66.7 and 47.6%, and those for SC group were 48.1, 29.6 and 7.4% ( P = 0.027), respectively. Estrogen receptor (ER)-negative status of primary tumor, disease-free interval from mastectomy to liver metastases (DFI) less than 24 months and patients who received systemic chemotherapy only were independently associated with poor prognosis ( P = 0.009; P = 0.023; P = 0.030). Conclusions The combined treatment of TACE and systemic chemotherapy may prolong survival for liver metastases in breast cancer after mastectomy.
Immune Microenvironment-Related Genes Contribute to Clinical Prognosis in Patients with Triple-Negative Breast Cancer

Abstract:Abstract Background:Triple-negative breast cancer (TNBC) is a high-risk breast cancer subtype, which accounts for 15% to 20% of all breast cancers and generally has a poor prognosis. TNBC patients have high recurrence post-surgery and high risk of metastasis to other organs. The tumor microenvironment (TME) plays important roles in the carcinogenesis, development, and metastasis of tumors. Methods: This study aimed to investigate the effects of immune and stromal cell-related genes on TNBC prognosis. The ESTIMATE algorithm was used to calculate the immune/stromal scores of TNBC samples from the GEO database. The samples were divided into high- and low- score groups and the differential expression genes were identified using the limma package within R. Functional enrichment and protein-protein interaction (PPI) network analyses revealed that these genes are primarily involved in immune responses. Results:Survival analysis in both GSE21653 and KM-plotter website showed that 36 genes were significantly related to disease-free survival (DFS) of TNBC. Another survival analysis by R package survival in GSE58812 indicated that 14 genes of 36 were greatly interrelated to DFS of TNBC. Moreover, the expression levels of some of these genes were verified through immunohistochemical staining and RT-qPCR. Finally, four genes importantly associated with TNBC prognosis were identified with Cox-LASSO analysis. Time-dependent receiver-operating characteristic (ROC) analysis displayed an area under the curve (AUC) of 0.95 for one-year survival rate, indicating the four genes performed very well for prognosis prediction. Conclusions:In conclusion, we identified four genes, including BIRC3, CD8A, GNLY and TRIM22, that are possibly associated with the TME and are potential prognostic and therapeutic markers of TNBC.

Abstract:Objective To analyze the clinical characteristics,survival,and prognosis of the patients with different molecular subtypes of breast cancer.Methods Clinical data of 304 patients with histopathologically confirmed resectable breast cancer,treated in the 1st Affiliated Hospital of Kunming Medical University from Feb.2002 to Feb.2004,were collected and analyzed in this study.ER,PR and HER-2 in all the excised tissues were determined by immunohistochemistry,and according to the results,the patients were divided into luminal,HER-2 and triple-negative breast cancer.The clinical characteristics,recurrence and survival of the patients with different subtypes of breast cancer were analyzed.Results In this study,luminal,HER-2 and triple-negative breast cancer accounted for 53.0%,20.4% and 26.6%,and the median age of patients with the three subtypes was 50.8,50.3and 47.1,respectively.Histological grade of luminal type was lower than other subgroup of breast cancer.The percentage of luminal type with histological grade I was 43.1%.HER-2 type was commonly seen in premenopausal women(66.1%),its histological grade was between luminal and triple-negative type.Triple-negative breast cancer was more likely to be found in younger patients frequently with a familial history of breast cancer(9.9%).Histological grade of triple-negative breast cancer was higher than other groups.The percentage of triple-negative patients with histological grade III was 49.0%.Among the three type patients,there was no significant difference in menopausal status,tumor stage,pathologic types and lymph node metastasis.By Feb.2009,the median time of follow-up was 71 months(60～84 months).Patients with HER-2 or triple-negative breast cancer were found to have a significantly higher recurrence and metastasis rate than the patients with luminal type breast cancer,but no significant difference was observed between triple-negative and HER-2 types.HER-2 type patients were more likely to develop bone metastases than triple-negative patients(46.7% vs 13.3%,P0.05),and triple-negative patients were more likely to develop lung metastases than HER-2 patients(46.7% vs 20.3%,P0.05).Up to February 2009,the 5-year disease-free rates(DFS)for patients with Luminal,HER-2 and triple-negative types were 90.1%,75.8% and 76.5%(P0.05),respectively.The overall 5-year survival rates(OS)of the three groups were 96.9%,85.5% and 87.7%(P0.05),respectively.Conclusion Patients with different subtype of breast cancer have different clinical characteristics,recurrence pattern and prognosis.
Discovery of an independent poor-prognosis subtype associated with tertiary lymphoid structures in breast cancer

Abstract:Introduction: Tertiary lymphoid structures (TLSs) are ectopic lymphoid formations that arise in non-lymphoid tissues due to chronic inflammation. The pivotal function of TLSs in regulating tumor invasion and metastasis has been established across several cancers, such as lung cancer, liver cancer, and melanoma, with a positive correlation between increased TLS presence and improved prognosis. Nevertheless, the current research about the clinical significance of TLSs in breast cancer remains limited. Methods: In our investigation, we discovered TLS-critical genes that may impact the prognosis of breast cancer patients, and categorized breast cancer into three distinct subtypes based on critical gene expression profiles, each exhibiting substantial differences in prognosis (p = 0.0046, log-rank test), with Cluster 1 having the best prognosis, followed by Cluster 2, and Cluster 3 having the worst prognosis. We explored the impact of the heterogeneity of these subtypes on patient prognosis, the differences in the molecular mechanism, and their responses to drug therapy and immunotherapy. In addition, we designed a machine learning-based classification model, unveiling highly consistent prognostic distinctions in several externally independent cohorts. Results: A notable marker gene CXCL13 was identified in Cluster 3, potentially pivotal in enhancing patient prognosis. At the single-cell resolution, we delved into the adverse prognosis of Cluster 3, observing an enhanced interaction between fibroblasts, myeloid cells, and basal cells, influencing patient prognosis. Furthermore, we identified several significantly upregulated genes (CD46, JAG1, IL6, and IL6R) that may positively correlate with cancer cells' survival and invasive capabilities in this subtype. Discussion: Our study is a robust foundation for precision medicine and personalized therapy, presenting a novel perspective for the contemporary classification of breast cancer.

Abstract:Objective To investigate the associations between the different breast cancer subtypes and survival in Chinese women with operable primary breast cancer. Methods A total of 1538 Chinese women with operable primary breast cancer were analyzed in this study, the median follow-up was 77 months. Estrogen receptor (ER), progesterone receptor (PR), and HER2 status were available for these patients. Results Luminal A (ER+ and/or PR+, HER2-) had a favorable disease-free survival (DFS) and overall survival (OS) compared with other subtypes in the entire cohort. Using the luminal A as a reference, among the patients with lymph node positive disease, HER2+ (ER-, PR-, HER2+) had the worst DFS (hazard ratio, HR=1.80, 95% CI 1.11 to 2.91, P =0.017) and luminal B (ER+ and/or PR+, HER2+) had the worst OS (HR=2.27, 95% CI 1.50 to 3.45, P <0.001); among the patients with lymph node negative disease, triple-negative (ER-, PR-, HER2-) had the worst DFS (HR=2.21, 95% CI 1.43 to 3.41, P <0.001), whereas no significant difference in DFS between HER2+ and luminal B or luminal A was observed. Conclusion As compared with luminal A, luminal B and HER2+ have the worst survival in patients with lymph node positive disease, but this is not the case in patients with lymph node negative disease; triple-negative subtype has a worse survival in both lymph node positive and lymph node negative patients.
