Abstract:Objective To explore the clinical effects of C2 dome-like expansive laminoplasty and C2 expansive opendoor laminoplasty in upper cervical(involve or above C2 segment) ossification of the posterior longitudinal ligament.Methods All of 39 patients (22 males and 17 females) withcervical ossification of the posterior longitudinal ligament up to C2 which causedcompression symptoms were operated with posterior cervical surgery in ourhospital from January 2013 to June 2017.The average age was 55.74+7.91 years old,ranging from 39 to 71 years.Among these patients,21 patients underwent C2 domelike expansive laminoplasty and 18 underwent C2 expansive open-door laminoplasty,the patients in both groups underwent the C3-C7 expansive open-door laminoplasty.The preoperative and postoperative Neck Disability Index (NDI),Japanese Orthopedic Association (JOA) score,the xisual analog scale(VAS) and the space available for the spinal cord (SAC) of C2 segment in X-ray were measured and statistically analyzed.Results All patients were followed up,the follow-up time was from 15 to 63 months (mean 42.3±17.7 months).There was no significant difference in the general condition,NDI,JOA score and JOA score improvement rate between preoperative and postoperative follow-up in 2 groups.There were significant differences in the NDI,JOA,VAS and SAC between preoperative and postoperative follow-up in 2 groups.The VAS score in C2 dome-like expansive laminoplasty group 1.52± 1.2 was significantly lower than that in C2 expansive open-door laminoplasty group 2.06±0.87(t=-2.23,P＜0.05),while the SAC in C2 expansive open-door laminoplasty group 14.11±1.023 was significantly higher than that in C2 dome-like expansive laminoplasty group 1 L86± 1.014(t=-6.89,P＜0.05).No failure of internal fixation or recurrent compression was found during follow-up.Conclusion For patients with ossification of posterior longitudinal ligament up to C2 or higher level,both C2 dome-like expansive laminoplasty and C2 expansive open-door laminoplasty can achieve good results.The SAC in C2 expansive open-door laminoplastygroup was superior to that in C2 dome-like expansive laminoplasty group,while the postoperative pain was more obvious.There was no significant difference in postoperative neurological recovery between the two groups.Using C2 dome-like expansive laminoplasty could reduce postoperative axial pain than the C2 expansive open-door laminoplasty surgery.For patients withossified tissue in the spinal canal,which occupies more than 50％ of the sagittal diameter of the spinal canal,or with developmental spinal stenosis,C2 expansive open-door laminoplasty may berecommend to get more adequate decompression.
[Use Sagittal Reconstruction CT for Making Decisions Regarding the Surgical Strategy for Cervical Ossification of the Posterior Longitudinal Ligament].

Abstract:OBJECTIVE:Use sagittal reconstruction CT to verify the surgical strategy for cervical ossification of the posterior longitudinal ligament (OPLL).METHODS:A retrospective study of 161 patients (106 males and 55 females) who had undergone surgery for OPLL from July 2007 to November 2010 was performed. The mean age at surgery was 54.5 years (range from 26 to 77 years). The mean follow-up period was 28 months (12 - 54 months). There were 40 patients accept anterior approach surgeries (anterior group) which include 14 cases of anterior cervical corpectomy and fusion and 26 cases of anterior cervical discectomy and fusion. There were 120 patients accept posterior approach surgeries (posterior group) which was spinous process-splitting laminoplasty for cervical myelopathy using coralline hydroxyapatite. One patient accepted combined anterior and posterior approach. According to the sagittal reconstruction CT, the main reason for spinal cord compression was cervical disc herniation in anterior group, and OPLL in posterior group. The level of spinal cord compression was 1 to 2 levels in anterior group, and 1 to 5 levels in posterior group with a major of 2 to 4 levels. As the classification of OPLL, segmental type and circumscribed type were major of segmental type in anterior group and all of the four types were in posterior group, the distribution of each type was average. The patients of posterior group were classified into two groups according to the modified K-line classification, and clinical results were compared between the two groups. The modified K-line was defined as a line that connects the midpoints of the spinal canal at C(2) and C(7) on sagittal CT myelography. Compression to the spinal cord did not exceed the K-line in the modified K-line(+) group and did exceed it in the modified K-line(-) group. Clinical data were compared using t-test or χ(2) test. Correlation analysis was used to determine the relationships of C(2)-C(7) angulation between sagittal reconstruction CT and neutral position X-ray.RESULTS:The patient of anterior group had better recovery rate of the JOA score (72% ± 27%) than the posterior group (59% ± 35%) at the latest follow-up (t = 2.238, P = 0.027). In posterior group, the patients of modified K-line(+) group had better recovery rate of the JOA score (63% ± 37%) than the K-line(-) group (49% ± 30%) at the latest follow up (t = 2.150, P = 0.034). The C(2)-C(7) angulation on sagittal reconstruction CT was 11° ± 9° which has significantly correlated with the C(2)-C(7) angulation on neutral position X-ray which was 10° ± 10° (r = 0.947, P < 0.01).CONCLUSIONS:Considering the selection of surgical approach, it should be combined with the main clinical diagnosis for spinal cord compression, the level of compression, the classification of OPLL and the kyphotic alignment of the cervical spine. The modified K-line is a simple and practical tool for making decisions regarding the surgical strategy for cervical OPLL patients.
Extended Laminoplasty for Ossification of Posterior Longitudinal Ligament Involving the C2 Segment

Abstract:BACKGROUND: To evaluate effectiveness and safety of extended laminoplasty for the treatment of ossification of posterior longitudinal ligament (OPLL) involving the C2 segment. METHODS: A total of 132 cases who accepted laminoplasty for treating the OPLL from 2010 to 2015 were included and divided into Group A (n = 26, with C2 OPLL and C2-C7 laminoplasty), Group B (n = 29, with C2 OPLL and C3-C7 laminoplasty), and Group C (n = 77, without C2 OPLL and had C3-C7 laminoplasty). Clinical and radiologic outcomes were compared among the 3 groups. RESULTS: At the final visit, significant improvements in japanese Orthopedic Association score, Neck Disability Index, Health-Related Quality-of-Life Short Form 36 Physical Component Summary, and Visual Analog Scale score in neck were observed in all 3 groups. However, all the C2-C7 Cobb angle, C2-C3 Cobb angle, sagittal vertical axis, and number of cases with cervical or segmental kyphosis in the 3 groups were not significantly changed. Although total blood loss, operation time, maximum Visual Analog Scale score in neck, incidence of axial pain, change of C2-C7, or sagittal vertical axis in Group A were all significantly larger than that in Group B or Group C, differences in neurologic recovery rate, Neck Disability Index, Health-Related Quality-of-Life Short Form 36 Physical Component Summary, change of the C2-C3 Cobb angle, and incidence of cervical or segmental malalignment among the 3 groups were not significant at the final visit. CONCLUSIONS: If decompression indication for cases with OPLL involving the C2 segment was appropriate, extended laminoplasty up to the C2 segment could provide satisfactory neurologic recovery and would not accelerate cervical or segmental malalignment progression.
Biomechanical and Clinical Study of Anterior Transarticular Screw Fixation for Treatment of Traumatic Instability of C1-C2 Joint

Abstract:Objective To evaluate the biomechanical stability of C1-C2 after anterior C1-C2 transarticular screw fixation and assess its curative effect clinically. Methods Eight human cadaveric occipitocervical spine specimens which have been harvested freshly were tested in three modes of loading,flexion-extension, lateral bending, and axial rotation, and four configurations including intact, after an odontoid fracture, and after anterior or posterior C1-C2 transarticular screw fixation. All tests were performed in load and torque control. Clinically, 20 patients who had C1 -C2 traumatic instability were instructed to be operated with anterior C1 -C2 transarticular screw fixation technique, planting cancellated granulate bone into the space between muscular tubercle of arias and odontoid bone. Results Both the anterior and posterior C1-C2 transarticular screw fixation techniques limited motion at the C1-C2 articulation at the tested directions and there were no significant differences between the two techniques. In the 20 patients, one with complete cervical cord injury died of pulmonary infection a month later, and the rest 19 were followed up about 7 months to 3 years (mean 18 months). They all had no trauma on vertebral artery and cervical cord during their cervical operations, and had aflantoaxial synostoses. Conclusion The anterior C1-C2 transarticular screw fixation not only is easy to manipulate and provides the same stability as posterior C1-C2 transarticular screw fixation, but has a less risk of injury on vertebral artery and cervical cord.

Abstract:Objective To discuss the surgical indications,results and complications of anterior or posterior approach for the treatment of severe ossification of posterior longitudinal ligament(OPLL)in the cervical spine.Methods Between 2002 and 2006,34 patients(29 males and 5 females,mean age 57.2 years)with severe cervical ossification of posterior longitudinal ligament(the canal stenosis rate exceeding 50%)were surgically treated in our hospital.Among them,14 patients(12 males and 2 females)underwent anterior corpectomy and fusion,and the other 20 patients (17 males and 3 females)underwent posterior laminectomy and fixation.The radiological data was compared between two groups by different approach,including spinal canal diameter,cervical lordosis,occupying rate,type of OPLL,extent of OPLL and cord flattening rate.The preoperative and postoperative neurological status was evaluated by JOA scoring system and the improvement rate was also calculated.Results Radiological studies showed anterior approach was selected for local-or segmental-type OPLL extending less than 3 vertebrae,and posterior approach was used for continuous-or mixed-type OPLL extending 3 or more vertebrae.Surgical approach was selected according to the type and extent of OPLL After following up 6 months to 3 years(mean,1.5 years),the average JOA score of the patients with anterior approach increased from 9.3±1.8 to 14.2±1.3.with an improvement rate of 62.3%±15.2%in neurological function;the average JOA score of those with posterior approach in creased from 8.7±1.6 to 11.4±1.2.with an improvement rate of 33.5%±12.7%.The difference between two groups was statistically significant(P＜0.01).Conclusion Anterior corpectomy and fusion is effective and safe for severe OPLL less than 3 vertebrae.and posterior laminectomy and fixation is an alternative option for that extending 3 or more vertebrae.
Study of operative complications of ossification of posterior longitudinal ligament

Abstract:[Objective]To Probe the causes of significant operatiive complications of ossification of posterior longitudinal ligament as well as the treatment outcomes in order to reduce the surgical morbidities and to suggest preventive measures of complications.[Method]The surgical data of 85 cases with ossification of posterior longitudinal ligament were reviewed form March 2002 to May 2006.In continuous long segment OPLL which were treated by bilaminectomy with internal fixation system;short segment OPLL which were treated by corpectomy of anterior cervical approach with bone of autograft and anterior cervical spine locking plates.[Result]Totally 66 cases were followed-up from 3 to 25 months with an average of 13 months.Complication of posteior cervical approach:8 cases with the pain of neck-shoulder,which resulted from postoperative radiculopathy and nerve root irritation or injury.Most of 8 cases were recovered in 2 to 20 weeks by conservative treatment such as antalgica,dehydration and physiotherapy.Four cases with quadriparesis or symptom deteriorate,which resulted from reperfusion injury of spine cord.Two cases were recovered by hyperbaric oxygen and medication and 1 cases recovered not enough.Two cases with posterior cervical hemotoma were caused by bleeding of smaller blood vessels and obstruction of drainage.One case of CSF leakage were cured in 3 days with cervical spinal immobilization and moderate local compressiom.Two cases of local infection were cured during 20 days with antibiotics or combined with debridement and suturing.Complications of anterior cervical approach:a cases with quadriparesis or symptom deteriorate;1 case of CSF leakage.Complications related to instruments included 1 case of titanium net subsidence and 1 case of single lateral mass screw back-out.[Conclusion]Many kinds of operative complications could occur in either anterior or posterior approach of surgery of ossification of posterior longitudinal ligament.Preoperative good preparation,intraoperative carefull operation and postoperative strengthened management are the key points to decrease and prevent operative related complications.
Percutaneous Fully-endoscopic Anterior Transcorporeal Procedure for the Treatment of Isolated Ossification of the Posterior Longitudinal Ligament in the Cervical Spine: A Case Report

Abstract:Background: With the development of spinal endoscopic techniques, on the basis of our previous experience in treating various types of cervical disc herniation with this endoscopic technique, we took the lead in applying the percutaneous fully endoscopic anterior transcorporeal procedure to be utilized in the treatment of the isolated cervical ossification of the posterior longitudinal ligament (OPLL). Case presentation: A 66-year-old male patient who weighed 57 kg, with a height of 169 cm was admitted to the hospital on September 16, 2021 because of recurrent pain and numbness in the neck, shoulder, and right arm for 2 years, which as aggravated for the last 2 weeks. Two years ago, the patient developed neck and shoulder pain accompanied by right arm pain without obvious predisposing factors, and numbness in the first web space of the right hand. In the last 2 weeks, he had difficulty moving the right arm, but no pain or numbness in the contralateral arm. MRI and CT scans demonstrated that the ossified posterior longitudinal ligament of the cervical 5/6 vertebrae with spinal canal stenosis and seriously compressed the spinal cord patient was treated with a percutaneous fully endoscopic anterior transcorporeal procedure. Conclusion: Our percutaneous fully endoscopic anterior transcorporeal procedure is a feasible, minimally invasive surgery for treating isolated ossification of the posterior longitudinal ligament in the cervical spine.
The Safety of Combined Posterior-Anterior Approaches for Cervical Ossification of the Posterior Longitudinal Ligament

Abstract:Objective To evaluate the applicability and safety of posterior single open-door vertebral canaloplasty combined with anterior decompression,fusion and internal fixation for the management of cervical ossification of the posterior longitudinal ligament,the authors retrospectively reviewed 68 consecutive procedures of this type performed at their respective institutions and studied the surgical effects and surgical complications,surgical indications.Methods Retrospectively analyzed 68 patients with the disease of cervical ossification of the posterior longitudinal ligament operated by combining posterior-anterior approachs surgical treatment from March in 1999 to April in 2006,All the patients were followed up minimum of 1 years(mean 18 months).JOA scores system and Nurick grading system were used to evaluate the effect and safety of the treatment.Results JOA scores were improved from preoperative(6.7±1.1) to postoperative(15.1±0.6),T-test was used to compare preoperative and postoperative JOA scores.The statistical results were significantly different(P0.001,α=0.05).The improvement rate of posterior-anterior unites to cure the cervical ossification of posterior longitudinal ligament was(81.5±5.6)%;Nurick grading were improved from(2.8±1.2) to(0.7±0.5),and were statistically analyzed with the method of rank-sum test of the ordered Variable data.The statistical results were significantly different(P0.001,α=0.05).Approaching segment degeneration increased in 3 cases.There were no cases of spinal cord or nerve root abiding injury,strut graft extrusion,or anterior plate or screw fracture,or cervical kyphosis,and other severe complacations.Conclusion The combined anterior-posterior decompression and instrumentation procedure provides the thoroughly decompression for cervical spinal cord,corrects cervical kyphosis,and keeps the stabilization of the cervical spine.Furthermore,it lowers the rate of injuring spinal cord,poor prostecdtive efficacy,and diminishs anterior plate failure or strut graft extrusion and pseudoarticulation.A better surgery outcome and higher safety are obtained with this combined approach.
Anterior Surgery for Cervical Ossification of the Posterior Longitudinal Ligament Associated with Dural Ossification

Abstract:Objective To discuss the radiologieal characteristics, surgical treatment and clinical re-sults of ossification of the posterior longitudinal ligament (OPLL) associated with dural ossification (DO) in the cervical spine. Methods Between January 2005 and March 2008, 13 patients (11 males and 2 females,age ranging 43-72 years, mean 53.6 years) with OPLL was found to be associated with DO in anterior ap-proach. The type of OPLL was distributed as follows: three local-type, two segmental-type, five continuous-type, and three mixed-type. The OPLL extended 1-5 vertebrae, with an average of 2.8 vertebrae. All patients underwent anterior eorpectomy and resection of OPLL. During operation, the dura mater was dissected and remained intact in six patients and tore or defected in other seven patients. Results Eight patients were presented with typical double-layer sign in preoperative CT scans, two patients with a low-intensity in the whole ossification, and the occupying rate of OPLL exceeding 90% in other three patients. Five patients de-veloped eerebrospinal fluid (CSF) leakage, and three patients healed after staying in bed and local pressure for 3-5 days. The other two patients experienced a CSF pseudocyst, which gradually disappeared after 1 month of repeated puncture and aspiration. After a follow-up of six months to two years (mean, one year), the mean JOA score increased from 8.1 points before operation to 13.2 points after operation, and the mean im-provement rate of neurological status was 57.3%. There was no significant difference in neurological im-provement between the patients with or without DO removed. Conclusion Preoperative CT three-dimension-al reconstruction was helpful to diagnose OPLL associated with DO in the cervical spine before operation. O-PLL associated with DO was not the contraindication to the anterior approach for OPLL. Removal of ossifica-tion and complete decompression were critical to the surgical results in anterior approach.
Novel Algorithm for Surgical Management of Cervical Ossification of Posterior Longitudinal Ligament: A Retrospective Cohort Study With 2‐Year Follow‐Up

Abstract:The aim of this study was to elucidate a novel algorithm to guide the choice of surgical approaches for COPLL based on its location and extent. We proposed the following three main criteria for the classification: (1) the positional relationship between ossification and UP, (2) the K‐line, and (3) the ossification segment. Object With the development of new technologies, the surgical algorithm for ossification of posterior longitudinal ligament (OPLL) in the cervical spine also needs to be updated. The aim of this study is to elucidate a new surgical classification algorithm to guide the choice of surgical approaches for cervical OPLL based on its location and extent. In this algorithm, anterior controllable antedisplacement and fusion (ACAF) will be used as a new surgical option. Methods This is a single‐centered, retrospective, cohort study utilizing a novel algorithm based on the following three criteria: (1) the positional relationship between ossification and uncinate process (UP), (2) the K‐line, and (3) the ossification segment for surgical decision‐making. Patients diagnosed with cervical OPLL who received surgical intervention guided by the algorithm were included. Patient demographics, Japanese Orthopedic Association (JOA) scores, surgical time, imaging data before and 2 years after surgery, and the occurrence of complications were extracted from the database. Paired t‐test was used for intragroup comparison, and one‐way ANOVA test was used for the intergroup analyses. Results Based on this novel algorithm, 15 patients with Type I, 8 patients with Type II a, 2 patients with Type II b1, 5 patients with Type II b2 were included. The decision‐making for the surgical techniques used in each patient followed the recommendation of the novel algorithm. A Review of Prognostic Factors for Surgical Outcome of Ossification of the Posterior Longitudinal Ligament of Cervical Spine.

Abstract:For patients with ossification of the posterior longitudinal ligament (OPLL) who have neurological-symptoms, surgery is necessary but not always effective. Various clinical factors influence the surgical outcome. The studies identifying these factors have been inconclusive and conflicting. It is essential for surgeons to understand the significance of the factors and choose the optimal therapeutic strategy for OPLL. The objective of this review is to determine the clinical factors predictive of the surgical outcome of cervical OPLL. The authors conducted a review of literature published in the English language. They examined studies in which the correlation between clinical factors and outcome were statistically evaluated. The results showed that the traverse area of the spinal cord, the spinal cord-evoked potentials (SCEPs), the increase of the range of motion in the cervical spine (ROM), diabetes, history of trauma, the onset of ossification of the ligament flavum (OLF) in the thoracic spine, snake-eye appearance (SEA) and incomplete decompression may be predictive factors. Age at surgery seems to be closely related to the outcome of posterior surgical procedure. Whether the neurological score, OPLL type, pre-operative duration of symptoms, focal intra-medullar high signal intensity in T2-weighted (IMHSI) and progression of OPLL or kyphosis and expansion of the spinal canal predict the surgical outcome remains unclear. The use of uniform neurological score and proper statistic analysis should facilitate comparison of data from different studies. It is important to analyze the effect of each factor on groups with different surgical procedures as well as patients with different compressive pathology. Research on the etiology and pathology of cervical myelopathy due to OPLL should be helpful in precisely understanding these clinical factors and predicting surgical outcome.
Diagnosis and surgical management of cervical ossification of the posterior longitudinal ligament

Abstract:Background context: The diagnosis and treatment of multilevel cervical ossification of the posterior longitudinal ligament (OPLL) is continuing to evolve as its effects become more readily recognized and surgical alternatives expand. Purpose: To review the clinical, neurodiagnostic and surgical management of OPLL. Study design/setting: Patients with early OPLL, often in their mid-forties, present with radiculopathy or mild/moderate myelopathy. Radiographically, hypertrophy of the posterior longitudinal ligament with punctate ossification appears opposite multiple disc spaces. Patients with classic OPLL frequently become symptomatic in their mid-fifties with radiographic characteristics showing ossification of the ligament behind the vertebrae alone (segmental), behind the vertebrae including the intervertebral disc spaces (continuous), and combinations of the segmental and continuous variants and OPLL opposite disc spaces alone. Both magnetic resonance imaging (MRI) and computed tomography (CT) examinations are critical. MRI better delineates the extent of soft tissue abnormalities in three dimensions, including the cervicothoracic junction, whereas CT more readily identifies the foci of frank ossification. Surgical alternatives include anterior, posterior or combined approaches. Anterior surgical options include plated multilevel anterior discectomy and fusion, anterior cervical corpectomy with fusion (ACF), or plated multilevel ACF with differing posterior fusion techniques. Posterior surgical options vary from laminectomy with or without simultaneous fusion and laminoplasty. Although outcomes with different approaches vary, many direct anterior resection techniques achieve more favorable results because of appropriate and adequate resection of the ligament. Conclusions: The clinical and neuroradiographic documentation of OPLL and its appropriate surgical management anteriorly, posteriorly or circumferentially remain a therapeutic challenge.

Abstract:BACKGROUND CONTEXT: Surgical management of ossification of the posterior longitudinal ligament (OPLL) is associated with complications. However, surgical complications for OPLL have not been clearly documented. PURPOSE: To review and summarize the incidence of surgical complications of OPLL in the cervical spine and evaluate the impact of surgical approach (anterior or posterior), year of study publication, follow-up duration, and the surgical outcome on complication incidence. STUDY DESIGN: Systematic review. METHODS: An English literature review was conducted especially on surgical complications of cervical OPLL. The incidence of complications was statistically summarized, with its correlation to surgical approaches, year of study publications, follow-up duration, and the surgical outcome was statistically evaluated. RESULTS: Twenty-seven retrospective studies, including a total of 1,558 patients, were reviewed. The overall incidence of surgical complications of cervical OPLL was 21.8%. Neurologic deficit (8.3%, overall rate), cerebrospinal fluid leakage (CFL) (5.1%), axial pain (3.5%), and implant complications (3.5%) were relatively common. The incidence of complications for posterior approach was not statistically different from those for anterior procedures. However, with regard to individual complication, C5 palsy and axial pain occurred more frequently in patients approached posteriorly, whereas CFL, implant complications, hoarseness, dysphagia, and dyspnea appeared more common in anterior cases. CONCLUSIONS: There is a relatively high incidence of surgical complications for cervical OPLL compared with other cervical degeneration diseases. It is, therefore, necessary for surgeons to take into consideration the risk of surgical complications when communicating with patients for decision making and to alert complications during or after surgical procedures. Treatment for Thoracic Ossification of Posterior Longitudinal Ligament with Posterior Circumferential Decompression

Abstract:Objective: To report the results of the posterior approach for thoracic ossification of posterior longitudinal ligament (TOPLL) by using a special "L" osteotome.Methods: The present study enrolled 16 consecutive patients (9 men and 7 women) between May 2009 and September 2013. All patients underwent a posterior circumferential decompression osteotomy and segmental instrumentation with interbody fusion. The mean age at surgery was 57.3 years (range, 37-68 years). Patients' data, clinical manifestation, blood loss, length of surgery, complications, visual analog scale (VAS), Japanese Orthopedic Association (JOA), and Frankel grading system before and after surgery were collected and evaluated, retrospectively.Results: The average follow-up period was 30 +/- 19 months (range, 12-50 months). All patients were successfully treated with posterior compression and segmental instrumentation with interbody fusion. The average operation time was 261.6 +/- 51.3 min (range, 190-310 min). The mean blood loss was 980.3 +/- 370.5 mL (range, 600-2100 mL). All patients had subjective improvement of motor power and gait. Average preoperative and postoperative JOA scores were 4.2 +/- 1.7 and 7.8 +/- 2.5 points, respectively. Differences in the overall JOA scores showed significant postoperative improvement. At the last follow-up, all patients improved either by one or two Frankel grades. There was a significant difference between preoperative VAS scores and those 3 months after surgery (P < 0.05). No significant difference was observed between the 3-month and 12-month results (P > 0.05). Cerebrospinal fluid (CSF) leakage occurred in 3 patients. Acute neurological deterioration was encountered postoperatively in 1 patient.Conclusion: Treatment with posterior transpedicular osteotomy and circumferential decompression was found to be safe, effective, reliable, and technically feasible, and keeping the thoracic cavity intact avoids many shortcomings of anterior surgery and results in a satisfactory spinal decompression.
New anterior controllable antedisplacement and fusion surgery for cervical ossification of the posterior longitudinal ligament: a biomechanical study

Abstract:Objective: The traditional anterior approach for multilevel severe cervical ossification of the posterior longitudinal ligament (OPLL) is demanding and risky. Recently, a novel surgical procedure-anterior controllable antedisplacement and fusion (ACAF)-was introduced by the authors to deal with these problems and achieve better clinical outcomes. However, to the authors' knowledge, the immediate and long-term biomechanical stability obtained after this procedure has never been evaluated. Therefore, the authors compared the postoperative biomechanical stability of ACAF with those of more traditional approaches: anterior cervical discectomy and fusion (ACDF) and anterior cervical corpectomy and fusion (ACCF). Methods: To determine and assess pre- and postsurgical range of motion (ROM) (2 Nm torque) in flexion-extension, lateral bending, and axial rotation in the cervical spine, the authors collected cervical areas (C1-T1) from 18 cadaveric spines. The cyclic fatigue loading test was set up with a 3-Nm cycled load (2 Hz, 3000 cycles). All samples used in this study were randomly divided into three groups according to surgical procedures: ACDF, ACAF, and ACCF. The spines were tested under the following conditions: 1) intact state flexibility test; 2) postoperative model (ACDF, ACAF, ACCF) flexibility test; 3) cyclic loading (n = 3000); and 4) fatigue model flexibility test. Results: After operations were performed on the cadaveric spines, the segmental and total postoperative ROM values in all directions showed significant reductions for all groups. Then, the ROMs tended to increase during the fatigue test. No significant crossover effect was detected between evaluation time and operation method. Therefore, segmental and total ROM change trends were parallel among the three groups. However, the postoperative and fatigue ROMs in the ACCF group tended to be larger in all directions. No significant differences between these ROMs were detected in the ACDF and ACAF groups. Conclusions: This in vitro biomechanical study demonstrated that the biomechanical stability levels for ACAF and ACDF were similar and were both significantly greater than that of ACCF. The clinical superiority of ACAF combined with our current results showed that this procedure is likely to be an acceptable alternative method for multilevel cervical OPLL treatment.

Abstract:OBJECTIVE:To investigate the clinical effects of 360 degree circular decompression and transpedicle screw fixation to treat the ossification of thoracic posterior longitudinal ligament by posterior approach.METHODS:From December 2009 to November 2013, 18 patients with ossification of thoracic posterior longitudinal ligament ossification were treated with 360 degree circle decompression and transpedicle screw fixation by posterior approach. There were 8 males and 10 females,ranging in age from 32 to 67 years old, with an average of 51 years old. Four patients were accompanied with ligamentum flavum ossification. Longitudinal ossifications in 5 cases were located in the upper thoracic vertebra and 13 cases in the middle and lower thoracic vertebra. Five cases were typical type, 4 cases were segmental type, 6 cases were continuous type and 3 cases were mixed type. All the patients had the posterior spinal canal decompression combined with longitudinal ligament resection, interbody fusion with bone graft and internal fixation surgeries. The operation time,blood loss and complications were recorded. JOA score were applied to evaluate the neurological function recovery pre-surgery, 2 days post-surgery and the latest follow-up. The surgery effects were evaluated by Epstein-Schwall method.RESULTS:The operation time ranged from 3 to 6 hours (mean, 4.2 hours). The blood loss ranged from 800 to 4 000 ml (mean, 1 800 ml). All the patients were followed up, and the duration ranged from 6 months to 3 years, with a mean of 1.8 years. The JOA score increased from preoperative 4.30 ± 2.60 to 7.60 ± 2.40 2 days after surgery, and 7.80 ± 1.90 at the latest follow-up (t = 4.61, P < 0.001). The JOA scores between 2 days after surgery and the latest follow-up had no significant differences (t = 0.28,P = 0.78). The neurological recovery rate was 74% 2 days after surgery and 71% at the latest follow-up. There were 4 cases got an excellent result,10 good,3 fair and 1 poor according to Epstein-Schwall evaluation method. Four patients had cerebrospinal fluid leakage, 3 patients had intercostal nerve paralysis or pain, and 1 patient had superficial incision infection. The neurological function in 3 patients became worse in the second day posteratively , and among them, 2 patients were recovered at the latest follow-up and 1 patient had no changes. All the patients got fusion of bone graft and no internal fixation loosening and fractures occurred.CONCLUSION:360 degree circular decompression and transpedicle screw fixation can resect different types of thoracic longitudinal ligament ossification, and can achieve a good clinical effect.
Surgical Management of Cervical Disc Herniation Associated with Ossification of the Posterior Longitudinal Ligament

Abstract:Objective To discuss the clinical features,surgical method and outcome of cervical discherniation associate with ossification of the posterior longitudinal ligament.Methods Twenty-six patients of cervical disc herniation associated with ossification of the posterior longitudinal ligament from June 2003 to June 2008 were retrospectively analyzed.There were 16 males,10 females,with a mean age of 45 years.One to three levels of cervical disc herniation associated with two or more levels of ossification of the posterior longitudinal ligament were revealed by X-ray film,CT and MRI.The preoperative mean visual analogue scale (VAS) and Japanese Orthopedic Association (JOA) score were 5.3 points (range,4-7 points) and 9.3 points (range,5-12 points) respectively.Thirteen patients were decompressed by only anterior discectomy,11 by corpectomy and 2 by discectomy followed with posterior laminectomy.Results All patients experienced a remarkable recovery from radiculopathy,numbness,weakness and gait disturbance.Follow-up of 18 months(range,12-36 months) showed the mean VAS and JOA score increased to 2.5(1-4) points and 14.2 (11-16)points.The mean improvement rate of JOA was 63.2%.Leakage of cerebrospinal fluid which occurred in 2 cases disappeared afteR proper treatments without other complications.Conclusion The clinical symptoms of ossification of the posterior longitudinal ligament may appear or aggravate because of cervical disc herniation.The outcome is favorable after identifying the primary compressing factor followed by resection of the herniated cervical disc.

Abstract:Study Design: This is a single-center, retrospective, cohort study. Objective: The objective of this study was to propose a surgical classification algorithm guiding the choice of surgical approaches for ossification of the posterior longitudinal ligament (OPLL) in the thoracic spine based on the characteristics of each patient's specific pathology. Summary of Background Data: Surgical decision-making in this OPLL patient population requires balancing a more definitive decompression of the thoracic spinal cord through direct decompression against a higher risk of significant complications. Materials and Methods: Patients with a primary diagnosis of thoracic OPLL who received surgical decompression in a tertiary spine center between May 2009 and June 2015 were included. Surgical planning was guided by our classification algorithm according to the location and the extent of OPLL in each patient. Patient demographics, neurological function using the 11-point Japanese Orthopaedic Association scale, length of procedure, estimated blood loss, the occurrence of major complications, etc., were extracted from the database of electronic medical record system. The Kruskal-Wallis test and Fisher exact test were used when data were not normally distributed. Results: A total of 115 patients (33 men and 82 women) were included in this study, among whom 108 had a follow-up of at least 12 months (average: 51.7 +/- 22.2 mo, range: 12-100 mo). On the basis of our classification algorithm, 11 patients were type I, 26 were type II, 60 were type III, 11 were type IV, and the choice of surgical decompression techniques used in each patient followed that recommended by the algorithm in all cases. On average, the Japanese Orthopaedic Association score improved from 5.0 +/- 1.8 preoperatively to 8.7 +/- 1.7 postoperatively, with a Hirabayashi improvement rate of 63.6%. Conclusion: A new surgical classification algorithm guiding the choice of approach for decompression of OPLL in the thoracic spine was validated in a series of 108 patients through an analysis of their clinical outcomes and surgical complications.

Abstract:To observe and analyze the results of ossification of posterior longitudinal ligament(OPLL)of cervical spine with different surgical approaches,the clinic data of 127 patients with cervical OPLL were reviewed and followed up for 5 to10 years,which were respectively decompressed from anterior and posterior approaches and pre-and post-operational Imaging results and Neurological function were evaluated,also it discuss and evaluate the operation methods and outcomes of the OPLL patients and the influence of the rate of spinal stenosis and cervical curvature.The results show that the cervical curvature and Neurological function in the anterior operation group was better improved and maintained than the posterior group(P0.05).Therefore,the considerable factors that influence the outcomes of OPLL include the rate of spinal stenosis and cervical curvature,which are also significant to the surgery strategies of OPLL.
