Abstract:The purpose of this paper is to present salient background information on the oral care situation and organization in China and to review epidemiological literature that has been published on adults in Chinese dental journals during the last 20 years as a context for understanding ongoing oral health research activities in China. Searches were conducted through Medline as well as Chinese language catalogues and around 30 Chinese dental journals. Most oral health surveys on adults were conducted after 1970 mainly on residents living in big cities, which are also the location of major medical universities. Surveys conducted among farmers were uncommon. Dental caries and periodontal disease were the major dental diseases studied. Only a few surveys on people's oral health knowledge, attitudes and behavior were conducted. Papers concerning dental treatment needs and utilization of dental services among adults were scarce. Sample size was usually not a major problem in these surveys but the sampling methods and diagnostic criteria used were often poorly defined. Mean DMFT scores between 2 and 6 were reported in the 35-44-year-olds and between 9 and 16 in the elderly younger than 75 years. Calculus and gingivitis were reported to be common in adult Chinese; the proportion of subjects with shallow and deep periodontal pockets was not high. Complete edentulousness in the 60-69-year-olds was usually reported to be less than 10%. About 14% of the 35-44-year-olds and 19% of the 65-74-year-olds did not brush their teeth daily.

Perspectives on dental education in mainland China

Abstract:The Chinese dental education system has undergone major changes over the past three decades. Although a number of approaches have been adopted for the modification of the dental curriculum, the greatest challenge remains as to how to develop enough qualified dentists to meet the dental care needs of China's 1.3 billon population. In many areas the limited funds have been mainly used for the enhancement of academic status while the clinical training programmes for the improvement of practical skills of the clinicians has been greatly neglected. In addition, the discrepancies in the distribution of dental resources have been increasingly alarming and have the tendency toward becoming even more serious in the future. The aim of this review is to describe dental education in China in the hope that more favourable changes could take place and the system itself could be further optimised.

Abstract:Objective: To review the current oral health status and oral health care models in China in an effort to provide recommendations for the future implementation of these models.Methods: A systematic literature review was conducted. The Medline, EMBASE, CNKI and Wanfang databases were searched for English and Chinese articles reporting relevant data from 1949 to the present. Data from three national oral health epidemiology surveys, Chinese government reports and national statistics yearbooks from 2011 to 2015 were also included.Results: The oral health status of preschool children were significantly improved over the past 10 years, while caries experience among 35 to 45-year-old and 65 to 74-year-old groups showed an increase in 2005. The status of poor oral hygiene was observed for both adolescent and elderly groups. The ratio of dentist-to-population in China was reported as 1: 10,000 in 2009, which was much lower than that of developed countries. The workforce of the dental service is distributed unevenly and remains insufficient for such a highly populated country. Although the need for dental treatment was perceived as high, the true demand for dental service in China was relatively low and not seen as critical. This situation clearly did not reflect so well with true oral disease conditions. There are several basic social medical insurance systems available in China, which covered most of the population's need for medical attention, but seldom covered dental treatment.Conclusion: National oral health policy in China should emphasise oral health promotion, especially in school education for children and young adults, to further strengthen daily toothbrushing, use of fluoride toothpaste and dental floss, and actively promote annual oral health examination. Oral health management should focus on cost-effective primary and secondary prevention with the long-term goal of maintaining oral health.
Comparison of dental education and professional development between mainland China and North America

Abstract:Different educational and professional developments within the dental field create different sets of missions, norms, and practices regarding dental diseases and their appropriate treatment. This review has addressed differences in dental education and professional development between mainland China and North America. Many factors influence the choice of model and it is very difficult to predict which model will become predominant. However, there is growing sentiment that the independent faculty model in North America is logical and superior to the model, which 'integrates' dental and medical education in mainland China. Many North America dental schools place a high priority on preclinical and clinical training in the curriculum in order to expose students to patient oral health needs and systemic dental problems much earlier than in mainland China. North America dental schools promote and embrace students self-learning skills by the use of PBL, CRL, and TRAD education methodologies and new e-based technologies and approaches whereby students learn rather than are taught. In mainland China, the traditional lecture-based format is still employed in the majority of dental schools; however, strategies to enhance students self-learning skills is increasingly utilised in most well-known Chinese dental schools. The Chinese dental education model, which treats dentistry as a sub-specialty of medicine, has brought about fundamental differences, with the dentist functioning essentially as a stomatologist. For example, China has built up a large oral and maxillofacial surgery society, and craniofacial surgery is performed to a much broader extent by Chinese dentists than by most North American counterparts. In North America, dentists engage in full-time work, attend continuing training/education programmes, belong to an association, gain legal status, and construct a code of ethics emphasising the quality of care delivered to the public. Currently, continuing dental education in North America is available through a variety of venues involving licensing authorities, universities and private programmes. The concept of professional development in mainland China is relatively new and is still considered primarily in the context of promotion or achieving a higher professional title. Mandatory continuing dental professional education requirements do not guarantee the competence of members of the profession. Today, the Chinese government and society place increasing emphasis upon the accountability of self-regulating professions. Rather than attempting to summarise the current scope of dental education and professional development between mainland China and North America, this paper hopes to enhance mutual understanding, and promote greater academic exchanges in dental education.

Abstract:The most recent Chinese health care reform, scheduled to run until 2020, has been underway for a number of years. Oral health care has not been explicitly mentioned in the context of this reform. However, oral health is an integral part of general health and the under-servicing of the Chinese population in the area of dental care is particularly high. The article describes how this problem could be addressed.Based on present scientific knowledge, specifically on evidence-based strategies and long-term empirical experience from Western industrialised countries, as well as findings from Chinese pilot studies, the author outlines a preventive oral health care system tailored specifically to the conditions prevailing in China. He describes the background and rationale for a clearly structured, preventive system and summarises the scientific cornerstones on which this concept is founded. The single steps of this model, that are adapted specifically to China, are presented so as to facilitate a critical discussion on the pros and cons of the approach.The author concludes that, by implementing preventive oral care, China could gradually reduce the under-servicing of great parts of the population with dental care that largely avoids dental disease and preserves teeth at a price that is affordable to both public health and patients. This approach would minimise the danger of starting a cycle of re-restorations, owing to outdated treatment methods. The proposal would both fit in well with and add to the current blueprint for Chinese health care reform.

Abstract:Only recently have behavioral aspects been taken into consideration in oral health studies in China; thus, relevant information on perceptions in adult Chinese about oral health care is inadequate. In 1996-97, a large-scale oral epidemiological study was conducted in Southern China. Among its objectives were to describe the oral health knowledge, attitudes, and practices of the urban and rural adults living in Southern China and to analyze the influence of selected demographic and socio-economic factors. From 8 urban and 8 rural communities in Guangdong Province, 1573 35- to 44-year-olds and 1515 65- to 74-year-olds were sampled. Specially trained interviewers from the staff of local government or hospitals conducted face-to-face structured interviews. Almost all of the middle-aged and more than 90% of the dentate elderly surveyed claimed that they brushed their teeth every day and used toothpaste during toothbrushing, but awareness about fluoride content was lacking. The respondents had poor oral health knowledge but positive attitudes toward oral health, providing a basis for more community-based oral health education programs, especially targeting adults who are less-well-educated and have fewer socio-economic advantages.

Abstract:This article focuses on the following sections : training of varied levels of medical professionals and joint creative and advanced levels of physician sciemists ; step-by-step building of the Chinese medical professional training system in order to provide efficient health care providers; build and strengthen the family medicine and primary care education system, expand the community health care service and form a stable medical care system which focuses on individuals; and strengthen and reorganize the educational systems for training better quality and suitable countryside and rural area health care providers. Statistical Analysis of Current Oral Health Care and Dental Education Resources in China.

Abstract:OBJECTIVE:To collect information and statistical data regarding the current oral health care and dental education resources in China.METHODS:Electronic databases were searched and a literature review conducted. The homepages of relevant universities and colleges were visited to collect dental education information. In addition, private conversations with related experts were conducted.RESULTS:Compared with the 3rd National Oral Health Survey (2005), the ratio of gingival bleeding in individuals aged 33 to 44 years has increased in the last 10 years. The average percentage of residents visiting departments of dentistry in public hospitals is less than 10%. The total number of dentists and assistant dentists increased to 167,227 in 2016, with a ratio of 1.21 per 10,000 people. There is a great imbalance in the distribution of dental practitioners among the provinces. There are 101 dental schools or departments of stomatology that provide 5-year dental training courses and offer bachelor's degrees in dentistry, with another 93 dental institutions offering shorter 3-year training courses for assistant dentists.CONCLUSION:The results of the present study show that there has been significant change during recent years in China. However, the ratio of dental practitioners to the population as well as the number of dental visits carried out is still much lower than in developed countries and does not satisfy the demand for dental care in China. The quality and quantity of dental education, including continuing dental education, calls for improvement in the future.

Abstract:Objective: To describe the function of oral health services and the delivery of oral health care in PR China. Design: Cross-sectional surveys. Self-administered questionnaires gathered information on professional practices and attitudes of Chinese dentists. Patients were examined clinically for caries and services rendered. Patients were interviewed about oral health status, reason of dental visits and consumption of services, perceived need for care and self-care practices. Setting: Wuhan City of the Hubei Province of China. Subjects: Random samples of dentists (n=250) working with child and adult patients in 1998 in Wuhan City. Each dentist had a representative sample of 20 patients attending consecutively for care enrolled in the study. Results: On average, the dentists saw 12 patients per day, with most time devoted to restorative treatment and extraction. The majority of dentists held the opinion that in China little attention is given to preventive care. Forty per cent of the patients were new to the dentists and more than half attended care because of pain or acute symptoms, The reported mean number of decayed, missing, and filled teeth varied from 2.6 for 12-year-old patients to 10.7 for patients aged 65-74. Daily toothbrushing was practised by more than 90% of the patients in all age groups, however, the tradition of regular dental visits was weak. Most patients (51-75 per cent) attended for care due to acute problems or pain. Conclusion: The study indicated the need for reorientation of the Chinese oral health services towards prevention.

Abstract:Relatively limited information is available about the oral health in the Guangdong Province of Southern China, the closest neighbor to Hong Kong. The study intended to explore the oral health status, knowledge, attitudes, and dental care utilization in the Guangdong population as a basis for formulating strategies for oral health prevention and treatment. Through multi-stage stratified and quota-sampling, individuals from urban as well as rural communities were selected (5- to 6-; 12-; 35- to 44-; and 65- to 74-year-olds; total N = 6251). Structured interviews and clinical examinations were performed. Inter-examiner reliability was high (K = 0.60-0.96). Samples of community water were taken for fluoride assessment. Overall, the sample surveyed was acceptably representative of the population, with some under-representation of rural residents and agricultural workers. Re-weighting was performed in appropriate analyses. The data analysis model used in the Second International Collaborative Study was used as a guide for the present data analysis. This approach has not previously been used on a Mainland Chinese population.

Abstract:A series of regulations and policies for Dental Practitioner Qualification were issued during modern times (1906-1949) in China from local to national levels, in which the certification for qualification had been largely based on dental education and training experiences. The present article reviews the evolution and progression of dental practitioner qualification management, as well as its relationship to dental education before 1949, which may hopefully provide a historical background for the development of modern dentistry and enhance the understanding of the current management process for dental practitioner qualifications in China.

This paper is aimed to introduce the scope, give a brief history, update clinical and basic research, and discuss the future of oral and maxillofacial surgery in China. Abstract:Along with the economic development, improvement of living level of population, and also as a consequence of extensive oral heath education from dental health workers and dental professionals, there is a great promotion and enhancement of populace awareness of oral health maintaining. Since good oral hygiene not only plays an important role in prevention oral disease but also contributes to shaping the perfection of modern life, an expert forum on Oral Hygiene and Modern Life was arranged recently in Shenzhen, China. In this meeting, a panel of experts and dental specialists from mainland of China, Hong Kong and Taiwan discussed intensively and shared their penetrating opinions on the captioned topics including importance and measures of oral hygiene maintaining in oral disease prevention as well as the influence of oral hygiene on the individual's self-image and behaviors. Abstract:OBJECTIVES:To describe oral health behaviour, illness behaviour, oral health knowledge and attitudes among 35-44 and 65-74-year-old Chinese; to analyse the oral health behaviour profile of the two age groups in relation to province and urbanisation, and to assess the relative effect of socio-behavioural risk factors on dental caries experience.METHODS:A total number of 4,398 35-44-year-olds and 4,399 65-74-year-olds were selected by multistage stratified cluster random sampling which involved 11 provinces in China. Data were collected by self-administered structured questionnaires and clinical examinations (WHO criteria).RESULTS:32% of the 35-44-year-olds and 23% of the 65-74-year-olds brushed at least twice a day but only 5% used fluoridated toothpaste; 30% and 17% respectively performed 'Love-Teeth-Day' recommended methods of tooth brushing. A dental visit within the previous 12 months was reported by 25% of all participants and 6% had a dental check-up during the past two years. Nearly 15% of the subjects would visit a dentist if they experienced bleeding from gums; about 60% of the subjects paid no attention to signs of caries if there was no pain. Two thirds of the urban residents and one fifth of the rural participants had economic support for their dental treatment from a third party, either totally or partially. Significant variations in oral health practices were found according to urbanisation and province. At age 35-44 years 43% of participants had daily consumption of sweets against 28% at age 65-74 years. Dental caries experience was affected by urbanisation, gender, frequency, time spent on and method of tooth brushing. Knowledge of causes and prevention of dental diseases was low with somewhat negative attitudes to prevention observed.CONCLUSION:Systematic community-based oral health promotion should be strengthened and preventive-oriented oral health care systems are needed, including promotion of further self-care practices and the use of fluoridated toothpaste.

Abstract:Oral disease patterns change over time and it is important to conduct epidemiological surveillance population surveys regularly to monitor the situation. The overall objectives of the present survey were: (1) to describe the oral health status of Chinese children and adults in the 2015-2016 4th National Oral Health Survey; (2) to investigate the oral health knowledge, attitudes, and behaviours among children and adults; (3) to explore the association among sociodemographic characteristics, oral health behaviours, and the oral health status of Chinese children and adults. A national oral health survey was conducted among a representative sample of Chinese children and adults. Local residents from the five age groups - 3 to 5 years, 12 to 15 years, 35 to 44 years, 55 to 64 years, and 65 to 74 years - were selected. A multi-stage cluster sampling method was adopted for the identification of participants. All 31 provinces, autonomous regions and municipalities of the mainland of China were included. All participants were clinically examined and information on their oral health status, including oral mucosal lesions, dental caries experience, periodontal health status, dental fluorosis, and dental prosthesis status, was collected according to the procedures and criteria recommended by the World Health Organization (WHO). A calibration training programme and quality control procedures were conducted to ensure the reliability of the findings. Information on sociodemographic background, oral health behaviours, knowledge and attitude was also collected from all five age groups through a questionnaire survey.

Abstract:The principal problems of healthcare services in China are “difficulty in seeing a doctor”and “high expense of getting medical service” (commonly known in Chinese as “kan bing nan, kan bing gui”). The central Chinese government has already launched the bottom-up cascading medical system and two-way referral system recently in order to solve these problems (1). Only when patients go to medical institutions in an orderly fashion, can we see the hope of breaking the kan bing nan, kan bing gui (2). However, we face a number of obstacles when implementing the referral policies. The biggest obstacle is the lack of Human Resource (HR) for primary care both in capacity and volume (3). The central Chinese government has launched a series of policies to deal with the shortage of HRs in rural areas. Profound measurements involve postgraduate training for General Practitioner (GP) (a three-year plan beginning in 2010 for producing health professionals for rural areas and improving rural retention), “3 2” medical education model (3-year diploma education and 2-year postgraduate GP training), and in-service training for physicians in rural areas (4). It is not the time to assess their effectiveness, however, these measurements are certain to improve the capacity of Community Health Service (CHS) institutions.

Abstract:To understand the oral health status of urban and rural residents in China, the Fourth National Oral Health Epidemiological Survey (FNOHES) was conducted in 2015. Based on the principles of multi-stage, stratified, equal capacity, and random sampling, the subjects included five groups of residents aged 3–5, 12–15, 35–44, 55–64, and 65–74 years in all 31 provinces, cities, and autonomous regions except Hong Kong, Macao, and Taiwan. The total sample size was 172,425. The survey covered two areas, namely oral health status and knowledge, attitude and oral health behavior. Residents in China had serious caries and poor periodontal condition. The prevalence of caries was above 50% in all age groups, subjects without periodontal disease accounted for <50%, and fewer than 50% of subjects brushed their teeth twice a day. Based on the results of the FNOHES, the following suggestions were proposed for improving the oral health in China in the future: to establish and improve the comprehensive prevention and control system for oral diseases, implement a comprehensive prevention and control strategy for priority groups, strengthen the oral health education for all people to improve oral health literacy, enhance dynamic monitoring to scientifically evaluate oral health status, and coordinate multi-party resources to establish and improve the oral health service security system. The oral health status of Chinese residents and their oral health behavior habits need to be improved. With the support of the administrative departments of the government and unremitting efforts of the oral health staff, all sectors of society must be coordinated to strengthen oral health education for the whole population.
