Abstract:Vaccination against hepatitis B virus (HBV) is recommended worldwide. The aim of this study was to assess the efficacy of infant hepatitis B vaccination and revaccination in 0- to 8-year-old children in the context of protective anti-HBs levels and cellular immune responses. Using a random questionnaire survey, 1695 pre-school children were recruited as research subjects during January 2015 to June 2017. Blood samples were obtained to measure HBV serological markers as well as peripheral immunocytes. The children were divided into non-, low- and hyper- responsive groups (NR, LR, and HR) based on the vaccination efficacy. Additionally, the effect of revaccination on the NR group was evaluated at 1 month after completion of the vaccination course. Among a total of 1695 children, 1591 (93.86%) were infants who were followed while undergoing their primary course of hepatitis B vaccination at the 0-1-6 month schedule, and 1249 (79.30%) of them developed antibodies against HBsAg (anti-HBs) titers greater than 10 IU/L. The results of immunocyte studies indicated that the CD8+ T cells, CD4+CD45RO+ T cells, CD8+CD45RA+ T cells, and T follicular helper (Tfh) cells increased significantly in NR compared with HR. However, lymphocytes, CD4+ T cells, and CD4+CD45RA+ T cells in NR were lower than that in HR. 96 of the non-response cases showed seroprotection after revaccination among 103 cases. Therefore, most of the preschool children who received hepatitis B vaccine in infancy achieved significant seroprotection. Seroconversion rates of individuals revaccinated after initial vaccination failure were significantly higher than those after primary vaccination. Different vaccination efficacy groups showed significant changes in circulating immunocytes, which might be a factor affecting the recombinant HBV vaccine's immune effectiveness.

Abstract:The aim of our study was to compare the efficacy of two dosages of hepatitis B immunoglobulin (HBIG) combined with HBV vaccine (HBVac) to prevent mother-to-child transmission (MTCT) of hepatitis B in HBsAg- and HBeAg-positive mother. We enrolled 331 mother-infant pairs with HBsAg- and HBeAg-positive maternal state from the Women's Hospital School of Medicine of Zhejiang University. Newborns were randomly distributed into two groups according to the dosages of HBIG injection: 100 IU and 200 IU. Newborns from both groups were injected with HBVac in the same doses. We compared the immune outcomes between the two groups and explore the influencing factors of immune outcomes through regression analysis. There was no statistically significant relationship between HBsAg serological transmission of newborns and dosages of HBIG in HBsAg- and HBeAg-positive mother (p > .05). The Logistic regression showed that high DNA load is a risk factor for passive-active immunoprophylaxis failure for both 100 IU and 200 IU group, but higher-dosage HBIG is not necessary for higher-viral-load pregnant women with HBsAg- and HBeAg-positive. In conclusion, combined application of HBVac and a single dose of 100 IU HBIG can achieve the ideal MTCT interruption results for HBsAg- and HBeAg-positive pregnant women.IMPACT STATEMENT What is already known on this subject? Passive-active immunoprophylaxis is proved to be effective in preventing mother-to-child transmission of hepatitis B. Hepatitis B vaccine combined with 100 IU or 200 IU immunoglobulin is mostly recommended in China. What do the results of this study add? At present, there is still a lack scientific basis for improving existing strategies and measures to prevent mother-to-child transmission of hepatitis B in China. What are the implications of these findings for clinical practice and/or further research? 100 IU and 200 IU immunoglobulin show equivalent blocking effect, and combined use of hepatitis B vaccine and 100 IU immunoglobulin is more cost-effective.

Abstract:Objective To study the efficacy of the combination of 10μg yeast-recombinated hepatitis B vaccine (HepB) and hepatitis B immunoglobulin (HBIG) administrated to infants born to hepatitis B surface antigen (HBsAg)-positive mothers.Methods Investigation on blocking HBV mother-to-child transmission of children born to HBsAg-positive mothers with a combination of HBIG and 10μg HepB was conducted in Haidian District,Beijing,2013-2015.Data,including demographic information,the status of mother's hepatitis B e antigen (HBeAg) before delivery,and information of vaccination,such as the time of HBIG and of HepB,the type of hospital of birth,the dosage of HBIG,et al,were collected through questionnaires.5 ml blood samples of the children's were collected and then separated into the serum.Serological samples were tested for antibody to HBsAg (anti-HBs),HBsAg,and antibody to hepatitis B core antigen (anti-HBc).The positive rates of HBsAg,anti-HBc and anti-HBs were calculated and compared for different children.Results A total of 377 children born to HBsAg-positive mothers were included.Of these children,344 (91.25％) was positive for anti-HBs,33 (8.75％) was negative for antiHBs.The univariate analysis showed the positive rate of anti-HBs was significantly different for the children with the intervals between the time of administration of HepB3 and blood samples collected (x2 for corrected =30.432,P ＜ 0.001).The multiple logistic analysis showed children administrated different doses of HBIG (P =0.018) and with the intervals between the time of administration of HepB3 and blood samples collected (P ＜ 0.001) were significantly associated the anti-HBs positive or not.Compared with the children administrated 100IU HBIG,the children administrated 200IU HBIG were significantly less likely positive for anti-H Bs (OR =0.352,95 ％ CI:0.148 ～ 0.834).And compared with the children with the intervals between the time of blood sampled and administration of HepB3 ＜ 3 years,children with the intervals ≥ 3 years were significantly less likely positive for anti-HBs (OR =0.119,95％ CI:0.054 ～ 0.262).Conclusion Combination of HBIG and 10 μg HepB can effectively block mother-to-child transmission of HBV for children born to HBsAg-positive mothers.The levels of antiHBs declined over time,particularly 3 years after the administration of HepB3.Postvaccination serologic testing for infants born to HBsAg-positive mothers,especially for both HBsAg-and HBeAg-positive mothers,is important to determine the status of the infants for HBV biomarker,and for the HepB booster.
A Long-term Follow-up of Hepatitis B Vaccine Inoculated and HB Immunoglobulin Combinely Immunized Infants Born to HBsAg Positive Mothers

Abstract:In order to investigate the occurrence of chronic hepatitis B virus(HBV) infection in HB vaccine inoculated infants born to HBsAg positive mothers and its affecting factors and also to study the effect of combined use of HB immunoglobulin (HBIG) and HB vaccine on the blocking of perinatal transmission of HBV, we divided 624 infants born to HBsAg positive mothers into two groups randomly. The first group of infants were simply vaccinated with HB vaccine 20μg at,0, 1, 6 months old. The second group of infants were inoculated with HBIG 100 IU at their birth and further vaccinated with HB vaccine 20μg at 1,2,7 months old. The HBsAg of the two groups of infants was tested at the age of 1,3,6,12,18,24 months old and every year by EIA. They were all followed up for 6.34±1.71 years averagely. The follow-up results showed that there were 89 childeren infected chronically by HBV, among them 82% appeared to be HBsAg positive, firstly seen before the age of 6 months mainly attributing to uterus infection. The HBV chronic infection rate of infants born to HBsAg, HBeAg positive mothers (HB vaccine immunized, 24.5%; HBIG+HB vaccine, 10.6%, P<0.005) was higher than that of infants born to HBsAg positive mothers (HB vaccine immunized, 10.7%; HBIG+HB vaccine 3.8%, P<0.025). Moreover, the chronicity rate of the infants whose HBsAg firstly appeared within 6　　months and were born to HBsAg, HBeAg positive mothers was higher than that of the infants born to HBsAg positive months (68%,40% respertively in HB vaccine inoculated group, P<0.025). Infants immunized combinely with HBIG and HB vaccine had their chronic HBV infection decreased from 17.6% to 5.0% at l year old as compared with that of HB vaccine inoculated infants. The total chronic HBV infection rate in children born to HBsAg, HBeAg positive mothers decreased from 24.5% to 10.6% (P<0.005) and that in children born to HBsAg positive mothers decreased from 10.7% to 3.8% (P<0.025). The HBV chronicity rate of HBsAg positive infants born to HBsAg, HBeAg positive mothers also decreased and that of infants born with HBsAg negative, but became positive after 1-6 months decreased from 40.9% to 0% (P=0.0315). The HBsAg seronegative conversion rate in l month old infants born to HBsAg positive mothers increased from 26.4% to 58.8% (P<0.025). All these data indicated that infants born to HBsAg positive mothers, though had been HB vaccine immunized, still had chronic infection, that was chiefly due to chronic uterus infection by HBV. The single use of HB vaccine can not completely block the uterus infection of HBV, but when combinatively used with HBIG, the blocking effect is more efficient, the chronicity of HBV infection also can be avoided.
The antibody persistence after immunisation with hepatitis B vaccine combined with different doses of hepatitis B immunoglobulin among infants born by HBsAg positive mothers: a three-year following-up study

Abstract:Objective To provide a scientific basis for making booster immunization strategy and explore the antibody persistence of immunization with hepatitis B vaccine (YDV,10μg,at the age of 0,1,and 6 months) combined with different doses of hepatitis B immunoglobulin (100IU or 200IU) in 3 years among infants born by hepatitis B surface antigen positive mothers.Methods To observe the dynamic change and persistence of antibody among infants,titers of the antibody against hepatitis B surface antigen (anti-HBs) were detected respectively among children at the age of 7 months,1,2 and 3 years after the primary vaccination.Results Children's no,low,normal and high antibody response rates were 0.76％,5.34％,38.17％,53.18％ (2.54％ infected with HBV) by 7 months,which changed to 25.13％,44.67％,18.73％ and 7.68％ (3.79％ infected with HBV) respectively by 3 years old,the difference was statistically significant (P ＜ 0.001).The rate of low and cumulative no antibody response rate was 69.80％ at the age of 3 years.The GMC of anti-HBs dropped to 59.06m IU/ml by three years old from 997.56mIU/ml at the age of seven months (P ＜ 0.001) in the following-up periods.There were three new HBV infections by three years old among the children (3/429),with low,normal or high antibody response to hepatitis B primary vaccination respectively at the age of 7 months.Conclusion The study results suggest that serological markers of HBV should be tested on a regular basis among children born by hepatitis B surface antigen positive mothers,and after 1 or 2 years,immunity should be strengthened with supplementary HBV vaccination.

Abstract:This study was aimed to evaluate the efficacy and immunogenicity of combined hepatitis B vaccination with plasma-derived vaccine (PDV) and recombinant vaccine (RV). A total of 329 infants was recruited, including 224 high-risk infants born to hepatitis B e antigen-positive mothers and 105 low-risk infants born to hepatitis B e antigen-negative mothers. The high-risk infants received four doses of hepatitis B vaccine at 0, 1, 2 and 12 months of age with five different schedules. Group A1 and A2 infants were vaccinated with PDV as the first dose and RV (SB vaccine for group A1, MSD vaccine for group A2) as the remaining three doses. Group B1 and group B2 infants were vaccinated with PDV as the first two doses and RV (SB vaccine for group B1, MSD vaccine for group B2) as the remaining two doses. Group C infants received four doses of PDV. Low-risk infants were vaccinated with PDV at birth, and RV at 1 and 6 months of age (group D1, using SB vaccine; group D2, using MSD vaccine). At completion of vaccination schedules, 20 of 224 high-risk infants (9%) were positive for hepatitis B surface antigen. The overall protective efficacy was 90%. Hepatitis B surface antibody (anti-HBs)-positive rate ranged between 94 and 100% among the remaining infants. The protective efficacy and immunogenicity were similar among groups except that the mean level of anti-HBs in group C, D1 and D2 infants tended to be lower than that of the other four groups. To ensure an optimal immune response, four doses of vaccine are recommended in high-risk infants when two types of vaccine are to be used in combination.

Abstract:Background/aims: A birth dose of hepatitis B immunoglobulin (HBIG), in combination with hepatitis B vaccine (HepB), is recommended for infants born to hepatitis B surface antigen (HBsAg)-positive mothers. However, the optimal dosage of HBIG remains to be resolved. This prospective cohort study aimed to compare the efficacy of two dosages of HBIG combined with HepB to prevent mother-to-child transmission (MTCT) of HBV. Methods: From 2009 to 2011, we prospectively enrolled mother-infant pairs with positive maternal HBsAg in China. Infants were assigned to receive one dose of 100 IU or 200 IU HBIG within 12 h of birth according to maternal numbering, followed by completion of the 3-dose 10 mu g HepB series. At 7 months, post-vaccination serologic testing (PVST) was performed in 545 and 632 infants in 100 IU and 200 IU HBIG groups, respectively, among whom, 451 and 529 were followed up to 12 months. Results: Maternal and birth characteristics were comparable between infants in 100 IU and 200 IU HBIG groups. At 7 months, the rates of perinatal infection were 1.5% (8/545) and 1.9% (12/632) in 100 IU and 200 IU HBIG groups, respectively (p = .568). One non-responder infant in 200 IU HBIG group became newly infected at 12 months. The antibody to hepatitis B surface antigen (anti-HBs) positive rates were 98.5% (529/537) and 98.2% (609/620) in 100 IU and 200 IU HBIG groups at 7 months, respectively (p = .704), and the corresponding figures were 98.2% (431/439) and 97.1% (496/511) at 12 months (p = .266). The anti-HBs geometric mean concentrations were comparable between two groups at 7 months (707.95 mIU/mL vs. 602.56 mIU/mL, p = .062) and 12 months (245.47 mIU/mL vs. 229.09 mIU/mL, p = .407). Conclusions: One birth dose of 100 IU HBIG, combined with the HepB series, might be enough for preventing MTCT of HBV in infants born to HBsAg-positive mothers. (C) 2017 Elsevier Ltd. Abstract:Background: Despite the use of hepatitis B (HB) vaccine and hepatitis B immunoglobulin (HBIG), a portion of infants are still non-or low-responders, or even immunoprophylaxis failure. We aimed to determine the immune response in the infants from the mothers being positive for hepatitis B surface antigen (HBsAg), by which the infants received three doses of HB vaccine in combination with two-dose 200 IU HBIG injections.Methods: In this retrospective study, 621 infants from HBsAg-positive mothers in Beijing YouAn Hospital between January 2008 and December 2009 were included. All the infants were given three doses of 10 mu g HB vaccine (at 0, 1 and 6 months of age) and two-dose of 200 IU HBIG (at birth and in 2 weeks of age). Serum HBsAg and antibody to HBsAg (anti-HBs) in all the infants were determined at 7 months of age.Results: Of the 621 infants, 2.9% were immunoprophylaxis failure (positive for HBsAg), 1.4% were non-responders (anti-HBs undetectable), 95.7% were responders. The 594 responders could be categorized into three subsets, 22 were 10 to 99 IU/L for anti-HBs levels, 191 were 100 to 999 IU/L, and 381 were >= 1000 IU/L. The immunoprophylaxis failure rate was at 0% and 5.2% for the infants of HBeAg-negative and HBeAg-positive mothers(P<0.001). Infants from mothers with detectable HBV DNA had higher incidence of immunoprophylaxis failure than those of mothers without detectable HBV DNA (P = 0.002). The factors including gender, birth weight, gestation weeks, the rates of maternal HBeAg-positive, and detectable HBV DNA did not contribute to the no response to HB vaccination.Conclusions: Through vaccination by three doses of HB and two-dose of HBIG, majority of the infants (95.7%) achieved a protective level of anti-HBs at 7 months of age. Maternal HBeAg-positive and HBV DNA detectable were associated with the immunoprophylaxis failure, but not contribute to the non-or low-response to HB vaccination.

Abstract:Purpose: Mother-to-child transmission (MTCT) is the main mode of spread of hepatitis B virus (HBV), worldwide. The authors perfonned a meta-analysis to compare the effects of three measures for prevention of MTCT. Materials and Methods: Maine (PubMed) was searched and experts were contacted for references. Odds ratio (OD) and 95% confidence intervals (CI) were used as measures of effect sizes. Results: A meta-analysis was performed on randomized controlled trials and non-randomized studies comparing the index of MTCT among five groups of pregnant women. Among infants born to I ffisAg-positive mothers, there were no significant diffizences in anti-HBs positive rates between the vaccine-only group and combined-immunization group by age 8-12 months. In infants aged 7-15 months and infants aged 16-24 months, anti-HBs positive rate was 92.6% and 84.0% respectively. Immunoprophylaxis failure rate in mother's age < 28 group was significantly higher than that of & nbsp; >= 28 group, and that in vaccine-only group was significantly higher than vaccine plus HBIG group. Conclusions: HBIG and HB vaccine as an immune prophylaxis in infants born to HBV carrier mothers, beginning after birth, is effective in preventing MTCT of HBV.
The efficacy of immune interruptions on blocking mother-toinfant transmission of hepatitis B virus

Abstract:To investigate the efficacy and the roles of immune interruptions on blocking mother-to-infant transmission of hepatitis B virus (HBV),667 pregnant women who were seropositive for hepatitis B surface antigen (HBsAg) were enrolled. According to the lab test results of HBV e antigen (HBeAg) and HBV DNA in the mothers, the mothers were divided into either HBeAg-positive group or HBeAg-negative group, and either HBV DNA-positive group or HBV DNA-negative group. Depending on whether the mothers were receiving hepatitis B immunoglobulin (HBIG) once a month during the third trimester of pregnancy, they were divided into either the HBIG-receiving group or the non-HBIG-receiving group. All infants received regular passive and active HBV immunization after birth. Infant serum HBV markers were determined 8-12 months after birth. Twenty infants born to HBsAg-positive mothers were infected with HBV and the failure rate of immune interruptions was 3.0%. The failure rate of immune interruptions in the HBeAg-positive group was 8.7%, which was significantly higher than that observed in HBeAg-negative group 0.2% (P<0.001). However, the HBV surface antibody (HBsAb) positive rates of the infants between the two groups were not statistically different (P=0.988). The overall failure rate of immune interruptions of infants born to the HBV DNA-positive group was 8.1%, and the infants infected with HBV were all born to the mothers whose serum HBV DNA levels were above 6 log10 copies/ml. There were no statistical differences in the incidence of perinatal HBV infection between mothers receiving HBIG and those not receiving HBIG (3.7% vs 2.7%, P=0.479). No statistical differences were seen in HBsAb positive rates versus vaccination of the infants between the two groups (84.4% vs 82.4%, P=0.519). Among HBeAg-positive pregnant women receiving HBIG, the failure rate of immune interruptions in their infants was 8.4%, compared with 8.9% in those who did not receive HBIG, with no statistical differences (P=0.892). Importantly, 10 out of 11 infants who were infected with HBV were already serum HBsAg-positive immediately after birth, suggesting intrauterine infection either via venous or cord blood. These results indicate that preventative vaccine immunization plus HBIG administration does not block intrauterine transmission of HBV. Approaches to decrease the serum HBV DNA load in pregnant women by antiviral treatment may efficiently block mother-to-infant transmission.
Further Improving the Efficacy of HBV Vaccination in the Prevention of Mother-to-infant Transmission of Hepatitis B

Abstract:Perinatal transmission from mothers infected with HBV to their newborn infants is a major route of HBV infections in China.The risk of chronic HBV infection falls in a range of 70%-90% from mothers who are HBeAg-positive,and about 5%-20% from those who are HBeAg-negative.Previous studies have found that the blocking rate of mother-to-child transmission by hepatitis B vaccination is associated with the doses of hepatitis B vaccine(HepB)and/or hepatitis B immunoglobulin(HBIG).Our study showed that the median level of HBV DNA in the HBeAg-positive pregnant women was significantly higher than that in the HBeAg-negative group(median 8.06 vs 2.57 lg IU/ml,P0.001).The HBV protection rate of the infants immunized with HepB+HBIG was significantly higher than that of those immunized with HepB alone.However,there was no significant difference between the 100 IU HBIG group and the 200 IU HBIG group.The geometric mean concentrations(GMC) of anti-HBs in infants immunized with 10 μg Hansenula recombinant HepB(H-HepB) was higher than that in those accepted 5 μg Saccharomyces cerevisiae recombinant HepB(S-HepB).Moreover,the non/low-response rate of anti-HBs in the 10 μg H-HepB group was much lower than that in the 5 μg S-HepB or 10 μg S-HepB group.Within groups that were administered the same kinds of HepB,the higher the dose,the higher rate of anti-HBs response.Follow-up results of 7-month old infants showed that the HBsAg-positive rates were strongly correlated with the HBeAg status of their mothers,the HBeAg-positive mothers are more likely corresponding to failed prevention in their infants(higher HBsAg-positive rate) comparing to HBeAg-negative mothers.Therefore,HepB immunization strategy for newborns should be further optimized.We have recommended the increase of HepB dose from 5 μg to 10 μg,and the implementation of combined HepB+HBIG for all neonates whose mothers are positive in both HBsAg and HBeAg.
Follow-up study on relationship between hepatitis B e antigens and intrauterine HBV infection after immunoprophylaxis

Abstract:Objective To investigate the effect of maternal or neonatal HBeAg positive on the intrauterine infection of HBV and the production of anti-HBs in the infants.Methods 194 HBsAg-positive pregnant women were consecutively collected and investigated as subjects from ward of Maternal and Children Health hospital of Shaanxi province,and their infants were followed up.The relationship of maternal or neonatal HBeAg positive with intrauterine infection of HBV,chronicity and the production of anti-HBs in the infants were analyzed.Results Ten of 196 neonates were identified as occurrence of HBV intrauterine infection.The pregnant women's HBeAg positive and neonatal HBeAg positive were important risk factors of HBV infection in uterus(RR=4.76,5.53,respectively),and there were statistical significance(P<0.05).One infant who was HBsAg/HBeAg-positive at birth,born to HBeAg-positive mother,developed to HBsAg carrier.The difference of absorbency value of anti-HBs was no statistically significance between infants born to HBeAg-positive mothers and those born to HBeAg-negative mothers;and among infants of HBeAg-positive at birth or HBeAg-negative at birth during followed up at 1,4,7 months of age(P>0.05).Conclusion HBeAg may cross placenta freely,so it can't be used as an diagnosis indicators of HBV intrauterine infection.HBeAg in neonates or pregnant women played an important role in HBV intrauterine transmission and its chronicity,but not in the production of anti-HBs.

Abstract:Background: Vertical mother-to-child transmission (MTCT) of the hepatitis B virus (HBV) remains an important issue. Timely administration of hepatitis B immunoglobulin (HBIG) and of the HBV vaccine is effective in preventing MTCT in infants born to HBV-infected mothers. However, HBIG is often not easily available in low-income countries or regions. Methods: We compared in a retrospective cohort study the HBV vaccine efficacy alone and in combination with HBIG in preventing vertical MTCT in infants born to HBeAg-negative carrier mothers in Jiangsu province, China. Based on the administration of the HBV vaccine and HBIG shortly after birth, children were divided into two groups: Group 1, administration of the HBV vaccine alone, and Group 2, concurrent use of HBIG and of the HBV vaccine. Results: A total of 620 infants born to HBeAg-negative carrier mothers were enrolled into this study. Group 1 included 195 children who had received the HBV vaccine alone after birth, and Group 2, 425 children who had received both HBIG and the HBV vaccine. Children were followed up to the age of 68 and 42 months, respectively. MTCT of HBV occurred in 0% (0/195) in Group 1 (HBV vaccine alone) and 0% (0/425) in Group 2 (HBV vaccine and HBIG) (p = 1.00). Conclusion: In this retrospective cohort study, we found that HBV vaccination alone shortly after birth was effective in preventing MTCT of HBV in infants born to HBeAg-negative carrier mothers.

Abstract:Background: Combined immunization with hepatitis B immunoglobulin (HBIG) plus hepatitis B vaccine (HB vaccine) can effectively prevent perinatal transmission of hepatitis B virus (HBV). With the universal administration of HB vaccine, anti-HBs conferred by HB vaccine can be found increasingly in pregnant women, and maternal anti-HBs can be passed through the placenta. This study was designed to evaluate the effect of hepatitis B immunization on preventing mother-to-infant transmission of HBV and on the immune response of infants towards HB vaccine.Method: From 2008 to 2013, a prospective study was conducted in 15 centers in China. HBsAg-positive pregnant women and their infants aged 8-12 months who completed immunoprophylaxis were enrolled in the study and tested for HBV markers (HBsAg, anti-HBs, HBeAg, anti-HBe and anti-HBc). Antepartum administration of HBIG to HBsAg-positive women was based on individual preference. HBsAg-negative pregnant women and their infants of 7-24 months old who received HB vaccines series were enrolled and tests of their HBV markers were performed.Results: 1202 HBsAg-positive mothers and their infants aged 8-12 months were studied and 40 infants were found to be HBsAg positive with the immunoprophylaxis failure rate of 3.3%. Infants with immunoprophylaxis failure were all born to HBeAg-positive mothers of HBV-DNA >= 6 log(10) copies/ml. Among infants of HBeAg-positive mothers, immunoprophylaxis failure rate in vaccine plus HBIG group, 7.9% (29/367), was significantly lower than the vaccine-only group, 16.9% (11/65), p = 0.021; there was no significant difference in the immunoprophylaxis failure rate whether or not antepartum HBIG was given to the pregnant woman, 10.3% (10/97) vs 9.0% (30/335), p = 0.685. Anti-HBs positive rate was 56.3% (3883/6899) among HBsAg-negative pregnant women and anti-HBs positive rate was 94.2% in cord blood of anti-HBs-positive mothers. After completing the FIB vaccine series, anti-HBs positive rate among infants with maternal anti-HBs titers of <10 IU/L, 10-500 IU/L and >= 500 IU/L was 90.3% (168/186), 90.5% (219/242) and 80.2% (89/111) respectively, p = 0.011. Median titers of anti-HBs (IU/L) among infants in the three groups was 344.2, 231.9 and 161.1 respectively, p = 0.020.Conclusions: HBIG plus MB vaccine can effectively prevent mother-to-infant transmission of HBV, but no HBV breakthrough infection was observed in infants born to HBeAg-negative mothers who received HB vaccine with or without HBIG after birth. Antepartum injection of HBIG has no effect on preventing HBV mother-to-infant transmission. High maternal titer of anti-HBs can transplacentally impair immune response of infants towards HB vaccine. Study on the Blocking Effect of Injecting HBsAg Positive Mothers During Pregnancy and Their Infants at Birth with HB Immunoglobulin on Mother-to-infant Transmission of HBV

Abstract:Objective To study the effect of HB immunoglobulin (HBIG) on HBsAg positive mothers during the third trimester of pregnancy and combined use of HBIG and HB vaccine to infants on the blocking of vertical transmission of HBV and the status of hepatitis B resulting from mother-to-infant transmission. Methods A total of 645 HBsAg positive mothers were divided into two groups, group 1 without HBIG and their infants with three doses of plasma-derived vaccine given at 0, 1 and 6 months; group 2 with HBIG200IU respectively during the third trimester of pregnancy and their infants with HBIG200IU at birth and three 5 μg doses of recombinant vaccine given at 0, 1 and 6 months. All infants were followed up for the hepatitis B detection at the age of 1, 3, 6, 12, 24 months by enzyme-linked immunoadsorbent assay (ELISA). Results 9 infants which were infected chronically with HBV were all in group 1, among them 8 were HBsAg positive before the age of 6 months mainly attributing to intrauterine infection. No infants were infected chronically by HBV in group 2. The chronical infection rate of HBV of infants in group 1 was significantly higher than that in group 2, ( χ 2=4.008, P0.05). Conclusions Use of HB immunoglobulin (HBIG) to HBsAg positive mothers during the third trimester of pregnancy and combined use of HBIG and HB vaccine to infants could block the chronicity of HB resulting from intrauterine infection.
Protective effect of regular immunoprophylaxis on infants against perinatal hepatitis B virus infection

Abstract:Objective To assess the protective effect of vaccination in routine application on hepatitis B virus (HBV) exposed infants and to clarify whether hepatitis B immunoglobulin (HBIG) administration of pregnant women may reduce the risk of maternal-fetal transmission of HBV. Methods Serum samples of 6398 pregnant women at gestation of 15-20 weeks from 6 urban and 8 rural areas across Jiangsu province were previously tested for serologic markers of HBV by ELISA from July 2002 to August 2004. In this study, infants born to 419 HBV carrier mothers were taken as the study group, while infants born to 453 non-carrier mothers were taken as the control group by stratified random sampling. They were followed-up and screened for HBV markers during October 2009 to March 2010. Information including HBIG administration during pregnancy, HBV vaccination and HBIG administration of the infants were collected. χ2 test or Fisher′s exact method were used to compare the rates and the comparison of the means was by t test. Results The follow-up rates of the study group and control group were 71.12% (298/419) and 72.41% (328/453), respectively. Of the 298 infants born to HBV carrier mothers, 11 (3.7%) were positive for HBsAg, while none of the 328 infants born to non-carrier mothers was HBsAg positive (χ2=12.32, P<0.01). All of the 11 children were born to mothers with both HBsAg and HBeAg positive, and nine of the 11 children were not injected HBIG or not immunized with hepatitis B vaccine within 24 hours after birth, with only one received regular vaccination and detailed information was unknown in one case. The positive rates of anti-HBs in the study group and the control group were 69.46% and 69.21% respectively (χ2=0.01, P=0.95). HBsAg positive rate of the children born to pregnant women treated with HBIG during late pregnancy (n=92) was 2.17% (n=2), whereas that in the children born to women not treated with HBIG (n=197) was 4.57% (χ2=0.98, P=0.51). Conclusions The protective effect of immunoprophylaxis in routine application against perinatal HBV infection in Jiangsu province is good. Efforts are required to emphasize the importance of HBIG administration in infants born to HBV carrier mothers, especially in HBeAg positive mothers within 24 hours after delivery. Treatment of HBsAg positive pregnant women with HBIG in third trimester would not decrease the risk of maternal-fetal transmission of HBV.

Abstract:OBJECTIVE:To evaluate long-term efficacy of hepatitis B vaccination in newborns.METHODS:In the past 13 years we followed up the children who had been immunized with hepatitis B vaccine at birth in 1986 every other year. At each follow-up visit, every child was tested for HBsAg, anti-HBs and anti-HBc.RESULTS:HBsAg positive rate of these birth cohort was low (range between 0.46% and 0.98%) during 13 years' follow-up. Long-term efficacy of the vaccination in preventing HBsAg carriage was confirmed.CONCLUSIONS:Thirteen years later no booster was after needed.

Abstract:The aim of this study was to evaluate changes in hepatitis B surface antibody titers (anti-HBs) after booster vaccinations in children aged 5-15 y and to provide suitable immunization strategies. A total of 2208 children were initially enrolled in screening, and 559 children were finally included. The participants were divided into 2 groups according to their pre-booster anti-HBs levels: Group I, <10mIU/ml and Group II, 10mIU/ml. Group I was administered 3 doses of booster hepatitis B vaccine (0-1-6months, 10g), and Group II was administered 1 dose of booster hepatitis B vaccine (10g). The antibody titer changes were examined at 4 time points: 1month after dose 1 and dose 3, and 1year and 5years after dose 3. The protective seroconversion rates at those points were 95.65%, 99.67%, 97.59% and 91.05% (p < 0.001), respectively, in Group I, and 100.00%, 99.87%, 99.66% and 98.21% ((2) = 6.04, p = 0.11), respectively, in Group II. The GMT in subjects aged 5-9y were higher than that in subjects aged 10-15y in both Group I and Group II at 1month after dose 1, but no difference was observed at the other three time points. This study demonstrates that booster vaccination has a good medium-term effect. A booster dose for subjects with protective antibodies is not necessary but effective, and 3 doses of hepatitis B vaccination are recommended for those who have lost immunological memory. Receiving booster immunization at the age of10-15years may be more appropriate for individuals living in HBV high epidemic areas

Abstract:Objective evaluate the long-term efficacy of the recombinant hepatitis B vaccines (HBV) among the newbornswith vaccination at birth. Methods During 1996-1997, 135 newborns were selected from Deqing according to the inclusioncriterion. They were divided into 2 groups: a group of 35 newborns whose mother was HBsAg positive) and a group of 100newbornswhose mother was HBsAg negative. All 135 newborns routinely received 3 doses of yeast -derived hepatitis Bvaccines (i.e. the first dose at birth, the second dose at 1 month old, and the third dose at 6 months old) . Serologicalmarkers to HBV were repeatedly assessed at 3 follow-up stages (i.e. the first follow-up at 12 months, the second follow-upat 2010, the third follow-up at 2012) . Results Participants remained in the study at 3 follow-upstages were 123(91.11%), 95(70.37%) and 46(34.07%) respectively. Participants' serum HBsAg were negative at all 3 follow-upstages. Among participants whose mothers were HBsAg positive, 3 participants were found to be HBcAb positive in 2010,and no new HBcAb positive participants were found in 2012. The rates of HBsAb positive at 3 follow-up stages were 88.89%, 81.48%, and 80.00% respectively. The HBsAb geometric mean concentrations (GMCs) of participants at their 12 monthsold were significantly positively associated with those in 2010 and those in 2012(P<0.05) . Among participants whosemothers were HBsAg negative, no HBcAb positive participants were found. The rates of HBsAb positive at 3 follow-up stageswere 91.18%, 54.41%, and 52.78% respectively. No correlation was found among HBsAb GMCs of participants at 12 monthsold, in 2010 and in 2012. No correlation was found between boost vaccination and the rate of HBsAg positive, afteradjustment of the HBsAg status of their mothers. Conclusion The efficacy of the yeast-derived HBV could sustain for at least13-15 years, and the general population do not need booster immunization. After the 3-dose immunization, the HBsAblevels of the healthy mothers' 12-months-old children were related to those of their adolescence.

Abstract:Objective To investigate the dynamic changes in hepatitis B virus serologic markers(HBVM) in infants born by HBsAg-carrying mothers,who were immunized with high-titer immunoglobulin(HTIG) and hepatitis B vaccine,and evaluate the clinical and epidemiological significance thereof.Methods Two hundred and nine HBsAg-carrying pregnant women and their 213 newborns(4 twins) were consecutively involved from Dec.2002 to Oct.2004,and followed up when infants were at 1,7,12,24 and 36 months of age.HBsAg,anti-HBs,HBeAg,anti-HBe and anti-HBc in neonatal sera were detected by ELISA.Results One hundred and eighty-two infants(85.45%) born of HBsAg-positive mothers were followed up.Of them 13 were HBsAg positive at birth.At the end of follow-up,all of them changed negative except two cases who were persistently positive for HBsAg.No one in 169 infants who were HBsAg-negative at birth changed to HBsAg-positive.About 57.69%(30/52) neonates born by both HBsAg and HBeAg positive mothers showed detectable levels of HBeAg.Similar to HBsAg,HBeAg was detectable in only 2 infants who were also HBsAg positive after 7 months of age,and all the other infants became HBeAg-negative.The anti-HBs positive rate of newborn infants of HBsAg-carrying mothers was 6.70%(12/179).After immunizing by HTIG and hepatitis B vaccine,the anti-HBs positive rate reached 82.46%(47/57) at 1 month of age and 91.77%(145/158) at 7 month of age.73.68%(28/38) of infants aged of 36 months showed detectable levels of anti-HBs.The anti-HBe positive rate of infants aged 1 month was 49.12%(28/57),and the anti-HBc positive rate was 100%(57/57).All infants became anti-HBe-negative at age of 24 months.The anti-HBc positive rate was still 84.81%(134/158) at 7 months of age and decreased to 21.05%(8/38) at 36 months of age.Conclusions The efficacy of HTIG combining with hepatitis B vaccine is fine in high risk infants born by HBsAg positive mothers.HBeAg,anti-HBe and anti-HBc can pass through human placenta from mother to fetus passively.
