Abstract:Purpose To generate microplasmin ( μ Plm) using recombinant microplasminogen ( μ Plg) and recombinant tissue plasminogen activator (rt-PA) before intravitreous injection and to investigate the efficacy of μ Plm in inducing posterior vitreous detachment (PVD). Methods Forty-eight female or male New Zealand white rabbits were randomized into three groups. Recombinant human μ Plg was incubated with rt-PA with a 200:1 molar ratio at 37°C for 40 min. The right eyes of groups 1, 2, and 3, were injected with 0.5, 1.0, and 1.5 U μ Plm in 0.1 ml respectively, and 0.1 ml balanced salt solution (BSS) was injected into the left eye as controls. Scanning electron microscopy (SEM), gross specimen examination, B-ultrasonography and optical coherence tomography (OCT) were performed to detect vitreoretinal interface. Results Over eighty percent of recombinant human μ Plg could be activated to active μ Plm by rt-PA after 40 min incubation. Complete PVD was found at vitreous posterior pole of μ Plm-treated eyes without morphological change of retina. Complete PVD of 25, 75, and 87.5% rabbit eyes was induced by 0.5, 1.0 and 1.5 U recombinant μ Plm respectively on day 1. The remnants of vitreous cortex at the posterior pole were dependent on the concentration of μ Plm. Among the four approaches for detecting PVD, SEM, gross specimen examination, and B-ultrasonography were more effective methods than OCT. Conclusion Intravitreous injection of 1.5 U μ Plm can effectively induce complete PVD in rabbit eyes on day 1 without morphological change of retina.
A novel recombinant human microplasminogen induced complete posterior vitreous detachment without morphological change of retina in juvenile rabbits

Abstract:Vitreomacular traction syndrome results from persistent vitreoretinal adhesions in the setting of partial posterior vitreous detachment (PVD). Vitrectomy and reattachment of retina is an effective therapeutic approach. The adhesion between vitreous cortex and internal limiting membrane (ILM) of the retina is stronger in youth, which brings difficulties to induce PVD in vitrectomy. Several clinical investigations demonstrated that intravitreous injection of plasmin before vitrectomy could reduce the risk of detachment. In our study, a novel recombinant human microplasminogen (rhμPlg) was expressed by Pichia pastoris. Molecular docking showed that the binding of rhμPlg with tissue plasminogen activator (t-PA) was similar to plasminogen, suggesting rh μPlg could be activated by t-PA to generate microplasmin (μPlm). Moreover, rhμPlg had higher catalytic activity than plasminogen in amidolytic assays. Complete PVD was found at vitreous posterior pole of 125 μg rhμPlg-treated eyes without morphological change of retina in juvenile rabbits via intraocular injection. Our results demonstrate that rhμPlg has a potential value in the treatment of vitreoretinopathy.

Abstract:Purpose : To realize the high production of recombinant human microplasminogen (r-mPlg) with Pichia pastoris and demonstrate the efficacy of r-mPlg in inducing posterior vitreous detachment (PVD). Methods : Recombinant plasmid pAO815-3mPlg was constructed and transformed into SMD1168 cells. Positive recombinant clones were selected with MD plate and cultured in BMG medium, then induced in BMM medium. A protein band corresponding to mPlg with molecular mass of 29 kDa was revealed in SDS-PAGE and confirmed by Western blot. Anion-exchange chromatography and plasminogen activity assay kit were used to obtain purified r-mPlg with biological activity. Twenty eyes of freshly slaughtered pigs were divided into 4 groups, 5 eyes in each group. Group 1 served as normal control. Intravitreal injection of 0.1 ml BSS, 1000 IU/0.1 ml recombinant streptokinase (r-SK) and 1000 IU/0.1 ml r-SK plus 3 U/0.1 ml r-mPlg was done respectively to groups 2, 3, and 4. After incubation at 37 degrees C for 60 min, all eyes were processed for light microscopy, scanning electron microscopy (SEM), and transmission electron microscopy (TEM). Results : r-mPlg, which has potential fibrinolytic activity, was successfully obtained with yield of 30 mg/L and purity of 97%. PVD was demonstrated by SEM in group 4 but not in other three groups. The retina and the inner limiting membrane (ILM) were well preserved in all eyes. Conclusion : r-mPlg, which has potential fibrinolytic activity, can be produced through Pichia pastoris expression system. Three U of r-mPlg combined with 1000 IU r-SK was effective in producing PVD without damaging the retina.
The Effects of Plasmin Kringle Domains Deficiency Mutant on the Interface of Vitreous and Retina in Rabbit Eyes

Abstract:Background The vitreoretinal traction plays a critical role in the formation of macular hole and cystoid macular edema.Enzymatic vitreolysis has potential in relieving vitreoretinal traction as a simple and less invasive method in comparison with pars plane vitrectomy.ObjectiveThis study is to investigate the effects of plasmin mutant with kringle domains deficiency(Plm△K)on vitreoretinal interface in new Zealand white rabbits.Methods Plm△K was prepared through activating plasminogen mutant with Kringle domains deficiency (Plg△K) by tissue plasminogen activator (tPA).100μL of Plm△K at the dose of 0.5,1.0 and 1.5μmol/min was injected respectively into the vitreous of 48 New Zealand white rabbits and 16 eyes for each dose.B-scan and optical coherence tomography (OCT) were performed to detect the structure variety at the vitreoretinal interface in 1 day and 7 days after injection.The gross anatomy analysis with triamcinolone acetonide fine particle suspension,as well as histopathological examinations by scanning electron microscopy,was performed in the different time points mentioned above.Results Two peptide chains were determined with the relative molecular weight about 26000 and 5000 by the gel imaging analysis of reduced SDS-PAGE.Separation of the posterior vitreous cortex from retina was found after intravitreous injection under the B-scan and OCT.The ultrastructure change of vitreoretinal interface as well as the examination of fine particle suspension by triamcinolone acetonide demonstrated the same outcome.The degree of remnants of vitreous cortex showed the negative correlation with the dosage of Plm△K (r=-0.9516,P=0.048).No significant correlation was found between the degree of remnants of vitreous cortex and the action time(r=-0.720,P=0.470).There was no obvious morphological difference in outer layer of retina between control eyes and Plm△K-treated eyes.No drug-related adverse event was found after intravitreous injection of Plm△K.Conclusion Intravitreous injection of Plm△K alone can induce complete separation of vitreous from retina.This procedure is safe and effective. Abstract:Purpose: To determine whether intravitreal administration of microplasmin (mu Plm) will degrade fibronectin (FN) and laminin (LN) in rat retina during mu Plm-induced posterior vitreous detachment (PVD). Methods: Increasing doses of mu Plm, from 0.01U to 0.03 U, were injected into the left eyes of 60 Sprague-Dawley rats to induce PVD. The right eyes were injected with the same volume of balanced salt solution (BSS). Histochemistry, scanning electron microscopy (SEM), and phase contrast microscopy were performed after 1 day and 7 days, to assess the remnant vitreous cortex. The FN and LN level located at the vitreoretinal interface and the outer retina were detected by immunohistochemistry. Results: mu Plm induced complete PVD in a dose-dependent fashion, without internal limiting membrane (ILM) damage (P=0.0001, r=-0.479). The FN and LN in the photoreceptor cell layer (PCL) were completely degraded in all mu Plm-treated eyes. In eyes with complete PVD, the FN, but not the LN, was completely removed from the ILM by mu Plm treatment. Conclusion: Intravitreal injection of microplasmin degraded FN and LN at the vitreoretinal junction as well as at the outer retina.
Posterior Vitreous Detachment with Plasmin in the Isolated Human Eye

Abstract:. Purpose: To investigate the efficacy and safety of intravitreal injection of plasmin in producing posterior vitreous detachment (PVD), and the possible role of plasmin in degrading adhesion glycoproteins of the internal limiting membrane (ILM). Methods: A total of 20 young human cadaver eyes were randomized into four groups. Groups 1, 2, and 3 were injected with 1, 2, and 3 U of human plasmin, respectively, and group 1 was injected with 0.1 ml balanced salt solution as control. Optical and transmission electron microscopy were used to examine the ultrastructure of the vitreoretinal interface. The amounts of fibronectin (FN) and laminin (LN) in the ILM were determined using electron-immunocytochemical techniques. The survival of retinal cells was evaluated using flow cytometry. Analysis of variance (ANOVA) and the Tukey test were used for statistical analysis. Results: Microscopy showed that plasmin, especially in group 4, cleaved the attachment of the vitreous collagen fibrils to the ILM. There was no evidence of damage to the inner retina except some post-mortem lytic changes. The amounts of LN and FN in the ILM were decreased following injection of plasmin. There were statistically significant differences in the amounts of FN in groups 3 and 4 compared to the control group ( P <0.05), and in the amount of LN in group 4 ( P <0.05). Retinal cell viability was similar in both treated and control eyes. Conclusion: Human plasmin can disrupt the attachment of the posterior hyaloid to the ILM without producing morphological changes or acute toxicity of the inner retina of the human eye. PVD can be induced completely by injection of 3 U plasmin. The enzyme may be useful for removing the cortical vitreous during vitreous surgery.
A pilot study on purification of human plasminogen from cord blood plasma and inducing posterior vitreous detachment

Abstract:Background Many studies and clinical trials of pharmacologic vitreolysis are already under way to try to improve vitreo-retinal surgery and to liquefy and detach the vitreous from the retina ultimately, including chondroitinase,hyaluronidase,dispase and plasmin. However, there has not been any report on purification of human plasminogen from cord blood plasma and inducing posterior vitreous detachment of the animal eye at present.Objective This study was designed to isolate and purify the production of human plasminogen (Plg) from cord blood plasma with ethanol precipitation and evaluate the efficacy of Plg in inducing posterior vitreous detachment (PVD).Methods Human Plg was Separated and purified from cord blood plasma by ethanol precipitation method. The protein band corresponding to Plg with molecular mass of 92 000 was revealed in SDS-PAGE and confirmed by MALDI-TOF and Mascot database. Anion-exchange chromatography and plasminogen activity assay kit were used to obtain purified Plg with biological activity. Twenty-five fresh pig eyes were enucleated and assigned to 5 groups and 5 eyes for each group. The normal eyes were used as control group. Balanced salt solution(BSS)of 0.1 ml was intravitreally group and standard substance group. All of the eyes were then incubatedfor 60 minutes under the 37 ℃. Retinal histopathology and ultrastructure were examined under the light microscopy, scanning electron microscopy ( SEM ) and transmission electron microscopy (TEM). Results The Plg with potential fibrinolytic activity was successfully extracted and purified from cord blood plasma by ethanol precipitation method. No posterior vitreous detachment (PVD) was seen in normal control group, BSS group and r-SK group following the intravitreal injection under the sem. However,PVD was demonstrated in r-SK+ Plg group and standard substance group under the SEM. The inner limiting membrane ( ILM ) and the retina were well preserved in all of the experimental eyes. No retinal morphology and ultrastructural abnormality were found under the light and SEM and TEM. Conclusion Ethanol precipitation is a feasible way to isolate and purify Plg from human cord blood plasma. Extracted Plg shows potential fibrinolytic intravitreal injection of Plg.
Thrombolysis of Plasmin-Δk on Photochemically Induced Branch Retinal Vein Occlusion after Intravitreal Injection in Rats

Abstract:Background Retinal vein occlusion is a common retinal vascular diseases.Thromblysis and anticoagulation therapies are main approaches.However,systemic thrombolysis is relatively inefficient,and it often enhances the risk of hemorrhage.Objective This study was to investigate the therapeutic effects of PLM-ΔK,a kringle deficiency mutant of plasmin,on photochemically induced branch retinal vein occlusion (BRVO) after intravitreal injection.Methods BRVO models were established by the combination of caudal vein injection of Rose Bengal with argon laser radiation of periphery area of retinal veins in SD rats.Forty model rats were randomized into balance salt solution (BSS) group and 0.01 U,0.02 U,0.03 U PLM-ΔK group,and 10 μl corresponding drug was intravtreally injected 12 hours after modeling.Ophthalmoscopy and fundus fluorescein angiography (FFA) were performed to observe the change of retinal veins.The animals were sacrificed 3 days after intravitreal injection,and hematoxylin and eosin staining was used for the histopathological and ultrastructural examination of retinas.The retina of the rats was isolated for the stretched preparation of retina.The expressions of fibronectin (FN) and laminin (LN) in eyeball wall were assayed by immunofluorescence technology.The use and care of the animals complied with Statement of the Association for Research in Vision and Ophthalmology.Results The revascularization of over 2 retinal veins was found in 0,3,6 and 8 rats in the BBS group and 0.01 U,0.02 U,0.03 U PLM-ΔK group 3 days after intravitreal injection,respectively,showing a significant difference among the groups (x2=9.635,P =0.022),and the rat number with revascularization in 0.01 U PLM-ΔK group was not significantly different from that in BSS group (Z=-1.558,P =0.119),but the difference between 0.03 U PLM-ΔK group and 0.01 U PLM-ΔK group was significant (Z=-2.762,P=0.006).In the third day after intravitreal injection,retinal vein thrombus were found in the BSS group under the light microscope,and angiogenesis was seen on the retinal flatmount nuclear.In the 0.03 U PLM-ΔK group,posterior vitreal detachment was exhibited under the light microcope,and no retinal new vessel and cell damage were seen.FN was strongly expressed in the inner limiting membrane (ILM) layer,photocyte layer,outer limiting membrane (OLM) layer,choroid and scleral layer,and LN was expressed mainly in the ILM,OLM and scleral layer in the BSS group.However,the expression intensities of FN and LN were obviously weakened in the 0.03 U PLM-ΔK group.Conclusions Intravitreal injection of PLM-ΔK can enhance the reperfusion of occluded branch retinal vein and serve as a potential therapeutic drug for BRVO.Also it can permeate into choroid after intravitreal injection to degradate FN and LN.
Video Microscope Recording of the Dynamic Course of Thrombosis and Thrombolysis of the Retinal Vein in Rabbits.

Abstract:Purpose: The purpose of this study was to investigate the dynamic course of experimental thrombosis and thrombolysis of the retinal veins.Methods: Dynamic changes in the blood flow in the retinal veins were documented on a digital recorder through a microscope-mounted video camera and were analyzed on a monitor by video playback. Photochemical thrombus formation was induced by intravenous injection of 30% Rose Bengal followed by endoillumination of individual branch retinal veins of the eyes of 20 anesthetized pigmented rabbits. Subsequently, 10 rabbits were treated with an infusion in an ear vein of 3 mg/kg recombinant tissue plasminogen activator, whereas 10 control rabbits were administered with similar volumes of normal saline solution. Occlusion and recanalization were confirmed histologically and assessed by video microscopy.Results: At the site exposed to light, photochemical injury resulted in the formation of a white thrombus, stagnation of blood flow, and subsequent complete venous occlusion in 20 rabbits. In this study, of the 10 animals in the recombinant tissue plasminogen activator treatment group, 9 ( 90%) showed evident thrombolysis, whereas none of the control group animals showed evident thrombolysis. The video showed that the massive thrombus disrupted into nonuniform fragments, which were quickly washed away by the blood flow, and the circulation was reestablished with no recurrence of secondary obstruction.Conclusion: In vivo data suggest that video microscopy may provide a visual approach for observing the dynamic changes occurring during experimental thrombus formation and lysis by the early administration of recombinant tissue plasminogen activator; this approach may assist in future investigation of thrombus research of ocular fundus. [Experiment Study of Infusing Tpa in Retinal Vein for Treatment of Retinal Vein Occlusion].

Abstract:OBJECTIVE:To Investigate the effectiveness and complications of injection of tissue plasminogen activator into a retinal vein for treatment of retinal vein occlusion via micropuncture of the retinal vein.METHODS:Thirty miniature pigs (one eye of each pig) were produced photodynamic thrombi of retinal vein by injecting Rose bengal (20 mg/kg) into the ear vein, and randomly divided into three groups: 1. control (n = 8), 2. infusion of sterile water (n = 8) or 3. infusion of tissue-type plasminogen activator (n = 14) in obstructed retinal vein. In the first group no micropuncture of retinal vein was performed. In the others micropuncture of retinal vein were performed by a micromanipulator that we designed and sterile water was infused in retinal vein in the second group or tissue-type plasminogen activator was infused in the third one. Fluorescent angiography and histological study were performed to document the morphological alterations with clinical observations.RESULTS:18 eyes were successfully performed micropuncture of retinal vein in the 22 eyes (81.4%). Except stopping operation due to surgical accident, there were 7 eyes that the thrombi were lysed in 9 eyes by infusion of tissue-type plasminogen activator in obstructed retinal vein (77.8%). If the micropipette was not accurate for puncturing the vessels, the vessels' wall could be ruptured and hemorrhage appeared. If thrombi of retinal veins were lysed, no bleeding and no leakage of the infused drug were observed from the site of micropuncture after micropipette was withdrawn from the vessel. Sterile water failed to flush the clot downstream. When the thrombus failed to be dislodged, the infusion fluid would back flow into the distal retinal vein and, an exudative retinal detachment would occur. In the first group, the retina exhibited hemorrhages and edema due to the vein occluded. In the second group, sterile water resulted in exaggerated retinal edema and exudative retinal detachment. In the third group, after thrombi of retinal veins were lysed, the hemorrhages and edema of retinal tissues were reduced significantly in few days.CONCLUSIONS:Infusing thrombolytic agents into the thrombosed retinal vein is a new method for treatment of retinal vein occlusion. This surgery must performed accurately by a special design. This study demonstrated that the rate and dose of thrombolytic agents infused in retinal vein should be limited in case of injuring retina tissues.
Experimental Study of Posterior Vitreous Detachment with Plasmin in Human Eyes

Prevention of GM6001 on experimental proliferative vitreoretinopathy in rabbit

Abstract:Objective: Matrix metalloproteinase is one of causes of proliferative vitreoretinopathy (PVR), and the overexpression of tissue inhibitors of metalloproteinases( TIMPs) play an important role on the normal physiology function of cells. Present study was to determine the efficacy of GM6001 , a synthetic TIMPs, in the treatment of experimental PVR induced by intravitreal injection of dispase. Methods: 0.075 U of dispase was injected into vitreous cavity of both eyes to induced PVR in 20 pigmented rabbits. One week following induction of PVR, the right eyes of rabbits received 100 μmol/L of GM6001 0.05 ml and left eyes received PBS 0.05 ml intravitreally at a two-week interval for 6 weeks. The clinical observation of retina and vitreous was performed under slim-lamp and ophthalmoscope 1 week and 2,3,4,6 weeks after establish of model. PVR severity between the GM6001-treated eyes and the control eyes was compared by score grading. The histopathologic examination of PVR was made by immunohistochemical stain. Results: The mean PVR score in the treated eyes and control eyes was 1.19 and 2.54 respectively (P < 0.05). Clinically significant PVR with traction retinal detachment was formed in 71% of rabbits in the control eyes and 11% of rabbits in GM6001-treated eyes. The mean proliferation cell index was 18.8% and 36.2% in GM6001-treated eyes and that in control eyes was 41. 1% and 55. 0% at 2 and 4 weeks after operation with a significant difference between them (P<0.05). The positive rate of PCNA was 8.87%, 11.94%, 22.41% and 27.61% in GM6001-treated eyes and 9.41% , 24.10%, 44.31% and 41.79% in control eyes in 1,2,4,6 weeks after operation respectively (P<0.05). Conclusion: These results suggest that intravitreous administration of GM6001 has an inhibitory effect on experimental proliferative vetreoretinopahy.
An experimental study on tissue plasminogen activator inducing posterior vitreous detachment in pigs

Abstract:Objective: To investigate the efficacy of intravitreal injection of Tissue Plasminogen Activator (t-PA) combined with cyclocryoapplication on production of posterior vitreous detachment (PVD) in pig eyes and evaluate the methods of judging PVD in experimental animal study. Methods: Twenty eyes of 10 young pigs randomly received an intravitreal injection of t-PA (50 μg/0.1ml) and the other one buffered salt solution (BSS) immediately after the cyclocryoapplication via conjuctiva. PVD were evaluated by ultrasonic examination (6 and 12 hours), light microscopy (12 hours), transmission electron microscopy (12 hours) and scanning electron microscopy (12 hours). Results: The eyes injected t-PA combined with cyclocryoapplication exhibited a higher incidence of PVD compared to fellow eyes under the electron microscopy (8:1, P < 0.05) and the results of ultrasonic examination (4:1 for 6 hour, 5:1 for 12 hours) and light microscopy (6:1) showed no significant difference between t-PA group and BSS control (P > 0.05). Conclusion: Intravitreal injection of t-PA 50 μg combined with cyclocryoapplication can induce PVD in pigs. Transmission and scaning electron microscopy are more operative for the judgement of PVD than ultrasonic examination and light microscopy.

Abstract:Purpose: Posterior vitreous detachment (PVD) is implicated in numerous retinal pathologies. A necessary step in developing new therapies, an area of significant interest, is a quantifiable assessment of posterior vitreous adhesion (PVA) that is also clinically relevant. Methods: A 23-gauge vitrector was used at varying levels of vacuum to attempt PVD induction in a porcine eye model injected with either balanced salt solution (BSS) (control) or plasmin (2, 3, or 5 U), which can pharmacologically induce PVD. Results: The average minimum vacuum necessary to induce a PVD was 395 ± 28 mm Hg for BSS alone, 385 ± 58 mm Hg for 2 U of plasmin, 265 ± 53 mm Hg for 3 U of plasmin, and 145 ± 28 mm Hg for 5 U of plasmin. We demonstrated a dose-dependent response curve with increasing amounts of plasmin, leading to a statistically significantly lower minimum vacuum necessary to induce a PVD except between BSS and 2 U plasmin. Conclusions: A dose-dependent relationship between plasmin concentration and PVD was demonstrated. We believe that this model offers significant benefits over prior work as it minimizes confounding manipulations and offers a quantitative assessment that is translatable to in vivo surgical models. Translational relevance: This is the first methodology to quantitatively assess the degree of vitreous adhesion in situ.

Abstract:OBJECTIVES:It is challenging to deliver the therapeutic drug effectively to the posterior ocular disease location with optimized exposure and long-term effects when treating proliferative vitreoretinopathy (PVR). The objective of this study is to develop a novel biodegradable and long-acting ocular implant for PVR therapy with ligustrazine as the active ingredient.METHODS:The ligustrazine implants were prepared with poly(DL-lactide-co-glycolide) using a hot-melting extrusion. The physicochemical properties of the implants were characterized. The effectiveness of the selected ligustrazine implants was evaluated in a PVR rabbit model. Furthermore, the in-vitro drug release profile and pharmacokinetics were compared, and in-vitro/in-vivo correlations were evaluated.KEY FINDINGS:The optimal implants had an ideal zero-order in-vitro drug release profile, which was correlated with the in-vivo drug absorption fraction in the vitreous bodies of the rabbits. The sustained-release ligustrazine implants significantly reduced the development of PVR in the animal model.CONCLUSIONS:Ligustrazine implants can be used to treat posterior ocular disease in rabbit animal models, and it provides more choices for medical research on posterior ocular disease.

Abstract:OBJECTIVE:To evaluate the experimental induction of posterior vitreous detachment (PVD) by intravitreous injection of hyaluronidase and perfluoroethane (C(2)F(6)).METHODS:Fifteen rabbits (30 eyes) were divided into 3 experimental groups,the contralateral eyes in same animals served as the controls. Eyes in group A and B were received two vitreous injections of 15 IU of hyaluronidase at an interval of 5 d. The eyes in group C and all control eyes were injected with balanced salt solution (BSS). Seven days after injection, the experimental eyes in group A and C were received 0.5 ml of Fifteen rabbits (30 eyes) were divided into 3 experimental groups, the contralateral eyes in same animals served as the controls. Eyes in group A and B were received two vitreous injections of 15 IU of hyaluronidase at an interval of 5 d. The eyes in group C and all control eyes were injected with balanced salt solution (BSS). Seven days after injection,the experimental eyes in group A and C were received 0.5 ml of C(2)F(6) injection. The ocular and retinal signs were examined for 8 following weeks and then killed for histological examination.RESULT:Five eyes in group A (100.0%) showed complete separation of the vitreous cortex from the retina (PVD), three eyes in group B(60.0%) showed partial PVD, and no PVD was detected in group C and all control eyes. On electroretinogram no significant difference was found in amplitude and latency of a-(or b-) wave in both experimental and control eyes, between before and after experiments. No evidence of ocular or retinal toxicity was revealed by light or scanning electronic microscopy in all eyes.CONCLUSION:Vitreous injection of hyaluronidase combined with perfluoroethane, as a safety method, can induce posterior vitreous detachment without mechanical vitrectomy.

Abstract:A 19-year-old woman presented sudden visual loss in the left eye for 1 day. Ophthalmic examination showed superior hemi-retinal whitening with thickening of the inner retinal layer and shadowing of the outer retina layer and delayed perfusion of the superior hemi-retinal artery and cilioretinal artery. We performed surgery at 34 hours after the onset. After vitrectomy, a 48G micro-needle was connected to the 1-cc syringe of the viscous fluid control unit of the vitrectomy machine. The needle tip was inserted into the lumen of the superior peripapillary branch retinal vein. Then balanced salt solution was injected without tissue-type plasminogen activator, till it entered the vein and nearby artery. After surgery, the patient's visual acuity restored to 1.0 at 7 days and kept unchanged for 4 weeks. This is the first report on intra-retinal vein cannulation without tissue-type plasminogen activator for 34 hours of hemi-central retinal artery occlusion. It is worth trying for improving visual acuity despite certain morphologic damage. ( Chin J Ophthalmol, 2020, 56: 536-538).
Efficacy Evaluation of Tissue Plasminogen Activator with Anti-Vascular Endothelial Growth Factor Drugs for Submacular Hemorrhage Treatment: A Meta-Analysis

Abstract:Submacular hemorrhage (SMH) is the accumulation of blood in the macular area that can severely damage the macular structure and visual function. Recently, the intraocular administration of tissue plasminogen activator (TPA) with anti-vascular endothelial growth factor (anti-VEGF) drugs was reported to have a positive effect on SMH. This meta-analysis aimed to explore the efficacy and safety of the drug combination. We systematically searched the Web of Science, MEDLINE, EMBASE, and Cochrane Library databases and screened relevant full-length literature reports. The quality of the reports was assessed by two independent reviewers. The best-corrected visual acuity (BCVA) and foveal thickness (FT) were considered the main indicators of efficacy. RevMan 5.4 software was used for this meta-analysis. Twelve studies were analyzed, and the results showed that BCVA at 1 month (p < 0.001), 3 months (p < 0.001), 6 months (p < 0.001), and the last follow-up (p < 0.001) was improved relative to the preoperative value. The postoperative FT was lower than the preoperative FT (p < 0.001). No significant difference in efficacy was observed between subretinal and intravitreal TPA injections (p = 0.37). TPA with anti-VEGF drugs is safe for SMH treatment and can significantly improve BCVA and reduce FT.
Vitrectomy with Subretinal Injection of Recombinant Tissue Plasminogen Activator for Submacular Hemorrhage with or without Vitreous Hemorrhage

Abstract:Purpose: To evaluate the outcomes and prognostic factors of pars plana vitrectomy (PPV) combined with subretinal injection of recombinant tissue plasminogen activator (rt-PA) for submacular hemorrhage (SMH) patients with or without vitreous hemorrhage. Methods: Sixty-four eyes of 64 patients with SMH patients underwent PPV with subretinal injection of rt-PA. Best-corrected visual acuity (BCVA), SMH displacement, and postoperative complications were analyzed. Predictive factors of the final BCVA were determined using multivariant linear regression. Results: There were 26 eyes with VH and 38 without VH BCVA significantly improved in both VH group (from 2.27±0.40 to 1.25±0.70 LogMAR) and non-VH group (from 1.76±0.55 to 0.85±0.65 LogMAR). Completely displacement of SMHs was observed in 47 (73.43%) eyes. Postoperative complications included recurrent SMH (4.69%), recurrent vitreous hemorrhage (10.94%), rhegmatogenous retinal detachment (3.13%), and epiretinal membrane (4.68%). Treatment-naive condition, early surgery, and younger age were significantly associated with better final BCVA ( B =0.502, 0.303, and 0.021, respectively, with all p <0.05). Conclusion: PPV combined with subretinal rt-PA injection is an effective treatment for SMH patients with and without VH.

Abstract:Proliferative vitreoretinopathy (PVR) is the most common cause of anatomic failure in retinal detachment surgery. To understand the molecular mechanisms, vitreous proteomes of patients with PVR were investigated by two-dimensional-nano-liquid chromatography coupled with tandem mass spectrometry. Vitreous samples of moderate PVR (grade B), and severe PVR (grade C or D) were aspirated during pars plana vitrectomy before infusion. In the current study, 129, 97 and 137 proteins were identified in vitreous of normal control, moderate and severe PVR, respectively. In PVR vitreous samples, complement components, serine proteinase inhibitors, and extracellular proteins were up-regulated or appeared, while normal cytoskeleton and metabolism proteins were down-regulated or disappeared. It was noteworthy that the proteins involved in transcription and translation regulation increased in vitreous with PVR. Among 102 PVR-specific proteins, kininogen 1 was specifically detected in both vitreous and the corresponding serum. Therefore, it can be concluded that PVR is a complicated pathology process with great amount of proteins involved in metabolism dysfunction, immune reactions, and cytoskeleton remolding. Kininogen 1 may be a candidate biomarker of PVR. Further investigations of these special proteins will provide additional targets for treatment or prevention of ocular proliferative diseases.
