Abstract:Acupuncture and electroacupuncture (EA) as complementary and alternative medicine have been accepted worldwide mainly for the treatment of acute and chronic pain. Studies on the mechanisms of action have revealed that endogenous opioid peptides in the central nervous system play an essential role in mediating the analgesic effect of EA. Further studies have shown that different kinds of neuropeptides are released by EA with different frequencies. For example, EA of 2 Hz accelerates the release of enkephalin, b-endorphin and endomorphin, while that of 100 Hz selectively increases the release of dynorphin. A combination of the two frequencies produces a simultaneous release of all four opioid peptides, resulting in a maximal therapeutic effect. This finding has been verified in clinical studies in patients with various kinds of chronic pain including low back pain and diabetic neuropathic pain. q 2003 Elsevier Ireland Ltd. Abstract:A review on acupuncture and its neurochemical basis may not seem to be within the general scope of this series. However, the phenomenon has attracted the interest of scientists from several disciplines, including phar macologists, and the use of drugs as probes has been instrumental in estab lishing the neurochemical basis of acupuncture. We have found it necessary to limit our review to studies of acupuncture for the relief of pain since research in other areas is still in its infancy. Since much of the work in this field has been published in periodicals not easily accessible to the general scientific community for linguistic or other reasons, we have chosen to provide a fairly extensive documentation. Although acupuncture has a very long history as a means for relieving pain (1, 2), its scientific study began very recently. Early studies revealed that the analgesic effect was blocked by procaine infiltration of acupuncture points and that it was not possible to induce analgesia in paraplegic or hemiplegic patients, pointing to the importance of afferent transmission (3-6). Involvement of chemical mediators in the central nervous system (CNS) was suggested from experiments where the cerebrospinal fluid (CSF) of donor rabbits given acupuncture was infused into the cerebral ventricles of recipient rabbits, increasing their pain thresholds (7). An important series of findings came in the 1970s when several investiga tors presented carefully controlled experimental studies using different ap proaches. Abstract:Chronic pain is a debilitating and rather common health problem. The present shortage in analgesic drugs with a favorable spectrum but without remarkable side effects furthered the search for alternative therapeutic manipulations. Increasing evidence from both basic and clinical research on acupuncture, a main alternative therapy of traditional Chinese medicine, suggests that chronic pain is sensitive to acupuncture procedures. Clarification of the underlying mechanisms is a challenge of great theoretical and practical significance. The seminal hypothesis of Geoffrey Burnstock and the astounding findings of Maiken Nedergaard on the involvement of purinergic signaling in the beneficial effects of acupuncture fertilized the field and led to an intensification of research on acupurines. In this review, we will summarize the state-of-the-art situation and try to forecast how the field is likely to develop in the future.

Abstract:Sheng's acupuncture manipulation at bone-nearby acupoints is a set of needling manipulation of the chief physician of TCM, SHENG Xie-sun, summarized through his over 50 years clinical experiences and on the basis of Internal Classic. Regarding this manipulation, on the premise of acupoint selection based on syndrome differentiation, the acupoints close to bone are possibly selected and punctured, with the needle tip toward bone edge, and followed by the technique to achieve reducing purpose. Clinically, the significant immediate analgesia can be achieved in pain disorders such as headache and toothache. Professor Sheng thought, corresponding to the location of needle insertion and needling depth, the tissue layers of needle tip passing through should be considered specially. The site of needle insertion should be changeable so as to ensure the needle tip reaching the bone. This manipulation for analgesia provides a certain guide for acupuncture study, especially for the mechanism study on acupuncture analgesia.

Abstract:In the present paper, the author retrospects the historical developmental stages of acupuncture anesthesia-analgesia from 1) preliminary exploration:the creative application of acupuncture therapy to surgery with lower dosage or without anesthetics began in Shanghai and Xi'an in 1958; 2) clinical popularization:the acupuncture anesthesia-analgesia became popularized from 1965, peaked in 1970s' and declined from 1980s'; 3) mechanism study:from 1970 to 2000, a large quantity of research papers were published to confirm the validity of acupuncture analgesia (AA), and in 1997, NIH Consensus Conference for acupuncture was held, promoting the related research in the world; 4) focused research:from 2007 to now, the acupuncture anesthesia-analgesia was listed as one of the key research program in China; 5) theoretical summary:in view of the uncertainty of the scientific basis of the meridian system of traditional Chinese medicine, the nerve fibers distributed under the acupoint are now considered to be the indispensable components for AA, and the knowledge of modern neuroscience is competent to explain the underlying mechanism of AA; 6) returning to reality:AA has been accepted by the international scientific community, and the "acupuncture-assisted anesthesia", or "compound acupuncture and anesthetics anesthesia" has been used as the first choice for some types of surgical operation. The research on acupuncture anesthesia-analgesia greatly facilitates clinical application of acupuncture therapy in the world, and also promotes the development of acupuncture therapy itself and neuroscience. However, the transformation between theory and practice and their mutual interaction are endless endeavor. Thus, a careful review of the half-century-long practice of acupuncture anesthesia-analgesia is painstakingly necessary in order to clarifying the mechanisms, to raise the efficacy of clinical practice and to correct the possible errors referred to this concept.

Abstract:Pain and inflammatory diseases are important clinical indications of acupuncture, which have been widely accepted in the international community. Previous studies have been focusing on rapid analgesia of acupuncture through the regulation of nervous system, but few studies on the inflammation regulatory mechanisms in which acupuncture inhibits the peripheral sensitization-induced pain. Based on studies concerning acupoint mechanisms of acupuncture actions and related researches on acupuncture regulating neuroendocrine and immune systems, we put forward the scientific hypothesis that acupuncture regulates neuroendocrine-immune (NEI) network and key response media therein, so as to achieve anti-inflammatory and analgesic effects in target organs. We have established a platform for acupuncture at ST36 to alleviate inflammatory pain in adjuvant induced arthritic rats. Based on the complex network analysis of multi-dimensional data from multi-time point and multi-site detection of NEI common signaling molecules, we have clarified the regulatory effects of acupuncture on NEI network and corresponding downstream immune network. Results indicated that monocytes/macrophages are the key targeting cells of acupuncture regulation, and acupuncture may display the anti-inflammatory and analgesic effects by regulating polarization of T cells in lymph nodes and polarization of M1/M2 macrophages in inflamed joints/paws. In addition, we have spotted a key molecule for acupuncture analgesia, CXCL1, as well as clarified the novel central analgesic mechanism of acupuncture mediated by CXCL1/CXCR2 desensitization. Thereby, we have provided novel evidence of the anti-inflammatory and analgesic actions of acupuncture through regulating NEI network and several key substances, highlighting a systemic research paradigm for investigating mechanisms of acupuncture actions.

Abstract:Pain is a kind of complex physiological and psychological symptom, which makes the person debilitated and uncomfortable. Some persistent pain is unbearable for the patients, reducing the quality of life and bringing considerable pressure to the individuals and society. Pain killers seem to be effective in analgesia for patients, but their safety and addiction are crucial issues. From the theory of traditional Chinese medicine (TCM), the blocked meridian is the main cause of pain, and effective acupuncture can play a positive analgesic effect. Acupuncture that can date back thousands of years is one of the ancient medical practices in China. Its safety and effectiveness are respected. Based on its superior safety and inferior side effects, it has been gradually recognized as a therapeutic intervention method for complementary medicine, which is also generally used to treat multiple pain diseases. It is shown by modern medical studies that neurotransmitters are the material basis for the acupuncture effect, and the effect of acupuncture analgesia is related to changes in neurotransmitters. However, the specific mechanism has not been elucidated. This review aims to comprehensively discuss the historical evolution of acupuncture analgesia, clinical research of acupuncture analgesia, comparison of acupuncture and drug therapy, the neurotransmitter mechanism of acupuncture analgesia, the effect of acupuncture manipulation on analgesia, and bibliometric analysis of acupuncture treatment for pain, to explore the superiority and related mechanism of acupuncture analgesia from different aspects, and to provide a more effective treatment for alleviating patients’ pain.

Abstract:It has long been accepted that acupuncture, puncturing and scraping needles at certain points on the body, can have analgesic and anesthetic effects, as well as therapeutic effects in the treatment of various diseases. This therapy, including acupuncture anesthesia, has drawn the attention of many investigators and become a research subject of international interest around the world. Numerous studies have demonstrated that the nervous system, neurotransmitters, endogenous substances and Jingluo (meridians) may respond to needling stimulation and electrical acupuncture. An abundance of information has now accumulated concerning the neurobiological mechanisms of acupuncture, in relation to both neural pathways and neurotransmitters/hormonal factors that mediate autonomic regulation, pain relief and other therapeutics. Early studies demonstrated that the analgesic effects of electroacupuncture (EA) are mediated by opioid peptides in the periaqueductal gray. Recent evidence shows that nitric oxide plays an important role in mediating the cardiovascular responses to EA stimulation through the gracile nucleus-thalamic pathway. Other substances, including serotonin, catecholamines, inorganic chemicals and amino acids such as glutamate and α-aminobutyric acid (GABA), are proposed to mediate certain cardiovascular and analgesic effects of acupuncture, but at present their role is poorly understood. The increased interest in acupuncture health care has led to an ever-growing number of investigators pursuing research in the processes of the sense of needling touch, transduction of needling stimulation signals, stimulation parameters and placebos. In this Review, the evidence and understanding of the neurobiological processes of acupuncture research have been summarized with an emphasis on recent developments of nitric oxide mediating acupuncture signals through the dorsal medulla-thalamic pathways.

THE FUTURE OF ACUPUNCTURE ANESTHESIA:From Acupuncture Anesthesia(aa) to Acupuncture-Assisted Anesthesia(aaa)

Abstract:In this article the author tried to summarize more than 30 years' practice of acupuncture anesthesia(AA) in China,and to comment on the future development of this medical technique.(1) Acupuncture-induced analgesia is the key element of acupuncture anesthesia.(2) Acupuncture is able to reduce the pain produced by nociceptive inputs from surgical operations,but is theoretically impossible to use acupuncture for a toeal abolishment of surgical pain.(3) Quantitatively,acupuncture is able to reduce the amount of anesthetic drug by 40～50% in many surgical operations including the cranial and abdominal operations.(4) The beneficial points of AA in combination with drug anesthesia depend mainly on the reduction of anesthetic usage, resulting in a smoothening of the fluctuation of vital parameters during the surgery, acceleration of the post-operative recovery and shortening of the period of postoperative hospitalization,and partly on the possibility of surgeon-patient interaction and cooperation during the surgical procedure.(5) The difficulties of performing classical acupuncture manipulations have been solved since the availability of the Han's Acupoint Nerve Stimulator(HANS).(6) It is suggested to use acupuncture in combination with anesthetics,and this technique could be termed acupuncture-assisted anesthesia(AAA) which keeps most of the strong points of AA and puts the patient in a absolutely pain-free status.(7) From the hospital management point of view,there should be new regulations to encourage the anesthesiologists to use AAA whenever it is considered beneficial to the patient.

Abstract:Acupuncture and electroacupuncture effects are mediated through a variety of neural and neurochemical mechanisms. Research done in the early 1970s first elucidated the mechanisms for the effect of acupuncture anesthesia. Further experiments showed that this effect can be transferred from one rabbit to another by cerebrospinal fluid (CSF) transfusion. Further investigation explored the role of classic central neurotransmitters in the mediation of acupuncture analgesia, including catecholamines and serotonin. The availability of rat models for electroacupuncture, using the tail flick latency as a bioassay, allowed further experiments to be done to explain the basis for the effects. Differential release of central nervous system (CNS) opioid peptides by electroacupuncture has been noted, with 2-Hz electroacupuncture triggering release of enkephalins and β-endorphins, and 100-Hz stimulation selectively increased the release of dynorphin in the spinal cord. A combination of both frequencies allows synergistic interaction among the three endogenous opioid peptides and a powerful analgesic effect. In addition, multiple acupuncture treatments with the optimal time spacing may result in an accumulation of electroacupuncture effect. A bimodal distribution of analgesic effect can be noted if a large group of rats is given electroacupuncture ("low responders" and "high responders"). The mechanism of low response is at least twofold: a low rate of release of opioid peptides in the CNS, and a high rate of release of CCK-8, which exerts potent anti-opioid effects. A newly discovered anti-opioid peptide is orphanin FQ, which has also been linked to negative feedback control of electroacupuncture stimulation. Further research will serve to elucidate the mechanisms of acupuncture treatment in opiate detoxification, and how different disease entities may respond differently to electroacupuncture.

Abstract:OBJECTIVE:To review and discuss the Chinese and English literature on the use of pain-related evoked potentials(PREP) and short-latency somatosensory EP(SLSEP) in acupuncture research.METHODS:China National Knowledge Infrastructure Database and MEDLINE were searched for the following key words:acupuncture and PREP or SLSEP.RESULTS:Thirty-seven articles were included in the review.Researchers usually use PREPs to study the analgesic effect of acupuncture,observe influential factors,or for mechanistic exploration.In the SLSEP studies,researchers focused on response characteristics of acupuncture,acupoint specificity,and influential factors of the treatment.There were some problems with the study design and conclusions.CONCLUSION:Researchers could use PREP and SLSEP to objectively validate the effects of acupuncture and explore its mechanisms using nerve electrophysiology.Further studies can benefit from observing more acupoints’ effects using PREPs or SLSEPs and investigating the placebo effect of acupuncture.

Abstract:Acupuncture is an ancient Chinese method to treat diseases and relieve pain. We have conducted a series of studies to examine the mechanisms of this ancient method for pain relief. This article reviews some of our major findings. Our studies showed that acupuncture produces analgesic effect and that electroacupuncture (EA) is more effective than manual acupuncture. Furthermore, electrical stimulation via skin patch electrodes is as effective as EA. The induction and recovering profiles of acupuncture analgesia suggest the involvement of humoral factors. This notion was supported by cross-perfusion experiments in which acupuncture-induced analgesic effect was transferred from the donor rabbit to the recipient rabbit when the cerebrospinal fluid (CSF) was transferred. The prevention of EA-induced analgesia by naloxone and by antiserum against endorphins suggests that endorphins are involved. More recent work demonstrated the release of endorphins into CSF following EA. In addition, low frequency (2 Hz) and high frequency (100 Hz) of EA selectively induces the release of enkephalins and dynorphins in both experimental animals and humans. Clinical studies suggesting its effectiveness for the treatment of various types of pain, depression, anxiety, spinally induced muscle spasm, stroke, gastrointestinal disorders, and drug addiction were also discussed.

Abstract:BACKGROUND: The activation of subcutaneous mast cells (MCs) helps to trigger the analgesic effect induced by acupuncture (AP), a traditional oriental therapy, that has been gradually accepted worldwide. This work aimed to reveal whether the serotonin (5-hydroxytryptamine, 5-HT) released from MCs plays an important role in this process, which has a controversial effect in the mechanism of pain.METHODS: In vivo tests, a 20-min session of AP was applied at Zusanli acupuncture point (acupoint) of acute ankle arthritis rats. Pain thresholds of the injured hindpaw were assessed to reflect the pain state, and the targeting substances in the interstitial space of the treated acupoint were sampled by microdialysis. In vitro experiments, exogenous 5-HT (exo-5-HT) was introduced to mediate adenosine triphosphate (ATP) release from cultured MCs.RESULTS: Needling promoted 5-HT accumulation at the Zusanli acupoint, which was prevented by sodium cromolyn. AP's analgesic effect was suppressed by the inhibition of 5-HT receptors at the acupoint, especially 5-HT1A subtype. In vitro tests, mechanical perturbation mimicking needling stimulation induced MCs to release 5-HT. 1 μM and 10 μM of exo-5-HT facilitated ATP release, which was restrained by blocking of 5-HT1 receptors rather than 5-HT3 receptors. As 5-HT, ATP and adenosine were also transiently accumulated in the treated acupoint during needling. Promoting ATP hydrolysis or activation adenosine A1 receptors duplicated AP analgesic effect. Finally, the inhibition of ATP receptors by suramin or pyridoxal phosphate-6-azo tetrasodium salt hydrate (PPADS) prevented AP analgesic effect.CONCLUSIONS: Our results suggest that MC-associated 5-HT release at acupoints contributes to AP analgesia, and the mediation of ATP secretion through 5-HT1A receptors might be the underlying mechanism at play. ATP could facilitate adenosine production or the propagation of needling signals.

Abstract:Background: Pain management is a great burden on society; therefore, cost-effective and nonaddictive treatments for pain are urgently required. Needling of painful spots has been applied in acupuncture along with dry needling <DN) to treat pain without opioids at minimal costs. However, no attempt has been made to examine DN, trigger point(TrP) physiology, and acupuncture to identify potential areas for pain-management research. This review compares the modalities of acupuncture and DN based on the current research on TrP physiology and diagnostics to advance both modalities. Methods: A narrative review of the literature on TrP physiology, its associated diagnostics, and the techniques of DN and acupuncture was performed. Results: Diagnostic imagery may benefit the study and treatment of TrPs using needling. Acupuncture and DN techniques are similar in their applications. However, the warm needling technique is established in acupuncture but not in dry needling. Additionally, translational difficulties have inhibited crossdiscipline learning. Conclusions: Historical evidence suggests a need to examine the use of heat in needling further. Additional research should be conducted on TrP categories to determine if a relationship with the needling technique can be established. Furthermore, interdisciplinary communication would benefit both modalities.

Abstract:This study investigated whether immediate acupuncture effects in the acupoint are histamine dependent. Both histamine injection and manual acupuncture stimulation increased the pain threshold (PT) after treatment compared with the model group (P < 0.01), producing an analgesic effect. After pretreatment with clemastine, an H1 receptor antagonist and an antipruritic, the increase in the animals' pain threshold after acupuncture was suppressed compared with the Acu group (P < 0.01); however, there was no interference with the acupuncture-induced degranulation of mast cells. Pretreatment with disodium cromolyn did not suppress the increase in PT induced by the histamine injection at Zusanli (ST-36). We conclude that in adjuvant-induced arthritic rats, acupuncture analgesic effects are histamine dependent, and this histamine dependence determines the acupoint preference of acupoints away from the target site in acupuncture practice.

Abstract:Cancer pain is one of the common complications in patients with intermediate or advanced cancer, which brings severe sufferings to patients and their families, and also seriously interferes with the anti-tumor process. Acupuncture is an effective method for the treatment of cancer pain, which has the advantages of simple operation, quick effect and fewer side effects, and is widely used in clinic. In this paper, we reviewed the relevant literature on the mechanisms of acupuncture in the treatment of cancer pain both at home and abroad in recent years, and summarized the analgesic mechanisms of acupuncture for cancer pain from the peripheral level and the central level. It is found that acupuncture can relieve cancer pain by regulating immune inflammatory response, expression of ion channels and pain-related receptors, remising central sensitization, activating endogenous pain modulating system and suppressing pain transmission pathways. In addition, we also think that the use of high-throughput multi-omics techniques and neuroimaging methods to detect and analyze the brain areas or body fluids before and after acupuncture should be the focus of acupuncture analgesia research in the future.
Inhibition of Spinal Interlukin-33/ST2 Signaling and Downstream ERK and JNK Pathways in Electroacupuncture Analgesia in Formalin Mice

Abstract:Although acupuncture is widely used to manage pain, it remains highly controversial, largely due to the lack of a clear mechanism for its benefits. Here, we investigated the role of IL-33, a novel interleukin (IL)-1 family member, and its receptor ST2 in the analgesic effects of electroacupuncture (EA) on formalin-induced inflammatory pain. The results showed that 1) EA stimulation of ipsilateral Zusanli (ST 36) and Yanglingquan (GB 34) acupoints for 30 min remarkably suppressed the two phases of formalin-induced spontaneous pain; 2) subcutaneous or intrathecal administration of recombinant IL-33 (rIL-33) significantly inhibited the analgesic effect of EA, whereas the ST2 antibody potentiated EA analgesia in formalin mice; 3) EA treatment decreased the up-regulation of IL-33 and ST2 protein following formalin injection; and 4) the suppression of the formalin-induced expression of spinal phosphorylated ERK and JNK induced by EA treatment was significantly attenuated following subcutaneous rIL-33 delivery, and was further decreased by the ST2 antibody. These data suggest that EA alleviates formalin-induced inflammatory pain, at least partially, by inhibiting of spinal IL-33/ST2 signaling and the downstream ERK and JNK pathways.
