Abstract:Excessive activation of the transforming growth factor beta signaling pathway and disorganized cellular skeleton caused by genetic mutations are known to be responsible for the inherited thoracic aortic aneurysms and dissections (TAAD), a life-threatening vascular disease. To investigate the genotype-phenotype correlation, we screened genetic mutations of fibrillin-1 (FBN1), transforming growth factor-β receptor-1 (TGFBR1) and transforming growth factor-β receptor-2 (TGFBR2) for TAAD in 7 affected families and 22 sporadic patients. Of 19 potential mutations identified in FBN1, 11 appeared novel while the others were recurrent. Two mutations were detected in TGFBR2. Eight patients carried no mutation in either of these genes. Characterization of FBN1 c.5917+6T>C in transfected HEK293 cells demonstrated that it caused skipping of exon 47, leading to the loss of the 33th calcium binding epidermal growth factor-like domain associated with Marfan syndrome. Compared with exon 46, skipping of 47 did not cause patients ectopia lentis in all carriers. To correlate genotypes with phenotypes in different human ancestries, we reviewed the published mutational studies on FBN1 and found that the probability of cardiovascular defects were significantly increased in Chinese patients with premature termination codon or splicing mutations than those with missense mutations (91.7 % vs 54.2 %, P = 0.0307) or with noncysteine-involved point mutations than those with cysteine-involved mutations (88.9 % vs 33.3 %, P = 0.0131). Thus, we conclude that exon 47 skipping of FBN1 leads preferentially to cardiovascular defects and human ancestries influence genotype-phenotype correlation in TAAD.

Abstract:Genetic study can provide important insight into the etiology of aortic dissection. To explore the pathogenesis and natural history of aortic dissection, a number of genes have been identified through microarray chip screening and undergone testing of polymorphisms to find mutations strongly associated with the disease. The results suggested aortic dissection to be a multi-gene disorder. Multiple genes probably work together to promote its development. Several diseases with a genetic predisposition are closely connected with aortic dissection, which also implied a role of genetic changes and malfunction in this disease.
Molecular Characterization and Clinical Investigation of Patients with Heritable Thoracic Aortic Aneurysm and Dissection.

Abstract:Objectives: Thoracic aortic aneurysm and dissection has a genetic predisposition and a variety of clinical manifestations. This study aimed to investigate the clinical and molecular characterizations of patients with thoracic aortic aneurysm and dissection and further explore the relationship between the genotype and phenotype, as well as their postoperative outcomes.Methods: A total of 1095 individuals with thoracic aortic aneurysm and dissection admitted to our hospital between 2013 and 2022 were included. Next-generation sequencing and multiplex ligation-dependent probe amplification were performed, and mosaicism analysis was additionally implemented to identify the genetic causes.Results: A total of 376 causative variants were identified in 83.5% of patients with syndromic thoracic aortic aneurysm and dissection and 18.7% of patients with non-syndromic thoracic aortic aneurysm and dissection, including 8 copy number variations and 2 mosaic variants. Patients in the "pathogenic" and "variant of uncertain significance" groups had younger ages of aortic events and higher aortic reintervention risks compared with genetically negative cases. In addition, patients with FBN1 haploinsufficiency variants had shorter reintervention-free survival than those with FBN1 dominant negative variants.Conclusions: Our data expanded the genetic spectrum of heritable thoracic aortic aneurysm and dissection and indicated that copy number variations and mosaic variants contributed to a small proportion of the disease-causing alterations. Moreover, positive genetic results might have a possible predictive value for aortic event severity and postoperative risk stratification. (J Thorac Cardiovasc Surg 2023;166:1594-603)

Abstract:Objective To screen the differentially expressed genes between the aorta from acute type B aortic dissections with those from normal controls and search the genes related to the pathogenesis of this disease.Methods From Sep.2005 to Feb.2008,six descending aorta specimens from patients with a-cute Stanford B dissection were taken during surgery;six normal descending aorta specimens were gotten from multi-organ donors.The gene expression profiles between two groups were compared with 6 pieces of Phalanx Whote Genome Micmarray.The data were analyzed with several professional data processing software and ouline websites.Quantitative real time pelymerase chsin reaction(Real time PCR)Was used to check the reliability of this data.Results Totally 6375 genes were valid in this experiment.Of those,670 genes were difierentially expressed in dissected and control aorta.21 8 ones were up-regulated.and 452 ones were down.regulated.Fourteen ones were related to cell.eell adhesion:12 genes to the generation of extra-cellular matrix components;4 genes to cell-matrix adhesion;14 genes to cytoskeleton formation;10 genes to immune and inflammatory response;10 genes to collagen production;12 genes to cell apoptosis.The genes including PKD1,PKD2,SPP1,COL1A1,COL3A1.COL6A2 and ACTC were verified by Real time PCR test.Conclusion The result demonstrated the complexity of the dissecting process on a molecular level.Some of these difierentially expressed genes probably resulted in the development of aortic dissection finally.

Abstract:Thoracic aortic aneurysms leading to acute aortic dissections are the major diseases that affect the thoracic aorta. Approximately 20% of patients with thoracic aortic aneurysms and dissections (TAAD) have a family history of TAAD, and these patients present younger with more rapidly enlarging aneurysms than patients without a family history of aortic disease.A large family with multiple members with TAAD inherited in an autosomal-dominant manner was identified. The ascending aortic aneurysms were associated with slow enlargement, a low risk of dissection, and decreased penetrance in women. Genome-wide linkage analysis was performed, and a novel locus on chromosome 12 was identified for the mutant gene causing disease in this family. Of the 12 male members who carry the disease-linked microsatellite haplotype, 9 had ascending aortic aneurysms with an average diameter of 4.7 cm at an average age of 52.4 years (range, 32 to 76 years) at the time of diagnosis; only 1 individual had progressed to acute aortic dissection, and no other members with aortic dissections were identified. Women harboring the disease-linked haplotype did not have thoracic aortic disease, including 1 aged 84 years. Sequencing of 9 genes within the critical interval at the chromosome 12 locus did not identify the mutant gene.Mapping a locus for ascending thoracic aortic aneurysms associated with a low risk of aortic dissection supports our hypothesis that genes leading to familial disease can be associated with less-aggressive thoracic aortic disease.
Integrated analysis identifies hypertension-GWAS-loci regulated, differentially expressed genes in aorta and blood of aortic dissection

Abstract:Abstract Background: Aortic dissection is a life-threatening condition caused by a tear in the intimal layer of the aorta. Hypertension is the most common risk factor of aortic dissection. However, only a small proportion of subjects with hypertension will develop aortic dissection. Genetic basis and effect genes determining the development and progression of aortic dissection remained to be identified. In this study, we attempted to recognize the underlying effect genes from reliable large-scale genome-wide association studies (GWASs) of hypertension.Methods: As most GWAS locus functions through its biological role in gene expression regulation, we initially converted the GWAS signals to transcriptomic profiles in aorta using the Functional Summary-based Imputation (FUSION) algorithm. The FUSION derived genes were then checked whether they were differentially expressed in aorta of subjects with and without aortic dissection.Results: We found 23 genes that were regulated by hypertension GWAS loci and were altered in aorta of dissection patients. In particular, the DCAF16 gene could be detected in blood, providing a possibility of non-invasive early detection or prediction of hypertensive individuals at risk of aortic dissection.Conclusions: Our analyses identified effect genes in aorta and provided a possibility of non-invasive early detection of aortic dissection.Trial registration: Retrospectively registered.

Abstract:Objective This study aimed at exploring the causative genes and summarizing the clinical characteristics in two Chinese families with thoracic aortic aneurysm and dissection ( TAAD ) .Methods The whole exome capture and high throughput sequencing were applied to identify the causative gene.Family members were examined for features of syndromic ge-netic diseases by clinician and geneticist.Results Four known TAAD candidate genes were identified in family TAA01:rs140598(FBN1), rs185661462(MYH11), rs77620762(MYLK3), and rs111426349(TGFBR1).The TGFBR1 mutation (c.1459C>T) had been confirmed to co-segregate with the TAAD phenotype in all affected family members.Early onset of aortic root dilatation was significant in this family , and the average age at diagnosis of aortic root dilatation or aneurysm was23. 2 years.ACTA2(c.445C>T) was proved in family TAA02, and livedo reticularis was confirmed.Conclusion The causa-tive genes were identified via whole exome capture and high throughput sequencing in two TAAD families .Early onset of aortic root aneurysm was proved in TAA01, while livedo reticularis was found in TAA02.
Pathway analysis of differentially expressed genes in patients with acute Stanford type A aortic dissection

Abstract:Objective To study the pathogenesis of aortic dissection by pathway analysis of differentially expressed genes in patients with acute Stanford type A aortic dissection. MethodsAscending aorta specimens from patients with acute Stanford type A dissection were taken during surgery,and compared with those from normal ascending aorta of organ donors using the gene expression microarray.Data were analyzed with GenomeStudio Gene Expression Module v1.0.Validation of microarray gene expression was performed with quantitative real-time PCR.All differentially expressed genes were submitted for pathway analysis using KEGG database. ResultsAccording to our definition,1 309 genes were differently expressed in dissected and control aorta specimens.The result of quantitative real-time PCR was consistent with that of microarray.Compared with the control group,patients with aortic dissection had significantly different gene-expression pathways involving focal adhesion,vascular smooth muscle contraction and regulation of stanford type A actin cytoskeleton. Conclusions Genes with different expression might have interacted through focal adhesion,vascular smooth muscle contraction and regulation of actin cytoskeleton pathways and played a contributory role in the pathogenesis of aortic dissection.
Genetic diagnosis of acute aortic dissection in South China Han population using next-generation sequencing

Abstract:Acute aortic dissection (AAD) is a clinically “silent,” but emergent and life-threatening cardiovascular disease, and hereditary factors play an important etiologic role in the development of AAD. The purposes of this study are to definitize the diagnostic yield of 59 AAD patients, investigate the molecular pathological spectrum of AAD by NGS, and explore the future preclinical prospects of genetic diagnosis on AAD high-risk groups. We performed next-generation sequencing (NGS) based on screening of the 69 currently aortic dissections/aneurysms-associated genes on 59 sporadic AAD samples from South China. A Kaplan-Meier survival curve was constructed to compare the event-free survival depending on variant number. Overall, 67 variants were detected in 39 patients, among which 4 patients were identified with pathogenic variants and 13 patients were diagnosed with likely pathogenic variants. Seventeen genotype positive patients were identified in aggregate, and the diagnostic yield of our study is 28.8%. All genotype-positive variants were distributed in 11 genes, FBN1 variants were in the largest number among genotype-positive variants, which were detected for 4 times, ACTA2 for 3 times, ABCC6 and TGFBR1 twice, and NOS3, MYLK, XYLT1, TIMP4, TGFBR2, CNTN3, and PON1 once. Individuals with three or more variants showed shorter mean event-free survival than patients with fewer variants. Our observations broaden the genetic pathological spectrum of AAD. Furthermore, our research uncovered two susceptibility genes FBN1 and ACTA2 for Stanford type A AAD patients. Finally, our study concluded that the number of variants an individual harbored was an important consideration in risk stratification for individualized prediction and disease diagnosis.

Abstract:Background: Spatial transcriptomics enables gene expression events to be pinpointed to a specific location in biological tissues. We developed a molecular approach for low-cell and high-fiber Stanford type A aortic dissection and preliminarily explored and visualized the heterogeneity of ascending aortic types and mapping cell-type-specific gene expression to specific anatomical domains. Methods: We collected aortic samples from 15 patients with Stanford type A aortic dissection and a case of ascending aorta was randomly selected followed by 10x Genomics and spatial transcriptomics sequencing. In data processing of normalization, component analysis and dimensionality reduction analysis, different algorithms were compared to establish the pipeline suitable for human aortic tissue. Results: We identified 19,879 genes based on the count level of gene expression at different locations and they were divided into seven groups based on gene expression trends. Major cell that the population may contain are indicated, and we can find different main distribution of different cell types, among which the tearing sites were mainly macrophages and stem cells. The gene expression of these different locations and the cell types they may contain are correlated and discussed in terms of their involvement in immunity, regulation of oxygen homeostasis, regulation of cell structure and basic function. Conclusion: This approach provides a spatially resolved transcriptome− and tissue-wide perspective of the adult human aorta and will allow the application of human fibrous aortic tissues without any effect on genes in different layers with low RNA expression levels. Our findings will pave the way toward both a better understanding of Stanford type A aortic dissection pathogenesis and heterogeneity and the implementation of more effective personalized therapeutic approaches.

Abstract:Thoracic aortic aneurysm and dissection (TAAD) affects many patients globally and has high mortality rates if undetected. Once thought to be solely a degenerative disease that afflicted the aorta due to high pressure and biomechanical stress, extensive investigation of the heritability and natural history of TAAD has shown a clear genetic basis for the disease. Here, we review both the cellular mechanisms and clinical manifestations of syndromic and non-syndromic TAAD. We particularly focus on genes that have been linked to dissection at diameters <5.0 cm, the current lower bound for surgical intervention. Genetic screening tests to identify patients with TAAD associated mutations that place them at high risk for dissection are also discussed.

Association of Family History With Incidence and Outcomes of Aortic Dissection

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Aortic dissection (AD) is a life-threatening emergency. However, the heritability and association of family history with late outcomes are unclear.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>The purpose of this study was to evaluate the effect of family history of AD on the incidence and prognosis of AD and estimate the heritability and environmental contribution in AD in Taiwan.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Both cross-sectional and cohort studies were conducted using Taiwan National Health Insurance database. A registry parent–offspring relationship algorithm was used to reconstruct the genealogy of this population for heritability estimation. The cross-sectional study included 23,868 patients with a diagnosis of AD in 2015. The prevalence and adjusted relative risks (RRs) were evaluated, and the liability threshold model was used to examine the effects of heritability and environmental factors. Furthermore, a 1:10 propensity score–matched cohort comprising AD patients with or without a family history of AD was included to compare late outcomes in the cohort study.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>A family history of AD in first-degree relatives was associated with an RR of 6.82 (95% confidence interval [CI]: 5.12 to 9.07). The heritability of AD was estimated to be 57.0% for genetic factors, and 3.1% and 40.0% for shared and nonshared environmental factors, respectively. After excluding individuals with Marfan syndrome or bicuspid aortic valve, a family history of AD was associated with an RR of 6.56 (95% CI: 4.92 to 8.77) for AD. Furthermore, patients with AD and a family history of AD had a higher risk of later aortic surgery than those with AD without a family history (subdistribution hazard ratio: 1.40; 95% CI: 1.12 to 1.76).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>A family history of AD was a strong risk factor for AD. Furthermore, patients with AD with a family history of AD had a higher risk of later aortic surgery than those with no family history of AD.</p>

Gene Expression Profiling in Acute Stanford Type B Aortic Dissection.

Abstract:Objective: To compare the gene expression profiles of the aorta specimens between patients with Stanford type B aortic dissection (AD) and controls. Materials and Methods: Samples of descending aorta were collected from patients with type B AD (n = 12) and from multiorgan donors as controls (n = 12). Phalanx whole genome microarray was used to analyze differential gene expression. Results: Of the 6375 probes validated, 623 genes were found to be differentially expressed between patients with type B AD and controls (fold change >= 2). Gene ontology analysis identified significantly enriched gene groups pertaining to cell-cell adhesion, extracellular matrix, cell-matrix adhesion, cytoskeleton, immune and inflammatory response, and apoptosis. Conclusion: Genes encoding components related to integrity and strength of the aortic wall were downregulated, whereas those related to inflammatory response were upregulated in type B AD. The altered patterns of gene expression indicate preexisting structural defects that are probably a consequence of insufficient remodeling of the aortic wall.
Integrative Analysis of Transcriptome-Wide Association Study and Mrna Expression Profile Identified Candidate Genes and Pathways Associated with Aortic Aneurysm and Dissection

Abstract:Background: Aortic aneurysm and dissection (AAD) are a set of life-threatening diseases. This study aimed to investigate the genetic mechanisms of AAD by integrating transcriptome-wide association study (TWAS) and mRNA expression profile. Methods: The genome-wide association study (GWAS) summary data of AAD was obtained from the UK Biobank, which contains 452,264 White British individuals, including 1470 AAD patients. The TWAS analysis was per -formed by integrating expression quantitative trait loci (eQTL) data of aorta and the GWAS dataset of AAD using the FUSION software. The TWAS significant genes and differentially expressed genes (DEGs) identified by mRNA expression profile of aortic dissection were integrated to find common genes and biological process. For TWAS significant genes, protein-protein interaction (PPI) network analysis was further conducted based on STRING database. Results: TWAS identified 423 genes with P < 0.05. After comparing the results of TWAS and mRNA expression profile, 11 overlapping genes (PDE8B, IKBKE, HMGA1, PKM, CHST1, DUS3L, S100A16, PTGS1, RAB38, PDLIM5, NOL6) and 15 common gene ontology (GO) terms (including extracellular matrix organization, external encapsulating structure organization, cell-substrate adhesion, actin filament-based process, focal adhesion, protein kinase activity) were identified. 9 hub genes of the TWAS results were identified via PPI network analysis, including RPS9, RPS18, RSRC1, DNAJC3, HBS1L, PRKCA, NCAM1, ITGB3, FTSJ3. Conclusion: Multiple candidate genes and biological processes associated with AAD were identified by the present integrative study of TWAS and mRNA expression profile. Further studies are needed to elucidate the genetic mechanisms of AAD.

Abstract:Purpose of ReviewOver the past 2 decades, our understanding of the role of genetics in the development and progression of aortic disease has grown considerably. This review serves as a foundational text for the reader to gain a thorough understanding of the basics of this emerging field while also displaying the latest breakthroughs which have had, and will continue to have, important impacts on clinical decision-making and patient outcomes. This review focuses on aneurysmal disease, arguably the richest subgroup of aortic diseases for genetic research.Recent FindingsThoracic aortic aneurysm and dissection (TAAD) is driven primarily by genetic factors. Genetic variants which drive TAAD tend to occur in genes involved in the structure and function of the aortic wall, specifically the extracellular matrix, TGF-β pathway, and smooth muscle cell contractile units. At least 37 gene variants (pathological or of unknown significance) have been found to be directly associated with the development of TAAD, independent of traditional cardiovascular (CV) risk factors such as smoking and hypertension. The standard suggestion for definitive treatment is prophylactic surgery at an aortic diameter of 5.0–5.5 cm, but several gene variants have been shown to dissect commonly at much smaller diameters (such as TGFβR1/2 or ACTA2), and so mutation-specific guidelines are leading to earlier surgery and better patient outcomes in these aggressive cases.Abdominal aortic aneurysms (AAAs) also have a genetic component, but these are primarily driven by CV risk factors. Many gene variants have been found which are closely associated with these risk factors, such as those in the lipid metabolism pathway (SORT1). Four variants have been found through GWAS studies to directly influence AAAs independent of general risk factors, though the clinical implications are yet unclear.SummaryNew variants are discovered every year, and each creates a new avenue of research. As more data is collected on patients with each variant, such as the rate of growth, mode of inheritance, age at dissection, and diameter at dissection, we can craft more patient-specific guidelines for different subgroups of affected individuals, with the aim of achieving better long-term survival.

Abstract:Introduction: Aortopathy which is characterized by aneurysms, dissection, dilation of the aorta, can occur secondarily to several syndromes. Because of the genetic heterogeneity, we designed a gene panel containing 15 genes responsible for inherited aortopathy, to identify genetic mutations and help make a precise diagnosis. Methods: Here we tested 440 probands with aortopathy using panel assay. Variants were screened by bioinformatics analysis and confirmed using Sanger sequencing. Multiplex ligation-dependent probe amplification(MLPA) was used to detect exonic deletions/duplications. Results: The results showed that 136 individuals (31%) tested positive for a (likely) pathogenic mutation, 123 (28%) had a variant of uncertain significance (VUS). Most of the (likely) pathogenic mutations (124/136) were located in the fibrillin-1 (FBN1) gene. Abstract:Acute aortic dissection is one of the most severe vascular diseases. The molecular mechanisms of aortic expansion and dissection are unclear. Clinical studies have found that statins play a protective role in aortic dissection development and therapy; however, the mechanism of statins’ effects on the aorta is unknown. The Gene Expression Omnibus (GEO) dataset GSE52093, GSE2450and GSE8686 were analyzed, and genes expressed differentially between aortic dissection samples and normal samples were determined using the Networkanalyst and iDEP tools. Weight gene correlation network analysis (WGCNA), functional annotation, pathway enrichment analysis, and the analysis of the regional variations of genomic features were then performed. We found that the minichromosome maintenance proteins (MCMs), a family of proteins targeted by statins, were upregulated in dissected aortic wall tissues and play a central role in cell-cycle and mitosis regulation in aortic dissection patients. Our results indicate a potential molecular target and mechanism for statins’ effects in patients with acute type A aortic dissection.

Identification of COL3A1 Variants Associated with Sporadic Thoracic Aortic Dissection:A Case-Control Study

Abstract:Thoracic aortic dissection (TAD) without familial clustering or syndromic features is known as sporadic TAD (STAD). So far, the genetic basis of STAD remains unknown. Whole exome sequencing was performed in 223 STAD patients and 414 healthy controls from the Chinese Han population ( N = 637). After population structure and genetic relationship and ancestry analyses, we used the optimal sequence kernel association test to identify the candidate genes or variants of STAD. We found that COL3A1 was significantly relevant to STAD ( P = 7.35 × 10 −6 ) after 10 000 times permutation test ( P = 2.49 × 10 −3 ). Moreover, another independent cohort, including 423 cases and 734 non-STAD subjects ( N = 1157), replicated our results ( P = 0.021). Further bioinformatics analysis showed that COL3A1 was highly expressed in dissected aortic tissues, and its expression was related to the extracellular matrix (ECM) pathway. Our study identified a profile of known heritable TAD genes in the Chinese STAD population and found that COL3A1 could increase the risk of STAD through the ECM pathway. We wanted to expand the knowledge of the genetic basis and pathology of STAD, which may further help in providing better genetic counseling to the patients.

Abstract:Background: There is a paucity of evidence on people with thoracic aortic aneurysm and dissection. We aimed to determine the prevalence of genetic variants and their associations with phenotypes. Methods: In this cross-sectional single-centre cohort study of consecutive patients who underwent endovascular or open-surgical repair of thoracic aortic aneurysm and dissection, genetic analysis was performed using four-stage Next Generation Sequencing, and findings were confirmed with Sanger sequencing. We collected personal and family history on comorbidities, clinical examination, anthropometrics, skeletal deformities, joint function, and ophthalmological measures. Cardiovascular risk and phenotype scores were calculated. Results: Ninety-five patients were eligible (mean age 54 ± 9 years, 70% males, 56% aortic dissection). One-fifth had a family history of aortic disease. Furthermore, 95% and 54% had a phenotype score of ≤5 and ≤2, respectively. There were no significant differences in the distribution of phenotype characteristics according to age, sex, aortic pathology, or performed invasive procedures. Genetic variants of uncertain significance were detected in 40% of patients, with classic mutations comprising 18% of all variants. We observed no significant association with cardiovascular and phenotype scores but with higher joint function scores (p = 0.015). Conclusion: Genetic variants are highly present in clinically relevant aortic pathologies. Variants appear to play a larger role than previously described. The different variants do not correlate with specific phenotypes, age, pathology, sex, or family history.
