Abstract:Aspergillus is a genus of filamentous, ubiquitous, and opportunistic pathogenic fungi. Patients with underlying lung disease are susceptible to Aspergillus due to their immunodeficiency, but the symptoms become atypical as other lung diseases are contracted (1-3). Aspergillus fumigatus, A. flavus, and A. niger are the primary pathogenic species in most literatures (4, 5). The principal forms of pulmonary aspergillosis (PA) are allergic bronchopulmonary aspergillosis (ABPA), chronic (and saprophytic) pulmonary aspergillosis (CPA), and invasive pulmonary aspergillosis (IPA). In non-neutropenic patients, various forms are considered a semi-continuous spectrum of aspergillosis (4). The analysis of the clinical features can deepen our understanding of the spectrum of clinical manifestations of Aspergillus infection and provide references for its diagnosis.
The Diagnosis and Surgical Treatment for 56 Cases with Pulmonary and Pleural Aspergillosis

Abstract:OBJECTIVE:To summarize the experience of diagnosis and surgical treatment for pulmonary and pleural aspergillosis.METHODS:The clinical data of cases with pulmonary and pleural aspergillosis were analyzed retrospectively between September 1972 and June 2003. There were 53 cases with pulmonary aspergillosis and 3 cases with pleural aspergillosis. Aspergillus was found preoperatively in 8 patients by sputum culture (5 cases) or needle biopsy of the lung (2 cases) or fibro-bronchoscopic biopsy (1 case). All patients were treated with surgical procedures following X-ray film or CT scan.RESULTS:Of 53 cases with pulmonary aspergillosis, 42 lobectomies, 3 segment-Pneumonectomies, and 8 wedge resections were performed. Of three cases with pleural aspergillosis following eliminating their diseased foci in residual pleural space, two underwent thoracoplasty, one underwent postoperative closed chest drainage for one and an half month with fluconazole injected into residual pleural space repeatedly for 1 month (200 mg/100 ml, 1 time per 2 or 3 days). No operative death and major postoperative complications occurred. None of the patients had recurrent symptoms at follow-up.CONCLUSION:We recommend aggressive surgical resection for pulmonary and pleural aspergillosis, and the surgical result is excellent.

Clinical Analysis on Ninty-Six Cases of Secondary Aspergillosis in COPD Patients

Abstract:Objective To discuss the clinical features and control measure of respiratory infection associated with aspergillosis secondary to chronic obstructive pulmonary disease.Method 96 cases of COPD patients with aspergillus in their sputum culture,admitted into the First Affiliated Hospital of Zhejiang University from June 2007 to June 2008,were collected and their clinical manifestations,treatment and treatment outcome were analyzed.Result Among the 96 cases of COPD patients with aspergillosis,9 cases patients died after treatment,87 cases improved including 45 cases treated with non-antifungal agents,38 cases with fluconazde and 13 cases with the addition of Voriconazole.Conclusion COPD patients with aspergillosis were increasing in recent years.The risk factors of aspergillus infection includes repeatedly long-term hospitalization,advance in age,irrational antibiotics and hormones application,as well as latrogenic and invasive operation.Early diagnosis,early treatment and rational precautionary can be a key to reduce the occurrence of respiratory infection by aspergillus.
The Clinical Characteristics of Patients with Nonneutropenic Invasive Pulmonary Aspergillosis.

Abstract:Objective: The goal of this study was to reveal the clinical manifestations of nonneutropenic invasive pulmonary aspergillosis (IPA), which are different from those of neutropenic patients. Methods: The clinical data of patients with nonneutropenic IPA were collected at the Department of Respiratory and Critical Care Medicine, Jinling Hospital, from February 2009 to November 2019. We analyzed the general conditions, clinical manifestations, imaging findings, and laboratory tests of these IPA patients. Results: A total of 116 patients with nonneutropenic IPA (31 proven and 85 probable) were included. They had an average age of 59.8 years. The most common underlying disease was chronic obstructive pulmonary disease (COPD, n = 33). The common clinical symptoms included cough (93.1%, n = 108), expectoration (59.5%, n = 69), fever (57.8%, n = 67), hemoptysis (30.2%, n = 35), and dyspnea (40.5%, n = 47). The common CT imaging manifestations included consolidation (47.4%, n = 55), cavities (47.4%, n = 55), air crescent sign (14.7%, n = 17), and nodules (8.6%, n = 10). Multiple lesions (74.1%, n = 86) were more common than single lesions (17.2%, n = 20) and diffuse lesions (8.6%, n = 10). The positive rate of laboratory tests was 88.2% (30/34) for BALF galactomannan (GM), 55.4% (56/101) for serum GM, 45.3% (48/106) for 1,3-β-D-glucan (BDG), 43.3% (46/106) for sputum culture, and 36.4% (20/55) for BALF culture. Patients who had high serum GM level [GM optical density index (ODI) >1] were more likely to have severe respiratory symptoms and higher serum ferritin. Further investigation showed that there was a positive correlation between serum GM level and serum ferritin level. Conclusion: The clinical symptoms and radiological manifestations of nonneutropenic IPA are diverse and often lead to delayed diagnosis. It is important to become more vigilant of aspergillosis in nonneutropenic patients in order to achieve early diagnosis and treatment and to reduce mortality.

Abstract:OBJECTIVE:The purpose of this study was to describe the approaches to the diagnosis and the treatment of chronic necrotizing pulmonary aspergillosis (CNPA).METHODS:The study included 8 patients with CNPA confirmed by histopathological studies. The patients were admitted to the First Affiliated Hospital of College of Medicine of Zhejiang University during 2000-2004. The clinical data were analyzed, and related literature was reviewed. The clinical, radiological and pathological characteristics, and the approaches to the diagnosis and the treatment were described.RESULTS:CNPA was a rare form of pulmonary aspergillosis. Common presentations included productive cough (8/8), haemoptysis (7/8) and constitutional symptoms (5/8). Radiographically, the air crescent sign was suggestive of the diagnosis, which was found in 4 patients. Consolidation was also common in the other 4 cases. The histological feature of CNPA included the presence of tissue invasion by Aspergillus, tissue necrosis and granulomatous inflammation.CONCLUSIONS:The clinical manifestations of CNPA are non-specific. Radiography plays a pivotal role in the diagnosis of CNPA. Treatment with antifungal medications is indicated once the diagnosis is made. Pulmonary resection should be considered when patients have prolonged illness or frequent haemoptysis.
Clinical Analysis of 18 Pulmonary Aspergillosis Cases

Abstract:Objective To get a better understand of pulmonary aspergillosis. Method A retrospective analysis of the clinical features of 18 pulmonary aspergillosis cases was performed.Result 83% of the patients had accompanying underlying diseases;The incidence of symptoms was 89%.Chest imaging findings:Lesions were mainly in the right lung,more common in the upper lobes,and mostly located in the peripheral;The cases of Lobar pneumonia-type accounted for 17% of all the patients,aspergilloma-type 50%,nodular or mass-type 33%,bronchopneumonia-type 6% and diffuse lesion-type 22%,with 22% of cases complicated with pleural effusion. Histopathological examination showed septate hyphae branching at an angle of approximately 45°.50% of patients underwent lobectomy and received antifungal medication after operation,while the other 50% received antifungal medication only; 89% of patients were fully recovered or improved,while 11% died.Conclusion Most of pulmonary aspergillosis patients have underlying diseases.Some types of pulmonary aspergillosis have typical imaging changes; Diagnosis requires a combination of past history,clinical manifestations,chest imaging findings and related laboratory examinations,and confirmed diagnosis is according to histopathological examination.To treat the diseases,most of patients need drug application,and some patients may undergo surgery.

Abstract:Background We report an invasive pulmonary aspergillosis (IPA) with negative (1,3)-β-D-glucan and dynamically elevated white blood cells combined with procalcitonin proven by bronchoalveolar lavage fluid (BALF) culture. Methods Appropriate laboratory tests are carried out. Chest CTs were performed to assess the lungs. The cause of infection was determined using BALF culture. Results Serum (1,3)-β-D-glucan was negative, white blood cells and procalcitonin were significantly higher than normal. The bronchoscopy revealed obvious necrotic detritus and pseudo membrane in the trachea, left and right main bronchi, and branches. BALF culture revealed the presence of Aspergillus. Conclusions Negative (1,3)-β-D-glucan is not safe to rule out invasive pulmonary aspergillosis. BALF culture is critical for IPA diagnosis.

Abstract:Aspergillus molds are ubiquitous environmental molds that can cause devastating invasive infections in immunocompromised patients. These infections often go unrecognized in critically ill patients. This case describes a 68 year-old female resident of a long-term nursing facility with history of dementia, nonalcoholic fatty liver disease with cirrhosis, chronic kidney disease stage III and insulin-dependent type 2 diabetes who presented with vomiting, diarrhea and leg swelling. She developed hypotension and was treated for sepsis but found to have negative routine infectious workup. Chest imaging showed nodular densities and bilateral opacities. She developed acute renal failure and hypoxic respiratory failure followed by acute decompensated cirrhosis with refractory volume overload and hypotension and was eventually transitioned to comfort care measures. Autopsy ultimately showed invasive pulmonary aspergillosis. Here we review the diagnosis and management of invasive fungal infections in critically ill patients without typical risk factors or clinical findings for invasive fungal disease. Invasive fungal infections are frequently missed and carry high mortality rates, therefore warranting consideration in critically ill populations.

Abstract:The association of noncavitary untreated lung cancer with coexisting pulmonary Aspergillosis in immunocompetent patients is an extremely rare occurrence. The present paper aims to summarize the clinical data, and gives an overview of the current knowledge on the etiology, diagnosis and treatment of this condition.We report four cases and review seven cases in the literature.The four cases describe pulmonary solid tumor with coexisting pulmonary Aspergillosis condition. All cases underwent complicated procession for the terminal diagnosis.Clinicians should be alert to underlying malignant disease if airway Aspergillus infection is suspicious in a patient without strong risk factors for invasive fungal disease. On the other hand, when lung cancer is coexisting with airway necrotizing Aspergillosis, clinicians should properly manage these two diseases simultaneously.

Epidemiology of Invasive Pulmonary Aspergillosis in Patients with Liver Failure: Clinical Presentation, Risk Factors, and Outcomes.

Abstract:Objective Invasive pulmonary aspergillosis (IPA) is a severe and often lethal infection. The possible risk factors, clinical presentation, and treatment of patients with simultaneous liver failure and IPA have received little attention in previous studies. The aim of this study was to investigate the epidemiology of IPA in patients with liver failure in an effort to reduce patient mortality. Methods The patients with liver failure (including acute liver failure , sub-acute liver failure , acute-on-chronic liver failure and chronic liver failure) were recruited from 2011 to 2016. The clinical data of these patients were retrieved for the study. Results In total, 1077 patients with liver failure were included in this study. Of the 1077 patients, 53 (4.9%) had IPA. Forty-four (83%) patients with IPA died. Independent risk factors for IPA were male sex (hazard ratio [HR]=2.542), hepatorenal syndrome (HR=2.463), antibiotic use (HR=4.631), and steroid exposure (HR=18.615). Conclusions IPA is a fatal complication in patients with liver failure. Male sex, hepatorenal syndrome, antibiotic use, and steroid exposure were independent risk factors for IPA. When patients with liver failure have these risk factors and symptoms of pneumonia such as cough or hemoptysis, clinicians should be cautious about the possibility of IPA.

Abstract:Invasive pulmonary aspergillosis (IPA) is no longer a rare disease in the intensive care unit (ICU) on account of the increased number of immunocompromised hosts admitted, the application of invasive treatment procedures and the widespread use of broad-spectrum antibiotics. Different from those with agranulocytosis, symptoms and signs of IPA in ICU patients are subtle or non-specific which are unable to be distinguished from those with bacterial pneumonia or even with noninfectious diseases. Thus, accurately diagnosing of IPA is quite challenging. In this review, we will navigate through the IPA diagnostic tests currently available and how they apply to ICU population. Special attention is paid to the non-culture- based tests as well as have a look into what the future holds.

Abstract:Aspergillus is the most common fungal pathogen of the respiratory mycosis. The respiratory diseases caused by Aspergillus are mainly classified into four clinical types, namely allergic bronchopulmonary aspergillosis, invasive aspergillosis, aspergilloma, and chronic pulmonary aspergillosis. Among the four clinical types of pulmonary aspergillosis, invasive aspergillosis is the most dangerous and difficult to treat disease. The diagnosis of invasive aspergillosis is established initially on clinical aspects and confirmed by the finding of pathogen in culture and histopathology. A number of methods can be conducted to obtain tissue samples from patients with invasive aspergillosis, chronic pulmonary aspergillosis or aspergilloma. Besides BAL detection and transbronchial biopsy, a fine-needle biopsy can be performed and guided by CT to diagnose these diseases. Antifungal agents including those with activity against Aspergillus have been developed. Several agents such as voriconazole, amphotericin B lipid complex, caspofungin, micafungin and anidulafungin are promising in the treatment of the pulmonary aspergillosis in the future. Moreover, under certain condition, surgical intervention is needed to combine with antifungal therapy to deal with aspergilloma or chronic pulmonary aspergillosis, and the use of glucocorticoids to control the inflammatory response is also very important in therapy of allergic bronchopulmonary aspergillosis besides antifungal agents.

Abstract:Timely detection of Aspergillus infection is crucial given the high mortality rate of pulmonary aspergillosis (PA). Here, the diagnostic performances for PA of mycological culture, Aspergillus real-time PCR, and metagenomic next-generation sequencing (mNGS) assay from bronchoalveolar lavage fluid, were evaluated. In total, 139 patients with suspected fungal pneumonia were enrolled between December 2021 and July 2023, collecting 139 bronchoalveolar lavage fluid samples for real-time PCR and culture, with 87 undergoing mNGS assay. The sensitivity, specificity, positive predictive value, negative predictive value, and area under the curve with 95% CIs of these assays for PA were as follows: 35.3% (14.2%-61.7%), 100.0% (94.0%-100.0%), 100.0% (54.1%-100.0%), 84.5% (79.3%-88.6%), and 0.676 (0.560-0.779), respectively, for culture; 82.4% (56.6%-96.2%), 98.3% (91.1%-100.0%), 93.3% (66.4%-99.0%), 95.2% (87.6%-98.2%), and 0.903 (0.815-0.959), respectively, for same diagnostic performance of real-time PCR and mNGS; and 94.1% (71.3%-99.9%), 96.7% (88.5%-99.6%), 88.9% (67.1%-96.9%), 98.3% (89.6%-99.7%), and 0.954 (0.880-0.989), respectively, for real-time PCR combining mNGS; real-time PCR, mNGS, and their combination significantly improved in area under the curve values over culture (P < 0.001), but real-time PCR testing and mNGS had no significant difference with each other and their combination. Overall, the performance of culture was limited by low sensitivity; both real-time PCR and mNGS assays as single diagnostic tests are promising compared with culture and combined tests.
Clinical and Microbiological Characterization of Invasive Pulmonary Aspergillosis Caused by Aspergillus lentulus in China

Abstract:Invasive aspergillosis (IA) due to Aspergillus lentulus is associated with high mortality. In this study, we investigated the clinical and microbiological characteristics of 6 fatal cases of proven or probable IA caused by A. lentulus in China. Underlying immunosuppression, prior antifungal exposure, and intensive care unit (ICU) hospitalization were important risk factors for invasive A. lentulus infection. Phenotypic differences were observed for A. lentulus isolates including slower growth, reduced sporulation, and inability to grow at 48°C, compared with Aspergillus fumigatus complex. ITS sequencing was unable to distinguish A. lentulus from A. fumigatus, but sequencing of the benA, CaM, and rod A loci enabled reliable distinction of these closely related species. Phylogenetic analysis further confirmed that the ITS region had little variation within the Aspergillus section Fumigati while the benA gene offered the highest intraspecific discrimination. Microsatellite typing results revealed that only loci on chromosomes 1, 3, 5, and 6b generated detectable amplicons for identification. All A. lentulus isolates showed in vitro resistance to multiple antifungal drugs including amphotericin B (MIC range 4 to 8 μg/ml), itraconazole (MIC 2 μg/ml), voriconazole (MIC of 4–16 μg/ml), and posaconazole (MIC of 0.5–1 μg/ml). However, MECs for the echinocandin drugs ranged from 0.03–0.25, ≤0.008–0.015, and ≤0.015–0.03 μg/ml for caspofungin, micafungin, and anidulafungin, respectively. A. lentulus is an emerging fungal pathogen in China, causing fatal disease, and clinicians as well as laboratories should be alert to their increasing presence.

Abstract:A 61-year-old woman with pulmonary alternariosis and aspergillosis was reported. The patient presented with recurrent hemoptysis and cough for 3 years. Alternaria was identified by fungal culture. Biopsy specimen showed pulmonary aspergillosis. The patient had been treated with voriconazole at 400 mg/d through intravenous guttae for 7 days, and then switched amphotericin B at 25 mg/d through intravenous guttae for 11 days. The patient was treated with voriconazole at  400 mg through oral when she was discharged from hospital. After the treatment, the clinical symptoms of hemoptysis and cough were recovered, and the lung CT examinations showed normal.

Abstract:Species within the Aspergillus spp. cause a wide range of infections in humans, including invasive pulmonary aspergillosis, chronic pulmonary aspergillosis, and allergic bronchopulmonary aspergillosis, and are associated with high mortality rates. The incidence of pulmonary aspergillosis (PA) is on the rise, and the emergence of triazole-resistant Aspergillus spp. isolates, especially Aspergillus fumigatus, limits the efficacy of mold-active triazoles. Therefore, host-directed and novel adjunctive therapies are required to more effectively combat PA. In this review, we focus on PA from a microbiome perspective. We provide a general overview of the effects of the lung and gut microbiomes on the growth of Aspergillus spp. and host immunity. We highlight the potential of the microbiome as a therapeutic target for PA.
