Abstract:Cryosurgery with liquid nitrogen was employed for the treatment of 60 patients with primary liver cancer (PLC) at the Liver Cancer Research Unit, Zhong Shan Hospital, Shanghai Medical University, the People's Republic of China, from November 1973 to August 1987. Of 60 patients, subclinical stage amounted to 35% (21/60), moderate stage 55% (33/60) and late stage 10% (6/60). There were 21 cases with small PLC (less than or equal to 5 cm). The postoperative course was uneventful in all of the 60 patients. These was no operative mortality, and there was no complications such as rupture of tumor, secondary bleeding, bile leakage, or abdominal infection. The 1-year, 2-year, 3-year, 4-year, and 5-year survival rates were 51.7% (30/58), 33.9% (19/56), 20.8% (11/53), 15.6% (7/45), and 11.4% (5/44), respectively, for the whole series. Among the 21 patients with tumor nodules less than or equal to 5 cm in diameter, the 1-year, 2-year, 3-year, 4-year, and 5-year survival rates were 76.2% (16/21), 61.9% (13/21), 50.0% (9/18), 41.2% (7/17), and 37.5% (6/16), respectively. These results indicate that hepatic cryosurgery is a promising, safe, and simple treatment for neoplastic disease of liver. Cryosurgery with liquid nitrogen can be considered the surgery of choice for nonresectable PLC in patients without jaundice, ascites, and noncompensated liver function, and the whole tumor mass can be involved in the frozen area.
Surgery for Primary Liver Cancer: A Forty Years Survey

Abstract:The authors report the progress of surgery in 3227 patients with pathologically proven primary liver cancer (PLC) over the past four decades (Jan. 1958-Dec. 1997), and the factors improving long-term outcome. The 5-, and 10-year survival after resection of PLC was 44.3% and 33.2%, respectively, for the whole series (n = 2276), and 65.5% and 47.6%, respectively, for patients with small PLC (< = 5cm, n = 861). The 5-year survival after cryosurgery was 39.8% for the whole serias (n = 235), and 55.4% for patients with small PLC (n = 80). The 5-year survival of 93 patients receiving sequential resection after cytoreduction therapy was 64.5%, The 5-year survival after re-resection for recurrence tumor (n = 202) was 35.4% ; 328 patients survived more than 5 years ; 187 of these patients (57.0%) were small PLC, and 109 patients survived more than 10 years. Microwave surgery was used for resectable and unresectable PLC. Encouraging changes in the prognostic pattern were observed when the PLC data 1958-1967 (n = 118), 1968-1977 (n = 356), 1978-1987 (n = 715) and 1988-1997 (n = 2038) were compared ; the 5-year survival being 2.8%, 7.3%, 27.1% and 52.5% respectively, and the 10-year survival being 2.8%, 4.3%, 19.8% and 39.9% respectively. It is concluded that early detection and resection of small PLC is the leading approach to get long-term survivors, cytoreduction and sequential resection might be an important approach to improve the prognosis of patients with unresectable PLC, re-resection for subclinical recurrence and metastasis after an initial curative resection was also important to prolong survival further. Microwave surgery and cytosurgery as minimally invasive therapy have certain advantages in the treatment of PLC.

Abstract:Between November 1973 and December 1996, the in situ freezing of tumor, i.e., cryotherapy, was performed with liquid nitrogen (-196 degrees C) on 235 patients with primary liver cancer (PLC). There were no operative mortalities or severe complications. The 5-year survival was 39.8% for the 235 PLC patients, and 55.4% for the 80 patients with small PLC (< or = 5 cm). When analyzed with respect to treatment modalities without considering the size of the tumor, the 5-year survival was 26.9% for 78 PLC patients treated by cryotherapy alone; 39.6% for 58 PLC patients treated by cryotherapy plus hepatic artery ligation and perfusion; 46.0% for 27 PLC patients treated by cryotherapy for residual tumor plus resection of the main tumor; and 60.4% for 72 PLC patients treated by cryotherapy followed by resection of the frozen tumor. These results indicate that cryotherapy is a safe and effective treatment for PLC.

Abstract:Unquestionably, progress has been made in the early detection and early treatment of primary liver cancers (PLCs), although most remain unresectable, mainly because the cancer is advanced and coexists with liver cirrhosis, particularly in Oriental patients. Thanks to the progress of regional cancer therapy, a multidisciplinary approach, and changing concepts about surgical oncology, it has been proved that some unresectable but not far advanced PLCs are potentially convertible to being resectable, particularly those cancers confined to the right lobe of a cirrhotic liver. A retrospective analysis of 571 unresectable PLCs revealed the following: (1) There was an increase in 5-year survivals in the series, from 0% during the 1960s (n=61), to 4.8% during the 1970s (n=163), to 21.2% during the 1980s (n=347). It might be a result of the increase in double- or triple-modality treatments in these series (from 9.8%, to 19.6%, to 70.3%, respectively) and in the sequential resection rate after cytoreduction (from 0%, to 2.5%, to 14.7%). (2) The combination of hepatic artery ligation, hepatic artery cannulation and infusion, and intrahepatic arterial radioimmunotherapy has resulted in better shrinkage of the tumor, a higher sequential resection rate, and a higher 5-year survival (28.2%). (3) Of the 55 patients who had initially unresectable PLCs and yielded “cytoreduction and sequential resection”, the 5-year survival was 58.5%. It is concluded that cytoreduction and sequential resection might be an important approach to improving the prognosis of patients with unresectable PLCs.
The Role of Cryosurgery in the Treatment of Hepatic Cancer: a Report of 113 Cases.

Abstract:From November 1973 to June 1992, cryosurgery with liquid nitrogen (−196°C) was performed on 113 patients with hepatic cancer, including 107 patients with primary liver cancer (PLC) and 6 patients with secondary liver cancer (SLC). Of the 107 PLC patients, the subclinical stage constituted 30.8% (33/107), the moderate stage 61.7% (66/107), and the late stage 7.5% (8/107). There were 32 cases with small PLC (up to 5 cm). Liver cirrhosis was observed in 86.0% (92/107). We designed flat cryoprobes for freezing surface tumors, and single and multiple trocar cryoprobes for freezing tumors deep within the hepatic parenchyma. Intraoperative ultrasound was used for monitoring hepatic cryolesions. There were no operative mortalities and complications, such as rupture of a tumor, delayed bleeding, or bile leakage. The 5-year and 10-year survival rates were 22.0% and 8.2%, respectively, for the 107 PLC patients and 48.8% and 17.1%, respectively, for the 32 patients with small PLC. Of the 6 SLC patients, survival ranged from 2 months to 90 months (average, 23.2 months). One SLC patient has been well for 7 years and 6 months after cryosurgery. These results indicate that cryosurgery, the in situ freezing of cancer, is a safe and effective treatment for unresectable hepatic cancer.
The Changing Role of Surgery in the Treatment of Primary Liver Cancer

Abstract:For decades, the role of surgery in the treatment of primary liver cancer (PLC) was important but limited. However, a comparison of pathologically proven PLC during the three periods 1958-1966, 1967-1975, and 1976-1984 revealed that as a result of alpha fetoprotein (AFP) serosurvey, changing concepts in surgical oncology, and introduction of new surgical modalities, the role of surgery has become greater. The increasing proportion of subclinical PLC (0, 7.2, and 21.2%) has favored the increasing series resection rate (16.1, 34.7, and 39.6%) and palliative surgery (13.7, 17.0, and 29.8%). The results indicated that early resection, reoperation for subclinical recurrence, resection of huge PLC in stages, and combination of palliative surgery other than resection might be responsible for the increasing 5-year survival rate (1.7, 7.1, and 19.5%) in the entire series.
[Cryosurgery for Primary Hepatic Cancer of 87 Patients].

Abstract:From november 1973 to June 1991, cryosurgery with liquid nitrogen was performed on 87 patients with primary liver cancer (PLC). Of these, 27 patients was of stage I (31.0%), 52 in stage II (59.8%), and 8 stage III (9.2%). There were 30 patients with PLC of < or = 5 cm (34.5%). Liver cirrhosis was found in 73 patients (83.9%). In the beginning, plate-like cryoprobes and thermocouples were used to monitor the frozen area. Later on we designed single- and multiple-needle cryoprobes for freezing tumors deeply into the hepatic parenchyma and intraoperative ultrasound was used to monitor hepatic cryolesions. The 1-year, 3-year, and 5-year survival rates were 60.5%, 32.0%, and 20.2%, respectively. Among the 30 patients with PLC of < or = 5 cm, the 1-year, 3-year, and 5-year survival rates were 92.6%, 66.6%, and 50.8%, respectively. There were no operative mortality and complications such as rupture of the tumor, delayed bleeding, and bile leakage. These results indicate that cryosurgery is a safe and effective local treatment for unresectable PLC.
Three Decades of Progress in Surgery for Primary Liver Cancer

Abstract:This chapter reports the progress of surgery in the treatment of 2388 patients with pathologically proven primary liver cancer (PLC) during the past three decades. The 5- and 10-year survival rates after resection of PLC were 39.3% and 29.2%, respectively, for the whole series (n = 1650), and 61.9% and 45.4%, respectively, for patients with small PLC (≤ 5cm). The 5-year survival rate after cryosurgery was 37.9% for the whole series (n = 191) and 53.1% for patients with small PLC (n = 56). The 5-year survival of 71 patients receiving sequential resection after cytoreduction therapy was 66.0%, and the 5-year survival after reresection for recurrence of tumor (n = 147) was 34.5%. Of 214 patients who survived more than 5 years, 113 patients (52.8%) had a small PLC, and 57 patients survived more than 10 years. Encouraging changes in the prognostic pattern were observed when the PLC data of 1958–1970 (n = 178), 1971–1982 (n = 582), and 1983–1994 (n = 1628) were compared, the 5-year survival rates being 4.8%, 11.2%, and 45.7%, respectively, and the 10-year survival rates being 4.2%, 7.5%, and 33.0%, respectively. Some aspects to prolong survival further are discussed.
Cytoreduction and Sequential Resection - A Hope for Unresectable Primary Liver-Cancer

Abstract:For decades, unresectable primary liver cancer (PLC) determined by operation was incurable. However, a retrospective study of 24 years' materials with unresectable PLC indicated that 5-year survival of unresectable PLC has increased from 0% in 1966-1977 (n = 137) to 16.9% in 1978-1989 (n = 345). This encouraging improvement was mainly a result of cytoreduction therapy followed by sequential resection. Multimodality combination treatment with hepatic artery ligation, plus hepatic artery infusion with chemotherapy, plus radioimmunotherapy (or radiotherapy) yielded the highest sequential resection rate (30.6%) and 5-year survival (28.0%) as compared with double combination and single modality treatment. The 5-year survival of 33 patients receiving sequential resection after cytoreduction therapy was 63.2%. It is suggested that cytoreduction and sequential resection might offer a hope for surgically verified unresectable PLC.
Multimodality Treatment in Advanced Primary Liver Cancer.

Abstract:This paper reports the long-term results of multimodality treatment in 1639 patients with pathologically proven primary liver cancer (PLC) over the past three decades. In this series, patients in subclinical stage constituted 23.9% (391/1639), moderate stage 63.0% (1032/1639), and late stage 13.2% (216/1639). There were 381 patients (23.2%) with small PLC (< = 5 cm). The PLC was coexistent with liver cirrhosis in 86.4% (1416/1639). The 5-year survival after resection of PLC was 45.5% for the whole series (n = 896), and 62.7% for patients with a tumor < = 5 cm (n = 345). The 5-year survival after hepatic artery ligation (HAL) and hepatic artery infusion (HAI) (n = 124) was 18.1%, while the 5-year survival after palliative resection (n = 175) was 12.5%. The 5-year survival of 40 patients receiving second-look resection after cytoreduction therapy was 68.4%. Cryosurgery was performed on 107 PLC patients, the 5-year survival being 22.0% for the whole series, and 48.8% for the 32 patients with small PLC. The 5-year recurrence rate after resection was 55.3%. The 5-year survival after reoperation for recurring tumor (n = 90) was 40.8%. It is suggested that surgery remains the modality of choice, and it plays a more important role in the treatment of PLC. The results of palliative surgery (HAL + HAI) seem better than those of palliative resection; combined multimodality treatment and second-look resection, and new surgical techniques might offer hope for unresectable advanced PLC.

Abstract:Purpose Large primary liver cancer (PLC) more than 10 cm in diameter is not infrequently encountered in clinical practice. This study evaluated the clinicopathological features and long-term results after surgery for large PLC. Methods Comparison of clinicopathological data between patients with PLC ≥10 cm ( n =1,227) and PLC <10 cm ( n =2,349) during the same period. Results In comparison with patients with PLC <10 cm, patients with PLC ≥10 cm were significantly younger ( P <0.01), had a lower incidence of asymptomatic tumors (9.1% vs 39.5%, P <0.001), higher α-fetoprotein levels ( >400 ng/ml, 78.3% vs 49.2%, P <0.001), higher γ-glutamyl transpeptidase levels ( >6U, 87.7% vs 70.5%, P <0.001), a lower incidence of a history of hepatitis (45.0% vs 61.4%, P <0.001) and associated macronodular cirrhosis (cirrhotic nodules ≥0.3 cm, 59.8% vs 66.6%, P <0.001), poor differentiation of tumor cells (Edmondson grade 3–4, 24.3% vs 19.7%, P <0.01), a lower percentage of single nodule tumors (59.9% vs 75.4%, P <0.001) and well-encapsulated tumors (28.5% vs 62.1%, P <0.001), a higher proportion of tumor emboli in the portal vein (20.5% vs 9.0%, P <0.001), a lower resection rate (50.6% vs 86.8%, P <0.001), a lower curative resection rate (54.8% vs 78.3%, P <0.001), a higher operative mortality rate (4.5% vs 2.3%, P <0.001), and less local resection (52.5% vs 80.2%, P <0.001). The 5- and 10-year survival rates after resection were 26.2% and 17.5%, respectively, for patients with PLC ≥10 cm ( n =621), and 54.3% and 39.5%, respectively, for patients with PLC <10 cm ( n =2039) ( P <0.01). Conclusions Large PLC had specific clinicopathological features. Surgery is the first choice of treatment. In selected patients, resection is safe and offers the chance of long-term survival. Large PLC does not exclude the possibility of cure.
A TACE Oriented Comprehensive Therapy for Unresectable Primary Liver Cancer

A Surgery Oriented Serial Treatments of 191 Patients with Large Primary Liver Cancer

Abstract:OBJECTIVE:To discuss the methods and effects of a series of surgery oriented therapies in the treatment of large primary liver cancers.METHODS:From January 1993 to June 1999, 191 patients with large liver cancer were treated chiefly by operation. The size of the primary cancer varied from 5.2 to 19.7 cm (median 9.4 cm). Various kinds of liver resections were performed in 121 patients and, as a supplement, deep cryosurgery was carried for out the rest 70 patients. Importable drug delivery system (IDDS) was instituted intraoperatively. Transcatheter arterial chemo-embolization (TACE, THP 30-60 mg, E-ADM 20-40 mg, CDDP 40-80 mg, MMC 10-20 mg, iodine oil 5-30 ml), percutaneous ethanol injection (PEI), bioimmunotherapy and the traditional Chinese medicine were used pre- and post-operatively. CT angiography (CTA) and CT during arterial portography (CTAP) were used to find satellite nodules. Early recurrence was diagnosed by AFPmRNA monitor in peripheral blood. Child's classification plus branch chain amino acid/aromatic amino acid (BCAA/AAA) were adopted in evaluating the pre-operative liver functions.RESULTS:Remarkable results were observed after this surgery oriented serial treatment. The 1-, 3- and 5-year survival rates in the resection group were 75.8%, 45.6% and 30.4%, respectively. The 1- and 3-year survival rates in the cryosurgery group were 63.2% and 37.0%. The operative mortality was 1.6%. Recurrence rates were 69.2% in AFPmRNA positive group and 33.3% in AFPmRNA negative group. Comparing this two groups, significant difference was observed (P < 0.05). BCAA/AAA were lower than 1.5 in two patients died after resection.CONCLUSION:A serial treatment with surgery as the chief modality gives satisfactory results in large primary liver cancers. This regimen should be adhered to as a main strategy in dealing with large liver cancers. AFPmRNA in the peripheral blood, signifying a recurrence, may become a hope of a new clinical parameter. BCAA/AAA plus Child's classification is able to evaluate more accurately the hepatic functional reserve before surgery.
Long-term Results of Surgery for Small Primary Liver Cancer in 514 Adults

Abstract:During 1958–1993, 2030 patients with pathologically proven primary liver cancer (PLC) were retrospectively reviewed. Comparison between small PLC (<-5 cm,n=514) and large PLC (>5 cm,n=1516) revealed that small PLC had a higher resection rate (92.4% versus 49.1%), lower operative mortality (1.7% versus 5.2%), a higher percentage of single tumour nodules (78.0% versus 53.4%), a higher percentage of well encapsulated tumour (74.5% versus 35.8%) and higher survival rates after resection (5-year, 63.8% versus 36.6%; 10-year, 46.8% versus 28.5%). No significant difference was found between survival following limited resection (n=440) and lobectomy (n=34) in patients with small PLC. Re-resection of any subclinical recurrence or solitary pulmonary metastasis after small PLC resection was done in 70 cases. These results indicate that resection is still the modality of choice for treatment of small PLC; limited resection instead of lobectomy was the key to increasing resectability and decreasing operative mortality; re-resection of subclinical recurrence was important to prolong survival further.

Abstract:At the Zhong Shan Hospital, Shanghai Medical University, between 1960 and 1991, liver resection was performed in 896 patients with primary liver cancer; local resection was performed in 552 patients (61.6%), left lateral segmentectomy in 114 (12.7%), left hemihepatectomy in 157 (17.5%), extended left hemihepatectomy in 19 (2.1%), right hemihepatectomy in 50 (5.6%), and extended right hemihepatectomy in 4 (0.4%). The overall operative mortality was 4.6%, but it was 22.0% in 1960–1970, 7.0% in 1971–1980, and 2.8% in 1981–1991. Encouraging changes in the prognostic pattern were observed when comparing the data for 1960–1970 (n=59), 1971–1980 (n=115), and 1981–1991 (n=722): the 5-year survival rate was 14.0%, 36.0%, and 50.8%, respectively, and the 10-year survival rate was 12.3%, 25.5%, and 40.8%, respectively. Significant differences in survival patterns were noted when these were analyzed on the basis of tumor size (≤5 vs >5cm), curative resection, tumor number, tumor capsule, and tumor emboli in the portal vein. In the entire series, 135 patients have survived for more than 5 years after resection, and 40 patients for more than 10 years after resection. One patient has survived for 32 years and is still alive, free of disease. The approaches to decreasing operative mortality and prolonging survival rate are discussed.

Abstract:A literature review was written to introduce the choice of therapy for primary liver cancer (PLC).It was concluded that hepatic resection, liver transplantation, cryotherapy, microwave coagulation treatment, radiofrequency ablation, transarterial embolization or transarterious chemoembolization, percutaneous ethanol injection, radiotherapy and biotherapy could increase the survival rate, and surgical resection combined with other therapeutic methods is the main treatment for primary liver cancer.

Abstract:Alternating chemotherapy and fractionated radiotherapy were carried out in 32 patients with surgically proven unresectable primary liver cancer (PLC). After initial surgical intervention of hepatic artery ligation and cannulation, the tumor was localized with silver clips.

Abstract:Objective: To assess the efficacy and safety of computed tomography (CT)-guided percutaneous cryoablation in treating malignant liver tumors located explicitly at high-risk sites. Methods: Data were collected retrospectively from patients with malignant liver tumors undergoing percutaneous cryoablation at Tianjin Medical University Cancer Hospital between January 2018 and December 2021. In all, 46 patients with malignant liver tumors at non-high-risk sites were matched 1∶1 according to the maximum tumor diameter. Technical success rate, complete ablation rate, and complications at 12 and 24 months post-surgery were evaluated. A statistical analysis of the ablation effect difference between the high-risk site and non-high-risk site groups was conducted. Univariate and multivariate logistic regression analyses were performed to identify risk factors. Results: Both groups demonstrated a 100% intraoperative technical success rate, and no major complications related to cryoablation were observed. The complete ablation rate was 82.6% (38/46) and 71.7% (33/46) in the high-risk group and 84.8% (39/46) and 73.9% (34/46) in the non-high-risk group at 12 and 24 months, respectively. There was no significant difference in complete ablation rates between the two groups (P>0.05). Multivariate analysis identified the distance between the tumor edge and high-risk site ≤5 mm and preoperative trans-arterial chemoembolization (TACE) treatment as independent risk factors for cryoablation effect. Conclusion: CT-guided percutaneous cryoablation is a safe and effective approach for patients with malignant liver tumor at high-risk sites. Our results emphasize the importance of proper preoperative planning and intraoperative manipulation.
Long-term Survivors after Resection for Primary Liver Cancer. Clinical Analysis of 19 Patients Surviving More Than Ten Years

Abstract:From July 1958 to June 1978, a total of 333 cases with pathologically proven primary liver cancer (PLC) were admitted to the Zhong Shan Hospital, Shanghai Medical University, Shanghai, the people's Republic of China. Of these, 39.6% (132 of 333) were resected and 14.4% (19 of 132) survived over 10 years after resection for PLC. These 19 patients surviving over 10 years were investigated in this paper. All 19 patients underwent radical resection, including right hemihepatectomy in two cases, left hemihepatectomy in ten cases, left lateral segmentectomy in three cases, and local resection in four cases. By the end of June 1988, follow-up varied from 10 years and 1 month to 26 years and 7 months, with a mean follow-up of 15 years and 4 months. All 19 patients are still alive with free of disease. The longest survival patient had a tumor measuring 10 X 8 X 6 cm in size and underwent local resection. Upon follow-up after 26 years and 7 months, the patient was found to be still living and well. Two patients with intraperitoneal ruptured PLC have survived for 19 years and 4 months, and 16 years and 11 months, respectively, after resection of the tumors free of disease and have returned to work. Subclinical recurrence of PLC was discovered in one patient in whom reoperation with cryosurgery was carried out. The patient has been in good condition with negative alpha-fetoprotein (AFP) for 8 years and 10 months after cryosurgery. Subclinical solitary pulmonary metastasis was detected in two patients because of a secondary rise in AFP level. Reoperations were carried out and the metastatic tumors were removed. These two patients are still in good health with negative AFP 9 years and 6 months, and 10 years and 1 months, respectively, after reoperation. These results indicate that early and radical resection are the principal factors influencing long-term survival; reoperation for subclinical recurrence and solitary metastasis remains an important approach to prolong survival further; intraperitoneal rupture of PLC does not exclude the possibility of cure; new surgical techniques, such as cryosurgery and bloodless hepatectomy, have been shown to be effective in some patients.
Changed Clinical Aspects of Primary Liver Cancer in China During the Past 30 Years.

Abstract:BACKGROUND:Primary liver cancer (PLC) is one of the most frequently seen tumors in China. Thirty years ago, patients with PLC were often detected at relatively late stage, with a palpable mass or marked clinical symptoms and poor prognosis. In the past 30 years, the diagnosis and treatment of PLC have been greatly improved with better prognosis.METHODS:In order to study the changes of PLC during the 30 years, the clinical data of 3250 patients with PLC from 10 medical institutions of China were collected, analyzed, and compared with those of 3254 PLC patients before the 30 years.RESULTS:In the 3250 patients aged 1-80 years, with an average age of 49.1 years, the male to female ratio (2.3:1) was lower than that before the 30 years. 73.5% of the 3250 patients sought medical advice within 3 months after the onset of the disease in contrast to 63.8% before the 30 years. Compared with those patients before the 30 years the symptoms and signs were alleviated generally. The HBsAg positive rate was 81.0%, but the HCV-Ab positive rate was 13.2%. The AFP level in 75% of patients was elevated, but in the remaining 25% was normal. 1912 patients (58.8%) were confirmed pathologically. Among them 1755 patients (91.8%) had hepatocellular carcinoma. The overall resection rate was 46.3%. Those who had early, middle, late stage carcinoma accounted for 29.9%, 51.5%, and 18.6% respectively in contrast to 0.4%, 47.0%, and 52.6% reported before the 30 years. The 1-, 3-, 5-year survival rates of the patients were 66.1%, 39.7%, and 32.5% respectively, whereas 93.5%, 70.1%, and 59.1% for the early stage patients, and 65.3%, 30.5%, and 23.5% for the middle stage patients. The half and 1-year survival rates of the late stage patients were 52.5%, and 14.7%, respectively.CONCLUSION:Comparison with the clinical data before and after the 30 years show that PLC can be diagnosed early. More PLC patients tend to undergo resection while receiving a better conservative treatment, which ensures a prognosis.
