Abstract:Objective To investigate the therapeutic effect of treatment with biofeedback for functional constipation of children.Methods Thirty - seven cases of children functional constipation were treated with biofeedback,and the variation of anus rectum measure and clinical symptoms of pretherapy and post - treatment were compared.Results Anal resting pressure,maximum coarctation pressure,rectum threshold of perception current,defecation urgency threshold,maximally tolerated dose and clinical symptoms of post - treatment were significantly better than pretherapy(P<0.05),the total effective rate was 81.08%.Conclusion Treatment with biofeedback can ameliorate anus rectum pressure effectively and can be used for children functional constipation safely and effectively.

Abstract:Objective:To observe the effects of biofeedback therapy in patients with the clinical symptoms of constipation and anal elet romyographic activities.Methods:Eighty-nine outlet obstructive constipation patients were treated by biofeedback therapy.Compare the difference before and after the treatment in clinical symptoms, anal elet romyogr aphicactivities and colonic transit time.Results:After biofeedback therapy, the clinical symptoms of constipation in patients with effective rate of 77.53%, after treatment with symptoms, abnormal pelvic floor muscle contraction was significantly reduced compared with before treatment(all P<0.01), after treatment with bowel urgency threshold decreased(P=0.01), gastrointestinal tract, the experiment was significantly improved compared with before treatment(P <0.05).Conclusion:Biofeedback treatment of outlet obstruction constipation is an effective way.
[Forlax in the Treatment of Childhood Constipation: a Randomized, Controlled, Multicenter Clinical Study].

Abstract:OBJECTIVE:To determine the therapeutic effectiveness and safety of polyethylene glycol 4000 (forlax) in the treatment of constipation in children over 8 years old.METHODS:This study was designed as a randomized, positive medicine (lactulose) controlled multicenter trial. A total of 216 children with constipation from 8-18 years old from 7 hospitals across China who were matched with a uniform entry criteria were enrolled in this study. The 216 patients were randomized to receive either oral forlax (20 g/d, n=105) or lactulose (15 mL/d, n=111) for 2 weeks. The therapeutic effects, including bowel movement frequency, stool consistency, clinical complete remission rate of constipation and abdominal symptoms, and the safety of forlax and lactulose were evaluated at 1 and 2 weeks of treatment.RESULTS:The median weekly frequency of bowel movement in the forlax group increased by 4 and 5 times respectively after 1 and 2 weeks of treatment, and increased by 3 and 4 times in the lactulose group (P < 0.05). The stool consistency of the two groups was both improved significantly after treatment. The Bristol score of stool consistency of the forlax and lactulose groups were 3.41+/-1.11 and 3.64+/-1.33 respectively (P < 0.05) after 1 week of treatment, and were 4.26+/-0.89 and 3.63+/-1.33 respectively (P < 0.05) after 2 weeks of treatment. The clinical complete remission rate of constipation in the forlax and lactulose groups was 70% and 40% respectively (P < 0.05) by week 1 of treatment, and that was 72% and 41% respectively (P < 0.05) by week 2 of treatment. Abdominal pain disappeared in 75% of patients in the forlax group but in only 57% in the lactulose group by week 2 of treatment (P < 0.05). No serious adverse events happened and no abnormalities were found in laboratory tests and physical examinations in the two groups after medication.CONCLUSIONS:Forlax is safe and effective in the treatment of constipation in children over 8 years old.

Abstract:BACKGROUND: Functional constipation (FC) is a common functional gastrointestinal disease with various clinical manifestations. It is a physical and mental disease, which seriously affects patient physical and mental health and quality of life. Biofeedback therapy is the treatment of choice for FC, especially outlet obstructive constipation caused by pelvic floor dysfunction. High-quality nursing is a new nursing model in modern clinical work and a new concept of modern nursing service.AIM: To explore the effect of biofeedback combined with high-quality nursing in the treatment of FC.METHODS: A total of 100 patients with FC admitted to our hospital from March 2015 to July 2019 were selected for clinical observation. These patients were randomly divided into two groups of 50: Experimental group (biofeedback combined with high-quality nursing treatment group) and control group (biofeedback group).RESULTS: The constipation symptom score of the experimental group was significantly lower than that of the control group, and the difference was statistically significant (P < 0.05). The anal canal resting pressure and initial defecation threshold of the experimental group were significantly lower than those of the control group, and the maximum squeeze systolic pressure of the anal canal of the experimental group was significantly higher than that of the control group (P < 0.05). The Self-Rating Anxiety Scale and Zung's Self-Rating Depression Scale scores of the two groups were significantly lower than before treatment. The Self-Rating Anxiety Scale and Self-Rating Depression Scale scores of the experimental group were significantly lower than those of the control group (P < 0.05). The patient satisfaction score of the experimental group was significantly higher than that of the control group (P < 0.05).CONCLUSION: The application of biofeedback combined with high-quality nursing in the treatment of FC has significant advantages over pure biofeedback treatment, and it is worthy of promotion in clinical work.

Polyethylene Glycol 4000 Treatment for Children with Constipation: A Randomized Comparative Multicenter Study

Abstract:The aim of this study was to evaluate the efficacy and safety of polyethylene glycol 4000 (PEG 4000) for the treatment of constipation in children over 8 years of age. A total of 216 children from 7 hospitals were enrolled. A total of 105 patients received oral PEG 4000 (20 g/day) and 111 patients received oral lactulose (15 ml/day) for 2 weeks. The stool frequency, stool consistency and abdominal pain of the patients were monitored. In the PEG group, following one week and two weeks of treatment, the median weekly stool frequency improved from 2 times prior to treatment to 6 and 7 times, respectively, following treatment. The clinical remission rates of the PEG and lactulose groups following one week of treatment were 70.48 and 39.64%, respectively, and following two weeks of treatment were 72.38 and 41.44%, respectively. Abdominal pain disappeared in 74.6% of patients following two weeks of PEG 4000 treatment. No significant clinical adverse effects or abnormalities in the laboratory tests were observed in the two treatment groups. In conclusion, PEG 4000 is a safe and more effective drug compared to lactulose for the treatment of constipation in children.

Abstract:Objectives:To study and observe the influence of biofeedback therapy on the pelvic floor mus-cle and urodynamic state of patients with stress incontinence.Methods:60 patients with stress incontinence in our hospital from April 2011 to April 2014 were selected and randomly divided into control group (conventional pelvic floor muscle training group)and observation group (biofeedback therapy group),30 cases in each group.The mic-turition related indexes,pelvic floor muscle,urine pad test and ICS urinary incontinence quality of life question-naire,urodynamic indexes of two groups before the treatment and 4th and 8th week after the treatment were respec-tively detected and compared.Results:The micturition related indexes,pelvic floor muscle and urodynamic indexes of the observation group at 4th and 8th week after the treatment were all significantly better than those of control group,all P <0.05,with significant differences.Conclusion:The influence of biofeedback therapy for the pelvic floor muscle and urodynamic state of patients with stress incontinence is great,which is more helpful for the effective and quick improvement of disease.

Abstract:Objective To study the clinical effects of biofeedback therapy combined with mental and psychological treatment for functional constipation (FC). Methods According to the Rome Ⅱ diagnostic criteria, 90 patients with functional constipation were recruited into this study. Each patient received Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) and then 15 times of biofeedback therapy plus mental and psychological treatment. The clinical symptom scores were established to observe the improvement of constipation symptoms after treatment. Results Of the 90 patients, 11.1%(10/90) were cured, 38.9% (35/90) were markedly effective, 33.3%(30/90) were effective, and 16.7%(15/90) were invalid. The total efficiency rate was 83.3% in the 90 FC patients after biofeedback therapy plus mental and psychological treatment. The FC patients' SAS score and SDS score were significant improved after treatment ( < 0.01). Conclusions Biofeedback combined with mental and psychological treatment can significantly improve the clinical symptoms of patients with functional constipation. Scores of anxiety and depression in FC patients are significantly improved compared with those before treatment. The study indicates that biofeedback combined with mental and psychological treatment has good effect on the treatment of functional constipation.
[The effects of biofeedback training on clinical symptoms, psychological status and quality of life in patients with functional constipation]

Abstract:Objective: To explore the effects of biofeedback training on clinical symptom, psychological state and quality of life in patients with functional constipation (FC). Methods: Forty-nine patients with FC diagnosed by Rome III were enrolled and received biofeedback training. Bowel symptom measure, Zung's self-rating anxiety scale (SAS), self-rating depression scale (SDS) and Chinese version of the MOS 36-item short form healthy survey (SF-36) were recorded to assess the effects before and after treatment. Results: After biofeedback training, clinical symptom of patients with FC was greatly improved: there was a very significant decrease in total and subscales scores of bowel symptom including spontaneous frequency of bowel movements, straining effort, sensation of anorectal obstruction/blockage, stool consistency and bloating. Patients with FC also improved their quality of life as well as psychological status after biofeedback. All subcategories of SF-36 including general health, physical function, bodily pain, role physical, vitality, social function, role emotion and mental health showed marked increase. Compared to the scores before biofeedback training, SAS (41.0±8.1 vs 46.5±11.9) and SDS (44.0±8.2 vs 51.2±11.5) scores decreased significantly after biofeedback training. Conclusion: Biofeedback training can improve clinical symptom, psychological status and quality of life in patients with FC.

Abstract:In order to study the quality of life of patients with functional constipation based on dynamic magnetic resonance defecation, the biofeedback therapy combined with comprehensive nursing intervention was used to diagnose and treat the patients, so as to explore its clinical efficacy and its impact on patients’ quality of life. The obstructed defecation surgical treatment carries frequent recurrences, and dynamic magnetic resonance imaging defecography evaluated and elucidated the underlying anatomic features. This research selected 80 patients who came to our hospital for treatment of functional constipation and evaluated and recorded various clinical indicators before and after treatment in the form of questionnaire survey. The results showed that the clinical symptom scores of patients with functional constipation before and after treatment were greatly different P < 0.05 . Thus, the biofeedback therapy combined with comprehensive nursing intervention showed a good clinical effect in the treatment of patients with functional constipation and significantly improved the quality of life of patients, showing high clinical application and promotion value. A convenient diagnostic procedure is represented by the dynamic magnetic resonance imaging in females, especially pelvic floor organs dynamic imaging during defecation.

Abstract:BackgroundPelvic floor dyssynergia (PFD) is one of the causes of chronic constipation. Laxative-based therapies are not very effective in treating this type of constipation. The therapeutic effectiveness of three therapeutic strategies, including biofeedback (BOF) alone, BOF+ fiber laxative (psyllium), and BOF +osmotic laxative (polyethylene glycol; [PEG]), was assessed in patients with constipation secondary to PFD.MethodsEighty-eight patients with constipation were included during a period from 2017 to 2018. Thirty-two patients were treated with BOF alone, 25 patients received BOF+ fiber laxative (psyllium), and 31 patients received BOF+ osmotic laxative (PEG) for 2 to 3 months. A checklist was used to compare outcomes before and after the interventions.ResultsSatisfaction rates from the treatments were 60.83%, 46.88%, and 41.32% in the BOF, BOF + psyllium, and BOF + PEG groups, respectively. Women had a higher satisfaction rate compared to men. Defecation quality improved, and the frequency of enema usage decreased (p <0.05) in all the groups after intervention. Difficulty in evacuation, need for digitation, and return to the toilet after defecation significantly improved in the BOF alone group. Using laxatives reduced straining during evacuation and increased the duration of defecation. All the three regimens reduced rectal bleeding (p <0.05).ConclusionsCombinations of laxatives with BOF did not offer significant therapeutic benefit. As laxatives may cause dissatisfaction and incomplete/prolonged defecations in patients with PFD, adding laxatives to the BOF regimen is not recommended for these patients.

Abstract:Constipation with obstructive defecation and fecal incontinence are problems that are frequently encountered in the gastrointestinal clinic (1). Until recently, most patients were treated with supportive or empirical therapy or surgery. However, over the last decade, biofeedback therapy has emerged as a useful adjunct for the treatment of these problems. Biofeedback therapy is a learning process that is based on “operant conditioning” techniques. The governing principle is that any behavior—be it a complex maneuver such as eating or a simple task such as muscle contraction—when reinforced, its likelihood of being repeated increases (2).

Abstract:Biofeedback (BF) is the therapy of choice for functional defecation disorders of any etiology. BF was reported as an effective short‐term cure for constipation in up to 63% of patients. A straining questionnaire and a digital rectal examination augmented with abdominal palpation on straining were associated to BF response. Background Functional defecation disorders (FDD) are a common etiology of refractory chronic constipation (CC). FDD diagnosis (dyssynergic defecation [DD] and inadequate defecatory propulsion [IDP]), requires diagnostic tests including anorectal manometry (ARM) and balloon expulsion test (BET). Biofeedback (BF) is the treatment of choice for DD. The aims of our study were to evaluate: the outcome of BF in a group of constipated patients with defecatory disorders of any etiology; the efficacy of two simple diagnostic tools in predicting BF outcome in the short‐term. Methods One hundred and thirty‐one refractory CC patients failing the BET underwent BF therapy. Before BF, all patients underwent the following: ARM. Straining questionnaire. The answers were: "belly muscles"; "anal muscles"; "both"; "Don't know/No answer." Digital rectal examination augmented by abdominal palpation on straining (augmented‐DRE). The BF therapist was blinded to ARM, straining questionnaire, and augmented‐DRE results. Key Results Eighty‐one patients responded to BF. Gender, age, and IBS‐C showed no significant impact on BF response. Therapeutic Effect of Changqiang Acupoint Injection on Children with Functional Fecal Incontinence

Abstract:Objective: Functional fecal incontinence in children (FFIC), also known as functional fecal disorder in children, is a type of childhood diseases characterized by excretory dysfunction. In the study, we observed the clinical effect of Changqiang acupoint injection on functional fecal incontinence in children (FFIC) by randomized controlled clinical trials. Materials and methods: 140 children with functional fecal incontinence who met the screening criteria were respectively assigned into the treatment group (100 cases) and the control group (40 cases) according to the completely randomized controlled design. In the treatment group, Chuankezhi injection was administrated at the Changqiang acupoint once a week for three weeks (one course of treatment). In contrast, the subjects in the control group was instructed to receive sphincter exercise and defecation training, followed by an assessment on the therapeutic efficacy after one course of treatment. Results: The overall response rate of the two groups was 93.00% (93/100) and 57.50% (23/40), respectively, with a statistically significant difference (P<0.01). The treatment group showed a performance superior to the control group based on the Cleveland Clinic Florida Fecal Incontinence Score System (CCF-FI) — dry stool incontinence, liquid incontinence, gas incontinence, lifestyle change, necessity of using pads or antidiarrheals, the ability to delay defecation, and the total score. The inter-group comparison revealed a statistically significant difference (P<0.01). Conclusion: Changqiang acupoint injection has significant curative effect on FFIC as demonstrated by significant relief in the symptoms associated with fecal incontinence, and thus is considered as a useful approach to be widely applied in clinical practice.

[Long-term outcomes of individualized biofeedback training based on the underlying dysfunction for patients with imperforate anus].

Abstract:Objective Although better anorectal function was achieved due to the advanced operation procedures, the fecal incontinence and constipation still remain as major postoperative complications. The aim of this study was to design a individualized biofeedback program based on the underlying dysfunctions and to assess the short-term and long-term clinical and quality of life outcomes for patients with imperforate anus. Methods Thirty-one patients were treated using individualized biofeedback program according to their different dysfunctions after completed assessment of anorectal function. The many of special biofeedback training methods that consisted of the strengthening perianal muscles biofeedback, sensory discrimination biofeedback, synchronizing external sphincter contraction biofeedback and improving defecation dynamic biofeedback were selected to make up the individualized biofeedback training program. The clinical scores and quality of life scores were assessed at short-term and long-term follow-up. Results The clinical scores and quality of life scores in patients at before training group, after biofeedback training group, short-term follow-up group and long-term follow-up group were 3.2 +/- 1.1 and 8.7 +/- 1.1, 5.2 +/- 0.6 and 11.0 +/- 0.8, 5.0 +/- 0.6 and 10.8 +/- 0.9, and 4.6 +/- 0.7 and 10.0 +/- 0.7 respectively. After biofeedback training, the first leak volume and all parameters that evaluated the function of the perianal muscles were increased significantly, the abnormal rectal threshold sensations and defecation dynamics were reverted to normal, and 82% patients who have not external anal sphincter reflex acquired the new reflex. The latencies of pudendo-anal reflex have significant difference in patients with poor response compared with those patients with good response before and after the biofeedback training. At short-term follow-up 12 patients (57%) maintained the clinical outcome and only 9 patients (43%) have regressions slightly. At long-term follow-up 7 patients (33%) maintained the clinical outcome very well and 14 patients (67%) have regressions. Although the clinical and quality of life scores have decreased slightly at follow-up, they were still increased significantly compared with before biofeedback training group. Conclusion The causes of postoperative fecal dysfunction in patients with imperforate anus are multifactorial, and the individualized biofeedback is more suitable for improvement of the clinical outcome and the quality of life, and it maintain good clinical outcome and quality of life at short-term and long-term follow-up.
Biofeedback in patients with ileoanal pouch dysfunction: a specialist centre experience

Abstract:Objectives: To report outcomes following biofeedback for functional problems associated with an ileoanal pouch. Incontinence and evacuatory disorders associated with the ileoanal pouch can be particularly problematic and difficult to treat using conventional therapies. Biofeedback therapy is a behavioural treatment that offers a non-surgical approach as an alternative or adjunct for patients. Materials and methods: This was a retrospective single centre study. We reviewed the notes of all patients attending for biofeedback at our institution between January 2012 and October 2017 and identified all those that did so for ileoanal pouch related problems. We recorded patient reported subjective improvements following biofeedback. The validated International Consultation on Incontinence Questionnaire was used to assess improvement in incontinent symptoms and the evacuatory disorder questionnaire was used to assess improvement in evacuatory disorders. Results: Twenty-six patients with ileoanal pouch related problems underwent biofeedback. Based on patients' feedback at next clinical encounter following biofeedback, nine reported much improvement, 11 reported some improvement and six reported no improvement. In the group treated for incontinence, quality of life improved significantly from a median pre-treatment score of 80 to a post-treatment score of 41 (p = .01). Biofeedback reduced pain, bloating straining and laxative use in patients with evacuatory disorders. Conclusions: Biofeedback may be associated with significant improvement in quality of life as well as possible improvements in symptoms related to both incontinence and evacuatory disorders. It is probably an underused service. Further larger prospective studies are required to properly assess the efficacy of biofeedback in ileoanal pouch related dysfunction.
Efficacy of Non-pharmacologic Auxiliary Treatments in Improving Defecation Function in Children With Chronic Idiopathic Constipation: A Systematic Review and Network Meta-analysis

Abstract:Background: Non-pharmacologic auxiliary treatments have been considered crucial therapies for treating chronic idiopathic constipation (CIC) during the past decades worldwide. Several treatment patterns are available, but their relative efficacy is obscure because there are no head-to-head randomized controlled trials, especially in children. We conducted this network meta-analysis to evalute the effectiveness of these therapies in improving defecation function based on their direct comparisons with standard medical care. Methods: Medline, Embase, and Cochrane Central were searched for randomized controlled trials (RCTs) published in English from inception to October 2020, assessing the efficacy of auxiliary therapies (behavior therapy, physiotherapy, biofeedback, or anorectal manometry) in children with CIC. We extracted data for endpoints, risk of bias, and evidence quality. Eligible studies in the meta-analysis reported the data of a dichotomous assessment of overall response to treatment (response or not) or defecation frequency per week after treatment. The hierarchical Bayesian network meta-analysis was used in the study. We chose a conservative methodology, random effects model, to pool data which could handle the heterogeneity well. The relative risk (RR) with 95% confidence intervals (CIs) was calculated for dichotomous outcomes. For continuous results, weighted mean difference (WMD) with related CIs was calculated. The included treatments were ranked to define the probability of being the best treatment. Results: Seven RCTs (838 patients) met inclusion and endpoint criteria. Based on an endpoint of the absence of constipation (Rome criteria) with laxatives allowed, physiotherapy plus standard medical care (SMC) had the highest probability (84%) to bethe most effective therapy. When the treatment response was defined as an absence of constipation with not laxatives allowed, biofeedback plus SMC ranked first (probability 52%). Physiotherapy plus SMC ranked first when the endpoint was based on defecation frequency per week with laxatives allowed (probability 86%). Conclusion: Almost all auxiliary therapies are effective complementary therapies for treating CIC, but they needed to be used simultaneously with SMC. Nevertheless, because of the small number of eligible studies and their small sample sizes, the differences in treatment duration and the endpoints, large sample RCTs with long-term follow-up are required for further investigation.

The Effect of Biofeedback Treatment for Children with Non‐neurogenic Voiding Dysfunction: A Systematic Review and Meta‐analysis

Abstract:Children's non-neurogenic voiding dysfunction (NVD) is a syndrome characterized by lower urinary tract symptoms (LUTs) because of the inability to relax the external sphincter. Patients with NVD always suffer from urinary tract infections (UTI), incontinence, constipation. The aim of this study is to assess the efficacy of biofeedback treatment for children's NVD.PubMed, Embase, Cochrane library database were searched for all relevant studies. Two independent reviewers decided whether to include the study, conducted quality evaluation, and extracted article data. A random-effects model was used to calculate overall effect sizes. Risk ratio (RR) and mean difference (MD) with 95% confidence interval (CI) served as the summary statistics for meta-analysis. And sensitivity analysis was subsequently performed.Fifteen studies and 1274 patients were included in the systemic review, seven RCTs and 539 patients were included in meta-analysis. Meta-analysis showed efficacy of biofeedback treatment in following aspects, (1) relieving UTI (RR: 1.71, 95% CI: 1.11 to 2.64), (2) reducing PVR (MD: 9.51, 95% CI: 2.03 to 16.98), (3) increasing maximum urine flow rate (MD: 4.28, 95% CI: 2.14 to 6.42) and average urine flow rate (MD: 1.49, 95% CI: 0.53 to 2.46), (4) relieving constipation (RR: 1.59, 95% CI: 1.12 to 2.26)，(5) improving abnormal voiding pattern (RR: 1.75, 95% CI: 1.30 to 2.36) and abnormal EMG during voiding (RR: 1.55, 95% CI: 1.25 to 1.91). The improvement of UTI symptoms, maximum urine flow rate and average urine flow rate took a longer time (12 months). In terms of daytime incontinence (RR: 1.20, 95% CI [0.96, 1.50], p = 0.11), nighttime incontinence (RR: 1.20, 95% CI [0.62, 2.32], p = 0.58), no significant difference was found between biofeedback treatment and standard urotherapy. The qualitative analysis showed that biofeedback treatment was beneficial for NVD.Compared with standard urotherapy, biofeedback treatment is effective for some symptoms, such as UTI and constipation, and can improve some uroflowmetric parameters, such as PVR. Biofeedback treatment seems to have a better long-term effect.

Abstract:Chronic constipation is a prevalent disorder with considerable impact on healthcare costs and quality of life. Most patients would respond to conservative measures in primary care. Patients with refractory constipation are commonly referred to dedicated centers for appropriate investigations and management. After testing, three main subtypes of constipation are commonly identified: normal colon transit, slow transit, and functional defecation disorders. The etiology of functional defecation disorders is consistent with maladaptive behavior, and biofeedback therapy has been considered a valuable treatment option. Being safe and only marginally invasive, retraining has been historically employed to manage all types of refractory constipation. There are a number of strongly held beliefs about biofeedback therapy that are not evidence-based. The aim of this review was to address these beliefs concerning protocols, efficacy, indications, and safety, with a special focus on the relevance of identifying patients with a functional defecation disorder who are ideal candidates for retraining. Randomized controlled trials support the effectiveness of biofeedback therapy for severe, refractory constipation due to functional defecation disorders. Limitations of the treatment are discussed, but biofeedback remains the safest option to successfully manage this hard-to-treat subtype of constipation.

The effect of biofeedback on nonneurological dysfunctional voiding in children: A meta-analysis and systematic review

Abstract:Objective: This study was conducted to investigate the effect of biofeedback (BF) on the rehabilitation of children with nonneurological dysfunctional voiding (NDV). Methods: RCTs were retrieved from various databases (published from inception to February 29, 2024). The effects of the BF and non-BF treatments were compared. A random-effects model was used to evaluate the combined data. Results: Meta-analysis revealed that BF increased the maximum urinary flow rate (SMD = 3.78, 95% CI 1.33∼6.22), improved urination time (SMD = 5.88, 95% CI 3.75∼8.01), and reduced the postvoid residual (SMD = -19.18, 95% CI -27.03∼-11.33) and urinary tract infection incidence (RR = 0.43, 95% CI 0.21∼0.87). Electromyogram activity (RR = 0.46, 95% CI 0.25∼0.84) and abnormal urination patterns (RR = 0.51, 95% CI 0.35∼0.74) improved, with effects persisting for more than 1 year. However, the effect of BF on the mean urinary flow rate in children with NDV was significant only after 1 year of follow-up (SMD = 1.90, 95% CI 0.87∼2.92). Conclusion: Existing evidence indicates that BF can enhance urinary parameters and patterns in children with NDV. However, its effectiveness in addressing constipation, daytime urinary incontinence, and nocturnal urinary incontinence is not substantial. High-quality randomized controlled trials can offer additional insights.

Abstract:AIM: To discuss the classification and pathophysiological mechanism of constipation caused by spastic pelvic floor syndrome (SPFS) in adults,and to investigate the mechanism of biofeedback training in treating this type of constipation. METHODS: Biofeedback training was per-formed on 18 adult patients suffered from SPFS-caused constipation using the instruments pro-duced by Medtronic Company with the basic circuit voltage of 1.53 μV for about 20-minute relaxation exercises. The basic resting and stimu-lated voltages of external anal sphincter (EAS) were obtained by the anal electrode,and then compared using statistical analysis. RESULTS: Based on the resting voltage and the voltage change of EAS during simulation,the SPFS-caused constipation was classified into 3 types: type Ⅰ was characterized with a high resting voltage and paradoxical activity of EAS (44.44%); type Ⅱ was characterized with a high resting voltage of EAS (33.33%); type Ⅲ was characterized with paradoxical activity of EAS (22.22%). The patients with high resting voltages covered a percentage of 77.77%. There existed significant differences among three types (P= 0.001). CONCLUSION: High resting voltages and sec-ondary paradoxical activity may be the mecha-nisms of SPFS-caused constipation. A significant decrease of resting voltage and inhibition of EAS paradoxical activity can be obtained through biofeedback training.
