Abstract:We investigated a novel method combining principal component analysis (PCA) and supervised learning technique, support vector machine (SVM), for classifying carcinoma lesion from normal tissue with light-induced autofluorescence. The autofluorescence spectral signals were collected in vivo from 85 nasopharyngeal carcinoma lesions and 131 normal tissue sites from 59 subjects during routine nasal endoscopy. With the combined PCA and SVM classifying algorithm, the achieved overall accuracy is over 97%, companied with 95% sensitivity and 99% specificity for discriminating carcinoma from normal tissue. In comparison with the previously developed algorithms based on PCA method, this new method outperforms threshold- and probability-based PCA algorithms in all instances. The experimental results indicate great promise for autofluorescence spectroscopy based detection of small carcinoma lesion in the nasopharynx and other tissues.

Abstract:Cell-intrinsic reporters such as luciferase (LUC) and red fluorescent protein (RFP) have been commonly utilized in preclinical studies to image tumor growth and to monitor therapeutic responses. While extrinsic reporters that emit near infrared I (NIR-I: 650-950 nm) or near-infrared II (NIR-II: 1000 -1700 nm) optical signals have enabled minimization of tissue autofluorescence and light scattering, it has remained unclear as to whether their use has afforded more accurate tumor imaging in small animals. Here, we developed a novel optical imaging construct comprised of rare earth lanthanide nanoparticles coated with biodegradable diblock copolymers and doped with organic fluorophores, generating NIR-I and NIR-II emissive bands upon optical excitation. Simultaneous injection of multiple spectrally-unique nanoparticles into mice bearing tumor implants established via intraperitoneal dissemination of LUC+/ RFP+ OVCAR-8 ovarian cancer cells enabled direct comparisons of imaging with extrinsic vs. intrinsic reporters, NIR-II vs. NIR-I signals, as well as targeted vs. untargeted exogenous contrast agents in the same animal and over time. We discovered that in vivo optical imaging at NIR-II wavelengths facilitates more accurate detection of smaller and earlier tumor deposits, offering enhanced sensitivity, improved spatial contrast, and increased depths of tissue penetration as compared to imaging with visible or NIR-I fluorescent agents. Our work further highlights the hitherto underappreciated enhancements in tumor accumulation that may be achieved with intraperitoneal as opposed to intravenous administration of nanoparticles. Lastly, we found discrepancies in the fidelity of tumor uptake that could be obtained by utilizing small molecules for in vivo as opposed to in vitro targeting of nanoparticles to disseminated tumors. In Vivo Detection of Cervical Intraepithelial Neoplasia by Multimodal Colposcopy

Abstract:Cervical cancer is the leading cause of cancer death for women in developing countries. Colposcopy plays an important role in early screening and detection of cervical intraepithelial neoplasia (CIN). In this paper, we developed a multimodal colposcopy system that combines multispectral reflectance, autofluorescence, and RGB imaging for in vivo detection of CIN, which is capable of dynamically recording multimodal data of the same region of interest (ROI). We studied the optical properties of cervical tissue to determine multi-wavelengths for different imaging modalities. Advanced algorithms based on the second derivative spectrum and the fluorescence intensity were developed to differentiate cervical tissue into two categories: squamous normal (SN) and high grade (HG) dysplasia. In the results, the kinetics of cervical reflectance and autofluorescence characteristics pre and post acetic acid application were observed and analyzed, and the image segmentation revealed good consistency with the gold standard of histopathology. Our pilot study demonstrated the clinical potential of this multimodal colposcopic system for in vivo detection of cervical cancer.

Abstract:To examine the cervical lesions by using the tumor-targeted near-infrared (NIR) fluorescent dyes TMTP1-PEG4-ICG, a dual modal colposcope with visible reflectance imaging and fluorescence imaging was developed. NIR fluorescence imaging and visible light reflectance imaging were integrated together on a colposcope by designing the specific optics and adopting a dual sensor charge coupled device (CCD) camera. Patients with squamous cell carcinoma, adenocarcinoma, cervical intraepithelial neoplasia (CIN) and cervicitis were examined using this dual modal colposcope to validate its potential of cervical cancer detection. Fluorescent dye TMTP1-PEG4-ICG was applied to the cervix 30 minutes before inspection. The fluorescence images were collected after wiping the unbound fluorescent dye using normal saline. Signal to back ratios (SBR) of the fluorescence images were analyzed and compared with the histological analysis. The results suggest that the fluorescent colposcope combined with tumor-specific near-infrared fluorescent dyes TMTP1-PEG4-ICG could help to evaluate cervical lesions in real time.
Real-time In vivo Diagnosis of Nasopharyngeal Carcinoma Using Rapid Fiber-Optic Raman Spectroscopy

Abstract:We report the utility of a simultaneous fingerprint (FP) (i.e., 800-1800 cm-1) and high-wavenumber (HW) (i.e., 2800-3600 cm-1) fiber-optic Raman spectroscopy developed for real-time in vivo diagnosis of nasopharyngeal carcinoma (NPC) at endoscopy. A total of 3731 high-quality in vivo FP/HW Raman spectra (normal=1765; cancer=1966) were acquired in real-time from 204 tissue sites (normal=95; cancer=109) of 95 subjects (normal=57; cancer=38) undergoing endoscopic examination. FP/HW Raman spectra differ significantly between normal and cancerous nasopharyngeal tissues that could be attributed to changes of proteins, lipids, nucleic acids, and the bound water content in NPC. Principal components analysis (PCA) and linear discriminant analysis (LDA) together with leave-one subject-out, cross-validation (LOO-CV) were implemented to develop robust Raman diagnostic models. The simultaneous FP/HW Raman spectroscopy technique together with PCA-LDA and LOO-CV modeling provides a diagnostic accuracy of 93.1% (sensitivity of 93.6%; specificity of 92.6%) for nasopharyngeal cancer identification, which is superior to using either FP (accuracy of 89.2%; sensitivity of 89.9%; specificity of 88.4%) or HW (accuracy of 89.7%; sensitivity of 89.0%; specificity of 90.5%) Raman technique alone. Further receiver operating characteristic (ROC) analysis reconfirms the best performance of the simultaneous FP/HW Raman technique for in vivo diagnosis of NPC. This work demonstrates for the first time that simultaneous FP/HW fiber-optic Raman spectroscopy technique has great promise for enhancing real-time in vivo cancer diagnosis in the nasopharynx during endoscopic examination.

Abstract:The label‐free imaging and spectroscopy method was studied on cervical unstained tissue sections obtained from 36 patients. The native fluorescence spectra of tissues are analyzed by the optical redox ratio (ORR), which is defined as fluorescence intensity ratio between NADH and FAD, and indicates the metabolism change with the cancer development. The ORRs of normal tissues are consistently higher than those of precancer or cancerous tissues. A criterion line of ORR at 5.0 can be used to discriminate cervical precancer/cancer from normal tissues. The sensitivity and specificity of the native fluorescence spectroscopy method for cervical cancer diagnosis are determined as 100% and 91%. Moreover, the native fluorescence spectroscopy study is much more sensitive on the healthy region of cervical precancer/cancer patients compared with the traditional clinical staining method. The results suggest label‐free imaging and spectroscopy is a fast, highly sensitive and specific method on the detection of cervical cancer.

Abstract:The study reports the usage of a spatially resolved fibre‐optic probe with multiple fibres for fluorescence spectra collection from cervical tissue samples, addressing data imbalance problem using SMOTE, dimensional reduction and feature extraction using PCA, and ANN for classification purpose. Cervical cancer can be detected at an early stage through the changes occurring in biochemical and morphological properties of epithelium layer. Fluorescence spectroscopy has the ability to identify these subtle changes non‐invasively and in real time with good accuracy in comparison with conventional techniques. In this paper, we report the usage of a custom designed spatially resolved fibre‐optic probe (SRFOP), which consists of 77 fibres in two concentric rings, for the detection of cervical cancer using fluorescence spectroscopy technique. The aim of this study is to classify different grades of cervical precancer on the basis of their fluorescence spectra followed by a robust classification algorithm. Fluorescence spectra of 28 cervical tissue samples of different categories have been recorded using six detector fibres of FOP at different spatial locations with the source fibre (SF). A 405 nm laser diode source has been utilised to excite the samples and a USB 4000 Ocean Optics spectrometer to collect the output spectra in the wavelength range 400–700 nm. Principal component analysis (PCA) was applied to the collected spectra to reduce the dimensionality of the data while preserving the most significant features for classification. The first 10 principal components, which captured the majority of the variance in the spectra, were selected as input features for the classification model. Classification was then performed using an artificial neural network (ANN) with a specific architecture, including an input layer, hidden layers, and a softmax activation function in the output layer. Experimental and classification results both demonstrate that proximal fibres (PFs) perform better than distal fibres (DFs) in capturing the discriminatory features present in the epithelium layer of cervical tissue samples as PF collect most of the signal from the epithelium layer. The combined approach of spatially resolved fluorescence spectroscopy and PCA‐ANN classification techniques is able to discriminate different grades of cervical precancer and normal with an average sensitivity, specificity and accuracy of 93.33%, 96.67% and 95.57%, respectively.

Abstract:Breast and cervical cancers are becoming the leading causes of death among women worldwide, but current diagnostic methods have many drawbacks, such as being time-consuming and high cost. Raman spectroscopy, as a rapid, reliable, and non-destructive spectroscopic detection technique, has achieved many breakthrough results in the screening and prognosis of various cancer tumors. Therefore, in this study, Raman spectroscopy technology was used to diagnose breast cancer and cervical cancer. A total of 225 spectra were recorded from 87 patients with cervical cancer, 60 patients with breast cancer, and 78 healthy individuals. The obvious difference in Raman spectrum between the three groups was mainly shown at 809 cm −1 (tyrosine), 958 cm −1 (carotenoid), 1004 cm −1 (phenylalanine), 1154 cm −1 (β-carotene), 1267 cm −1 (Amide III), 1445 cm −1 (phospholipids), 1515 cm −1 (β-carotene), and 1585 cm −1 (C = C olefinic stretch). We used one-way analysis of variance for these peaks and demonstrated that they were significantly different. Then, we combined the detected Raman spectra with multivariate statistical calculations using the principal component analysis-linear discrimination algorithm (PCA-LDA) to discriminate between the three groups of collected serum samples. The diagnostic results showed that the model's accuracy, precision, recall, and F1 score of the model were 92.90%, 92.62%, 92.10%, and 92.36%, respectively. These results suggest that Raman spectroscopy can achieve ultra-sensitive detection of serum, and the developed diagnostic models have great potential for the prognosis and simultaneous screening of cervical and breast cancers.

Abstract:Nasopharyngeal cancer (NPC) is an endemic with high incidence in Southern China and Southeast Asia countries. Screening for NPC under conventional white light imaging (WLI) nasopharyngoscope examination remains a great clinical challenge due to its poor sensitivity. Here, we developed an integrated 4-modality endoscopy system combining WLI, autofluorescence imaging (AFI), diffuse reflectance spectroscopy and Raman spectroscopy technologies for in vivo endoscopic cancer detection for the first time. A pilot clinical test of the system for NPC detection was conducted, in which 283 in vivo Raman and diffuse reflectance spectral data sets from 30 NPC patients and 30 healthy subjects were acquired under the guidance of AFI and WLI. Both high diagnostic sensitivity (98.6%) and high specificity (95.1%) for differentiating cancer from normal tissue sites were achieved using this system combined with principal component analysis-linear discriminant analysis diagnostic algorithm, demonstrating great potential for improving real-time, in vivo diagnosis of NPC at endoscopy.

Abstract:Raman spectroscopy provides a unique biochemical fingerprint capable of identifying and characterizing the structure of molecules, cells, and tissues. In cervical cancer, it is acknowledged as a promising biochemical tool due to its ability to detect premalignancy and early malignancy stages. This review summarizes the key research in the area and the evidence compiled is very encouraging for ongoing and further research. In addition to the diagnostic potential, promising results for HPV detection and monitoring treatment response suggest more than just a diagnosis prospective. A greater body of evidence is however necessary before Raman spectroscopy is fully validated for clinical use and larger comprehensive studies are required to fully establish the role of Raman spectroscopy in the molecular diagnostics of cervical cancer.

Rapid Fiber-optic Raman Spectroscopy for Real-Time In Vivo Detection of Gastric Intestinal Metaplasia during Clinical Gastroscopy

Abstract:We report a unique simultaneous fingerprint (FP) and high-wavenumber (HW) Raman spectroscopy technique coupled with a beveled fiber-optic Raman probe for improving in vivo detection of gastric intestinal metaplasia (IM)-precancerous lesions in real-time during clinical gastroscopy. A total of 4,520 high-quality in vivo FP/HW gastric Raman spectra (normal = 4,178; IM = 342) were acquired from 157 gastric patients undergoing endoscopic examination. Multivariate diagnostic algorithms based on principal components analysis and linear discriminant analysis together with the leave-one tissue site-out, cross-validation on in vivo tissue Raman spectra yield the diagnostic sensitivities of 89.3%, 89.3%, and 75.0%; specificities of 92.2%, 84.4%, and 82.0%; positive predictive values of 52.1%, 35.2%, and 28.4%; and negative predictive values of 98.9%, 98.8%, and 97.2%, respectively, by using the integrated FP/HW, FP, and HW Raman techniques for identifying IM from normal gastric tissue. Further, ROC curves generated show that the integrated FP/HW Raman technique gives the integration area under the ROC curve of 0.92 for IM classification, which is superior to either FP (0.89) or HW Raman (0.86) technique alone. This work demonstrates for the first time that the simultaneous FP/HW fiber-optic Raman spectroscopy has great potential to enhance early diagnosis of gastric precancer in vivo during routine endoscopic examination. Clinical Study of Autofluorescence Imaging Combined with Narrow Band Imaging in Diagnosing Early Gastric Cancer and Precancerous Lesions.

Abstract:PURPOSE To evaluate the diagnostic values of autofluorescence imaging (AFI) combined with narrow band imaging (NBI) in diagnosing early gastric cancer and precancerous lesions. METHODS 140 patients were investigated for lesions such as gastric mucosal roughness, erosion, plaque, abnormal color, bump or pitting by conventional endoscopy. AFI endoscope was used for the observation, and the endoscopic diagnosis was performed according to the fluorescence forms under the endoscopic AFI mode, as well as the changes of gastric mucosal capillaries and gastric pits under the NBI mode. The corresponding lesions were also biopsied for pathological examination. RESULTS 78 patients were diagnosed as gastritis (including superficial and atrophic gastritis), 45 as intestinal metaplasia, 6 as dysplasia, and 11 as early gastric cancer. The sensitivity and specificity of AFI-NBI combination were 88.89 and 91.58% in the diagnosis of intestinal metaplasia; 83.33 and 98.51% for dysplasia; and 90.91 and 99.22% for early gastric cancer. All of them were significantly higher than the simple fluorescence endoscopy. CONCLUSIONS The AFI-NBI combination could improve the detection rate of early gastric cancer and precancerous lesions.
Rapid Detection of Nasopharyngeal Cancer Using Raman Spectroscopy and Multivariate Statistical Analysis.

Abstract:Optical spectroscopic techniques, including Raman spectroscopy, have shown promise for in vivo cancer diagnostics in a variety of organs. In this study, the potential use of a home-made Raman spectral system with a millimeter order excitation laser spot size combined with a multivariate statistical analysis for the rapid detection and discrimination of nasopharyngeal cancer from normal nasopharyngeal tissue was evaluated. Raman scattering signals were acquired from 16 normal and 32 nasopharyngeal carcinoma tissue samples. Linear discriminant analysis (LDA) based on principal component analysis (PCA) and partial least squares (PLS) were employed to generate diagnostic algorithms for the classification of different nasopharyngeal tissue types. Spectral differences in Raman spectra between the two types of tissues were revealed; the normalized intensities of Raman peaks at 1,001, 1,207 and 1,658 cm-1 were more intense for nasopharyngeal carcinoma tissue compared to normal tissue, while Raman bands at 848, 936 and 1,446 cm-1 were stronger in normal nasopharyngeal samples. The PCA-LDA algorithm together with leave-one-out cross validation yields a diagnostic sensitivity of 81% and a specificity of 87%, while the PLS method coupled with subwindow permutation analysis improves the diagnostic sensitivity and specificity to 85 and 88%, respectively. Therefore, Raman spectroscopy combined with PCA-LDA/PLS demonstrated good potential for improving the clinical diagnosis of nasopharyngeal cancers.

Abstract:Cervical cancer remains a major cause of morbidity and mortality among women, especially in the developing world. Increased synthesis of proteins, lipids and nucleic acids is a pre-condition for the rapid proliferation of cancer cells. We show that scanning near-field optical microscopy, in combination with an infrared free electron laser (SNOM-IR-FEL), is able to distinguish between normal and squamous low-grade and high-grade dyskaryosis and between normal and mixed squamous/glandular pre-invasive and adenocarcinoma cervical lesions, at designated wavelengths associated with DNA, Amide I/II and lipids. These findings evidence the promise of the SNOM-IR-FEL technique in obtaining chemical information relevant to the detection of cervical cell abnormalities and cancer diagnosis at spatial resolutions below the diffraction limit (≥0.2 μm). We compare these results with analyses following attenuated total reflection Fourier-transform infrared (ATR-FTIR) spectroscopy; although this latter approach has been demonstrated to detect underlying cervical atypia missed by conventional cytology, it is limited by a spatial resolution of ~3 μm to 30 μm due to the optical diffraction limit.

Detecting Temporal and Spatial Effects of Epithelial Cancers with Raman Spectroscopy

Abstract:Epithelial cancers, including those of the skin and cervix, are the most common type of cancers in humans. Many recent studies have attempted to use Raman spectroscopy to diagnose these cancers. In this paper, Raman spectral markers related to the temporal and spatial effects of cervical and skin cancers are examined through four separate but related studies. Results from a clinical cervix study show that previous disease has a significant effect on the Raman signatures of the cervix, which allow for near 100% classification for discriminating previous disease versus a true normal. A Raman microspectroscopy study showed that Raman can detect changes due to adjacent regions of dysplasia or HPV that cannot be detected histologically, while a clinical skin study showed that Raman spectra may be detecting malignancy associated changes in tissues surrounding nonmelanoma skin cancers. Finally, results of an organotypic raft culture study provided support for both the skin and the in vitro cervix results. These studies add to the growing body of evidence that optical spectroscopy, in this case Raman spectral markers, can be used to detect subtle temporal and spatial effects in tissue near cancerous sites that go otherwise undetected by conventional histology.

Abstract:Raman-based methods offer a promising approach for in vivo biological detection. However, the fluorescence of biological samples will significantly affect Raman measurement accuracy. Moreover, due to the existence of excitation wavelength-dependent fluorescent molecules in biological tissues, especially porphyrin molecules, the fluorescence also exhibits significant wavelength dependence. To achieve high-quality Raman spectra of biological tissue, in this work we proposed a dual-wavelength Raman method. Two lasers with different wavelengths were used to excite optical signals in the same region, and the ordinary fluorescence and additional wavelength-dependent fluorescence in the biological samples could be eliminated by two-step normalization calibration; thus, the accuracy of Raman measurement was significantly enhanced. We applied this method to early cancer diagnosis and identified several molecules and structures worthy of attention in carcinogenesis for esophageal tissue, such as phenylalanine and the CC bonds of porphyrins. Normal, precancerous, and early cancer samples were successfully identified by the changes in biomolecules with associated degrees of malignancy. Thus, the imaging and diagnosis of indefinite tumors were realized, which verified the potential of the dual-wavelength Raman method in biological detection.
Real-time in vivo diagnosis of laryngeal carcinoma with rapid fiber-optic Raman spectroscopy

Abstract:We assess the clinical utility of a unique simultaneous fingerprint (FP) (i.e., 800-1800 cm-1) and high-wavenumber (HW) (i.e., 2800-3600 cm-1) fiber-optic Raman spectroscopy for in vivo diagnosis of laryngeal cancer at endoscopy. A total of 2124 high-quality in vivo FP/HW Raman spectra (normal = 1321; cancer = 581) were acquired from 101 tissue sites (normal = 71; cancer = 30) of 60 patients (normal = 44; cancer = 16) undergoing routine endoscopic examination. FP/HW Raman spectra differ significantly between normal and cancerous laryngeal tissue that could be attributed to changes of proteins, lipids, nucleic acids, and the bound water content in the larynx. Partial least squares-discriminant analysis and leave-one tissue site-out, cross-validation were employed on the in vivo FP/HW tissue Raman spectra acquired, yielding a diagnostic accuracy of 91.1% (sensitivity: 93.3% (28/30); specificity: 90.1% (64/71)) for laryngeal cancer identification, which is superior to using either FP (accuracy: 86.1%; sensitivity: 86.7% (26/30); specificity: 85.9% (61/71)) or HW (accuracy: 84.2%; sensitivity: 76.7% (23/30); specificity: 87.3% (62/71)) Raman technique alone. Further receiver operating characteristic analysis reconfirms the best performance of the simultaneous FP/HW Raman technique for laryngeal cancer diagnosis. We demonstrate for the first time that the simultaneous FP/HW Raman spectroscopy technique can be used for improving real-time in vivo diagnosis of laryngeal carcinoma during endoscopic examination.

Abstract:Background The challenges in cancer diagnosis underline the need for continued research and development of new diagnostic tools and methods. This study aims to explore an effective, noninvasive, and convenient diagnostic tool using urine based near-infrared spectroscopy (NIRS) analysis combined with machine learning algorithm. Methods Urine samples were collected from a total of 327 participants, including 181 cancer cases and 146 healthy controls. These participants were randomly spit into train set ( n = 218) and test set ( n = 109). NIRS analysis (4,000 ∼10,000 cm −1 ) was performed for each sample in both train and test sets. Five pretreatment methods, including Savitzky-Golay (SG) smoothing, multiplicative scatter correction (MSC), baseline removal (BSL) with fitting polynomials to be used as baselines, the first derivative (DERIV1), and the second derivative (DERIV2), and combination with “scaling” and “center”, were investigated. Then partial least-squares (PLS) and linear support-vector machine (SVM) classification models were established, and prediction performance was evaluated in test set. Results NIRS had greatly overlapping in peaks, and PCA analysis failed in separation between cancers and healthy controls. In modeling with urine based NIRS data, PLS model showed its highest prediction accuracy of 0.780, with DERIV2, “scaling” and “center” pretreatment, while linear SVM displayed its best prediction accuracy of 0.844, with raw NIRS. With optimization in SVM, the prediction accuracy could improve to 0.862, when the top 262 features were involved as variables. Discussion This pilot study combining urine based NIRS analysis and machine learning is effective and convenient that might facilitate in cancer diagnosis, encouraging further evaluation with a large-size multi-center study.
