Abstract:•We examined the effect of combined use of alcohol and cocaine on injury and aggressive incidents among 616 patients in treatment for substance abuse.•Respondents reported their use of alcohol and/or cocaine within 3 and 6 h before a recent injury or aggression and reported detailed quantity and frequency information on their typical use of alcohol and cocaine.•The rate ratio of acute injury and aggressive incident due to alcohol and/or cocaine use was estimated using the Mantel-Haenszel estimator.•In the 6h window before the event, combined use of alcohol and cocaine was each significantly (P< 0.05) related to a recent injury or aggressive incident.•Rate ratio of aggression was significantly greater for combined use within a 3-h window than a 6-h window.
