Abstract:Objective: Chronic diseases are complex and persistent clinical conditions that require close collaboration among patients and health care providers in the implementation of long-term and integrated care programs. However, current solutions focus partially on intensive interventions at hospitals rather than on continuous and personalized chronic disease management. This study aims to fill this gap by providing computerized clinical decision support during follow-up assessments of chronically ill patients at home.Methods: We proposed an ontology-based framework to integrate patient data, medical domain knowledge, and patient assessment criteria for chronic disease patient follow-up assessments. A clinical decision support system was developed to implement this framework for automatic selection and adaptation of standard assessment protocols to suit patient personal conditions. We evaluated our method in the case study of type 2 diabetic patient follow-up assessments.Results: The proposed framework was instantiated using real data from 115,477 follow-up assessment records of 36,162 type 2 diabetic patients. Standard evaluation criteria were automatically selected and adapted to the particularities of each patient. Assessment results were generated as a general typing of patient overall condition and detailed scoring for each criterion, providing important indicators to the case manager about possible inappropriate judgments, in addition to raising patient awareness of their disease control outcomes. Using historical data as the gold standard, our system achieved a rate of accuracy of 99.93% and completeness of 95.00%.Conclusions: This study contributes to improving the accessibility, efficiency and quality of current patient follow-up services. It also provides a generic approach to knowledge sharing and reuse for patient-centered chronic disease management. (C) 2017 Elsevier Inc. All rights reserved.
Development and initial validation of the chronic hepatitis B quality of life instrument (CHBQOL) among Chinese patients

Abstract:Purpose This study developed and tested preliminary measurement properties of a Chinese scale specifically designed to measure HRQOL in patients with chronic hepatitis B (CHBQOL). Methods We conducted 94 individual interviews with CHB patients to solicit items and five hepatitis expert interviews along with three rounds of cognitive interviews to identify problems with relevance and understanding of content. A cross-sectional validation study was then conducted to evaluate measurement properties ( n = 578). Factor analysis was used to determine the latent structure of the scale. Reliability was evaluated through Cronbach’s alpha coefficients and intra-class correlation coefficients (ICCs). Measurement model adequacy, convergent, discriminant, and known-groups validity were also examined. Results A scale of 30 items was drafted. After item reduction, the remaining 23 items were assigned to the CHBQOL Somatic symptoms, Emotional symptoms, Belief and Social stigma domains, which had acceptable goodness of fit ( χ 2 / df = 3.13, GFI = 0.90, AGFI = 0.88, RMSEA = 0.06, SRMR = 0.05). All the CHBQOL domains had satisfactory reliability with Cronbach’s α coefficients ranging from 0.73 to 0.91 and ICCs were higher than 0.70 except for Belief domain (ICC = 0.54). Convergent and discriminant validity were acceptable as supported by significant item-domain correlations (0.64–0.90). In general, the correlations between CHBQOL and the SF-36 dimensions met hypotheses. Significant differences were found by mean scores in the subgroups of demographic and clinical variables, supporting the known-groups validity. Conclusions The CHBQOL instrument proved to be an appropriate tool for assessing HRQOL among Chinese CHB patients.

Abstract:Background: To develop a quantitative system to enable straightforward objective assessment of health status in real time.Methods: On the basis of routine urinalysis with an automated urine analyzer, we obtained the urinalysis ill-health index (UIHI), which was the sum of urinary protein, pH, specific gravity, and color. Clinical laboratory parameters including stress indicators (neutrophilic leukocytes and glucose) and over-nutrition indicators (triglycerides, cholesterol, and uric acid) were monitored. The electrocardiography and blood pressure were also examined. A total of 1128 subjects were selected randomly from those who underwent physical examination to verify the UIHI.Results: The range of the UIHI score was 0 to 5. The cutoff value was 2.75. There were correlations of UIHI score with laboratory parameters and abnormal rates of BP and ECG. A greater UIHI score reflected worse health status (p < 0.05).Conclusions: These data suggest that the UIHI can reflect the health status of an individual. It can be derived easily from routine urinalysis in a clinical laboratory and may be a useful tool for the quantitative differentiation of health status in real time.
Mining Personal Health Satisfaction and Disease Index from Medical Examinations

Abstract:People have regular medical examinations every year, the primary purpose of these is not only to discover a specific disease but to have a clear overview of their current health. Therefore, it is essential to be able to scientifically analyze and evaluate the results of medical examinations, and provide people with comprehensive feedback on physical health satisfaction (PHS) or a specific disease index (DI) feedback. PHS can reflect the current state of a person’s body directly. In this paper, we propose a framework for calculating personal PHS and DI, by which the process of assessing people’s health is indicated. We use the public dataset of the National Center for Health and Nutrition to conduct a scientific analysis of the population medical examinations data. Finally, based on a comprehensive experiment of 6166 participants’ medical examinations data and some experiments with data from participants with heart disease, we verified the effectiveness of the proposed framework.

Abstract:Aims: This study aimed to construct a standardized health risk assessment index system (HRAIS) under the guidance of general health and facilitate the family doctors to carry out chronic disease management.Methods: Available chronic disease surveillance systems and indexes were reviewed to identify potential indexes. The Delphi method was performed to establish the HRAIS, and the analytic hierarchy process was used to calculate the index weight.Results: HRAIS included four first-level indexes and 38 second-level indexes. The authority coefficient was 0.86. The Kendall's W for the two rounds of Delphi consultation were 0.202 and 0.210 (<i>p</i> < 0.001). The weights of the first-level indexes from high to low were physiological health (0.409), psychosocial health (0.290), health-related behaviors (0.205), and environment (0.097). Thus, HRAIS is a multi-dimension and multi-index tool, which can be used as a guideline for family doctors in early screening, early intervention, and classified management of main chronic diseases.

Abstract:Development of cancer screening biomarkers usually follows the Early Detection Research Network 5-Phase guideline in Pepe et al. A key feature of this guide is that the phased development follows a sequential order, moving to the next phase only when the current phase study is complete and has met its target performance. Motivated by a newly funded Newly onset Diabetes cohort study, we propose a design evaluating new biomarkers to discriminate between cases and controls in the presence of an existing screening test. The proposed design achieves two goals: (1) avoiding bias in estimating sensitivity or specificity in predicting cancer at a given time period prior to clinical diagnosis, using data from both screening detected cancers in Phase IV study and clinically diagnosed cancers in Phase III study; and (2) building a panel with biomarkers for Phase III and IV studies based on all data. A simulation study shows that the proposed design outperforms both a conventional method using data in Phase III arm only and a naive method using data in Phase III and IV arms ignoring the difference between the time of screening the detected cancer and the time of clinical diagnosis. The proposed design yields a smaller standard error of the estimation and increases the statistical power to confirm biomarker performance. This proposed method has the potential to shorten the cancer screening biomarker development process, use resources more effectively, and bring benefits to patients quickly.
Developing an assessment tool for the healthy lifestyles of the occupational population in China: a modified Delphi-analytic hierarchy process study

Abstract:Lifestyle is an important factor affecting people's health. Evaluating and promoting healthy lifestyles among occupational population can not only improve work ability and efficiency, but also contribute to the prevention a variety of potential diseases. This study developed a preliminary index system based on literature review and group discussion. Two rounds of expert consultation were conducted on 12 experts using Delphi method. The questionnaire recovery rate of the two rounds of Delphi survey was over 85%, the expert authority coefficient was over 0.70, and Kendall W was 0.450 and 0.446, which were significant (P < 0.001). The weighting coefficient of each indicator was calculated using the hierarchical analysis method. Among them, the weighting coefficients of physical health lifestyle, mental health lifestyle, social adaptation lifestyle and occupational health lifestyle were 0.4133, 0.2922, 0.1078 and 0.1867, respectively. The consistency index CI = 0.024 and the consistency ratio CR = 0.027 of the first-level index judgment matrix. The weighting coefficient is acceptable and the indicators do not cause logical confusion. The healthy lifestyle assessment tool of the occupational population constructed in this study consists of 4 primary indicators, 13 secondary indicators and 45 tertiary indicators, which can provide a standardized and operable assessment tool for monitoring and evaluating the healthy lifestyle of the Chinese occupational population. At the same time, the weight analysis of various indicators through the analytic hierarchy process can also provide reference for the key areas of occupational health intervention.
Development and Validation of a Novel Combinational Index of Liquid Biopsy Biomarker for Longitudinal Lung Cancer Patient Management

Abstract:Objectives Many cancer biomarkers such as the circulating tumor cells/microemboli (CTCs/CTM) have been reported significant associations with clinical outcomes. However, different biomarkers have different sensitivities and specificities for cancer types and cohort patients, and synergistic effects between certain biomarkers have also been observed, leading to the inaccurate, fluctuating, and even controversial results when multiple biomarkers are analyzed together. In this paper, a novel combinational index, P-score, was developed for monitoring and predicting the disease condition of lung cancer patients during follow-up visits. Materials and Methods There were totally 13 return patients with 54 blood samples involved in this study to examine the number of CTC and CTM. Information from one group of 7 patients including 27 blood samples with published clinical data was employed to develop while those from another group of 4 patients containing 14 blood samples with unpublished clinical data were used to validate the P score in prediction. Enumerations were based on immunofluorescent staining images. Distributions of CTC/CTM and their frequencies in stratified patients were carefully examined and analyzed the ROC curve and AUC value to develop the P score and P score-based prediction model. Results and Conclusion We found that the predictive power of P-score was not only comparable to the traditional cancer marker, in comparison with individual CTC/CTM, more false positives could be corrected by using P-score, thereby to improve the accuracy of analysis. From our preliminary validation tests, the prognosis and disease progression monitored longitudinally by P-score were further confirmed by clinical outcome data from physicians and its sensitivity was even better than those from individual biomarkers. We believe that this novel combinational indicator could be a promising tool to interpret clinical outcomes more accurately from multiple factors, particularly useful for the early prognosis and longitudinal monitoring in cancer patient management.

Abstract:Heart disease can lead to physical disability and mortality, ranking second among the top 10 causes of death according to the Ministry of Health and Welfare. This study aims to examine the impact of the interactive assessment application on patients' public health knowledge. In this study, a single-group pretest and posttest experimental design was adopted. Thirty-six hospitalized patients diagnosed with heart failure participated in the pretest and posttest assessments. The findings demonstrate that the developed application led to an increase in the number of recorded physiological measurements, effectively enabling patients to manage their blood pressure. The heart failure self-management application was observed to improve patients' understanding and awareness of heart failure disease, improving their self-management skills.

Abstract:Background: and Purpose: Enhancing patient satisfaction remains crucial for healthcare quality. The utilization of artificial intelligence (AI) in the Internet of Health Things (loHT) can streamline the medical examination process. Most Traditional Chinese Medicine (TCM) examinations are non-invasive and contribute significantly to patient satisfaction. Our aim was to establish an intelligent physical examination system that amalgamates TCM and Western medicine and to conduct a preliminary investigation into its effectiveness in enhancing the satisfaction of patients with chronic diseases. Materials and methods: Experts from clinical departments, the equipment department, and the software development department were invited to participate in group discussions to determine the design principles and organizational structure of the intelligent physical examination system. This system integrates TCM and Western medicine. We compared the satisfaction levels of patients examined using the intelligent physical examination system with those examined using the traditional medical examination system. Results: An intelligent physical examination system, combining TCM and Western medicine, was developed. A total of 106 patients were finally enrolled (intelligent group vs. control group) to evaluate satisfaction. There were no statistically significant differences between the intelligent group and the control group in age, gender, education, or income level. We identified significant differences in five aspects of satisfaction: 1) the physical examination environment; 2) the attitude and responsiveness of doctors; 3) the attitude and responsiveness of nurses; 4) the effectiveness of obtaining results; and 5) the information regarding physical examination and medical advice (p < 0.05). Furthermore, these differences remained statistically significant even after adjusting for age, gender, education, and income level. Conclusions: The intelligent physical examination system effectively capitalized on the advantages of combining AI with the integration of TCM and Western medicine, substantially optimizing the medical examination process. In comparison to the traditional physical examination system, the intelligent system significantly enhanced patient satisfaction. Future improvements could involve integrating chronic disease follow-up technology into the system.
Construction of Health Effect-Oriented Comprehensive Evaluation Index System for Essential National Public Health Service

Abstract:Objective To construct an effect-oriented comprehensive evaluation index system for national basic public health services, and to provide methods and tools for innovating evaluation method of basic public health projects and promoting the transformation from process evaluation to health outcome evaluation. MethodsPrimary, secondary and tertiary indexes were constructed and selected with literature review, field interview of senior professionals, expert Delphi consulting and logical analysis; subjective and objective weights of the indexes were calculated through analytic hierarchy process and entropy weight method. ResultsThrough literature analysis and interview consultation, an index pool consisting of 3 first-, 25 second- and 104 third-level indicators was preliminarily formed, including 47 third-level indicators reflecting health effects. For the first round of expert Delphi consultation on indicator screening, the positive coefficient of experts is 97.5%, the expert authority coefficient is 0.82, and the expert familiarity coefficient is 0.88. The final comprehensive evaluation index system consists of three first-level indexes of project investment, implementation and health effect, with the weights of 11.35%, 26.48% and 62.18%, along with 20 second- and 54 third-level indexes. Among the health effect indicators, the weight of awareness of and satisfaction to health services was the highest (16.39%), followed by the weight of health management in chronic disease patients (14.97%), children′s health management (12.42%), maternal health management (12.42%), and health education (5.43%). Among the three-level effect indexes, the weight of residents′ satisfaction to health service was the highest (7.25%), followed by the incidence of hypertension and diabetes complications in the managed population (5.35%) and the incidence of diabetes complications in the managed population (5.27%). Besides the health statistics from routine monitoring system in China, the electronic health records in residents′ information system can be used as a main data source for health effect indicators. ConclusionThe comprehensive index system constructed in this study for the evaluation on essential national public health service is effect-oriented and can be used for regular and periodical evaluation on the basis of making full use of residents′ health records in the basic public health information system.
Index design of core elements of health examination report and pilot data analysis

Abstract:Objective:To analyze the core elements′ index design of physical examination reports and pilot data.Methods:We searched and analyzed the core element indexes of the physical examination report in databases such as CNKI and Wanfang through the literature analysis method, forming the initial core element indicator set. Special working meetings were organized to discuss revisions and improvements. The core elements′ index set was formed using a mixed research method combined with the quantitative analysis results of pilot data and expert opinions. Six hundred and thirteen cases from five hospitals were included in this study, including Anzhen Hospital, Peking University Third Hospital, Beijing Physical Examination Center, Beijing Tongren Hospital, China-Japan friendship hospital.Results:The subjects were (49.5±17.6) years old, including 302 males (49.3%) and 311 females (50.7%). The physical examination conclusion indicates that the number of health problems is between five and eight, with no significant difference between men and women. The health history data analysis of different sexes showed that hypertension in men was significantly higher than in women (17.9% vs. 10.6%, P=0.010). The lifestyle and eating habits survey showed that men had significantly higher incidences of smoking and drinking than women (24.8% vs. 2.2%, P?0.001; 9.9% vs. 1.3%, P?0.001). The exercise habits and sleep survey showed that the proportion of men who did not exercise was lower than that of women (13.2% vs. 22.8%, P=0.022). There was no statistically significant difference in sleep duration between the sexes. Conclusions:The core elements of health examination reports can standardize the collection of physical examination data and improve its sharing and application. This can allow the reports to better assist the decisions based upon physical examination report data.
Changes in Patient-Reported Health Status in Advanced Cancer Patients from a Symptom Management Clinic: A Longitudinal Study Conducted in China

Abstract:Objectives. The integration of patient-reported health status has been increasingly emphasised for delivering high-quality care to advanced cancer patients. This research is designed to track health status changes over time in Chinese advanced cancer patients to explore the risk factors affecting their health status. Methods. Advanced cancer patients were recruited from Peking University Cancer Hospital. An electronic patient-reported outcome (ePRO) system with validated measurements was used to collect the data. ANOVA, the chi-square test, the nonparametric Kruskal–Wallis H test, and generalized estimating equation (GEE) analysis were used for the data analysis. Results. One hundred and three patients completed a baseline survey (T = 0) and two follow-up surveys (T1 = 14 days, T2 = 28 days). Chi-square test results indicate a significant decrease in the percentage of patients reporting moderate or severe difficulty experienced by patients in terms of mobility, pain/discomfort, and anxiety/depression. However, there is a significant increase in the percentage of patients reporting moderate or severe difficulty in self-care and usual activities. Scores on the visual analogue scale in the EQ-5D-5L instrument (EQ-VAS) are associated with patients' income, and the degree of moderate or severe anxiety/depression is found to be associated with employment status. The GEE results show that pain, loss of appetite, poor walking status effected by symptoms, depression, and anxiety has worsened the health status. Conclusions. The health status of Chinese advanced cancer patients under ePRO follow-up in China significantly improves in the physical and psychological dimensions, accompanied by a decrease in usual activities and self-care. Routine screening and rational supportive care are recommended in oncology for cancer care. Based on the rational application of ePRO, longitudinal studies exploring the potential mechanisms of health status changing would provide more beneficial guidance for improving the quality of life in patients with advanced cancer.

The preliminary development and clinical verification of the positive index score scale of "Heart Arthralgia Syndrome"

Abstract:Background: In recent years, the age of onset for coronary heart disease (CHD) has become one of the leading causes of death worldwide. The medical treatments occasionally cause side effects; therefore, there is still an urgent need to develop new therapeutic modalities for CHD in clinical practice. "Heart Arthralgia Syndrome (HAS)" is a general term for CHD with arthralgia symptoms proposed by our team based on clinical experience. At present, there is little in-depth research on the treatment of HAS by TCM. Pick Complex Therapy (PCT) is an innovative and developed theory of collateral acupuncture therapy for HAS. Methods: We collected data from 276 patients who met the criteria for (coronary heart disease with numbness of neck, shoulder, waist, and leg). We selected 24 diagnostic criteria for HAS by means of multiple methods, including Cronbach's α coefficient, retest reliability, subjective evaluation, discrete trend, Pearson's rank correlation coefficient and factor analysis method. We thereafter evaluated the reliability, validity and responsiveness of the scale. In the clinical validation phase, we verified whether the preliminary developed positive index (PI) scale can guide clinical practice. Forty (40) patients with HAS were selected in the study. SPSS23.0 statistical software was used for statistical processing and analysis. Results: Assessment results of the initial PI scale for HAS: the average time to complete the scale was 7.47 ± 3.59 minutes. Cronbach's α coefficient for the initial table was 0.711, the retest reliability was 0.897, the Kaiser-Meyer-Olkin test result was 0.844, and the Bartlett test result was 2502.300. Following maximum variance rotation analysis, the cumulative variance contribution rate was determined to be 66.605%. In the clinical validation phase of the PI scale, we tested 40 patients before and after the PCT treatments. After 3 measurements, the correlation between the PI scale for HAS and the angina pectoris grading scoring method table decreased gradually. The last 2 measurement results of study indicated that there was a significant correlation between the PI scale and thrombin time, while physical and chemical examination showed no significant changes. Conclusion: The PI scale for HAS can be widely used in the clinic as a preliminary evaluation tool for guiding PCT.

Abstract:Objective To establish a set based on the work flow and the step laboratory monitor quality control target.Methods The methods of literatures review,focus group discussion and specialists consultation were employed to develop the index system of quality control of detection relevant articles of health.Results The index of quality control system was composed of fifteen indicators,Among them there were five key indexes,include records of completed sampling rate,transport and storage of the records in accordance with the conditions rate,detection methods controlled rate,quality control efficient rate,detection accuracy rate results.Conclusion Identify indicators conforms to practical work need in Center for Disease Control and Prevention(CDC),and may provide the basis of quality control in laboratory examination.
Fuzzy Method for Assessing the Health Status of Cardiovascular System

Abstract:Objective To study the method of evaluating the health status of cardiovascular system,provide a guide of early prevention and treatment for subjects with potential cardiovascular diseases.Method 51 incipient hypertensives and 39 health subjects were selected.Four physiological indexes were obtained by exercise experiment.The subjection functions for the indexes based on statistics were built respectively.The evaluating function of health was finally constructed by discriminant matrix of Analytic Hierarchy Process.Result There are significant differences between two groups in four physiological indexes.Scores computed by the evaluating function accorded with the true health status of two groups.Conclusion The constructed evaluating function could reflect true health status of test objects.This fuzzy analysis method provided a new idea for evaluating health status of cardiovascular system.
Developing Computerized Adaptive Testing for a National Health Professionals Exam: An Attempt from Psychometric Simulations

Abstract:Introduction: The accurate assessment of health professionals' competence is critical for ensuring public health safety and quality of care. Computerized Adaptive Testing (CAT) based on the Item Response Theory (IRT) has the potential to improve measurement accuracy and reduce respondent burden. In this study, we conducted psychometric simulations to develop a CAT for evaluating the candidates' competence of health professionals. Methods: The initial CAT item bank was sourced from the Standardized Competence Test for Clinical Medicine Undergraduates (SCTCMU), a nationwide summative test in China, consisting of 300 multiple-choice items. We randomly selected response data from 2000 Chinese clinical medicine undergraduates for analysis. Two types of analyses were performed: first, evaluating the psychometric properties of all items to meet the requirements of CAT; and second, conducting multiple CAT simulations using both simulated and real response data. Results: The final CAT item bank consisted of 121 items, for which item parameters were calculated using a two-parameter logistic model (2PLM). The CAT simulations, based on both simulated and real data, revealed sufficient marginal reliability (coefficient of marginal reliability above 0.750) and criterion-related validity (Pearson's correlations between CAT scores and aggregate scores of the SCTCMU exceeding 0.850). Discussion: In national-level medical education assessment, there is an increasing need for concise yet valid evaluations of candidates' competence of health professionals. The CAT developed in this study demonstrated satisfactory reliability and validity, offering a more efficient assessment of candidates' competence of health professionals. The psychometric properties of the CAT could lead to shorter test durations, reduced information loss, and a decreased testing burden for participants.
Development and validation of a condition-specific measure for chronic periodontitis: Oral health impact profile for chronic periodontitis.

Abstract:Aim: The aim of this study was to develop and validate the first condition-specific tool to assess oral health-related quality of life (OHRQoL) impairment in association with chronic periodontitis and to use it as an outcome measure in clinical trials. Methods: Twenty-three potential items were developed using a combined method consisting of a literature review, semi-structured patient interviews (n=20) and expert panel 1. Subsequently, item selection was carried out by means of qualitative content analysis and quantitative classical test theory (CTT) with 110 patients and expert panel 2. Consequently, 18 items were selected and included in the final oral health impact profile for chronic periodontitis (OHIP-CP) instrument. The validity and reliability of the OHIP-CP were then examined. Results: A total of 238 patients participated in the validation phase of OHIP-CP. The final 18-item questionnaire was shown to have a three-domain structure. The three components "pain and functional limitation", "psychological discomfort" and "psychological disability and social handicap" explained 49.9%, 9.9%, and 6.2% of the variance, respectively, together accounting for 66.1% of the variance. In addition, the OHIP-CP was shown to be valid in distinguishing patients with varying severity of chronic periodontitis. For the convergent validity, the reported impact of chronic periodontitis based on the OHIP-CP significantly correlated with the global oral health rating. All of its three domains were found to have good levels of internal consistency with Cronbach's alpha >.7, and excellent test-retest reliability. Conclusion: The OHIP-CP is the first condition-specific OHRQoL instrument for patients with chronic periodontitis. This short, reliable and valid questionnaire may serve as a valuable tool in future clinical trials.
