Abstract:97 Background: Multi-agent chemotherapy combined with limb salvage surgery has been established as the standard of care of primary bone malignancy. Reconstruction with non-expandable endoprosthesis causes limb length discrepancy. Due to the high cost and unavailability of non-invasive expandable endoprosthesis at the authors’ institutional. Hemiarhtroplasty or epiphysis minimal invasive megaprosthesis is usually used at the authors’ institution. In order to improve the stability of hemiarthroplasty, a simple-hinged endoprosthesis with thin stem was used to preserve the growth plate in adjacent bone on ipsilateral side. Bone scintigraphy was performed in routine follow-ups. We asked whether the uptake of 99mTc-MDP on preserved epiphysis could predict longitudinal growth potential of epiphysis. Materials and Methods: From January 2010 to September 2017, 24 consecutive skeletal immature patients diagnosed of osteosarcoma or Ewing’s sarcoma, received endoprosthetic reconstructionafter tumor removal (6 with hemiarthoplasty prosthesis, 18 with epiphysis-minimal invasive prosthesis). Eleven children (>9 years old) reconstructed with adult-type rotation-hinged prosthesis were enrolled as control group. All patients underwent 99mTc-MDP whole-body bone scintigraphy 3 to 8 months (mean 4.8±1.5 mo) after limb salvage surgery and also 3-month-interval roentgenography for at least 24 months to measure the total tibial length. The tibial annual growth length (L) was calculated based on the measurement of tibial length and interval time between the roentgenography. In bone scintigraphy, proximal tibial plate uptake(PTPU) was evaluated by visual. The uptake ratio between ipsilateral proximal tibial growth plate and the contralateral side (R) was calculated by delineation of region of interest to measure its average activity count. Growth disturbance of growth plate was compared among three different type of endoprosthesis. Correlation between the L value and PTPU were also analyzed. Results: In patients with hemiarthroplasty prosthesis or epiphysis-minimal invasive endoprosthsis, L value of both of the ipsilateral and contralateral tibia were higher than those in the control group (Pvalue equals 0.001 and 0.016 respectively). On bone scintigraphy, 2 out of 6 cases with hemiarthroplasty and 17 out of 18 cases with epiphysis-minimal invasive endoprosthsis showed decreased PTPU in ipsilateral side, while the PTPU in contralateral side were normal. Meanwhile, PTPU in both sides of adult-type rotation-hinged prosthesis was decreased. The L value of tibia with decreased PTPU was lower than those with normal PTPU (2.3±1.1cm vs 1.1±0.9cm,t=4.995,P<0.001). Spearman rank correlation analysis showed that there was a positive correlation between R value and divisor of bilateral L value in all patients (r=0.549, P=0.001). Conclusions: The hemiarthroplasty prosthesis or epiphysis-minimal invasive endoprosthsis are helpful in preservation of the growth potential of tibia. Evaluation of PTPU on 99mTc-MDP bone scintigraphy may predict long term limb length discrepancy after endoprosthetic reconstruction surgery.
