Abstract:Summary In this study, we attempted to determine the epidemiology and clinical characteristics of hip fracture in the elderly. We find that elderly people with hip fracture have multiple comorbidities and suffer numerous complications. Introduction We attempted to explore the epidemiology and clinical characteristics of hip fracture in the elderly. Methods One thousand five hundred thirty-nine patients aged over 65 years were included in the retrospective study. From the medical records, information was gathered about pre-fracture conditions, as well as fracture type, surgical details, laboratory indicators, postoperative complications, length of stay, outcomes, and costs of hospitalization. Binary logistic regression was used to screen for potential risk factors for perioperative complications and postoperative death, and general linear models were used to determine factors that influenced the cost of surgical treatment. Results The average age of hip fracture patients in our study was 82.20 ± 6.82 years old, and the male-to-female ratio was 1:2.82. In 1356 patients who underwent hip surgery, the incidence of perioperative complications was 6.71% (91/1356), and the postoperative mortality rate was 1.11% (15/1356). Factors associated with perioperative complications were male sex, heart function class III or higher, serum albumin < 35 g/L, respiratory diseases, and perioperative blood transfusion ( P < 0.05). Perioperative blood transfusion was an independent risk factor for postoperative death after hip fracture in the elderly ( P < 0.05). The main factors that influenced hospitalization expenses related to elderly hip fracture patients were type of surgery, method of anesthesia, length of stay, perioperative complications, and outcomes ( P < 0.05). Conclusions Elderly people with hip fracture have multiple comorbidities and suffer numerous complications. Thus, randomized intervention studies should focus on prevention of complications that might be avoidable.

Abstract:Hip fracture is a public health concern because of its considerable morbidity, excess mortality, great risk of disability, and high societal healthcare costs. China has the largest population of older people in the world and is experiencing rapid population aging and facing great challenges from an increasing number of hip fractures. However, few studies reported the epidemiology, especially at a national level. We aimed to evaluate trends in hip fracture incidence and associated costs for hospitalization in China.We conducted a population-based study using data between 2012 and 2016 from the national databases of Urban Employee Basic Medical Insurance and Urban Resident Basic Medical Insurance in China, covering about 480 million residents. Data from around 102.56 million participants aged 55 years and older during the study period were analyzed. A total of 190,560 incident hip fracture patients (mean age 77.05 years, standard deviation 8.94; 63.99% female) were identified. Primary outcomes included the age- and sex-specific incidences of hip fracture. Associated annual costs for hospitalization were also calculated. Incidence was described as per 100,000 person-years at risk, and 95% confidence intervals were computed assuming a Poisson distribution. Hip fracture incidence overall in China did not increase during the study period despite rapid population aging. Incidence per 100,000 was 180.72 (95% CI 137.16, 224.28; P < 0.001) in 2012 and 177.13 (95% CI 139.93, 214.33; P < 0.001) in 2016 for females, and 121.86 (95% CI 97.30, 146.42; P < 0.001) in 2012 and 99.15 (95% CI 81.31, 116.99; P < 0.001) in 2016 for males. For both sexes, declines in hip fracture incidence were observed in patients aged 65 years and older, although incidence was relatively stable in younger patients. However, the total absolute number of hip fractures in those 55 years and older increased about 4-fold. The total costs for hospitalization showed a steep rise from US$60 million to US$380 million over the study period. Costs for hospitalization per patient increased about 1.59-fold, from US$4,300 in 2012 to US$6,840 in 2016. The main limitation of the study was the unavailability of data on imaging information to adjudicate cases of hip fracture.Our results show that hip fracture incidence among patients aged 55 and over in China reached a plateau between 2012 and 2016. However, the absolute number of hip fractures and associated medical costs for hospitalization increased rapidly because of population aging.

Abstract:Objective To analyze the epidemiological characteristics of 2 859 patients with hip fractures,so as to provide reference for the prevention and treatment of elderly patients with hip fractures.Methods The clinical data of 2 859 patients with hip fractures in Changhai Hospital,Second Military Medical University from Jan.2004 to Dec.2016 were collected,including age,gender,injury mechanism,fracture type,injury-operation time,hospital stays,bleeding volume and operation method.SPSS 16.0 software was used to analyze the epidemiological characteristics of elderly patients with hip fractures.Results (1) The total number of in-patients with hip fractures in our hospital increased annually,with the proportion of femoral neck fractures being the highest (1 602/2 859,56.03％),followed by intertrochanteric fractures.(2) The age of the patients with hip fractures was 52-104 years old,and there was a significant difference in the age distribution (P＜0.05),with the patients aged 71-80 years being the most majority,accounting for 37.39％ (1 069/2 859).(3) The proportion of female patients with hip fractures (1 916/2 859,67.02％) was higher than that of male patients (943/2 859,32.98％).(4) The distribution of injury mechanism was significantly different (P＜0.05).The number of fractures caused by walking falls was the highest,accounting for 87.65％ (2 506/2 859).(5) The distribution of fracture types was significantly different in different genders (P＜0.05).The proportion of femoral neck fractures was higher in female patients,while the proportion of intertrochanteric fractures was higher in male patients.(6) The incidences of hip fractures were the highest in patients with diabetes or cardiovascular disease,accounting for 26.76％ (765/2 859) and 25.85％ (739/2 859),respectively.Conclusion Femoral neck fractures and intertrochanteric fractures are the most common types in elderly patients with hip fractures.The distributions of age,gender,injury mechanism and complications of elderly patients have certain epidemiological characteristics,which provides clue to further improve the prevention and treatment of hip fractures.

Abstract:Abstract Background Hip fracture is common, with high morbidity and expense, which becomes a major health-care problem. Very few multicenter studies of the cost of care for hip fracture in China have been done. The aim of this study was to describe and analyse the hospitalisation cost associated with hip fractures, to compare the hospitalisation cost with the local income level, and to provide rational proposals for Chinese national medical insurance policy. Methods In this multicenter, cross-sectional study, we collected and analysed data from Chinese National Medical Data Center Database. The inclusion criteria were based on the main diagnosis of femoral neck or intertrochanteric fracture and a discharge date between Dec 30, 2013, and Dec 31, 2015. Hospitalisation costs, age, diagnosis, treating methods, hospitalisation provinces, and hospitalisation length of stay were collected and analysed using SPSS 23.0. We excluded any datasets that did not have the full complement of information listed above. The gross domestic product (GDP) per capita and average annual disposable income were obtained from National Bureau of Statistics of the Peopleu0027s Republic of China. The effect of various factors on the cost of hospitalisation was investigated using ANOVA analysis and multivariable linear regression analysis. The level of significance was set at p Findings 27 205 cases from 73 central hospitals in 24 provinces were included in the study. The average cost of all patients was ¥53 440 (median ¥51158, SD ¥35 238), which was more than the GDP per capita in 2014 (¥46 629) and 2·65 times the average annual disposable income of 2014 (¥20 167). According to the multiple linear regression analysis, the diagnosis, treatment methods, and length of hospital stay had a statistically significant effect on hospitalisation costs. Furthermore, some age groups (0–19 years, 60–69 years) also had significant effects. The average cost of care for patients with femoral neck fractures was ¥12 284·22 less than for the care of patients with intertrochanteric fractures, with the standardised coefficient of 0·263; the average cost of hip replacement was ¥18560·65 higher than that of internal fixation, with the standardised coefficient of 0·263, which indicated a strong effect of the surgical approach to cost. The length of hospital stay was the most important factor, and the hospitalisation cost increased with increasing number of days spent at hospitals. The distribution of average hospitalisation cost of hip fracture patients varied a lot across different provinces, presenting a multicenter, radicalised distribution pattern, which did not match with the local income level. Interpretation The regional economic and medical development is highly inconsistent, and the hospitalisation cost of hip fracture has become a great burden to many patientsu0027 families, especially those living in economically underdeveloped regions with high medical expense. However, the medical insurance now only covers the hospitalisation cost to a limited extent. Thus, we call for further enhancement of the Chinese medical insurance policy, which should be implemented according to the distribution characteristics of hospitalisation cost and local income levels. Abstract:Objective To describe and analyse the hospitalisation cost of patients with hip fracture under the influence of various factors and to provide references for Chinese national medical insurance policy. Materials and methods All data were collected from the Chinese National Medical Data Centre database, which contained the hospitalisation data of 73 tertiary hospitals from 24 provinces. The included patients were first hospitalised with the main diagnosis of femoral neck or intertrochanteric fracture, and were discharged between 1 January 2014 and 31 December 2015. Secondary hospitalisation, multiple trauma or pathologic fracture (except for osteoporotic fracture) patients or patients with missing data were excluded. The impact of various factors on the cost was investigated using analysis of variance and multivariable linear regression analysis. The Gross Domestic Product per capita and average annual disposable income were obtained from the website of the National Bureau of Statistics. Results 27 205 cases were included in the study. The mean cost of all patients was (sic)53 440. 60-69 years age group had a significantly higher cost compared with 80 years and above age group. The mean cost of femoral neck fractures was lower than that of the patients with intertrochanteric fractures; the mean cost of hip replacement was higher than that of internal fixation, which showed a strong effect of the surgical approach to the cost. Patients in low-income provinces spent less than those in high-income provinces, while the gap between high-income and middle-income provinces were relatively small. Conclusion The hospitalisation cost of hip fracture has become a great burden to the patients' families. The Chinese medical insurance policy may need further consideration of the demographic and economic factors.
[Preliminary Analysis of Management for Hospitalized Elders with Hip Fractures].

Abstract:OBJECTIVE:To analyze various clinical parameters of elderly hip fractures so as to improve the management of elder hip fractures in China.METHODS:The data of elder patients with hip fracture (primary diagnosis was fracture of femoral neck or intertrochanter) admitted into our department between 2002 to 2010 were collected. And the relevant clinical parameters included case number, age, ratio of concurrent chronic disease and duration and cost of hospitalization. The software of SAS was used for statistical analysis.RESULTS:A total of 1626 patients (M/F = 547/1079) ≥ 65 yr old with femoral neck fracture were admitted. Average age was 74.7 ± 6.4 yr (65 - 99) and annual average increasing rate 0.5% (-0.1% - 1.8%). The ratio of concurrent chronic disease was 53.3%. Average duration of hospitalization was 18.3 ± 10.9 days (1 - 114) and annual average increasing rate was -6.3% (-19.2% - 8.4%). Average cost of hospitalization was 38 758.04 ± 24 558.15 yuan (76.8 - 339 987.49) and annual average increasing rate 6.4% (-8.7% - 40.0%). A total of 892 patients (M/F = 362/530) ≥ 65 yr with femoral intertrochanteric fracture were admitted. Average age was 76.7 ± 6.8 yr (65 - 105) and annual average increasing rate 1.3% (-1.8% - 4.3%). The ratio of concurrent chronic disease was 55.8%. Average duration of hospitalization was 15.7 ± 8.7 days (1 - 78) and annual average increasing rate -4.5% (-22.1% - 8.0%). Average cost of hospitalization was 35 183.45 ± 21 427.47 yuan (75.3 - 148 150.41) and annual average increasing rate 18.3% (-3.7% - 79.9%).CONCLUSION:The number, age and therapeutic cost of elder patients with hip fracture are increasing continuously. Elderly hip fracture is becoming a serious problem of public health.

Abstract:The age-specific rates of hip fractures have been declining in most countries in the West but a few studies suggest that the rates might be increasing in areas of Asia that are undergoing urbanization. We previously conducted a population-based study of hip fracture rates in Beijing, China, in 1990 to 1992 that included validation of hip fracture cases. Using a similar approach to validate cases, we estimated the age-specific hip fracture rates in Beijing, China, for 2002 to 2006. Specifically, we obtained hospital discharge data for hip fractures that were reported to the Beijing Bureau of Public Health. To confirm the diagnoses, Beijing residence, and find cases missed by the public records we checked individual cases in the public health records against medical records in a random sample of Beijing hospitals. The rates from public health data were adjusted for these under- and overestimations. We found that between 1990 and 1992 and 2002 and 2006, the adjusted age-specific rates of hip fracture over age 50 years increased 2.76-fold (95% confidence interval [CI], 2.68-2.84) in women and 1.61-fold (95% CI, 1.56-1.66) in men. Over age 70 years, the age-specific rates increased 3.37-fold (95% CI, 3.28-3.47) in women and 2.01-fold (95% CI, 1.95-2.07) in men. From 2002 to 2006, the rates over age 50 years increased 58% in women and 49% in men. We conclude that the rate of hip fracture has been rising very rapidly in Beijing, China. Therefore, the burden of hip fractures may be shifting rapidly from the West to urbanizing areas of the East.

[Descriptive Analysis of Fracture Hospitalization Rate in Adults from 10 Regions of China].

Abstract:Objective: To analyze the epidemiological distributions of fracture hospitalization. Methods: The present study included participants who participated in the baseline survey of China Kadoorie Biobank (CKB) and excluded participants who were lost to follow up or died before 2009, leaving a total of 506 004 participants. Negative binomial regression models were used to analyze the epidemiological distribution of any fracture and fracture at five specific body sites (upperlimb, lowerlimb, spine, pelvis and hip) from 2009 to 2016 in 10 regions. Results: During a median follow-up of (7.7±1.2) years (total person-years 3 899 814), we documented 17 118 cases of fracture hospitalizations. The crude fracture hospitalization rate was 4.39/1 000 person-years. After controlling for the increasing age of the fixed cohort, the hospitalization rates of fractures at various body sites increased from 2009 to 2016, with an annual growth rate (95%CI) of 9.1% (8.3%-9.9%) for any fracture. The fracture hospitalization rate was higher in rural than in urban areas except for hip fractures (P<0.05) and the hospitalization rate of any fracture were 5.42/1 000 and 3.24/1 000 person-years in rural and urban areas, respectively. Fracture hospitalization rate increased by age. In participants aged <50 years, men had higher fracture hospitalization rates than women except for pelvis fracture, while in those aged ≥50 years, women had higher fracture hospitalization rates than men. Conclusions: Fracture hospitalization rates increased by age and also showed upward selular trends. As China has begun the aging process, fractures impose a heavier burden on society. It is of great significance to prevent osteoporosis-related and injury-related fractures in order to reduce fractures incidence.

Abstract:In this international study, we examined the incidence of hip fractures, post‐fracture treatment, and all‐cause mortality following hip fractures, based on demographics, geography, and calendar year. We used patient‐level healthcare data from 19 countries and regions to identify patients aged 50 years and older hospitalized with a hip fracture from 2005 to 2018. The age‐ and sex‐standardized incidence rates of hip fractures, post‐hip fracture treatment (defined as the proportion of patients receiving anti‐osteoporosis medication with various mechanisms of action [bisphosphonates, denosumab, raloxifene, strontium ranelate, or teriparatide] following a hip fracture), and the all‐cause mortality rates after hip fractures were estimated using a standardized protocol and common data model. The number of hip fractures in 2050 was projected based on trends in the incidence and estimated future population demographics. In total, 4,115,046 hip fractures were identified from 20 databases. The reported age‐ and sex‐standardized incidence rates of hip fractures ranged from 95.1 (95% confidence interval [CI] 94.8‐95.4) in Brazil to 315.9 (95% CI 314.0‐317.7) in Denmark per 100,000 population. Incidence rates decreased over the study period in most countries; however, the estimated total annual number of hip fractures nearly doubled from 2018 to 2050. Within 1 year following a hip fracture, post‐hip fracture treatment ranged from 11.5% (95% CI 11.1%‐11.9%) in Germany to 50.3% (95% CI 50.0%‐50.7%) in the United Kingdom and all‐cause mortality rates ranged from 14.4% (95% CI 14.0%‐14.8%) in Singapore to 28.3% (95% CI 28.0%‐28.6%) in the United Kingdom. Males had lower use of anti‐osteoporosis medication than females, higher rates of all‐cause mortality, and a larger increase in the projected number of hip fractures by 2050. Substantial variations exist in the global epidemiology of hip fractures and post‐fracture outcomes. Our findings inform possible actions to reduce the projected public health burden of osteoporotic fractures among the ageing population.

Gender Differences in Modifiable Risk Factors for Hip Fracture: 10-Year Follow-Up of a Prospective Study of 0.5 Million Chinese Adults.

Abstract:BACKGROUND:Little is known about the incidence rates and importance of major modifiable risk factors for hip and major osteoporotic fractures in low- and middle-income countries. We estimated the age- and sex-specific incidence of hip, major osteoporotic, and any fractures and their associated risk factors in Chinese adults.METHODS:This was a prospective study of 512,715 adults, aged 30-79 years, recruited from 10 diverse areas in China from 2004 to 2008 and followed up for 10 years. Age- and sex-specific incidence rates were estimated, and Cox regression was used to yield adjusted hazard ratios (HRs) and population attributable fractions for risk factors.RESULTS:The incidence rates of hip fracture in Chinese adults were 5.1 (95% confidence interval [CI] 5.0-5.3) per 10,000 person-years; they were higher in women than in men and increased by two- to threefold per 10-year older age. Among men, five risk factors for hip fracture, including low education (HR = 1.23; 95% CI 1.04-1.45), regular smoker (1.22, 1.03-1.45), lower weight (1.59, 1.34-1.88), alcohol drinker (1.18, 1.02-1.36), and prior fracture (1.62, 1.33-1.98), accounted for 44.3% of hip fractures. Among women, lower weight (1.30, 1.15-1.46), low physical activity (1.22, 1.10-1.35), diabetes (1.62, 1.41-1.86), prior fracture (1.54, 1.33-1.77), and self-rated poor health (1.29, 1.13-1.47) accounted for 24.9% of hip fractures. Associations of risk factors with major osteoporotic or any fractures were weaker than those with hip fractures.CONCLUSIONS:The age- and sex-specific incidence rates of hip fracture in Chinese adults were comparable with those in Western populations. Five potentially modifiable factors accounted for half of the hip fractures in men and one quarter in women.
Worldwide research tendency and hotspots on hip fracture: a 20-year bibliometric analysis

Abstract:We analyzed the overall knowledge structure, development trends, and research hotspots of 7684 publications related to hip fracture through the bibliometric method. Our results indicate this area has received more and more attention from researchers. Prevention of complications will be the focus of future studies. Introduction: Hip fracture is an international public health problem, with high morbidity, mortality, and associated health care costs. Research on hip fracture has been developed rapidly in recent years, but no bibliometric studies have been performed. We aimed to identify the publication changes in scientific output relating to hip fracture over the past two decades. Methods: The scientific output relating to hip fracture from 2000 to 2019 was identified and selected from the Web of Science Core Collection. Excel 2019 was used to summarize the quantitative indicators including publication number, citations, H-index, journal's impact factors, and journal citation reports. VOS viewer and CiteSpace software tools were used for co-authorship, citation, co-citation, and co-occurrence analyses between countries, institutions, journals, authors, references, and keywords. Data were analyzed on November 13, 2020. Results: A total of 7684 publications were extracted. The USA was the leading contributor in this field with the largest publications (1876, 24.41%), the most citations (75,423 times), and the highest H-index (124). The number of publications in the Western European region is 1.82 times higher than that of North America region, and 3.59 times that of Eastern Asia region. The most productive institutions on hip fracture were University of Maryland (160). Injury (506) had the highest number of publications, while Osteoporosis International (20,483 times) was the most co-cited journal. Magaziner J and Parker MJ were the key researchers. The keywords were stratified into five clusters: cluster 1 ("operative approaches study"), cluster 2 ("rehabilitation study"), cluster 3 ("osteoporosis study"), cluster 4 ("outcomes and complications study"), and cluster 5 ("epidemiology study"). For hotspots, "tranexamic acid" showed a relatively latest average appearing years of 2017.52, followed by "30-day-mortality," "readmission," and "length of stay." Conclusions: There will be an increasing number of publications on the hip fracture research based on the current global trends, and the USA stays ahead in this field. In terms of region, Western Europe had the greater impact than North America. It is recommended to pay attention to the promising hotspots, such as tranexamic acid, 30-day-mortality, readmission, and length of stay.
Age-specific 1-Year Mortality Rates after Hip Fracture Based on the Populations in Mainland China Between the Years 2000 and 2018: a Systematic Analysis.

Abstract:We used statistical approaches to calculate 1-year mortality rates and reveal the relationship between age and the 1-year mortality rate after hip fracture based on data from mainland China between the years 2000 and 2018. Data on the 1-year mortality rates after hip fracture in mainland China remain limited and localized. We aimed to analyze the 1-year mortality rates and reveal the variations in 1-year mortality by age after hip fracture based on data from mainland China. We searched PubMed, EMBASE, Cochrane Library, CNKI, Wanfang, and CBM-SinoMed for all relevant articles in English or Chinese to estimate the 1-year mortality rates after hip fracture in mainland China. A random-effects meta-analysis model was fitted to pool the overall 1-year mortality rates. A multilevel mixed-effects meta-regression model was developed. Based on the final model, the age-specific 1-year mortality rates after hip fracture in mainland China were generated. The pooled estimate of the 1-year mortality rate was 13.96% after hip fracture (95% CI 12.26 to 15.86%), 17.47% after femoral intertrochanteric fracture (95% CI 14.29 to 21.20%), and 9.83% after femoral neck fracture (95% CI 6.96 to 13.72%) between the years 2000 and 2018. We found that the 1-year mortality rates ranged from 2.65% (95% CI 1.76 to 3.99%) in those aged 50~54 years to 28.91% (95% CI 24.23 to 34.30%) in those aged 95~99 years after hip fracture; ranged from 1.73% (95% CI 0.58 to 4.99%) in those aged 50~54 years to 50.11% (95% CI 46.03% to 53.97%) in those aged 95~99 years after femoral intertrochanteric fracture; and ranged from 1.66% (95% CI 1.31 to 2.11%) in those aged 60~64 years to 37.71% (95% CI 27.92 to 48.63%) in those aged 95~99 years after femoral neck fracture. In this systematic review and meta-analysis, we calculated the 1-year mortality rate after hip fracture in mainland China and found that this rate was lower than that in most countries. We also estimated the age-specific mortality rates for different age groups after hip fracture. These findings will be beneficial for the prevention and treatment of hip fracture in mainland China.

Abstract:The accelerated growth of older individuals worldwide has increased the number of patients presenting with fragility hip fractures. Having a hip fracture can cause excess mortality, and patients with hip fracture have a higher risk of death than those without hip fracture. Most studies have treated hip fracture as a single, homogeneous condition, but hip fracture includes two major anatomic types: intertrochanteric fracture and femoral neck fracture. Few studies have specifically evaluated 1-year mortality risk in older individuals with femoral intertrochanteric fracture. The aim of this study was to evaluate 1-year mortality and factors associated with mortality in older individuals with femoral intertrochanteric fracture.

Abstract:Objectives:National data on the admission rate, distribution, in-hospital mortality, and economic burden of traumatic fractures in China is unclear. We aimed to conduct a cross-sectional population-based study to determine such above data at the national level in China.Methods:A national administrative database was used to review all traumatic fracture hospitalizations in China during 2020, from which a total of 2,025,169 inpatients with traumatic fractures was retrieved. Admission rates and in-hospital mortality rates stratified by age, sex, and region were calculated. The causes of traumatic fracture and economic burden were described.Results:The admission rate of traumatic fractures of all China population in 2020 was 1.437‰. The admission rate increased with age and varied with genders and causes of injuries. Falls are the leading cause of traumatic fracture hospitalization, followed by road traffic injuries. The most common diagnoses were femoral neck fractures, with a number of 138,377. The in-hospital mortality was 1.209‰. Road traffic injuries led to the highest in-hospital mortality. The median length of stay was 10 days, with the median hospitalization cost of ¥20,900 (about $3,056).Conclusion:Traumatic fractures are concerning conditions with a high admission rate and in-hospital mortality in China, which are mainly caused by falls and road traffic injuries. The government should implement more public health policies to enhance the health of the elderly and improve transportation safety to prevent traumatic fractures.

Abstract:Hip fractures represent a significant public health issue due to high incidence in aging society. Our study further proved that increased risk for hip fractures in adults is associated with weather conditions. Purpose: Hip fractures represent a significant public health issue due to high incidence in aging society. Evidence of the short-term effects of weather on the risk of hip fracture is limited and inconsistent. We aimed to examine the associations between weather conditions and daily hospital admissions for hip fracture in adults in China. Methods: A national time-series analysis between 2014 and 2017 was conducted. Data on daily hospital admissions for hip fracture were obtained from the database of Urban Employee Basic Medical Insurance (UEBMI) and Urban Resident Basic Medical Insurance (URBMI). Weather conditions were acquired from the China Meteorological Data Sharing Service Center. Based on a time-stratified case-crossover design, conditional Poisson regression was used to estimate the impact on relative risk (RR) of weather conditions on hospital admissions for hip fracture. Results: During the study period, a total of 137,504 hospital admissions for hip fractures were identified. All analyzed weather conditions showed consistent significant associations at lag 0 day for each 10 mm increase in precipitation, 10 m/s in wind speed, and 10°C in temperature, with the RR value being 1.079 (95% CI, 1.074-1.083) for precipitation, 1.404 (95% CI, 1.346-1.465) for wind speed, and 1.558 (95% CI, 1.546-1.570) for temperature. Women were more vulnerable to be affected by precipitation and temperature. Conclusion: In conclusion, increased risk for hip fractures in adults is associated with weather conditions. The improved understanding of the relationship between weather conditions and hip fractures hospital admission can be useful for resource allocation and provider preparedness.

Abstract:This study is the first to measure global burden of hip fracture in patients aged 55 years and older across 204 countries and territories from 1990 to 2019. Our study further proved that the global burden of hip fracture is still large. Hip fractures among males are perhaps underestimated, and older adults should be given more attention.PURPOSE:Hip fracture is a tremendous universal public health challenge, but no updated comprehensive and comparable assessment of hip fracture incidence and burden exists for most of the world in older adults.METHODS:Using data from the Global Burden of Diseases (GBD) 2019, we estimated the number and rates of the incidence, prevalence, and years lived with disability (YLD) of hip fracture across 204 countries and territories in patients aged 55 years and older from 1990 to 2019.RESULTS:In 2019, the incidence, prevalence, and YLDs rates of hip fracture in patients aged 55 years and older were 681.35 (95% UI 508.36-892.27) per 100000 population, 1191.39 (95% UI 1083.80-1301.52) per 100000 population, and 130.78 (95% UI 92.26-175.30) per 100000 population. During the three decades, the incidence among people aged below 60 years showed a downward trend, whereas it showed a rapid upward trend among older adults. All the numbers and rates of hip fractures among females were higher than those among males and increased with age, with the highest number and rate in the highest age group. Notably, the male to female ratio of the incidence for people aged over 55 years increased from 0.577 in 1990 to 0.612 in 2019. Falls were the leading cause among both sexes and in all age groups.CONCLUSIONS:The incidence and the number of hip fractures among patients aged 55 years and older increased over the past three decades, indicating that the global burden of hip fracture is still large. Hip fractures among males are perhaps underestimated, and older adults should be given more attention.

Abstract:[Objective] To investigate the epidemiologic features of fall-related injuries, and its relationships with income, education level, and senior residents in Shanghai, China.[Methods] A multi-stage stratified cluster sampling approach with probability proportional to size was used to select 45 857 residents in Shanghai for home visits.Mortality and demographic data were collected from Shanghai Vital Registration System from 2001 to 2011.Records on fall-related hospitalized cases and fatalities from 164 hospitals in Shanghai were also analyzed.Logistic regression was used to control confounding from age, gender, education levels and income.[Results] During 2001 to 2011, the average annual income increased by 206.63% in Shanghai; while the fall specific mortality rate increased from 10.63 per 100 000 in 2001 to 14.76 per 100 000 in 2011.The morbidity of fall was 14.70 per 1 000 in the survey.Those with family average monthly incomes lower than 500 Yuan RMB were 6.5 times as likely as to suffer from fall-related injuries than those with higher than 5 000 Yuan RMB.The residents with an education level of primary school and under showed the highest fall specific mortality and morbidity rates, 57.85 per 100 000(73.23%) and 33.96 per 1 000(49.24%), 22 times and 5.5 times compared with those with college degree or above, respectively.The Luwan and Jing'an districts showed the highest mortality rates(24.10 per 100 000 and 19.59 per 100 000 respectively) with leading proportions of senior residents.The mortality rate of fall in the children 5 years was the highest among children aged 0-14 years.The mortality and morbidity of fall increased dramatically with age among those aged 50 years or above, among which the 50-54 age group accounted for the largest proportion and the morbidity accelerated.The female showed higher mortality and morbidity rates of fall than the male.Combined head injuries accounted for about half of the total fatalities of fall.The relative risk of fall-related injuries was higher in lower income, lower education level, and higher age groups, with odds ratios of 1.39, 1.27, and 1.21, respectively.[Conclusion] The prevention and control of fall-related injuries confronts a huge challenge.Income, education level, and age are the main risk factors for fall-related injuries.Populations under 5 years old, over 50 years old, with an education level lower than primary school, and with lower income should be the targets for intervention.More efforts are needed, such as installing comprehensive fall-related injury prevention programs to the new responsibility system of family doctors.

Abstract:Background: Osteoporotic vertebral fractures cause pain and disability, which result in a heavy socioeconomic burden. However, the incidence and cost of vertebral fractures in China are unknown. We aimed to assess the incidence and cost of clinically recognized vertebral fractures among people aged 50 years and older in China from 2013 to 2017. Materials and methods: This population-based cohort study was conducted by using Urban Employee Basic Medical Insurance (UEBMI) and Urban Resident Basic Medical Insurance (URBMI) data in China from 2013 to 2017, which covered more than 95% of the Chinese population in urban areas. Vertebral fractures were identified by the primary diagnosis (i.e. International Classification of Diseases code or text of diagnosis) in UEBMI and URBMI. The incidence and medical cost of these clinically recognized vertebral fractures in urban China were calculated. Results: A total of 271 981 vertebral fractures (186 428, 68.5% females and 85 553, 31.5% males) were identified, with a mean age of 70.26 years. The incidence of vertebral fractures among patients aged 50 years and over in China increased similar to 1.79-fold during the 5 years, from 85.21 per 100 000 person-years in 2013 to 152.13 per 100 000 person-years in 2017. Medical costs for vertebral fractures increased from US$92.74 million in 2013 to US$505.3 million in 2017. Annual costs per vertebral fracture case increased from US$3.54 thousand in 2013 to US$5.35 thousand in 2017. Conclusion: The dramatic increase in the incidence and cost of clinically recognized vertebral fractures among patients aged 50 and over in urban China implies that more attention should be given to the management of osteoporosis to prevent osteoporotic fractures.
