Abstract:To assess the feasibility of a wound care model using 4th-generation mobile communication technology standards (4G) with smart phones or smart glasses for wound management.This wound care model is an interactive, real-time platform for implementing telemedicine changing wound dressings, or doing operations. It was set up in March 2015 between Jinhua in Zhejiang province and Shanghai, China. which are 328 km apart. It comprised of a video application (APP), 4G net, smart phones or smart glasses, and a central server.This model service has been used in 30 patients with wounds on their lower extremities for 109 times in 1 month. Following a short learning curve, the service worked well and was deemed to be user-friendly. Two (6.7%) patients had wounds healed, while others still required wound dressing changes after the study finished. Both local surgeons and patients showed good acceptance of this model (100% and 83.33%, respectively).This telemedicine model is feasible and valuable because it provides an opportunity of medical service about wound healing in remote areas where specialists are scarce.

Abstract:Wound management, especially for chronic wounds, has emerged as a major healthcare challenge, which brings great pain and other negative impacts to patients, and accounts for significant portions of health care budgets. The common practice of covering wounds with dressings, owing to the lack of information about wound healing underneath, cannot provide insights into the status of the wound. Changing the dressings to inspect wounds not only disturbs normal healing process of wounds, but also causes pain to affected individuals. Therefore, it is necessary to rely on parameters in the wound microenvironment such as temperature, pH, moisture level, and etc., by continuously monitoring, to indicate the wound status and healing stages. Moreover, intelligent on-demand treatment systems also can be integrated into the smart dressing, called new-type skin bioelectronic systems to be applicated in wound repair. Besides monitoring, these kinds of skin bioelectronic systems can offer effective treatments automatedly when the wound deteriorates. It makes timely treatment possible and avoids delayed treatment and uncovering the dressing to give medicine. In this review, wound healing-related indexes in the wound microenvironment are discussed, applications of various smart dressings with functions of monitoring or/and treatment are summarized.

Abstract:The increasing numbers of cases of wound disease are now posing a big challenge in China. For more convenience of wound patients, wound management in community health care centers under the supervision of a specialist at general hospitals is an ideal solution. To ensure an accurate diagnosis in community health clinics, it is important that "the same language" for wound description, which may be composed of unified format description, including wound image, must be achieved. We developed a wound information management system that was built up by acquisition terminal, wound description, data bank, and related software. In this system, a 3G mobile phone was applied as acquisition terminal, which could be used to access to the data bank. This documentation system was thought to be an appropriate proposal for community wound care because of its objectivity, uniformity, and facilitation. It also provides possibility for epidemiological study in the future.

This paper, based on real-time web communication technology HTML5-WebSocket, describes the development of a scalable, real-time, multi-parameter, remote monitoring system. The system consists of sensors, mobile front control side, and a module-based remote monitoring platform. It has a variety of network functions, such as transmission of multiple physiological parameters, search, and export. In addition, the system provides real-time monitoring, data review, waveform check, and real-time doctor-patient communication. Abstract:With the accelerated global urbanization and life rhythm, an increasing physical and mental stress has been imposed, which led to a variety of problems that could not be ignored. This paper proposes a novel web-based telemedicine system that is focusing on psychotherapy. Considering the requirement of the psychological treatment, the function of system contains high definition video monitoring, two-way voice talkback, physiology parameters monitoring and data management based on database. The solution of the system is presented in the paper, including the hardware and software design. Abstract:The incidence of chronic wounds has been increasing over the past 20 years. However, the standardized diagnosis and treatment practice of chronic refractory wounds have not been established. In addition, the properties of the wound are characterized by morphology and thus correct description of the wound in medical history collection plays a vital role, which directly affects the definitive diagnosis. To develop more accurate format of clinical history record which can correctly reflect a patient's course and treatment progress, and to standardize the medical history record of chronic refractory wounds, at the national or regional level, we designed the WoundCareLog APP. It acts as a recording and communication tool for wound healing specialists at all levels of medical institutions in China. The WoundCareLog APP is fully compatible to meet the criteria and requirements of conventional medical records by embedding 9 modules. In addition, the demands for morphological description of wounds in wound healing diagnosis and treatment have been fulfilled by enroll of digital imaging technology to overcome the inadequacies of traditional medical history records.

Abstract:The invention discloses a webpage-based remote comprehensive diagnosis and treatment system, which comprises a server side and a client side. The server side is provided with a built-in WEB server, a high-definition video encoding module and a bidirectional voice communication subsystem. The built-in WEB server is used for acquiring various data, waits to be connected with users and realizes data transmission with the client side in a communication manner after being connected with the users successfully. The high-definition video encoding module completes acquisition and encoding of video data of the server side, wherein the encoding is completed by the aid of a hardware encoding chip. The bidirectional voice communication subsystem is mainly used for realizing communication between doctors and patients during the remote medical treatment process, realizing bidirectional voice cross-platform communication while solving the problems of noise and time delay in communication. The client side is a WEB-based high-definition video decoding client side. Compared with the conventional remote medical treatment system, the webpage-based remote comprehensive diagnosis and treatment system has the advantages of portability, easiness in use, high adaptability and communication quality and the like.

Abstract:Background Chronic wounds have been a great burden to patients and the health care system. The popularity of the internet and smart devices, such as mobile phones and tablets, has made it possible to adopt telemedicine (TM) to improve the management of chronic wounds. However, studies conducted by different researchers have reported contradictory results on the effect of TM on chronic wound management. Objective The aim of this work was to evaluate the efficacy and safety of TM in chronic wound management. Methods We systematically searched multiple electronic databases (MEDLINE, EMBASE, and Cochrane Central Register of Controlled Trials [CENTRAL]) to identify eligible studies published from inception to June 12, 2019. Inclusion criteria were randomized controlled trials (RCTs) and interventional cohort studies that investigated the use of TM in chronic wound management. RCT and observational data were analyzed separately. A meta-analysis and qualitative analysis were conducted to estimate endpoints. Results A total of 6 RCTs and 6 cohort studies including 3913 patients were included. Of these, 4 studies used tablets or mobile phones programmed with apps, such as Skype and specialized interactive systems, whereas the remaining 8 studies used email, telephone, and videoconferencing to facilitate the implementation of TM using a specialized system. Efficacy outcomes in RCTs showed no significant differences in wound healing (hazard ratio [HR] 1.16, 95% CI 0.96-1.39; P=.13), and wound healing around 1 year (risk ratio [RR] 1.05, 95% CI 0.89-1.23; P=.15). Noninferiority criteria of TM were met. A decreased risk of amputation in patients receiving TM was revealed (RR 0.45, 95% CI 0.29-0.71; P=.001). The result of cohort studies showed that TM was more effective than standard care (HR 1.74, 95% CI 1.43-2.12; P<.001), whereas the outcome efficacy RR of wound healing around 1 year (RR 1.21, 95% CI 0.96-1.53; P=.56) and 3 months (RR 1.24, 95% CI 0.47-3.3; P=.67) was not significantly different between TM and standard care. Noninferiority criteria of TM were met for wound healing around 1 year in cohort studies. Conclusions Currently available evidence suggests that TM seems to have similar efficacy and safety, and met noninferiority criteria with conventional standard care of chronic wounds. Large-scale, well-designed RCTs are warranted.

Abstract:Objective The application of picture archival and communication (PACS)in Chinese hospitals allows experience sharing by top experts from tier-1 hospitals.This article focuses on the advantages of applying the remote educational framework for radiology departments in grass-root hospitals. Methods We established several conference centers equipped with video conference devices and PACS for telemedicine in the information center.During these teleconferences,doctors in rural hospitals can exchange real-time information including imaging data via PACS,audio and video signals with their remote counterparts using 20M broadband network.Results Imaging telediagnosis and teaching mode have upgraded the diagnosis ability of doctors in rural hospitals,saved the cost of expert resources,and reduced the cost of talented personnel education in rural hospitals.Conclusion In the future,distance imageology teaching mode will be a platform for medical exchanges integrating medical care,teaching and research;therefore,it has value of application.

Abstract:Purpose To establish a telemedicine system, which serves as a reliable method of military health service for overseas peacekeeping units and meets the requirements of accompanying peacekeeping missions and soldiers'growing demand for medical and health services .Methods A system was developed that can carry out telemedicine services and establish connection between peacekeeping units and the base hospital through the In -ternet.Results With the telemedicine system, visual and audio materials, electrocardiogram, ultrasound, text, voice and video can be transmitted; special examinations, such as ECG and ultrasound, and remote specialized di-agnosis and treatment, such as ENT and Dermatology, can be successfully carried out .Conclusion To extend service scope and content with the telemedicine system , several measures should be taken, including military and civilian integration, establishment of rules and regulations , coordination and cooperation, so as to strengthen the operation, maintenance and support of the system .

Abstract:Background Telemedicine that combines information technology and health care augments the operational model of traditional medical services and brings new opportunities to the medical field. China promotes telemedicine with great efforts, and its practices in the deployment of telemedicine platforms and delivery of services have become important references for the research and development in this field. Objective Our work described in this paper focuses on a regional telemedicine platform that was built in 2014. We analyzed the system design scheme and remote consultations that were conducted via the system to understand the deployment and service delivery processes of a representative telemedicine platform in China. Methods We collected information on remote consultations conducted from 2015 to 2020 via the regional telemedicine platform that employs a centralized architectural system model. We used graphs and statistical methods to describe the changing trends of service volume of remote consultation, geographical and demographic distribution of patients, and waiting time and duration of consultations. The factors that affect consultation duration and patient referral were analyzed by multivariable linear regression models and binary logistic regression models, respectively. The attitudes toward telemedicine of 225 medical practitioners and 225 patients were collected using the snowball sampling method. Results The regional telemedicine platform covers all levels of medical institutions and hospitals in all 18 cities of Henan Province as well as some interprovince hospitals. From 2015 to 2020, 103,957 remote medical consultations were conducted via the platform with an annual increasing rate of 0.64%. A total of 86.64% (90,069/103,957) of medical institutions (as clients) that applied for remote consultations were tier 1 or 2 and from less-developed regions; 65.65% (68,243/103,945) of patients who applied for remote consultations were aged over 50 years. The numbers of consultations were high for departments focusing in the treatment of chronic diseases such as neurology, respiratory medicine, and oncology. The invited experts were mainly experienced doctors with senior professional titles. Year of consultation, tier of hospital, consultation department, and necessity of patient referral were the main factors affecting the duration of consultations. In surveys, we found that 60.4% (136/225) of medical practitioners and 53.8% (121/225) of patients had high satisfaction and believed that telemedicine is of vital importance for the treatment of illness. Conclusions The development of telemedicine in China shows a growing trend and provides great benefits especially to medical institutions located in less developed regions and senior citizens who have less mobility. Cases of remote consultations are mainly for chronic diseases. At present, the importance and necessity of telemedicine are well recognized by both patients and medical practitioners. However, the waiting time needs to be further reduced to improve the efficiency of remote medical services.
