Abstract:Objective\r\nTo investigate the adverse risk factors of urinary function recovery after radical prostatectomy.\r\n\r\n\r\nMethods\r\nThe clinical data of 126 patients treated with laparoscopic radical prostatectomy from October 2015 to October 2016 were collected and analyzed retrospectively. The mean age was 70 years (48-85 years). The BMI was 16.8-24.7 kg/m2(mean 24.6 kg/m2). The PSA was 1.73-274.5 ng/ml (median 12.06 ng/ml). 26 cases had positive surgical margins. There were 62 cases in lower age group ( 20 ng/ml for two groups were 22, 17, 20 and 29, 18, 20(P=0.41). The Gleason≤6, 7, ≥8 scores for two groups were 6, 18, 38 and 7, 24, 33(P=0.168). All patients were followed up for 48 weeks, and the risk factors for patients with poor control of urinary function were analyzed.\r\n\r\n\r\nResults\r\nThe overall recovery rate of continence 12 weeks after surgery was 46.8%(59/126), with lower age group 56.5%(35/62) and higher age group 37.5%(24/64)(P\u003c0.05). The rate of 24 weeks after surgery was 88.1%(111/126), with lower age group 93.5%(58/62)and higher age group 82.8%(53/64)(P\u003c0.05). In a univariate logistic regression test, the age (P=0.034), BMI(P=0.044)and nerve sparing (P=0.005)were associated with recovery of urinary continence within 12 weeks after surgery (P\u003c0.05). In multivariate logistic regression analysis, age(P=0.011), BMI(P=0.022)and nerve reserved (P=0.014) were independent factors that predicted the recovery of continence within 12 weeks after surgery (P\u003c0.05).\r\n\r\n\r\nConclusions\r\nYounger age, lower BMI and intraoperative nerve preservation independently predicted recovery of continence within 12 weeks. Abstract:OBJECTIVE:To evaluate urinary continence recovery time and risk factors of urinary continence recovery after robot-assisted laparoscopic radical prostatectomy (RARP). METHODS:From January 2019 to January 2021, a consecutive series of patients with localized prostate cancer (cT1-T3, cN0, cM0) were prospectively collected. RARP with total anatomical reconstruction was performed in all the cases by an experienced surgeon. Lymph node dissection was performed if the patient was in high-risk group according to the D'Amico risk classification. The primary endpoint was urinary continence recovery time after catheter removal. Postoperative and pathological variables were analyzed. Continence was rigo-rously analyzed 48 hours, 1 week, 4 weeks, 12 weeks, and 24 weeks after catheter removal. Continence was evaluated by recording diaper pads used per day, and all the patients were instructed to perform the 24-hour pad weight test until full recovery of urinary continence. The patient was defined as continent if no more than one safety pad were needed per day, or no more than 20-gram urine leakage on the 24-hour pad weight test. Time from catheter removal to full recovery of urinary continence was recorded, and risk factors influencing continence recovery time evaluated. RESULTS:In total, 166 patients were analyzed. The mean age of the enrolled patients was 66.2 years, and the median prostate specific antigen (PSA) was 8.51 μg/L. A total of 59 patients (35.5%) had bilateral lymphatic dissection, and 28 (16.9%) underwent neurovascular bundle (NVB) preservation surgery. Postoperative pathology results showed that stage pT1 in 1 case (0.6%), stage pT2 in 77 cases (46.4%), stage pT3 in 86 cases (51.8%), and positive margins in 28 patients (16.9%). Among patients who underwent lymph node dissection, lymph node metastasis was found in 7 cases (11.9%). Median continence recovery time was one week. The number of the continent patients at the end of 48 hours, 1 week, 4 weeks, 12 weeks, and 24 weeks were 65 (39.2%), 32 (19.3%), 34 (20.5%), 24 (14.5%), and 9 (5.4%). Two patients remained incontinent 24 weeks after catheter removal. The continence rates after catheter removal at the end of 48 hours, 1 week, 4 weeks, 12 weeks, and 24 weeks were 39.2%, 58.4%, 78.9%, 93.4%, and 98.8%, respectively. Univariate COX analysis revealed that diabetes appeared to influence continence recovery time (OR=1.589, 95%CI: 1.025-2.462, P=0.038). At the end of 48 hours, 4 weeks, 12 weeks, and 24 weeks after catheter removal, the mean OABSS score of the continent group was significantly lower than that of the incontinent group. CONCLUSION:RARP showed promising results in the recovery of urinary continence. Diabetes was a risk factor influencing continence recovery time. Bladder overactive symptoms play an important role in the recovery of continence after RARP.

Abstract:OBJECTIVE:There are many causative factors for the occurrence of post-prostatectomy incontinence. Objective clinical characteristics, surgical techniques and pelvic floor muscle therapy are the most important ones. The present study was to identify the risk factors associated with urinary incontinence after radical retropubic prostatectomy (RRP).METHODS:A total of 263 patients were recruited for this multivariate analysis. After a close follow-up, a series of pre-, peri- and post-operative factors were recorded and analyzed.RESULTS:Urinary continence after radical prostatectomy was 14.8% at 4 weeks and 94.7% at 16 weeks. The most important recovery interval for urinary continence was 4 - 16 weeks post-operation. Multivariate analysis revealed that age (P = 0.015), blood transfusion (P = 0.017), previous TURP (transurethral resection of the prostate) (P = 0.006) and neoadjuvant hormonal therapy (P = 0.005) were the important risk factors for urinary incontinence.CONCLUSION:During RRP, optimized preservation of urethral rhabdosphincter length, nerve sparing and early postoperative functional exercises can improve the recovery of urinary continence. Age, blood transfusion and previous TURP are the independent prognostic factors. Neoadjuvant hormonal therapy may improve urinary continence through increasing the preoperative length of membranous urethra.
Predictors of Urinary Continence Recovery after Modified Radical Prostatectomy for Clinically High-Risk Prostate Cancer

Abstract:Purpose: To retrospectively determine predictors of urinary continence (UC) recovery in clinically high-risk prostate cancer (PCa) patients treated with modified radical prostatectomy (RP). Materials and Methods: A total of 184 patients with clinically high-risk PCa who underwent modified RP in a single Chinese center were retrospectively reviewed. Pelvic floor muscle training with biofeedback was routinely performed after catheter removal. UC was defined as wearing 0 or 1 protective pad daily. Univariate and multivariate Cox regression analyses were performed to determine the predictors of UC recovery. Results: The median age at surgery was 69.5 years (range 48-82), and the median follow-up duration was 40 months (range 12-111). Only 40 patients (21.7%) received a nerve-sparing procedure. For patients with restored UC, the median time to continence was 1 month (range 1-24). UC recovery at 1 month, 6 months, 12 months and the most recent follow-up was observed in 99(53.8%), 158 (85.9%), 171 (92.9%) and 174(94.6%) patients, respectively. Multivariate Cox regression analysis showed that patient age < 70 years (hazard ratio 1.684, P = .003) and smaller prostate volume (hazard ratio 0.989, P = .036), but not the surgical approach or treatment with a nerve-sparing procedure, independently predicted UC recovery. Conclusion: Age < 70 years and smaller prostate volume were independent predictors of UC recovery in clinically high-risk PCa patients. The adverse factors of high-risk disease were not significantly associated with UC recovery. These results may help surgeons preoperatively counsel patients regarding expected UC outcomes following RP.

Abstract:Radical prostatectomy is one of the most effective treatments for men with clinically localized prostate cancer. Though technical innovations,especially laparoscopic techniques,have developed rapidly for the last decade, urinary incontinence remains one of the most troubling side effects of the operation. While the injury of urethral sphincter and its innervations was considered as the most important reason for incontinence, factors influencing postoperative continence that have been considered include clinical features of patients, such as the patient's age, size of the prostate, prior prostatic surgery and features of the surgical technique itself, such as preservation of full functional-length urethra, preservation of the neurovascular bundles, bladder neck preservation or reconstruction and pelvic floor muscle training during the early phase after the surgery. Nonetheless, many factors above have not been clearly established, and controversy goes on among different studies. This article reviews factors that may influence urinary continence after laparoscopic radical prostatectomy.

Comparison in Efficacy of Periurethral Reconstruction Leading to Urinary Continence Improvement After Robot-assisted Radical Prostatectomy

Abstract:Purpose: This study was designed to evaluate the efficacy of different periurethral structural reconstruction approaches to improve postoperative continence post robot-assisted radical prostatectomy (RARP), which remains a critical concern. Methods: This study included patients with prostate cancer who received single, combined, and nonreconstruction following RARP from April 2017 to May 2021. Medical details were recorded, and patients were followed up via phone or the outpatient clinic. Continence, potency recovery, and quality of life were evaluated. Continence rate was assessed by daily urinary pads use and was compared between groups with the Kaplan-Meier method. Potency recovery was assessed by the questionnaire score. All variables were further analyzed via univariable and multivariable regression analysis. Results: Sixty patients were included in nonreconstruction group. Fifty-two and 156 patients were included in the single and combined group, respectively. Across early postoperative follow-up period, both single and combined reconstruction group showed higher continence rate (p < 0.05). The combined approach showed an advantage over the single one in improving continence rate (p < 0.05). Univariable and multivariable regression analysis point to the reconstruction approach as an independent factor that affects postoperative continence recovery post RARP. Conclusions: Periurethral reconstruction approaches were found to be beneficial in promoting early continence recovery after RARP, without compromising preserved neurovascular bundle. Combination of anterior and posterior reconstruction could be more beneficial on continence recovery and should be considered in clinical practice.
Improve Recovery of Urinary Continence Following Laparoscopic Radical Prostatectomy: a Clinical Report of 51 Cases

Abstract:OBJECTIVE:To present the clinical results of 51 patients of clinically localized prostate cancer treated by laparoscopic radical prostatectomy, especially recovery of urinary continence. To analysis how to improve recovery of urinary continence.METHODS:From February 2004 to March 2008, we performed LRP on 51 patients. All patients were diagnosed by pathological result preoperatively. T1a-1b 4 cases (8%), T1c 15 cases (29%), T2a 7 cases (14%), T2b 5 cases (10%), T2c 20 cases (39%).RESULTS:Forty-nine cases were treated by LRP successfully. There were two cases converted to open surgery. Urine leaking happened in 3 cases, but recovered without intervention. Urinary catheter duration was 14 - 45 days (mean 16 days). Six months and 12 months after LRP, incontinence happened 7/39 cases (17.9%) and 5/20 cases (25%) respectively. Complete incontinence happened in one case. The ratio of incontinence happened in the first 20 cases and the following 31 cases were 6/20 (30%) and 7/31 (22%) respectively. There were two cases had rectal injury. Both cases underwent colostomy. Duration of follow up were from 3 - 53 months (mean 17 months). Two cases recurred. One of them was treated with hormone therapy, the other one died of operation for lung metastasis. Other patients' serum total PSA were less than 0.2 microg/L.CONCLUSIONS:Extraperitoneal laparoscopic radical prostatectomy is efficient and safe approach for localized prostate cancer. The correct handling of pubic prostatic ligaments, external urethral sphincter and neurovascular bundle are important for recovery of urinary continence. And so does the experience of LRP.
Retzius‐sparing Robot‐assisted Radical Prostatectomy Improves Early Recovery of Urinary Continence: a Randomized, Controlled, Single‐blind Trial with a 1‐year Follow‐up

Abstract:ObjectiveTo evaluate the impact of Retzius‐sparing robot‐assisted radical prostatectomy (posterior approach) on early recovery of urinary continence (UC) compared to the conventional approach (anterior approach) for the treatment of clinically localized prostate cancer (PCa).MethodsA total of 110 consecutive patients with clinically localized PCa were prospectively randomized in a 1:1 ratio to an anterior group (n = 55) or a posterior group (n = 55). The primary outcome was immediate UC, defined as freedom from any pad use within 1 week after removal of the urinary catheter. The UC rate following surgery was also calculated with Kaplan–Meier curves, and the log‐rank test was used for statistical comparison. Intra‐operative outcomes, pathological data and oncological outcomes, including positive surgical margin (PSM) status and biochemical recurrence‐free survival (BCRFS), were also compared between the two groups. The comparison of the two approaches was also analysed in subgroups after risk stratification.ResultsOf the patients who underwent the posterior approach, 69.1% achieved immediate UC compared with 30.9% in the anterior group (relative risk 2.24, 95% confidence interval [CI] 1.48–3.51; P = 0.000). The relative Kaplan–Meier curves for UC during the 12‐month follow‐up revealed statistically better recovery in the posterior group when compared with the anterior group (hazard ratio [HR] 1.51, 95% CI 1.01–2.24; log‐rank P = 0.007). No statistically significant differences were observed between the groups regarding complications (P = 0.399), PSM status (P = 0.225) or BCRFS (HR 4.80, 95% CI 0.97–23.78; log‐rank P = 0.111). In sub‐analyses, no significant difference between the two approaches with regard to UC recovery in patients with high‐risk PCa was observed (HR 1.26, 95% CI 0.63–2.51; log‐rank P = 0.415).ConclusionsThe Retzius‐sparing approach significantly improved early recovery of UC compared to the conventional approach. Further prospective studies are needed to confirm the benefits of the Retzius‐sparing approach for clinically localized PCa, especially for high‐risk cases.
“Total Reconstruction” of the Urethrovesical Anastomosis Contributes to Early Urinary Continence in Laparoscopic Radical Prostatectomy

Abstract:PURPOSE:To demonstrate the effect of total reconstruction technique on postoperative urinary continence after laparoscopic radical prostatectomy (LRP).MATERIAL AND METHODS:LRP was performed using a standard urethrovesical anastomosis in 79 consecutive patients (Group-A) from June 2011 to October 2012, and a total reconstruction procedure in 82 consecutive patients (Group-B) from June 2012 to June 2013. The primary outcome measurement was urinary continence assessed at 1, 2, 4, 12, 24 and 52 weeks after catheter removal. Other data recorded were patient age, body mass index, International Prostate Symptoms Score, prostate volume, preoperative PSA, Gleason score, neurovascular bundle preservation, operation time, estimated blood loss, complications and pathology results.RESULTS:In Group-A, the continence rates at 1, 2, 4, 12, 24 and 52 weeks were 7.59%, 20.25%, 37.97%, 58.22%, 81.01% and 89.87% respectively. In Group-B, the continence rates were 13.41%, 32.92%, 65.85%, 81.71%, 90.24% and 95.12% respectively. Group-B had significantly higher continence rates at 4 and 12 weeks after surgery (P<0.001 and P=0.001). There were no significant differences between the groups with respect to patient's age, body mass index, prostate-specific antigen level, prostate volume, IPSS, estimated blood loss, number of nerve-sparing procedures and postoperative complications.CONCLUSIONS:Total reconstruction technique in the procedure of urethrovesical anastomosis during LRP improved early recovery of continence.
Preoperative Risk Factors for Early Postoperative Urinary Continence Recovery after Non-Nerve-sparing Radical Prostatectomy in Chinese Patients: a Single Institute Retrospective Analysis

Abstract:Urinary incontinence (UI) remained a significant complication after radical prostatectomy and led to impaired quality of life. Early continence is a goal to take into consideration for better patient satisfaction after radical prostatectomy. To identify the independent preoperative risk factors associated with UI after radical prostatectomy (RP), we evaluated 446 patients treated with non-nerve-sparing RP between 2010 and 2013 at our institution. The incontinence rate was 98.7% and 46.6% after catheter removal and 3 months after surgery, respectively. We examined several preoperative factors including age, smoking, body mass index (BMI), comorbidities such as hypertension and diabetes, preoperative prostate-specific antigen (PSA) level, Gleason score, and preoperative pelvic floor muscle exercise (PFME). The results suggested preoperative PFME was the only independent protective risk factor for immediate continence after catheter removal. At 3 month following surgery, age at surgery represented a risk factor for delayed continence, while BMI and preoperative PFME were the protective risk factors for postoperative UI. Our results supported that age and preoperative PFME were predictive factors for early continence after RP. These findings could help clinicians to counsel men and their partners about postoperative incontinence.
A Retrospective Study to Evaluate the Effect of Preoperative Hormonal Therapy on Continence Recovery

Abstract:ObjectiveTo evaluate whether different preoperative hormonal therapy options affect postoperative continence and to identify risk/protective factors for continence recovery. MethodsThis is a retrospective analysis of several clinical trials (NCT04356430, NCT04869371, NCT04992026 and NCT05406999). Data from patients treated with hormonal therapy followed by RARP were collected and analyzed. Continence was defined as 0 pad/day or one safety pad. ResultsThe study included 230 patients with adequate information. The median time to continence recovery is 8 weeks. A total of 216 (93.9%) participants recovered to urinary continence within 12 months after surgery. 21 (9.1%) participants achieved immediate continence. 69, 85, 27 and 14 participants restored continence at 1 month, 1-3 month, 3-6 month, 6-12 month, accounting for 30.0%, 40.0%, 11.7% and 6.1% accordingly. No difference in continence recovery was found among different preoperative hormonal treatment options (p=0.821). Cox regression showed that membranous urethral length (MUL) was the only independent factor influencing urinary continence recovery either in the univariate analysis (OR=1.13, 95%CI: 1.04-1.22, p=0.002) or in the multivariate analysis (OR=1.12, 95%CI: 1.04-1.20, p=0.002). Different preoperative treatment options were not associated with urinary recovery. More advanced preoperative T stage (OR=0.46, 95%CI: 0.24-0.85, p=0.014) delayed the recovery of immediate continence. MUL was associated with continence restoring at 1 month (OR=1.20, 95%CI: 1.03-1.39, p=0.017), 3 month (OR=1.27, 95%CI: 1.07-1.51, p=0.006), 6 month (OR=1.34, 95%CI: 1.07-1.67, p=0.011) and 12 month (OR=1.36, 95%CI: 1.01-1.84, p=0.044). ConclusionThere is no difference in postoperative continence recovery among ADT, ADT+Docetaxel and ADT+Abiraterone preoperative treatment options. More advanced T stage indicated poor immediate continence recovery. Longer membranous urethral length was a promotional factor for both short-time and long-time continence recovery.

Abstract:Abstract Introduction and Objectives Urinary incontinence (UI) remains a significant concern after radical prostatectomy. As a part of a nursing follow‐up, we evaluate quality‐of‐life (QoL) which includes UI recovery. Patients proposed to Retzius‐sparing robot assisted radical prostatectomy (RS‐RARP) have a bundle of appointments starting pre‐operatively and continuing after surgery 1 week, 1, 3, 6, 9 and 12 months. In these, we started a urinary rehabilitation programme (URP) performing Kegel Exercises, adaptative and behavioural strategies. The objective is to evaluate timeline until urinary continence recovery after RS‐RARP, since this is one of the most asked question by our patients. Materials and Methods Between July 2017 and April 2020 we conducted a prospective observational single‐centre study with a total of 208 patients submitted to RS‐RARP. We applied the Expanded Prostate Cancer Index Composite‐26 (EPIC‐26) questionnaire pre and postoperatively at 3, 6, 9 and 12 months, and studied UI by evaluating pad count measured in the 3rd question. The inclusion criteria are: patients that did not have adjuvant or salvage treatments during the follow up; have all pre‐ and post‐operative evaluations; who were continent pre‐operatively; and started the URP. A total of 72 patients were included. We have considered the criteria of no pad use for continence recovery. Every patient provided written informed consent for study inclusion, approved by the Institutional Ethics Committee (Approval 07.07.2017). Results Preliminary analysis of enrolled patients reveals that 57 (79,16%) recovered continence during the 12 months period. By 3‐month mark, we observed that 42 (73,68%) patients were continent; 7 (12,28%) recovered continence at 6‐month mark; 4 (7,02%) recovered continence at 9‐month mark and 4 (7,02%) recovered continence 1 year after surgery. We further analysed the total number of pads used daily by the 15 patients (20,83%) that had UI at 12‐month mark: 13 patients (86,67%) reported using 1 pad a day and 2 patients (13,33% or 2,77% of all studied population) reported using 3 or more pads a day. Conclusion The preliminary results demonstrate a gradual improvement in UI over the first year, with a significant number of patients recovering at 3‐month mark. These positive results are most probably consequences of both the surgical approach and the URP. Is required a sub‐group analysis to further delineate factors impacting continence recovery, aiding in postoperative rehabilitation care plans that will impact positive outcome on QoL associated with a fast UI recovery. For the latter, digital tools to conduct patient reported outcomes are necessary to ascertain the extent of impact in QoL. Furthermore, we need to rethink remote strategies to improve patient compliance as a significant number of them were excluded for not attending on‐site nursing appointments.
Pre- and Postoperative Parameters on Magnetic Resonance Imaging Predict Continence Recovery after Laparoscopic Radical Prostatectomy

Abstract:Background: To evaluate the association between pre- and postoperative parameters on magnetic resonance imaging (MRI) and continence recovery after laparoscopic radical prostatectomy (LRP). Methods: 73 patients who underwent LRP were retrospectively reviewed. Demographic characteristics, clinicopathologic outcomes and several MRI parameters before and after surgery were evaluated. Continence was defined as no pad per day. Early continence recovery was defined as continence recovery within 3 months. Kaplan-Meier analyses and log-rank test were used to compare time to continence recovery. Cox proportional-hazards regression analyses were performed to identify independent predictors of continence recovery after LRP.Results: Patients with smaller prostatic volume, shorter intravesical prostatic protrusion length (IPPL), longer preoperative membranous urethral length (MUL), lower MUL-removal rate, triangular vesicourethral anastomosis (VUA) and neurovascular bundle sparing experienced a faster continence recovery (All, p < 0.05). Multivariate analyses revealed IPPL (hazard ratio [HR]: 0.94, p = 0.044), preoperative MUL (HR: 1.10, p = 0.032), MUL-removal rate (HR: 0.91, p = 0.007) and shape of VUA (square vs. triangle, HR: 2.30, p = 0.012) were independent predictors of continence recovery after LRP.Conclusion: IPPL, preoperative MUL, MUL-removal rate and shape of VUA were promising parameters on MRI for predicting continence recovery after LRP.
Analysis of the predicting factors for erectile function recovery after laparoscopic radical prostatectomy

Abstract:Objective To evaluate the recovery rate of erectile function (EF) and identify various factors predicting the recovery of EF in men undergoing laparoscopic radical prostatectomy (LRP) in our center.Methods From January 2011 to December 2012,a total of 106 men with localized prostate cancer underwent LRP in our center by one surgeon were included,and we gathered the preoperative EF condition and perioperative factors,such as patients' age,comorbidity and surgical procedure,et al.We followed up all the patients by telephone in August 2013 to collect the EF recovery data of these paticnts after LRP,with the mean follow-up of 18 (8-31) months.We analyzed the perioperative factors in order to predict the EF recovery after LRP.Results A total of 60 patients with preoperative sexual activity were included,and there was no difference in baseline data compared with other cases.Recovery of potency was defined as postoperative penile erection.The recovery rate of EF was 46.7％ (28/60) without the use of any drugs or devices for erection assistance.In our study,the recovery rate of EF in age ≤60 years was 90.0％ (9/10),and it was better than other two older groups (P =0.001).In patients whose preoperative IIEF-5 score was 22-25,the recovery rate of EF was 66.7％ (8/12),and it was better than those with IIEF-5＜22 (P=0.006).The recovery rate of EF in patients with preserved neurovascular bundle was 53.1％ (26/49) and it was 83.3％ (5/6) in patients with preserved accessory pudendal artery,which was better than those without preservation (P=0.036 and P =0.023).Conclusions In our study,age and preoperative EF were the significant factors to predict potency recovery,and preservation of neurovascular bundle and accessory pudendal artery during LRP were the positive predictor factors.
Urinary Continence Recovery after Retzius-Sparing Robot Assisted Radical Prostatectomy and Adjuvant Radiation Therapy

Abstract:Retzius-sparing robot-assisted radical prostatectomy (RS-RARP) allows the preservation of the structures advocated to play a crucial role in the continence mechanism. This study aims to evaluate the association between adjuvant radiation therapy (aRT) and urinary continence (UC) recovery after RS-RARP. For the purpose of the current study, all patients submitted to RS-RARP for prostate cancer (PCa) at a single high-volume European institution between January 2010 and December 2021 were identified. Only patients that harbored pT2 stage with positive surgical margins or pT3/pN1 stage with or without positive surgical margins were included in the analyses. Two groups of patients were identified as follows: patients who had undergone aRT and patients submitted to observation (no-aRT patients). As per definition, aRT was delivered within 1-6 months after surgery. After 1:1 propensity score matching, 124 aRT patients were compared with 124 no-aRT patients who continued standard follow-up protocol after surgery. UC recovery was 81 vs. 84% in aRT vs. no-aRT patients (p = 0.7). In multivariable Cox regression analyses, aRT did not reach the independent predictor status for UC recovery at 12 months. In the subgroup analysis including only aRT patients, only the nerve-sparing technique was independently associated with UC recovery at 12 months. Conversely, the type of aRT (IMRT/VMAT vs. 3D-CRT) did not reach the independent predictor status for UC recovery at 12 months. The current study is the first to address the association between aRT and UC recovery in patients treated with RS-RARP for PCa. Based on our data, aRT is not associated with worse UC recovery. In the cohort of patients treated with aRT, the nerve-sparing technique independently predicted UC recovery.

Abstract:Purpose: To evaluate the impact of preoperative magnetic resonance imaging anatomic features on urinary continence recovery after laparoscopic radical prostatectomy (LRP). Methods: We retrospectively analyzed 150 consecutive prostate cancer patients who underwent LRP between July 2015 and June 2018 in our institution. Patients reporting freedom from using safety pad (0 pads/day) were defined as urinary continent. We evaluated the association of urinary continence recovery after LRP and the perioperative variables, including age, prostatic volume (PV), intravesical prostatic protrusion length (IPPL), membranous urethral length (MUL), and neurovascular bundle (NVB) sparing status. Kaplan-Meier and log-rank tests were used to compare urinary continence rates between groups. Univariate and multivariate Cox proportional hazards regression analyses were used to identify independent predictors for urinary continence recovery. Results: 60.6% (91/150), 83.3% (125/150), and 96.0% (144/150) of the patients experienced urinary continence recovery at 3, 6, and 12 months, respectively. There was significant difference in PV, IPPL, MUL, and NVB sparing between groups stratified by urinary continence status at 3, 6, and 12 months. Kaplan-Meier curves of urinary continence rates showed significant differences between groups stratified by PV (<50 mL vs. >= 50 mL, p < 0.001), IPPL (<5 mm vs. >= 5 mm, p < 0.001), MUL (>= 15 mm vs. <15 mm, p < 0.001), and NVB sparing status (yes vs. no, p = 0.003), respectively. On univariate analysis, PV, IPPL, MUL, and NVB sparing were significantly associated with urinary continence recovery (all, p < 0.05). On multivariate analysis, only IPPL (HR = 0.94, p = 0.003) and MUL (HR = 1.10, p < 0.001) were independent predictors for urinary continence recovery. Conclusion: Patients with larger IPPL and shorter MUL have higher chances of delayed recovery of urinary continence after LRP. IPPL and MUL were reliable morphometric parameters for predicting urinary continence.

Abstract:Objective: To evaluate the application of a rehabilitation management protocol for urinary incontinence after robot-assisted laparoscopic prostatectomy (RALP). Methods: We conducted a retrospective cohort study of 114 patients who underwent RALP between August 2021 and November 2021 as the control group and a prospective analysis of 114 patients who underwent RALP between May 2022 and August 2022 as the experimental group. The rehabilitation management protocol focused on preoperative stage, postoperative care, day of catheter removal, 1 month postoperative, 3 months postoperative, 6 months postoperative, and 12 months or more postoperative. Results: The 24-h pad test was significantly lower in the experimental group compared with the control group at 2 and 6 months after RALP (both P < 0.01). The scores of the international consultation on incontinence questionnaire-short form (ICIQ-SF) in the experimental group were significantly lower than those in the control group at 1 month after RALP (P < 0.01).The scores of quality of life in the experimental group were significantly higher than those of the control group at 1, 2, and 6 months after RALP (all P < 0.01).The scores of Broome Pelvic Muscle Self-efficacy Scale (BPMSES) were lower than those of the control group at 1, 2, 3, and 6 months after RALP (all P < 0.01). Conclusion: The application of the rehabilitation management protocol had significant beneficial effects on urinary functions and quality of life in patients with prostate cancer after RALP.
