An Unusual Cause of Dysphagia and Melena.

Abstract:Question: A 51-year-old man complained of a dry cough, dysphagia, odynophagia, and melena for 10 months. He had a >30 pack-years smoking history, but did not consume an excessive amount of alcohol. He had no history of dyspnea, respiratory tract bleeding, or massive hemorrhage of the gastrointestinal tract. No clinically important signs were found during physical examination. Laboratory results exhibited a positive hemoccult stool test and mild anemia (hemoglobin, 107 g/L). The patient was found to have a large, soft tissue density mass in the upper esophagus on a computed tomography scan (Figure A, white arrow). Esophagogastroduodenoscopy then revealed a large, thick-pedunculated, nonpulsatile, partially erosive and necrotic subepithelial tumor (SET) 15–23 cm from the incisors (Figure B). The lesion was diagnosed as a hypopharyngeal SET because its root arose from the lateral wall of the right hypopharynx on esophagogastroduodenoscopy (Figure C, white arrow). The large body of the tumor extended into the upper esophageal lumen and led to significant narrow of the esophageal inlet. The patient’s melena and chronic blood loss anemia were attributed to erosion and necrosis of the tumor (Figure D). Based on these findings, what is the most likely diagnosis? Pharyngeal endoscopic submucosal dissection was attempted under general anesthesia (Supplementary Video). The tumor root was carefully exposed, and submucosal injection was performed around the thick peduncle. Mucosal incision was performed at the oral side of the peduncle. The submucosal layer beneath the tumor was then dissected directly from the oral side to the anal side, and saline solution was injected repeatedly during dissection when necessary (Figure E). The tumor was en bloc resected and removed with a snare. The specimen was 4 × 8 cm and weighed 16 g (Figure F). Pathologic examination showed an angiofibroma with no evidence of malignancy (Figure G; H&E × 100). The patient had no procedure-related adverse events, and his symptoms completely resolved.
