Abstract:Eccrine porocarcinoma is an extremely rare skin adnexal malignant neoplasia with highly invasive and metastatic potential. We report an additional case of eccrine porocarcinoma with intracranial metastases. This case is characterized by a complete record of the progress of eccrine porocarcinoma, its immunohistochemistry after three operations showed a progressive increase in the level of Ki-67 index. We herein report a case of a 37-year-old-male with eccrine carcinoma occurring on the left posterior occipital scalp which invaded the skull and dura, presenting with progressive headache. This patient has performed three surgeries in total. During the last hospitalization, he underwent an extended surgical resection, lymphadenectomy, myocutaneous flap transplantation and vascular anastomosis in our institution. After surgery, he was treating with radiotherapy at 200 Gray in 12 fractions. But one year after the operation, he developed chest tightness, imaging examination and biopsy puncture revealed pulmonary metastasis. Intracranial metastasis of eccrine porocarcinoma is a late event with poor prognosis. This case emphases on that progressively increased level of Ki-67 index may predict more chance to occur the intracranial metastasis of scalp eccrine porocarcinoma, long-term follow-up and appropriately dense follow-up interval is necessary.
Generalized Type 2 Segmental Disseminated Superficial Actinic Porokeratosis Coexisted with Multiple Cutaneous Squamous Cell Carcinomas: Analysis of Two Cases.

Abstract:Porokeratosis (PK) is defined as hyperpigmented macules or patches with a distinctive, ridge-like hyperkeratotic border which is histologically characterized by a cornoid lamella. Here, we report two cases of linear porokeratosis which converted to multiple cutaneous squamous cell carcinoma after long history progression. In addition, patient 2 was accompanied by secondary dermal amyloid deposits, which was rare reported.

Abstract:Porocarcinoma is an unusual, locally aggressive and potentially fatal neoplasm. Several cutaneous malignancies have been described in association with porocarcinoma, including squamous cell carcinoma, basal cell carcinoma and tricholemmal carcinoma. Previous reports have indicated that the occurrence of malignant tumours in combination with porocarcinoma is extremely rare, in particular with regard to Bowen disease (BD). We report an uncommon case of porocarcinoma occurring synchronously in a single BD lesion in a 63-year-old woman with multiple BD lesions. The clinical and histological findings confirmed this diagnosis.

Abstract:Abstract Background: Porokeratosis is a rare, acquired or inherited disorder of keratinization. There are numerous clinical types of porokeratosis and they could coexist in one patient and in multiple members of an affected family. However, coexist of disseminated superficial actinic porokeratosis (DSAP) and porokeratosis ptychotropica (Ppt) is rare. Case presentation: A 45-year-old man presented with long-standing skin lesions. Physical examinations found numerous small, brown 2- to 4-mm patches on his face and several hyperkeratotic, verrucous plaques on his trunk and extremities. His father and one of his brothers also had similar lesions for years. Skin biopsies showed a cornoid lamella in the epidermis. And we identified c.155 G>A mutation of the mevalonate kinase (MVK) gene convertting a serine residue to asparagine (p.Ser52Asn) was the causative mutation for porokeratosis in this family. The clinicopathologic diagnosis of DSAP and Ppt with a novel MVK gene mutation was made. The hyperkeratotic plaques on the patient's scrotum were completely removed by microwave knife for more than 10 times. Conclusion: We report an unusual case of DSAP coexisting with Ppt and identified a novel MVK gene mutation in this patient's family. The microwave knife is an effective and safe therapy for porokeratosis.

Abstract:Porokeratosis is a group of hereditary or acquired disorders of epidermal keratinization, characterized by keratotic lesions with an atrophic center and a prominent peripheral ridge. At least seven clinical variants have been identified: the plaque type, disseminated superficial porokeratosis, disseminated superficial actinic porokeratosis, linear porokeratosis, giant porokeratosis, porokeratosis plantaris palmaris et disseminate, and punctuate porokeratosis. Genital porokeratosis is a rare condition even in disseminated forms. We described two male patients who had porokeratosis only affecting the scrotum. The patients were treated by surgical excision under local anesthesia. One-year follow-up revealed no sign of recurrences and malignant changes.

Porokeratoma: a Case Report

Abstract:A case of porokeratoma is reported.A 35-year-old man presented with asymptomatic verrucous nodules on both buttocks for more than 1 year.Skin examination at the first visit revealed two pea-sized verrucous nodules on bilateral buttocks.The lesions recurred at the primary site 3 months after surgical resection.At the second visit,there were 4 verrucous nodules,including 1 on the left buttock and 3 on the right buttock.Histopathological examination showed a well-defined lesion characterized by acanthosis and compact,thick orthokeratosis with seveal broad cornoid lamellae.A diagnosis of porokeratoma was made.The lesion was resected again followed by oral acitretin treatment.No recurrence was observed during 15 months of follow up.

Dear Editor, A 23‐year‐old‐man presented with a 5‐year history of progressive pruritic brown hyperkeratotic plaques on his buttocks with slightly elevated borders. Scattered hyperkeratotic satellite lesions could be observed peripherally (a). Histopathological examination showed multiple cornoid lamellae with underlying dyskeratotic keratinocytes in the epidermis (b). The diagnosis of porokeratosis ptychotropica was established. Porokeratosis ptychotropica is a rare and under‐recognized variant of porokeratosis, characterized by brown itching verrucous plaques in the genitogluteal region with multiple cornoid lamellae seen in histology. Lucker and colleagues first coined this diagnosis in 1995, using the Greek words ptyche (fold) and trope (turning) to describe the flexural distribution of the disease.1 The pathogenesis of this disease remains unknown. Malignant transformation of the porokeratotic lesion can occur in 7·5% of cases, mostly associated with long‐term, linear, large‐size lesions.2

Abstract:Porokeratosis is a rare hereditary chronic progressive keratosis of the skin disease , it is autosomal dominant inheritance . In the five types of porokeratosis , disseminated superfacial actinic poro-keratosis is the most common . At present , it has been reported that there are 6 genes related to the inci-dence of disseminated superficial actinic porokeratosis : MVK , MVD , FDPS , SSH1 , SART3 , and SLC17 A9 . This article reviews the discovery of its causative genes , possible pathogenic mechanisms , a-nalysis of genotype-phenotype correlation .

While PK often converts into squamous cell carcinoma (SCC), conversion from disseminated superficial porokeratosis (DSP) alone is rarely reported except for one case in which DSP and LP coexisted and converted to SCC. Here, we report the case of a patient with SCC converted from DSP alone, presenting with coin-sized macules on the bottom right of his waist that developed into an ulcer at the center. The patient underwent radiation therapy, which effectively treated the SCC but did not resolve the PK. This article highlights regular follow-up and undergo comprehensive diagnosis, both of which are beneficial to enable early detection and management of DSP that has converted to into SCC; in addition, standardized medical treatment may help improve the treatment therapeutic effect of in similar diseases. Keywords: disseminated superficial porokeratosis, porokeratosis, squamous cell carcinoma Porokeratosis (PK) is a rare, genetically chronic progressive skin disease characterized by dike-like verrucous bulges at the edges, atrophy in the center, and histological manifestations involving cornoid lamella. 1 PK can be classified as porokeratosis of Mibelli (PM), disseminated superficial actinic porokeratosis (DSAP), disseminated superficial porokeratosis (DSP), porokeratosis palmaris et plantaris disseminate (PPPD) and Linear Porokeratosis (LP), etc. 2 Porokeratosis often develops into cancer: studies show that 6.9% to 30% of cases will develop into non-melanoma skin cancer (NMSC), 3 including basal cell carcinoma, Merkel cell carcinoma and squamous cell carcinoma, with SCC having the highest risk ratio among the three types. 4 According to reports, among the many clinical subtypes of PK, the top three canceration rates of SCC are for LP at 19%, PM at 7.6%, and DSAP at 3.4%. 4 Compared with other subtypes of PK, there is currently only one reported case of the transformation of DSP into SCC, and it is due to coexistence with LP; 5,6 in contrast, in DSP alone, canceration is rare, which may be related to DSP's relatively rare incidence. 7 In the present report, we describe the case of a 61-year-old male patient with a history of DSP of approximately 20 years and a relevant family medical history who was finally diagnosed with squamous cell carcinoma (SCC) under the premise of no coexistence of multiple PK variants and no history of prolonged sun exposure. A 61-year-old male presented with a 20-year history of multiple annular brown macule lesions surrounded by elevated borders all over the body. Twenty years ago, he had undergone total surgical resection at a local hospital, but the diagnosis was not available to us. Although the patient's condition had improved following total surgical resection, the symptoms suddenly recurred and worsened, despite the absence of sun exposure, 10 years previously. During this period, he did not seek professional medical treatment until one year prior to presentation at our clinic, and was diagnosed with DSP at the time. Because the patient's symptoms did not improve significantly after treatment with oral acitretin (20 mg/day) and topical vitamin E cream, the patient did not follow medical advice to continue regular treatment. Over the course of the next six months, the patient experienced gradual swelling, thickening, and plaque formation on a rash located on the bottom right of his waist. Subsequently, the area turned into an ulcer, which was accompanied by mild pain. The patient attempted self-treatment with topical medications; however, no significant improvement was observed. Consequently, the patient sought further treatment at our department. The patient denied history of any other chronic disease and sunburn, but admitted during consultation that his uncle had similar diseases. Dermatological examination revealed scattered brown macules with annular keratotic rims, varying from pin cap to broad bean. Some of these brown macules merged to form an annular configuration with raised edges resembling dikes, well-demarcated borders, and a slightly atrophic center. Keratocystoma of the Parotid Gland: a Clinicopathological Study and Literature Review

Abstract:The authors investigated the clinicopathological characteristics of keratocystoma of the parotid gland. Two cases of parotid gland keratocystoma in the files of Nanjing Stomatological Hospital were analysed. These slowly growing parotid gland tumours occurred in two women aged 29 and 49 years. The cut surface showed multilocular cystic lesions filled with keratin materials. Histologically, there were multi-cystic spaces and solid epithelium islands, containing keratinized lamellae. Without cytological atypia, the lining stratified squamous epithelium showed apparent keratinization through an orthokeratotic or parakeratotic pathway. No skin appendage formation was observed. Both cases immunoreactively stained positively for AE1/AE3 and CK5/6 but negatively for CK8/18, S-100 and Calponin. There was no evidence of recurrence 3 or 4 years after superficial parotidectomy. The data from these two cases and cases previously published suggest that keratocystoma of the parotid gland is a benign cystic neoplasm. Surgical resection is apparently sufficient for cure.

Abstract:Disseminated superficial actinic porokeratosis (DSAP) is an uncommon autosomal dominant chronic keratinization disorder, characterized by multiple superficial keratotic lesions surrounded by a slightly raised keratotic border. Thus far, although two loci for DSAP have been identified, the genetic basis and pathogenesis of this disorder have not been elucidated yet. In this study, we performed a genome-wide linkage analysis in three Chinese affected families and localized the gene in an 8.0 cM interval defined by D12S330 and D12S354 on chromosome 12. Upon screening 30 candidate genes, we identified a missense mutation, p.Ser63Asn in SSH1 in one family, a frameshift mutation, p.Ser19CysfsX24 in an alternative variant (isoform f) of SSH1 in another family, and a frameshift mutation, p.Pro27ProfsX54 in the same alternative variant in one non-familial case with DSAP. SSH1 encodes a phosphatase that plays a pivotal role in actin dynamics. Our data suggested that cytoskeleton disorganization in epidermal cells is likely associated with the pathogenesis of DSAP.

Abstract:Disseminated superficial actinic porokeratosis is an autosomal dominant cutaneous disorder characterized by many uniformly small, minimal, annular, anhidrotic, and keratotic lesions. The genetic basis for this disease is unknown. Using a genomewide search in a large Chinese family, we identified a locus at chromosome 12q23.2-24.1 responsible for disseminated superficial actinic porokeratosis. The fine mapping study indicates that the disseminated superficial actinic porokeratosis gene is located within a 9.6 cM region between markers D12S1727 and D12S1605, with a maximum two-point LOD score of 20.53 (theta = 0.00) at D12S78. This is the first locus identified for a genetic disease where the major phenotype is porokeratosis. The study provides a map location for isolation of a gene causing disseminated superficial actinic porokeratosis.

Abstract:Sir: Since the proposal of the term “porokeratotic eccrine ostial and dermal duct nevus” by Abell and Read in 1980, fewer than 30 cases have been published. The typical presentation has been congenital, asymptomatic papules and plaques on one or more of the extremities, rarely involving the trunk, face, or scalp.1,2 Lesions follow Blaschko’s lines with variable extent of involvement. The Blaschkoid configuration reflects clinical mosaicism attributed to a novel postzygotic mutation, chromosomal alterations during embryogenesis or, less frequently, functional mosaicism of an X-linked defect. Kroumpouzos et al. reported a similar pattern of clinical presentation in a rare case of systemized porokeratotic eccrine and hair follicle nevus.3 The histologic hallmark of porokeratotic eccrine ostial and dermal duct nevus is cornoid lamellae packed within epidermal invaginations overlying eccrine units. This has been attributed to either a hyperplastic acrosyringium with relative sparing of the glandular portion or a hyperplastic epidermal invagination associated with an eccrine-like duct.4 Although it has been speculated that an abnormal clone of epidermal cells at the base of the parakeratotic columns may be prone to malignant degeneration, we found no documentation of associated malignancy. We present a case of congenital porokeratotic eccrine ostial and dermal duct nevus in a 39-year-old woman whose ulcerated lesions within the porokeratotic eccrine ostial and dermal duct nevus revealed squamous cell carcinoma. The patient was born with diffuse, whirled hyperpigmentation of her trunk and extremities. Since the age of 6, these lesions had undergone intermittent pruritic blistering and gradually evolved into painful, nonhealing wounds. Two years before her current presentation, she was evaluated for worsening symptoms. Biopsy specimens obtained at that time were benign, revealing columns of cornified parakeratotic cells overlying epidermal invaginations and adnexal ostia. When referred for surgical evaluation, hyperpigmented patches and plaques were noted in a Blaschkoid arrangement throughout the trunk and upper and lower extremities (Fig. 1). The distribution pattern of the nevus had not changed since birth. Multiple oozing ulcers are present.Over an 11-month period, she underwent staged excision of all ulcerated lesions, followed by either primary closure or split-thickness skin grafts. A total of 36 specimens were sent for pathologic examination. Thirty-one (86 percent) of these revealed squamous cell carcinoma adjacent to the cornoid lamella (Fig. 2). Note the angled column of parakeratosis, diminished granular layer, and dyskeratosis (hematoxylin and eosin; original magnification, ×200).A case of diffuse, painful, nonhealing ulcers revealing squamous cell carcinoma within congenital porokeratotic eccrine ostial and dermal duct nevus lesions is presented. This is an unusual finding in what is otherwise known as a benign clinical entity that rarely requires surgical therapy. One may consider malignant transformation of an abnormal clone of epidermal cells at the base of the parakeratotic columns. Alternatively, malignant transformation has been documented in rare cases of linear epidermal nevus—an entity that bears close resemblance to porokeratotic eccrine ostial and dermal duct nevus clinically. However, the distinction between linear epidermal nevus with parakeratotic features and porokeratotic eccrine ostial and dermal duct nevus has not been adequately established in the current literature.5 Further investigation into the genetic and molecular specifics of porokeratotic eccrine ostial and dermal duct nevus pathogenesis is required. Abstract:Disseminated superficial actinic porokeratosis (DSAP) is the commonest clinical type of porokeratosis, characterized by many uniformly small, annular, anhidrotic and keratotic lesions developing on areas of sunlight exposure.1 In this study, we describe a novel premature termination mutation in FDPS gene in a Chinese family with DSAP. The proband (II-2, Figure 1a) was a 72-year-old woman from a family of four generations, in which her daughter had the similar cutaneous presentations. This article is protected by copyright. Abstract:BACKGROUND:Disseminated superficial actinic porokeratosis (DSAP) is a rare autosomal dominant genodermatosis characterised by annular lesions that has an atrophic centre and a prominent peripheral ridge distributed on sun exposed area. It exhibits high heterogeneity, and five linkage loci have been reported. The mevalonate kinase (MVK) gene located on 12q24 has been confirmed as one of the disease-causing genes. But, the pathogenesis of a large part of DSAP remains unclear so far.METHODS:The recruited with DSAP carried no MVK coding mutations. Exome sequencing was performed in two affected and one unaffected individual in Family 1. Cosegregation of the candidate variants was tested in other family members. Sanger sequencing in 33 individuals with familial DSAP and 19 sporadic DSAP individuals was performed for validating the causative gene.RESULTS:An average of 1.35×10(5) variants were generated from exome data and 133 novel NS/SS/indels were identified as being shared by two affected individuals but absent in the unaffected individual. After functional prediction, 25 possible deleterious variants were identified. In Family 1, a missense variant c.932G>A (p.Arg311Gln) in exon 10 of SLC17A9 was observed in cosegregation with the phenotype; this amino acid substitution was located in a highly conserved major facilitator superfamily (MFS) domain in multiple mammalian. One additional missense variant c.25C>T (p.Arg9Cys) in exon 2 of SLC17A9 was found in Family 2.CONCLUSIONS:The result identified SLC17A9 as another pathogenic gene for DSAP, which suggests a correlation between the aberrant vesicular nucleotide transporter and the pathogenesis of DSAP.
