The influence of prostate Volume on the optimal choice of the biopsy protocol

Abstract:Objective: To decide the rational prostate biopsy protocols after retrospective analysis the result of different prostate biopsy. Methods: A total of 461 patients with the indications of abnormal digital rectal examination and/or serum PSA higher than the level of 4ng/ml were enrolled in this study. According to prostate volume, they were divided into four groups, including group 1 (≥ 15ml), group 2(≥ 35 ml), group 3(≥ 55ml), and group 4(≥ 75ml). All patients underwent 10 cores biopsy. Among them, we compared the positive rate of 10 and 8 and 6 biopsy methods.An additional 1-3 biopsy were performed from each clear edge area of focus detected by transrectal ultrasound. Results: Of 461 patients, 148(32.1%) were positive for prostate cancer. The positive rates decreased significantly from 49.6% to 21.8% as prostate volume increased in four groups. The lowest cancer detection rates(10.3%) were in the clear edge area of focus. The best cancer detection rates (84% to 95%) were happened in sextant biopsy protocols laterally placed cores including the apex/ lateral mid gland/lateral base. The positive rate of 8 cores biopsy protocol consisting of the apex/mid gland/lateral mid gland/lateral base was lower (96%) than that of the 10 cores biopsy protocol(98%). The 10 cores biopsy protocol for patients with prostate volume above 35ml in the cancer detection rate was better than that of the optimal 8 cores biopsy. Conclusion: The classic sextant biopsy protocol might be inadequate for all prostate volumes. The 8 core biopsy protocol consisting of the apex/mid gland/lateral mid gland/lateral base can be suit for the patients with prostate volume of 15 to 35ml, whereas. The 10 core biopsy protocol should be recommended for patients with prostate volume above 35ml.
