Abstract:Objective: To analyze the CT features of solid-pseudopapillary tumor of the pancreas (SPTP). Methods: Nineteen patients with SPTP proved pathologically underwent two-phase or three-phase enhanced CT, and the CT findings were analyzed combined with pathological findings. Results: SPTP manifested as solitary and exogenous masses, appeared as lobulated in 12 patients, as oval in 6, and similar to round in 1 patient. The maximal diameter of SPTP was beyond 5 cm in 10 patients, complete capsule was observed in 17 patients, incomplete capsule was observed in 1 patient, no capsule was fourd in 1 patient and wall nodular was seen in 4 patients. The lesion showed as a heterogeneous mass composed of solid and polycystic parts. After contrast administration, the solid composition showed mild-moderate enhancement in arterial phase, and progressive delayed enhancement in portal phase. Hemorrhage was seen in 2 patients, calcification was seen in 7 patients, pancreatic duct dilation was showed in 1 patient. Pseudopapillary structure, hemorrhage, necrosis and cystic degeneration were seen in 18 patients under microscopy. Mesenteric lymph nodule metastasis was showed in 1 patient, vascular invasion in 2, pancreatic tissue invasion in 3 and duodenal invasion in 1 patient. Conclusion: SPTP has characteristic CT features. Non-enhanced and multiphase contrast-enhanced CT scanning are helpul to the diagnosis and differential diagnosis of SPTP.

Abstract:Purpose:To evaluate the relationship between the CT features and pathologic findings of medullary thyroid carcinoma(MTC)and follicular thyroid carcinoma(FTC).Methods:The CT features of 21 patients with 25 MTC lesions and 15 FTC lesions were analyzed retrospectively,the pathologic findings were compared.Results:①The lesion density,border and calcification shape of these two kinds of lesions on CT plain imagings were not different significantly(P=0.444,P=0.139,P=0.325).②On enhenced CT imagings,the lesion border of these two kinds of cancers was different,MTC's(17/22)border was less clear than that of FTC(6/15)(P= 0.038).Compared with the Pathology,MTC general appeared no capsule and invasion,while FTC appeared capsule integrity or merely invaded partly.③The density of 4 MTC and 3 FTC became heterogeneous on postcontrast CT imagings,histologic findings showed massive amyloid deposits in MTC,while a lot of collagen in FTC.④The border features of 4 MTC and 5 FTC on plain and contrast CT were changed,which had relationship with the lesion's capcule and neoplastic capillary vessels.⑤ MTC(18/21)was more easy to have cervical lymph nodes metastases than FTC(1/15),and there was significant difference(P<0.001).Conclusion:Plain and enhanced CT imagings were helpful in the differential diagnosis of MTC and FTC.CT features linked closely to pathologic appearances.
Conventional US, elastography and contrast enhanced US features of papillary thyroid microcarcinoma predict central compartment lymph node metastases

Abstract:Lymph node metastases at the time of diagnosis have a major impact on both therapeutic strategy and tumor recurrence for patients with papillary thyroid microcarcinoma (PTMC). Our objective was to evaluate the usefulness of PTMC characteristics on ultrasonography for predicting central compartment lymph node metastases (CCLNM) of PTMC. One hundred twenty seven patients who underwent surgery for PTMC were enrolled in this study. The relationship between the CCLNM and the characteristics on conventional US, elastographic and contrast enhanced ultrasound (CEUS) were investigated. Univariate analysis indicated that PTMCs with CCLNM were more often nodule irregular shape, microcalcifications, hyperenhancing or isoenhancing parametric maps and peak index ≥1 at preoperative US and CEUS than those without CCLNM ( P < 0.01, 0.05, 0.01 and 0.05 respectively). Multivariate analysis showed that microcalcification (OR:2.378, 95% CI: 1.096–5.158) and hyperenhancement or isoenhancement (OR:2.8, 95% CI: 1.287–6.094) were predictive for the presence of CCLNM. Elastography score was not significantly different between the groups. Our study indicated that preoperative thyroid nodule characteristics on conventional US and CEUS may serve as a useful tool to predict central compartment lymph node metastases in PTMC.
Ultrasound characteristics of medullary thyroid carcinoma

Abstract:Objective:To analyze the ultrasound characteristics of medullary thyroid carcinoma (MTC), in order to provide a reference for the diagnosis and preoperative management of MTC.Methods:Eighty-five MTC patients with complete ultrasound and clinical data were retrospectively selected from the Cancer Hospital of the University of Chinese Academy of Sciences (Zhejiang Cancer Hospital) from October 2009 to February 2020, and 150 PTC patients were selected from October 2019 to February 2020 as a control group. All patients underwent surgical treatment at our hospital and were diagnosed by routine pathology after surgery. Preoperative serum calcitonin test results were collected. The t-test was used to compare the differences in age and nodule size between the two groups; the χ2 test was used to compare the differences in gender, edges, morphology, blood supply, and neck metastatic lymph nodes between the two groups. Factors with a statistically significant difference in univariate analysis were used for multivariate logistic regression analysis.Results:Approximately 97.0% (65/67) of MTC patients had increased serum calcitonin to varying degrees. MTC and PTC were significantly related to gender (male/female: 46/39 vs 46/104, χ2=12.525, P＜0.001), age [(50.11±13.34) years vs (43.63±11.88) years, t=3.838, P＜0.001], and nodule size [(21.66±13.46) mm vs (14.10±9.93) mm, t=4.916, P＜0.001], and the two groups differed significantly in nodule edges (smoothed/unsmoothed: 56/29 vs 19/131, χ2=70.709, P＜0.001), shape (regular/irregular: 34/51 vs 101/49, χ2=16.582, P＜0.001), aspect ratio (＞1/≤1: 10/75 vs 46/104, χ2=10.679, P=0.001), blood supply (low/high: 20/65 vs 65/85, χ2=9.217, P=0.002), and lymph node metastasis (yes/no: 57/28 vs 75/75, χ2=6.413, P=0.011). Multivariate logistic regression analysis showed that smooth tumor edges, irregular shape, and neck lymph node metastasis were independent risk factors (OR=34.786, 95%CI: 12.552-96.405, P＜0.001; OR=0.194, 95%CI: 0.082-0.457, P＜0.001; OR=0.277, 95%CI: 0.109-0.705, P＜0.001).Conclusion:MTC has the characteristics of being a large mass, smooth edges, irregular shape, rich blood supply, aspect ratio ≤1, and being prone to neck lymph node metastasis. Combined serum calcitonin can improve the diagnostic accuracy.
Medullary Thyroid Carcinoma: Evaluation of CT and Pathologic Findings

Abstract:Objective To evaluate the CT features and pathologic findings of MTC. Methods The CT features of 28 patients with MTC confirmed pathologically were analyzed retrospectively and correlated with pathologic findings. Results The tumor invaded one lobe of the thyroid in 22 cases(78.6%), two lobe in 6 cases(21.4%). 5(17.9%) of them presented with heterogeneous hypodense masses and 23(82.1%) of them presented with homogeneous hypodense solitary masses . 20 patients had metastatic lymph nodes. The thyroid masses were enhanced slightly, while the lymph nodes were enhanced obviously. Correlating with pathologic findings, the heterogeneous lesions composed of cystic areas and/or necrosis, while the homogeneous lesions composed of homogeneously distributed tumor cells with no cystic areas or necrosis. The metastatic lymph nodes were hypervascular. Conclusion The CT features of MTC may be described as homogeneous diffuse hypodense mass, with slight enhancement,and invasion of one thyroid lobe in most cases. The metastatic lymph nodes are characterized with enhancement.

Abstract:Objective To compare the ultrasonic features between medullary thyroid carcinoma (MTC) and papillary thyroid carcinoma(PTC). Methods Preoperative sonographic features of 27 patients with MTC (33 nodules) and 187 patients with PTC (200 nodules) were retrospectively reviewed, including position, size, echogenicity, internal structure, border, margin, shape, anteroposterior and transverse diameter ratio (A/T),calcification and blood supply. Results The main sonographic features of MTC nodules were upper middle location, relatively large size, hypoechogenicity or marked hypoechogenicity, solid content, unclear border, non-smooth margin, irregular shape, A/T<1, calcification and rich blood flow. There were significant differences between MTC and PTC in position, size, border, margin, shape, A/T, calcification and blood flow(P<0.05), while no significant differences in echogenicity and internal content ( P>0 . 05 ) . Serum calcitonin tested in 15 patients with MTC increased in varying degrees . Conclusion MTC has both general sonographic features of malignant nodules and specific characteristics. Some atypical MTC was easily misdiagnosed, which should be diagnosed using ultrasonography and serum calcitonin.
Contrast-Enhanced Ultrasound Features In Papillary Thyroid Carcinomas With Different Sizes

Abstract:Objective: This study aims to analyze the differences in the contrast-enhanced ultrasound features between papillary thyroid microcarcinoma (PTMC) and papillary thyroid carcinoma (PTC) (diameter > 10 mm). Methods: A total of 59 thyroid cancer patients with 63 pathologically confirmed thyroid cancer nodules were enrolled in this retrospective study. Contrast-enhanced ultrasound features were evaluated for each nodule. Results: Patients with PTMC more frequently exhibited dotted enhancement, well-defined margins, and hypoenhancement, while patients with PTC (diameter > 10 mm) more often exhibited diffuse heteroenhancement, ill-defined margins, isoenhancement and hypoenhancement (P=0.001, 0.000, and 0.000, respectively). Peak intensity, wash in slope, and area under the curve were significantly higher in PTC (diameter > 10 mm) than in PTMC (P=0.000, 0.000, and 0.005, respectively); rising time, time to peak, and time from peak to one half were significantly shorter in PTC (diameter > 10 mm) than in PTMC (P=0.030, 0.003, and 0.002, respectively). Multivariate stepwise logistic regression analysis revealed that peak intensity, time from peak to one half and wash in slope were the most significantly different parameters (odds ratio =6.392, P=0.001; odds ratio =0.777, P=0.011 and odds ratio =0.021, P=0.049, respectively). Conclusion: The distinct differences of contrast-enhanced ultrasound in PTMC and PTC (diameter > 10 mm) provide important insights into the pathology of them, which is essential for optimal clinical diagnosis and treatment.

Abstract:This study was designed to explore differences in the ultrasonographic characteristics of medullary thyroid carcinoma (MTC) and papillary thyroid carcinoma (PTC). This study included 35 cases of MTC and 96 cases of PTC that were surgically and pathologically confirmed. Preoperative ultrasound images were retrospectively reviewed by two physicians (with 5 years' experience in thyroid ultrasound) under the premise of unknown pathological results. Various ultrasonic features of nodules were assessed objectively. The clinical features of components were determined by other physicians. Age, sex, unilateral or bilateral involvement of thyroid gland, lesion size, margin, shape, echogenicity, calcification, intranodular blood flow, cervical lymph node, and tumor node metastasis (TNM) stage were compared between MTC and PTC groups. Age, sex, involvement of the thyroid gland, margin, and calcification were similar for the MTC and PTC groups. Compared with the PTC group, the lesion size in the MTC group was significantly larger (P < 0.001). A taller-than-wide shape (aspect ratio > 1) was significantly less likely in the MTC group than the PTC group (P < 0.001). A mixed echogenicity was significantly more common in the MTC group than the PTC group (P = 0.003). The MTC group had significantly enhanced intranodular blood flow (P < 0.001). The TNM stage of the MTC group was significantly higher than that of PTC group (P = 0.001). Medullary thyroid carcinomas differ significantly from PTCs in lesion size, shape, echogenicity, and intranodular blood flow.
[CT and Pathological Manifestations of Intraductal Papillary Mucinous Neoplasm of Pancreas: a Comparative Study].

Abstract:OBJECTIVE:To evaluate the diagnostic value of CT in pancreas intraductal papillary mucinous neoplasm (IPMN) by analyzing its CT feature and pathological findings.METHODS:The clinical and CT data was analyzed among 39 patients with IPMN whose diagnosis was confirmed by pathology. The CT manifestations were classified into 3 types: simple main pancreatic duct enlargement; main pancreatic duct enlargement combined with pancreatic cystic lesion; and simple pancreatic cystic lesion. The correlation between the CT types and Takada pathological types (main duct type, branch type, and mixed type) was analyzed. All the cases were pathologically classified into benign and malignant/boundary groups. Statistical tests of the difference of CT features (mural nodule, septa, size, caliber of main pancreatic duct and common bile duct) between the 2 groups were performed.RESULTS:The CT type I matched the main duct type, the CT type II mainly matched the branch type and mixed type, and the CT type III matched the branch type (P < 0.001). The probability of benign lesion was 92% with no mural nodule in the lesion, while the probability of benign lesion was only 42% with mural nodule presented (P = 0.003). In terms of the septa, there was no significant difference between benign and malignant lesions (P = 0.793). The size of malignant/boundary lesions exceeded that of benign lesions (P = 0.016). There were no significant difference in calibers of main pancreatic duct and common bile duct between the benign and malignant/ boundary groups. Without considering pathological grouping the caliber of main pancreatic duct exceeded that of the common bile duct in all the cases (P = 0.02).CONCLUSION:CT typing of IPMN well matches the pathological typing which benefits the CT diagnosis of IPMN. The caliber of main pancreatic duct usually exceeds that of common bile duct in IPMN. This feature contributes to its diagnosis.
Progress of Clinical Pathology and Application of PET-CT on Thyroid Carcinoma

Abstract:OBJECTIVE:Identify the significance of variants in papillary thyroid carcinoma (PTC), the role of extracellular matrix (ECM), MMPs, cell adhesion molecular (CAM) and cytokine in lymphatic metastasis in PTC and the value of PET-CT in diagnosis of thyroid carcinoma.METHODS:Five hundred and five cases of PTC which had complete medical records and followed up surveys were selected from the files. Clinical biological characteristic of the histological variants was investigated. Sixty cases of PTC were selected. As the important parts of micro ecosystems, ECM, MMPs, CAMs and cytokine were investigated in use of tissue chip and method of immunohistochemistry. In addition, a group of cases of thyroid carcinoma including PTC was analyzed, follicular thyroid carcinoma and medulla thyroid carcinoma (MTC) and their reports of PET-CT as the initiation.RESULTS:The variants could be divided into three groups in terms of the rate of cervical lymph node metastasis. The high-metastases group included diffuse sclerosing variant, tall cell variant, column cell variant and diffuse follicular variant. The low-metastases group included macrofollicular variant and papillary microcarcinomas. Others are included in the moderate-metastases group. The rate of cervical lymph node metastasis of each group were 83.0% , 55.5% and 34.1% (P < 0. 05), respectively. The 10-year-survival were 75.3% , 95.8% and 100.0% , respectively. The 20-year-survival were 31.2%, 80.3% and 87.5%, respectively. The positive rate of LN, FN, MMP-2, MMP-9, TIMP-2, CD, Integrinbeta-1, ICAM-1, EGFR, TGFR-beta, VEGF-C and E-Cad in metastasis ranged from 51.6% (Integrinbeta-1) to 98.3% (CD), and that in primartumor ranged from 46.7% (FN) to 98.3% (ICAM-1). The expression of E-cad in primartumor was lower than that in normal tissues (P < 0.05). The sensitivity of PET-CT was 100% and the specificity was 85.7%.CONCLUSIONS:The characteristic has significant difference among the variants. Individual treatment should be performed in terms of different variants. Furthermore, some molecules play an important role in the lymph node metastasis of PTC and may be considered as the focus of future study. In addition, compared with CT and Bus, PET-CT is more sensitive and has its unquestionable advantage as a whole body examination, especially for staging and detecting micro metastases, though it requires a high expense.
A Comparative Study of Ultrasonic Features between Papillary Thyroid Microcarcinoma and non-Microcarcinoma

Abstract:Objective To discuss ultrasound features of papillary thyroid microcarcinoma(PTMC)and non-microcarcinoma(non-PTMC).Methods The ultrasonic features of 253 cases of papillary thyroid carcinoma (PTC),as confirmed by pathology,were retrospectively analyzed.Patients were diagnosed with PTMC if the tumor size was ≤10 mm in diameter and nonPTMC if the tumor size was ＞10 mm in diameter.The shape,boundary,internal and rear echo,ratio of length/width,blood flow,calcification,and lymph node metastasis were compared between the patients with PTMC and non-PTMC.Results In total,266 nodules were identified including 135 PTMC nodules and 131 non-PTMC nodules.While 21 PTMC nodules were missed diagnosed and 7 nodules were misdiagnosed as benign nodules;4 non-PTMC nodules were missed diagnosed and 5 nodules were misdiagnosed as benign nodules.Statistically significant differences in the boundary,ratio of length/width,internal echo,blood flow and lymph node metastasis were observed between the PTMC and non-PTMC groups (P＜0.05).However,no statistically significant differences were observed with regard to the shape,calcification,rear echo between the two groups (P＜0.05).Conclusions The ultrasound image is different between PTMC and non-PTMC.
Study on invasive histopathological features of papillary thyroid carcinoma with tall cell variant

Abstract:Objective To study the invasive histopathological features of papillary thyroid carcinoma(PTC)with tall cell variant(TCV).Methods A retrospective analysis of 19 170 cases of PTC in Ruijin Hospital,Shanghai Jiao Tong University School of Medicine from January 2018 to May 2023 was performed to analyze and the clinicopathological features between TCV-PTC group and classic PTC(cPTC)group.Results Pathological results showed 1 380 cases in TCV-PTC group and 15 578 cases in cPTC group.TCV-PTC had higher proportion and(or)mean value in patients'age,extraglandular invasion,nerve invasion,vascular invasion,maximum diameter of cancer focus,multifocality,lymph node metastasis and BRAF mutation,but the proportion of patients with coexistent Hashimoto's thyroiditis was lower than cPTC,and all results had significant difference(P<0.05).Conclusions Compared with the cPTC,TCV-PTC has stronger local invasive characteristics and lymph node metastasis rate,which provides the basis for the subsequent clinical treatment.
Comparison of the Diagnostic Value of US and MRI for Papillary Thyroid Carcinoma

Abstract:Objective To compare the image features of magnetic resonance imaging (MRI) and ultrasound (US) for papillary thyroid carcinoma(PTC).Methods From January 2015 to April 2016,86 consecutive patients underwent surgery and pathologically confirmed PTCs in the People 's Hospital Affiliated to Jiangsu University were selected.All patients received neck US and MRI examination before thyroid surgery .For each case,the US and MRI features emphasized included the echogenic /signal,margin,shape,anteroposterior to transverse diameter ratio (A/T), microcalcifications and lymph node metastasis .Statistical analysis was performed using the χ2 test.Results In the comparison of US and MRI features,A/T≥1(41.9％in US and 62.8％in MRI) and lymph node metastasis(88.0％in MRI;56.0％in US) demonstrated statistically significant differences (χ2 =7.551,P=0.009;χ2 =6.349,P=0.025),and no significant differences were observed in the margin ,shape(P=0.724,P=0.316).Conclusion The MRI features of PTCs included A/T and lymph node metastasis was superior to US .
Solid Pseudopapillary Tumors of Pancreas: CT Imaging Features and Correlation with Pathological Findings

Abstract:Objective To analyze the CT features of solid pseudopapillary tumor of pancreas(SPTP).Methods Eight patients with SPTP received triple phasic contrast-enhanced CT,and its consistency with pathological findings was analyzed.Results SPTP manifested as a solitary and exogenous mass,with a heterogeneous pattern composed of solid and polycystic parts.The shape of SPTP was spherical in four patients,oval in two patients,and shallow lobulation in another two patients.In contrast-enhanced phase images,the solid parts showed mild to moderate enhancement in both arterial and portal phases.Complete capsule was showed in seven patients,and nodules were found in the wall of mass in five patients.Hemorrhage was seen in two patients and calcification was showed in five patients.The dilation of pancreatic duct was found in one patient,liver metastases was showed in one patient,and the invasion of tumor to posterior gastric wall was found in one patient.Pseudopapillary structure,hemorrhage,necrosis,and cystic degeneration were showed in all patients by histological study.Conclusion SPTP has certain characteristic CT imaging manifestations consistent with histological features,which is helpful for the diagnosis and differential diagnosis of SPTP.
Primary Malignant Lymphoma of the Thyroid:Evaluation with CT and Correlated with Pathologic Findings

Abstract:Objective:To study the CT features and pathologic findings of PMLT.Methods:The CT features of 13 patients with PMLT confirmed pathologically were analyzed retrospectively and correlated with pathologic findings.Results:According to the shape of the masses,the CT appearances were classified into three types:1 patient had a solitary nodule (type 1),9 had larger mass (type 2),which was often homogeneous hypodense,and 3 had permeating sign (type 3),which manifested as hypodense tumor tissue outside the enhanced thyroid outline.Correlating with pathologic findings,the thyroid tissue was permeated by atypical lymphocyte.The blood vessel wall in the mass was also invaded.Conclusion:The CT appearance of PMLT is usually manifested as larger mass,with homogeneous low density and invasion of both lobes.It is characterized by the permeating sign.

Abstract:Objective To study the enhanced patterns of papillary thyroid carcinoma (PTC)on contrast-enhanced ultrasound(CEUS),and explore the relationship between the degree of enhancement and extracapsular invasionand cervical lymph node metastasis of the tumor.Methods Seventy-three PTCs in 61 patients selected for thyroid surgery underwent conventional ultrasound andcontrast-enhanced ultrasound. The enhanced patterns were analyzed.The differences between different degree enhancement and extracapsular invasion and cervical lymph node metastasis of PTCs were compared.Results Seventy-three PTCs enhanced patterns showed that 52(71 .2%)nodules with hypoenhancement,13(1 7.8%)nodules with isoenhancement,8 (1 1 .0%)nodules with hyperenhancement.Twenty-three (44.2%,23/52 )nodules with extracapsular invasion in hypoenhancing patterns,and 1 5 (71 .4%,1 5/21 ) nodules with extracapsular invasion in isoenhancing or hyperenhancing patterns.There were significant differences between them(P <0.05).Twenty-five (48.1 %,25/52 ) nodules with cervical lymph node metastasis in hypoenhancing patterns,and 13 (61 .9%,13/21 ) nodules with cervical lymph node metastasis in isoenhancing or hyperenhancing patterns,there was no significant difference between them(P >0.05 ).Conclusions The degree of enhancement are correlate to invasiveness in PTCs,PTCs with isoenhancement or hyperenhancement patterns were more often with extracapsular invasion.

Abstract:Objective ToexploretheMRIperformanceofpapillarythyroidcarcinomawithhobnailfeatures(HPTC)andthedifferential diagnosisfromclassicpapillarythyroidcarcinoma(CPTC).Methods Dataof93patientswithconfirmedPTCandMRIexamination oneweekbeforesurgerywereretrospectivelycollected.Thedifferencesoftheage,sex,tumorsize,ADCvaluesand MRIfeaturesof HPTCandCPTCweredetermined.Results 10HPTCand78CPTCwereincludedinthisstudy.ThefrequencyofT2WIsignificantly highsignal,segmentationlinearlowsignal,lacesignandgyrus-likestructureofHPTCwassignificantlyhigherthanthatofCPTC (P=0.000).ComparedtoCPTC,HPTCshowedmoreirregularshape(P=0.005),largervolume(P=0.025),moremixedT2WI andDWIsignals(P=0.002,0.028,respectively),higherADCvalues(P=0.019),anddelayedmass-likeenhancement(P=0.041). Conclusion HPTClesionshavelargervolume,irregularshape,mixedT2WIsignals,mixedDWIsignalsandhigherADCvalues. Significantlyhighsignal,segmentationlinearlowsignal,lacesignandgyrus-likestructurearecommonlyobservedonT2WI.Delayed mass-likeenhancementpatternismorecommon.ThesecharacteristicscouldcontributetothediagnosisofHPTC.

Abstract:Objective: To compare CT features and pathologic findings of intrahepatic biliary cystadenoma and cystadenocarcinoma for improving the diagnosis by CT imaging. Methods: Three cases of cystadenocarcinoma and 5 cases of cystadenoma were enrolled. All of the cases were pathologically confirmed and had undergone CT plain and three phase enhancement scanning. Result: All these cases showed round cystic lesion with well-defined border. In the 3 cases of cystadenocarcinoma, 1 case showed intracystic hemorrhage; 2 cases with homogeneous density; 2 cases manifested multi-room with septa; 1 case showed calcifications of capsule wall; 3 cases showed gross nodule on the capsule wall. Of the 5 cases of cystadenoma, cystic fluid displayed homogeneous fluid attenuation; 2 cases had small nodules. After injection of contrast medium, cyst wall and nodules showed continued enhancement. CT findings were similar to that of gross pathology specimen, with a round and clear boundary cystic lesion. Under microscopy, wall of the cyst was lined with a single layer of epithelium. The subepithelial layer showed dense slender spindle stromal cell, with fewer vessels in tumor interstitium. Conclusions: MSCT findings of intrahepatic biliary cystadenoma and cystadenocarcinoma have certain characteristics; CT scan and dynamic enhancement can reflect its pathological features, which will be useful for diagnosis.

Abstract:Objective To compare the ultrasound features of papillary thyroid carcinoma with or without cervical lymph node metastasis (LNM). Methods Patients suspected of thyroid cancer underwent the conventional ultrasound and contrast-enhanced ultrasound (CEUS) examinations. Patients' age,sex,and ultrasound characteristics of lesions were recorded. With the surgical pathology as the golden standard,the ultrasound features were compared between the cervical LNM group and non-LNM group. Results Of 144 patients,51 had cervical LNM and 93 did not. Patients' ages,sex and number of lesions had no significant difference between two groups (all P>0.05). Tumor with LNM had maximum size greater than 0.85 cm,ill-defined margin (P=0.000),irregular shape (P=0.007),internal heterogeneous echogenicity (P=0.007),microcalcification (P=0.020),internal heterogeneous low-enhancement (P=0.002),peripheral non-enhancement ring (P=0.030),and extracapsular extension (P=0.000). Conclusion Conventional ultrasound and CEUS are helpful for predicting the cervical LNM of PTC. CEUS can obtain more accurate diagnostic results for the extracapsular extension,which contributes to the prediction of cervical LNM.

Abstract:Objective:To discuss the ultrasonographic features of papillary thyroid carcinoma(PTC) under the background of Hashimoto's thyroiditis(HT). Methods:The ultrasonographic features of single cancer node in a cohort of 92 cases,which consisted of 35 cases of HT with PTC(HT group) and 57 cases of PTC(non-HT group),were retrospectively viewed and the difference of these ultrasonographic features in two groups were analyzed by means of SPSS 16.0 statistical software in this work. Results:In the HT group,the proportion of heavy calcification in cancer node was 45.71%,which was obviously higher than 17.54% in non-HT group(χ2=8.488,P=0.004,OR=3.958(95%CI: 1.526-10.264)).In non-HT group,cancer node exhibited low echo with micro-calcification.In non-HT group,the proportion of micro-calcification in cancer node was 63.16%,which was obviously higher than 34.29% in HT group(χ2=7.244,P=0.007,OR=1.784(95%CI:1.177-2.703)).Also,the proportion of low echo from cancer node in non-HT group was 86.96%,which was markedly higher than 54.28% in HT group(χ2=11.286,P=0.001,OR=3.257(95%CI: 1.559-6.806)).However,the attributes of cancer node in terms of size,shape,boundary,and CDFI had no statistic differences in two groups(P0.05). Conclusion:Analyses of the ultrasonographic manifestations of HT concomitant with cancer node is helpful for improving the diagnostic rate of thyroid cancer under the HT background.
