Abstract:Purpose: The management of recurrent head and neck cancer pose a challenging problem. To assess the feasibility, efficacy, and morbidity of 125I seed implantation for recurrent head and neck carcinoma. Materials and Methods: Between September 2002 and June 2009, seventeen patients with head and neck carcinoma underwent 125I seed implantation under CT or ultrasound guidance. All patients underwent a detailed tumor volume study using CT scans with 5 mm sections 1 – 2 weeks before seed implantation. Following adequate local anesthesia, thirteen patients underwent seed implantation guided by color Doppler ultrasound and three patients guided by CT scan when needed. Interstitial needles (18 – gauge) were inserted into the tumor in parallel arrays approximately 1 cm apart in GTV with a margin of 0.5 – 1.0 cm, in order to avoid the puncture of large blood vessels or organs. 18 – Gauge needles were kept 1.0 cm apart from blood vessels. 125I seeds were implanted using a Mick applicator with a spacing of approximately 1.0 cm center-to-center. Implant dosimetry was routinely obtained up to 24 h following seed implantation using 5 mm CT sectioning scans to identify the three - dimensional seed placement. The actuarial D90 of the 125I seeds implanted was 90 – 160 Gy (median, 126 Gy). Median followup was 10 months (range, 3–48). Results: Median local control was 16 months, the 1- and 2-yr local control rates were 66.5% and 49.9%, respectively. The 1- and 2-yr survival rates were 51.3% and 38.5%, respectively, with a median survival of 16 months (95% CI, 7.7 – 24.3). None of the patients experienced greater than serious Grade 4 toxicity events. Conclusions: 125I seed implants were feasible and effectively salvage treatment for patients with recurrent head and neck cancers, local control was good, and complications were minimal. Abstract:The recurrent carcinoma has been difficult to manage after surgery and radiotherapy, extensive resection of locally recurrent rectal cancer is associated with significant morbidity and mortality. Re-irradiation, even in combination with chemotherapy has shown very short survival. We assess the feasibility and efficacy of CT-guided interstitial permanent brachytherapy with (125)I or (103)Pd seeds for recurrent rectal cancer after multiple treatments. Fifteen patients with locally recurrent rectal carcinoma received (125)I or (103)Pd seed implants under CT guidance. The minimal peripheral dose of seed implants was 110-165 Gy (median 150 Gy). Two weeks after seed implantation, a 50 Gy of stereotactic radiotherapy was given to one patient; four patients received 2-4 cycles of chemotherapy. A median follow-up was 8 months (range 4-50 months). The duration of pain-free survival was 0-50 months (median 7 months). Local control was maintained for 3-50 months (median 7 months). The 1- and 2-year local controls were 16.2 and 8.1%, respectively. Eleven patients died: two (18.2%) of local recurrence, seven (63.6%) of local recurrence and metastases, and two (18.2%) of metastases. Four patients (26.7%) survived the median survival was 9 months. The 1- and 2-year actuarial overall survival rates were 42.9% and 10.7%, respectively. One patient (7.6%) experienced a grade 4 toxic event; there was no associated neuropathy. CT-guided radioactive seed implantation is feasible and safe as a salvage or palliative pain relief treatment for patients with recurrent rectal cancers after surgery and radiotherapy.

Abstract:To investigate the safety and feasibility of percutaneous (125)I seed implantation for patients with recurrent salivary glands carcinomas. Nineteen (19) patients with recurrent salivary glands carcinomas underwent (125)I seed permanent implantation under ultrasonography or CT guidance and the feasibility and efficacy of (125)I seed implantation were assessed. The postplan evaluation showed that the actuarial D90 of (125)I seeds ranged from 90 to 160 Gy (median, 130 Gy). The activity of each (125)I seed ranged from 0.4 to 0.8 mCi (median, 0.68 mCi). The total number of sources implanted ranged from 3 to 80 (median, 31). The follow-up ranged from 3 to 40 months (median, 8 months). The 1-, 2-year local control rates were 59.5% and 12.4%, respectively, with a median local control of 10 months. The 1-, 2-, 3-and 5 year survival rates were 71.4%, 35.7%, 23.8%, and 23.8%, respectively (median, 10 months). Of the 19 patients, 2 (10.5%) died of local recurrence, and 4 (21.1%) died of metastases; 1 patients died of pneumonia; 1 patient died of heart disease. One (1) developed ulceration with tumor progression. Blood vessel damage and neuropathy were not observed. Percutaneous (125)I seed implantation is a feasible, safe salvage for patients with recurrent salivary glands carcinomas.

Abstract:Twenty-nine (29) patients with recurrent head and neck carcinomas underwent I-125 seed permanent implantation under ultrasonography guidance and the feasibility and efficacy of I-125 seed implantation were assessed. The postplan evaluation showed that the actuarial D90 of I-125 seeds ranged from 90 to 160 Gy (median, 130 Gy). The activity of each I-125 seed ranged from 0.35 to 0.8 mCi (median, 0.6 mCi). The total number of sources implanted ranged from 3 to 61 (median, 22). The follow-up ranged from 3 to 40 months (median, 8 months). The 1-, 2-, and 3-year local control rates were 53.1%, 34.8%, and 17.4%, respectively, with a median local control of 16 months (95% confidence interval, 5.8-26.1). The 1-, 2-, and 3-year survival rates were 54.1%, 27.5%, and 27.5%, respectively (median, 13 months; 95% confidence interval, 6.0-19.9). Of the 25 patients, 5 (17.2%) died of local recurrence and 7 (24.1%) died of metastases; 2 patients showed recurrences at 3 and 8 months after seed implantation and subsequently died of pneumonia. One (1) patient died of heart disease. One (1) developed ulceration with tumor progression. Blood vessel damage and neuropathy were not observed. Percutaneous ultrasound-guided I-125 seed implantation is a feasible, safe salvage for patients with recurrent carcinomas of the head and neck.
Salvage 125I Brachytherapy for Liver Metastases of Colorectal Cancer in Anatomically Challenging Locations after Failure of Systemic Chemotherapy-a Retrospective Study.

Abstract:PURPSOE: Colorectal cancer liver metastasis (CCLM) in anatomically challenging locations is difficult to treat. This retrospective study aimed to evaluate the effectiveness and safety of permanent 125 I seeds implantation (ISI) for treatment of CCLM in anatomically challenging locations after failure of systemic chemotherapy. METHODS AND MATERIALS: A total of 31 liver metastases (in 25 patients) were treated by ISI under computerized tomography guidance from January 2011 to December 2017. Post-treatment follow-up was for 40 months. Adverse events were classified using the National Cancer Institute's Common Terminology Criteria for Adverse Events. Tumor response was evaluated by the mRECIST criteria. Objective response rate, overall survival rate, and complications were retrospectively analyzed. RESULTS: All ISI procedures were performed successfully. Most patients only complained of fatigue and mild pain after ISI. Only one patient had liver rupture during the procedure. Serum alanine aminotransferase and aspartate aminotransferase levels at 1 month after ISI were not significantly different from pre-procedure levels ( p > 0.05). Computed tomography at 6 months after ISI treatment showed completed response in 11 (11/31, 35.5%) lesions, partial response in 14 (14/31, 45.2%) lesions, stable disease in 4 (4/31, 12.9%) lesions, and disease progression in 2 (2/31, 6.5%) lesions; thus, the objective response rate was 80.6%. Median survival was for 12 months. The 1 and 2 year overall survival rates were 52.0% and 20.0%, respectively. CONCLUSIONS: 125 I seeds implantation for CCLM in anatomically challenging locations is safe and effective. Survival benefit is limited in the salvage setting where patients have high intrahepatic tumor load after failed systemic chemotherapy. (C) 2022 American Brachytherapy Society. Abstract:Objective: To investigate the efficacy of CT-guidance interstitial Iodine-125 seed brachytherapy as a salvage therapy for recurrent head and neck carcinoma. Methods: A total of 122 patients who had been treated for recurrent head and neck carcinoma with CT-guided Iodine-125 seed permanent implantation were conducted a retrospective analysis at Peking University Third Hospital from February 2003 to December 2015. The study included 78 male and 44 female patients. Of the 122 patients, 76 had undergone radical surgery, 106 had received EBRT. Among the patients who underwent EBRT, the total dose delivered to PTV ranged from 30 to 140 Gy (median, 68.4 Gy). The actuarial median number of the implanted Iodine-125 seeds was 38 (range, 5-158). The specific activity of Iodine-125 seeds ranged from 14.8 to 28.9 MBq/seed (median, 22.2 MBq). The evaluation of post plan showed the actuarial D90 ranged from 46 to 282 Gy (median, 121 Gy). The overall local control and survival times were determined by using the Kaplan-Meier method from SPSS 13.0.Univariate analysis was performed on the local control rate and overall survival rate. Results: Tumor responses rate was 75.4%. The median local control time was 10.0 months (95% CI 9.8-24.2 months), and the 1-, 2-, 3-, and 5-year local control were 41.9%, 21.2%, 3.7%, and 3.7%, respectively. Univariate analysis showed that the local control in D90≥120 Gy group had an increasing tendency, but no statistical difference were found. The effect of local control in the squamous cell carcinoma group was slightly worse than that in the non-squamous cell carcinoma group (P=0.032). Multi-factor analysis showed that the effect of local control in the squamous cell carcinoma group was slightly poor (P=0.03). The median survival time was 14 months (95% CI 14.4-35.8 months), and the 1-, 2-, 3- and 5-year survival rate were 51.5%, 34.2%, 19.4%, and 19.4%, respectively. The three factors, such as the tumor responses, KPS status before the seed implantation, and the D90 after the seed implantation, had a tendency to improve the total survival, but there was still no statistical differences. Multivariate analysis showed no clear influence factors. Conclusions: Interstitial permanent Iodine-125 seed implantation is an effective salvage re-irradiation modality for recurrent head and neck carcinoma after previous surgery and/or EBRT. CT image-guided method could yield the reliable seeds configuration and accurate dose distribution.

Abstract:Purpose The present study aimed to explore the safety profile and clinical efficacy of CT-guided radioactive seed implantation in treating local recurrent rectal carcinoma. Materials and methods CT-guided 125 I seed implantation was carried out in 20 patients with locally recurrent rectal carcinoma. 14 of the 20 patient had prior adjuvant external-beam radiation therapy (EBRT). The treatment planning system (TPS) was used preoperatively to reconstruct three dimensional images of the tumor and to calculate the estimated seed number and distribution. The median matched peripheral dose (MPD) was 120 Gy (range, 100-160 Gy). Results Of the 20 patients, 12 were male, 8 were female, and ages ranged from 38 to 78, with a median age of 62. Duration of follow-up was 3-34 months. The response rate of pain relief was 85% (17/20). Repeat CT scan 2 months following the procedure revealed complete response (CR) of the tumor in 2 patients, partial response (PR) in 13 patients, stable disease (SD) in 3 patients, and progressive disease (PD) in 2 patients. 75% of patients had either CR or PR. Median survival time was 18.8 months (95% CI: 3.5-22.4 months). 1 and 2 year survival rates were 75% and 25%, respectively. 4 patients died of recurrent tumor; 4 patients died of distant metastases; 9 patients died of recurrent tumor and distant metastases. 3 patients survived after 2 year follow up. Two patients were found to have mild hematochezia, which was reversible with symptomatic management. Conclusion CT-guided 125 I seed implantation appeared to be a safe, useful and less complicated interventional treatment option for local recurrent rectal carcinoma.

Abstract:Objective To assess the value of interstitial ~(125)I seed implantation for metastatic and re- current bone neoplasms.Methods From 2002 to 2004,14 patients with metastatic or recurrent bone neo- plasms were so treated.The number of ~(125)I seeds implantated ranged from 3-145 with radioactivity varying from 18.5-29.6 MBq.Matched peripheral dose (MPD) of ~(125) I seed implantation were 115 -145 Gy.The treat- ment quality was validated after ~(125)I seed implantation.Results The median follow-up time was 12.4 months(range 7-29).The mean D_(90) to the tumor was 108.12 Gy(range 27-166 Gy),with 31.9 Gy(range 6.2-70.0 Gy) in 10 patients with spinal column or paraspriral lesions.The responsive rate of pain relief was 92%.70% of the above 10 patients enjoyed either normal or improved ambulation.The 1-year local control rate was 82%,1-year overall survival rate was 53%.No complications were observed during the periperative period.Conclusion Interstitial ~(125)I seed implantation is safe and effective in the treatment of metastatic or recurrent bone neoplasms.
An Investigation of 125 I Seed Permanent Implantation for Recurrent Carcinoma in the Head and Neck after Surgery and External Beam Radiotherapy

Abstract:Background A preliminary assessment was conducted of the feasibility, efficacy, and morbidity of 125 I seed implantation for recurrent head and neck carcinoma after surgery and external beam radiotherapy. Methods Nineteen patients with recurrent head and neck carcinomas underwent 125 I seed implantation under ultrasound or computed tomography guidance. The actuarial D90 of 125 I seed implantation ranged from 90 to 160 Gy (median, 131 Gy). The follow-up period ranged from 3 to 44 months (median, 11 months). Results The median local control was 24 months (95% confidence interval, 10.2 to 37.8). The one- year, two-year and three-year local controls were 73.3%, 27.5% and 27.5%, respectively, whereas the one-year, two-year and three-year survival rates were 53.0%, 18.2% and 18.2%, respectively, and the median survival was 13 months (95% confidence interval, 6.6 to 19.4). A total of 26.3% of patients (5/19) died of local recurrence and 21.1% of patients (4/19) died of metastases. One suffered from a grade 1 skin reaction. Conclusions 125 I seed implantation is feasible and safe as a salvage treatment for patients with recurrent head and neck cancers. The high local control results and low morbidity merits further investigation.
CT-guided Permanent 125I Seed Interstitial Brachytherapy for Recurrent Retroperitoneal Lymph Node Metastases after External Beam Radiotherapy

Abstract:PURPOSE: To evaluate the feasibility, efficacy, and safety of permanent I-125 seed interstitial brachytherapy reirradiation in patients with retroperitoneal lymph node recurrence under CT guidance.METHODS AND MATERIALS: Seventeen patients with 19 retroperitoneal lymph node recurrence after external beam radiotherapy underwent CT-guided I-125 seed implant brachytherapy from October 2007 to August 2014. Treatment for all patients was preplanned using a three-dimensional radiation therapy planning system 3-5 days before brachytherapy; dosimetry verification was performed immediately after brachytherapy.RESULTS: The actuarial D90 (dose delivered to 90% of the target volume) was 100-198 Gy (median, 126.5 Gy). In 9 patients, pain intensity decreased to mild pain 1-3 weeks after brachytherapy. Pain-free survival ranged 2-15 months (median, 5 months; 95% confidence interval [CI]: 0.1, 9.9). The overall response rate was 19 of 19 (100%). The median local control time was 15 months (95% CI: 2.3, 27.7). The 6-, 12-, and 24-month local control rate was 88.0%, 63.2%, and 42.1%, respectively. Twelve patients (70.6%) developed distant metastases and died. Two patients (11.8%) are alive with distant metastases but no evidence of local recurrence. Three patients (17.6%) are alive with no evidence of local recurrence. Median overall survival was 10 months (95% CI: 5.7, 14.3); the 1- and 2-year survival rates were 38.1% and 15.3%, respectively. No major complications related to the procedure occurred during or after brachytherapy.CONCLUSIONS: Reirradiation with CT-guided permanent I-125 seed interstitial brachytherapy is feasible, safe, and effective as pain relief or salvage treatment for patients with recurrent retroperitoneal lymph nodes. Abstract:Background The management of pediatric recurrent or metastatic soft tissue sarcoma after multimodal treatment remains challenging. We investigated the feasibility, efficacy, and morbidity of permanent interstitial 125 I seed implantation under image guidance as a salvage treatment for pediatric patients with recurrent or metastatic soft tissue sarcoma. Methods This was a retrospective study of 10 patients who underwent percutaneous ultrasound or computed tomography (CT) guided permanent 125 I seed implantation. Postoperative dosimetry was performed for all patients. Actuarial D90 was 121–187.1 Gy (median, 170.3 Gy). The number of 125 I seeds implanted was 6–158 (median, 34.5), with a median specific activity of 0.7 mCi per seed (range, 0.62–0.8 mCi); total activity was 4.2–113.76 mCi. Follow-up time was 6–107 months (median, 27.5 months); no patients were lost to follow-up. Results The overall response rate (complete response + partial response) was 8/10 (80 %), including two patients with complete response (CR) (20 %) and five patients with partial response (PR) (60 %). Local control rates after 1 and 2 years were 70.1 and 62.3 %, respectively, with a mean local control time of 70.6 months (95 % confidence interval (CI) 45.1–96.0). Survival rates after 1 and 2 years were 68.6 and 57.1 %, respectively, with a mean survival time of 65.3 months (95 % CI 34.1–96.5). Three patients died from distant metastasis; one died from local recurrence 12 months after seed implantation. Three patients suffered a grade I skin reaction and one developed ulceration. No severe adverse neurologic sequelae or blood vessel damage occurred. Conclusions Image guided permanent interstitial 125 I seed implantation as a salvage treatment appears to have a satisfactory outcome in children with recurrent or metastatic soft tissue sarcoma.

Abstract:Objective To evaluate the efficacy and adverse effects of 125I seeds implantation for pelvic recurrence of cervical cancer (PRCC) after radiotherapy.Methods From July 2005 to October 2015,36 PRCC patients (median 44 years) after radiotherapy in Peking University Third Hospital were enrolled in this retrospective study.All patients underwent 125I seeds implantation under ultrasound or CT guidance.Treatment planning was performed before implantation to estimate the number and activity of 125I seeds.The seed numbers ranged from 10-140 (median:62.5),and the activity ranged from 18.5-29.6 (median:25.9) MBq.Postoperatively,the median dose delivered to 90％ gross tumor volume (D90) was 127.3 Gy.Kaplan-Meier method was used to calculate local progress free survival (LPFS) rate and overall survival (OS) rate,and log rank test and Cox regression were used for univariate and multivariate analyses.Results The median follow-up time was 11.5 months.The local control rate was 88.89％(32/36).The 1-year LPFS rate was 34.9％ and the 1-year OS rate was 52.0％.Thirty-one patients died,of which 22 (70.97％,22/31) died from cancer.Univariate analysis showed that the location of recurrence (x2=5.195),volume of lesion (hazard ratio (HR)=1.012) and D90(HR=0.988) were significantly correlated with LPFS (all P＜0.05).Multivariate analysis showed that the location of the recurrence was significantly related with LPFS (HR =0.215,P＜0.05).The 1-year LPFS rates of pelvic wall recurrence and central recurrence were 41.6％ and 26.7％ (x2 =5.195,P＜0.05),and 1-year OS rates were 54.7％ and 49.5％ (x2 =2.535,P＞0.05),respectively.Vaginal fistula,which may be caused by the treatment,occurred in 1 case.No other sever adverse effects were observed.Conclusions 125I seeds implantation is a safe and effective treatment for PRCC after radiotherapy.With the treatment of 125I seeds implantation,patients with pelvic wall recurrence may achieve better therapeutic effects than those with central recurrence.

Abstract:To evaluate the efficacy and safety of 125I seeds implantation for refractory chest wall (CW) metastasis or recurrence under CT guidance. In addition we assessed initial data obtained on the therapeutic response for refractory CW metastasis or recurrence. Twenty consecutive patients underwent permanent implantation of 125I seeds (from Jul. 2004 to Jan. 2011) under computed tomography (CT) guidance. Postoperative dosimetry was routinely performed for all patients. The actuarial D90 of the implanted 125I seeds ranged from 100 Gy to 160 Gy (median: 130 Gy). The activity of 125I seeds ranged from 0.5 mCi to 0.78 mCi (median: 0.71 mCi). The total number of seeds implanted ranged from 8 to 269 (median: 53). The follow-up period ranged from 3 to 54 months (median: 11.5 months). The survival and local control probabilities were calculated by the Kaplan-Meier method. Among all the 20 patients, 3 patients had complete remission CR (15%), 12 patients had partial remission PR (60%), 5 patients had stable disease SD. The 1-, 2-, 3- and 4-year tumor control rates were all 88.7% respectively. The 1- and 2-, 3-, 4-year cancer specific survival rates were 56.5% and 47.1%, 47.1%, 47.1% respectively. The 1- and 2-, 3-, 4-year overall survival rates were 53.3% and 35.6%, 35.6%, 35.6% respectively, with a median survival of 15 months (95% CI, 7.0-22.9). Mild brachial plexus injury was seen in one patient; grade 1 or 2 skin reactions were seen in 6 patients (30%) who had received external beam radiation therapy (EBRT) before. No grade 3 and 4 skin side effects were found. Rib fracture, ulceration, pneumothorax or hemopneumothorax were not seen. Interstitial permanent implantation of 125I seeds under CT guidance is feasible, efficacious and safe for refractory CW metastasis or recurrence.
Interstitial implantation of ~(125)I seeds for the treatment of soft tissue tumors

Abstract:Objective To investigate the efficacy and safety of ~(125)I seeds interstitial implantation for soft tissue tumors. Methods Interstitial implantation of ~(125)I seeds was performed on 12 patients with soft tissue tumors (15 lesions including 2 primary lesions and 13 local recurrent or metastatic lesions). A mean of 60 (7-80) seeds were implanted into each lesion. The radioactivity of the ~(125)I seeds ranged from 0.5 to 0.8 mCi (mean, 0.6 mCi), and the matched peripheral doses (MPD) was 140-160 Gy. Quality check was carried out after the implantation. And then, supplemental radiation therapy was performed on 2 lesions with a dosage of 30 and 60 Gy respectively (2.0 Gy/f). Results The patients were followed up for a median of 17 months (3-34 months). The rate of pain relief was 83%(5/6); actual clinical effective rate (CR+PR+SD>2 months) was 93%(14/15); both 1- and 2-year local control rates were 83%; and 1- and 2-year survival rates were 72.9% and 62.5%, respectively. The median survival time was 32 months. Three patients had distant metastasis. Five patients died of lung metastasis (1 case) or multiple organ failure (4 cases). No complication or side-effect was observed in the patients, except skin ulceration in one patient. Conclusions Interstitial implantation of ~(125)I seed combined with or without external beam radiation therapy is convenient, effective, safe, and minimally invasive for soft tissue tumor, especially for metastatic or recurrent tumors.

Abstract:OBJECTIVE:To observe the effect of 125I seeds implants in advanced head and neck cancer.METHOD:Treatment fields were designed to encompass the tumor/edema volume with a 2-3 cm margin, based on assessment of diagnostic CT images, 125I seeds were inserted into there and permanent left there for brachytherapy in 5 patients with advanced head and neck cancer including recurrent oropharyngeal cancer, supraglottic cancer and occult cancer.RESULT:Local tumor were controlled evidently and the patients suffering were alleviated, but 1 case occurred skin necrosis and 1 case occurred skin necrosis combined with internal carotid rupture.CONCLUSION:Permanent 125I implants offer useful pallition for patients with advanced recurrent head and neck cancer but need prevention fatal complication.

Abstract:To evaluate the response rate, local control, overall survival, pain control rate and treatment-related toxicity of CT-guided radioactive seed implantation for locally recurrent rectal cancer (LRRC). From September 2003 to October 2011, thirty-one patients with locally recurrent rectal cancer received 125I or 103Pd seeds implantation under CT guidance in our center. Each patient underwent three-dimentional treatment planning pre-implantation and dosimetric verification post-implantion. The range of activity of seed was from 0.40 to 1.40 mCi, and the range of seeds number was from 33 to 137. The range of D90% was from 75.91 to 159.32 Gy. The followup rate was 93.5%, the median followup time was 15.7 months (4.2 ∼ 98.1 months). The response rate of pain relief was 95.2%. The overall response rate was 51.6%, in which complete response rate was 16.1% and partial response rate was 35.5%. The 1-, 2- and 3-year local control rates were 32.3%, 11.3% and 11.3%, respectively. The median local control survival was 8.0 month. The 1-, 2- and 3-year survival rates were 67.6%, 36.0% and 7.5%, respectively. The median overall survival was 21.5 months. Six patients were observed complications. CT-guided radioactive seed implantation is a minimally invasive treatment for locally recurrent rectal cancer with satisfied efficacy and tolerable complications. It is another treatment option for locally recurrent rectal cancer, especially for those with previous pelvic radiation. These findings need to be validated by conducting further studies with larger cohorts.

Abstract:Objective To investigate the technological feasibility,efficacy and morbidity of cr guided ~(125)Ⅰseed implantation for recurrent rectum cancer.Methods Twenty-three patients with recurrent rectum cancer were treated with cr guided interstitial ~(125)Ⅰseed brachytherapy.In 20 patients the procedure was performed under epidural anesthesia and 3 patients under local anesthesia.Treatment planning system was used to calculate the number of seeds,the space distribution and the introduction of the seeding needles.Matched peripheral dose (MPD) of ~(125)Ⅰseed implantation ranged from 90-120 Gy for patients who had had external radiotherapy,and 140- 160Gy for those who had not.The planning target volume(PTV)was clinical target volume(CTV)plus 1 cm margin.The range of radioactivity of the ~(125)Ⅰseeds was 18.5-25.9 MBq.All these 23 patients had CT scan at 5 mm intervals after implantation for quality evaluation,together with routine chest,pelvic X-ray films within 24-48 hours after seed implantation.Three patients received three-dimensioual conformal radiation therapy(3DCRT) to a total dose of45-50 Gy,with 2-3 Gy/f.Follow-up time was from 3 to 28 months.Results All patients was able to tolerate seed implantation well.Complete pain relief was observed in 12/15,and partial relief in 2/15 and no response in 1/15,with a response rate of 93%.The local control rate was 87%.The 1-and 2-year survival rate was 93% and 50% respectively.Two of four patients have died of dissemination to the lung after 8 and 12 months. One seed has migrated into the pelvis without causing any untoward morbidity.Conclusion CT guided ~(125)Ⅰseed implantation for recurrent rectum cancer is safe,minimally invasive,causing only mild morbidity.It possesses a high efficacy,yet it should be given in combination with external beam radiation and chemotherapy,should distant metastasis be observed.

Abstract:To assess the feasibility, efficacy, and morbidity of CT image-guided radioactive iodine 125 (125I) seed implantation for in-field recurrence of NSCLC with prior radiation. 14 patients received CT-guided percutaneous permanent implantation of 125I seeds for in-field recurrence of NSCLC between October 2010 and August 2014. All patients received radiotherapy before seed implantation. The tumor volumes were measured using CT scans at 5-mm intervals one week before seed implantation. Postoperative dosimetry was routinely performed for all patients. The D90 (the doses delivered to 90% of the target volume defined by CT using dose-volume histogram) of the implanted 125I seeds ranged from 83Gy to 224Gy (median: 111 Gy). The activity of 125I seeds ranged from 0.58mCi to 0.75mCi (median: 0.72mCi). The total number of seeds implanted ranged from 16 to 61 (median: 39). The survival and local control probabilities were calculated by the Kaplan-Meier method. The median local control was 7months (95% CI, 4.26-9.74 months). The 1- and 2-year local control rates were 33.3%, and 33.3%, respectively. The estimate median overall survival is 18 months (95% CI, 11.91-24.09 months). The 1- and 2-year overall survival rates were 67.5%, 27.0% respectively. 71.4% of the symptoms (10/14) had disappeared or improved. No patients had radiation pneumonia, or radiation esophagitis in follow-up. One patient developed lethal hemoptysis 18 months after seed implantation. Interstitial permanent implantation of 125-I seeds under CT guidance is feasible, efficacy and safe method for in-field recurrence of NSCLC with prior radiation.
