Abstract:A 69-year-old asymptomatic man was found to have an unruptured aneurysm of the right coronary sinus of Valsalva with the presence of an intraluminal thrombus. The diagnosis was made by computed tomography and by transthoracic echocardiography. Through an aortic and right ventricular approach, we successfully removed the thrombus and repaired the aneurysm of the sinus of Valsalva.

Abstract:A 61-year-old Chinese man presented with a nearly 30-year history of an anomalous origin of the left coronary artery. He had been diagnosed with an anomalous origin of the left coronary artery in 1989. He then underwent regular echocardiographic examinations and it was found that his heart was gradually enlarging. After a >20-year asymptomatic period, he developed recurrent chest discomfort and palpitation. Coronary computed tomography angiography suggested that the left coronary artery anomaly originated from the pulmonary artery; additionally, the right coronary artery was tortuous and thickened. Coronary angiography showed that the right coronary artery was huge and buckling. The patient underwent corrective surgery of the anomalous origin of the left coronary artery from the pulmonary artery, aortic valve mechanical valve replacement, mitral valve plasty, and tricuspid valve plasty in Fuwai Hospital (National Center of Cardiovascular Disease of China), and the anatomic results of the surgery were good.

Abstract:Background: Anomalous origin of the right coronary artery from the pulmonary artery (ARCAPA) is a rare congenital heart disease affecting about 0.002% of the population. Knowledge of ARCAPA is almost collected from case reports. The aim of this study was to provide a rare case to better understand this rare congenital coronary anomaly.Case presentation: We report a rare case of an 18-year-old male who was initially referred because of heart murmur. Dilated and tortuous coronary arteries were detected by echocardiography and congenital coronary anomaly was suggested. Further coronary CT angiography confirmed the diagnosis of ARCAPA. Although dual coronary system provides favorable long-term outcome, bypass surgery was considered technically difficult due to the huge mismatch of caliber between the right coronary artery and graft vessels. Eventually, simple right coronary artery ligation was performed. The patient was followed up for about 5 years without evidence of atherosclerosis or myocardial ischemia.Conclusions: ARCAPA presents as a rare congenital heart disease with variable clinical manifestations. Surgical treatment is highly recommended to re-establish dual coronary system and prevent further complications. To our best knowledge, only about 200 cases of ARCAPA has been reported.
Identification of Anomalous Origin of Coronary Arteriesin Adults with 64-Slice Spiral CT

Abstract:Objective To summarize the appearance of anomalous origination of coronary arteries performed with 64-slice spiral CT. Methods From May 1, 2005 to May 1, 2006, 2 274 cases who underwent CT angiography were analyzed retrospectively. Coronaries with anomaly origins were demonstrated in 24 cases (1.06%, 22 males and 2 females, average age 52.8 years old with range 36-79 years old). Multi-planar reconstruction (MPR), maximum intensity projection (MIP) and volume rendering (VR) were used to demonstrate the origin and course of the coronary arteries. Results The coronary anomalous originations included 5 cases of right coronary arteries (RCA) with high location in right sinus, and 6 cases of RCA from the left coronary sinus. High locations of left main () segments from left sinus of Valsalva were seen in 4 cases, and 2 cases from posterior left sinus. Five cases demonstrated separate origins of the left circumflex (LCx) and left anterior descending (LAD) from the left sinus of Valsalva. One LCx artery origin from the right sinus of Valsalva was shown, while one case of shared a common trunk with RCA. Conclusion The coronary anomalous origins and courses could be demonstrated clearly on 64-slice CT angiography, which is valuable to cardiologists.

Abstract:Purpose: The purpose of this study was to assess the clinical value of dual-source computed tomography (DSCT) in the detection and dynamic evaluation of the anomalous origin of the right coronary artery originating from the left coronary sinus of Valsalva (ARCAOLS) with an interarterial course. Materials and Methods: Thirteen patients with ARCAOLS with an interarterial course and 11 controls with normal origin of the right coronary artery (RCA) were included into this study from December 2006 to April 2008. The origin and course of the RCA were determined on maximum intensity projection, volume rendering, and virtual endoscopy images. The diameter of the proximal segment of the RCA was measured dynamically in all phases of the cardiac cycle; the systolic stenostic rate of the proximal RCA was computed. The angle between the aorta and the proximal segment of RCA was also measured. Results: The average heart rate was 71.92 and 70.76 beats/min for patients and controls, respectively. The systolic image quality score of proximal RCA was 4, whereas the diastolic image quality score of proximal RCA was 3.82. Reformatted images could clearly display the ARCAOLS with a smaller orifice than that of the left coronary artery, coursing in an acute angle between the ascending aorta and the pulmonary artery trunk (P = 0.000). In addition to 2 cases without complete computed tomography data, 11 other cases had complete data being used for dynamic assessment. The average systolic diameter of the proximal RCA in mean (SD) was 1.76 mm (0.54 mm), whereas the diastolic diameter was 2.13 mm (0.62 mm) (P = 0.004) for patients with a stenostic rate of the proximal RCA of 16.83 (13.47). The average systolic diameter of the proximal RCA was 3.49 mm (0.61 mm), whereas the diastolic diameter was 3.78 mm (0.63 mm) (P = 0.000) for 11 controls. Conclusions: Compared with normal controls, the patients with ARCAOLS had a smaller orifice, an acute angle between the aorta and the RCA, and a narrower diameter of the proximal RCA. Dual-source computed tomography can clearly show the anomalous origin, orifice, angle, and course of RCA and dynamically evaluate the diameter changes of proximal RCA during the cardiac cycle, providing useful clues to clarify the mechanism of myocardial ischemia.

Abstract:Objective: To evaluate the diagnostic value of parasternal pulmonary artery (PA) short-axis view for the anomalous origin of left coronary artery (LCA) from the pulmonary artery by echocardiography.Methods: A total of 13 patients (3 boys) aged from 2 months to 12 years were enrolled. Transthoracic echocardiography, including cross-sectional imaging and color Doppler flow imaging, were performed, and their diagnoses were confirmed by operation.Results: Among the 13 patients, 7 had LCA originated from the left posterior wall of PA, 2 from the posterior wall, and 4 from the right posterior wall. The PA short-axis view could visualize the anomalous origin of the LCA from left posterior or posterior wall of PA clearly. The LCA and aortic wall were overlapping at 3-4 o'clock at PA short-axis view in the patients with LCA originating from the right posterior wall of PA. It was similar with the image of the LCA originating from the aorta. But the blood flow was opposite to that of LCA with normal origin.Conclusions: The parasternal PA short-axis view is a good view to visualize the anomalous origin of the LCA. Abstract:Background and Objective. Dual-source CT (DSCT) has been used to detect coronary artery anomalies. The purpose of this study was to assess the incidence of anomalous origin of the coronary artery in Chinese adults. Methods. We summarised all patients who underwent DSCT coronary angiography (CTCA) from December 2006 to February 2008, and data of anomalous origin of the coronary artery in Chinese adults were recorded. Results. 1879 patients underwent CTCA during that period; 24 patients with an anomalous origin of the coronary artery were detected, giving an incidence of 1.3%. Fifteen patients had an anomalous origin of the right coronary artery (12 from left coronary sinus, 3 high takeoff), eight patients had an anomalous origin of the left coronary artery (LCA from posterior sinus of Valsalva in three cases, LCX from the right coronary sinus, LCX from RCA, high takeoff, LCA from right coronary sinus, and single coronary artery in one case, respectively), and one patient had an anomalous origin of both coronary arteries (high takeoff). Conclusion. The incidence of anomalous origin of the coronary artery in Chinese adults in this study is 1.3%. DSCT can clearly visualise the anomalous origin and course of the coronary artery and is a useful screening modality. (Neth Heart J 2010;18:466-70.)

Abstract:Anomalous aortic origin of a coronary artery (AAOCA) from the inappropriate sinus of Valsalva has received increasing attention due to its association with malignant cardiac events in otherwise healthy and asymptomatic individuals. Despite this, we still face tremendous challenges in detecting the anomalies in a timely fashion to prevent sudden cardiac death. Here, we present two cases of anomalous left coronary artery with single ostium (shared with right coronary artery); in patients with normal cardiac function. The diagnosis of AAOCA in both cases was first suspected on transthoracic echocardiography and was later established using either coronary computed tomography angiography or invasive coronary angiography. Although angiography is the gold standard for diagnosing AAOCA, echocardiography is a more frequently used imaging modality in the workup of cardiac patients.

Abstract:Objective To discuss the diagnostic value of dual-source CT(DSCT) angiography on detecting coronary anatomy and anomalies.Methods The coronary artery imaging data of 638 patients who underwent DSCT angiography were reviewed from May 2009 to October 2009.Results For 638 cases,left-side dominance type was about 2.8%(18 cases),right-side dominance type 89.0%(568 cases),balanced type 8.2%(52 cases).The total of coronary anomalies was about 9.4%(60 cases),which included myocardial bridging 32 cases,RCA high takeoff 3 cases,left coronary artery high takeoff 8 cases,accessory coronary artery 8 cases,absence of LM trunk 2 cases,single coronary artery 2 cases,coronary aneurysm of LCX 1 case,coronary fistula between noncoronary sinus and left atrium 1 case,LCX from RCA 1 case,and RCA from left sinus 1 case.Conclusions The DSCT can be used to observe coronary anatomy and anomalies accurately,which is a valuable skill to screen early coronary anomalies.

Abstract:Objective:The purpose of this study was to assess the clinical value of dual-source CT(DSCT) in detecting anomalous origin of coronary artery for adult patients without congenital heart disease(CHD).Methods:We summarized all patients who underwent DSCT coronary angiography from Sep 2008 to Feb 2009,and data of anomalous origin of coronary artery for adult patients without CHD were recorded.Results:1879 patients has no CHD in 1881 patients underwent DSCT coronary angiography during the duration,24 patients with anomalous origin of coronary artery were detected,giving the incidence of 1.3%.15 patients had anomalous origin of right coronary artery(12 from left coronary sinus,3 high take off),8 patients had anomalous origin of left coronary artery(LCA from noncoronary sinus in 3 cases,LCX from left coronary sinus,LCX from RCA,high take off,LCA from right coronary siuns,and single coronary artery in one case,respectively),1 patient had anomalous origin of both coronary arteries(high take off).Conclusion:The incidence of anomalous origin of coronary artery for adult patients without CHD in this study is 1.3%.DSCT can clearly visulaize anomalous origin and course of coronary artery as a useful screening modality.

Abstract:Objective:To explore the angiographic distribution of the anomalous ostium of right coronary artery (RCA) and improve the technical tips for catheter incubation during right coronary angiography.Methods:A total of 24 861 patients who underwent coronary angiography in Zhongshan Hospital,Fudan University from May 2015 to May 2017 were reviewed,and the cases with anomalous RCA origin were identified.The aortic root at left anterior oblique 40°-45° view during coronary angiography was set as an imaginary plane,which was divided into four quadrants.The ostia distributions of anomalous RCAs,the image features and the technical essentials were analyzed.Results:Totally 147 cases had anomalous RCA origin and all underwent coronary angiography successfully.Among them,67 patients (45.6％,Type A) had an ectopic RCA origin from left sinus of valsalva (LSOV),48 patients (32.7％,Type B) had an anomalous RCA origin from left side of ascending aorta,and 32 patients (21.7％,Type C) had an anomalous RCA origin from right side of ascending aorta.In angiography image,some special signs named as "sea gull sign" and "horn sign" were observed in cases with anomalous RCA origin from LSOV or aortic sinus crest.Such as transposition of the heart and widened aortic root,the anomalous RCA originated from ascending aorta near coronary sinus in angiography image were actually originated from LSOV in some cases.Conclusions:The anomalous RCA arising from LSOV is the most common variation,followed by arising from ascending aorta.The RCA arising from ascending aorta above right coronary sinus is relatively rare.Understanding of the distribution regularity and image features of ectopic RCAs is helpful for coronary angiography and intervention.

Abstract:A 36-year-old female patient was admitted with 1-hour history of chest pain and short of breath. Electrocardiogram displayed dynamic ST segment depression and T-wave inversion in precordial leads (V1–V6) (Fig. 1, panels A and B), and the serum level of cardiac troponin T was significantly elevated. Coronary angiography showed mild stenosis of left main coronary artery (panel C, arrowed) and normal left anterior descending, circumflex and right arteries (panels C and D). The multislice spiral CT revealed aneurysms of left-, rightand non-coronary sinus of Valsalva (panel E), and the proximal portion of left main coronary artery was displaced superiorly and compressed by the left sinus Valsalva aneurysm (panels F and G, arrowed). The sinus Valsalva aneurysms are generally isolated and are frequently involved in rightor non-coronary sinus of Valsalva [1,2]. There have been extremely rare reports of aneurysms arising from all sinuses of Valsalva, especially accompanied with acute myocardial infarction resulted from left sinus Valsalva aneurysm. We describe the case of a 58-year-old female who presented to the emergency department with chest discomfort and shortness of breath and received a workup for acute coronary syndrome. She underwent a cardiac catheterization, which incidentally found an anomalous left anterior descending artery with a right sinus of Valsalva origin, an absent left circumflex coronary artery, and a dominant right coronary artery of unusually large caliber and distribution. There were no identified atherosclerotic plaques. This anomalous configuration of the coronary arteries is exceptionally rare. She required medical management with daily oral acetylsalicylic acid 81 mg, atorvastatin 80 mg, twice daily metoprolol tartrate 50 mg, and hydrocodone/acetaminophen 7.5mg/325 mg oral tablet to be taken every 4 h, as needed for severe pain. Despite optimal medical management, she continued to have chronic angina. A surgical evaluation by a cardiovascular surgeon deemed her anomaly to be inoperable.
