Abstract:Eosinophilic gastroenteritis (EG) is a rare, distinct clinical entity, and EG associated with eosinophilic cystitis (EC) is extremely rare and has not been well documented. Here, we report two cases of EG and coexistent EC along with findings from computed tomography (CT) and magnetic resonance imaging (MRI). An 18-year-old male with a history of hematuria, urgency and occasional urodynia for two weeks and a 34-year-old male with a history of abdominal distention for one week were admitted to our hospital. Abdominal contrast-enhanced CT in both patients revealed wall thickening in different parts of the gastrointestinal tract with inhomogeneous reinforcement, coexistent with local or diffuse bladder wall thickening with progressive enhancement, and also showed that the bladder mucosal lining was nondestructive. Pelvic MRI showed that the local or diffuse thickened bladder wall was iso-intense on T1-weighted images, hypointense on T2-weighted images, and slightly restricted on diffusion weighted imaging (DWI) in one case. After therapy, the thickened wall of the gastrointestinal tract and urinary bladder had improved markedly in the two cases. To the best of our knowledge, this is the first report on the radiological imaging of EG and coexistent EC by both CT and MRI and the first with DWI findings.
Unexpected Multiple Gastrointestinal Cancers in a Patient with Chronic Eosinophilia: A Case Report

Abstract:While the relationship between eosinophilia and tumors has been extensively studied, eosinophilia is primarily observed clinically after tumor development, with no prior reports of eosinophilia occurring before tumor onset. This case report presents a unique instance of eosinophilia preceding the development of gastric cancer in an old male patient. The patient was evaluated after being diagnosed with gastric cancer. Investigations included physical examinations, hematological evaluations, molecular assays for genetic variations associated with eosinophilia, tumor marker assessments, staging using the AJCC system, histopathological examinations, and comprehensive sequencing of tumor tissue. The patient exhibited disseminated erythematous macules and had a history of eosinophilia. Genetic testing for germline variations associated with eosinophilia was negative, and molecular assays did not detect significant genetic alterations linked to clonal eosinophilia. Staging revealed T1N0MX gastric cancer, with histopathology showing high-grade intraepithelial neoplasia and positive PD-L1 expression. Notably, sequencing identified mutations in the NOTCH1 and NOTCH2 genes, which are known to regulate eosinophil migration. After surgical intervention, pathological examinations confirmed high-grade squamous cell carcinoma in the esophagus and high-grade intraepithelial neoplasia in the stomach, without eosinophil infiltration. Surprisingly, the peripheral blood eosinophilia dramatically decreased following tumor resection. This case underscores the potential for eosinophilia to occur prior to tumor development, challenging the current understanding of the relationship between eosinophilia and cancer. Further research is warranted to explore the implications of eosinophilia in cancer pathogenesis and its clinical significance.
[Clinical Characteristics and Etiology of Children with Hypereosinophilia].

Abstract:OBJECTIVE:To analyze the clinical characteristics and etiology of hypereosinophilia in children. METHODS:Clinical data of 88 children with hypereosinophilia admitted in Children's Hospital of Zhejiang University School of Medicine during April 2009 and May 2015 were retrospectively reviewed. The clinical manifestations, etiologies, and the correlation of disease severity with different etiologies were analyzed. RESULTS:The main clinical manifestations were fever, abdominal pain, cough and/or tachypnea, skin rash, hemafecia and diarrhea, which were observed in 19 (21.6%), 15 (17.0%), 14 (15.9%), 13 (14.8%), 11 (12.5%) and 6 (6.8%) cases, respectively. For etiologies, there were 28 cases (31.8%) induced by infections, including 11 cases of acute bronchial pneumonia, 9 cases of parasite infection, 4 cases of septicemia, 2 cases of urinary system infection, 1 case of cellulitis and 1 case of cholecystitis complicated with pancreatitis. The etiologies for the rest cases were allergic diseases (25 cases, 28.4%), eosinophilic gastroenteritis (20 cases, 22.7%), immunodeficiency (3 cases, 3.4%, all were moderate to severe eosinophilia), ABO hemolysis (2 cases, 2.3%), hematologic neoplasms (2 cases, 2.3%), eosinophilic cystitis (1 case, 1.1%), eosinophilic granulomatosis with polyangitis (1 case, 1.1%), nephrotic syndrome (1 case, 1.1%), and 5 cases (5.7%) were of unknown causes. CONCLUSIONS:Fever, abdominal pain, cough and/or tachypnea are the most common clinical manifestations in children with hypereosinophilia. Infection, allergic diseases and eosinophilic gastroenteritis are the most common etiologies, and parasites are the most common pathogen identified. Differential diagnosis of primary immunodeficiency should be considered in children with moderate to severe eosinophilia.

Abstract:Background:The clinical presentation of eosinophilic gastroenteritis( EG)is nonspecific and has not received much concern by physicians and pathologists. The diagnosis of EG was delayed in many cases. Aims:To explore the clinical features of EG. Methods:The clinical,laboratory,endoscopic and radiologic features and treatment in patients who were diagnosed as EG from October 2011 to September 2013 in Zhongshan Hospital, Fudan University were analyzed retrospectively. Results:Median age of 10 EG patients was 41. 9 years. Four patients had a history of allergy or asthma. The time from onset to diagnose was 25 days on average. The most common symptoms were abdominal pain with bloating, diarrhea or vomiting. Eight patients had hypereosinophilia. Abdominal CT revealed uniform edema or stratified thickness of intestinal wall or ascites in 7 patients. Endoscopy revealed erythema,edema and erosion in antrum,duodenum or jejunum in 6 patients. All cases were confirmed as having eosinophilic infiltration by mucosal biopsy or examination of ascites. Seven patients were successfully treated with corticosteroid. One patients experienced relapse after discontinuing corticosteroids during following up. Conclusions:EG may be more common than previously recognized and should be considered in the differential diagnosis of unexplained abdominal pain with peripheral eosinophilia or uniform edema or stratified thickness of intestinal wall. Multiple biopsies in multiple sites including descending duodenum and pathological examination for finding eosinophil infiltration are the keys to confirm the diagnosis. Corticosteroids are effective in relieving symptoms and improving eosinophilia.
Eosinophilic Gastroenteritis: a Clinicopathological Study

Abstract:Objective To evaluate the clincopathological presentation of eosinophilic gastroenteritis(EG). Methods Seven patients with EG were studied. their clinical manifestation, laboratory tests, endoscopy, treatment and prognosis were analyzed from the hospital records. Results (1) The patients of predominant mucosal type were presented with abdominal pain and diarrhea, and those of serosal type with abdominal pain and bloating.Some accompanied with nausea, vomiting and low fever.(2) Peripheral and bone marrow eosinophilia were presented in all seven patients. Eosinophil counts waxed and waned with symptoms and dramatically responsed to steroid therapy.(3) ESR, C-reactive protein and fibrinogen were in normal range, and the level of IgG was decreased.(4) The ascites were exudative with eosinophilia.(5) Some patchy lesions such as erosion and edema in antrum, duodenum and cecum were presented in endoscopy.Biopsy showed predominant eosinophil infiltration in mucosa.(6) It responsed to steroid therapy, and all symptoms and eosinophilia would be relieved within one week.(7) Symptoms may relapse but with good prognosis.Conclusion Eosinophilic gastroenteritis should be considered in the differential diagnosis of unexplained gastrointestinal symptoms especially in the presence of peripheral eosinophilia. Mucosal biopsy would be helpful to establish the diagnosis.

Abstract:Objective:To investigate the clinical features,diagnosis,treatment and prognosis of eosinophilic gastroenteritis(EG),and to analyze the cause of misdiagnosis.Methods:A total of 16 EG patients in Zhongshan hospital from 2005 to 2011 were studied.The medical history,clinical features,laboratory examination,endoscopic findings and treatment were retrospectively analyzed.Results:(1)The symptoms of EG patients included diarrhea(9 cases),abdominal pain(7 cases),hematochezia(5 cases),fever(2 cases),incomplete intestinal obstruction(2 cases) and ascites(1 case);(2)Among the 16 EG patients,9 cases were associated with allergic constitution;(3)Peripheral blood eosinophil(EOS) count increased significantly in 9 cases,and declined when symptoms were eased;(4)Endoscopic manifestations of EG patients were not specific.In the mucosa,there were sheet erosion,shallow ulcers,congestive spots or bleeding spots,mainly in antrum,duodenum,terminal ileum,ileocecal junction.The biopsy showed a large number of EOS infiltration;(5)imaging findings were not specific.CT or gastrointestinal barium meal examination results of 13 cases(81.3%) of mucous membrane type were not specific.Two cases(12.5%) of muscular type demonstrated incomplete intestinal obstruction when intestinal muscle layer was involved.One case(6.25%) of membrane type demonstrated ascites;(6)Hormones was first-line drugs for the treatment of EG,and it can quickly relieve symptoms and supress eosinophilia;(7)EG was a type of self-limited allergic disease.Although it might recur,the prognosis was good.Conclusions: The clinical manifestations and endoscopic findings of EG lack specificity,so it is easy to be misdiagnosed,Combining clinical features with deep biopsies of the gastrointestinal mucosa tissue is the key of diagnosis.

Abstract:Objective. To evaluate the clinical manifestations, endoscopic, and radiological characteristics, histological features, and treatment of eosinophilic gastroenteritis in adult patients. Methods. The clinical records of 34 patients diagnosed as eosinophilic gastroenteritis and eight patients who had abdominal symptoms and unexplained peripheral eosinophilia but no evidence of tissue eosinophilic infiltration were reviewed and analyzed. Results. Thirteen patients had a history of asthma or allergic rhinitis, while 10 had an allergic history. Peripheral eosinophilia occurred in 83.3% of patients, IgE was elevated in half, and alpha 2-macroglobulin was elevated in 92.8% of patients. The small intestine(82.4%) was the most common site involved. Erythema was the predominate feature seen on endoscopy. Radiographic study generally revealed thickening of the small intestinal wall. Eosinophilia generally involved the lamina propria of mucosa, but any layer of the gut could be affected, even in sites which had normal endoscopic or radiographic appearance. In all, 15.4% (6/39) of patients had Helicobacter pylori infection. Symptom remission within 1 week was found in 80% (20/25) of patients treated with steroids and in 58.8% (10/17) of patients treated with symptomatic treatment. In all, 85.7% (18/21) of patients had their eosinophil counts return to normal within 2 weeks in the steroid treatment group, but none (0/13) in the other group. Conclusion. Eosinophilic gastroenteritis may be more common than previously recognized. Multiple biopsies obtained from the both normal and abnormal appearing areas in the second part of the duodenum are highly recommended. Steroids are effective in relieving symptoms and improving eosinophilia.

Abstract:Eosinophilic gastrointestinal disorders (EGIDs) are a rare group of disorders characterized by eosinophilic infiltration of the gastrointestinal (GI) tract causing organ dysfunction. Eosinophilic esophagitis (EoE) is the most common of these disorders and can present with dysphagia and food impaction. Diagnosis is made through esophagogastroduodenoscopy (EGD) with biopsies demonstrating greater than 15 eosinophils per high-power field. In healthy individuals, the esophagus should be devoid of eosinophils however the rest of GI tract has a gradually increasing baseline level of eosinophils from the stomach to the colon.

Abstract:Eosinophilic Gastroenteritis (EGE) is a rare disorder marked by eosinophilic infiltration and peripheral eosinophilia. It can occur anywhere in the gastrointestinal tract without specific causes of eosinophilia. While its exact cause is unknown, hypersensitivity is significant. Symptoms include nausea, vomiting, abdominal pain, and weight loss. Diagnosis involves endoscopic biopsies, and prevalence is increasing with more endoscopic procedures. First-line treatment is steroids. We describe a case of diffuse abdominal pain leading to EGE diagnosis.

Abstract:Objective To explore clinical characteristics and key points of diagnosis and treatment of eosinophilic gastroenteritis(EG).Methods Clinical data of a patient with eosinophilic gastroenteritis combined with refractory ascites in our hospital was retrospectively analyzed.Results The patient was admitted for diarrhea for more than half a month,abdominal pain,abdominal distension and vomiting for four days.The patient suffered diarrhea after eating spicy and hot pot food half a month ago,but was not correctly diagnosed and treated in our hospital.The patient was admitted for abdominal pain,abdominal distension and vomiting in an other hospital four days before,and was diagnosed as having acute gastroenteritis and after correlative therapy was not effective,the patient was sent to our hospital,and was diagnosed as having acute ileus,unexplained ascites,adenomyosis and right renal cyst by results of physical examination,abdominal erect position of X-ray plain film,B ultrasound and CT.The patient complained that unexplained ascites and significant hypoalbuminemia had appeared twice last year after eating the same food.Examinations of blood routine and ascites routine showed high counts of acidophilic leukocytes,and final diagnosis was EG.The patient was discharged after significant symptoms were alleviated with glucocorticoid therapy.Conclusion EG is a rare disease without specific clinical manifestations.Patients with repeated abdominal distension,diarrhea and ascites with high counts of haemal acidophilic leukocytes should be considered as having EG.Endoscopy and biopsy should be performed as early as possible so as to confirm diagnosis.

Abstract:Eosinophilic colitis is a rare disease. It is part of the eosinophilic gastroenteritis, characterized by eosinophilic infiltration of the tissues that affects any segment of the digestive tract, but more frequently the stomach and small intestine and rarely the colon in isolation. The clinical case of a 29- year-old patient is described, who initially presented data of acute pancreatitis and later acute diarrhea with sluggishness and scant mucus. The analysis showed a complete blood count with eosinophilia 9.9% Immunoglobulin E with a positive result + 2500 IU/mL (<100IU/mL). Endoscopy and colonoscopies show diffuse inflammatory changes, confirming with the histopathological study infiltration of eosinophils throughout the gastrointestinal tract, conservative treatment was started with a good response. Eosinophilic gastroenteritis has a benign course, with significant infiltration of eosinophils in the wall of the digestive tract, which rarely involves only the colon. Its cause and pathogenic mechanisms are unknown; It classically presents in patients between the third and fifth decades of life, although it can affect any age group. The diagnostic criteria are: 1) gastrointestinal symptoms; 2) eosinophilic infiltration of one or more areas of the digestive tract demonstrated by biopsy; 3) absence of eosinophilic infiltration in organs outside the digestive tract; and 4) absence of parasitic infection. Eosinophilic gastroenteritis are pathologies that require a high index of suspicion for their diagnosis.
A Case Report of Eosinophilic Esophagitis with Dysphagia

Abstract:This paper reports a case of eosinophilic esophagitis who presented with dysphagia. The patient was a 54-year-old woman with diffuse esophageal thickening on ultrasonographic gastroscopy. Pathology demonstrated scattered eosinophilic infiltration within the squamous epithelium of the esophagus, accompanied by eosinophilic microabscess, and the counting density of eosinophilic cells was about 30/HPF, which was consistent with the diagnostic criteria of eosinophilic esophagitis (moderately active). After oral administration of prednisone acetate 30 mg/d and gradual reduction of the dose, the patient's clinical symptoms were significantly eased, and the imaging confirmed the improvement of the lesions. This article summarizes the patient's diagnostic and therapeutic process, and reviews the relevant literature in order to enhance clinicians' understanding of this disease.
Clinical analysis of 12 cases of children with eosinophilic gastroenteritis

Abstract:Objective To approach the diagnosis and treatment of eosinphilic gastroenteritis in children. Methods In 12 cases of children with eosinophilic gastroenteritis in Nanjing Children's Hospital Affiliated to Nanjing Medical University from May 2009 to Nov.2011,the clinical manifestations,laboratory data,endoscopic and pathological findings and treatment were collected and analyzed. Results In the 12 patients,mucosal type was present in 8 cases,muscle-type in 1 case,and serous type in 3 cases. Most of the pediatric eosinophilic gastroenteritis had no specific clinical manifestations. From direct endoscopic vision,edema and erosion were the main performance of gastrointestinal mucosa. Histopathologic examination revealed that there was dense infiltration in gastrointestinal mucosa lamina propria by eosinophils. The effects of dietetic and antianaphylactic treatments were good. Conclusion The clinical manifestations of pediatric eosinophilic gastroenteritis are various and nonspecific. The dietetic and antianaphylactic treatments have obvious therapeutic effect . About 1 to 2 weeks of glucocorticoids therapy can rapidly alleviate the symptoms,and it continues for 3 months by reducing the dosage gradually.

Abstract:INTRODUCTION: Chronic diarrhea is a common clinical presentation in GI practice and numerous causes need to be considered during the evaluation including infectious, inflammatory, and metabolic or functional bowel disorders. CASE DESCRIPTION/METHODS: A 58-year old white, obese male patient was admitted because of 2-year history of recurrent episodes of non-bloody, watery diarrhea with up to 12 daily bowel movements associated with crampy abdominal pain. He noted nausea and vomiting with a 30-pound weight loss but denied dysphagia, odynophagia, and GERD symptoms. There was no sick contact and no travel. His past medical history was significant for diarrhea-predominant irritable bowel syndrome (IBS), chronic pancreatitis, and seasonal allergies. He lived alone with several dogs, cats, and rabbits. Two years ago, EGD and colonoscopy showed unspecific duodenitis and gastritis, and 2 colonic adenomas were removed. Persistent peripheral eosinophilia of 15-25% ( > 1,500 Eosinophils/mm3) was noted for the previous 2 years. The physical examination was unremarkable. Tests for helminths, parasites, and myeloid neoplasms remained negative. Colonoscopy was performed and showed normal mucosa. Congested duodenal mucosa and mild gastric erythema were noted during EGD (Figure 1). Dense eosinophilic infiltrates along the entire length of the digestive tract were noted without evidence of pathological organisms, celiac disease, and microscopic colitis (Figure 2). Abundant mast cells were identified in corresponding segments of the GI tract (Figure 3). Tryptase levels were normal, IgE was > 10,000 IU/ml (nl 0-100), and skin testing was positive to cat standard, ragweed, and rye, but negative for food allergies. RAST showed positive results with dog and cat dander and dairy. The patient was advised to exclude dairy products from his diet. In follow up, his symptoms improved, and eosinophilia was no longer detectable. DISCUSSION: Episodic chronic watery diarrhea represented a diagnostic challenge in this case because of underlying IBS-D and chronic pancreatitis. Furthermore, the persistent moderate eosinophilia and the highly unusual intense eosinophilic inflammation of the entire intestinal mucosa resulted in comprehensive testing to exclude helminthic and parasitic infections and hematologic malignancies. Resolution of symptoms and peripheral eosinophilia upon exclusion of dairy suggested that concurrent eosinophilic esophagitis, gastroenteritis, and colitis were due to an allergic reaction to dairy.Figure 1.: Representative endoscopic images of the gastrointestinal tract.Figure 2.: Representative photomicrographs (hematoxylin-eosin stain) of histopathologic changes with unusually dense eosinophilic infiltrate in the lamina propria, epithelium, and muscularis. Eosinophilic gastroenteritis with multiple serous membrane effusion as the first sign: a case report and literature review

Abstract:Multiple serous membrane effusion (MSSE) as the first sign of eosinophilic gastroenteritis is extremely rare, and its clinical features and treatment methods have not been well described. The clinical characteristics, diagnosis, and treatment methods of MSSE in a 44-year-old woman were retrospectively reviewed. Laboratory testing revealed an elevated eosinophil count and serum immunoglobulin E level. The levels of all tumor markers were normal, but the CA125 level in serum and ascitic fluid was significantly increased. Ultrasonography showed a large amount of ascites and a moderate amount of pleural effusion. Echocardiography showed a small amount of pericardial effusion. Chest and abdominal computed tomography showed gastrointestinal wall thickening. Moreover, eosinophilic infiltration was detected in duodenal and rectal biopsy samples that had been collected during endoscopic examination of the upper gastrointestinal system. The patient was treated with 30 mg of prednisolone, and seafood was excluded from her diet for 4 weeks. The prednisolone was tapered over 8 weeks and continued at 5 mg prednisolone daily thereafter. The MSSE and peripheral eosinophilia showed a dramatic response to the steroid treatment. This case indicates that we should be highly aware of MSSE as the first clinical manifestation of eosinophilic gastroenteritis.

[Clinical characteristics of 267 children with eosinophilic gastrointestinal disease: a multicenter study]

Abstract:Objectives: To explore the clinical manifestations, endoscopic findings, histopathological changes, treatment, and prognosis of eosinophilic gastrointestinal disease (EGID) in children, with the aim of enhancing awareness among pediatricians about this condition. Methods: Data of 267 children with EGID were prospectively collected from January 2019 to July 2022 at Jiangxi Children's Hospital, Hunan Children's Hospital, and Henan Children's Hospital. The age of onset, symptoms, physical signs, laboratory examination results, endoscopic findings, histopathological changes, and treatment outcomes were observed. Results: Among the 267 children with EGID, the majority had mild (164 cases, 61.4%) or moderate (96 cases, 35.6%) clinical severity. The disease occurred at any age, with a higher prevalence observed in school-age children (178 cases). The main symptoms in infants were vomiting and hematemesis, while in toddlers, vomiting and bloody stools were prominent. Abdominal pain and vomiting were the primary symptoms in preschool and school-age children. Nearly half (49.4%) of the affected children showed elevated platelet counts on hematological examination, but there was no significant difference in platelet counts among children with mild, moderate, and severe EGID (P>0.05). Endoscopic findings in EGID children did not reveal significant specificity, and histopathological examination showed no specific structural damage. Among them, 85.0% (227 cases) received acid suppression therapy, 34.5% (92 cases) practiced dietary avoidance, 20.9% (56 cases) received anti-allergic medication, and a small proportion (24 cases, 9.0%) were treated with prednisone. Clinical symptoms were relieved in all patients after treatment, but three cases with peptic ulcers experienced recurrence after drug discontinuation. Conclusions: Mild and moderate EGID are more common in children, with no specific endoscopic findings. Dietary avoidance, acid suppression therapy, and anti-allergic medication are the main treatment methods. The prognosis of EGID is generally favorable in children.

Abstract:Eosinophilic gastrointestinal disorders (EGIDs) refer to eosinophilic infiltration of various sections of the gastrointestinal tract in the absence of secondary causes. Diagnosis of EGID requires histological evidence of eosinophilic infiltration of the GI tract. Here, we present a case of a young male with biopsy-proven eosinophilic gastroenteritis with a concomitant established diagnosis of immune thrombocytopenic purpura (ITP). Presently, EGIDs remain an underexplored clinical entity. While its pathophysiology is not fully understood at this time, TH2 mediated activation of B-cells and subsequent stimulation of eosinophils locally appears to be at play. This is in contrast to the TH1 predominant cytokine profile underlying ITP, which this patient also has. Treatment typically involves dietary modifications and glucocorticoids.

Abstract:Eosinophilic gastroenteritis (EGE) is a digestive disorder in children and adults that is characterised by eosinophilic infiltration in the stomach and intestine. Depending on the dominant layer of infiltration, this disorder is classified into three types, namely, mucosal, muscularis, and subserosa. Subserosa clinical presentation usually begins with ascites and is sometimes associated with symptoms of intestinal obstruction. Ascites is characterised by marked eosinophilia in ascitic fluid. EGE with ascites simulating appendicitis and peritonitis is rare in paediatric patients. This publication aims to report a case series of EGE with clinical features, treatment responses, and a review of the current strategies in the management of eosinophilic ascites.
