Bone marrow mesenchymal stem cells modulate immune tolerance in a rat model of renal transplantation:A comparison with cyclosporine A

Abstract:BACKGROUND: Results of previous animal study revealed that four times of injection with 1×10 7 mesenchymal stem cells (MSCs) before or after renal transplantation can induce immune tolerance, prolong survival time of graft, but the effect is significantly weak compared with the cyclosporine A. OBJECTIVE: To compare the effect of various doses and to further define precise administration regimen by regulating administration time of MSCs on the basis of previous studies. DESIGN, TIME AND SETTING: An in vivo trial of cytology was performed at the Animal Experiment Center of Nanjing Medical University (Nanjing, Jiangsu, China) from March to December in 2008. MATERIALS: Male Wistar rats as donors and male Lewis rats as recipients, totally 35 pairs, were selected to create models of renal transplantation. Using density gradient centrifugation method, bone marrow MSCs were harvested from 3 healthy male Wistar rats. METHODS: Following model induction, recipients were randomly divided into 4 groups: low-dose MSCs group (n=10), high-dose MSCs group (n=11), cyclosporine A group (n=8), and control group (n=6). Low-dose MSCs and high-dose MSCs groups were respectively subjected to injections of 1×10 6 and 1×10 7 MSCs 1 week prior to blood flow patency, once a day, for 2 weeks. Cyclosporine A group received cyclosporine A diluted by phosphate buffered saline 2 days after transplantation via intraperitoneal injection, 0.5 mg/(kg.d), once a day till death. Control group underwent phosphate buffered saline after transplantation, once a day till death. MAIN OUTCOME MEASURES: Biochemistry methods were used to detect the levels of creatinine in serum at 3, 5, 10, 15, 25 and 40 days following transplantation. Histology, animal survival days were performed to identify the function of the recipients in 2 animals at 4 days following transplantation in each group. RESULTS: A total of 35 recipient rats were included in the final analysis following model induction. Compared with cyclosporine A group, animal survival was significantly prolonged by MSCs in the high-dose MSCs group, and the effect was weak in the low-dose MSCs group. Creatinine levels were decreased in the low-dose MSCs and high-dose MSCs groups compared with the control and cyclosporine A groups at day 5 following transplantation (P < 0.05), and the difference was not significant at other time points. Renal interstitium infiltration was significantly improved, and no special changes were found in most renal glomerulus and arteries in the high-dose MSCs and cyclosporine A groups compared with low-dose MSCs and control groups. Typical severe acute injection was observed in the control group. Injection was significantly relieved in the low-dose MSCs group. CONCLUSION: Before transplantation and after twice agents treatment, 1×10 7 MSCs were given daily at 2 weeks following transplantation, which can effectively reach immunoloregulation, protect postoperative graft function, prolong animal's survival stage, showed similar effects as cyclosporine A.
