Abstract:Objective To study the mechanism of visual cortical deficit in patients with anisometropic amblyopia with BOLD-fMRI retinotopic mapping. Materials and Methods Eight anisometropic amblyopes underwent fMRI with retinotopic mapping and dots stimuli (6cpd, 4Hz, 0.5).1.5T MRI system was used to obtain functional images of visual cortex.Responses of the amblyopic and fellow fixing eye were compared in primary and secondary visual cortex.The fMRI degicits were analyzed regressively. Results Amblyopic eyes showed significantly less activation (T value) in V1, V2, V3, VP and V7 visual area compared with fixing eyes (P0.05).There were no statistic differences in V3a, V4 and V8 area (P0.05) between the two eyes.No causal relations were found between the activation loss of striate and exura-striate cortex (P0.05). Conclusion There are functional deficits in both primary and secondary visual cortex without causal relationship in amblyopes.The dorsal and ventral visual pathways are all affected in secondary visual cortex.FMRI retinotopic mapping provides a new way to investigate primary and secondary visual cortex deficit in amblyopes.
[Comparison of Deficits in Visual Cortex Between Anisometropic and Strabismic Amblyopia by Fmri Retinotopic Mapping].

Abstract:OBJECTIVE:To study the neural mechanism of visual cortical deficits between anisometropic and strabismic amblyopia comparatively by BOLD-fMRI retinotopic mapping. METHODS:Ten anisometropic amblyopes, 10 strabismic amblyopes and 9 normal subjects underwent fMRI with retinotopic mapping and luminous spots stimuli (spatial frequency: 6 cpd, contrast: 0.5). 1.5T MRI system was used to obtain functional images of visual cortex. Responses in primary and secondary visual cortex were compared among the dominant (normal subject group), anisometropic and strabismic amblyopic eyes by one-way ANOVA, successively analyzed by paired-samples t test between amblyopic eyes and fellow fixing eyes (anisometropic and strabismic amblyopia group respectively). Their fMRI deficits of amblyopes were analyzed regressively in two amblyopia groups respectively. RESULTS:The result of one-way ANOVA showed significantly a lower activation (average T value) in V1, V2, V3, Vp and V7 visual areas (P < 0.05, P values 0.018, 0.007, 0.002, 0.000, 0.025 respectively) between anisometropic amblyopia and normal group. This was in accordance with the result of paired-samples t test between amblyopic eyes and fellow fixing eyes in anisometropic amblyopia group (P < 0.05, P values 0.035, 0.007, 0.020, 0.009, 0.023 respectively). Statistical difference was found in V1, V2 and Vp areas between strabismic amblyopia and normal group (P < 0.05, P values 0.010, 0.007 & 0.003 respectively). The paired-samples t test in strabismic amblyopia group showed statistical difference only in V2, Vp areas (P < 0.05, P values 0.026 and 0. 009 respectively. ). So the two results were discordant. Between the two amblyopic groups, there was no statistical difference (P > 0.05) except in V7 area (P < 0.05, P value = 0.048). There was no causal relation between the primary visual cortical deficits and the secondary cortex in amblyopia (P > 0.05). CONCLUSION:Anisometropic amblyopia and strabismic amblyopia both have functional deficits in the primary and secondary visual cortex. The neural mechanism of secondary visual cortical deficits may be more complex than decreased cortex activation induced by the deficit of primary cortex. In the primary cortex, strabismic amblyopia and anisometropic amblyopia have neuronal deficits and/or abnormal interaction. In addition, strabismic amblyopia may also have suppressive influences of the fixing eyes upon the amblyopic eyes. Anisometropic amblyopia has the neural undersampling at a high spatial frequency in the secondary visual cortex as compared to amblyopic amblyopia.
Correlation Between the Visual Cortical Fmri Response and the Contrast Sensitivity in Human Amblyopia

Abstract:Objective To assess the correlation between the visual cortical fMRI response and the contrast sensitivity in human amblyopia.Methods Ten anisometropic amblyopes,10 strabismic amblyopes and 9 normal subjects underwent fMRI with retinotopic mapping and luminous spots stimuli(spatial frequency:6 cpd,contrast:0.5).1.5T MR was used to obtain functional images of visual cortex.Then the visual functional areas and the average T value were figured out.Comparing with the normal group,the functional deficits in anisometropic and strabismic amblyopia were observed.The contrast sensitivity was tested.The contrast sensitivity and the average T value of the impaired visual cortical areas were analyzed regressively in the two amblyopia groups,respectively.Results Amblyopic eyes showed that V1,V2,V3,VP and V7 areas in anisometropic amblyopia group,V1,V2 and Vp areas in strabismic amblyopia group had functional deficits compared with the dominant eyes of the normal group(P0.05).There was no statistical difference between the normalized average T value in the impaired visual cortical areas and the normalized contrast sensitivity value.Conclusion No correlation is found between the visual cortical fMRI response and the contrast sensitivity in human amblyopia.The causes may include that the contrast sensitivity deficit is due to the thresholds elevation of the neurons with low contrast at the higher spatial frequency,but fMRI can't detect this change effectively,as well as the technical limitations of fMRI itself and the type of stimulus.

Abstract:Although there is general agreement that the fMRI cortical response is reduced in humans with amblyopia, the deficit is subtle and has little correlation with threshold‐based psychophysics. From a purely contrast sensitivity perspective, one would expect fMRI responses to be selectively reduced for stimuli of low contrasts. However, to date, all fMRI stimuli used in studies of amblyopia have been of high contrast. Furthermore, if the deficit is selective for low contrasts, one would expect it to reflect a selective M‐cell loss, because M‐cells have much higher contrast gain than P‐cells and make a larger contribution to the threshold detection of stimuli of low spatial and medium temporal frequencies. To test these two predictions, we compared % BOLD response between the eyes of normals and amblyopes for low‐ and high‐contrast stimuli using a phase‐encoded design. The results suggest that the fMRI deficit in amblyopia depends upon stimulus contrast and that it is greater at high contrasts. This is consistent with a selective P‐cell contrast deficit at a precortical or early cortical site. Hum Brain Mapp, 2010. © 2010 Wiley‐Liss, Inc.
Study on the Functional Deficit of Brain Cortex in Human Ametropic Amblyopia by Fmri Based on Different Task Design Modals

Abstract:Objective: To investigate the functional deficit of brain cortex in human ametropic amblyopia. Methods: The regional brain activation in ametropic amblyopia and healthy subjects evoked by visual stimuli of red-black and green-black chessboard patterns were detected by blood oxygenation level dependent-functional magnetic resonance imaging (BOLD-fMRI) based on block-design and event-related design. Results: The major activated brain regions during visual stimulation with block-design modal were in the visual cortex in both ametropic amblyopic and healthy groups; While under the visual stimulation with event-related design modal, the activation were showed in the brain areas that implicated in visual attentional and conscious modulation such as frontal, cingulated, parahippocampal, precentral and temporal regions besides visual fields in both groups. And the intensity of activated areas in ametropic amblyopic group was lower significantly than that in healthy group. Conclusions: The reduced activities of the neurons or reduced synchronization among activity neurons in visual cortex may be major causes of visual cortical deficit in people with ametropic amblyopia. The decreased activation in the regions which related to visual attention and consciousness in ametropic amblyopic group may suggest that the neural network activities among these brain areas and the roles of these areas on visual awareness and cognition in ametropic amblyopic patients become weaker compared to healthy people.

Abstract:OBJECTIVE:To elucidate the difference in the pathogenesis between strabismic amblyopia and anisometropic amblyopia.METHODS:Using an EPI gradient echo sequence in 1.5 T MRI and visual electrophysiological system, the pattern visual evoked potentials and calcarine activation by monocular viewing of check-board pattern with reversal were examined in 18 strabismic amblyopes and 15 anisometropic amblyopes. The calcarine activation in terms of binocular voxels index and dependence on the spatial frequency of the stimuli were compared. The correlation between PVEP P(100) latency and activation intensity was also analyzed.RESULTS:The binocular voxels index was lower in the strabismic amblyope than that in the anisometropic amblyope. The calcarine activation from amblyopic eyes in strabismic amblyopes was more suppressed at higher spatial frequencies (1, 2 c/d); while that from amblyopic eyes in anisometropic amblyopes was more suppressed at all spatial frequencies (0.25, 0.5, 1 and 2 c/d). Inverse correlations existed between PVEP P(100) latency and activation intensity in two types amblyopia (P < 0.05).CONCLUSIONS:The binocular interaction is not equal in these two different types of amblyopia. Meanwhile, the amblyopia-related deficits in the low and high-spatial-frequency pathway of visual system are not similar between these two different types of amblyopia.
Fmri Study on Face Recognition in Anisometropic Amblyopia Patients

Abstract:Objective To investigate the cerebral areas that response to face recognition and make a preliminary quantitative analysis on their BOLD responses properties.Methods BOLD-fMRI technology and block design were adopted,and the visual stimulations of face and object were presented to 10 amblyopia patients and 11 normal subjects.The data were analyzed with Functional NeuroImages software package to generate brain activation maps.Results Visual cortex,ventri-temporal cortex and the latter parts of parietal lobe were activated in the face and object recognition in all subjects.The accurate positions in the fusiform areas for face and object recognition were different slightly.In amblyopia patients,the extent of face activation areas reduced obviously.Essential activation deficit for the face recognition was in anisometropic amblyopia.Refractive correction significantly increased the amblyopic eye's cortex activation.Conclusion In amblyopia patients,the cortex activation areas stimulated with the lazy eyes are much less than that with the sound eyes.The cortex deficit in the areas processing face recognition is obvious,but the functional deficit of areas for the object processing is not significant.The extent and the intensity of the activation are significantly increased after refraction correction.This further confirms that the refraction correction is useful for improving the visual acuity and its recovery.
BOLD-fMRI Study of Myopic Anisometropia

Abstract:Objective Using BOLD-fMRI technology to explore whether there is function lesion in visual cortex with myopic anisometropia.Methods BOLD-fMRI activity maps in 8 cases of myopic anisometropia were collected using 1.5 T MRI unit,the difference in location and realm of activity maps between two eyes and activity maps of the inferior eye before and after corrected were compared,the characteristics and mechanism of them were analysed.Results There was no distinct difference between two eyes with myopic anisometropia in visual cortex activity(P<0.05).The cortical activity of inferior eye boosted up when the refractive errors were connected.Conclusion In human with myopic anisometropia,there are not distinct cortex lesion and dominant eyes metastasis.However,glasses can enhance cortex activity distinctly.
BOLD Responses to Different Temporospatial Frequency Stimuli in V1 and V2 Visual Cortex of Anisometropic Amblyopia

Abstract:PURPOSE. Functional magnetic resonance imaging (fMRI) is the most advanced neuroimaging technique. The aim of this study was to investigate the blood oxygenation level-dependent (BOLD) of V1 and V2 visual cortex in anisometropic amblyopia with fMRI and explore the neural mechanism of amblyopia.METHODS. fMRI was performed with a 3.0-T MRI scanner during reversal checkerboard visual stimulation with different spatial frequencies (SF) of 0.4, 2, and 8 cpd in 2 states of temporal frequencies (TF) of 6 Hz and 8 Hz in a group of patients with anisometropic amblyopia (n=5) and a group of normal observers (n=4). Data were processed by SPM software offline. Responses of different eyes were compared in different conditions.RESULTS. The BOLD signal magnitude in V1 and V2 visual cortex of amblyopic eyes was significantly lower than the fellow eyes with anisometropic amblyopia at low SF (0.4-2 cpd) (p<0.05), but it was significantly higher than the fellow eyes at high SF (8 cpd) (p<0.05). The BOLD signal magnitude in V1 and V2 visual cortex of amblyopic eyes was significantly lower than the nondominant eyes in normal subjects in all conditions (p<0.001).CONCLUSIONS. There are cortical deficits in V1 and V2 visual cortex of anisometropic amblyopia, which may be useful for selecting an optimum stimulus at proper temporospatial frequency.
Blood Oxygen Level Dependent-Fmri Study of Brodmann 17, 18, 19 in Anisometropic Amblyopia Patients

Abstract:Objective To assess the influence of anisometropic amblyopia on different level of visual cortex with blood oxygen level dependent-fMRI techniques(bold-fMRI).Methods 1.5T SIMENS SONATA MRI was used.The amblyopic eyes and the fellow sound eyes before and after ametropia correction on 10 anisometropic amblyopes and 8 normal subjects ware studied respectively.The activation on different levels of visual cortex were observed.Results The cortex activaion area of the amblyopic eyes was less than that of the fellow sound eyes on Brodmann 17,18,19.The cortex activation of amblyopic eyes increased greatly when the diopter was corrected.However,there was not distinct difference between two eyes of the 8 normal subjects.Conclusion Although ametropia correction can increase the cortex activity of amblyopic eye partly,there is still distinct function lesion in brodmann 17,18,19 area for anisometropic amblyopic eyes.
Observation of brain activity on anisometropic amblyopia with fMRI based on amplitude of low frequency fluctuation

Abstract:Objective: To observe the changes of brain activity on human with anisometropic amblyopia with fMRI based on amplitude of low-frequency fluctuation (ALFF). Methods: Nine patients with anisometropic amblyopia of the left eye (amblyopia group) and twelve healthy subjects (control group) underwent fMRI scanning during visual stimuli of white-black chessboard. After data preprocessing, ALFF was obtained and compared intra- and inter-groups with t-test. Results: One patient with amblyopia group was excluded after data preprocessing. In amblyopia group, when left eyes were stimulated, ALFF decreased in the left lingual gyrus, right occipital lobe, right precentral gyrus, left posterior cingulate gyrus, left superior parietal lobule as well as left precuneus and cuneus, but increased in the left middle frontal gyrus, left superior frontal gyrus, left superior temporal gyrus, ritht amygdaloid body and right putamen; Meanwhile, when right eyes were stimulated, ALFF decreased in the corpus callosum and left middle temporal gyrus, but increased in the bilateral occipital lobes, right middle frontal gyrus and right caudate nucleus compared with that of control group. In amblyopic group, compared with right eye, when left eye was stimulated, ALFF decreased in the bilateral occipital lobes and increased in the right inferior frontal gyrus, left caudate nucleus, thalamus and anterior cingulate gyrus. Conclusion: The method of detecting ALFF for the task-fMRI allows direct observation of the neural activity changes in multiple brain areas of human with amblyopia.
Bold-functional MRI Study of the Abnormal Cortex and the Relationship Between the Inpairment of Vision and the Decreased Activation of the Visual Cortex in Anisometropic Amblyopia

Abstract:Objective To assess the activation changes on Brodmann areas 17,18 and 19 in anisometropic amblyopia and compare the features of the cortex activations before and after refractive correction on amblyopic eyes.Methods Bold-fMRI technique on 1.5 T Simens Sonata MRI and the blocks designation mode was used.The visual stimulation task was a black and white checkerboard with frequencies of 1 cycle/degree and 8 Hz.The baseline stimulus is a white cross-line at the center of the screen with black background.No other light or influence was allowed in the scanning room.The distance between the eyes and screen was 0.6 m.The experimental group included 10 anisometropic amblyopes.TSE and EPI sequence were used for the anatomical and functional data acquisitions.For experimental group,the activation areas of the visual cortex on both eyes were consequently measured before and after refractive correction.All data were analyzed online with t-test and offline with AFNI software.The threshold value was set to 0.001.During data processing,motion correction and three-dimensional smooth were used in all data.The time-signal intensity curves and the functional images were obtained.After Talairach coordinated the function images of every one,the activation areas were measured in Brodmann areas 17,18 and 19.The SPSS 12.0 software was used for statistic analysis.The difference of the cortical activations of amblyopic eyes before and after refractive correction was analyzed.The cortical activations of amblyopic eyes and the fellow eyes after refractive correction were compared.The visual acuity of the amblyopia eyes was measured.And the correlation between the lesion of cortical activation and visual acuity was analyzed.Results It was found that the cortex was activated obviously in the calcarine cortex around,occipital lobe,LGN and temporal lobe.The activation areas of amblyopic eyes was(3.7±0.4)×104 voxels,and(4.1±0.5)×104 voxels after corrected.The visual cortical activation increased greatly after refractive correction(Z=-2.197,P=0.028).After refractive correction Brodmann areas 17 was(0.44±0.07)×104 voxels,Brodmann areas 18 and 19 was(3.7±0.3)×104 voxels.While for the fellow sound eyes the activation areas of Brodmann 17 was(0.47±0.07)×104 voxels,and the activation areas of Brodmann 18 and 19 was(3.4±0.4)×104 voxels.The amblyopia eyes were still less than that of the fellow eyes distinctly in all of Brodmann areas 17,18 and 19(Z=-2.050,-2.524;P= 0.040,0.012).In this experiment,all amblyopic eyes have changes on visual acuity and visual cortex activation.The interoculer difference of cortex activation was 0.05±0.04,while the interoculer difference of visual acuity was 0.58±0.22.There was no remarkable linear relationship between them.Conclusion There are remarkable changes of cortical activation in Brodmann area 17,18 and 19 on anisometropic amblyopia.Refractive correction can increase amblyopic eye's cortex activation obviously.There is no linear correlation between the decreased cortical activation and visual acuity.This suggests that anisometropic amblyope should be treated as early as possible in the crucial period.
Structural and functional deficits in human amblyopia.

Abstract:Many neuroimaging tools have been used to assess the site of the cortical deficits in human amblyopia. In this paper, we aimed at detecting the structural and functional deficits in humans with amblyopia, with the aid of anatomic magnetic resonance imaging (aMRI) and functional MRI (fMRI). We designed the visual stimulus to investigate the functional deficits, and delineated the V1/V2 areas by retinotopic mapping. Then we performed the brain parcellation to calculate the volume of the subcortical structure on each individual, and reconstructed the cortical surfaces to measure the cortical thickness. At last, the statistical comparison was carried out to find the structural abnormities and their relationship to the functional deficits. Compared with the normal controls, it is found that the hemisphere difference existed on the unilateral amblyopia subjects, and the functional deficit might come along with the changes in the cortical volume, especially in the occipital lobe. The examined results may provide insight to the study of the neural substrates of amblyopia.
Resting-State Functional Connectivity of the Primary Visual Cortex in Children with Anisometropia Amblyopia

Abstract:Introduction: This study aimed to explore the functional connectivity of the primary visual cortex (V1) in children with anisometropic amblyopia by using the resting-state functional connectivity analysis method and determine whether anisometropic amblyopia is associated with changes in brain function. Methods: Functional magnetic resonance imaging (fMRI) data were obtained from 16 children with anisometropia amblyopia (CAA group) and 12 healthy children (HC group) during the resting state. The Brodmann area 17 (BA17) was used as the region of interest, and the functional connection (FC) of V1 was analyzed in both groups. A two-sample t test was used to analyze the FC value between the two groups. Pearson's correlation was used to analyze the correlation between the mean FC value in the brain function change area of the CAA group and the best corrected visual acuity (BCVA) of amblyopia. p < 0.05 was considered statistically significant. Results: There were no significant differences in age and sex between the CAA and HC groups (p > 0.05). Compared to the HC group, the CAA group showed lower FC values in BA17 and the left medial frontal gyrus, as well as BA17 and the left triangle inferior frontal gyrus. Conversely, the CAA group showed higher FC values in BA17 and the left central posterior gyrus. Notably, BCVA in amblyopia did not correlate with the area of change in mean FC in the brain function of the CAA group. Conclusion: Resting-state fMRI-based functional connectivity analysis indicates a significant alteration in V1 of children with anisometropic amblyopia. These findings contribute additional insights into the neuropathological mechanisms underlying visual impairment in anisometropic amblyopia.

Data acquisition was performed in 1.5 Tesla MRI scanner, processed by SPM software offline, comparisons were made between the number of activated voxel of eyes.Results The activation level (voxel number and T value of activation) of amblyopia eyes was lower than good eyes in amblyopia patients (t_(AN)=3.47, P0.05; t_(AT)=2.57, P0.05). There was no statistical interocular difference in control cases (t_(NN)=1.28, P0.1; t_(NT)=-0.02, P0.5).Conclusion The activation of visual motion of amblyopia eyes decreases. It may suggest that there is functional deficits in visual motion area of cortex.
Mapping Visual Contrast Sensitivity and Vision Loss Across the Visual Field with Model-Based fMRI

Abstract:Peripheral vision is crucial for daily activities and quality of life, yet traditional measures of visual function like visual acuity primarily assess central vision. Visual field tests can evaluate peripheral vision but require extended focus combined with precise fixation, often very challenging for patients with severe sight loss. Functional MRI (fMRI) with population receptive field (pRF) mapping offers a non-invasive way to map scotomas but is limited by its reliance on single contrast levels and the necessity of accurate fixation. We developed an fMRI-based approach to measure contrast sensitivity across the visual field without the need for precise fixation. By combining large-field stimulation with varying spatial frequencies and contrast levels with either pRF mapping or a retinotopic atlas based on anatomical landmarks, we modeled contrast sensitivity in the primary visual cortex (V1) over a large (40 deg) expanse of the visual field. In seven normal-sighted participants, we characterized differences in V1 cortical sensitivity across eccentricities and visual quadrants, finding reliable and reproducible patterns of sensitivity differences at individual and session levels. Additionally, our method effectively visualized cases of simulated and disease-linked sensitivity loss at the cortical level. Crucially, we demonstrated that these results could be largely recovered using a structure-based retinotopic atlas, eliminating the need for pRF mapping and precise fixation - although such an approach reduced sensitivity. This approach, integrating large-field stimulation with a retinotopic atlas, offers a promising tool for monitoring vision loss and recovery in patients with various visual impairments, addressing a significant challenge in current clinical assessments.

Abstract:Objective: To investigate the active dependence of visual cortex on the spatial and temporal frequencies of visual signals in human ametropic amblyopia.Methods:The brain cortex activation in ametropic amblyopia and healthy subjects evoked by visual stimuli of white-black chessboard with different spatial and temporal frequencies were detected by blood oxygenation level dependent-functional magnetic resonance imaging.Results:The sensitivity of ametropic amblyopic eyes decreased to visual signals with higher spatial or temporal frequencies.Conclusion:The reduced neuron activities and synchronization among activity neurons in visual cortex may result in the transmission and integration deficit of visual information in visual neural network,which will become more obvious when more visual information is given.
Spatial And Global Sensory Suppression Mapping Encompassing The Central 10 Degrees Field In Anisometropic Amblyopia

Abstract:PURPOSE. We investigate the efficacy of a novel dichoptic mapping paradigm in evaluating visual function of anisometropic amblyopes.METHODS. Using standard clinical measures of visual function (visual acuity, stereo acuity, Bagolini lenses, and neutral density filters) and a novel quantitative mapping technique, 26 patients with anisometropic amblyopia (mean age = 19.15 +/- 4.42 years) were assessed. Two additional psychophysical interocular suppression measurements were tested with dichoptic global motion coherence and binocular phase combination tasks. Luminance reduction was achieved by placing neutral density filters in front of the normal eye.RESULTS. Our study revealed that suppression changes across the central 108 visual field by mean luminance modulation in amblyopes as well as normal controls. Using simulation and an elimination of interocular suppression, we identified a novel method to effectively reflect the distribution of suppression in anisometropic amblyopia. Additionally, the new quantitative mapping technique was in good agreement with conventional clinical measures, such as interocular acuity difference (P < 0.001) and stereo acuity (P = 0.005). There was a good consistency between the results of interocular suppression with dichoptic mapping paradigm and the results of the other two psychophysical methods (suppression mapping versus binocular phase combination, P < 0.001; suppression mapping versus global motion coherence, P = 0.005).CONCLUSIONS. The dichoptic suppression mapping technique is an effective method to represent impaired visual function in patients with anisometropic amblyopia. It offers a potential in ``micro-'' antisuppression mapping tests and therapies for amblyopia.

Abstract:Many studies using random dot kinematograms have indicated a global motion processing deficit originated from extrastriate cortex, specifically middle temporal area (MT) and media superior temporal area (MST), in patients with amblyopia. However, the nature of this deficit remains unclear. To explore whether the ability of motion integration is impaired in amblyopia, contrast sensitivity for moving plaids and their corresponding component gratings were measured over a range of stimulus durations and spatial and temporal frequencies in 10 control subjects and 13 anisometropic amblyopes by using a motion direction discrimination task. The results indicated a significant loss of contrast sensitivity for moving plaids as well as for moving gratings at intermediate and high spatial frequencies in amblyopic eyes (AEs). Additionally, we found that the loss of contrast sensitivity for moving plaids was statistically equivalent to that for moving component gratings in AEs, that is, the former could be almost completely accounted for by the latter. These results suggest that the integration of motion information conveyed by component gratings of moving plaids may be intact in anisometropic amblyopia, and that the apparent deficits in contrast sensitivity for moving plaids in anisometropic amblyopia can be almost completely attributed to those for gratings, that is, low-level processing deficits.
