Abstract:Objective To investigate treatment method for existing and impending benign pathological femoral neck and trochanteric lesions.Methods A retrospective study wad done on 25 patients with benign proximal femur lesions treated between 2002 and 2004.All patients were treated with lesion curettage,cauterization,autografting and/or allografting.Then,dynamic condylar screw (DCS) or hip joint-span external fixator were selected according to the involved area of the lesion of the proximal femur.The duration of follow-up was mean 2.3 years (2-4 years).Results Sound bony union was seen on the radiographs of all patients by 3-6 months after surgery.There found no local recurrence or avascular necrosis of the femoral head,or complications like necrosis,infections or fractures.Function of the patients in the current series was graded excellent,assessed by the validated outcome measures MSTS-87,MSTS-93 and TESS lower extremity.Conclusion DCS or hip joint-span external fixator may be better alternative treatment of benign pathological femoral neck and trochanteric lesions,compared with conventional surgical methods such as simple bone grafting,traction and hip spica cast.

Abstract:Objective To explore the clinical effects of internal fixation with plate by the improved anterolateral approach to treatment of terminal femoral fractures.Methods Internal fixation with plate by improved anterolateral approach was applied to 78 patients with complex comminuted bone fractures of terminal femora from August 2008 to January 2012.Results During the follow-up of 10 months to 2 years,73 patients reached complete healing without screw relaxation,plate rupture and malformation(e.g.limb shortening and knee varus or eversion);the other 5 ones who suffered from nonunion reached complete healing by the second stage bone grafting.The excellent and good rate of knee joint function was up to 91 %.Conclusion With simple procedure,reliable fixation,integral protection of knee extensor device,mini-operative damage and propitiousness to functional recovery of knee joint,the internal fixation with plate by the improved anterolateral approach is a relatively good therapeutic approach to the complex comminuted bone fractures of terminal femora.

Abstract:Objective To investigate the clinical effect of subtrochanteric osteotomy plus dynamic hip/condyle screw (DHS/DCS)fixation to treat the fibrous dysplasia of the proximal femur with coxa vara.Methods Twenty-six clinical cases of femoral fibrous dysplasia with coxa vara were retrospectively analysed from April 2001 to May 2010.There were 9 males and 17 females,with a median age of 19(10 to 53).Forteen patients presented with monostotic disease,and 12 with polyostotic disease.The length of the lesion for polyostotic disease was from 9 cm to 36 cm,while for monostotic disease was from 7 cm to 15 cm.Seventeen cases were merged with pathologic fracture.The neck shaft angles were from 65 to 110 degree preoperation.The shortage of limbs was from 1.5 cm to 4.5 cm.Twenty-one patients involved in femoral neck were fixed with DHS and the other 5 cases with DCS.Results The duration of operation was from 80 to 170 min.The amout of bleeding was from 280 to 1650 ml with the average of 960 ml.The average postoperative neck shaft angles were 127 degree(119 to 140).The shortened limbs were extended 2.3 cm for average (1.5 to 3.6 cm).The follow-up time was from 9 to 118 months with the average of 39 months.All patients with osteotomy were healed.The neck shaft angle of 1 case decreased from post-operative 126°to 115°56months post-operatively,no coxa adducta recurrented and all internal fixations were in position.Hip screw backed out through the barrel in one case with the shortage of femoral neck.One case had femoral fracture after an injury.According to Guille function standard,24 cases were regarded as satisfied and 2 cases as unsatisfied.Conclusion Subtroehanteric osteotomy plus DHS/DCS fixation can effectively correct the fibrous dysplasia of the proximal femur with coxa vara,and significantly improve the function.
Treatment of Peritrochanteric Fracture with the Dynamic Condylar Screw

Abstract:Objective:To study the indications and complications in treatment of peritrochanteric fracture with the dynamic condylar screw(DCS).Methods :Twenty patients who sustained an unstable peritrochanteric fracture were performed indirectly reduction and fixation using a dynamic condylar screw.Twelve cases with intertrochanteric fracture (Evans classification:Type Ⅰunstable 10 cases;Type Ⅱ2 cases)and eight cases with subtrochanteric fracture.Results:Follow-up eight months to three years,mean two years.All fracture healed.There is only one case has a complication of minor varus hip.Conclusion:Treatment of peritrochanteric fracture with the dynamic condylar screw is a new application of DCS.It has the better results and can be an alternative treatment for peritrochanteric fractures in particular of unstable intertrochanteric fractures as well as subtrochanteric fractures.DCS is indicated for peritrochanteric fractures with greater trochanter extension failing by other devices.By means of indirectly reduction and fixation technique,it need not to stabilize the lesser trochanter as well as bone grafting.Repeating to insert the lag screw and early weight bearing might cause coxa vara.

Abstract:[Objective] To study therapeutic effect of internal fixation with dynamic hip screw (DHS) and cannulated screws for treating femoral intertrochanteric fiactures. [Methods] From August 1999 to January 2005, fourty-two patients with femoral intertrochanteric fracture were treated by reduction and fixation with dynamic hip screw (DHS) and cannulated screws. [Results] All patients were followed up for average 17 months (ranged from 10 months to 3 years). The range of motion, the collodiaphyseal angle and Dong′s evaluation standard were adopted as main parameters to evaluate the outcome of the operation. All fractures healed without the three complications of hardware fixation. There were 2 cases of lightly coxa vara. According to Dong′s evaluation method, the excellent or good rate was 95.2%. [Conclusion] Using DHS and cannulated screws to treat the intertrochanteric fiacture easily leads to anatomical reduction and rigid fixation, and gives large definitive rotational stability.

Abstract:Objective: To observe the curative effects of the application of the dynamic hip screw in the treatment of intertrochanteric fractures. Methods: Fifty-six patients sufferred intertrochanteric fractures had been treated with dynamic hip screw and followed up. Results: All but one cases had been followed up for 6 to 36 months, all fractures healed for the first rate without screw broken or internal fixity brokendown, coxa adducta etc, the excellent rate of joint function was 83. 7%. Conclusion: The dynamic hip screw is one of the best treatment for intertrochanteric fractures due to its advantages of satisfactory positioning, strong fixation, little complication and early exercise could be obtained.

Abstract:Objective To discuss the practical experience in treating complicated femoral subtrochanteric fracture by dynamic condylar screw( DCS),so as to improve the curative effect of this type fracture and to reduce complication. Methods There were 37 cases of complicated subtrochanteric fractures from the year 2003 to 2011.According to the Seinsheimer classification,there were 28 cases of typeⅣ,9 cases of typeⅤ. Dynamic condylar screw fixation was conducted in all the 37 cases. Results All the 37 cases were followed up for 9 months-3 years with the satisfaction rate of being 97. 30%. The flexion of hip joint lose 15°-25°in 2 cases compared with the uninjured side. No hip varus or hip vaigus was observed. Harris scoring system showed excellent in 33 cases,good in 2cases and poor in 2 cases,with the excellent and good rate of being 94. 60%. Conclusion Dynamic condylar screw is a good treatment method for treating femoral subtrochanteric fracture. Good technical operation can achieve reliable curative effect and low cost.

Abstract:Introduction: Basicervical femoral neck fractures are challenging fractures in geriatric populations. The goal of this study was to determine whether compression hip screw (CHS) constructs are superior to cephalomedullary constructs for the treatment of basicervical femoral neck fractures. Methods: Thirty cadaver femurs were osteotomized and received a CHS with derotation screw, a long cephalomedullary nail (long Gamma nail), or a short cephalomedullary nail (short Gamma nail). All constructs were loaded dynamically in compression until dynamic failure. Results: All failed CHS constructs demonstrated superior femoral head cutout. In the long Gamma nail and short Gamma nail groups, constructs failed by nail cutout through the medial wall of the trochanter or rotationally. Normalized fluoroscopic distance was found to increase markedly with an increasing cycle count when considering all treatment groups. Conclusions: Given our results and those of previous studies, we could not determine superiority of one implant and recommend that surgeons select fixation constructs based on the individual patient's anatomy and the surgeon's comfort with the implant.

Abstract:The goal for surgical treatment of femoral neck fractures is achieving union through an anatomic reduction and stable fixation while avoiding osteonecrosis. This report describes the case of a 50-year-old male who presented with pain and swelling in his right hip and inability to bear weight over his right lower limb following a road traffic accident. The X-ray revealed a right-sided femoral neck fracture (Garden Type IV and Pauwels Type III). The patient was surgically treated with the GS Kulkarni (GSK) controlled dynamic hip screw. On follow-up, the bone healing time was 14 weeks and the patient achieved a Harris hip score of 93.65 with significant improvement.

Abstract:Objective To introduce the experience and analyze the clinical outcome of applica- tion of reinforced bar-ring hybrid external fixator combined with limited internal fixation for complex frac- turcs of tibia plateau in patients.Methods From Aug.2002 to Feb.2005,21 patients with complex fractures of tibial plateau were treated with reinforced bar-ring hybrid external fixatar combined with lim- ited internal fixation.According to Schatzker classification,there were 13 patients with typeⅤfracture and 8 with typeⅥfracture.Voluntary and non-weight beating knee joint motion to partially weight bear- ing was taken one week after operation.Progressive loading was taken continuously until there was clinical evidence of fracture stability and then full weight bearing was done.Results The mean follow-up peri- od was 15.9 months (10-24 months).All the fractures got united and the mean union time was 4.2 months(3-8 months).All the injured knees could extend to less than 5 degrees and flex to more than 90 degrees after rehabilitation.Evaluated by Merchant score system for knee joint,81% of patients achieved excellent healing (7 cases) and good healing (10 cases).Conclusion Operation by using reinforced bar-ring hybrid external fixator combined with limited internal fixation can reduce complications and avoid stiffness of the knee joint.It is an effective method for complex fractures of tibial plateau.

Abstract:BACKGROUND:Intertrochanteric fracture can obtain good curative effects after active effective internal fixation in early stage. <br> OBJECTIVE:To compare dynamic hip screw+trochanter stabilizing plate and Gamma nail fixation for treatment of Evans type IIIB and type IV intertrochanteric fracture. <br> METHODS:A total of 67 patients with Evans type IIIB and type IV intertrochanteric fracture, including 28 cases undergoing dynamic hip screw+trochanter stabilizing plate and 39 cases undergoing Gamma nail, were selected from Chongming Branch, Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from February 2009 to November 2012. Al patients received antibiotics and functional exercises after treatment. Time of therapy, intraoperative bleeding amount, fracture healing time and Harris Hip Score were retrospectively analyzed in patients of both groups. <br> RESULTS AND CONCLUSION:Al patients were fol owed up for 6-45 months, averagely 21 months. (1) One patient affected infection in the dynamic hip screw+trochanter stabilizing plate group. He was healed after fixation removal half a year after drug exchange. One obese patient experienced injury again during out-of-bed activity at 3 weeks after treatment in the Gamma nail group. Fracture appeared at distal end of lock pin, and healed after replaced by Gamma nail. Trochanteric stabilizing plate was unstable in four patients of the Gamma nail group. They affected severe pneumonia after over 6 weeks of lying in bed, and healed after treatment in the medical department (2) Coxa vara appeared in two cases of the dynamic hip screw+trochanter stabilizing plate group, and three cases in the Gamma nail group. Fracture healing was achieved in al patients. (3) The time of therapy was longer, and intraoperative bleeding amount was significantly more in the dynamic hip screw+trochanter stabilizing plate group compared with the Gamma nail group (P<0.05). No significant difference in healing time and Harris Hip Score was detected between the two groups (P>0.05). Dynamic hip screw+trochanter stabilizing plate and Gamma nail are effective methods for treatment of Evans type IIIB and type IV intertrochanteric fracture. To choose a suitable fixator, we should aggregately analyze fracture stability, healing state and the degree of osteoporosis.
459 Cases of Dynamic Hip Screws for Peritrochanteric Fractures

Abstract:Objective To investigate the clinical results of using DHS for different types of per-itrochanteric fracture.Methods 459cases using dynamic hip screws (DHS )from Jan.,1996to Dec.,2000were studied retrospectively in the respects of Sander' s scores,postoperative complications and X -ray outcomes.The mean follow -up time was 40.7mont hs.Results According to the results of Sander' s scores,265cases(68.2%)were rated as excellent,83cases(21.4%)good,38cases(9.8%)fair,and 2cases(0.6%)poor.The main complication was fixation f ailure,which was most obvious in Typ e A 2.3and A 3.The mean union time for peritrochanteric fractures was20.3±7.6weeks and for subtrochante ric fractures 31.8±10.2weeks.Con-clusion DHS is quite effective for peritroch anteric fractures if well indicated,while in Type A 2.3,it should be combined with trochanter supportin g plate(TSP)and it is contraindicated for Type A 3.[

Abstract:Trochanteric femur fractures are frequently fixed with a four-hole side plate sliding hip screw device, but in recent decades two-hole side plates have been used in an attempt to minimize operative time, surgical dissection, blood loss and post-operative pain. The aim of this review was to determine whether two-hole sliding hip screw constructs are an acceptable option for fixation of AO-OTA 31-A1 and A2 trochanteric femur fractures. An electronic MEDLINE® database search was performed using PubMed®, and articles were included in this review if they were reporting historical, biomechanical, clinical or outcome data on trochanteric fracture fixation using a two-hole sliding hip screw device. A two-hole dynamic hip screw with a minimally invasive muscle-splitting approach is recommended for fixation of AO-OTA 31-A1 simple trochanteric fractures; this implant is biomechanically safe, and allows the use of a minimally invasive muscle-splitting approach which potentially provides better clinical outcome, such as decreased surgical trauma, shorter operative time, less blood loss, decreased analgesics use, and shorter incision length. As the majority of reviewed publications relate to the dynamic hip screw, it is not clear whether the above recommendations can be extended to any other sliding hip screw device. Treatment of Pathologic Peritrochanteric Fractures Using Sliding Hip Screws Augmented with Cerclage Reconstruction Plates

Abstract:We proposed a new method to augment the traditional sliding hip screw (SHS) with cerclage reconstruction plates to treat pathologically impending and actual peritrochanteric fractures as well as to revise open reductions and internal fixations to increase the construct strength against the shearing force, thus reducing the implant failure rate. In this retrospective study, patients with peritrochanteric pathology with at least two years of follow-up who underwent augmentation with cerclage reconstruction plates (modified SHS) and conventional SHS between 1 May 2015 and 31 May 2017 were divided into groups A (n = 12) and B (n = 28), respectively. Demographic data, surgery duration, blood loss, complications, and local radiotherapy were analyzed. The average surgery duration was significantly longer in group A (p = 0.013). The estimated intraoperative and perioperative blood losses were not significantly different between the groups. The implant survival rates were not significantly different under competing risk analysis. The success rate of a revision surgery with modified SHS was excellent, and implant survival time was >2 years, as observed with the previous SHS constructs. Subtrochanteric region involvement and a postoperative visual analog scale ≥4 could be risk factors of implant failure and revision surgery. This technique can be an alternative treatment for difficult pathologic peritrochanteric fractures, especially those with previous plating failure.

Comparison of the Dynamic Hip Screw with the Dynamic Hip External Fixator for Intertrochanteric Fractures: Report of a Randomized Controlled Trial

Abstract:Background: Pelvic fracture is one of the most common fractures in the elderly, especially in the intertrochanteric region. Therefore, in the present study, an external fixator was designed specifically for intertrochanteric fractures. The present study aimed to compare the operating time, amount of bleeding, and mortality rate between the patients who received either dynamic hip external fixators (DHEF) or dynamic hip screw (DHS). Methods: In 2018, 46 patients with intertrochanteric fracture due to trauma and high anesthesia risk were included in the study and randomly assigned to two groups of control (n=24, patients treated with DHS) and intervention group (n=22, patients treated with the DHEF). Treatment was carried out using the DHEF which was newly designed and placed outside the patient's body under short and light anesthesia. After 3 and 12 months of follow-up, the two groups were compared for some variables, including mortality rate, pain intensity, Harris hip score (HHS), cut-off rate of the device, femoral neck angles before and after the operation, hemoglobin changes, hematocrit levels before and after the operation, the number of injected blood units, and the number of hospitalization days. Results: Mortality rate was higher in open surgery with DHS. The assessment of variables in both intervention and control groups demonstrated that duration of operation (P<0.001), hospitalization length, time to union (P=0.001), pain intensity five days after the operation, as well as changes in Hb and HCT, were significantly higher in the control group than the intervention group. The mean HHS scores of 83.5±14.3 and 78.2±11.5 were gained for the DHEF and DHS groups, respectively (P=0.22). Conclusion: Considering the superior results of treatment with the external fixator in comparison with the DHS, such as lower mortality rate and fewer complications, a dynamic hip external fixator can be prescribed in patients with intertrochanteric fractures and high anesthesia risk.

Abstract:Background: Intertrochanteric fractures of femur are one of the most common fractures encountered in the elderly population. Dynamic hip screw (DHS) with a side plate is the standard modality of treatment especially in the case of stable fractures because it creates a controlled collapse at fracture site, leading to union. A comparative study between minimally invasive surgical technique with the conventional surgical technique for the fixation of intertrochanteric fractures with the DHS device was done. Methods: Thirty patients with stable intertrochanteric fractures (31A1.1, 31A1.2, and 31A1.3) were treated with the conventional open technique and another thirty with a new minimally invasive technique. Patients in both groups were followed up for at least 9 months. Results: There was less preoperative and postoperative blood loss, less soft-tissue destruction, less pain postoperatively, shorter hospital stays, and early mobilization and faster union with the minimally invasive technique. Conclusion: The study concludes that minimally invasive technique is superior to conventional (open) DHS in stable fractures.

Abstract:Objective: To observe the effects of different internal fixations on the treatment of pertrochanteric hip fractures. Methods: 158 cases of pertrochanteric hip fractures treated with different internal fixations, including anatomic steel, Gamma nail and dynamic hip screw, from January 1999 to October 2004, combined with functional exercise, were summarized and the advantages and disadvantages of different fixations were evaluated. Results: All of 158 cases were followed up for 6 to 36 months with a mean of 14 months. The rate of excellent or good in all patients was 94.61% by Harris criterion and 17 cases have surgical complications (10.18%). The complication rates of the three types of treatment had no difference. Conclusion: Anatomic steel, Gamma nail and dynamic hip screw are all ideal and reliable internal fixation material for pertrochanteric hip fractures as long as they are selected properly before operation.
