Effects of small volume hypertonic-hyperoncotic solution on neuron histology and function of hippocampus after cardiopulmonary resuscitation in rats

Abstract:Background: In recent years, some researches show that small volume hypertonic-hyperoncotic solution (HHS) can play a protective role on global brain ischemia resuscitation in rats. Objective: To investigate the effect of HHS on the neuron histology and function of hippocampus at 72 hours after asphyxial cardiac arrest and cardiopulmonary resuscitation in rats. Design, time and setting: Randomized control experiments were performed in the Department of Anesthesia, Union Hospital, Tongji Medical College of Huazhong University of Science and Technology and the Department of Anesthesia in the First Hospital of Sun Yat-sen University (Guangzhou, Guangdong, China) from May 2004 to May 2007. Materials: HHS was supplied by Fresenius Company (Germany), comprising 7.2% NaCl and 6% hetastarch. Thirty-six SD rats were employed to induce asphyxial cardiac arrest and cardiopulmonary resuscitation models. Methods: Rats were randomly divided into 3 groups (n=12): sham operation, normal saline group and HHS group. Immediately after spontaneous circulation restoration, each group was injected with the corresponding solutions. Main outcome measures: At 72 hours following spontaneous circulation restoration, the rat hippocampal neurons were observed and assessed using transmission electron microscope and terminal deoxynucleotidyl transferase-mediated dutp-FITC nick end labeling (TUNEL), and Caspase-3 activity assay was done by AMC. Rat hippocampa] function was detected with maze method. Results: All 36 SD rats were involved in the final analysis. Delayed neuron apoptosis in the hippocampus was detectable in the normal saline group and HHS group. TUNEL-positive cells in the hippocampus were reduced and Caspase-3 expression on CA1 region was obviously lower of the HHS group compared to the normal saline group (P < 0.05), which indicated a decrease of rat exploring activities after cardiopulmonary resuscitation. Conclusion: Immediately after spontaneous circulation restoration, small volume HHS can prevent Caspase-3 activation in CA1 region and inhibit the apoptosis of delayed neuronal apoptosis in hippocampus, which will play a protective role on the rat hippocampus.
