Alterations of gastric mucosal enterochromaffin cells in patients with postinfectious functional dyspepsia

Abstract:Background: It has been demonstrated that enterochromaffin cells (EC cells) play important roles in the development of postinfectious irritable bowel syndrome (PI-IBS). Because functional dyspepsia (FD) also belongs to functional gastrointestinal disorders, it is supposed that the mechanism of postinfectious FD (PI-FD) would be similar to that of PI-IBS. Aims: To investigate the quantitative and functional alterations of gastric mucosal EC cells in PI-FD patients, and to appraise preliminarily the potential mechanism of PI-FD. Methods: Thirty-five PI-FD patients, 30 non-PI-FD patients, and 20 healthy volunteers were enrolled. In these 35 PI-FD patients the stool cultured for pathogenic bacteria showed that 14 were positive (PI-FD-A group), while 21 were negative and its diagnosis was based on clinical manifestations (PI-FD-B group). Gastric mucosal EC cells were counted with 5-hydroxytryptamine (5-HT) immunohistochemical staining. The concentration of mucosal 5-HT was determined by high-performance liquid chromatography with an electrochemical detector. Ultrastructure of EC cells was observed by transmission electron microscope, and the number of EC cells located within 5 μm of nerve fibers was counted. Results: The number of EC cells and the concentration of 5-HT in gastric mucosa of PI-FD patients were significantly greater than those of non-PI-FD patients and healthy controls (P<0.05), and the number of EC cells was positively correlated with the severity of mucosal inflammation (P<0.05). Under electron microscopy, more organelles were seen in EC cells, special secretory granules were observed in the cytoplasm of some cells, and the number of EC cells located within 5 μm of nerve fibers in PI-FD patients was significantly greater than that in non-PI-FD patients and healthy controls (P<0.05). No statistical difference was observed in number of EC cells and concentration of 5-HT between PI-FD-A and PI-FD-B groups. Conclusions: Proliferation and activation of gastric mucosal EC cells are founded in PI-FD patients, EC cell activation pathway is probably involved in the pathogenesis of PI-FD.
