Bone delayed union after osteoporotic intertrochanteric fracture in the elderly patients with type 2 diabetes mellitus

Abstract:BACKGROUND: It has been reported that type 2 diabetes mellitus can result in metabolic disorders of carbohydrate, fat and protein, tissue repairing disorder and delayed fracture healing, and even nonunion. OBJECTIVE: To explore the relationship between type 2 diabetes mellitus(T2DM)and elderly osteoporotic intertrochanteric fracture healing, and the effect of insulin treatment on the relationship. DESIGN, TIME AND SETTING: The retrospective case analysis was done at the Department of Orthopaedic Surgery, the Sixth People’s Hospital, Shanghai Jiao Tong University between January 2003 and April 2007. PARTICIPANTS: 124 elderly patients with Evan’sⅢ type intertrochanteric fractures including 58 male and 66 female, they were equal to or more than 60 years and were admitted into the Sixth People’s Hospital of Shanghai Jiaotong University between January 2003 and April 2007 and treated with proximal femoral nail (PFN). METHODS: According to the WHO diagnostic criteria, 124 subjects were divided into T2DM group (n=60) and non-DM controls (n=64). The T2DM group was subdivided into insulin treatment group (n=33) and non-insulin treatment group (n=27), respectively. MAIN OUTCOME MEASURES: The fracture union condition and incidences of nonunion and delayed union. RESULTS: All patients were followed up for more than 10 months, averagely for (16.8±4.2) months. The incidences of intertrochanteric fracture delayed union and nonunion were higher in the T2DM group compared with the non-DM controls (P < 0.05), while the excellent and good rate of intertrochanteric fracture healing was lower in the T2DM group compared with the non-DM controls (P < 0.01). The incidences of intertrochanteric fracture delayed union or nonunion were higher in the non-insulin treatment group compared with the insulin treatment group (P < 0.05), while the excellent and good rate of intertrochanteric fracture healing was lower in the T2DM group compared with the non-DM controls (P < 0.01). CONCLUSION: T2DM is an important factor of bone delayed union or nonunion in elderly patients with osteoporotic intertrochanteric fractures, insulin treatment can make the blood glucose better and decrease the incidences of bone delayed union and nonunion.
