The Experimental Study of Absorbable Chitin Conduit for Bridging Peripheral Nerve Defect with Nerve Fasciculu in Rats

Abstract:To investigate the possibility of constructing artificial peripheral nerves using de-acetyl chitin conduit, the sciatic nerves defect model was built at left legs in SD rats. They were divided into 3 groups randomly: group A: nerve graft in situ (n = 12, gap distance 10 mm); group B: biological chitin conduit bridging the peripheral nerve defect (n = 12, gap distance 10 mm); group C: biological chitin conduit bridging the peripheral nerve defect with nerve fibers in conduits (n = 12, gap distance 10 mm). Electrophysiological examination, histological examination and re-myelinated axons counting were applied after 6th and 12th week after operation, respectively. Regenerated nerve fibers were seen in the distal nerve segments of all three groups. The nerve conduction velocity and the re-myelinated axons counting of group A were better than that of group C at both 6th and 12th week time points (p < 0.05). The nerve conduction velocity and the re-myelinated axons counting of group C were better than that of group B at both 6th and 12th week time points (p<0.05). The repair effects of chitin conduit with nerve fibers in conduit bridging peripheral nerve defect (10 mm) were better than that of simple conduit bridging group, and that of group A (nerve graft group) was better than that of group C.

Abstract:Objective To improve the physicochemical and biological properties of perineural tubes made form materials currently used for clinical purposes. Methods The chitosan conduits with different permeabilities were coated tightly with enteric coat to form three sorts of composite biomembrane. These conduits were bridged between the sciatic nerve defects of SD rats by nerve allograft, and silicon conduits were taken as controls. The electromyography and morphography were then studied to evaluate their reparative effects. Results The composite biomembranechitosan conduits could be absorbed almost completely at 16 weeks after implantation; Better regeneration of the nerve both morphologically and electrophysiologically were demonstrated in the group of semipermeable composite conduits comparing to the nonpermeable and macroporing groups. Abstract:A porous, biodegradable, natural chitin/chitosan nerve conduit was constructed. Scanning electron microscopy confirmed that it was homogeneous and highly porous. FT-IR spectra showed that there were no residues arising from the preparation process in the conduit. Addition of chitin to the chitosan solution increased the mechanical strength and maximum tensile strength from 7.2 to 9.6 MPa. Preliminary animal tests indicated that porous chitin/chitosan conduits did not swell in vivo and were compatible with surrounding tissue.

Abstract:Care for patients with peripheral nerve injury is multifaceted, as traditional methods are not devoid of limitations. Although the utilization of neural conduits shows promise as a therapeutic modality for peripheral nerve injury, its efficacy as a standalone intervention is limited. Hence, there is a pressing need to investigate a composite multifunctional neural conduit as an alternative treatment for peripheral nerve injury. In this study, a BDNF-loaded chitosan-based mimetic mussel polymer conduit was prepared. Its unique adhesion characteristics allow it to be suture-free, improve the microenvironment of the injury site, and have good antibacterial properties. Researchers utilized a rat sciatic nerve injury model to evaluate the progression of nerve regeneration at the 12-week postoperative stage. The findings of this study indicate that the chitosan-based mimetic mussel polymer conduit loaded with BDNF had a substantial positive effect on myelination and axon outgrowth. The observed impact demonstrated a favorable outcome in terms of sciatic nerve regeneration and subsequent functional restoration in rats with a 15-mm gap. Hence, this approach is promising for nerve tissue regeneration during peripheral nerve injury.

Abstract:Treating peripheral nerve defects represents a clinical challenge, and nerve conduits lacking an internal scaffold lead to limited large nerve gap regeneration. Here, we bridged 10-mm sciatic nerve defects in rats with a chitosan conduit filled with 0, 0.5, or 1.0 mg of simvastatin in Pluronic F-127 hydrogel. We assessed subsequent nerve regeneration using the sciatic functional index (SFI), electrophysiological assessments, Fluoro-Gold (FG) retrograde tracing, gastrocnemius muscle mass measurements, and histological and immunohistochemical assessments of nerve regeneration. Ten weeks after implantation, the chitosan conduit filled with simvastatin/Pluronic F-127 hydrogel promoted nerve regeneration; there were significant increases in the SFI, compound muscle action potential peak amplitude, motor nerve conduction velocity, FG-labeled neuron number in the dorsal root ganglia, myelin sheath thickness, axon diameter, gastrocnemius wet weight, and muscle fiber area percentage in the gastrocnemius muscle (all p<0.05). The expression levels of neurotrophic factors, such as pleiotrophin, hepatocyte growth factor, vascular endothelial growth factor, and glial cell line-derived neurotrophic factor, were also found to be increased. The results suggest that chitosan conduits filled with simvastatin/Pluronic F-127 hydrogel improved peripheral nerve regeneration and functional recovery in rats, which may have been related to the increased expression of several endogenous neurotrophic factors. Abstract:Peripheral nerve injury is a common traumatic illness in clinic, and long-gap injury in particular has been difficult to repair. In this study, a new type of biological nerve conduits with tunable diameters and lengths were fabricated from extracellular matrices (ECM) derived from porcine small intestinal submucosa. A peroneal nerve defect model was established by removing the left common peroneal nerve of pre-defined lengths in goats. The biological conduits were sutured in and their functions to repair 2 cm, 4 cm, 6 cm and 8 cm nerve defects were investigated for a 12 month period. Electrophysiological, histological, and TEM examinations showed normal nerve conductive velocities and regeneration of myelinated nerve fibers in 2 cm and 4 cm defects grafted with ECM-based conduits. The effects of these biological conduits on the repair of 2 cm and 4 cm nerve defects were comparable to those of allograft nerves. The repair function of nerve conduits was found to decrease with the defect lengths. The ECM-based nerve conduits may possess advantageous bioactive properties to support nerve tissue regeneration and have potential for future clinical use in long-gap nerve injury repair.
Study on Using a New Biodegradable Conduit to Repairing Rat's Peripheral Nerve Defect

Abstract:OBJECTIVE:Nerve conduit, a new way to repair peripheral nerve, has good prospect. Now, the main obstacle of the conduit's clinical using is the absence of material with both ideal physical property and good biocompatibility. In this study, the PLA is added to chitosan to form a novel material used to make a new nerve conduit. And the rat modal is used to get the data of the nerve conduit made of new material to repair the defect of peripheral nerve in vivo.METHODS:Chitosan-PLA Composite Biodegradable material was used to produce a nerve conduit with depth of 200 um, diameter of 1.5 mm. This new conduit was used to repair the sciatic nerve's defect of 5 mm long. The other two groups as control groups repairs the same defects using silicon conduit or nerve's autograft respectively. 12 weeks after operation, normal assessments were performed including movement of hindlimb, hindlimb's retraction after being pricked by needle and ulcer in claw's palm. Special assessment was performed including histology, ECG, image analysis and weighing of triceps muscle of calf. The data of three groups was statistically analyzed by group t test.RESULTS:As the normal observation showing: after 12 weeks, the regenerating nerve succeeded to path through the gap and dominated the muscle. Special assessment including ECG, image analysis and weighing of triceps muscle of calf showed that the group using Chitosan-PLA Composite conduit had the nervous regeneration better than silicon conduit group both in the nerve's quality and in axon's quantity. And the nervous regeneration of Chitosan-PLA Composite conduit group was as good as that of nervous autograft's group.CONCLUSIONS:Chitosan-PLA Composite Biodegradable conduit could repair the defect of peripheral nerves successfully. It is an ideal material for nerve conduit.

Abstract:Although autogenous nerve transplantation is the gold standard for treating peripheral nerve defects of considerable length, it still has some shortcomings, such as insufficient donors and secondary injury. Composite chitosan scaffolds loaded with controlled release of nerve growth factor can promote neuronal survival and axonal regeneration after short-segment sciatic nerve defects. However, the effects on extended nerve defects remain poorly understood. In this study, we used chitosan scaffolds loaded with nerve growth factor for 8 weeks to repair long-segment (20 mm) sciatic nerve defects in adult rats. The results showed that treatment markedly promoted the recovery of motor and sensory functions. The regenerated sciatic nerve not only reconnected with neurons but neural circuits with the central nervous system were also reconstructed. In addition, the regenerated sciatic nerve reconnected the motor endplate with the target muscle. Therefore, this novel biomimetic scaffold can promote the regeneration of extended sciatic nerve defects and reconstruct functional circuits. This provides a promising method for the clinical treatment of extended peripheral nerve injury. Abstract:OBJECTIVE:To investigate the possibility of bridging small peripheral nerve gap using a de-acetyl chitosan conduit.METHODS:The sciatic nerves of right sides were cut at SD rats. They were divided into 5 Groups randomly; Group A: epineurium suture in situ (n = 24); Group B: biological conduit with a small gap for bridging the peripheral nerve (n = 24, with 5 mm gap); Group C: epineurium suture with distal stump rotated 180 degrees (n = 24); Group D: bridging the nerve by biological conduits with a small gap, but the distal stump rotated 180 degrees (n = 24, with 5 mm gap); Group E: biological conduit with a small gap for bridging the peripheral nerve with NGF (n = 24). Electrophysiological examination, histological examination and myelinated axon counting were applied after 2, 4, 6, 8 weeks after operation respectively.RESULTS:Regenerated nerve fibers were seen in the distal nerve segments of all 5 groups; The nerve conduction velocity of small gap group (group B, D) was faster than that of corresponding simple epineurium suture group (group A, C) at all 2, 4, 6, 8 week time point (P < 0.05). The myelinated axon counting of small gap group (group B, D) was faster than that of corresponding simple epineurium suture group (group A, C) at all 4, 6, 8 week time point (P < 0.01), and there was no statistically significant difference at 2 week time point.CONCLUSION:The repair effects of chitin conduit bridging peripheral nerve with small gap (5 mm) are better than that of epineurium suture directly, and possess the potential to substitute the epineurium suture.
The Regeneration of Transected Sciatic Nerves of Adult Rats Using Chitosan Nerve Conduits Seeded with Bone Marrow Stromal Cell-Derived Schwann Cells.

Abstract:Autologous nerve grafts have been the 'gold standard' for treatment of peripheral nerve defects that exceed the critical gap length. To address issues of limited availability of donor nerves and donor site morbidity, we have fabricated chitosan conduits and seeded them with bone marrow stromal cell (BMSC)-derived Schwann cells as an alternative. The derived Schwann cells used were checked for fate commitment. The conduits were tested for efficacy in bridging the critical gap length of 12 mm in sciatic nerves of adult rats. By three months post-operation, mid-shank circumference, nerve conduction velocity, average regenerated myelin area, and myelinated axon count, in nerves bridged with BMSC-derived Schwann cells were similar to those treated with sciatic nerve-derived Schwann cells (p > 0.05) but significantly higher than those bridged with PBS-filled conduits (p < 0.05). Evidence is thus provided in support of the use of chitosan conduits seeded with BMSC-derived Schwann cells to treat critical defects in peripheral nerves. This provides the basis to pursue BMSC as an autologous source of Schwann cells for transplantation therapy in larger animal species.

Abstract:The incidence of peripheral nerve injury (PNI) is high worldwide, and a poor prognosis is common. Surgical closure and repair of the affected area are crucial to ensure the effective treatment of peripheral nerve injuries. Despite being the standard treatment approach, reliance on sutures to seal the severed nerve ends introduces several limitations and restrictions. This technique is intricate and time-consuming, and the application of threading and punctate sutures may lead to tissue damage and heightened tension concentrations, thus increasing the risk of fixation failure and local inflammation. This study aimed to develop easily implantable chitosan-based peripheral nerve repair conduits that combine acrylic acid and cleavable N-hydroxysuccinimide to reduce nerve damage during repair. In ex vivo tissue adhesion tests, the conduit achieved maximal interfacial toughness of 705 J m(-2) +/- 30 J m(-2), allowing continuous bridging of the severed nerve ends. Adhesive repair significantly reduces local inflammation caused by conventional sutures, and the positive charge of chitosan disrupts the bacterial cell wall and reduces implant-related infections. This promises to open new avenues for sutureless nerve repair and reliable medical implants.
EXPERIMENTAL STUDY ON NERVE CONDUIT COATED WITH CHITIN AND FILLED WITH A GUIDE-FIBER

Abstract:OBJECTIVE:To study the result of using nerve conduit coated with chitin and filled with a guide-fiber to repair peripheral nerve defect.METHODS:Twenty-four female adult SD rats were made the model of 14 mm-gap on bilateral sciatic nerve under sterile condition. The rats were randomly divided into 4 groups (n=6), group A: polymer polyglycolic-lactic acid (PGLA) nerve conduit coated with chitin and filled with a guide-fiber as experimental group to repair 14 mm-gap of rat sciatic nerve;group B: PGLA nerve conduit coated with chitin; group C: PGLA nerve conduit; group D: autograft (control group). The repair result was evaluated by normal observation, EMG testing and S-100 histological immunostaining analysis 4 and 12 weeks after operation.RESULTS:Four weeks after the operation, there were new regenerated immature fibers in groups A, B and C, 12 weeks after the operation, the regenerated nerve fibers were seen to have bridged the gap. There were myelinated fibers equably distributed and rarely new generated nerve fibers in distal parts of group D. The repair result of PGLA nerve conduit coated with a chitin and filled with guide-fiber was better than that of groups B and C (P<0. 05). There was significant difference of nerve fiber diameter, thickness of myelin sheath and fiber density in group D from those in groups A, B and C (P<0. 05), but there were degenerative changes such as vacuoles in sheaths and myelin separation in proximal and few new regenerated nerve fibers in distal parts of group D.CONCLUSION:PGLA nerve conduit coated with chitin and filled with a guide-fiber offers a possible substitute for the repair of peripheral nerve defect.

Abstract:Background: Autograft is still the gold standard technique for the repair of long peripheral nerve injuries. The addition of biologically active scaffolds into the lumen of conduits to mimic the endoneurium of peripheral nerves may increase the final outcome of artificial nerve devices. Furthermore, the control of the orientation of the collagen fibers may provide some longitudinal guidance architecture providing a higher level of mesoscale tissue structure.Objective: To evaluate the regenerative capabilities of chitosan conduits enriched with extracellular matrix-based scaffolds to bridge a critical gap of 15 mm in the rat sciatic nerve.Methods: The right sciatic nerve of female Wistar Hannover rats was repaired with chitosan tubes functionalized with extracellular matrix-based scaffolds fully hydrated or stabilized and rolled to bridge a 15 mm nerve gap. Recovery was evaluated by means of electrophysiology and algesimetry tests and histological analysis 4 months after injury.Results: Stabilized constructs enhanced the success of regeneration compared with fully hydrated scaffolds. Moreover, fibronectin-enriched scaffolds increased muscle reinnervation and number of myelinated fibers compared with laminin-enriched constructs.Conclusion: A mixed combination of collagen and fibronectin may be a promising internal filler for neural conduits for the repair of peripheral nerve injuries, and their stabilization may increase the quality of regeneration over long gaps.

Abstract:Nerve regeneration after delayed nerve repair is often unsuccessful. Indeed, the expression of genes associated with regeneration, including neurotrophic and gliotrophic factors, is drastically reduced in the distal stump of chronically transected nerves; moreover, Schwann cells undergo atrophy, losing their ability to sustain regeneration. In the present study, to provide a three-dimensional environment and trophic factors supporting Schwann cell activity and axon re-growth, we combined the use of an effective conduit (a chitosan tube) with a promising intraluminal structure (fresh longitudinal skeletal muscle fibers). This enriched conduit was used to repair a 10-mm rat median nerve gap after 3-month delay and functional and morphometrical analyses were performed 4 months after nerve reconstruction. Our data show that the enriched chitosan conduit is as effective as the hollow chitosan conduit in promoting nerve regeneration, and its efficacy is not statistically different from the autograft, considered the "gold standard" technique for nerve reconstruction. Since hollow tubes not always lead to good results after long defects (> 20 mm), we believe that the conduit enriched with fresh muscle fibers could be a promising strategy to repair longer gaps, as muscle fibers create a favorable three-dimensional environment and release trophic factors. All procedures were approved by the Bioethical Committee of the University of Torino and by the Italian Ministry of Health (approval number: 864/2016/PR) on September 14, 2016.

Mechanical properties and permeability of porous chitosan-poly(p-dioxanone)/silk fibroin conduits used for peripheral nerve repair

Abstract:Some poly(p-dioxanone) (PDO) homopolymers were first synthesized and the selected PDO was conjugated onto chitosan using a group-protecting method to produce chitosan-poly(p-dioxanone) (CH-PDO) copolymers with various PDO percentages changing from around 30 to 60 wt%. The CH-PDO with the PDO content of around 42 wt% was used to blend with prescribed amounts of silk fibroin (SF) to build porous single-lumen conduits that are intended to be used for long-gap peripheral nerve repair. Some genipin-crosslinked CH-PDO/SF conduits were endowed with an average porosity of around 60% in their porous wall, and with changed pore-sizes varying from around 10 to ca. 70 μm using optimized processing conditions. After being degraded in a PBS medium containing a certain amount of lysozyme for various periods up to 8 weeks, some optimal CH-PDO/SF conduits were able to retain their compressive load and deformation recovery at around 59 N/m and 73% in wet state, respectively. In addition, the achieved CH-PDO/SF conduits allowed the permeation of nutritional molecules with various molecular weights while showing a certain ability to prevent cells from infiltrating through the conduit wall. Cell culture confirmed that the optimized CH-PDO/SF conduits were able well supported the growth of rat glioma C6 cells. These results suggest that presently developed CH-PDO/SF conduits have promising potential for long-gap peripheral nerve repair.

Abstract:The peripheral nervous system consists of ganglia, nerve trunks, plexuses, and nerve endings, that transmit afferent and efferent information. Regeneration after a peripheral nerve damage is sluggish and imperfect. Peripheral nerve injury frequently causes partial or complete loss of motor and sensory function, physical impairment, and neuropathic pain, all of which have a negative impact on patients' quality of life. Because the mechanism of peripheral nerve injury and healing is still unclear, the therapeutic efficacy is limited. As peripheral nerve injury research has processed, an increasing number of studies have revealed that biological scaffolds work in tandem with progenitor cells to repair peripheral nerve injury. Here, we fabricated collagen chitosan nerve conduit bioscaffolds together with collagen and then filled neuroepithelial stem cells (NESCs). Scanning electron microscopy showed that the NESCs grew well on the scaffold surface. Compared to the control group, the NESCs group contained more cells with bigger diameters and myelinated structures around the axons. Our findings indicated that a combination of chitosan-collagen bioscaffold and neural stem cell transplantation can facilitate the functional restoration of peripheral nerve tissue, with promising future applications and research implications.

Abstract:BACKGROUND: Severe peripheral nerve injuries, such as deficits over long distances or proximal nerve trunk injuries, pose complex reconstruction challenges that often result in unfavorable outcomes. An innovative approach to repairing severe peripheral nerve damage involves using conduit suturing for nerve transposition repair. Cylindrical nerve guides are typically unsuitable for nerve transposition repair. Moreover, postsurgical adjuvant treatment is essential to promote the development of axonal lateral sprouts, proximal growth, and the restoration of neurostructure and function. The purpose of this research is to assess the impact of chitosan-based conduits with varying inner diameters on nerve transposition repair when combined with modified formula Radix Hedysari (MFRH).METHODS: Using chitosan, we created conduits with varying inner diameters on both ends. These conduits were then utilized to repair the distal common peroneal and tibial nerves in SD rats using the proximal common peroneal nerve. Subsequently, MFRH was employed as a supplementary treatment. The assessment of the repair's effectiveness took place 16 weeks postsurgery, utilizing a range of techniques, including the neurological nerve function index, neuroelectrophysiological measurements, muscle wet weight, and examination of nerve and muscle histology.RESULTS: The outcomes of our study showed that following 16 weeks of postoperative treatment, MFRH had a significant positive impact on the recovery of neuromotor and nerve conduction abilities. Moreover, there was a significant increase in the ratio of wet weight of muscles, cross-sectional area of muscle fibers, quantity and structure of regenerated myelinated nerve fibers, and the count of neurons.CONCLUSIONS: A combination of chitosan-based chitin conduits possessing different inner diameters and MFRH can considerably promote the regeneration and functional recovery of damaged nerves, which in turn enhances nerve transposition repair efficacy.

[Experimental study of using chitosan nerve conduit combined with PRP to repair facial nerve defect].

Abstract:Objective:The purpose of this study was to produce chitosan nerve conduit in order to investigate the feasibility of chitosan nerve conduit combined with autologous platelet-rich plasma (PRP) for repairing facial nerve defects.Method:Forty New Zealand white rabbits were randomly divided into four groups (n=10 in each group).Establishment of the facial nerve defect model of the upper buccal branches was placed in the nerve regeneration catheter and injected with the same amount of PRP and saline. The PRP was injected into the chitosan nerve conduit as group A. The physiological saline was injected into the chitosan nerve conduit as group B. The physiological saline was injected into the silicone tube as group C. The PRP was injected into the silicone tube as group D. Eight weeks later,facial nerve gross observation,facial nerve electrophysiological tests,histological observation,image analysis,valuation of nerve regeneration recovery were detected.Result:Five patients were lost to follow up (all five in the modified Semont group),and three patients failed to complete treatment (all three in the Epley group). The sequelae at the 3rd day and one week after modified Semont maneuver were 27 and 9,while 41 and 15 in Epley group. The efficacy rates at the 3rd day and one week after modified Semont maneuver were 91.7% and 98.3%,and 91.9% and 96.8% in Epley group retrospectively. The sequelae and short-term effective rate of patients in modified Semont group was no difference when compared with that in Epley group (P>0.05).Conclusion:The chitosan nerve conduit combined with PRP has a certain effect on the repair of facial nerve defects and is expected to be applied to the repair of clinical facial nerve defects.
