Abstract:T(H)1 cells were predominant in nasal polyp tissue. Polyps from atopic patients had more T(H)2 cells and eosinophils than nonatopic patients' polyps did. Abstract:Conclusions. Infiltration and activation of eosinophils is a characteristic of nasal polyposis. Allergic reaction is a risk factor for the accumulation of eosinophils in this disease. T-cell-derived interleukin 5 (IL-5) and autosecretion of IL-5 from activated eosinophils may be causative reasons for the extension and persistence of eosinophil inflammation. Objectives. To investigate whether eosinophils were accumulated and activated in nasal polyposis, and the roles of IL-5, eotaxin, and T cells in this process. Materials and methods. A retrospective study was conducted on 17 tissue samples from patients with nasal polyposis with allergy and 26 cases of non-allergic polyposis. Immunohistochemical staining by specific antibodies was carried out using the alkaline phosphatase anti-alkaline phosphatase method and the avidin-biotin complex technique.Results. The number of EG1-positive cells (pan eosinophil marker) was similar to the number of EG2-positive cells (activated eosinophil marker) in all tissue samples, although EG1- and EG2-positive cells were richer in allergic patients than those in non-allergic patients. Both EG1- and EG2-positive cells were correlated with CD3-positive cells (pan T cell marker) and IL-5-producing cells in allergic or non-allergic polyposis. A large proportion of IL-5 producing cells were eosinophils. Eotaxin protein was detected in all tissue samples and dominantly located in epithelial cells. Eotaxin expression between allergic and non-allergic subjects was not significantly different.
TSLP Signaling and TH2-type Inflammation is Enhanced in Eosinophilic but Not Noneosinophilic Nasal Polyps

Abstract:RATIONALE: Our previous published study shows that over half Chinese patients with nasal polyps (NPs) present noneosinophilic inflammation and only eosinophilic NPs showed enhanced TH2 type inflammation. Thymic stromal lymphopoietin (TSLP) signaling plays an important role in initiating and aggravating TH2 inflammation and is involved in bronchial asthma, atopic dermatitis, and in immunity to parasites. However, its role in NPs is unknown. The aim of this study is to investigate TSLP signaling and TH1 and TH2 attracting chemokines and their receptors expression in eosinophilic and noneosinophilic NPs. METHODS: TSLP expression was examined by real-time RT-PCR and immunohistochemistry, and its receptors TSLPR and IL-7alpha, and OX40L were detected by real-time RT-PCR. In addition, TH1 attracting[gamma-interferon-induced monokine (MIG)/CXCL9, interferon-inducible T cell alpha-chemoattractant (I-TAC)/CXCL11] and TH2-attracting[thymus and activation-regulated chemokine (TARC)/CCL17 and macrophage-derived chemokine (MDC)/CCL22] chemokines and their corresponding receptor CXCR3 and CCR4 expression were detected by real-time RT-PCR. RESULTS: TSLP protein and mRNA levels and TSLPR, IL-7alpha, and OX40L mRNA levels were only elevated in eosinophilic but not in noneosinophilic NPs compared with control. Although both eosinophilic and noneosinophilic NPs had increased TH2-attracting chemokines TARC and MDC expression, only eosinophilic NPs demonstrated increased CCR4 level compared with control. CONCLUSIONS: TSLP signaling and TH2-type inflammation is enhanced in eosinophilic but not in noneosinophilic NPs.

Abstract:Background: Locally produced IgE contributes to the initiation and development of eosinophilic inflammation in eosinophilic nasal polyps independent of systemic atopy. However, whether CXCR5(+)CD4(+) T follicular helper (T-FH) cells are involved in local IgE production at mucosal sites remains unexplored.Objective: We sought to explore the presence, phenotype, and function of CXCR5(+)CD4(+) T-FH cells in eosinophilic nasal polyp tissues compared with noneosinophilic nasal polyp and control normal nasal tissues.Methods: T-FH cell-surface phenotypes and subsets and B-cell subsets in nasal tissues and peripheral blood were studied by means of flow cytometry. Immunohistochemistry was used to detect the tissue location of T-FH cells. Sorted nasal T-FH cells and CXCR5(-) T cells were cultured with autologous naive B cells purified from blood.Results: Nasal T-FH cells expressed inducible costimulator, programmed cell death protein 1, and the transcription factor B-cell lymphoma 6 (Bcl-6) at an intermediate level when compared with bona fide T-FH cells in tonsils and circulating T-FH cells. Although counts of total T-FH cells and IL-21(+), IFN-gamma(+), and IL-17(+) T-FH cells were increased in both eosinophilic and noneosinophilic nasal polyp tissues compared with those in normal nasal tissues, IL-4(+) T-FH cell counts were only increased in eosinophilic polyp tissues. IL-4 and IL-21 were involved in polyp T-FH cell-induced IgE production from naive B cells, and nasal IL-4(+) T-FH cell counts correlated highly with local IgE levels in vivo. IL-4(+)Bcl-6(+)CD4(+) T-FH cells were identified in ectopic lymphoid structures in eosinophilic nasal polyps. T-FH cells also positively correlated with germinal center B cells and plasma cells in nasal tissues.Conclusion: Nasal IL-4(+) T-FH cells might be involved in local IgE production in eosinophilic nasal polyps.
Comparison of clinical characteristics of chronic rhinosinusitis with and without nasal polyps

Abstract:Objective:To compare the clinical characteristics of chronic rhinosinusitis(CRS) with nasal polyps(CRSwNP) and without nasal polyps(CRSsNP), and to provide evidence for formulating rational treatment strategies.Methods:The clinical data of 225 patients suffering from CRS in Jiangsu and Anhui provinces who were hospitalized for endoscopic sinus surgery from November 2018 to December 2019, which included the preoperative blood routine, blood biochemistry, serum total IgE(tIgE), allergen specific IgE(sIgE), paranasal sinus CT, and postoperative histopathological examinations, were retrospectively analyzed for statistical analysis.Results:Of the 225 patients, 148(65.8%) were in the CRSwNP group and 77(34.2%) in the CRSsNP group. There were no significant differences in gender, age, length of hospital stay, differential leucocyte counts and biochemical indexes in peripheral blood between the two groups( P>0.05). In patients with CRSwNP and CRSsNP, the positive rate of serum sIgE against at least one of allergen was 34.5% and 40.3% respectively( P>0.05), among which dust mites were most common with a positive rate of 23.0% and 26.0% respectively( P>0.05). There were also no statistically significant differences in positive rates of serum tIgE and sIgE against 20 common allergens, as well as inhaled and ingested allergens between the CRSwNP and CRSsNP groups( P>0.05). In addition, no significant difference was found in distribution of serum allergen-sIgE concentrations between these two groups( P>0.05). However, the Lund-Mackay scores of paranasal sinus CT scans were significantly higher in the CRSwNP group than those in the CRSsNP group in terms of the sinuses, ostiomeatal complex and the total scores( P<0.05). Moreover, the positive rate of lymphocyte and plasma cell infiltration in the tissue were more than 60% in both groups, while eosinophil infiltration was significantly higher in the CRSwNP group(43.2%) than that in the CRSsNP group(22.1%; P<0.05). Conclusion:At present, lymphocyte and plasma cell inflammation is still dominant in patients with CRS in this region. However, the positive rate of eosinophil infiltration in polyp tissues was 43.2% in patients with CRSwNP, which needs to arouse our recognition and further attention. The total positive rate of allergen sensitization in CRSwNP and CRSsNP patients was 34.5% and 40.3% respectively, with no significant difference. And the major allergen is dust mites.

Abstract:Objective: To investigate the type 2 innate lymphoid cells (ILC2s) in eosinophilic nasal polyps, non-eosinophilic nasal polyps and controls, and to compare the characteristics of ILC2s indifferent types of nasal polyp. Methods: Flow cytometric analysis was used to quantify the ILC2s and Th2 cells in tissues from 19 patients with chronic rhinosinusitis with nasal polyp (CRSwNP) and 6 controls. The patients were classified into two groups as eosinophilic chronic rhinosinusitis with nasal polyps (ECRSwNP) and non-eosinophilic chronic rhinosinusitis with nasal polyps (non-ECRSwNP) based on eosinophil counts under hematoxylin-eosin staining. ILC2 and Th2 cell frenquencies, measured as a percentage of CD45(+) cells, were compared among ECRSwNP group (n=9), non-ECRSwNP group (n=10) and control group (n=6) using Mann-Whitney U test .The correlation between ILC2, Th2 cell frequencies and eosinophil count in tissues was analyzed using Pearson correlation coefficient. Results: ILC2 frequencies in ECRSwNP group were significantly higher than non-ECRSwNP group and controls ((0.051±0.025) vs (0.011±0.017), (0.051±0.025) vs (0.004±0.004), Z value was -3.185, -3.186, respectively, both P<0.05). There was no significant difference between ILC2 frequencies in non-ECRSwNP and controls ((0.011±0.017) vs (0.004±0.004), Z=-0.712, P=0.492). Th2 cell frequencies in ECRSwNP and non-ECRSwNP group were significantly higher than controls ((0.500±0.437) vs (0.106±0.102), (0.275±0.170) vs (0.106±0.102), Z value was -2.946, -2.278, respectively, both P<0.05). There was no significant difference between Th2 frequencies in non-ECRSwNP group and ECRSwNP group ((0.275±0.170) vs (0.500±0.437), Z=-1.306, P=0.211). ILC2 frequencies significantly correlated with Th2 cell frequencies (r=0.571, P=0.011) and tissue eosinophilia (r=0.579, P=0.009). Th2 cell frequencies significantly correlated with tissue eosinophilia (r=0.844, P=0.001). There was no significant association between ILC2 and allergic status. Conclusion: ILC2s are elevated in ECRSwNP, and not influenced by allergic status, suggesting that ILC2s play an important role in ECRSwNP without allergic diseases.
Roles Played by the PI3K/Akt/HIF-1α Pathway and IL-17A in the Chinese Subtype of Chronic Sinusitis with Nasal Polyps

Abstract:Background. The Chinese subtype of CRSwNP may have a unique pathogenesis. This study was designed to seek the role of the PI3K/Akt/HIF-1α pathway and IL-17A in CRSwNP. Methods. The total IgE, ECP, and IL-17A levels were determined by UniCAP100 and ELISA. The activity of MPO was detected by the biochemical techniques. The protein expressions of HIF-1α, p-Akt, and PI3K were detected by the WB method. HIF-1α and IL-17A mRNA levels were measured by RT-PCR. Results. The CRSwNP group showed significantly elevated MPO activity, PI3K, p-AKT protein, HIF-1α, and IL-17A mRNA levels in nasal polyps. Stimulated by the TNF-α, the PI3K, p-AKT, HIF-1α, and IL-17A levels significantly elevated in the fibroblasts. Inhibited by the Wortmannin, those indicators significantly declined in the fibroblasts. Conclusion. The PI3K/Akt/HIF-1α pathway played a role in the pathogenesis of CRSwNP. The elevated IL-17A level might be responsible for the neutrophilic inflammation in CRSwNP. The PI3K/Akt/HIF-1α pathway might regulate the IL-17A-related inflammation in CRSwNP.

Abstract:Chronic rhinosinusitis with nasal polyps (CRSwNP) is a heterogeneous disease that can be classified as eosinophilic or noneosinophilic. Nasal polyps can exhibit different types of mucosal inflammation and responses to treatment. Imbalanced ratios of T-helper 17(Th17) and regulatory T (Treg) cells may contribute to the pathogenesis of nasal polyps. This study assessed the frequency of Th17 and Treg cells and related cytokines in patients with nasal polyps and tested for associations with mucosal remodeling. Surgical samples from 12 controls and 33 CRSwNP patients were analyzed histopathologically. The frequency of Th17 and Treg cells in peripheral blood mononuclear cells (PBMCs) and tissues were determined using flow cytometry. Th17 and Treg cells-related cytokines in plasma were measured by Cytometric Bead Array (CBA) multiplex assays and enzyme-linked immunosorbent assays (ELISAs). Eosinophilic CRSwNP (ECRSwNP) patients exhibited robust eosinophilia, whereas non-ECRSwNP patients were characterized by neutrophilia. Compared with non-ECRSwNP, an increased Th17/Treg ratio in ECRSwNP was associated with a less increased frequency of Th17 cells and a more striking reduction of Treg cells. An altered Th17/Treg cell ratio was positively correlated with eosinophilic and neutrophilic infiltration, submucosal basement membrane thickness, and the degree of subepithelial collagen deposition. Compared with non-ECRSwNP, ECRSwNP had higher levels of IL-17A and IL-4, and lower levels of IL-10 and TGF-β1, whereas non-ECRSwNP showed higher levels of IFN-γ and IL-6. Th17/Treg cell imbalance in nasal polyps (both in tissues and PBMCs) with distinct cytokine profile may contribute to different inflammatory patterns (eosinophilic versus neutrophilic inflammation) and corresponding features of mucosal remodeling. Effective strategies can be designed to target a Th17/Treg imbalance to restore immune homeostasis in nasal polyps.

CD8+T cells with distinct cytokine-producing features and low cytotoxic activity in eosinophilic and non-eosinophilic chronic rhinosinusitis with nasal polyps

Abstract:Summary Background CD 8 + T cells are important effectors of cell‐mediated immunity; however, their contribution to the pathogenesis of CRS is unclear. Objective This study aimed to characterize the cytokine‐producing features and cytotoxic activity of CD 8 + T cells, and their correlation with inflammation patterns in CRS with nasal polyps. Methods The expression of IFN ‐γ, IL ‐4, IL ‐5, IL ‐17A, forkhead box P3 ( FOXP 3), perforin, and granzyme B in CD 8 + T cells was studied by means of flow cytometry, immunohistochemistry, and immunofluorescence. The expression of CD 8 + T‐cell subset relevant chemokines and chemokine receptors was detected by means of real‐time RT ‐ PCR or ELISA . The cytotoxic activity of sorted CD 8 + T cells was defined by anti‐ CD 3‐redirected killing assay. Results Compared with controls, elevated percentages of total CD 8 + T cells and cytotoxic T lymphocyte (Tc) 1 ( IFN ‐γ + ), Tc2 ( IL ‐4 + ), and Tc17 ( IL ‐17A + ) cell subset, and decreased percentages of FOXP 3 + CD 8 + regulatory T cells, were found in both eosinophilic and non‐eosinophilic polyps with a Tc2‐skewed and Tc1/Tc17‐dominated response in eosinophilic and non‐eosinophilic polyps, respectively. Nasal CD 8 + T cells were found to produce similar or even higher levels of IFN ‐γ and IL ‐4 compared with CD 4 + T cells. Tc1 and Tc17, and Tc2 ( IL ‐4 + and IL ‐5 + ) cell subset percentages positively correlated with neutrophil and eosinophil counts in sinonasal mucosa, respectively. Strikingly, the expression of perforin and granzyme B and cytotoxic activity were significantly reduced in nasal CD 8 + T cells compared with their counterparts in peripheral blood. The expression of CXCL 16, CCL 17, and CCL 20 positively correlated with Tc1, Tc2, and Tc17 cell subset number in sinonasal mucosa, respectively. Conclusion and Clinical Relevance CD 8 + T cells have low cytotoxic activity; nevertheless, they are a significant and previously underappreciated source of inflammatory cytokine production in polyps. Different Tc cell subset domination may contribute to distinctly biased granulocyte inflammation in eosinophilic and non‐eosinophilic polyps.
Analysis of Helper T Cell Subsets and of the Expression of Its Related Transcription Factors in Nasal Polyps

Abstract:Purpose To investigate the difference in immunological pattern of nasal polyps in patients with and without allergic rhinitis(AR).Methods Sixteen cases of polyps without AR and sixteen with AR were involved in this study.The fresh samples were prepared into cell suspension for flow cytometric analysis.The liquid nitrogen stored tis- sues were analyzed with real-time quantitative PCR for the expression of GATA-binding protein(GATA-3)and T-box ex- pressed in T cells(T-bet)mRNA.Results TH1 subset was predominant in all the nasal polyps.The mean percentage of TH2 in nasal polyps with AR was significantly higher than that in the nasal polyps without AR.The nasal polyps dem- onstrated higher expression of GATA-3 and T-bet mRNA than that in the inferior turbinate tissue.The nasal polyps with AR showed higher levels in GATA-3 mRNA expression than that in the nasal polyps without AR.Conclusions The nasal polyps with and without AR are different in immunological patterns.Cytoimmunity participate in the pathogenesis of nasal polyps without AR.Cytoimmunity and humoral immunity are both involved in the pathogenesis of nasal polyps with AR. TREM-1 Neutrophil Activation Pathway is Suppressed in Eosinophilic Nasal Polyps

Abstract:Background: Chronic rhinosinusitis with nasal polyps (CRSwNP) is characterized by type 2 helper T-cell-skewed immune profiles, but the state of the neutrophil activation signaling pathway in CRSwNP is unknown. The purpose of this study was to examine neutrophil activation pathways in the pathogenesis of CRSwNP. Methods: Institutional review board approved the study in which tissue proteomes were compared between control (inferior turbinate) and CRSwNP (nasal polyps) (n = 10/group) using an aptamer-based proteomic array and confirmed by whole transcriptomic analysis. Protein expression was analyzed using Student's t test and Benjamini-Hochberg procedures followed by the application of Ingenuity Pathway, MetaCore, and Genemania bioinformatics analyses. Results: All the patients with CRSwNP (n = 10) had eosinophilic nasal polyps. Compared with controls, proteins associated with the triggering receptor expressed on myeloid cells 1 (TREM 1) neutrophil activation signaling pathway such as Calcineurin B, zeta chain associated protein kinase of 70 kD (ZAP70), 14-3-3 protein theta, 14 3 3 protein zeta/delta, protein kinase C delta type (PKC-D), Interleukin (IL)-17B, IL-17B receptor, IL-23, and IL-1B were significantly decreased in CRSwNP (fold change [FC]= -1.60, P=.003; FC = 1.85, P=.040; FC = -1.26, P < .001; FC = -1.05, P = .008; and FC = -1.31, P = .004; FC = -1.22, P < .001; FC = -1.09, P = .022; FC = -1.25, P < .001; and FC = -1.31, P=.014; respectively). In contrast, tissue eosinophil count (P<.001) and eosinophil associated proteins such as C-C motif chemokine 17, periostin, and galectin 10 were all significantly increased in CRSwNP (FC = 1.56, P = .009; FC = 3.95, P < .001; and FC = 2.44, P <.001; respectively). Furthermore, the FC of the studied proteins' expression significantly and positively correlated with FC of their mRNA expression (P = .001, r = .75). Conclusions: TREM-1 associated neutrophilic signaling pathway proteins are significantly suppressed in eosinophilic CRSwNP.
IgD-activated Mast Cells Induce IgE Synthesis in B Cells in Nasal Polyps

Abstract:Background: Although upregulated expression of local IgD has been reported in patients with chronic rhinosinusitis (CRS), its function is unclear. Objective: We sought to explore the expression and function of soluble IgD in patients with CRS, particularly CRS with nasal polyps. Methods: IgD levels in sinonasal mucosa were analyzed by using RT-PCR and ELISA. Numbers and phenotypes of IgD(+) cells were studied by means of immunohistochemistry, immunofluorescence, and flow cytometry. HMC-1 cells, a human mast cell line, and mast cells purified from eosinophilic polyps were cultured alone or with naive B cells purified from peripheral blood. The antigen specificity of nasal IgD was investigated by using ELISA. Results: The mRNA expression of immunoglobulin heavy constant delta gene, numbers of IgD(+) cells, and protein levels of secretory IgD in sinonasal mucosa were increased in patients with CRS with or without nasal polyps compared with control subjects. Numbers of IgD(+) plasmablasts were increased in both eosinophilic and noneosinophilic polyps, whereas numbers of IgD(+) mast cells were only increased in eosinophilic polyps. Cross-linking IgD induced serum preincubated HMC-1 cells and polyp mast cells to produce B-cell activating factor, IL-21, IL-4, and IL-13 and to promote IgM, IgG, IgA, and IgE production from B cells. In eosinophilic polyps expression of those B cell-stimulating factors in mast cells and close contact between mast cells and B cells were found. Moreover, positive correlations of total IgD levels with total IgE levels and eosinophilia and upregulation of specific IgD against house dust mites were discovered in eosinophilic polyps. Conclusion: IgD-activated mast cells can facilitate IgE production and eosinophilic inflammation in patients with CRS with nasal polyps.

Abstract:BACKGROUND: Total and specific immunoglobulin E (IgE) in nasal polyps (NP) is related to local eosinophilic inflammation. However, it remains unclear which antigens cause a stronger accumulation of eosinophils in NP. We investigated whether local IgEs to specific antigens correlated with local eosinophil accumulation.METHODS: Total IgE, antigen-specific IgEs [5 species of fungi, staphylococcal enterotoxins A and B (SEA and SEB), cedar pollen, ragweed pollen and house-dust mites] and eosinophil cationic protein (ECP) were measured in sinus mucosa homogenates and serum from 29 patients with chronic rhinosinusitis with NP (CRSwNP), 8 patients with allergic fungal rhinosinusitis (AFRS) and 8 control subjects without CRS. The level of ECP in NP was analyzed for correlations with the total and specific IgE levels in the serum and NP.RESULTS: In CRSwNP, antigen-specific IgEs [Alternaria > mite (Dp) > Aspergillus > SEB > Penicillium > mite (Df), but not the other fungi, SEA or pollens] in the sinus mucosa correlated significantly with the local ECP. In AFRS, antigen-specific IgEs [Penicillium > mite (Df) > Alternaria, but not the other fungi, SEs or pollens] in the sinus mucosa correlated significantly with the local ECP.CONCLUSIONS: Local total IgE and antigen (fungi, mites and SEs)-specific IgEs in NP are related to the local eosinophilic inflammation of AFRS and CRSwNP.

Abstract:T-lymphocyte subpopulations and B7-H1/programmed death-1 (PD-1) positive lymphocytes infiltrating nasal polyps were evaluated in 17 patients with chronic sinusitis with nasal polyposis. Peripheral blood samples were also obtained from the patients and from 11 healthy controls. The CD4(+), CD8(+), CD3(+), CD19(+), B7-H1(+) and PD-1(+) lymphocyte populations were measured using flow cytometry. Lymphocytes from nasal polyps had significantly fewer CD4(+) but significantly more CD8(+) T-cells compared with lymphocytes from the peripheral blood of patients and controls. The percentages of CD19(+)/B7-H1(+) B-cells and of CD3(+)/PD-1(+) T-cells were significantly higher in the nasal polyp samples than in those from peripheral blood of patients and controls. Changes in the T-lymphocyte subpopulations and in the up-regulation of B7-H1 and PD-1 in lymphocytes infiltrating nasal polyps may be involved in the development of the chronic inflammation associated with nasal polyposis.

Abstract:BACKGROUND: Local immunoglobulin hyperproduction is observed in nasal polyps (NPs) with and without ectopic lymphoid tissues (eLTs).OBJECTIVE: Our aim was to identify the T-cell subsets involved in local immunoglobulin production independent of eLTs in NPs.METHODS: The localization, abundance, and phenotype of CD4+ T-cell subsets were studied by immunofluorescence, flow cytometry, and single-cell RNA sequencing. Purified nasal T-cell subsets were cultured with autologous peripheral naive B cells to explore their function. Programmed death ligand 1 and programmed death ligand 2 expression in NPs was investigated by immunofluorescence staining and flow cytometry.RESULTS: Accumulation of PD-1highCXCR5-CD4+ T cells outside lymphoid aggregates was found in NPs. Nasal PD-1highCXCR5-CD4+ T cells were characterized by a unique phenotype that was related to B-cell help and tissue residency and distinct from PD-1-/intCXCR5- and CXCR5+ CD4+ T cells in NPs as well as PD-1highCXCR5highCD4+ follicular helper T cells in tonsils. Compared with the frequencies of PD-1highCXCR5-CD4+ T cells and their IFN-γ+, IL-17A+, and IL-21+ subsets in the control inferior turbinate tissues, the frequencies of these cells and their subsets were increased in both eosinophilic and noneosinophilic NPs, whereas the frequencies of the IL-4+ and IL-4+IL-21+ subsets were increased only in eosinophilic NPs. Nasal PD-1highCXCR5-CD4+ T cells induced immunoglobulin production from B cells in a potency comparable to that induced by tonsillar follicular helper T cells. PD-1highCXCR5-CD4+ T-cell frequencies were correlated with IgE levels in eosinophilic NPs. PD-L1 and PD-L2 suppressed the function of PD-1highCXCR5-CD4+ T cells, and their levels were reduced in NPs. PD-1highCXCR5-CD4+ T-cell abundance was associated with the postsurgical relapse of NPs.CONCLUSION: PD-1highCXCR5-CD4+ T cells participate in local immunoglobulin production independent of eLTs in NPs.

Abstract:Objective To evaluate the lymphocyte subpopulations both in nasal polyps and the peripheral blood in patients with nasal polyps,and to investigate whether there was a local mucosal immune system in nasal polyps.Methods The lymphocyte subpopulations and their production of cytokines such as interferon-gamma(IFN-γ) and interleukin-10(IL-10) was evaluated by flow cytometry in nasal polyps and the peripheral blood from 17 nasal polyps patients and peripheral blood from 11 healthy persons.Results There were fewer CD4+ T lymphocytes(24.01%±9.20%) and more CD8+ T lymphocytes(35.81%±6.79%) in nasal polyps than in the corresponding peripheral blood(CD4+:39.69%±7.41%;CD8+:27.23%±4.30%,P0.01).No significant difference was observed in CD16+ CD56+ cells and CD3+ cells among the three groups(P0.05).The expression level of T lymphocyte producing IFN-γ and IL-10 in nasal polyps was higher than in the peripheral blood(P0.01).Conclusions The difference in lymphocytes subpopulations and their production of cytokines between nasal polyps and peripheral blood might suggest that a local mucosal immune system be present in the nasal polyps.

Abstract:B-cell activating factor of the TNF family is critical for the survival and maturation of B cells and play a role in the pathophysiology of chronic rhinosinusitis with nasal polyps (CRSwNP). In this study, nasal tissues were enrolled from 25 CRSwNP patients (asthmatic, 16; non-asthmatic, 9), 12 CRSsNP patients and ten control subjects, respectively. The immunoreactivity of BAFF, CD20 and CD138 were examined using immunohistochemistry staining. The mRNA expression of BAFF, CD20, εGLT, AID, GATA3 and CRTH2 were examined using real-time RT-PCR. The protein levels of BAFF, IL-5 and IgE were measured using ELISA assays and the Unicap system, respectively. We found the numbers of BAFF+ cells, CD20+ cells (B cells) and CD138+ cells (plasma cells) were significantly increased in polyp tissues compared with control groups. The concentrations of BAFF, IgE and IL-5 in tissue homogenates were also significantly increased in polyp tissues compared with control groups, and the BAFF protein level in the polyp homogenates was significantly associated with the IgE and IL-5 levels and with concomitant asthma in CRSwNP patients. Our findings indicate that BAFF expression is significantly increased in CRSwNP patients and may orchestrate inflammatory load in polyp tissues by regulating T and B cell-mediated response.
Clinical Examination of Tissue Eosinophilia in Patients with Chronic Rhinosinusitis and Nasal Polyposis

Abstract:Objective (1) Describe clinical and histopathologic findings in patients with chronic rhinosinusitis with nasal polyps (CRSwNP). (2) Determine if tissue and serum eosinophilia predicts disease severity in CRSwNP. Study Design Case series with chart review. Setting Academic hospital specializing in respiratory and allergic disease. Subjects Patients with CRSwNP treated from 2008 to 2010. Methods Clinical data were collected; sinus computed tomography (CT) scans were scored according to the Lund-Mackay system; and surgical specimens were evaluated for degree of tissue eosinophilia. Statistical analysis was performed to compare eosinophilia with indicators of disease severity. Results Seventy CRSwNP patients were included, with a mean Lund-Mackay score of 16.7; 62.1% of patients had severe asthma, and 62.9% were aspirin sensitive. Elevated tissue eosinophil level did not correlate with medication usage, olfactory symptoms, or Lund-Mackay scores, nor did it correlate with presence of asthma or aspirin-sensitivity (P = .09). Patients with mild asthma had significantly more tissue eosinophils versus patients with severe asthma, possibly because of the high amount of chronic corticosteroid use in severe asthmatics. There was no correlation between tissue and serum eosinophil counts (P = .97), but there was a significant positive correlation between CT score and peripheral eosinophil level (P < .05). Conclusions Higher serum eosinophil levels may indicate more extensive mucosal disease as measured on CT scan. Neither serum nor tissue eosinophilia predicted disease severity in our retrospective analysis of CRSwNP patients, and serum eosinophil level did not serve as a marker of tissue eosinophilia.
Type 2 Inflammation Suppression by T-regulatory Cells Attenuates the Eosinophil Recruitment in Mucosa of Chronic Sinusitis.

Abstract:Type 2 inflammation and eosinophilic infiltration are prominent pathologic features of chronic rhinosinusitis with nasal polyps (CRSwNP). The purpose of the present study was to determine the roles of Tregs in controlling type 2 inflammation and inhibiting eosinophilic infiltration in CRSwNP. A total of 134 nasal polyps, 67 ostiomeatal complex from chronic rhinosinusitis (CRS) and 62 normal nasal tissues from controls were collected to study the enumeration and function of Tregs cells and the expressions of cytokine profiles via immunofluorescence staining, flow cytometry, qRT-PCR, ELISA, and/or H&E staining. The effects of Tregs on type2 and type3 inflammations were determined in an eosinophilic chronic sinusitis (ECRS) mice model. It was confirmed that the CRSwNP displayed the features of Th2 and Th17 cells-mediated inflammation, accompanying by an increased level of eosinophilic infiltration and the eosinophil cationic protein (ECP), with a decreased frequency of Treg cells. Furthermore, the percentages of CD4+CD25+CD127lowTreg and CD4+CD25+Foxp3+Treg were only decreased in the polyps of CRSwNP but not in the paired peripheral blood. The CRSwNP possessed the decreased Nrp1+Tregs, Helios+Treg, and low TGF-β and interleukin (IL)-10 expressions in Tregs. The ECRS mice showed similar inflammatory characteristics to CRSwNP patients. The adoptive transfer of CD4+CD25+Foxp3+ Treg cells significantly decreased the inflammatory cytokines, eosinophilic chemotactic factors in the mucosa of the ECRS mice without alteration of the immune balance in the peripheral blood and spleen. In conclusion, CRSwNP showed high type 2 and type3 inflammation and defective Tregs. The induced regulatory T cell (iTreg) may correct the imbalance between immune tolerance and effect via limiting the eosinophil recruitment of mucosa in CRSwNP.

Abstract:BACKGROUND:The role of atopy to aeroallergens in chronic rhinosinusitis without nasal polyps (CRSsNP) remains unclear. This study aimed to investigate the mucosal immunopathologic characteristics of CRSsNP with and without atopy to inhalant allergens.METHODS:Thirteen nonatopic CRSsNP patients, 9 atopic CRSsNP patients, and 11 nonatopic control subjects were enrolled in this study. The expression of type 1, 2, and 17 cytokines, growth factors, and chemokines for T cell subsets and granulocytes in sinonasal mucosa was detected using Bio-Plex suspension chip technology or enzyme-linked immunosorbent assay (ELISA). Subjective symptoms were scored on a visual analogue scale (VAS), while disease severity on computed tomography (CT) was graded by the Lund-Mackay CT scoring system.RESULTS:There was no significant difference in VAS and CT scores between atopic and nonatopic CRSsNP. Compared with control, both atopic and nonatopic CRSsNP demonstrated increased interferon γ (IFN-γ) levels in sinonasal mucosa. In contrast, although no difference in interleukin 5 (IL-5), IL-13 and eotaxin-1 expression, or mucosal eosinophil infiltration, was found between the control and whole CRSsNP group, atopic CRSsNP manifested an increased expression of IL-5, IL-13 and eotaxin-1, as well as an enhanced infiltration of mucosal eosinophils in comparison with control and nonatopic CRSsNP. Mucosal eosinophil infiltration correlated with IL-5 and eotaxin-1 expression. No difference in IL-12, IL-4, IL-6, IL-17A, IL-8, myeloperoxidase, "regulated upon activation normal T cell expressed and presumably secreted" (RANTES), or chemokine (C-X-C motif) ligand 10 (CXCL10) protein expression was found among control, atopic CRSsNP, and nonatopic CRSsNP.CONCLUSION:Atopic and nonatopic CRSsNP have distinct mucosal immunopathologic profiles. CRSsNP is a heterogeneous disorder consisting of multiple groups of biological subtypes, or "endotypes," which may argue for different therapeutic strategies.
