Treatment of Helicobacter pylori Infection: Current Status and Considerations

Abstract:Proton pump inhibitor (PPI)-based triple therapy has been recommended as first choice for eradication of Helicobacter pylori (H. pylori), but increasing antibiotics resistance worldwide has made this therapy confronting challenge. It is the time for us to re-evaluate the international and national consensus on the management of H. pylori infection. An objective specific grading criteria would allow us to evaluate the effectiveness of the recommended regimens, of which the intention-to-treat (ITT) eradication rate more than 80% is only minimal requisite of the acceptable eradication rate, and higher eradication rate should be our goal. Currently, metronidazole resistance rate has been generally over 40% and clarithomycin resistance rate varies widely in different regions of our country. Clinical studies on treatment of H. pylori infection should be further emphasized here, not only for the re-evaluation of traditional therapy, but also for the search of more effective new therapies. Currently, numerous promising new therapies have already been recommended for further study and evaluation, both at home and abroad.
