Abstract:Objective To study the influence of different immune adjuvants on the switch of anti- body classes against pemphigus vulgaris antigen ECI-2 epitope(PVA ECI-2).Methods PVA ECI-2 fu- sion protein,combined with complete Freund's adjuvant(CFA)or aluminum hydroxide hydrate[Al (OH)_3],was used to immunize C57BL/6 mice.After immunization,the titers and subclasses of specific antibody as well as related cytokines were measured.The affinity of antibody subclasses with antigen and pathogenicity of antibody subclasses were studied by indiret immune-fluorescence(IIF)and in vitro culture of skin graft from neonatal mice,respectively.Results After immunization,the level of IFN-γa Thl type cytokine,was elevated in CFA group,while the level of IL-4,a Th2 type cytokine,was increased in A1 (OH)3 group.Specific antibodies were detected in both groups of mice.IgG2a subclass predominated in CFA group,but only IgG1 subclass was detected in the A1(OH)_3 group.IIF showed a weak fluorescence between the squamous epithelial cells with the addition of antisera from CFA group,but a relatively bright fluorescence with the addition of antisera from the AI(OH)_3 group.Furthermore,the in vitro culture of skin graft with the antisera from the AI(OH)_3 group showed intraepithelial blister,but that from the CFA group only showed mild acantholysis.Conclusion Different immune adjuvants may have direct influence on the antibody subclasses against PVA ECl-2.

Role of Th17 and Treg cells in the pathogenesis of pemphigus

Abstract:Objective: To investigate the role of Th17 and Treg (regulatory T) cells in immune mediated mechanism of pemphigus's pathogenesis. Methods: Peripheral blood mononuclear cells were prepared from 51 patients with pemphigus as well as 26 normal controls, and the frequencies of Th17 cells (CD3+CD8 -IL-17A+, ie, CD3+CD4+IL-17A +) and Treg cells(CD4+CD25hiFoxp3+) were determined by flow cytometry. Serum specific Dsg1, Dsg3 antibodies and subtypes of patients were detected by ELISA. And the relationship of Th17 cells, Treg cells and the antibodies were analysised. Results: Peripheral blood from pemphigus patients showed significantly higher levels of Th17 cells than that from normal controls (P = 0.014), specially in acute phase of pemphigus (P = 0.015 vs. normal control); The levels of Treg cells in both acute and stable phases of pemphigus were significantly lower than that in normal control group (P < 0.001 for acute and P < 0.05 for stable); Moreover, the levels of Treg cells were lower in acute than in stable pemphigus (P = 0.024). The levels of Th17 cells in peripheral blood of patients with pemphigus were negatively correlated with the levels of Treg cells (r = -0.532, P < 0.001). Anti-Dsg1 and -Dsg3 antibody titers were higher in acute phase than that in stable phase patients. Subtype antibody ratio of IgG4 to IgG1 increased in acute phase. Dsg1 and Dsg3 antibody titers, and subtype antibody didn't show any correlation with Th17 and Treg cells. Conclusion: Imbalance of Th17 and Treg cells in peripheral blood of pemphigus patients involves in pathogenesis of pemphigus. Antibody titers rise significantly during acute phase, and IgG4 subtype plays an important role in the pathogenesis of pemphigus.
The Intervention Of Immune Adjuvants On The Pathogenesis Of Pemphigus Vulgaris Antibody

Abstract:Pemphigus is a severe blistering disease. The antibody titer and the distinct isotype patterns correlated with the disease activity. To identify their functional properties and pathogenic potential, we immunized C57BL/6 mice with recombinant Dsg3 fusion protein plus complete Freund's adjuvant (CFA) or Aluminum Hydroxide hydrate (Alum). After immunization, the cytokine profiles on T cells, the antibody titers, and the isotypes were analyzed. The pathogenicity of different autoantibody isotypes were evaluated by antibody passive transfer approach. It was disclosed that IgG1 expression was persistent in the Alum group while IgG2a was predominate in the CFA group. Neonatal mice injected with sera from the Alum group and not the CFA group developed skin lesions clinically and histologically with IgG deposition on the epidermal keratinocytes. Our findings shed new light on the immunopathogenesis of PV and offer a new therapeutic strategy for this potentially fatal disorder.

Abstract:Pemphigus vulgaris (PV) is a severe autoimmune bullous disease involving both the skin and mucosal areas, and characterized by intraepithelial flaccid blisters and erosions. The pathogenesis of this disease is not yet completely understood, but novel insights into desmoglein biology and autoantibody pathogenesis have recently been published. Acantholysis in PV seems to result from a collective action of autoantibodies against various keratinocyte self antigens, of which desmogleins 1 and 3 are the most important. Additional antigens including desmocollins and nondesmosome components, such as the mitochondrion, might take part in disease activation. Recently, apoptosis was reported as a possible underlying mechanism of acantholysis. Furthermore, apoptolysis is believed to be the link between suprabasal acantholytic and cell-death pathways. We review the possible hypotheses of the pathogenesis of PV: the desmoglein compensation theory, the antibody-induced apoptosis theory, the basal-cell shrinkage hypothesis and the newly published apoptolysis theory.

Abstract:BACKGROUND: Pemphigus foliaceus (PF) and pemphigus vulgaris (PV) are closely related, but clinically distinct, autoimmune blistering diseases caused by autoantibodies against desmoglein (Dsg)1 and Dsg3, respectively. Using ethylenediaminetetraacetic acid (EDTA)-treated Dsg3 enzyme-linked immunosorbent assay (ELISA) we have shown that the proportion of anti-Dsg3 antibodies against calcium-dependent epitopes decreased upon shifting to the inactive phase in patients with PV.OBJECTIVES: To analyse the epitope profiles of anti-Dsg1 antibodies across the different activity stages of PF.METHODS: We evaluated five patients with PF who retained high serum levels of anti-Dsg1 antibodies in the inactive phase. Sera were obtained in both the active and inactive phases, and were analysed by EDTA-treated and exfoliative toxin-treated ELISAs. To map the epitopes of anti-Dsg1 antibodies, immunoprecipitation-immunoblotting was performed using a set of Dsg1/Dsg2 domain-swapped molecules.RESULTS: Anti-Dsg1 antibodies against the calcium-dependent epitopes of Dsg1 were the predominant antibodies in both the active and inactive phases. The proportion of anti-Dsg1 antibodies against the calcium-dependent epitopes did not change upon shifting to the inactive phase. The results of immunoprecipitation-immunoblotting showed that most of the anti-Dsg1 antibodies bound to the extracellular domains (EC)1-2 of Dsg1.CONCLUSIONS: In patients with PF, the calcium-dependent epitopes on EC1 and EC2 of Dsg1 contained definitively pathogenic and nonpathogenic epitopes. The disease activity might be differentially controlled by the antibodies between PF and PV depending on the presence or absence of the nonpathogenic epitope.

Abstract:Background Anti‐p200 pemphigoid is a rare autoimmune subepidermal blistering disease. Although the phenomenon of epitope spreading has been reported to be common in anti‐p200 pemphigoid, the association between its clinical and immunoserological features has yet to be elucidated. Objectives Our aim was to compare the clinical and immunoserological characteristics of anti‐p200 pemphigoid patients with and without epitope spreading. Methods We performed a retrospective cohort study encompassing 30 patients with anti‐p200 pemphigoid between January 2015 and December 2022. The clinical and immunoserological characteristics of anti‐p200 pemphigoid were analyzed using combined immunoserological assays. Results Epitope spreading was observed in 11 of 30 patients (36.7%) with anti‐p200 pemphigoid. Compared with patients in the non‐epitope spreading group, patients in the epitope spreading group showed more heterogeneous clinical presentations (P = 0.018), a higher proportion of mucosal involvement (P = 0.003), higher Bullous Pemphigoid Disease Area Index (BPDAI) scores for skin erosions/blisters (P = 0.018), mucosal erosions/blisters (P = 0.001), activity (P = 0.017) and total scores (P = 0.022), and required a higher initial dose of prednisone for disease control (P = 0.040). Conclusions This study supported the idea that anti‐p200 pemphigoid was prone to epitope spreading. Anti‐p200 pemphigoid patients with epitope spreading are more likely to present heterogeneous clinical phenotypes, frequent mucosal involvement, and a more severe and recalcitrant disease course.

Correlation between disease activity and pemphigus-specific antibody subclasses in patients with pemphigus

Abstract:Objective To assess the correlation between the subclasses of antibodies against desmoglein (Dsg) 1 and Dsg3 and disease activity in patients with pemphigus. Methods Sera were collected from 47 patients with pemphigus, and ELISA was performed to determine the titers and subclasses of antibodies against Dsg1 and Dsg3. The correlation of antibody titers and subclasses with disease activity was assessed.Results Clinical phenotype was associated with antibody profiles in these patients with pemphigus. Of 17 patients with mucocutaneous involvement, 14 (82.4%) had both anti-Dsg3 and anti-Dsg1 antibodies; of 16 patients with cutaneous involvement, 15 (93.7%) had anti-Dsg1 antibody, only 1 (6.3%) developed anti-Dsg3 antibody; of 6 patients with mucosal involvement, all ( 100% ) had only anti-Dsg3 antibody. The serum levels of antibodies against Dsg1 and Dsg3 were increased, but not in parallel with the disease severity in these patients.Moreover, the subclasses of antibodies were correlated with disease severity. IgG4 subclass antibodies against Dsg1 and Dsg3 predominated in patients with pemphigus at active stage, whereas IgG1 subclass in those at remission stage. The serum levels (expressed as absorbance value) of IgG4 and IgG1 subclass antibodies were 1.92 ± 1.21 and 0.60 ± 0.61 respectively, with the ratio of IgG4 to IgG1 more than 1, in patients with antiDsg1 antibodies at active stage, 0.03 ± 0.02 and 0.22 ± 0.11 respectively with the ratio of IgG4 to IgG1 less than 1, in those at remission stage, 2.35 ± 2.17 and 1.84 ± 1.16 respectively with the ratio of IgG4 to IgG1 more than 1 in patients with anti-Dsg3 antibodies at active stage, 0.15 ± 0.16 and 1.05 ± 0.77 respectively with the ratio of IgG4 to IgG1 less than 1 in those at remission stage. Conclusions The subclasses of pemphigus-specific antibodies are closely correlated with the disease severity in patients with pemphigus. The detection of subclasses and titers of anti-Dsg1 and anti-Dsg3 antibodies may aid in the diagnosis of and monitoring of disease severity in pemphigus.
Relationship of regulatory B cells with pathogenic antibodies in pemphigus

Abstract:Objective To explore the mechanisms underlying the mediating effect of regulatory B (Breg)cells in the pathogenesis of pemphigus.Methods Peripheral blood specimens were collected from 48 patients with pemphigus at different degrees of severity and 20 healthy controls.Flow cytometry was conducted to analyze the frequency of Breg(CD19 +CD24hiCD38hi)cells in peripheral CD19+ B cells,enzyme linked immunosorbent assay(ELISA)to determine the titer and subtypes of anti-desmoglein(Dsg)1 and 3 antibodies in sera from these subjects.The relationship of anti-Dsg 1 and 3 antibodies with the frequency of Breg cells was assessed by Spearman's rank correlation test.Results Significant differences were observed in the proportion of Breg cells in CD19+ B cells between patients with pemphigus and healthy controls(11.54％ ± 0.97％ vs.8.19％ ± 0.85％,P =0.04),as well as between patients with acute mild pemphigus and those with moderate and severe pemphigus(5.17％ ± 2.14％ vs.11.38％ ± 5.30％ and 11.17％ ± 5.31％,P =0.042,0.046,respectively).The frequency of Breg cells was positively correlated with the absorbance value for anti-Dsg1 antibodies of IgG4 subclass and IgG4/IgG1 ratio(r =0.527,0.565,respectively,both P ＜ 0.01).Conclusions Breg cells are mainly associated with the production of antibodies in and disease severity of pemphigus,while the actual mechanism of action remains unknown.

Abstract:Pemphigus is an autoimmune blistering disease of skin and mucous membranes. The classic types of pemphigus are pemphigus vulgaris and pemphigus foliaceus. In this review we summarize recent advancement in the etiology and the pathogenesis of pemphigus. Desmogleins--transmembrane glycoproteins involved in intracellular adhesion--were recognized as targets of pemphigus antibodies. It was found that the distribution and the expression of desmogleins can explain the difference in the localization of lesions in pemphigus vulgaris and pemphigus foliaceus. Pemphigus develops in a two-step process. The first step leads to the presence of a low titer of autoantibody, the second step results in a significant increase in the antibody titer which causes the clinical stage of the disease. Selective presentation of self peptides can explain the Major Histocompatibility Complex (MHC)--linked susceptibility to autoimmune diseases including pemphigus and rheumatoid arthritis. Peptides selective for the disease-associated molecules can be identified and used to search for microbiologic factors that can take part in the pathogenesis of pemphigus.
Modulation of desmoglein-3-specific T cell response and pathogenic antibody generation in mice.

Abstract:Pemphigus is a severe blistering disease of the skin and mucous membranes caused by pathogenic autoantibodies to desmosomal adhesion proteins desmoglein-3 (Dsg3) and desmoglein-1 (Dsg1). The antibody titer and the distinct isotype patterns correlated with the disease activity. To identify their functional properties and pathogenic potential, we immunized C57BL/6 and Balb/c mice with recombinant Dsg3 fusion protein plus complete Freund's adjuvant (CFA) or Aluminum Hydroxide hydrate (Alum). After immunization, the cytokine profiles on T cells, the antibody titers, and the isotypes were analyzed. The pathogenicity of different autoantibody isotypes was evaluated by antibody passive transfer approach. It was found that Th1 type cytokine interferon γ (IFN γ) was elevated in the CFA-treated group, while Th2 type cytokine interleukin-4 (IL-4) was increased in the Alum-treated group. IgG1 expression was persistent in the Alum group while IgG2a was predominant in the CFA group. Neonatal mice transferred with sera from the Alum group, but not the CFA group, developed skin lesions clinically and histologically with IgG deposition on the epidermal keratinocytes. Our findings suggest that distinct T cell responses could be switched after active immunization combined with different adjuvants, resulting in distinct anti-Dsg3 antibody isotypes with different pathogenic activities in disease development. These findings shed new light on the immunopathogenesis of PV and offer a new therapeutic strategy for this potentially fatal disorder.

Abstract:Pemphigoid diseases (PD) are autoimmune skin blistering diseases characterized by autoantibodies directed against proteins of the cutaneous basement membrane zone (BMZ). One of the major antigens is type XVII collagen (BP180), a transmembrane glycoprotein, which is targeted in four PDs: bullous pemphigoid, mucous membrane pemphigoid, linear IgA dermatosis, and pemphigoid gestationis. To date, different epitopes on BP180 have been described to be recognized by PD disease patients’ autoantibodies. Different BP180 epitopes were associated with distinct clinical phenotypes while the underlying mechanisms are not yet fully understood. So far, the main effects of anti-BP180 reactivity are mediated by Fcγ-receptors on immune cells. More precisely, the autoantibody–antigen interaction leads to activation of complement at the BMZ and infiltration of immune cells into the upper dermis and, by the release of specific enzymes and reactive oxygen species, to the degradation of BP180 and other BMZ components, finally manifesting as blisters and erosions. On the other hand, inflammatory responses independent of Fcγ-receptors have also been reported, including the release of proinflammatory cytokines and internalization and depletion of BP180. Autoantibodies against BP180 can also be found in patients with neurological diseases. The assumption that the clinical expression of PD depends on epitope specificity in addition to target antigens, autoantibody isotypes, and antibody glycosylation is supported by the observation that epitopes of PD patients differ from those of PD patients. The aim of the present review is to describe the fine specificities of anti-BP180 autoantibodies in different PDs and highlight the associated clinical differences. Furthermore, the direct effects after binding of the autoantibodies to their target are summarized.

Molecular Cloning and Protein Expression of Ec1-2 and Ec3-4 Epitopes of Pemphigus Vulgaris Antigen

Abstract:Objective To clone and express EC1-2 and EC3-4 epitopes of PVA ( pemphigus vulgaris antigen, desmoglein-3) in order to diagnose pemphigus and study the relationship between epitopes of PVA and anti-PVA antibody.Methods RNA was extracted from keratinocytes and the cDNA of epitopes EC1-2 and EC3-4 was synthesized by reverse transcription. Amplified genes of EC1-2 and EC3-4 were inserted into the expression plasmid, PGEX-4T-1, and transformed into E. coli BL21 by electric transduction. Recombinant fusion proteins of EC1-2 and EC3-4 epitopes were expressed by IPTG induction. These proteins were separated on SDS-PAGE gels and electroblotted to nitrocellulose to detect the anti-PVA antibody.Results The sequences of cloned EC1-2 and EC3-4 genes were identical to the sequence registered in PC/GENE. Expressed recombinant proteins reacted only to sera from patients with pemphigus vulgaris, not to sera from patients with bullous pemphigoid, systemic lupus erythematosus or normal persons.Conclusions These recombinant proteins are very specific in antigenicity. This may provide a new method for the diagnosis of pemphigus vulgaris (PV) or the differential diagnosis of other bullous cutaneous diseases via patient sera. It is also helpful in understanding the relationship between adhesion molecules and the pathogenic mechanism of pemphigus vulgaris.
The Imbalance of Th17 and Regulatory T Cells in Pemphigus Patients

Abstract:Pemphigus is a complex dermatologic autoimmune bullous disease whose pathogenic mechanism is not fully understood. Anti-desmoglein-1 (Dsg1) and anti- Dsg3 autoantibodies play an important role in the pathogenesis of pemphigus. Objectives: To investigate the role of T-helper17 (Th17) and regulatory T (Treg) cells in the pathogenesis of pemphigus in fifty-one patients and twenty-six healthy individuals (control group). Methods: Levels of CD3(+)CD8(-) IL-17 expressing Th cells and CD4(+)CD25(hi)Foxp3(+) Treg cells were determined by FACS in both groups, along with anti-Dsg1 and Dsg3 antibody titers. An analysis of the correlation between Th17 and Treg cells was performed. Results: Th17 cell numbers were significantly higher in pemphigus patients than in normal controls (P = 0.014), especially in the acute onset and chronic active stages (P = 0.004 and 0.022). Conversely, Treg cells in pemphigus patients were significantly fewer than in the control group (P<0.001). The same trend was observed between the acute onset and the remittent stage patients (P = 0.006). We found a negative correlation between Th17 and Treg cell populations (r = -0.532, P<0.001). Anti-Dsg1 and Dsg3 antibody titers were higher in patients in the active stage than in the remittent stage, with an increased IgG4/IgG1 subclass ratio. There was no statistically significant correlation between Th17/Treg ratios and anti-Dsg1 or Dsg3 antibody titers. Conclusion: These findings show an imbalance of Th17 and Treg cell populations in pemphigus patients, which might result in the activation and proliferation of effector T cells, further up-regulating B cell activity and antibody production.

Abstract:BACKGROUND:Autoantibodies against N-terminal domains and linker subdomains of envoplakin (EVPL) and periplakin (PPL) were frequently detected in sera of paraneoplastic pemphigus(PNP) patients.OBJECTIVES:To further investigate finer epitopes of EVPL and PPL, and evaluate their associations with clinical aspects of PNP.METHODS:We produced 12 overlapping truncated fragments of these regions in Escherichia coli, and measured their reactivities to sera of 65 PNP patients and 50 healthy volunteers by enzyme-linked immunosorbent assays (ELISA). Then appropriate statistics were performed to evaluate the correlation between antibodies against different fragments and clinical information of patients.RESULTS:EVPL-N1 (aa1-141) and EVPL-L3 (aa1684-1784) were recognized by PNP sera at the same sensitivity of 75.38% (49/65) with specificities of 98% and 92%, respectively. Although neoplasm types were not associated with any fragment, the ELISA of these fragments and indirect immunofluorescence on rat bladder complemented each other in detecting PNP. Meanwhile, levels of autoantibodies against EVPL-N3 were elevated with PNP accompanied with bronchiolitis obliterans or presented with lichen planus-like lesions (P<0.05). No influence of autoantibodies against any fragments on prognosis of the patients was observed by Cox regression test, though antibodies against some fragments were higher in the dead patients.CONCLUSIONS:The study proved antigenic epitopes were mainly concentrated on EVPL-N1 and EVPL-L3 in PNP. Autoantibodies against EVPL-N3 might associate with those patients accompanied with bronchiolitis obliterans or presented with lichen planus-like lesions.
Integrative Single-Cell Analysis Reveals Distinct Adaptive Immune Signatures in the Cutaneous Lesions of Pemphigus

Abstract:Pemphigus, an autoimmune bullous disease affecting the skin and mucosal membranes, is primarily driven by anti-desmoglein (Dsg) autoantibodies. However, the underlying immune mechanisms of this disease remain largely elusive. Here, we compile an unbiased atlas of immune cells in pemphigus cutaneous lesions at single-cell resolution. We reveal clonally expanded antibody-secreting cells (ASCs) that exhibit variable hypermutation and accumulation of IgG4 class-switching in their immunoglobulin genes. Importantly, pathogenic Dsg-specific ASCs are localized within pemphigus lesions and can evolve from both Dsg-autoreactive and non-binding precursors. We observe an altered distribution of CD4+ T cell subsets within pemphigus lesions, including an imbalance of Th17/Th2 cells. Significantly, we identify a distinct subpopulation of Th17 cells expressing CXCL13 and IL-21 within pemphigus lesions, implying its pivotal role in B cell recruitment and local production of autoantibodies. Furthermore, we characterize multiple clonally expanded CD8+ subpopulations, including effector GMZB+ and GMZK+ subsets with augmented cytotoxic activities, within pemphigus lesions. Chemokine-receptor mapping uncovers cell-type-specific signaling programs involved in the recruitment of T/B cells within pemphigus lesions. Our findings significantly contribute to advancing the understanding of the heterogeneous immune microenvironment and the pathogenesis of pemphigus cutaneous lesions, thereby providing valuable insights for potential therapeutic interventions in this disease.

Abstract:Ectopic lymphoid aggregates could be observed in several autoimmune diseases (e.g. rheumatoid arthritis). Pemphigus is an organ specific autoimmune disease and caused by circulating antibodies. Little is known about the features and function of lesional lymphocytes. The aim of this study is to illustrate the lymphoid neogenesis and B cell phenotype in pemphigus skin lesions. Methods: 197 HE-stained sections of pemphigus vulgaris (PV) and pemphigus foliaceus (PF) skin lesions in our department were collected. The histological grading of lymphoid aggregates was assessed according to a published grading system with grade 1 aggregates displaying a radial cell number between 2 and 5 cells, grade 2 between 6 and 10 and grade 3 greater than 10. Immunohistochemistry staining for CD3, CD20 and CD138 was proceeded and T, B and plasma cells were checked in selected 29 cases. Meanwhile, the B cells and plasma cells were compared between lesional skin and peripheral blood from the same patient by flow cytometry. Results: Grade 1 lymphoid neogenesis was observed in 195 of 197 cases (99.0 %), grade 2 in 133 of 197 (67.5 %) and grade 3 in 23 of 197 (11.7 %). There was no significant difference between PV and PF. The increasing B cell and plasma cells could be detected by immunohistochemistry in parallel to the grade of lymphoid aggregates. Compared to the peripheral blood, the lesional CD27+ B cell and plasma cell were significantly increased. Conclusions: Ectopic lymphoid aggregates could be detected in the skin lesion of pemphigus and the major B cell subset are memory B cells which might play a pathogenic role in the local immune response.
Antigen-specific helper T cells in patients with pemphigus vulgaris

Abstract:Objective To study the changes of antigen-specific T helper type 1 (Th1) cells and Th2 cells in patients with pemphigus vulgaris (PV) at different stages,so as to better understand the roles of autoreactive T cells in PV.Methods The DG3 (96-112) peptide was synthesized.Twenty-four patients with PV and 10 health checkup examinees were included in this study.Peripheral blood mononuclear cells (PBMCs) were obtained from the health checkup examinees and all the patients before treatment and seven patients about one month after treatment,and stimulated with the DG3 peptide of 10 mg/L for different durations.Then,enzyme-linked immunospot (ELISPOT) assay was performed to count the number of DG3-specific IFN-γ+ Th1 cells,IL-4+ Th2 cells and memory B cells.One-way analysis of variance (ANOVA) and t test were done to compare the number of cells between different groups,and Pearson correlation coefficient was used to evaluate the correlation among Th cells,memory B cells and anti-desmoglein 3 (Dsg3) antibody titers.Results In these patients,the male to female ratio was 1.67 ∶ 1,and the average age was (56.59 ± 14.66) years.Compared with the health check-up examinees,the patients with PV showed a higher absolute number of DG3-specific IFN-γ+ Th1 cells (420.18 ± 350.29 vs.145.12 ± 86.56,t =3.25,P ＜ 0.05),but a similar absolute number of specific IL-4+ Th2 cells (366.76 ± 192.44 vs.335.88 ± 164.96,P＞ 0.05) per 5 × 105 PBMCs.The percentage of DG3-specific IL-4+ Th2 cells in Th2 cells was 37.03％ ± 23.44％,and the percentage of IFN-γ+ Th1 cells was 23.62％ ± 16.77％ in peripheral blood of patients with PV.The number of DG3-specific IL-4+ Th2 cells per 5 × 105 PBMCs significantly decreased from 452.82 ± 199.29 before treatment to 241.68 ± 160.60 after treatment in seven patients (t =2.48,P ＜ 0.05).There was no significant correlation between specific Th cells,memory B cells and anti-Dsg3 antibody titers (all P ＞ 0.05).Conclusions The peptide DG3 (96-112) has pathogenic epitopes which could be recognized by specific Th cells of patients with PV.Both antigen-specific IFN-γ+ Th1 cells and IL-4+ Th2 cells play certain roles in the pathogenesis of pemphigns vulgaris,and IL-4+ Th2 cells appear to be more important.

Abstract:Pemphigus is an organ-specific autoimmune disease that targets skin and/or mucous membranes. Our previous study showed that infiltrating lymphocytes in pemphigus vulgaris (PV) lesions produce anti-desmoglein (Dsg) 1/3 antibodies after in vitro culture. In this study, we found diffuse ectopic lymphoid-like structures (ELSs) commonly present in the lesions of both PV and pemphigus foliaceus. Notably, pemphigus lesions contained centroblasts, plasmablasts, and plasma cells, which recapitulated the different stages of B cell differentiation. Elevated mRNA expression levels of the differentiation-related transcription factors BLIMP-1, IRF4, and BCL-6 were observed in pemphigus lesions. Moreover, B cell receptor repertoire analysis revealed the clonal expansion of the lesional B cells. Lesional B cells might recirculate among lesions, lymph nodes, and peripheral blood. Increased mRNA expression levels of multiple chemokines in pemphigus lesions and elevated expression levels of chemokine receptors on lesional B cells were also observed. Collectively, these results show that the ELSs in pemphigus lesions might act as a niche, supporting in situ B cell differentiation and clonal expansion.
The Study on Anti-Dsg1, Dsg3 Antibody Subclass Production and Cytokines Expression in the Patients with Paraneoplastic Pemphigus

Abstract:Objective To investigate antibody titers against Desmoglein1 and3( Dsg1 and3) and the IgG subclasses in serum and levels of IFN-γ,IL-4 in peripheral blood mononuelear cells ( PBMC) in the patients with paraneoplastic pemphigus ( PNP) in different stage. Methods Six patients with PNP,ten patients with pemphigus vugalris( PV) ,ten patients with pemphigus foliaceus( PF) and tewenty healthy people were enrolled in the study. Antibodies against Dsg1 and Dsg 3 and subclasses of the antibodies in serum were evaluated by ELISA. T cell cytokines,including IFN-γ and IL-4 were measured by flow cytometry ( FCM) . Results 1 Antibody titers against Dsg 1 and Dsg 3 in acute patients with PNP were remarkably increased compared with those in remission ( P 0. 05) . 2 IgG1 subclass in acute patients with PNP was at higher level and the ratio of IgG1/ IgG4 in acute patients with PNP was reversed to that in the patients with PV ( ratio of anti-Dsg1 IgG1/IgG4: P = 0. 001; ratio of anti-Dsg3 IgG1/ IgG4: P 0. 05) and PF ( ratio of anti-Dsg1 IgG1/IgG4: P 0. 01) . 3 The IFN-γ expression from CD8 + T cells or CD4 + T cells was significantly higher in PNP patients compared with that in controls ( P =0. 001) ,and it was remakably decreased in remission patients compared with acute patients( P 0. 05) . The IL-4 expression from CD4 + T cell was significantly lower in PNP group compared with PV( P 0. 05) and PF( P 0. 01) but there is no significant difference in acute or remission patients( P 0. 05) . Conclusion The subclasses of antibodies against Dsg1 and Dsg3 and cytokines expression in PNP patients are quite different when compared with PV and PF patients.
