Interventional therapy for patent ductus arteriosus with home-made septal occluders in 93 cases: Evaluation of curative effect and safety

Abstract:AIM: To observe the curative effect of non-surgical closure of patent ductus arteriosus (PDA) with septal occluders. METHODS: Ninety-three patients, who underwent interventional therapy with septal occluders in Shaanxi Provincial Corps Hospital, Chinese People's Armed Police Forces, were selected from January 2003 to December 2006, including 42 males and 51 females, aged 2-46 years, and 7-65 kg. The ductus diameters were ranging from 1.0 mm to 19 mm, and the pulmonary artery pressures were 2.39-14.36 kPa. The septal occluders (Shanghai Shape Memory Alloy Co., Ltd) were 4 mm to 30 mm. All the patients were examined with B-ultrasound before operation. Pulmonary arterial pressure test and aortic arch visualizations were performed during and after operation. RESULTS: Among 93 cases, 92 were followed up for one month, 87 for 3 months, and 57 for 6 months. ①Interventional therapies were successful except 1 case quitting for the treatment due to the diameter of persistent ductus was less than 1 mm. ②The proper size of septal occluders was identified according to the findings from B-ultrasound and aortic arch visualizations with 2-4 mm over the ductus diameter, and 6-8 mm over the window. ③No residual shunt or noise was found in 91 cases 15 minutes after operation, but residual shunt and noise were still heard some times in 1 case, and disappeared in the next day. ④Fifty-seven cases were followed up for more than half a year and no complication occurred in 56 of them. Anemia was found in 1 case one month after therapy, and restored after treatment including hemostasis, blood transfusion, and anti-inflammation. CONCLUSION: The septal occluders (made in China) are safe and effective for closure of PDA. No adverse effect like the biocompatibility of materials and body is found during the half-year follow up. It could serve as one of the methods for PDA.
