Hip movement and force distribution in the acetabulum

Abstract:Aim:To study the relation between hip movement and force distribution in the acetabulum, in order to provide evidences for the rational design of early postoperative rehabilitation for acetabular fracture. Methods: The experiment was carried out in the Institute of Biomechanics at Shanghai University from November to December in 2005. 1 Six adult pelvis after corrosion and wetting protection were adopted in this study, including hip joint and proximal 1/3 femur. 2 The pelvis were sawed asunder into two hip joints, totally 12 hip samples for fracture models of acetabular posterior wall. Then the fracture was fixed with steel plate. Loading test (150-600 N) was performed in the biomechanical models in the neutral position, 30° flexion and 15°, extension, respectively. 3 The force distribution on the postoperative fracture fragments of acetabular posterior wall under the conditions of different hip movement were researched by the means of biomechanics. Results: The displacement and stress in the fracture fragments of acetabular posterior wall changed with the hip movement under a load of 600 N. They were smaller in the neutral position, bigger in the flexed hip and the biggest in the extended hip, indicating significant differences (P < 0.05). Conclusion: Force distribution in the acetabulum is closely related to hip movement. A rational early postoperative rehabilitation of acetabular fracture should be personalized according to fracture site, fracture comminution and rigidity of internal fixation.
