Correlation between hypothalamus-pituitary-adrenal gland and depression

Abstract:Objective: To analyze synthetically the functional change of hypothalamus-pituitary-adrenal gland (HPA) and its correlation with depression. Data Sources: A computer-based online search of Medline database was undertaken to identify articles about the dependability between HPA and depression published in English by using the keywords of "HPA, Depression/Depressive Disorder" from January 1988 to December 2003. Meanwhile, Chinese articles about the dependability between HPA and depression were searched in Wanfang database and China Journal Full-text Database (CJFD) with the key words of "HPA,Depression/Depressive Disorder" in Chinese between January 1999 and December 2005. Study Selection: Inclusive criteria: 1 Researches of randomized blind control design, before and after control design and retrospective.2 Researches of the change of the function of HPA in patients with depression, the interaction between HPA and monoamine neurotransmitter, the contribution to the secretory melatonin, to diagnose depression, to predict therapeutic efficacy and relapse of symptom, the dependability between HPA and suicide. Exclusive criteria: the repetitive researches and individual reports. Data Extraction: Totally 40 articles related the dependability between HPA and depression were collected, and 32 of which were accorded with the inclusive criteria. Of the 8 excluded ones, 6 were repetitive researches, 2 were concerning with molecular genetics. Data Synthesis: 1 The level of corticotrophin releasing hormone (CRH)in cerebrospinal fluid increased, secretory adrenocorticotropin strengthened, cortisol level increased and desamethasone suppression test was non-suppression. 2 HPA could join the onset of depression by interaction with monoamine neurotransmitter. 3 The change of the function of HPA might result in decrease of melatonin secretion, which led to sleeping disturbance, gastrointestinal hypornotility, dyspepsia, decreasing appetite, loss of weight.4 As for diagnostic value for depression, sensitivity was high, but specificity was low. 5 To predict relapse of symptom and maintenance therapy by HPA functional change might have some value, but there was no certain result for predicting therapeutic efficacy. 6 No coincident finding on the relationship between the change of the function of HPA and suicide in depression. Conclusion: There is dependability between the change of the function of HPA and morbidity as well as physiosymptoma. The specificity of diagnostic value for depression is low. It can provide some objective basis for relapse of symptom and maintenance therapy. Dynamic detection in function of HPA can be performed in depressionqients so as to provide basis for clinical therapy and follow-up observation.
