Abstract:Safety instrumented system(SIS) is important for safety of industrial production.IEC 61508 require the establishment of safety integrity levels(SIL) for all the SIS for confirmation that they meet the requirement for functional safety.SIS in process industry are commonly in low demand rate.Research andvances of SIL verification for SIS in this mode is reviewed in this article,with summary of research on common cause failure modeling and caculation of average probability of failure on demand(PFD_(avg)).In the aspect of caculation of PFD_(avg),three common methods are introduced,including reliability block diagram,fault tree analsis and Markov model.Finally,several commercial softwares with SIL verificition function are introduced.

Abstract:The principle of SIL assignment is explained. Hazard matrix and risk graph presented in IEC61508 are discussed. A quantitative method for SIL assignment is put forward based on risk frequency and consequence. The quantitative method is general and easy to reuse as well as intuitive.

Abstract:For enhancing functional safety management on safety instrumented systems, a design of functional safety management software is presented based on the analysis of safety integrity level(SIL),safety requirement specification and safety integrity verification, which are included in safety life cycle. Hazard matrix and risk graph are two typical methods for SIL selection. Safety integrity verification uses Markov models and architecture constraints are also taken into account. The software is developed using C#. It can be used to assess not only safety but also availability for safety instrumented functions. Several types of report can be generated. The functional safety management software is a good tool and reference for correct functional safety management, helping users understanding the safety and availability of safety instrumented systems.

Abstract:Safety is paramount in rail transportation. The International Safety Standard IEC 61508, issued by the International Electrotechnical Commission (IEC) in 2000, describes the modern concept of safety and the specifications for safety evaluation in a systematic manner. On this basis, the EN 50129 railway transportation safety standard and safety integrity level(SIL) are established. Through the above analysis, the relationship between safety-related system reliability, acceptable risk, and safety index is summarized. It is clear that the safety index is based on acceptable risks. Nonetheless, as the economy and society advance, the safety index mandated by the SIL falls behind. Consequently, the principles and methodologies for calculating the safety index are presented. The paper proposes a reference for the Chinese railway safety index based on actual data from the Chinese railways over the past few years.

Abstract:In this study, a novel safety integrity level (SIL) determination methodology of safety instrumented systems (SISs) with parameter uncertainty is proposed by combining multistage dynamic Bayesian networks (DBNs) and Monte Carlo simulation. A multistage DBN model for SIL determination with multiple redundant cells is established. The models of function inspection test interval and function inspection test stages are alternately connected to form the multistage DBNs. The redundant cells can have different M out of N voting system architectures. An automatic modeling of conditional probability between nodes is developed. The SIL determination of SISs with parameter uncertainty is constructed by using the multistage DBNs and Monte Carlo simulation. A high-pressure SIS in the export of oil wellplatform is adopted to demonstrate the application of the proposed approach. The SIL and availability of the SIS and its subsystems are obtained. The influence of single subsystem on the SIL and availability of the SIS is studied. The influence of single redundant element on the SIL and availability of the subsystem is analyzed. A user-friendly SIL determination software with parameter uncertainty is developed on MATLAB graphical user interface.

Abstract:This paper presented a self-design test system of auto air-condition compressor.The system,based on SIEMENS SIMATIC S7-300 PLC,could complete all testing items automatically.Its basic characteristics,equipments,control flow and PLC hardware and software design were introduced.Practice proved that this system can improve the testing rate remarkably.

Abstract:Safety integrity level (SIL) verification is a critical step in the life cycle of safety instrumented systems (SIS). For the chemical process industry (CPI), SIL verification often means calculation of average probability of failure on demand (PFDavg) of a SIS. Markov analysis covers most aspects of the quantitative safety evaluation and shows great flexibility. However, for a complex SIS, the model building is so cumbersome that industrial users always try to avoid using it. A simplified approach by conducting Markov analysis on each channel of a SIS with any type of architecture and combining the results together is proposed in this paper. A case study is performed and the results prove that the simplified approach can simplify Markov modeling without loss of accuracy if a proper common cause failure (CCF) model is adopted.

Abstract:This paper discusses the message model arming at safety-related communication (SRC) in railway safety-critical system (SCS), and analyzes the effect of transmission code and safety code on safety integrity level (SIL) of railway SCS & communication failures. It is found that system safety could not always be improved when transmission code is lengthened out. So the paper gives a new concept that is called safety effective length of transmission code (SELTC), provides its mathematical definition and calculation formula, points out its two important properties, and proves its rationality. The new concept is very helpful to analyze and design the SRC.

Abstract:When evaluating safety integrity level(SIL) of safety instrumented system,parameter uncertainty is one of the important causes working on the accuracy of evaluation result.A SIL evaluation method based on Cauchy distribution Monte Carlo method was mainly discussed when the parameter is one interval distribution.After analyzing the example,its effect on the accuracy was pointed out and applied Monte Carlo to interval calculation in practice.

Abstract:In this paper,a risk analysis software system named as HASILT was presente,which was developed based on the functional safety international standards IEC61511,IEC61508.The system integrated the tools for performing Hazard and Operability(HAZOP) study,Layer of Protection Analysis(LOPA),Safety Integrity Level(SIL) analysis and some other accessorial activities.The system was a quite practical and high efficient system as displayed in its industrial applications.The system provides enterprises with a useful analysis and management platform,aiding them to perform process safety management.

Abstract:On March 13, 2023, in conjunction with the 19th Global Congress on Process Safety, four of the authors gathered to participate in a panel session. The discussion centered on the premise that many individual device failure rates published by some manufacturers and some certifying agencies are deemed—by members of the panel, many safety instrumented system (SIS) designers, and practicing engineers in the field—to be too low. This is especially true for mechanical final elements. A SIS designed using these unrealistic failure rates often results in estimated safety integrity levels (SILs) for the individual safety instrumented functions (SIFs) comprising the SIS, which would be achievable only under the most optimistic assumptions regarding installation, operation, and maintenance. These estimated SILs are unlikely to be achieved in the field. The panel discussed the underlying causes of this serious safety issue and proposed some possible solutions. This paper summarizes the panel discussion among four of the authors. Relevant graphics shown during the discussion and some panelists' quotations are included.

Abstract:IEC 61508 requires safety integrity verification for safety related systems to be a necessary procedure in safety life cycle. PFDavg must be calculated to verify the safety integrity level (SIL). Since IEC 61508-6 does not give detailed explanations of the definitions and PFDavg calculations for its examples, it is difficult for common reliability or safety engineers to understand when they use the standard as guidance in practice. A method using reliability block diagram is investigated in this study in order to provide a clear and feasible way of PFDavg calculation and help those who take IEC 61508-6 as their guidance. The method finds mean down times (MDTs) of both channel and voted group first and then PFDavg. The calculated results of various voted groups are compared with those in IEC61508 part 6 and Ref. [Zhang T, Long W, Sato Y. Availability of systems with self-diagnostic components-applying Markov model to IEC 61508-6. Reliab Eng System Saf 2003;80(2):133–41]. An interesting outcome can be realized from the comparison. Furthermore, although differences in MDT of voted groups exist between IEC 61508-6 and this paper, PFDavg of voted groups are comparatively close. With detailed description, the method of RBD presented can be applied to the quantitative SIL verification, showing a similarity of the method in IEC 61508-6.
Progress on Inherently Safer Design Methods in Chemical Process

Abstract:Inherently safer design methods and strategies were briefly reviewed in this article, and several indexes to evaluate the potential harms of different reaction routines were introduced. Multiply steady states exist in chemical processes because of the strong nonlinearity of the processes, the stabilities of the steady states were usually different from each other, and the characteristics of the stable steady states are also different even if they are very close to each other. In addition, singularity points accompanied by oscillation phenomenon should be avoided for stable operation and consistent product quality. A quantitative stability index was proposed to describe the stability of all stable steady states, and an index to describe how close an operation point was allowed to singularity points was proposed. These indexes were useful in inherently safer process design.

Abstract:Risk‐based techniques such as risk graph and Layer of Protection Analysis (LOPA) are used to determine the Safety Integrity Level (SIL) of safety instrumented functions to ensure that risk is reduced to a tolerable level. However, these techniques have some drawbacks. For instance, they need absolute and precise numbers to evaluate SIL parameters, which are rarely available or are highly uncertain. In addition, they are incapable of considering individual and societal risks simultaneously. Moreover, risk tolerance criteria are likely to be used incorrectly in the LOPA technique, and risk graph is difficult to calibrate. In the current paper, a novel comprehensive fuzzy arithmetic model has been developed to determine the required SILs in process industries. The fuzzy required Risk Reduction Factor (RRF) is calculated for both individual and societal risks. Fuzzy numbers are developed from crisp intervals, based on the expected interval of the fuzzy numbers. Expert fuzzy‐scaled elicitation has been applied to obtain the SIL parameters. In the proposed model, the overall risk tolerance criterion and apportionment factor are defined as SIL parameters for both individual and societal risks to ensure that the applied risk criteria are compliant with the requirements of the system. In addition, an approach is introduced for determining the required SIL based on the fuzzy required RRF. The proposed methodology was demonstrated to alleviate the limitations, and thus, can be considered as a more precise alternative to the conventional methods.

Abstract:The ITER Gas Injection System (GIS) Regulates fueling and impurity gases during plasma discharges within the tokamak vacuum vessel. To implement ITER investment protection, the GIS requires capable of interlock function to shielding the gases from ITER vacuum chamber when Disruption Mitigation System (DMS) is triggered or an abnormality is detected, in order to prevent further damage caused by gas fueling. This paper presents the preliminary design of the interlock system for the ITER GIS per ITER plant interlock system design rules and guidelines. The design adopts a redundant and fault-tolerant architecture with fail-safe programmable logic controllers to achieve the required ITERs target Interlock Integrity Level of SIL-2. The solutions here facilitate the integration with ITER instrumentation and control system, also establishing reference models for other large-scale control system platforms.

Abstract:Incomplete process hazard analysis (PHA) and poor knowledge management have been two major reasons that have caused numerous lamentable disasters in the chemical process industry (CPI). To improve PHA quality, a new integration framework that combines HAZOP, layer of protection analysis (LOPA), safety requirements specification (SRS) and safety integrity level (SIL) validation is proposed in this paper. To facilitate the integrated work flow and improve the relevant knowledge management, an intelligent software platform named HASILT has been developed by our research team. Its key components and functions are described in this paper. Furthermore, since the platform keeps all history data in a central case base and case-based reasoning is used to automatically retrieve similar old cases for helping resolve new problems, a recall opportunity is created to reduce information loss which has been cited many times as a common root cause in investigations of accidents.

Abstract:For safety-designing an equipment/system in civil aircraft, ARP4761 & SAE ARP 4754 is important common standard. But in these standards there has no clear description aircraft structure's effect on the equipment/system. In this present paper, aircraft structure's effect on design an equipment/system is considered by methods of FHA-FMEA-FTA-SSA quantitatively by conservative assumption. A flow chart of requirement considering structure-support effect is developed.

Structural Safety

Abstract:We review the developments in life safety and the incorporation thereof in the design and assessment of structures over the last 50 years. Various measures of life safety are presented that have been developed according to the marginal life saving cost principle based on individual, societal and economic considerations. Target probabilities of failure, or target reliabilities, are central to modern structural design and assessment. These are derived either through back-calibration to existing practice or through life cycle cost minimisation, both of which yield comparable safety levels, and are underpinned by lower bounds from life safety. Life cycle cost minimisation is reviewed here, which considers all direct and indirect costs of failure including loss of life and limb, as well as the costs and efficiency of increasing reliability. We discuss the incorporation of life safety into reliability-based design and assessment through the concept of the Life Quality Index, which uses key societal indicators, namely, the GDP and life expectancy, and health economics as a basis for specifying minimum reliabilities for both new and existing structures. The current state of advancement of reliability- and risk-informed design, and recommendations for future developments in life safety are considered.

Abstract:In recent years, curial incidents and accidents have been reported due to un-intended control caused by misjudgment of statistical machine learning (SML), which include deep learning. The international functional safety standards for Electric/Electronic/Programmable (E/E/P) systems have been widely spread to improve safety. However, most of them do not recom-mended to use SML in safety critical systems so far. In practical the new concepts and methods are urgently required to enable SML to be safely used in safety critical systems. In this paper, we organize five kinds of technical safety concepts (TSCs) for SML components toward accordance with functional safety standards. We discuss not only quantitative evaluation criteria, but also development process based on XAI (eXplainable Artificial Intelligence) and Automotive SPICE to improve explainability and reliability in development phase. Fi-nally, we briefly compare the TSCs in cost and difficulty, and expect to en-courage further discussion in many communities and domain.
